Abstract:An 11-year-old girl presented with frequent chest pain, fever, and a cough that she had had for more than 13 months, as well as pleural effusion and large pericardial effusion. She was misdiagnosed with tuberculosis and received anti-tuberculosis drugs for 6 months. Within the past year, she also underwent two more thoracotomies and a thoracoscopic partial pericardiectomy. The final diagnosis of pulmonary paragonimiasis was established once it was known that she had eosinophilia, always drank stream water, and tested positive for antibodies against Paragonimus. Since antiparasitic praziquantel therapy was effective, paragonimiasis should be considered as a possibility in the differential diagnosis of tuberculosis in children.
Drug-induced Erythroderma in Patients with Acquired Immunodeficiency Syndrome.

Abstract:BACKGROUND:To explore the clinical manifestations, diagnosis, and treatment of patients with acquired immunodeficiency syndrome (AIDS) complicated with drug-induced erythroderma.METHODS:The clinical data of 12 AIDS patients with drug-induced erythroderma in our hospital were retrospectively analyzed. The general information, offending medications, complications, modified severity-of-illness score for toxic epidermal necrolysis (SCORTEN) scores, and disease outcome spectrums were analyzed.RESULTS:Drug-induced erythroderma was mostly caused by antiviral drugs, antituberculosis drugs, antibiotics, traditional Chinese medicine, and immune checkpoint inhibitors. The spectrum of sensitizing drugs was broad, the clinical situation was complex, and infections were common. The affected areas were greater than 40% body surface area in all patients. The modified SCOTERN score averaged 3.01±0.99. All patients were treated with glucocorticoids, and nine patients were treated with intravenous immunoglobulin (IVIG) pulse therapy at the same time. The average time to effectiveness was 7.08±2.23 days, and the average hospital stay was 17.92±8.46 days. Eleven patients were cured, and one patient died of secondary multiple infections, who had a modified SCORTEN score of 5 points. The mortality rate in this study was 8.3%.CONCLUSIONS:The clinical situation of AIDS patients with drug-induced erythroderma in hospitalized patients is complex and the co-infection rate is high. The use of modified SCORTEN score may objectively and accurately assess the conditions, and the use of glucocorticoid combined with IVIG therapy may improve the prognosis.
Clinical epidemiology of 359 cases of acquired immunodeficiency syndrome and pulmonary tuberculosis co-infection

Abstract:Objective To analyze the epidemiology and clinical characteristics of acquired immunodeficiency syndrome (AIDS) and pulmonary tuberculosis (TB) co-infection.Methods A retrospective study was conducted with the clinical data of patients diagnosed with AIDS and TB in Shanghai Public Health Clinical Center during the period from 2011 to 2015.The outcome of the patients were evaluated by outpatient and telephone follow-up.The data were analyzed by descriptive analysis using SPSS 22.0 software package.Results A total of 359 patients with AIDS/TB co-infection were included in this analysis,including 325 males and 34 females,the highest proportion in 30-44 age group.The diagnosis was delayed in about 42.6％ of the patients.The clinical symptoms were mainly fever,cough and weight loss,but hemoptysis uncommon.Both lungs were affected in most cases,with lesions in at least 3 lung fields,but rare pulmonary cavity.T-SPOT.TB test showed lower positive rate.CD4+T lymphocyte count was 50 cells/μL or less in 50.7％ of the patients at their first test.About 43.5％ of the 69 patients with antimicrobial susceptibility data showed resistance to therapy.Majority (93.2％) of the patients with known viral status received antiretroviral treatment.Extra-pulmonary tuberculosis was identified in 282 cases.The complication and opportunistic infection included central nervous system infection,syphilis,hepatitis B virus infection,hepatitis C virus infection,pulmonary infection,and drug-induced liver injury.Of the 333 patients with known outcome,53 died,most (79.2％,42/53) within 6 months.Conclusions The patients with AIDS/TB co-infection showed higher proportion of young people.The CT finding was atypical.The patients showed lower positive rate for T-SPOT TB test and lower CD4+T lymphocyte count at their first test.Most patients had extra-pulmonary tuberculosis and other complications or opportunistic infections.

Abstract:OBJECTIVE To explore the value of imaging diagnosis of pulmonary infections in patients with acquired immunodeficiency syndrome(AIDS).METHODS A total of 389 HIV patients complicated with pulmonary infec-tions who were treated from Jan 2012 to Jan 2016 were enrolled in the study.The laboratory examination and CT examination were carried out,and the incidence of pulmonary infections and imaging manifestations of the HIV pa-tients were analyzed.RESULTS Of the 389 patients,176 were aged between 18 and 29 years old,accounting for 45.2%;among the patients with pulmonary infections,199 had pulmonary tuberculosis,accounting for 51.2%. Of the 199 patients with pulmonary tuberculosis,168 showed diversified tuberculosis lesions forms during the AIDS stage,the rest of 31 patients showed mild X-ray signs,and 5 patients had moderate pleural effusion.Among the 117 patients with sporotrichosis,the hairy groundglass infiltration and the symmetry reticular or nodular shad-ow dispersed around the bilateral hilus pulmonis were the typical X-ray manifestations,the whole lung was in-volved within a few days,which promoted the occurrence of diffuse consolidation,however,the reticular opacities still appeared in the lung periphery.CONCLUSION The value of the imaging diagnosis is high for the HIV patients complicated with pulmonary infections;CT,especially high resolution CT,may effectively show the characteris-tics of focus and provide guidance for clinical diagnosis and treatment so as to improve the prognosis of the pa-tients,and it is worthy to be promoted in the hospital.

Abstract:One case of acquired immunodeficiency syndrome (AIDS) complicated with Pneumocystis carinii pneumonia was retrospectively reviewed . The clinical manifestations and histopathological changes were analyzed . After a positive diagnosis of AIDS by laboratory examinations , the patient underwent a bronchoscopic lung biopsy . Pneumocystis carinii was detected in the obtained lung tissues by Gomori's methenamine silver staining (GMS ) and periodic acid-Schiff staining (PAS ) . With a sulfamethoxazole (SMZ) treatment ,the patient recovered from pneumonia .AIDS patients are vulnerable to Pneumocystis carinii ,and the complication often leads to interstitial pneumonia .The physicians should take measures to consider possible Pneumocystis carini infection and to select effective treatment after a positive detection .

Abstract:Introduction: Pneumocystis pneumonia (PCP) is one of the most common opportunistic infections in HIV-infected patients. However, coinfection with Tropheryma whipplei is infrequent in AIDS patients with PCP. Case Presentation: We report a 28-year-old male AIDS patient coinfected with T. whipplei and Pneumocystis jirovecii diagnosed in the bronchoalveolar lavage. After sulfamethoxazole-trimethoprim and meropenem treatment, the patient showed clinical improvement in 2 weeks. Conclusion: Clinicians need to be alert to the occurrence of T. whipplei infection in AIDS patients with PCP and timely diagnosis and antibacterial treatments are essential. This case may help clinicians for timely diagnosis of the coinfection of T. whipplei and P. jirovecii in AIDS patients.

Abstract:Objective To enhence the recognition of acquired immunodeficiency syndrome with pulmonary opportunistic infection. Methods The clinical data of two cases diagnosed in our hospital were reported and some literature was reviewed. Results The AIDS patients were at the high risk of suffering from pulmonary opportunistic infection, among which pneumocystis carinii pneumonia and fungal infection were the most common ones. Conclusion The incidence of AIDS with pulmonary opportunistic infection is increasing yearly. In order to diagnose it, much more attention should be paid to it.

Abstract:Chlamydia psittaci is the pathogen of psittacosis and infects a wide range of birds and even humans. Human infection occurs most commonly in those with a history of contact with birds or poultry. We describe a case of psittacosis in a human immunodeficiency virus infected patient in Zhejiang Province for the first time. C. psittaci infection was confirmed by nested polymerase chain reaction (PCR) and Real-Time PCR. Phylogenetic analysis revealed that the sequences from the patient’s samples clustered with genotype A in the same branch. Our study highlights the possibility of diagnosing psittacosis in patients with a chronic disease such as HIV-infected patients, and should increase awareness and surveillance of psittacosis in China.

Abstract:Here a rare case of primary splenic tuberculosis is reported. The immunocompetent patient was admitted to our department for fever, without definite pulmonary infection. He was ultimately diagnosed based on pathologic evidence after splenectomy and positive culture for mycobacterium. The clinical manifestation of the disease and diagnostic procedure used in this case will help clinicians recognize this disease.

Abstract:Background: In rare cases, people living with chronic human immunodeficiency virus (HIV) infection do not develop antibodies despite demonstrable infection. Delayed or missed diagnosis of HIV infection leads to a lack of timely therapy, resulting in rapid disease progression with opportunistic infections or malignancies. Case report: A 44-year-old Chinese man presented with sore throat, oral leukoplakia, fever, dyspnoea and diffuse ground glass-like lesions in both lungs. Serum cytomegalovirus DNA was detectable, and CD4+ T-cell count was low. The patient was suspected of being a person living with HIV despite of the repeatedly negative HIV antibody tests using enzyme-linked immunsorbent assay and Western blot. Subsequently, high-plasma HIV RNA viral load was found on two repeated tests, while HIV DNA was also positive. Thus, the patient was confirmed as presenting with HIV-seronegative acquired immunodeficiency syndrome (AIDS). The symptoms improved in response to effective anti-fungal and anti-retroviral therapy after diagnosis. Conclusion: This is the third reported case of an HIV-seronegative AIDS patient in China, which are also rarely reported globally. HIV nucleic acid testing is important to screen out HIV infection, especially in those who present with severe immunodeficiency but remain HIV serogenative.

Abstract:Pulmonary Tuberculosis (TB) is common in China, but tuberculosis with coagulation disorders and pancytopenia have rarely been reported in the past. In this report presented, a 70-year-old female was admitted to the hospital with poor appetite, dark urine, nausea, vomiting, fatigue, and bilateral lower limb edema; chest CT suggested diffuse infectious lesions in both lungs, coagulation dysfunction, and complete pancytopenia, which was initially considered to be caused by severe infection. However, the patient's symptoms did not improve by potent empiric antibiotics treatment, and a repeat chest CT showed that the lung lesions deteriorated more than before, and coagulation disorders and pancytopenia did not improve. Finally, the TB patient tested positive for enzyme-linked immunospot assay (ELISPOT) and metagenomic sequencing (mNGS) of Mycobacterium tuberculosis (MTB) using bronchoscopic alveolar lavage. So ati-TB was initiated with HRftELfx (isoniazid, 0.3 g qd; rifapentine, 0.45 g biw; ethambutol, 0.75 g qd; and levofloxacin, 0.5 g qd) regimen. Eventually, the patient's clinical symptoms improved significantly, the pulmonary lesions were absorbed, and the coagulation function and blood cell count returned to normal, which achieved a satisfactory treatment effect.

Abstract:Objective To investigate the characteristics of opportunistic infections in acquired immunodeficiency syndrome (AIDS) patients.Methods A comprehensive retrospective chart review study was conducted of 603 AIDS patients with different opportunistic infections in a special hospital authorized for AIDS patients.Results In all 603 patients,327 cases (54.2％) were infected with Pneumocystisjiroveci,251 cases (41.6％) were infected with fungus and 210 cases (34.8％) were infected with mycobacterium.Totally 367 cases (60.9％) had only one pathogen and 236 cases (39.1％) were co-infected with more than one pathogen.A total of 390 cases (64.7％) had a single site of infection with the respiratory tract being most frequently involved (271 cases,69.5％),and 213 cases (35.3％) had more than one infected site.The most common coinfected sites were oropharynx and respiratory tract (111 cases,52.1％).Conclusions AIDS patients are usually coinfected with multiple pathogenic organisms and multi-systems involved.Most of the opportunistic infections can be cured by timely diagnosis and effective treatment to prolong life and improve the quality of life of AIDS patients.
A case of eosinophilic granulomatosis with polyangiitis combined with pulmonary tuberculosis: A case report

Abstract:Rationale: Eosinophilic granulomatosis with polyangiitis (EGPA) is a rare autoimmune disease that can affect multiple organ systems. The standard treatment mainly relies on glucocorticoids and immunosuppressive agents. In our study, we present an EGPA patient who had pulmonary tuberculous mycobacteria infection, such cases are rarely reported. Patient concerns: A 71-year-old male patient was diagnosed with EGPA (systemic type) and pulmonary tuberculosis simultaneously. Diagnoses: The Five-Factor score indicated that the patient required glucocorticoids combined with immunosuppressive agents for induction therapy, however, the use of immunosuppressive agents would significantly inhibit antituberculosis treatment. Nowadays, treating active autoimmune disease in patients with infections remains a clinical challenge. Interventions: Considering the patient did not show life-threatening or severe organ involvement and reduced the effect of antituberculosis immunity, we used glucocorticoids alone. Outcomes: Finally, the patient had no adverse events, the eosinophil counts were markedly decreased and symptoms of EGPA were relieved. Lessons: The patient of EGPA combined with pulmonary tuberculosis successfully treated with glucocorticoids alone may provide significant support in selecting the appropriate treatments for similar cases in the future.

Abstract:Background: As disseminated extrapulmonary tuberculosis infection can involve multiple systems and result in atypical clinical manifestations that mimic other diseases, the diagnosis may be difficult, especially in elderly patients. Delaying treatment can adversely affect the prognosis. And to achieve better prognosis, early detection and diagnosis are necessary, as well as early initiation of comprehensive treatment. Case presentation: We present the case of a 78-year-old man with disseminated tuberculosis who developed the uncommon complication of urinary retention due to a psoas abscess, meningoencephalitis, and the rare secondary hemophagocytic lymphohistiocytosis syndrome. The patient achieved a favorable outcome following targeted therapy with antitubercular medications. Conclusions: This case highlights that disseminated extrapulmonary tuberculosis infection can present with a variety of manifestations, and may exhibit many rare and complex clinical presentations. Prompt and accurate diagnosis and treatment play a crucial role in improving prognosis for the patients with persistent fever.

Abstract:Disseminated tuberculosis first presenting as cutaneous lesion is uncommon and difficult to diagnose on account of its divers clinical forms. We present a 34-year-old obese woman with rash, finger swelling and fever. The patient was firstly manifested by fever and mass in palm and finger, gradually involving multisystems injury. She was diagnosed with disseminated tuberculosis in consideration of multiple system involvement, imaging characteristics, tuberculous granuloma in dermis and positive result of Mycobacterium culture. After two months of anti-tuberculosis treatment, fever and multisystems injury were controlled effectively, but skeletal and bone marrow involvement continued to progress, experiencing tuberculosis related acute hematopoietic stagnation. Continuing intensive therapy to one year, skeletal and hematological involvement improved and treatment was discontinued after two years. Follow-up to now with drug withdrawal for more than 1year, the patient remains in remission.
