Timing and technique of operation on foreign body in bronchus in children

Abstract:Objective To investigate timing and technique of surgical treatment for special bron- chial foreign body in children.Methods Twenty-two patients with foreign body in bronchus were re- viewed.There were 16 boys and 6 girls ranging in age from 3 to 12 years(mean 7.2±3.4 yr).Of the cases studied,peanuts and sunflower seed were aspirated from 13 cases,metallic foreign body was as- pirated from 3 cases and 6 from the others.All cases failed an endoscopic removal.Three cases under- went urgent operations and 19 cases received selective techniques.Results The operative techniques included bronchotomy and removal of foreign body in 17 cases,pulmonary lobectomy in 4 cases.Post- operative complications were pulmonary atelectasis(2),incision infection(1)respiratory insufficiency (1).One case of failed operation died of multiple organ failure.Conclusions Ascertaining the nature and location of the foreign body with selective surgical operation is important to the management of bronchial foreign body in children,the urgent operation should be avoid if possible.In cases of long term pulmonary suppuration or atelectasis unaffected by antibiotics administration,the possibility of foreign body in bronchus should be considered.
