Abstract:Objective To investigate the clinical features of acquired immunodeficiency syndrome (AIDS) with neurological complications. Methods Five with neurological complications out of 31 patients with confirmed AIDS in Shanghai area from 1992 to 2001 were analyzed. Results Of the five cases, there were one complicated with vacuolar myelopathy, one vacuolar myelopathy accompanied with dementia, one cerebral tuberculoma accompanied with dementia, one cerebral infarction accompanied with symptomatic epilepsia and trigeminal neuralgia, one polyneuropathy. Two of the vacuolar myelopathy are those showing first occurring manifestations. Conclusion AIDS patients are found at high risk of neurological complications, of which vacuolar myelopathy and AIDS related dementia are more common. If young patients who suddenly suffered from dementia, especially in patients accompanied with opportunistic infections, AIDS should be considered. In these cases serum HIV antibodies should be checked.

Abstract:Hydrocephalus is a common medical condition characterized by abnormalities in the secretion,circulation or resorption of cerebrospinal fluid (CSF), resulting in ventricular dilatation. The pathogenetic mechanism for the hydrocephalus is attributed to: the overproduction of CSF by the choroid plexus; the defect in CSF absorption and obstruction of CSF flow in the cerebral ventricles. However, the underlying etiology is poorly understood. With the development of genetic engineering, a growing body of evidence indicates that genetic factors play an essential role in the pathogenesis of hydrocephalus. It is the aim of this review to summarize these findings.

Abstract:Objective To observe ocular and general clinical characteristics of initial ophthalmic manifestation of patients in acquired immunodeficiency syndrome and human immunodeficiency virus (HIV) infection.Methods A retrospective study was conducted in 9 patients (15 eyes).All patients are males.The patients ages ranged from 35 to 63 years,with a mean age of (48±i0) years.All patients' visual acuity,intraocular pressure,slit-lamp microscopy and mydriatic indirect ophthalmoscopy,fluorescein angiography,the antibody of serum HIV,CMZ and CD4+T cell count,and serologic test for syphilis were documented.Patients were followed for a period between 2 to 56 months,with a median of 16 months.Results The diagnosis was confirmed by clinical and laboratory tests.Six patients were bilateral.In 15 eyes,8 eyes(53.3％) showed typical CMZ retinitis,3 eyes (20％) showed HIV retinopathy.2 eyes (13.3％) were diagnosised syphilitic uveitis while 2 eyes (13.3％) manifested as ocular neurosyphilis.CD4+T-lymphocytes counts in peripheral blood were between 2～256/mm3,median 20/mm3.Conclusions Ophthalmic symptoms maybe the initial reception in patients with HIV/AIDS.The serological examination for HIV should be considered for patients with unexplained retinopathy.

Abstract:AIM: Acquired hepatocerebral degeneration (AHD) is an exceptional type of hepatic encephalopathies (HE). It is characterized by neuropsychiatric and extrapyramidal symptomathology similar to that seen in hepatolenticular degeneration (Wilson's disease). In this paper, we report a case of AHD with unusual presenting features.METHODS: A 28-year-old man with AHD was described and the literature was reviewed.RESULTS: The man had a history of HBV-related liver cirrhosis. He was admitted to our hospital with apathy, dysarthria, mild consciousness impairment and extrapyramidal symptoms after hematemesis. By review of the literature, cases with AHD often did not present consciousness impairment. So our case was once diagnosed incorrectly as Wilson's disease.CONCLUSION: AHD is a rare syndrome and its variable clinical manifestations make it difficult to be diagnosed. But we believe that extensive examination and thorough understanding of the disease are beneficial to a correct diagnosis. Moreover, biocoene is effective in treating the case. Clinical analysis of HIV-Starting with central nervous system lesions in patients

Abstract:Objective The present study aimed to determine clinical presentation of HIV-associated central nervous system lesions in patients as the first presentation.Methods A retrospective study of 39 patients with HIV-associated cen-tral nervous system lesions who were hospitalized at Nanjing Brain Hospital from January 1st 2008 to December 31st 2015 was carried out.Results The mean age of the study population was 40.9 ±9.4 years,and median CD4 count was 55 cells/mm(3) (interquartile range (QR:25 ~105).The most common clinical presentations were headache (87.2%), Weight loss (71.8%), and fever (66.7%).Toxoplasma encephalitis (28.2%), AIDS dementia complex ( ADC) (17.9%),primary central nervous system lymphoma(PCNSL)(15.4%),unexplained encephalitis(15.4%)and Strepto-coccus meningitis (12.8%)、 Tuberculous meningitis (12.8%)、 intracranial space occupying lesion of unknown origin (12.8%)were the leading aetiologies of HIV-associated CNS disease of patients.Conclusion HIV-associated CNS dis-ease is common among HIV patients.CNS symptoms of these diseases are complex and sometimes easy to be misdiagnosed. We should pay attention to screening of anti-HIV antibody.

Abstract:A male in his 30s with a medical history of newly diagnosed HIV with a CD4 count of 292 cells/mm3 presented with a bilateral frontal headache and left upper and lower extremity weakness and paraesthesias. A few months prior, the patient experienced a desquamating rash on his scalp and a pruritic, papular genital rash, which both self-resolved. CT head without contrast revealed extensive vasogenic oedema involving the right basal ganglia, thalamus, temporal and occipital lobes. MRI of the brain with and without contrast revealed two enhancing masses in the right lentiform nucleus and right temporal-occipital junction with associated vasogenic oedema. Cerebrospinal fluid (CSF) studies confirmed cerebral toxoplasmosis with positive CSF Toxoplasma gondii PCR and neurosyphilis with positive serum rapid plasma reagin and CSF venereal disease research laboratory test. He was treated with trimethoprim/sulfamethoxazole and intravenous penicillin G with the resolution of his symptoms.

Abstract:One case of acquired immunodeficiency syndrome (AIDS) complicated with Pneumocystis carinii pneumonia was retrospectively reviewed . The clinical manifestations and histopathological changes were analyzed . After a positive diagnosis of AIDS by laboratory examinations , the patient underwent a bronchoscopic lung biopsy . Pneumocystis carinii was detected in the obtained lung tissues by Gomori's methenamine silver staining (GMS ) and periodic acid-Schiff staining (PAS ) . With a sulfamethoxazole (SMZ) treatment ,the patient recovered from pneumonia .AIDS patients are vulnerable to Pneumocystis carinii ,and the complication often leads to interstitial pneumonia .The physicians should take measures to consider possible Pneumocystis carini infection and to select effective treatment after a positive detection .

Abstract:A 40-year-old Japanese man with human immunodeficiency virus presented with disturbance of consciousness 2 years after dropping out. He was emaciated without any zoster skin lesions, and his CD4+ lymphocyte count was 4 cells/μL. Disseminated Mycobacterium avium infection and pneumocystis pneumonia were diagnosed, and a brain biopsy revealed varicella zoster virus (VZV) encephalitis with multiple real-time polymerase chain reaction tests and immunochemistry. Antiretroviral therapy and intravenous acyclovir were administered, and the patient survived with neurological sequelae. VZV encephalitis is rare, particularly among people with AIDS, but it is a significant disease with a poor prognosis.
Prognosis effect of intracranial hypertension in different types of cerebrospinal fluid drainage patients with AIDS merged with cryptococcal meningitis

Abstract:Objective To discuss the treatment strategies of intracranial hypertension among AIDS patients with cryptococcal meningitis,to reduce the mortality.Methods Thirty-four AIDS patients with cryptococcal meningitis with intracranial hypertension were treated by using repeated lumbar puncture,waist large pool catheter drainage,ommaya cystic drainage or ventricular peritoneal shunt,according to the level of initial intracranial pressure.The outcomes of different strategies of cerebrospinal fluid draining were compared.Results Six patients with intracranial pressure of 200-249 mmH2O were all cured.Out of the nine patients with intracranial pressure of 250-350mmH2O,eight recovered and one died.Among the nineteen patients with intracranial pressure higher than 350 mmH2O,thirteen recovered and six died.Twenty-seven patients were treated with repeated lumbar puncture,with twenty-two recovered and five dead.Four patients were treated with external drainage,with two recovered and two dead.Three patients were treated with external and internal drainage were all recovered.Conclusion Different strategies of lowering intracranial hypertension can be adopted to treat AIDS patients merged with new cryptococcal meningitis with intracranial hypertension,according to their initial level of intracranial pressure.Based on sufficient antifungal therapy,waist large pool catheter drainage,ommaya cystic drainage and ventricle peritoneal shunt were suggested to carry out as early as possible to control intracranial hypertension,improve clinical efficacy and reduce the mortality.

Abstract:A 78-year-old man was admitted to the hospital with a 4-day history of fever and confusion. Physical examination revealed oral dryness and decreased skin turgor. Blood tests showed hyponatremia (121.5 ‍mEq/l), and cerebrospinal fluid examination revealed positivity for herpes simplex virus 1 (HSV-1) via polymerase chain reaction. He was diagnosed with herpes simplex encephalitis and initiated acyclovir treatment. The hyponatremia was diagnosed as cerebral salt wasting syndrome (CSWS) and treated with hypertonic saline infusion and fludrocortisone. The cerebrospinal fluid HSV-1 DNA became negative, and the serum sodium levels normalized. Hyponatremia complicated with encephalitis is often caused by the syndrome of inappropriate secretion of antidiuretic hormone (SIADH), whereas CSWS is rare, mostly observed in tuberculous meningitis. Differentiating between the SIADH and CSWS is important as they require distinct therapeutic strategies.

Abstract:Objective:To investigate the clinical manifestations, imaging findings, characteristics of cerebrospinal fluid, treatment and prognosis in patients with acquired immune deficiency syndrome (AIDS)-related progressive multifocal leukoencephalopathy (PML) diagnosed by second generation sequencing techniques with cerebrospinal fluid.Methods:From October, 2017 to January, 2019, ten patients with AIDS-related PML were diagnosed by second generation sequencing techniques with cerebrospinal fluid, and their clinical data were retrospectively reviewed. Descriptive analysis was used.Results:Among the ten patients with AIDS-related PML, eight were males and two were females. The age was (38.7±8.2) years. The median CD4 + T lymphocyte count was 46/μL. The median human immunodeficiency virus (HIV) RNA was 5.99×10 4copies/mL. The main clinical manifestations included dyskinesia in four patients, dizziness in three patients, cognitive decline in two patients, and speech disorder in one patient. Three patients developed convulsion throughout the course of the disease. Image findings indicated that lesions could appear in any part, including lesions located under the tentorium of cerebellum in three cases, and above the cerebellum in seven cases. Mild increase of total protein was observed in the cerebrospinal fluid of four cases, while white blood cell count ((0-10)×10 6/L), sugar and chloride were normal in all cases. The reads of JC virus sequence in the cerebrospinal fluid ranged from 3-12 531 reads (median 67 reads). All the patients received antiviral therapy containing integrase inhibitors. Among the 10 patients, two died in hospital and eight survived, and two of them had survived for more than one year. The symptoms were improved in five patients and unchanged in three patients. Conclusions:PML is less common in patients with HIV/AIDS. The second generation sequencing techniques with cerebrospinal fluid can help to diagnose the disease together with clinical manifestations, routine detection of cerebrospinal fluid and imaging. Potent antiretroviral treatment may improve the prognosis of the patients.

Abstract:Abstract BackgroundAIDS existing with cerebral sparganosis is rare. Due to atypical symptoms of sparganosis, diagnosis and treatment are difficult and the misdiagnosis rate is very high. This paper analyzes the clinical data of the first case of AIDS existing with cerebral sparganosis in our hospital and reviews the relevant literature. It discusses the clinical manifestations, imaging features, diagnosis, and treatment of this disease and provides a reference for the treatment of AIDS existing with parasitic encephalopathy (cerebral sparganosis). Cerebral sparganosis has been reported worldwide especially in Asian countries. To date, this is the first reported case of sparganosis in HIV/AIDS patient.Case presentationA 34-year-old newly diagnosed HIV Chinese woman from Guanxi province presented at our hospital with a history of recurrent seizures for 5 years. The initial diagnosis was epilepsy but she was later diagnosed with cerebral sparganosis at our facility after thorough history, examination and investigations. Considering her immune status, she was treated with high dose praziquantel instead of surgery with significant improvement. She also received Zidovudine + Lamivudine + Nevirapine as the first-line antiretroviral treatment due to its high permeability across the blood-brain barrier.ConclusionClinicians should pay attention to this rare entity in cases of HIV/AIDS-related encephalopathies which can be easily misdiagnosed as toxoplasmosis or cerebral tuberculosis. Thorough epidemiological history, clinical presentations, serologic and radiological tests are essential to reach the correct diagnosis and rule out other infections. Although the standard treatment for cerebral sparganosis is surgery, we opted for pharmacological treatment in consideration of immunological complications.This case calls for more studies for standard treatment in HIV patients with cerebral sparganosis and requires clinicians to consider this rare entity when dealing with HIV related encephalopathies.
The syndrome of transient headache and neurological deficits with cerebrospinal fluid lymphocytosis (HaNDL):A case report with clinical follow-up study

Abstract:Objective To explore the clinical,electroencephalographic features and cerebrospinal fluid abnormalities in the syndrome of transient headache and neurological deficits with cerebrospinal fluid lymphocytosis (HaNDL). Methods A 27-year-old male presented with several episodes of numbness,weakness followed by migraine-like headache for over 1 month,sometimes associated with aphasia or agnosia. Cranial MRI and CTA were unremarkable during attacks. Ultrasonogra-phy revealed defect of foramen ovale. Follow-up study was performed during attacks and between attacks,including electro-encephalography and cerebral spinal fluid. Results The EEG showed asymmetrical excessive slowing waves during attacks. CSF revealed lymphocytic pleocytosis and elevated protein during attacks and between attacks. Flow cytometry revealed 90. 6% CD3 +,3. 3% CD19 +,3. 7% CD16 +CD56 +and the ratio of CD4 +and CD8 +was 1. 84. Over one month his symptoms were resolved and the patient remained asymptomatic during 6 months of follow up. Conclusions The attack of HaNDL syndrome can mimic stroke. It is characterized by focal neurologic deficits,headache and cerebrospinal fluid lymphocytosis. EEG abnormalities correlate with the clinical symptoms and can be used for follow-up.
Disease Spectrum and Prognostic Factors of 499 Cases of Acquired Immune Deficiency Syndrome Complicated with Central Nervous System Infections in Chongqing

Abstract:Objective To describe the disease spectrum,morbidity,mortality and prognostic factors of acquired immune deficiency syndrome (AIDS) patients complicated with central nervous system (CNS) infections.Methods The data of 4 426 AIDS patients from February 2013 to February 2017 in Chongqing public health medical center were collected,among which 499 cases had CNS infection.The morbidity and mortality of CNS infections were calculated.Association between different CNS infections and CD4+T cell counts was analyzed.Prognostic factors for the outcome of hospitalization were also studied.Mann-Whitney U test was used for continuous variables.Univariate and multivariate analyses were performed by logistic regression analysis.Results The morbidity of CNS infections in AIDS patients was 11.27％ (499/4 426).The most prevalent CNS infections were tuberculous meningitis (4.50％),cryptococcal meningitis (3.25 ％) and CNS infections with unknown etiology (1.11 ％).The mortality rate was 18.84％ (94/499),among which tuberculous meningitis accounted for 35 cases (17.59％),cryptococcal meningitis 23 cases (15.79％) and CNS infections with unknown etiology 19 cases (38.76％).The average CD4-T cell count level in those who died were significantly lower than that in those who survived (Z=2.51,P =0.001).Visual impairment,nuchal rigidity,positive pathologic reflexes,consciousness disturbance,CD4+T cell counts＜50 cells/μL and HIV RNA≥5 lg copies/mL at baseline were independent prognostic factors for mortality.Conclusions The morbidity and mortality of CNS infections are high among AIDS patients in Chongqing,and those patients with severe immunosuppression are usually affected.Older age,consciousness disturtance and severe immunosuppression are three independent risk factors for mortality.

Abstract:Background: In Beijing, from May to August, 2008, an epidemic of hand, foot, and mouth disease (HFMD) occurred with central nervous system (CNS) complications. METHODS: Clinical data of 91 inpatients who suffered from HFMD associated with aseptic meningitis and 54 HFMD cases associated with encephalitis were retrospectively analyzed, and were compared with 151 HFMD children without CNS complications. Results: 91 cases with a mean age of 44.5±32.6 months, had associated aseptic meningitis, and all survived. The most frequently associated symptoms were vomiting (84.6%), myoclonic jerks (60.4%), neck stiffness (54.9%) and headache (47.3%). 54 children with encephalitis (mean age 19.8±13.0 months) had symptoms of altered consciousness (68.6%), myoclonic jerks (77.2%), convulsions (27.8%), and cranial nerve involvement (18.5%). 3 cases died of neurogenic pulmonary edema. Among 54 children, 15 patients (27.8%) had associated acute flaccid paralysis (AFP). The MRI of 7 cases displayed myelitis, and 3 cases displayed brainstem encephalitis. 7 of 15 children still had neurologic deficits when they were discharged. There was a very significant difference in age of onset between children with aseptic meningitis and those associated encephalitis (P< 0.01). Most of abnormal neurologic findings except AFP occurred from days 2 to 4 of the clinical course. All 151 children without CNS complications recovered.
