Safey evaluation of retroperitoneoscopic surgery for adrenal pheochromocytoma

Abstract:[Objective] To compare the outcomes of retroperitoneoscopic surgery for PHE with the results of other adrenal pathologies,to evaluate the safety of retroperitoneal laparoscopic excision of pheochromocytoma.[Methods] Between June 2003 and January 2009,a single surgeon performed 137 retroperitoneoscopic adrenalectomies.Groups were generated according to patholoy:there are 102 cases of non-pheochromocytoma and 35 cases of pheochromocytoma.Clinical,intraoperative,postoperative,and follow-up data were prospectively collected and compared using t tests or χ2 tests.[Results] All laparoscopic procedures were successful performed by retroperitoneal approach,1 case of retroperitoneoscopic surgery for PHE were converted to open surgery,and no significant statistical differences were observed between pheo and non-pheo with respect to conversion to open adrenalectomy(P >0.05).There was no difference in the operative time(P >0.05),intraoperative blood pressure and heart rate variations(P >0.05) between the two groups.The patients with PHE had a higher mean estimated blood loss(P <0.05).The complication rate was 8.6% for the PHE group and 7.8% for the non-PHE group(P =0.8).The patients with PHE had a longer postoperative hospital stay(P <0.01) and overall hospital stay(P <0.01).Time in the intensive care unit(P <0.01) and time until oral intake(P <0.05) also were increased in the PHE population.There was no death between the two groups.4 cases lost during the follow-up of 7 to 36 months,1 case of adrenal cortical carcinoma died of liver metastasis,1 case of pheochromocytoma recurrenced,and the remaining symptoms were significantly reduced or disappeared,reviewed by ultrasound or CT no recurrence.[Conclusions] Based upon the experience and skills of the surgeons,retroperitoneal laparoscopie surgery for pheochromocytoma is feasible and safe the same as other adrenal diseases.
