Abstract:Objective To study the cerebral blood flow changes in irreversible comatose patients and investigate the correlation between the results of transcranial Doppler (TCD) and clinical evaluation of brain function. Methods TCD studies were performed in 92 patients who were excluded from the possibility of reversible coma and all of them had Glasgow Coma Scale of 3. Their brain stem reflections all disappeared. Detection of the blood flow in bilateral middle cerebral arteries was carried out through temporal window in all the patients and in 42 of the 92 cases the blood flow of the basilar artery was detected through occipital or peri-occipital window. Bilateral middle cerebral artery were taken as the main arteries for evaluation. The positive standard is the appearance of one of the following characteristic blood flow, i.e. brief systolic forward flow or systolic spikes and diastolic reverse flow, brief systolic forward flow or systolic spikes and no diastolic flow and no detectable flow signals. The examination was repeated after 12 h. Results 68 cases were positive in the initial examination (73.9%) and 74 cases were positive after 12 h(80.4%). Conclusion TCD can be an effective and objective index in the evaluation of irreversible termination of brain function.
Use of Transcranial Doppler Ultrasound to Assess Leptomeningeal Collateral Flow in Patients with Severe Occlusive Internal Carotid and Middle Cerebral Artery Disorders: Correlation with Cerebral Angiography

Abstract:Background: The leptomeningeal collateral (LMC) is dynamic in ischemic stroke especially in internal carotid artery (ICA) and middle cerebral artery (MCA) disorders. Transcranial doppler (TCD) is the only noninvasive and portable technique for the LMC; however, there is no standard for TCD in LMC assessment.Objectives: To establish a reference for TCD to assess the LMC in severe occlusive ICA and MCA disorders.Patients and Methods: Forty-four patients were divided into two groups according to the LMC flow in digital subtraction angiography (DSA). The velocities of the bilateral MCA, anterior cerebral artery (ACA), and posterior cerebral artery (PCA) were analyzed.Results: In the ACA-LMC group, the ratio of the stenosis-side ACA and MCA were significantly increased for the systolic velocity (P = 0.003). The sensitivities and specificities were 68.4% and 80.0%, respectively, when the ratio was 2.27 (P = 0.006). In the PCA-LMC group, the ratio of the stenosal side PCA and the opposite side PCA were significantly increased for the diastolic velocity (P = 0.031). The sensitivities and specificities were 77.8% and 75.0%, respectively, when the ratio was 1.38 (P = 0.026).Conclusion: TCD is a reliable tool for LMC assessment in patients with severe occlusive ICA and MCA disorders.

Abstract:OBJECTIVE:To evaluate the value of combined evoked potential, continuous EEG with transcranial Doppler to monitor the cases of severe cerebral vascular disease. METHODS:32 cases of severe cerebral vascular disease were admitted to the hospital and divided into severe or moderate groups based on the GCS (< or = or > 8) when the patients were admitted. BAEP and SEP were used to for monitoring of brain and brainstem functions. The BAEP and SEP results were divided into grade I, grade II and grade III according to Cant's classification. Continuous EEG monitored all patients by bedside. The EEG findings were divided into 6 grades based on Young's criteria. TCD monitoring by using bilateral 2 MHz Probes with probe holder were lasted at least 30 minutes per day through temporal windows and eye windows to explore the velocity of cerebral artery. The patients were classified into three groups according to the velocities, i.e. (1) the group with elevated velocity, (2) the group with reduced velocity, and (3) the group with normal velocity. The association among the brain and brain stem function, cortex function and cerebral flow velocity was analyzed with patient outcome in the two groups. RESULTS:(1) The mortality of the patients in the severe group was 42.86%, significantly higher than that in the group of moderate symptom (22.22%). The survivors with good outcome accounted for 33.33% in the moderate group, and those with independent disability accounted for 44.44% in the moderate group, much better than those in the severe group. (2) The numbers of the patients with III grade SEP and BAEP results in the severe group were higher than those in the moderate group. The mortality of the patients with III grade SEP and BAEP results was high and the most of survivors were found with obvious disability in outcome. The patients with grade I SEP and BAEP results showed a better outcome. (3) 32 patients of cerebral vascular disease were monitored with continuous EEG monitoring by bedside. The results revealed that the mortality of the patients with grade II EEG results was much higher than that of the patients with grade I EEG results. (4) The association of the brain blood flow velocity, either increased or decreased or within normal range was uncertain (all P > 0.05). CONCLUSION:The cerebral function of patients of severe cerebral vascular disease may be estimated precisely and objectively by using continuous monitoring of SEP, BAEP and EEG at bedside, which will provide valuable information about patient's prognosis and treatment.
Study on the effect of transcranial Doppler sonography on hemodynamics in patients with hypertension

Abstract:Objective To investigate the change of the intracalvarium artery hemodynamics in patients with hypertension so as to provide objective evidence for the clinical treatment.Methods The 120 patients with hypertension were enrolled in the study as investigartion group,and the 100 healthy subjects who had no hypertension,hyperglycosemia,hyperlipemia,cardiovascular and cerebrovascular diseases history were enrolled in the study as control group.The transcranial Doppler was used to detect the parameters of intracalvarium artery hemodynamics including contract peak value blood flow speed,vascular pulsatility index(PI),vascular resistance index(RI) in bilateral middle cerebral artery(MCA),anterior cerebral artery(ACA),posterior cerebral artery(PCA),bilateral vertebral artery(VA) and basal artery(BA).Results 109 cases(90.8%) were abnormal for blood flow in investigation group,which were significant higher than those in control group(P<0.05).The blood flow speed in every cerebral artery in investigation group was higher than that in control group,and there were significant difference in MCAL,MCAR,PCAL between the two groups(P<0.05).The vascular resistance index in every cerebral artery in investigation group was higher than that in control group,and there were significant differences in MCAL,MCAR,ACAL,ACAR and BA between two groups(P<0.05).The vascular pulsatility index in investigation group was significantly higher than that in control group(P<0.05).Conclusion TCD is an objective and effective examination way to detect the abnormal changes of the haemodynamics in patients with hypertension,which can supply objective and scientific basis for clinical treatment,as a result,which can lower the incidence of the cerebral vascular accidents in hypertension patients.

Abstract:IsTnACRANIAL TUMORS have been the subject of intensive and varied investigative efforts, being studied by progressive refinements of both biochemical and morphologic methodology. Experimentally induced intracranial tumors in the laboratory animal are a convenient and well-controlled prototype for study, affording the entirety of both normal and abnormal tissue in a relatively compact section. Though a welldocumented experimental method for the production of intracranial neoplasms in the rodent has been recognized since 1939, there have been relatively few physiologic and biochemical studies of these tumors.1-6 Experimentally induced tumors have been extensively studied from morphologic and histogenetic standpoints and have yielded valuable data relative to tumor classification.”7-10 As experimentally induced brain tumors are both morphologic and lethal facsimiles of spontaneous human brain tumors, it is noteworthy that no data correlating their histochemical properties with that of human tumors are available. In this investigation, an enzyme normally present in the central nervous system, alkaline phosphatase, was examined during various phases of intracranial tumorogenesis in the C,H mouse. A correlation was established between the enzymatic activity of alkaline phosphatase in induced tumors with that of similar human tumors.

Abstract:Background: To evaluate the diagnostic value of transcranial Doppler (TCD) and electroencephalography (EEG) in patients with vertebrobasilar insufficiency (VBI) during clinical diagnosis and treatment METHODS: Eighty patients diagnosed with VBI in our hospital from June 2018 to December 2019 were randomly selected as the observation group, and 80 healthy people who received physical examination in the same period were selected as the control group. The abnormal rate, main performance and results, and the peak velocity of blood flow and vertebrobasilar artery blood flow of the two groups were compared. Results: The abnormal rate of EEG and TCD in VBI patients was 38.75% (31/80) and the 93.75% (75/80), respectively. In TCD examination, ACA, PCA, MCA, and VA of both sides of the observation group were higher than those of the control group, while BA was lower than that of the control group (P < 0.05). The Vs, Vd, and Vm on both sides of BA and VA in the observation group were lower than those in the control group, while PI and RI were higher than those in the control group (P < 0.05). Conclusions: TCD examination is highly sensitive to the degree and pattern of cerebral ischemia in VBI patients. EEG examination will define the changes of brain cell function after cerebral ischemia. Therefore, EEG and TCD have their own advantages. The application of TCD and EEG can be considered in the early diagnosis, curative effect, and prognosis evaluation of VBI patients, so as to improve the accuracy of diagnosis and prognosis.

Abstract:Transcranial Doppler ultrasonography (TCD) is a noninvasive, bedside, portable tool for assessment of cerebral hemodynamics. Modern TCD head frames allow continuous hands-free emboli detection for risk stratification and assessment of treatment efficacy in several cardiovascular diseases. Identifying a focal stenosis, arterial occlusion, and monitoring the treatment effect of intravenous tissue plasminogen activator can easily be accomplished by assessing TCD waveforms and determining prestenotic and poststenotic mean flow velocities. TCD is an excellent screening tool for vasospasm in aneurysmal subarachnoid hemorrhage. The use of intraoperative TCD during carotid endarterectomy and stenting allows optimal intraoperative hemodynamic management. Other applications are also discussed.

Abstract:Objective To establish the clinical significance of cerebral blood flow dynamics for Transcrani- al Doppler(TCD) serial monitor after severe head injuries.Methods The study group was consisted of 60 patients of se- vere head injuries and the control group was consisted of 25 healthy persons for the general health examination.TCD serial monitor changes of MCA-Vs,MCA-Vm,MCA-PI values corresponding in 3,7, 10, and 14 days after severe head inju- ries were monitored.Results FOr the study group,the level of MCA-Vm values descended 3 day after severe head injuries compared with the control one, showing statistic significant diffence(P 0.05). The level Of MCA-Vm MCA-Vs values in the study group was the lowest 10 days after injuries compared with that of 3 days after injuries,showing statistic significant difference(P 0.05 - 0.01),and then returned normal gradually .At the same time,the level Of MCA-PI values had raised in 3 days(P 0.01 ),was the highest in 7 days(P 0.05) ,and then lowered. gradually. It showed that abnormal changes. of cerebral blood flow dynamics would occurr in 3 days after severe head injuries;which reached its peak in 7 - 10 days. Conclusion Not only can we know the changes of cerbral blood flow dynamic,but also we can evaluate intracranial pres- sures(ZCP)and cerebral perfusion pressures(CPP)by TCD serial monitor after severe head injuries.

Abstract:To clarify the significance of transcranial Doppler (TCD) monitoring for detection of raised intracranial pressure (ICP), cerebral ischemia, and neuronal dysfunction in comatose patients with severe brain damage, vasoreactive correlations during hyperventilation (HV) and changes in cerebral perfusion pressure (CPP) were evaluated by continuous monitoring of TCD in the middle cerebral artery, jugular bulb oxygen saturation (SjO2), end-tidal CO2partial pressure, ICP and CPP, and quantitative EEG (qEEG) . TCD and gEEG (total and spectral power bands) were monitored in a total of 32 cases of severe head injury, CPP in 26, and SjO2in 16 for 7 days (mean) . Peak systolic, mean, and end-diastolic velocity (Vs, Vm, Vd), pulsatility index (PI), resistance index, and cerebrovascular resistance (CVR=CPP/Vm) were evaluated. a) CO2reactivity (ΔVm/ΔCO2, ΔSjO2/ΔCO2) and correlations with ICP and SjO2were compared within ischemic, normal, and hyperemic groups, based on minimal SjO2during HV. b) Effects of CPP changes were correlated with gEEG. a) HV: 1) No significant differences in TCD parameters and ΔVm/ΔCO2were observed among the three groups. However, ΔSjO2/ΔCO2was significantly high in the order of ischemic>normal> hyperemic groups. 2) Close correlations of ICP with Vm and CVR were identified. 3) Significant correlations of SjO2with CVR, Vm, Vs, and PI were observed frequently in the ischemic group. b) CPP: 1) In cases of close correlation between CPP (<60 mmHg) and Vm, there were significant correlations between all gEEG power bands and Vm. 2) Loss of gEEG was observed in four cases of Vm<36 cm/s, Vd=0, and PI>2.74. Continuous TCD monitoring is useful for the early detection of decreased cerebral circulation and increased CVR associated with intracranial hypertension, cerebral ischemia, and neuronal dysfunction, but not always for easy interpretation of the underlying pathophysiology in patients with severe brain damage.

Abstract:Objective To evaluate the correlations between the cerebral hemodynamic changes of severe acute encephalopathy with the clinical features from emergency periods to stable periods and the value of transcranial Doppler ultrasonography( TCD)in cerebral function assess. Methods Thirty patients with acute brain diseases by assisted mechanical ventilator from Jun 2014 to May 2015 in PICU were included and followed up to Nov 2015,then grouped by Glasgow Coma Scale( GCS),MRI,prognosis( mental sequelae after half a year). Each subject was examined through the temporal bone window by TCD at emergency peri-ods and stable periods. The systolic cerebral blood flow velocity( sCBFV),and pulsatility index( PI)of bilat-eral middle cerebral artery( MCA),anterior cerebral artery,posterior cerebral artery were analyzed. Multivari-ant analysis of variance,repeated measures analysis of variance was conducted to compare sCBFV and PI of MCA among groups in different clinical periods. The variants included sex,disease diagnosis,prognosis, MRI,GCS,and the first abnormal TCD. The variation within groups was tested via a Hotelling T2 test. All sCBFV and PI of each artery and the D-value of sCBFV between the different periods were compared accord-ing to the prognosis groups. Results (1)Five patients could be lateralized,and their lateralization of MRI focus was consistent with the abnormal sides of sCBFV in the stable periods.(2)The D-value of sCBFV in left MCA between the different periods in the poor prognosis group was significantly higher than that in the good prognosis group[(71. 93 ± 58. 21)cm/s vs.(33. 20 ± 30. 23)cm/s,t = -2. 287,P =0. 033].(3) Multivariant analysis of variance showed that GCS classification and disease diagnosis were significantly cor-related with the cerebral hemodynamic changes respectively(P =0. 042,0. 005,respectively).(4)sCBFV and PI of left MCA reduced significantly in the stable periods than those in the emergency periods( P =0. 002,0. 003,respectivly). Conclusion The cerebral hemodynamic changes by TCD from emergency peri-ods to stable periods are consistent with the clinical status,dynamic evaluation by TCD may facilitate the evaluation of brain dysfunction in the severe acute encephalopathy.

Abstract:Transcranial Doppler (TCD) ultrasonography is a noninvasive ultrasound study, which has been extensively applied on both outpatient and inpatient settings. It involves the use of a low-frequency (≤2 MHz) transducer, placed on the scalp, to insonate the basal cerebral arteries through relatively thin bone windows and to measure the cerebral blood flow velocity and its alteration in many different conditions. In neurointensive care setting, TCD is useful for both adults and children for day-to-day bedside assessment of critical conditions including vasospasm in subarachnoid hemorrhage, traumatic brain injury, acute ischemic stroke, and brain stem death. It also allows to investigate the cerebrovascular autoregulation in setting of carotid disease and syncope. In this review, we will describe physical principles underlying TCD, flow indices most frequently used in clinical practice and critical care applications in Neurocritical Unit care.

Abstract:Stroke, primarily resulting from ischemic conditions, is the foremost cause of mortality and long-term impairment and is frequently associated with narrowing of the carotid arteries. Although carotid endarterectomy (CEA) is the treatment of choice, it carries the risk of cerebral ischemia and reduced blood flow. Transcranial Doppler (TCD) ultrasound offers a nonintrusive method to assess cerebral blood circulation during CEA, potentially enhancing surgical outcomes. The objective of this study was to assess the clinical utility and safety of TCD monitoring during CEA and to identify factors influencing postoperative complications.

The Role of Transcranial Doppler Ultrasound in Patients with Acute Ischemic Stroke Treated with Intravenous Thrombolysis

Abstract:Transcranial Doppler ultrasonography(TCD) yields satisfactory agreement with CT angiography(CTA) in the evaluation of patients with acute cerebral ischemia. TCD can provide real-time flow findings that are complentary to information provided by CTA. A Thrombolysis in Brain Ischemia(TIBI) classification is developed to noninvasively monitor intracranial vessel residual flow signals to assess recanalization. In patients with acute middle cerebral artery occlusion, flow diversion(FD), as a marker of leptomeningeal collateralization on TCD, predicts 90-day clinical outcome. End-diastolic velocity increase predicts recanalization.

Abstract:Background and purpose: Reversible cerebral vasoconstriction syndrome (RCVS) is a vascular headache disorder characterized by severe headaches with vasospasm of cerebral arteries. Transcranial Doppler ultrasonography (TCD) has been widely applied and validated in studying vasospasm of intracranial vessels, but the role of TCD in the diagnosis and monitoring of RCVS is less well established. We sought to determine the reliability of TCD for diagnosis and monitoring of RCVS. Methods: Patients admitted to an inpatient neurology service between 2011 and 2014 with a discharge diagnosis of RCVS were retrospectively analyzed for demographics, neuroimaging, and functional outcomes. Baseline and follow-up TCD flow velocities in the middle cerebral artery (Vmca) were compared relative to the final diagnosis. Results: The cohort consisted of fifteen patients (93% females; mean age 46.7 +/- 12.4 years); initial TCD evaluation was performed 10.9 +/- 6.6 (range 1-24) days after headache onset. Fourteen patients (93.3%) had increased flow velocities by initial TCD in at least one major cerebral blood vessel (MCA, ACA, PCA, vertebral, basilar). TCD flow velocities in the middle cerebral artery (Vmca) reached a mean peak of 163 cm/s three to four weeks after the onset of thunderclap headache. Conclusion: TCD is a non-invasive neuroimaging modality that may have potential for the initial diagnosis and subsequent monitoring of patients with suspected RCVS. Use of Transcranial Doppler Ultrasound for Diagnosis of Brain Death in Patients with Severe Cerebral Injury

Abstract:BACKGROUND The aim of this study was to investigate the use of transcranial Doppler (TCD) for diagnosis of brain death in patients with severe cerebral injury. MATERIAL AND METHODS This retrospective study enrolled 42 patients based on inclusion and exclusion criteria. All patients were divided into either the brain death group or the survival group according to prognosis. Blood flow of the brain was examined by TCD and analyzed for spectrum changes. The average blood flow velocity (Vm), pulse index (PI), and diastolic blood flow in reverse (RDF) were recorded and compared. RESULTS The data demonstrated that the average speed of bilateral middle cerebral artery blood flow in the brain death group was significantly reduced (P<0.05). However, the PI of the brain death group increased significantly. Moreover, RDF spectrum and nail-like sharp peak spectrum of the brain death group was higher than in the survival group. CONCLUSIONS Due to its simplicity, high repeatability, and specificity, TCD combined with other methods is highly valuable for diagnosis of brain death in patients with severe brain injury.
A Comparative Study of Transcranial Color-Coded Doppler (TCCD) and Transcranial Doppler (TCD) Ultrasonography Techniques in Assessing the Intracranial Cerebral Arteries Haemodynamics

Abstract:Cerebrovascular disease (CVD) poses a major public health and socio-economic burden worldwide due to its high morbidity and mortality rates. Accurate assessment of cerebral arteries' haemodynamic plays a crucial role in the diagnosis and treatment management of CVD. The study compared a non-imaging transcranial Doppler ultrasound (TCD) and transcranial color-coded Doppler ultrasound (with (cTCCD) and without (ncTCCD)) angle correction in quantifying middle cerebral arteries (MCAs) haemodynamic parameters. A cross-sectional study involving 50 healthy adults aged ≥ 18 years was conducted. The bilateral MCAs were insonated via three trans-temporal windows (TTWs—anterior, middle, and posterior) using TCD, cTCCD, and ncTCCD techniques. The MCA peak systolic velocity (PSV) and mean flow velocity (MFV) were recorded at proximal and distal imaging depths that could be visualised on TCCD with a detectable spectral waveform. A total of 152 measurements were recorded in 41 (82%) subjects with at least one-sided open TTW across the three techniques. The mean PSVs measured using TCD, ncTCCD, and cTCCD were 83 ± 18 cm/s, 81 ± 19 cm/s, and 93 ± 21 cm/s, respectively. There was no significant difference in PSV between TCD and ncTCCD (bias = 2 cm/s, p = 1.000), whereas cTCCD yielded a significantly higher PSV than TCD and ncTCCD (bias = −10 cm/s, p < 0.001; bias = −12 cm/s, p ≤ 0.001, respectively). The bias in MFV between TCD and ncTCCD techniques was (bias = −0.5 cm/s; p = 1.000), whereas cTCCD demonstrated a higher MFV compared to TCD and ncTCCD (bias = −8 cm/s, p < 0.001; bias = −8 cm/s, p ≤ 0.001, respectively). TCCD is a practically applicable imaging technique in assessing MCA blood flow velocities. cTCCD is more accurate and tends to give higher MCA blood flow velocities than non-imaging TCD and ncTCCD techniques. ncTCCD is comparable to non-imaging TCD and should be considered in clinical cases where using both TCD and TCCD measurements is needed.
