Abstract:Objective To determine the dose-response relationship of remifentanil inhibiting responses to endotracheal intubation when combined with propofol in severely preeclamptic patients undergoing caesarean section.Methods One hundred severely preeclamptic patients,aged 20-36 yr,weighing 32-36 kg,with body mass index ≤ 35 kg/m2,at 32-36 weeks of gestation,of ASA physical status Ⅰ or Ⅱ,undergoing caesarean section under general anesthesia,were randomly divided into 5 groups (n =20 each) using a random number table:group R1-group R5.Anesthesia was induced with iv injection of remifentanil 0.25 μg/kg (group R1),0.50 μg/kg (group R2),0.75 μg/kg (group R3),1.00 μg/kg (group R4) or 1.25 μg/kg (group R5),propofol 1.5-2.0 mg/kg,and succinylcholine 1.5 mg/kg.The patients were endotracheally intubated and mechanically ventilated.Anesthesia was maintained with inhalation of 2％ sevoflurane before birth,and iv injection of midazolam 30 μg/kg,fentanyl 6 μg/kg,and cis-atracurium 0.2 mg/kg,and continuous iv infusion of propofol 4-7 mg·kg-1 · h-1,and remifentanil 0.2 μg· kg-1 · h-1 after birth.The response to endotracheal intubation was defined as positive when SP≥ 160 mmHg after intubation and maintained at this level for more than 1 min.The ED50,ED95 and 95 ％ confidence interval (CI) of remifentanil inhibiting responses to endotracheal intubation were calculated by probit analysis.Results The ED50 and ED95 (95％ CI) of remifentanil inhibiting responses to endotracheal intubation were 0.65 (0.53-0.77) μg/kg and 1.30 (1.10-1.70) μg/kg,respectively,in severely preeclamptic patients undergoing caesarean section.Conclusion When combined with propofol,the ED50 and ED95 of remifentanil inhibiting responses to endotracheal intubation in severely preeclamptic patients undergoing caesarean section are 0.65 and 1.30μg/kg,respectively.
Comparison of Surgical Stress Index-Guided Analgesia with Standard Clinical Practice During Routine General Anesthesia A Pilot Study

Abstract:Background Surgical stress index (SSI), a novel multivariate index, has recently been proven to react well to surgical nociceptive stimuli and analgesic drug concentration changes during general anesthesia. We investigated the feasibility of application of SSI for guidance of remifentanil administration during propofol-remifentanil anesthesia. Methods Eighty patients scheduled for elective ear-nose-throat surgery were randomized into two groups, SSI-guided analgesia group (SSI group) and standard practice analgesia group (control group). In both groups, anesthesia was maintained with a propofol target-controlled infusion and adjusted stepwise by 0.5 microg/ml to keep bispectral index values between 40 and 60. In the SSI group, the predicted effect-site concentration of remifentanil was adjusted stepwise by 1 ng/ml to keep SSI values between 20 and 50, whereas in the control group, predicted effect-site concentration of remifentanil was adjusted according to traditional inadequate analgesia criteria. Anesthetics consumption, recovery times, and incidence of unwanted events were recorded. Results Remifentanil consumption (average normalized infusion rate) was lower in the SSI group than in the control group (mean +/- SD, 9.5 +/- 3.8 microg . kg(-1) . h(-1) vs. 12.3 +/- 5.2 microg . kg(-1) . h(-1); P < 0.05). The number of unwanted events was less in the SSI group (84) than in the control group (556; P < 0.01). Recovery times were comparable between groups. No patient reported intraoperative recall. Conclusions SSI-guided anesthesia resulted in lower remifentanil consumption, more stable hemodynamics, and a lower incidence of unwanted events.

The study aimed to investigate the protective effect of alfentanil, afentanyl derivative with short onset time and short duration, in reducing sufentanil-induced cough. Patients and methods: Eighty patients that scheduled for thyroid surgery under general anesthesia were randomly divided into the alfentanil group and normal saline group, with 40 cases per group. Patients in the alfentanil group received intravenous administration of 2 μg/kg alfentanil prior to sufentanil injection during general anesthesia induction, while the same dose of normal saline was administered in the normal saline group. The outcomes measures included the incidence and severity of cough and common side effects of opioids following the administration of sufentanil during the induction of general anesthesia, intraoperative hemodynamics parameters and major adverse events during anesthesia recovery period. Results: The incidence of cough within one minute after the injection of sufentanil during anesthesia induction was 40% in the normal saline group, and the pretreatment of alfentanil significantly reduced the incidence of sufentanil-induced cough to 5% (p < 0.05). Correspondingly, the patients in the alfentanil group had decreased severity of sufentanil-induced cough compared with the normal saline group (p 0.05). Furthermore, there were no significant differences between the two groups in intraoperative hemodynamic parameters, extubation time, or the incidences of emergence agitation, respiratory depression, delayed recovery from anesthesia and postoperative nausea and vomiting during Postanesthesia Care Unit stay (p > 0.05). Conclusion: Pretreatment with low-dose alfentanil (2 μg/kg) effectively and safely reduced both the incidence and severity of sufentanil-induced cough during anesthesia induction. Whereas, the use of intravenous sufentanil can be associated with a number of undesirable effects, such as nausea, vomiting, cough, chest tightness, dizziness and even respiratory depression, depending on the dose, injection speed and individual variation. A Comparison of Remifentanil Versus Sufentanil with Target-Controlled Infusion in Combined Inhalation Anesthesia for Surgical Patients:Effects on Hemodynamics and Postoperative Recovery

Abstract:OBJECTIVE:To compare the effect of remifentanil versus sufentanil with target-controlled infusion in combination with inhalation anesthesia for surgical patients on the parameters of hemodynamics and postoperative recovery. METHODS:Forty ASA I-II patients aged 18 - 65 years old with BMI (body mass index) < 30, undergoing colectomy or pedical screw interfix were enrolled. Upon the approval of institutional Ethics Committee, they were randomized to receive remifentanil or sufentanil at a target plasma concentration of 3 ng/ml and 0.3 ng/ml respectively in combination of inhalated anesthesia at 0.9 MAC (minimal alveolar concentration). The infusion of remifentanil was discontinued at the end of surgery while the infusion of sufentanil at 40 - 60 minutes before the end of surgery. The arterial blood pressure (ABP), heart rate (HR), electrocardiogram (ECG) and pulse blood oxygen saturation during anesthesia were monitored. The time between the termination of anesthetic use and recovery of spontaneous breathing and extubation were observed. And the incidence of postoperative pain and respiratory depression were recorded. RESULTS:As compared with the baseline values, BP and HR decreased significantly in both groups. BP was similar in both groups whereas HR was lower in Group R than that in Group S at post-induction, post-intubation, incision, the end of surgery and extubation (P < 0.05). The time from termination of anesthesia to recovery of spontaneous breathing was 1.8 ± 1.4 min in Group R. And it was significantly shorter than that in Group S (2.9 ± 1.5 min) (P < 0.05). The time from termination of anesthetic use to extubation was 6.8 ± 3.9 min in Group R. And it was also significantly shorter than that in Group S (9.1 ± 2.8 min) (P < 0.05). Seven patients experienced postoperative pain with visual analogue scale (VAS) > 4. And morphine was used for rescue analgesia in recovery room. CONCLUSION:When combined with inhalation anesthesia, the effects on hemodynamics are similar between the patients receiving the target-controlled infusions of remifentanil and sufentanil. Remifentanil offers a shorter time to recovery of spontaneous breathing and tracheal extubation.

Abstract:PURPOSE: During the induction of general anesthesia, opioids and endotracheal intubation may cause coughing. This study aimed to investigate the safety and effectiveness of an optimized drug induction scheme for general anesthesia to prevent coughing in patients.METHODS: A total of 220 patients aged 18 to 65 years who underwent surgery under general anesthesia with endotracheal intubation were randomly assigned to two groups, each with 110 patients. One group was administered a divided sufentanil bolus (group A) and the other with a single sufentanil bolus (group B). Anesthesia induction was performed according to the drug induction scheme of 1st, 2nd, and 3rd minutes. The primary outcome was a coughing episode associated with the administration of opioids during anesthesia induction. We also recorded the pain associated with drug injection, hemodynamics, and blood oxygen saturation during the induction of anesthesia.FINDINGS: All patients were included in the final statistical analysis. Compared with group B, the incidence of opioid induced cough (OIC) was significantly higher in group A (9.1% vs. 0, P = 0.001). There was no cough reaction of tracheal intubation in either group. There was no severe pain due to propofol and rocuronium injection in either group (P > 0.05). The mean arterial pressure (MAP), heart rate (HR), and peripheral oxygen saturation (SpO<sub>2</sub>) values were within the normal range at each time point during the induction period in both groups.IMPLICATIONS: According to the optimized 1st, 2nd, and 3rd minutes anesthesia induction regimen, with a single final intravenous bolus of sufentanil after the diluted rocuronium bromide administration, no sufentanil and tracheal intubation induced coughing reactions were observed.TRIAL REGISTRATION: The study protocol was registered in the Chinese Clinical Trial Registry (ChiCTR2200062749, http://www.chictr.org.cn/showproj.aspx?proj=175018) on August 17, 2022.

Patients and Methods: A total of 105 patients scheduled for elective breast surgery under general anesthesia were selected and randomly assigned into three groups: normal saline (Group C), low-dose nalmefene 0.1 μg·kg − 1 (Group LN), and high-dose nalmefene 0.25 μg·kg − 1 (Group HN). Sufentanil 0.5 μg·kg − 1 was injected intravenously within 2 s after 5 min of intervention. The count and severity of cough within 2 min after sufentanil injection, as well as the time to first cough, were recorded. In addition, we also collected intraoperative hemodynamic data, postoperative pain scores, the incidence of receiving rescue analgesics, and side effects up to 24 h after surgery. Results: Compared to Group C, the incidence of SIC was significantly lower in Group LN and HN (64.7% vs 30.3% and 14.7%, respectively; P < 0.001), but no significant difference was observed between the two groups ( P =0.126). Compared to Group C, the risk factors decreased by 53.4% (95% confidence interval [CI] =0.181– 0.735, P =0.008) in Group LN and by 75.9% (95% CI=0.432– 0.898, P =0.001) in Group HN. Of the patients with SIC, less frequent SIC within 2 min after induction and a lower proportion of severe coughs were observed than Group C ( P < 0.05), and no difference was detected between Group LN and HN. Additionally, the onset time to the first SIC did not differ significantly between the groups. Intraoperative hemodynamic data, postoperative pain scores, and side effects in the first 24 h did not differ among the groups. Conclusion: Preadministration of nalmefene prior to induction of general anesthesia effectively suppressed SIC in patients undergoing breast surgery, without affecting intraoperative hemodynamic fluctuation and postoperative pain intensity. A severe cough reflex may also cause multiple subconjunctival hemorrhages and periorbital ecchymosis, 7 and further increase the risk of aspiration pneumonia. 8 Therefore, SIC prevention is urgently required. Abstract:Objective To observe the efficiency and safety of sufentanil which used in laparoscopic cholecystectomy with remifentanil-based anesthesia to prevent the acute postoperative pain.Methods Forty patients were randomly and blindly assigned to sufentanil group(group S) and non-sufentanil group(group C,20 cases in each group).Suspended isoflurane and vecuronium 30 minutes before the end of surgery then stoped using propofol remifentanil at the end of surgery,while the former group was injected sufentanil with 0.1 μg/kg.VAS,SpO2,HR,SBP,DBP were observed at post-operative 1 h(T1),2 h(T2),4 h(T3),6 h(T4),12 h(T5),24 h(T6),recorded the complications of nausea,vomiting,irritability and so on.Results VAS of group S was significantly lower than that of group C at T1,T2,T3,T5(P<0.01),the VAS score was lower than that of group C at T4,T6(P<0.05),there was no significant difference between group S and group C in SpO2,HR,SBP,DPB at T1,T2,T3,T4,T5,T6(P>0.05),and there was no significant difference in nausea,vomiting,irritability in the two groups(P>0.05).Conclusion At the end of surgery,injected 0.1 μg/kg sufentanil to control the acute postoperative pain of remifentanil and hyperalgesia effectively,analgesia without affecting the awakening quality,no respiratory depression occurred,which is secure,flexible and effective analgesia after the laparoscopic cholecystectomy.

Abstract:Purpose: To investigate the effect of target-controlled infusion of remifentanil in combination with propofol on anesthesia and endotracheal intubation response in patients undergoing surgical aneurysm clipping. Methods: The clinical data of 106 patients undergoing surgical aneurysm clipping at the Central Hospital of Wuhan, Wuhan City, China from September 2020 to December 2022 were retrospectively analyzed. Subjects in control group were given fentanyl intravenous infusion anesthesia while study group was treated with target-controlled infusion anesthesia of remifentanil combined with propofol. Hemodynamics and blood-gas indices were evaluated before anesthesia (T1), immediately after endotracheal intubation (T2) and at extubation (T3). The incidence of anesthesia-related complications was determined. Results: In study group, the agitation score was decreased compared to that of control group, while the Ramsay sedation score was increased (p < 0.05). The incidence of cough during endotracheal catheter indwelling and extubation was 7. 27 % in study group, which was less than 21. 57 % in control group (p < 0.05). The cough score of study group was also reduced (p 0.05). Furthermore, the mean arterial pressure at T2 and T3 was upregulated and heart rate (HR) was downregulated in study group (p < 0.05). Recovery time of spontaneous respiration, time of eye-opening and extubation in study group were decreased compared to control group (p < 0.05). Conclusion: Target-controlled infusion of remifentanil in combination with propofol improves anesthetic effect in patients undergoing surgical aneurysm clipping, reduces tracheal intubation reaction, maintains hemodynamic stability and improves recovery quality of patients. Future study will be required to evaluate the efficacy and optimal effective concentration in a diverse population.

Abstract:Objective:To investigate the clinical effects on TCI of remifentanil combined with sufentanil in elderly patients during anesthesia.Methods:Totally 104cases of general anesthesia undergoing abdominal operation were randomly divided into group A and B with 52cases of each group.Group A was given 5ng/mL of remifentanil for induction of anesthesia,and 4ng/ml for maintenance of anesthesia, and then received PCIA at skin suture;group B was given 2ng/mL remifentanil and sufentanil for induction and maintenance of anesthesia,and then received PCIA at skin suture.At time points including before anesthesia(T1),before intubation(T2),1min after intubation(T3),1min after operation(T4)and 1 min after extubation(T5)of systolic blood pressure(SBP),diastolic blood pressure(DBP)and heart rate(HR)were compared between the two groups;the adverse cardiovascular event,VAS score at 4h,12h, 24h,12h,24h,postoperative cor,GLu and analgesic dosage were recorded.Results:The SBP,DBP in group B were significantly lower than that in A group(P0.01)at any time point,the HR of T2,T3,T4 were lower than those in group A,but the difference was not significant(P0.05);the incidence of adverse cardiovascular event,VAS scores,cor,GLu and postoperative analgesic dosage of group B were significantly lower than group A(P0.01).Conclusion:The TCI of remifentanil combined with sufentanil has a satisfactory clinical effects on reducing the perioperative stress response and postoperative pain in the elderly patients,it is worthy of clinical promotion.

Abstract:Background: This randomized controlled trial was performed to observe the effect of remimazolam with sufentanil on hemodynamics during anesthetic induction in elderly patients with mild hypertension undergoing orthopedic surgery of the lower limbs. Methods: Sixty elderly patients were randomly assigned to undergo general anesthesia with intravenous injection of either remimazolam besylate (25 mg/vial, batch number 10T11011; Yichang Humanwell Pharmaceutical Co., Ltd., Yichang, China) at 0.2 mg/kg (Group R, n = 30) or propofol at 1.5 mg/kg (Group P, n = 30). Both injections were completed within 15 to 20 s. If the bispectral index value did not reach 40 to 60, then 0.05 mg/kg of remimazolam was added in Group P and 1 mg/kg of propofol was added in Group R. When the BIS value reached 40 to 60, sufentanil was administered at 0.3 to 0.5 µg/kg and cisatracurium was administered at 0.15 to 0.2 mg/kg in both groups. Three minutes later, tracheal intubation and controlled ventilation were performed to maintain the end-tidal carbon dioxide partial pressure at 4.5 to 5.0 kPa. The mean arterial pressure (MAP), heart rate (HR), cardiac output (CO), continuous cardiac index (CI), systemic vascular resistance (SVR), and pulse oxygen saturation were recorded before induction (T0), when the eyelash reflex disappeared (T1), immediately after endotracheal intubation (T2), 1 min after endotracheal intubation (T3), and 5 min after endotracheal intubation (T4). The disappearance time of the eyelash reflex, injection pain, hypotension, bradycardia, hiccupping, nausea and vomiting, and other adverse events were observed. Results: The MAP, HR, CO, and CI at T1, T2, T3, and T4 were significantly higher in Group R than P, while SVR was significantly lower in Group R than P (P < 0.05). In Group P, the MAP, HR, CO, and CI were significantly lower and the SVR was significantly higher at T1, T2, T3, and T4 than at T0 (P < 0.05). Adverse events occurred in 8 (20%) patients in Group R and 22 (73%) in Group P. The total incidence of adverse events was significantly lower in Group R than P (P < 0 0.001). Conclusion: Remimazolam combined with sufentanil for general anesthesia induction has the advantages of small hemodynamic fluctuations, stable circulation, and few adverse reactions, making it suitable for elderly patients with mild hypertension. Differences in Sedation Level and Adverse Effects Produced by Equivalent Analgesic Doses of Remifentanil,sufentanil and Fentanyl

Abstract:Objective To examine the differences in the level of sedation and adverse effects produced by equivalent analgesic doses of remifentanil,sufentanil and fentanyl.Methods Eighty ASA Ⅰ female patients aged 18-39 yr with BMI of 18-25 kg/m2 scheduled for laparoscopic surgery under general anesthesia were randomly divided into 4 groups ( n =20 each):group control (group C) ; group remifentanil ( group R) ; group sufentanil (group S) and group fentanyl (group F).Remifentanil 2 μg/kg,sufentanil 0.2 μg/kg and fentanyl 2 μg/kg in normal saline 10 ml were infused iv over 2 min in groups R,S and F respectively.Depth of sedation was assessed and scored using OAA/S scale (5 =alert,1 =no response to prodding) and wavele index (WLI),before (baseline) and at 2,4,6,8 and 10 min after drug administration.Besides RR,pulse oxygen saturation,BP,HR were also monitored.The incidences of apnea,muscle rigidity,nausea and vomiting,pruritus,vertigo,bradycardia,profuse sweating and skin rash were measured and calculated.Results The 4 groups were comparable with respect to age,BMI and height.The lowest values of OAA/S scores,WLI and RR were significantly lower in groups R,S and F than in group C.Sufentanil produced the deepest sedation among the 3 opioids.Remifentanil produced strongest respiratory depression and nausea and vomiting.Conclusion The equivalent analgesic dose of sufentanil produces deeper sedation than that of remifentanil and fentanyl while remifentanil has the greatest impact on RR.
Clinical Application of Remifentanil in Aged Patients with Hypertension undergoing Parotid Tumor Surgery

Abstract:Objective: To explore the clinical application of remifentanil in aged patients with hypertension undergoing parotid tumor surgery.Methods: 52 patients,aged 65-82 years,ASAⅡ or Ⅲ,were randomly divided into group R and group F with 26 cases each group.Both groups were given total intravenous anesthesia with consistent induction approach.Patients in group R were continuously infused with remifentanil 0.05-0.2μg/kg·min-1 whereas those in group F were given fentanyl intermittently with the maximum dose 8.0g/kg during operation.Anesthesia was maintained with propofol and cis-atracurium in the two groups.Hemodynamic parameters,postoperative recovery and early adverse effects were recorded at different time points.Results: The patients in group R had more stable hemodynamics and less volume of blood loss compared with those in group F.Postoperative awake time and extubation time were significantly reduced in R group than those in group F(P0.01).There were no significant differences in recovery time of spontaneous breathing,postoperative restlessness,pain,nauseau and vomitting in two groups.Conclusion: Remifentanil is safe and practicable clinically for aged patients with hypertension undergoing parotid tumor surgery as it can maintain hemodynamics stable,decrease the volume of blood loss and provide a better awakeness.

Abstract:OBJECTIVE:To observe the effect of optimizing anesthetic injecting sequence during induction on fentanyl-induced coughing. METHODS:One hundred and twenty ASA I or II elective patients undergoing general anesthesia were randomly allocated to optimized group or control group: the optimized group induced with midazolam 0.06 mg/kg, followed by fentanyl 1 mg/kg at 1 min later, vecuronium 0.1 mg/kg at 1 min 55 s, propofol 1.5-2 mg/kg at 2 min, a second dose of 3 mg/kg fentanyl at 2 min 20 s, intubated at time 5 min; the control group was induced with the same medication but all the fentanyl (4 mg/kg) was injected at time 1 min. Coughing after fentanyl injection was observed and hemodynamic parameters were recorded. RESULTS:Hemodynamic changes were identical between the two groups indicated similar intubation response suppression. The incidence of fentanyl-induced coughing was significantly lower in the optimized group (4/60) than in the control group (23/60) (P < 0.01). CONCLUSION:Optimizing anesthetic injecting sequence during induction by separate fentanyl into two boluses significantly reduce fentanyl-induced coughing without affecting intubation response suppression.

Abstract:Purpose: To determine the anesthetic effect and related indicators following concurrent administration of remifentanil and propofol in cerebral hemorrhage surgery patients.Methods: A total of 88 cerebral hemorrhage patients admitted in Lishui People’s District Hospital, Nanjing, China, from December 2019 to December 2020, were assigned to two groups, viz, control group which received fentanyl and propofol for anesthesia, while study group was administered remientanil combined with propofol for anesthesia There were 44 patients in each group. Hemodynamic index, brain injury marker index, stress response index, awakening condition, propofol dosage, anesthetic effect, and adverse reactions were assessed and recorded.Results: Heart rate (HR), diastolic blood pressure (DBP), and systolic blood pressure (SBP) at T2 and T3 of the two groups were less than those at T1. At T3, the study group’s HR, DBP, and SBP were substantially higher than those of control group (p < 0.05); At 12 and 24 h after operation, brain injury markers and stress response indices in study group were significantly lower compared to control group (p < 0.05), while in comparison to control group (79.55 %), the degree of anesthesia in the study group was higher (95.45 %; p < 0.05). The incidence of adverse reactions in the study group (15.91 %) was lower than in control group (43.18 %; p < 0.05).Conclusion: Remifentanil, when combined with propofol anesthesia, stabilizes hemodynamics, protects against brain injury, and reduces stress reactions in patients undergoing cerebral hemorrhage surgery. The combination is also highly effective and safe. However, validation of these findings in larger clinical trials is required.

Abstract:Objective To evaluate the outcomes of application of sufentanil for induction andremifentanil for maintenance as an adjuvant to general anestheia in the elderly.Methods One hundredpatients,aged 65-70 years old,undergoing abdominal surgery were randomly divided into 5 groupswith 20 cases each.The depeth of general anesthesia with isoflurane inhalation was modulated by BIS.Mean arterial pressure(MAP).Heart rate(HR),bispectral index(BIS),and end tidal isofluraneconcentration(C_(ET)I_(SO))at the baseline(T_0),intubution(T_1),1 min(T_2)and 5 min after intubution(T_3),1 min(T_4),5 min(T_5)and 10 min after skin incision(T_6)and the abdominal exploration(T_7)were recorded.Visual analogue scale(VAS)pain score was assessed at 4 h after operation.Consumption of sufentanil in the first postoperative 24 h during PCIA was recorded. Recovery profileand perioperative side effects were also collected.Results The C_(ET)I_(SO) was higher in group FF thanthat in groups of SR,FR,SS and RR during anesthesia(P<0.05).VAS at 4 h after operation andconsumption of analgesics during 24-hours were higher in group RR and FR than those in groups ofSR.SS and FF(P<0.05 or P<0.01).The time of extubation was shorter in groups of SR,FR andRR than those in groups of SS and FF(P<0.01).The percentage of restlessness was higher in groupRR than that in groups of SR,FR,SS and FF(P<0.05).Conclusion Application of sufentaniI forinduction and remifentanil for maintenance as an adjvant to general anestheia in the elderly has theadvantages of stable hemodynamics.early recovery from anesthesia and less adverse effects.
Effect of Sufentanil and Tramadol on Preventing the Restlessness During Emergence after Remifentanil-Based General Anesthesia

Abstract:Objective To investigate the proposal of preventing the restlessness during emergence after remifentanil-based general anesthesia. Methods One hundred and eighty ASA class Ⅰ or Ⅱ patients undergoing elective laparoscopic cholecystectomy under general anesthesia were randomly assigned into 6 groups of Q1, Q2, Q3, S1, S2, and S3 with 30 cases each. Thirty minutes before the end of surgery, tramadol 1, 2, 3 mg/kg and sufentanil 0.1, 0.2, 0.3 μg/kg were administered in groups Q1, Q2, Q3 and groups S1, S2, S3 respectively. HR, MAP, SpO2, PETCO2, Restlessness Score (RS), Ramsay Sedative Score (RSS), Observer′s Assessment of Alertness/Sedation Score (OAAS) were recorded at 30 min before drug administration(T1),10 min(T2),20 min(T3),30 min(T4),40 min(T5) after that,before extubation(T6),during extubation(T7),and 10 min(T8),20 min(T9) after extubation. In addition, cortisol and glucose in plasma at T6 and T7, Body Comfort Score (BCS), extubation time, patient-controlled intravenous analgesia(PCIA) dosage and side reactions were observed. Results The RS of group Q1 and Q2 were apparently higher than group Q3 at T6-T8 (P0.05). The RS of group Q3 were higher than group S1, S2 and S3 (P0.05). There were statistically significant differences of the RS at T9 (Q1, Q2 vs. Q3, S1, S2 and S3, P0.05). The RSS of groups Q3, S1, S2 and S3 were around 2 at T6-T9. The RSS of group Q1 and Q2 were about 1 at T6-T8 .There were remarkable differences in OAAS at T6-T9 (Q1, Q2, Q3 vs. S1, S2, S3, P0.05). There were also significant differences in the values of OAAS in groups Q1, Q2 and Q3 at T6. The BCS of sufentanil was obviously higher that of than tramadol (P0.05). There were great differences in the increase a values of cortisol and glucose from T6 to T7 (Q1, Q2, Q3 vs. S1, S2, S3, P0.05). The values in them in group Q1, Q2, Q3 declined in an order of the dosages (P0.05). The major complication of group Q3 was vomit, while the main side effects of group S3 were urine retention, the inhibition of respiratory and circulatory system. Conclusion The application of 0.2 μg/kg sufentanil thirty minutes before the end of surgery can safely and perfectly prevent the restlessness during emergence after remifentanil-based general anesthesia, which will produce few side effects and reduce the of PCIA dosage after operation.

Abstract:Fentanyl-induced cough (FIC) is unwanted in the patients requiring stable induction of general anesthesia. This study was designed to evaluate the suppressive effects of butorphanol pretreatment on the incidence and severity of FIC during the induction of general anesthesia. A total of 315 patients of American Society of Anesthesiologists physical status I and II, scheduled for elective surgery under general anesthesia were randomized into 3 equally sized groups (n = 0105). Two minutes before fentanyl bolus, group I received intravenously 5mL normal saline, groups II and III received butorphanol 0.015 and 0.03mg/kg (diluted with saline to 5mL), respectively. Patients were then administrated with fentanyl 2.5g/kg within 5s. The incidence and severity of FIC was recorded for 2minutes after fentanyl bolus. During experimental period, the mean arterial pressure, heart rate, and peripheral capillary oxygen saturation (SpO(2)) were recorded before the administration of butorphanol or normal saline (T0), 2minutes (T1) after butorphanol injection, and 2minutes (T2) after fentanyl injection. The incidence of FIC was 31.4% in group I, 11.4% in group II, and 3.8% in group III. Group III had a lowest incidence of FIC among 3 groups (P<0.001, vs group I; P<0.05, vs group II). The severe FIC was not observed in groups II and III, but was recoded from 6 patients in group I. At 2minutes after fentanyl injection (T2), the mean arterial pressure was significantly higher in group I than that in groups II and III (P<0.01, vs group II; P<0.05, vs group III), but the values remained within safe limits. In conclusion, pretreatment with butorphanol could effectively and safely suppress FIC during anesthesia induction.
