TCD combined with evaluation of independent risk factors to predict intracranial vascular stenosis in patients with essential hypertension

Abstract:Objectives To assess the accuracy of transcranial Doppler ultrasound (TCD) as a screening modality for asymptomatic intracranial vascular stenosis with the use of CT angiography (CTA), and determine the value of TCD combine with evaluation of independent risk factors in predicting intracranial vascular stenosis in patients with essential hypertension. Design and methods A total of 426 inpatients with essential hypertension were recruited. Patients with history of stroke, transient ischemic attack and other intracranial diseases were excluded. Intracranial vascular stenosis was detected by TCD and CTA simultaneously, and total independent risk factors were evaluated according to the following criteria. The score was defined as 1 if patients whose age were ≥60 year-old , fasting plasma glucose ≥6.05 mmol/L, fibrinogen ≥3.25 g/L or who had left ventricular hypertrophy, otherwise the score was 0. Results The distribution of intracranial vascular stenosis detected by TCD was almost consistent to CTA. But the sensitivity of TCD was only 35.5% (49/138), its specificity was 89.5% (454/507), positive predictive value was 48.0%, and negative predictive value was 83.6% (Kappa=0.277, P<0.001). Evaluating the total independent risk factors of patients with CTA+ and TCD−, showed 39.3% (35/89) had one independent risk factor, 32.6% (29/89) had two independent risk factors and 12.4% (11/89) had three independent risk factors. If we evaluated the total independent risk factors of patients with TCD– we can greatly improve the sensitivity (89.86%, 124/138) of predicting intracranial vascular stenosis (Kappa=0.720, P<0.001). Conclusion The specificity of TCD in detecting asymptomatic intracranial vascular stenosis is just ok, but its sensitivity is very low. TCD combined with evaluation of independent risk factors can greatly improve the sensitivity, and can be an effective early screening method for asymptomatic intracranial vascular stenosis in patients with hypertension.
