Retroperitoneal laparoscopic surgery for adrenal pheochromocytoma

Abstract:Objective:To investigate the clinical application of retroperitoneal laparoscopic excision of adrenal pheochromocytoma.Methods:Retroperitoneal laparoscopic resection of adrenal pheochromocytoma was successfully performed on 19 patients (retroperitoneal laparoscopic group). Fourteen cases with adrenal pheochromocytoma underwent open surgery (open surgery group). The outcomes of the 2 groups were compared.Results:The mean dimension of the tumor was 4.1±1.7(range, 1.8 to 9.5)cm in retroperitoneal laparoscopic group VS 3.9±1.5(2.1 to 11.5)cm in open surgery group (P>0.05). The mean operation time was 90±35(range 40 to 180)min in retroperitoneal laparoscopic group VS 140±41(95 to 260)min in open surgery group (P<0.01). The mean volume of blood loss was 40±25(15 to 100)ml with no blood transfusion in retroperitoneal laparoscopic group VS 310±118(100 to 700)ml, with 6 patients receiving blood transfusion, in open surgery group (P<0.01). The mean post operative hospital stay was 6.0±1.5(4 to 8)days in retroperitoneal laparoscopic group VS 9.1±1.2(8 to 12)days in open surgery group (P<0.01).Conclusions:Based upon the experience and skills of the surgeons, retroperitoneal laparoscopic surgery for adrenal pheochromocytoma is safe and effective, less traumatic to the patients, with less postoperative discomfort and quicker recovery. The procedure should be the first choice of management for adrenal pheochromocytoma.
