Evaluation of Target-Controlled Infusion of Remifentanil Combined with Sevoflurane during Laparoscopic Operations

Abstract:Objective To study the effects of target-controlled infusion (TCI) of remifentanil combined with sevoflurane at different minimal alveolar concentration (MAC) on anesthetic outcome and post-anesthetic recovery during and after laparoscopic operations. Methods Forty-five ASA stage Ⅰ-Ⅱ patients undergoing selective laparoscopic cholecystectomy or laparoscopic oophorocystectomy were randomly divided into three groups (n=15 in each), and induced with the same method. In group A, anesthesia was maintained with TCI of remifentanil (6 ng/ml) combined with sevoflurane inhalation (0.4 MAC); in group B, TCI of remifentanil (4 ng/ml) and sevoflurane inhalation (0.65 MAC) were used; while in group C, anesthesia was maintained with TCI of 2 ng/ml remifentanil combined with 1.0 MAC of sevoflurane inhalation. The bispectral index (BIS), mean arterial pressure (MAP), heart rate (HR), and the time duration of post-anesthetic recovery of breath and spatial orientation ability, eyes opening, and extubation were recorded. Results During the operations, the vital signs were stable in the 3 groups without needing to adjust the concentrations of remifentanil and sevoflurane. The MAP and HR in the 3 groups were similar at all the time points (P>0.05), while the values of BIS at all the time points in group A were significantly higher than those in groups B and C; meanwhile the BIS at different time points in group B were significantly higher than those in group C (both P<0.05). The time duration of post-anesthetic recovery was significantly different among the 3 groups. The time durations for breath recovery, eyes opening, extubation, and recovery of spatial orientation ability in groups A and B were significantly shorter than those in group C (P<0.05). Moreover,patients in group A opened their eyes earlier than those in group B (P<0.05). Conclusions During a laparoscopic surgery, the anesthetic depuh is satisfying by using TCI of 4 ng/ml remifentanil combined with 0.65 MAC sevoflurane inhalation or 2 ng/ml remifentanil combined with 1.0 MAC of sevoflurane. TCI of 4 ng/ml remifentanil combined with 0.65 MAC sevoflurane inhalation is superior for post-anesthetic recovery.
