Abstract:Objective To explore the clinical, electroencephalographic features and cerebrospinal fluid abnormalities in the syndrome of transient headache and neurological deficits with cerebrospinal fluid lymphocytosis (HaNDL). Methods A 27-year-old male presented with several episodes of numbness, weakness followed by migraine-like headache for over 1 month, sometimes associated with aphasia or agnosia. Cranial MRI and CTA were unremarkable during attacks. Ultrasonogra-phy revealed defect of foramen ovale. Follow-up study was performed during attacks and between attacks, including electro-encephalography and cerebral spinal fluid. Results The EEG showed asymmetrical excessive slowing waves during attacks. CSF revealed lymphocytic pleocytosis and elevated protein during attacks and between attacks. Flow cytometry revealed 90. 6% CD3 +,3. 3% CD19 +,3. 7% CD16 +CD56 +and the ratio of CD4 +and CD8 +was 1. 84. Over one month his symptoms were resolved and the patient remained asymptomatic during 6 months of follow up. Conclusions The attack of HaNDL syndrome can mimic stroke. It is characterized by focal neurologic deficits, headache and cerebrospinal fluid lymphocytosis. EEG abnormalities correlate with the clinical symptoms and can be used for follow-up.
