Abstract:Fentanyl-induced cough (FIC) is unwanted in the patients requiring stable induction of general anesthesia. This study was designed to evaluate the suppressive effects of butorphanol pretreatment on the incidence and severity of FIC during the induction of general anesthesia. A total of 315 patients of American Society of Anesthesiologists physical status I and II, scheduled for elective surgery under general anesthesia were randomized into 3 equally sized groups (n = 0105). Two minutes before fentanyl bolus, group I received intravenously 5mL normal saline, groups II and III received butorphanol 0.015 and 0.03mg/kg (diluted with saline to 5mL), respectively. Patients were then administrated with fentanyl 2.5g/kg within 5s. The incidence and severity of FIC was recorded for 2minutes after fentanyl bolus. During experimental period, the mean arterial pressure, heart rate, and peripheral capillary oxygen saturation (SpO(2)) were recorded before the administration of butorphanol or normal saline (T0), 2minutes (T1) after butorphanol injection, and 2minutes (T2) after fentanyl injection. The incidence of FIC was 31.4% in group I, 11.4% in group II, and 3.8% in group III. Group III had a lowest incidence of FIC among 3 groups (P<0.001, vs group I; P<0.05, vs group II). The severe FIC was not observed in groups II and III, but was recoded from 6 patients in group I. At 2minutes after fentanyl injection (T2), the mean arterial pressure was significantly higher in group I than that in groups II and III (P<0.01, vs group II; P<0.05, vs group III), but the values remained within safe limits. In conclusion, pretreatment with butorphanol could effectively and safely suppress FIC during anesthesia induction.

Abstract:OBJECTIVE:To observe the effect of optimizing anesthetic injecting sequence during induction on fentanyl-induced coughing. METHODS:One hundred and twenty ASA I or II elective patients undergoing general anesthesia were randomly allocated to optimized group or control group: the optimized group induced with midazolam 0.06 mg/kg, followed by fentanyl 1 mg/kg at 1 min later, vecuronium 0.1 mg/kg at 1 min 55 s, propofol 1.5-2 mg/kg at 2 min, a second dose of 3 mg/kg fentanyl at 2 min 20 s, intubated at time 5 min; the control group was induced with the same medication but all the fentanyl (4 mg/kg) was injected at time 1 min. Coughing after fentanyl injection was observed and hemodynamic parameters were recorded. RESULTS:Hemodynamic changes were identical between the two groups indicated similar intubation response suppression. The incidence of fentanyl-induced coughing was significantly lower in the optimized group (4/60) than in the control group (23/60) (P < 0.01). CONCLUSION:Optimizing anesthetic injecting sequence during induction by separate fentanyl into two boluses significantly reduce fentanyl-induced coughing without affecting intubation response suppression.
Epidural Fentanyl Decreases the Minimum Local Analgesic Concentration of Epidural Lidocaine.

Abstract:Background Epidural lidocaine can be used when regional anesthesia needs to be established quickly, but the effect of co-administering epidural fentanyl on the minimum local analgesic concentration (MLAC) of lidocaine is not known. We compared the MLAC of epidural lidocaine in combination with different doses of fentanyl for epidural anesthesia in adults.Methods One hundred and twenty patients requiring epidural analgesia were randomly allocated to receive 20 ml of one of four solutions: lidocaine, or lidocaine plus fentanyl 1 mu g/ml, 2 mu g/ml, or 3 mu g/ml. The first patient in each group was administered 1% lidocaine weight by volume; subsequent patients received a concentration determined by the response of the previous patient to a higher or lower concentration according to up and down sequential allocation in 0.1% increments. Efficacy was assessed using a visual analog pain scale, and accepted if this was <= 10 mm on a 100 mm scale within 30 minutes. The extent of motor block and of nausea and vomiting were recorded at 30 minutes after administration of the epidural solution and two hours after surgery, respectively.Results The MLAC of lidocaine in those receiving lidocaine alone was 0.785% (95%CI 0.738-0.864). A significant dose-dependent reduction was observed with the addition of fentanyl: the MLAC of lidocaine with fentanyl at 2 mu g/ml was 0.596% (95%CI 0.537-0.660) and 0.387% with fentanyl at 3 pg/ml (95%CI 0.329-0.446, P <0.001).Conclusion Epidural fentanyl significantly reduces the dose of lidocaine required for effective epidural analgesia in adults without causing adverse side effects. The study aimed to investigate the protective effect of alfentanil, afentanyl derivative with short onset time and short duration, in reducing sufentanil-induced cough. Patients and methods: Eighty patients that scheduled for thyroid surgery under general anesthesia were randomly divided into the alfentanil group and normal saline group, with 40 cases per group. Patients in the alfentanil group received intravenous administration of 2 μg/kg alfentanil prior to sufentanil injection during general anesthesia induction, while the same dose of normal saline was administered in the normal saline group. The outcomes measures included the incidence and severity of cough and common side effects of opioids following the administration of sufentanil during the induction of general anesthesia, intraoperative hemodynamics parameters and major adverse events during anesthesia recovery period. Results: The incidence of cough within one minute after the injection of sufentanil during anesthesia induction was 40% in the normal saline group, and the pretreatment of alfentanil significantly reduced the incidence of sufentanil-induced cough to 5% (p < 0.05). Correspondingly, the patients in the alfentanil group had decreased severity of sufentanil-induced cough compared with the normal saline group (p 0.05). Furthermore, there were no significant differences between the two groups in intraoperative hemodynamic parameters, extubation time, or the incidences of emergence agitation, respiratory depression, delayed recovery from anesthesia and postoperative nausea and vomiting during Postanesthesia Care Unit stay (p > 0.05). Conclusion: Pretreatment with low-dose alfentanil (2 μg/kg) effectively and safely reduced both the incidence and severity of sufentanil-induced cough during anesthesia induction. Keywords: alfentanil, sufentanil, cough, anesthesia induction Sufentanil, a synthesized opioid agonist, has been widely used for the induction of general anesthesia in clinical practice due to its reliable analgesic efficacy, lack of histamine release and little effect on hemodynamics. Whereas, the use of intravenous sufentanil can be associated with a number of undesirable effects, such as nausea, vomiting, cough, chest tightness, dizziness and even respiratory depression, depending on the dose, injection speed and individual variation. Sufentanil-induced cough, with an incidence of up to 31.9%, 1 is usually transient and explosive, and in severe cases it can increase intracranial, intraocular and intraabdominal pressures, which may even cause a range of adverse effects in patients with high-risk comorbidities. 2 Several medicines, such as lidocaine, propofol, fentanyl, remifentanil, dezocine, dexmedetomidine, tramadol or magnesium sulfate, have been shown to be effective in reducing the incidence and / or severity of opioid-induced cough. 1,3–9 However, there are potential risks associated with these pharmacological interventions and it is therefore necessary to identify an alternative pretreatment agent with minimal complications to reduce sufentanil-induced cough. Alfentanil is a synthetic, short-acting opioid analgesic classified as a small molecule derivative of fentanyl. Alfentanil exhibits the less potency, the faster onset and shorter duration of action compared to analogous anesthetics such as fentanyl and sufentanil, making it an ideal analgesic supplement during various surgical procedures. Role of nalmefene in prevention of fentanyl-induced bucking in patients with induction of general anesthesia

Abstract:Objective To explore the role of nalmefene in prevention of fentanyl-induced bucking in patients with induction of general anesthesia and observe its influence on hemodynamic during intubation.Methods 80 patients with ASA ⅠtoⅡ were randomly divided into nalmefene group (group Ⅰ)and the control group (group Ⅱ).All patients were injected with phenobarbital sodium 30 mg and atropine 0.5 mg 30 minutes before induction of anesthesia.0.25 μg /kg of hy-drochloric acid natrium nalmene and the same capacity of physiological saline was applied 2 minutes before induction of intravenous for group Ⅱ and Ⅰ respectively.Coughing cases (incidence)and the intensity,blood pressure,heart rate ,pulse oxygen saturation before anesthesia induction,60 second after fentanyl injection and after intubation were observe between two groups.Results In-cidence rate of cough was 0% in group Ⅰ,which was significantly lower than 60% in group Ⅱ(P < 0 .0 5 ).Strength of coughing in group Ⅱ was much more intensive than that in group Ⅰ(P <0.05).The hemodynamic parameters after fentanyl injection in group Ⅰ was more steady than that in group Ⅱ (P <0.05),and there was no significant difference of hemodynamic response to intubation between two groups (P >0.05).Conclusion Preventive injection of nalmefene can effectively prevent coughing response to fentanyl injection during induction of anesthesia and im-prove the hemodynamic instability.
Effect of Optimizing the Induction Regimen in Preventing Cough Reactions in Patients Undergoing General Anesthesia: A Prospective Randomized Controlled Study

Abstract:PURPOSE: During the induction of general anesthesia, opioids and endotracheal intubation may cause coughing. This study aimed to investigate the safety and effectiveness of an optimized drug induction scheme for general anesthesia to prevent coughing in patients.METHODS: A total of 220 patients aged 18 to 65 years who underwent surgery under general anesthesia with endotracheal intubation were randomly assigned to two groups, each with 110 patients. One group was administered a divided sufentanil bolus (group A) and the other with a single sufentanil bolus (group B). Anesthesia induction was performed according to the drug induction scheme of 1st, 2nd, and 3rd minutes. The primary outcome was a coughing episode associated with the administration of opioids during anesthesia induction. We also recorded the pain associated with drug injection, hemodynamics, and blood oxygen saturation during the induction of anesthesia.FINDINGS: All patients were included in the final statistical analysis. Compared with group B, the incidence of opioid induced cough (OIC) was significantly higher in group A (9.1% vs. 0, P = 0.001). There was no cough reaction of tracheal intubation in either group. There was no severe pain due to propofol and rocuronium injection in either group (P > 0.05). The mean arterial pressure (MAP), heart rate (HR), and peripheral oxygen saturation (SpO<sub>2</sub>) values were within the normal range at each time point during the induction period in both groups.IMPLICATIONS: According to the optimized 1st, 2nd, and 3rd minutes anesthesia induction regimen, with a single final intravenous bolus of sufentanil after the diluted rocuronium bromide administration, no sufentanil and tracheal intubation induced coughing reactions were observed.TRIAL REGISTRATION: The study protocol was registered in the Chinese Clinical Trial Registry (ChiCTR2200062749, http://www.chictr.org.cn/showproj.aspx?proj=175018) on August 17, 2022.

Compare the Incidence and Severity of Fentanyl-induced Cough Between the Forearm Vein and Dorsal Vein of Foot

Abstract:Objective To compare the incidence and severity of FIC caused by injecting fentanyl through a vein in the forearm with through dorsal vein of foot.Methods Forty patients of ASA I,aged 18-55 year and weighing 47-78kg,undergoing elective surgical procedures were randomly allocated into two groups consisting of 20 patients.Both groups received i.v.fentanyl(3.0mg/kg) within 2 seconds.Group Ⅰreceived fentanyl through a vein in the forearm,whereas group Ⅱreceived fentanyl through dorsal vein of foot.The incidence of cough was recorded for 1min before the induction of anaesthesia,and graded as none(0 cough),mild(1-2 cough),moderate(3-4 cough) and severe(≥5 cough).Results The incidence of cough was 55% in group Ⅰand 5% in groupⅡ(P<0.05).The severity of cough in group Ⅰwas more than the severity in group Ⅱ.Conclusion Comparing with injecting fentanyl through vein of forearm,injecting fentanyl(3.0mg/kg) through dorsal vein of foot significantly reduces the incidence and severity of FIC in majority of the patients.
Prophylactic Intravenous Lidocaine at Different Doses for Fentanyl-Induced Cough (FIC): A Meta-Analysis

Abstract:To evaluate whether different doses of intravenous lidocaine are effective at preventing fentanyl-induced cough (FIC), we searched PubMed, Scopus, Cochrane Library, EMBASE and Web of Science, according to predefined criteria, for all articles published until June 2017. A meta-analysis and subgroup analysis were performed by combining the reported incidence of FIC. The odds ratio (OR) was used as a summary statistic. Eleven articles were included, with 965 patients in the lidocaine group and 745 patients in the control group. A pooled analysis indicated that the overall incidence of FIC was significantly different between the lidocaine group and the control group (OR, 0.27; 95% confidence interval (CI), 0.21–0.35; P < 0.05), as well as between the adult and paediatric subgroups. Sensitivity analysis showed that the results were stable. Subgroup analyses showed that compared to a placebo, both low (0.5–1.0 mg/kg) and high doses of lidocaine (1.5–2.0 mg/kg) were effective at reducing FIC incidence. There was no significant difference between low or high doses of lidocaine. Fentanyl doses added no significant heterogeneity as shown by meta-regression. The findings of this meta-analysis indicate that prophylactic intravenous lidocaine is effective at preventing FIC in both adults and children.

Abstract:To the Editor: We read with great interest the recently published article by Dr. Sun et al. [1], in which the authors demonstrated that intravenous administration of 0.1 mg/kg dezocine 10 min before general anesthesia completely suppressed fentanyl-induced cough during induction. The results were very inspirational and astonishing. However, we note that standard dose and usage of fentanyl do not cause a significant problem in clinical practice. Furthermore, we find that there is a clerical error in this article. The authors suggested that dezocine antagonizes j receptors, which in turn antagonize fentanyl-activated l receptors, thereby inducing cough. This statement was in disagreement with the results of their article, in which they showed dezocine was effective in suppressing fentanyl-induced cough during general anesthesia induction. From this point of view, it should be restated that dezocine antagonizes j receptors, which in turn antagonize fentanyl-activated l receptors, thereby reducing cough. In addition, the mechanisms of fentanyl-induced cough have been multifactorial including opioid receptors, C-fiber receptors, rapidly adapting pulmonary stretch receptors, histamine, and the citrate in the fentanyl injection fluid, just as the authors suggested [1]. However, the authors provided inadequate evidence to support their claim that dezocine counteracted the action of fentanyl by a central gating mechanism of cough suppression, predominantly via C-fiber receptors, or, whether dezocine inhibited the release of histamine by antagonizing and suppressing cough reflex? These speculations may have overextrapolated their study results because there is no evidence supporting the mechanisms of dezocine involved in the suppression of fentanyl-induced cough during general anesthesia induction. Therefore, more studies are needed to clarify the effects and the precise mechanisms of dezocine involved in suppressing fentanyl-induced cough during general anesthesia induction.

Patients and Methods: A total of 105 patients scheduled for elective breast surgery under general anesthesia were selected and randomly assigned into three groups: normal saline (Group C), low-dose nalmefene 0.1 μg·kg − 1 (Group LN), and high-dose nalmefene 0.25 μg·kg − 1 (Group HN). Sufentanil 0.5 μg·kg − 1 was injected intravenously within 2 s after 5 min of intervention. The count and severity of cough within 2 min after sufentanil injection, as well as the time to first cough, were recorded. In addition, we also collected intraoperative hemodynamic data, postoperative pain scores, the incidence of receiving rescue analgesics, and side effects up to 24 h after surgery. Results: Compared to Group C, the incidence of SIC was significantly lower in Group LN and HN (64.7% vs 30.3% and 14.7%, respectively; P < 0.001), but no significant difference was observed between the two groups ( P =0.126). Compared to Group C, the risk factors decreased by 53.4% (95% confidence interval [CI] =0.181– 0.735, P =0.008) in Group LN and by 75.9% (95% CI=0.432– 0.898, P =0.001) in Group HN. Of the patients with SIC, less frequent SIC within 2 min after induction and a lower proportion of severe coughs were observed than Group C ( P < 0.05), and no difference was detected between Group LN and HN. Additionally, the onset time to the first SIC did not differ significantly between the groups. Intraoperative hemodynamic data, postoperative pain scores, and side effects in the first 24 h did not differ among the groups. Conclusion: Preadministration of nalmefene prior to induction of general anesthesia effectively suppressed SIC in patients undergoing breast surgery, without affecting intraoperative hemodynamic fluctuation and postoperative pain intensity. Abstract:Objective To explore the clinical effect and safety of oxycodone hydrochloride in the induction of genenral anesthesia.Methods Sixty ASA Ⅰ or Ⅱ patients aged 35-75 years,sched-uled for colorectal surgery,undergoing general aneasthesia were randomly divided into two groups with 30 cases in each group:fentanyl group (group F)and oxycodone hydrochloride group (group O).Fentanyl 2 μg/kg+propofol 2 mg/kg+cisatracurium 0.1 5 mg/kg (group F)or oxycodone 0.2 mg/kg+propofol 2 mg/kg + cisatracurium 0.1 5 mg/kg (group O)was administered intravenously for general anesthesia induction.MAP and HR were monitored and recorded before the induction (T0 ),1 min before intubation (T1 ),at the time of intubation (T2 ),1 min after intubation (T3 )and 5 min after intubation (T4 ).Side-effect incidence of each group,operation time,wakening time,extuba-tion time and visual analogue scale (VAS)were recorded.Results Compared with group F,MAP in group O were lower than that of group F at T1 (P <0.05),while HR between two groups at T0-T4 showed no significant difference.In group F,there were statistical significances in MAP and HR be-tween T1-T3 and T0 ;in group O,there were statistical significances in MAP between T1-T2 and T0 , and HR at T2-T3 was significantly higher than that at T0 (P <0.05).No statistical significance were obtained between two groups in the operation time,wakening time and extubation time.The incidence of cough and the VAS of group O was less than that of group F with statistical significance (P <0.05).Conclusion Oxycodone hydrochloride used for the intubation of general anesthesia is practica-ble.
[Low-dose ketamine combined with fentanyl for intravenous postoperative analgesia in elderly patients]

Abstract:Objective: To investigate the clinical efficacy of and complications arising from low-dose ketamine combined with fentanyl for intravenous postoperative analgesia in comparison with the exclusive use of fentanyl in elderly patients. Methods: Eighty elderly patients were randomized into two equal groups following thoracoabdominal surgery, and received intravenous analgesia with the combination of 0.5 mg/ml ketamine, 5.0 microg/ml fentanyl and 50 microg/ml midazolam (KF group) and with 7.5 microg/ml fentanyl plus 50 microg/ml midazolam (FT group), respectively. The drugs used were diluted in 200 ml normal saline. For analgesic administration, a loading dose (2-4 ml) was given followed by a background infusion (2.5-3.5 ml), with patient-controlled bolus doses of 2.0-3.0 ml with lock-out time of 20 min via PCA pump (Automedical, Korea). The static pain score (VAS), sedation score, and incidences of nausea, vomiting, pruritus and hallucinations were recorded during the initial 48 h after the surgery. Results: The total analgesic dosage and PCA dosage in the two groups were similar (P>0.05). With similar VAS in the two groups (P>0.05), the sedative effect in KF group was much better than that in FT group (P<0.05) during the initial 48 h postoperatively. The incidences of nausea, vomiting and itching were lower in KF group than in FT group (P<0.05), and no illusion was reported in two groups during the initial 48 h. Conclusion: For producing comparable postoperative analgesic effect, low-dose ketamine combined with fentanyl can markedly reduce fentanyl requirement in the elderly patients and lowers the incidences of nausea, vomiting and itching in comparison with the exclusive use of fentanyl.

Abstract:Background . In the field of intravenous anesthesia, propofol is widely utilized as an induction agent. However, Propofol injection pain is a frequent adverse event that may result in discomfort for patients. Various strategies have been investigated to prevent or alleviate this pain, considering the presence of opioid receptors in the primary afferent nerve endings of peripheral tissues, which suggests a potential role of opioids in mitigating propofol-induced pain. Fentanyl, a short-acting pure opioid agonist commonly used for systemic analgesia during intraoperative and postoperative periods, has been found to possess peripherally mediated analgesic properties within its clinical dosage range. Therefore, the objective of this study was to evaluate the efficacy of a low dose of fentanyl in the fentanyl-propofol combination for reducing propofol injection pain. The objective of our study was to evaluate and compare the efficacy of two distinct doses of fentanyl in mitigating the pain associated with propofol injection. Materials and methods . The study enrolled 90 patients classified as ASA I–II who were scheduled for elective surgery. The study spanned over 4 months, from November 2022 to April 2023, and included patients aged 19 to 65 years. Patients were divided into three groups, each comprising 30 patients. The control group received only 5 ml (50 mg) of propofol. The group M1 received only 5 ml of a mixture of fentanyl and propofol, prepared with 20 ml (200 mg) of propofol and 2 ml (100 μg) of fentanyl, while the group M2 received only 5 ml of a mixture of fentanyl and propofol, prepared with 20 ml (200 mg) of propofol and 4 ml (200 μg) of fentanyl, at an injection speed of 0.5 ml/s. After 10 seconds of medication, patients were asked a standard question about the comfort of the injection, and a verbal rating scale (VRS) was used to assess propofol injection pain. Anesthesia induction was then continued following standard protocols. Statistical significance was set at p < 0.05 for all analyses. Results . The three groups were found to be similar in terms of patient characteristics. In the control group, the incidence of severe pain upon propofol injection was 46.7%, whereas it was 0% in both groups M1 and M2 (p < 0.05). Conclusion . The combination of fentanyl and propofol has been shown to effectively reduce the incidence of propofol injection pain. Interestingly, in this study, no significant difference was observed between the two different doses of fentanyl used in the mixture. This suggests that a low dose of fentanyl may be sufficient in achieving a pain-free environment during propofol induction, thereby offering a cost-effective approach in clinical practice.
Target-controlled Infusion of Propofol Combined with Remifentanil or Fentanyl: a Comparison of Anesthetic Depth

Abstract:Objective:To compare the anesthetic depth and postoperative recovery of propofol combined with remifentanil or fentanyl when administered through target-controlled infusion(TCI).Methods: Ninety patients receiving elective epigastric surgery were randomly divided into 3 groups: PF-A,PF-B and PR group.Through TCI,patients in PF-A group were administered with propofol(3 μg /ml) and fentanyl(2 μg/L),in PF-B group with propofol(3 μg/ml) and fentanyl(4 μg/L initially and 2 μg/L after intubation),and in PR group with propofol(3 μg/ml) and remifentanil(6 μg/L).Vital signs(including mean aortic pressure and heart rate) and bispectral index values were recorded pre-anesthesia,pre-and post-intubation,post-incision and during operation in 3 groups.Results: In PF-A group,post-intubation MAP and HR were significantly higher than those of pre-intubation(P0.05),with obvious stress response during intubation.Whereas there was no significant difference in all parameters between those pre-intubation and post-incision.The vital signs maintained stable during anesthesia and operation in PF-B and PR groups.There was no significant difference in BIS values between 3 groups.During recovery phase the awakening time of PR group was significantly shorter than those of PF-A and PF-B groups.Conclusion: Clinical administration of 4 μg/L fentanyl or 6 μg/L remifentanil can effectively suppress the hemodynamic response to tracheal intubation and incision in propofol TCI anesthesia.However,2 μg/L fentanyl is only enough to suppress the response to incision but not to intubation.

Abstract:Objective To determine the effect of fentanyl on emergence pain after intravenous remifentanil anesthesia.Methods Twenty-eight ASA Ⅰ or Ⅱ male patients, aged 26-58 years undergoing subtotal gastrectomy under general anesthesia were studied. Anesthesia was induced with midazolam 0.05 mg/kg, propofol 2 mg/kg, fentanyl 3 μg/kg and atracurium 0.8 mg/kg. The patients were mechanical ventilated after tracheal intubation for 5 min. Then fentanyl was injected again before incision by using modified Dixons up-and-down method. The time of total operation, extubation after operation and side effects such as nausea and vomiting were recorded.The atracurium infusion was terminated 30 min before the end of surgery. Remifentanil and propofol were terminated at the end of surgery. The score of Prince-Henry scores≥3 was defined as postoperative acute pain. ED50 and 95% confidence interval (CI) of fentanyl on postoperative acute pain were calculated.Results ED50 was 5.39 μg/kg. 95% CI was 5.21-5.58 μg/kg.One patient vomited 40 min after extubation.The average operation time was (93.8±10.2) min and the time of extubation was (14.2±3.5) min.Conclusion The ED50 and 95% CI of fentanyl for inhibition of acute pain after intravenous remifentanil anesthesia were 5.39 μg/kg and 5.21-5.58 μg/kg respectively.

Abstract:Introduction: The choice of medications used in rapid sequence intubation (RSI) can result in the difference between an acceptable outcome and a lethal one. When executed properly, RSI is a lifesaving intervention. Nonetheless, RSI may result in fatal complications such as peri-intubation cardiac arrest. The risk of peri-intubation cardiac arrest reportedly increases in patients who are profoundly hypoxic or hypotensive prior to endotracheal intubation. Medication choice for RSI may either optimize or deoptimize hemodynamic parameters, thereby impacting patient outcomes. Therefore, our study aimed to examine the association of change in mean arterial pressure (MAP) with and without the use of a predetermined dose of 50 micrograms (μg) intravenous fentanyl as a pretreatment agent during RSI. Methods: This prospective observational study included patients undergoing RSI at an academic emergency department (ED) over a three-year period between January 1, 2018–January 1, 2021. Average hemodynamic parameters were measured at the time of induction (prior to medication administration) and 10 minutes after induction. We categorized patients into fentanyl and non-fentanyl groups for analysis, and we compared data using chi-square and t-test as appropriate. Logistic regression analysis was conducted to account for potential confounding factors. Results: A total of 278 patients were included in the analysis, of whom 160 received fentanyl and 118 did not. The majority of the patients underwent RSI by trainees 95.0% of the time. The first-pass success rate was 77.7% in our sample and did not differ significantly between the two groups (P = 0.84). Unadjusted analysis showed a larger decrease in hemodynamic parameters in the fentanyl group compared to the non-fentanyl group; systolic blood pressure decreased by 11.2% vs 1.6%, diastolic blood pressure decreased by 13.7% vs 3.8%, and MAP decreased by 12.7% vs 3.2%. After adjusting for potential confounders, fentanyl was 2.14 times more likely to lower MAP by 10%. Conclusion: The use of 50 μg fentanyl for rapid sequence intubation in an ED is associated with higher odds of decreasing mean arterial pressure by at least 10% at 10 minutes from the time of induction. Therefore, it should be carefully dosed, and its use in clinical practice should be justified to avoid unnecessary complications.

Abstract:Background: Fentanyl is often administered during rapid sequence induction of anesthesia (RSI) in the emergency department (ED) to ameliorate the hypertensive response that may occur. Due to its more rapid onset, the use of alfentanil may be more consistent with both the onset time of the sedative and the commencement of laryngoscopy. As such, we compared the effect of alfentanil and fentanyl on post-induction hemodynamic changes when administered as part of a standardized induction regimen including ketamine and rocuronium in ED RSI. Methods: This was a double-blind pilot randomized controlled trial of adult patients requiring RSI in the ED of three urban Australian hospitals. Patients were randomized to receive either alfentanil or fentanyl in addition to ketamine and rocuronium for RSI. Non-invasive blood pressure and heart rate were measured immediately before and at two, four, and six minutes after induction. The primary outcome was the occurrence of at least one post-induction systolic blood pressure outside the pre-specified range of 100-160mmHg (with adjustment for patients with baseline hypertension). Secondary outcomes included hypertension, hypotension, hypoxia, first-pass intubation success, 30-day mortality, and the pattern of hemodynamic changes. Results: A total of 61 patients were included in the final analysis (31 in the alfentanil group and 30 in the fentanyl group). The primary outcome was met in 58% of the alfentanil group and 50% of the fentanyl group (difference 8%, 95% confidence interval: -17% to 33%). The 30-day mortality rate, first-pass success rate, and incidences of hypertension, hypotension, and hypoxia were similar between the groups. There were no significant differences in systolic blood pressure or heart rate between the groups at any of the measured time-points. Conclusion: Alfentanil and fentanyl produced comparable post-induction hemodynamic changes when used as adjuncts to ketamine in ED RSI. Future studies could consider comparing different dosages of these opioids.
Evaluation of Propofol Target Controlled Infusion with Fentanyl Intravenous Sedation on the Removal of Impacted Wisdom Tooth

Abstract:OBJECTIVE:To evaluate the effect and safety of propofol target controlled infusion (TCI) with a small dosage of fentanyl intravenous sedation on the removal of the third impacted molar tooth.METHODS:In the study, 58 ASA (American Society of Anesthesiology) I-II patients with impacted wisdom tooth removal (age: 18-45 years), were divided into two groups: control group (n = 26) received 40 mg lidocaine + propofol TCI; experimental group (n = 32) received 0.8 μg/kg fentanyl + propofol TCI. The complete evaluating indexes were recorded, and the data compared and analyzed.RESULTS:The differences of the gender, age, weight, operation time and sedation time were not significant. The heart rate and bispectral index of the experimental group on local anesthesia injection were significantly lower than those of the control group (P < 0.05). Other differences on heart rate, mean blood pressure, pulse oxygen saturation, respiration rate and bispectral index between the two groups were not significant during operation. The differences on recall of local anesthesia injection, incision and drilling between the two groups were not significant. The pain caused by the propofol injection, and the body movement caused by local anesthesia injection and talkativeness during operation in the experimental group were significantly lower than those in the control group (P < 0.05). No patient in the two groups had nausea and vomiting. After discharge, 19 patients in the experimental group (19/32), 14 patients in the control group (14/26) were sleepy after coming home. The difference of the patient's satisfaction between the two group was not significant, but the patient's satisfaction was significantly higher than the doctor's (P < 0.05), and the doctor's satisfaction in the experimental group was significantly higher than that in the control group (P < 0.05).CONCLUSION:Propofol TCI with a small dosage of fentanyl intravenous sedation on the removal of impacted wisdom tooth is effective and safe. Both patients and doctors have high degree of satisfaction on it.

Abstract:Objective To examine the differences in the level of sedation and adverse effects produced by equivalent analgesic doses of remifentanil,sufentanil and fentanyl.Methods Eighty ASA Ⅰ female patients aged 18-39 yr with BMI of 18-25 kg/m2 scheduled for laparoscopic surgery under general anesthesia were randomly divided into 4 groups ( n =20 each):group control (group C) ; group remifentanil ( group R) ; group sufentanil (group S) and group fentanyl (group F).Remifentanil 2 μg/kg,sufentanil 0.2 μg/kg and fentanyl 2 μg/kg in normal saline 10 ml were infused iv over 2 min in groups R,S and F respectively.Depth of sedation was assessed and scored using OAA/S scale (5 =alert,1 =no response to prodding) and wavele index (WLI),before (baseline) and at 2,4,6,8 and 10 min after drug administration.Besides RR,pulse oxygen saturation,BP,HR were also monitored.The incidences of apnea,muscle rigidity,nausea and vomiting,pruritus,vertigo,bradycardia,profuse sweating and skin rash were measured and calculated.Results The 4 groups were comparable with respect to age,BMI and height.The lowest values of OAA/S scores,WLI and RR were significantly lower in groups R,S and F than in group C.Sufentanil produced the deepest sedation among the 3 opioids.Remifentanil produced strongest respiratory depression and nausea and vomiting.Conclusion The equivalent analgesic dose of sufentanil produces deeper sedation than that of remifentanil and fentanyl while remifentanil has the greatest impact on RR.
