Detection of focal liver lesions using contrast-enhanced ultrasonography with low mechanical index

Abstract:Objective To evaluate the usefulness of contrast-enhanced, contrast-specific ultrasonographic technique with low mechanical index for characterization of focal liver lesions.Methods Contrast-specific ultrasonography was used to assess 363 patients with 389 focal liver lesions: 256 primary liver carcinomas, 53 hepatic metastases, 49 hemangiomas, 19 focal nodular hyperplasias, 7 hepatic abscesses and 5 inflammatory pseudotumors of the liver. Ultrasonographic features of hepatic lesions were analyzed and compared with clinical and pathologic results. Results On contrast-enhanced ultrasonography, the typical hemodynamic pattern of primary liver carcinoma was the whole-lesion enhancement or mosaic enhancement in the arterial phase with enhancement defect in the late phase (sensitivity, 93.7%; specificity, 69.9%). The enhancement pattern of metastases was whole-lesion or rimlike enhancement in the arterial phase and early wash-out in the portal phase (sensitivity, 41.5%; specificity, 96.7%); The most common enhancement pattern of hemangioma was that the enhancement appeared in the periphery first and progressively was filled into the lesion center (sensitivity, 95.9%; specificity, 99.1%). The enhancement pattern of focal nodular hyperplasia was that the whole lesion was enhanced early and rapidly in the arterial phase with a centrifugal configuration and presented iso-or hyperechogenic till the late phase (sensitivity, 84.2%; specificity, 98.9%). The specific enhancement of hepatic abscess was the honeycomb-like enhancement in all phases (sensitivity, 71.4%; specificity, 100%). No enhancement of a lesion in all phases was specific for inflammatory pseudotumor of the liver. Conclusions Contrast-enhanced ultrasonography is a promising approach in the noninvasive characterization of focal liver lesions and can be usefully employed as a first-line imaging technique clinically when a focal liver lesion is detectable on ultrasonography.
