Abstract:Background About 25% of pregnant women experience bleeding in the early stage, and half of them eventually progress to pregnancy loss. Progesterone serves as a useful biomarker to predict miscarriage in threatened miscarriage, yet its performance is still debated. Aim To evaluate the performance of single serum progesterone predicting miscarriage in early pregnant patients with threatened miscarriage. Method The online database was searched to yield the literature using the terms of ‘Abortion’, ‘Miscarriage’, and ‘serum Progesterone’, including PubMed, Scopus, Embase, Cochrane library, and China national knowledge infrastructure. Receiver operating characteristic (ROC) curve, likelihood ratio (LLR) and diagnostic odds ratio (DOR) and 95% confidence interval (CI) were computed. Publication bias was assessed by the deeks funnel plot asymmetry test. Subgroup analyses were conducted according to the progesterone level (< 12 ng/mL), recruited location and region, progesterone measurement method, exogenous progesterone supplement and follow up. Results In total, 12 studies were eligible to be included in this study, with sample sizes ranging from 76 to 1087. The included patients’ gestational age was between 4 and 12 weeks. No significant publication bias was detected from all included studies. The threshold of progesterone reported ranged from 8 to 30 ng/ml. The synthesized area under the ROC curve (0.85, 95% CI 0.81 to 0.88), positive LLR (6.2, 4.0 to 9.7) and DOR (18, 12 to 27) of single progesterone measurement distinguishing miscarriage were relatively good in early pregnant patients with threatened miscarriage. When the threshold of < 12 ng/mL was adapted, the progesterone provided a higher area under the ROC curve (0.90 vs. 0.78), positive LLR (8.3 vs. 3.8) and DOR (22 vs.12) than its counterpart (12 to 30 ng/mL). Conclusion Single progesterone measurement can act as a biomarker of miscarriage in early pregnant patients with threatened miscarriage, and it has a better performance when the concentration is <12 ng/mL. The Clinical Value of 3D Ultrasonic Measurement of the Ratio of Gestational Sac Volume to Embryo Volume in IoT-Based Prediction of Pregnancy Outcome

Abstract:The objective of the research study is to investigate the use of three-dimensional ultrasonic measurement technology, to determine the size of gestational sac and embryo volume, and to use the ratio of gestational sac volume to embryo volume in IoT-based prediction of pregnancy outcome. The abnormal and normal pregnancy identifiers are there, which assists in prediction of pregnancy outcomes: whether the pregnancy is normal or may suffer pregnancy loss during first trimester. For the observational study, 500 singleton pregnant women who made an appointment for delivery in Qiqihar Hospital from January 2015 to June 2019 were considered. The 500 pregnant women received transvaginal ultrasound at 6+0 ∼ 8+0 weeks of gestational age to measure gestational sac volume (GSV), yolk sac volume (YSV), and germ volume (GV). According to pregnancy outcome, they were divided into fine group (n = 435) and abortion group (n = 65). Among the 500 cases, 435 had normal delivery and 65 had abortions. According to the results of gestational age (GA) analysis, the pregnancy success rates at 6 (n = 268), 7 (n = 184), and 8 weeks (n = 48) were 85.8%, 87.5%, and 91.7%, respectively. Comparison of pregnancy failure rate among the three groups shows statistically significant difference. The morphology of germ, yolk sac, and gestational sac cannot be used as a predictor of pregnancy outcome in various degrees. The results of multivariate Cox proportional regression analysis show the following: the ratio of germ volume (GV) to gestational sac volume (GSV) (P=0.008) has an impact on the prediction of spontaneous abortion prognosis, showing statistically significant difference; yolk sac volume (YSV), germ volume (GV), and gestational sac volume (GSV) have no effect on the prediction of spontaneous abortion prognosis (P > 0.05). The ratio of GSV to germ volume has a strong prognostic value for pregnancy results. To a certain extent, the ratio of gestational sac volume to germ volume can predict spontaneous pregnancy abortion at 6th week of gestation, providing a theoretical basis for clinical ultrasound pregnancy examination indicators.
Comparison of high-frequency and transvaginal ultrasound in the diagnosis of early pregnancy

Abstract:Objective To investigate the diagnostic value of high-frequency ultrasound in the first trimester pregnancy.Methods Pregnant women with gestational sac within 70 mm from the skin were selected by abdominal ultrasonography and further examined with high-frequency ultrasound and transvaginal ultrasound respectively.The images of gestational sac,yolk sac and embryo were compared between two examinations.Results Of 108 pregnant women,high-frequency ultrasound and transvaginal ultrasound showed gestational sac,yolk sac and embryo in 94(87.04%) vs.97(89.81%),82(75.93%) vs.85(78.70%),and 58(53.70%) vs.58(53.70%),respectively.There was no significant difference in the detection rates of these indicators between the two methods(P>0.1).Conclusion Both high-frequency ultrasound and transvaginal ultrasound examinations may effectively show gestational sac,yolk sac and embryo in early pregnant women with thin belly.

Comparison on high-frequency and routine ultrasound in diagnosis of early pregnancy

Abstract:Objective: To observe the diagnostic value of high-frequency ultrasound in the early pregnancy. Methods: Six hundred women of early pregnancy were assigned into three groups according to pregnancy weeks: The first group: <4 weeks, gestational sac (GS) <10 mm; the second group: ≥4 weeks and <7 weeks, GS 10-26 mm; the third group: ≥7 weeks, GS ≥27 mm. Followed by high-frequency ultrasound, routine ultrasound was performed firstly. The ultrasound images of GS, yolk sac (YS) and embryo were compared between high-frequency and routine ultrasound in the three groups. Results: In the first group, GS, YS and embryo were displayed in 178, 137 and 28 women with high-frequency ultrasound, respectively, while GS and YS were displayed in 105 and 12 women, but no embryo was detected with routine ultrasound. The differences of displaying rate between high-frequency and routine ultrasound were statistical (all P<0.005). In the second group, GS, YS and embryo were displayed in 200, 193 and 178 women with high-frequency ultrasound, while in 200, 105 and 92 women with routine ultrasound. The displaying rates of yolk sac and embryo between high-frequency and routine ultrasound were statistically different (both P<0.005). In the third group, GS, YS and embryo were displayed in 200, 197 cases and 193 women with high-frequency ultrasound, and in 200, 189 and 182 women with routine ultrasound, respectively. No statistical difference was found between high-frequency and routine ultrasound (all P>0.05).Conclusion: When gestational age is less than 7 weeks, high-frequency ultrasound can improve the displaying rate of GS, YS and embryo. But for those which gestational age ≥7 weeks, the three images mentioned above present no difference of both high-frequency and routine ultrasound, but the images of high-frequency ultrasound are clearer.

Abstract:BACKGROUND Subchorionic hematoma (SCH) is a common complication in early pregnancy characterized by the accumulation of blood between the uterine wall and the chorionic membrane. SCH can lead to adverse pregnancy outcomes such as miscarriage, preterm birth, and other complications. Early detection and accurate assessment of SCH are crucial for appropriate management and improved pregnancy outcomes. AIM To evaluate the diagnostic efficacy of virtual organ computer-assisted analysis (VOCAL) in measuring the volume ratio of SCH to gestational sac (GS) combined with serum progesterone on early pregnancy outcomes in patients with SCH. METHODS A total of 153 patients with SCH in their first-trimester pregnancies between 6 and 11 wk were enrolled. All patients were followed up until a gestational age of 20 wk. The parameters of transvaginal two-dimensional ultrasound, including the circumference of SCH (Cs), surface area of SCH (Ss), circumference of GS (Cg), and surface area of GS (Sg), and the parameters of VOCAL with transvaginal three-dimensional ultrasound, including the three-dimensional volume of SCH (3DVs) and GS (3DVg), were recorded. The size of the SCH and its ratio to the GS size (Cs/Cg, Ss/Sg, 3DVs/3DVg) were recorded and compared. RESULTS Compared with those in the normal pregnancy group, the adverse pregnancy group had higher Cs/Cg, Ss/Sg, and 3DVs/3DVg ratios (P < 0.05). When 3DVs/3DVg was 0.220, the highest predictive performance predicted adverse pregnancy outcomes, resulting in an AUC of 0.767, and the sensitivity, specificity were 70.2%, 75% respectively. VOCAL measuring 3DVs/3DVg combined with serum progesterone gave a diagnostic AUC of 0.824 for early pregnancy outcome in SCH patients, with a high sensitivity of 82.1% and a specificity of 72.1%, which showed a significant difference between AUC. CONCLUSION VOCAL-measured 3DVs/3DVg effectively quantifies the severity of SCH, while combined serum progesterone better predicts adverse pregnancy outcomes.

Abstract:OBJECTIVE:To investigate the value of ultrasound for the assessment of endometrial receptivity during in vitro fertilization (IVF) and embryo transfer (ET).MATERIALS AND METHODS:Only those patients in their first IVF cycle, followed a standard long gonadotropin releasing hormone (GnRH) agonist long ovarian stimulation regimen, and had 2 highest scoring embryos transferred on day 3 were included in the study. A 3D ultrasound examination was performed on the day of human chorionic gonadotropin (hCG) injection to determine endometrial thickness, endometrial pattern, peak systolic velocity (PSV), end diastolic velocity (EDV), pulsatility index (PI) and resistance index (RI) of endometrial spiral arteries, endometrial volume, vascularization index (VI), flow index (FI) and vasculariztion flow index (VFI) of endometrial and subendometrial region. Analyses were carried on between pregnant and non-pregnant groups, and miscarriage and ongoing pregnant groups.RESULTS:435 patients were included in the study, in which 235 (58.8%) got clinical pregnancy, and 49 suffered miscarriage. Patients in both pregnant group and non-pregnant group, and miscarriage and ongoing pregnant group did not show significant difference in endometrial thickness, endometrial volume, endometrial pattern, uterine PI, RI, S/D, endometrial and subendometrial VI, FI and VFI. In relative thin endometrium group, pregnant patients had similar endometrial thickness, endometrial volume, endometrial pattern, uterine PI, RI, S/D, endometrial and subendometrial VI, FI and VFI with non-pregnant patients.CONCLUSION:3D PDUS parameters of endometrium measured on hCG day were not good predictors of pregnancy in IVF treatment, neither good predictors of ongoing pregnancy.

Abstract:Objective:To investigate the predicton value of parameters by three-dimensional ultrasound on pregnancy outcome during in vitro fertilization/intracytoplasmic sperm injection-embryo transfer (IVF/ ICSI-ET).Methods:Ultrasound parameters of 697 patients measured on hCG injection day were retrospectively analyzed.These variables were compared between pregnant and non-pregnant patients.Stratified analysis was then conducted according to the controlledovarianhyperstimulation (COH) protocol.Results:Compared with non-pregnant patients,pregnant women exhibited significantly lower age,infertility duration,IVF cycle and higher endometrial thickness,endometrial volume,endometrial spiral arteries peak systolic velocity (PSV),end diastolic velocity (EDV),subendometrial flow index (FI) and vascular flow index (VFI).There were significant differences in endometrial thickness and EDV between pregnant and non-pregnant women who were treated with short protocol.Logistic analysis showed that IVF cycle and endometrial thickness could predict the pregnancy outcome of patients treated with short protocol.Receiver operating characteristic (ROC) curve analysis was also conducted to evaluate the predictive value of endometrial thickness on pregnancy outcome.Conclusion:Threedimensional ultrasound measurement on hCG injection day does not play an important role in the prediction of pregnancy outcome during IVF/ICSI-ET,while endometrial thickness correlates with pregnancy outcome of patients treated with short protocol.
Value of yolk sac detected by transvaginal sonography in the first trimester as a prognostic factor of pregnancy outcomes

Abstract:【Objective】To investigate the predictive value of yolk sac which was detected using transvaginal sonography(TVS) in the first trimester after in vitro fertilization-embryo transfer(IVF-ET) for pregnancy outcomes. 【Methods】The women who were diagnosed with a single live embryo by TVS in the 6~12 gestational weeks from January 2010 to June 2011 in the reproductive center of our hospital were included in this study. They were divided into two groups according to the pregnancy outcomes: the ongoing pregnancy group(2 679 cases) and the early abor- tion group(504 cases). And then, investigate the yolk sac size and its role as a prognostic factor of pregnancy out- comes.【Results】In the ongoing pregnancy group, 2 436 cases with normal yolk sac size, 159 cases with physiologi- cal vanished yolk sac(in 11~12 gestational weeks), 22 in the small size, and 62 in the large size; While in the early abortion group, 203 yolk sac is in normal size, 121 in the small size, 41 in the large size; and 139 without visible yolk sac. The abnormal rate of yolk sac differed significantly between these two groups(P <0.05). The value of yolk sac in predicting an early miscarriage was with a sensitivity of 59.72%, specificity of 96.86%, positive predictive value of 78.18%, negative predictive value of 92.74%, and the diagnostic accuracy of 90.98%. 【Conclusions】Too large, too small or absent of yolk sac related to adverse pregnancy outcomes. Detection of yolk sac size by TVS had important value in predicting pregnancy outcomes.

Study on endometrial receptivity assessment with transvaginal ultrasonorgraphy

Abstract:Objective:To investigate the relationship between the sonographic appearance of the endometrium,the endometrial thickness and pregnancy outcome.Methods:A total of 520 women who underwent artificial insemination of donor for assisted reproductive treatment cycles in reproductiven center of National Research Institute for Family Planning were recruited.And the endometrial thickness and sonographic appearance of the endometrium were detected on the day of hCG administration and the window of implantation (7 days after ovulation) by transvaginal ultrasound.Women were divided into seven groups and six groups according to the thickness of endometria on the day of hCG administration and the window of implantation,respectively.The outcome of pregnancy was evaluated.Results:The clinical pregnancy rate in group A (28.95%) was significantly higher than that of group C (9.09%,P<0.05).The endometrial thickness on the day of hCG administration and the window of implantation in group B and group C were significantly greater than that of group A (P all <0.05).There was no significant difference in the endometrial thickness between pregnant women and non-pregnant ones on the day of hCG administration (P>0.05).The endometrial thickness on the window of implantation in pregnant women was significantly less than that of non-pregnant ones (P<0.05).The highest pregnancy rate was found in those with 10mm to less than 12mm of the endometrial thickness on the day of hCG administration,and no pregnancy occurred when the endometrial thickness was less than 7mm or not less than 15mm.In addition,the highest pregnancy rate was found in those with 9mm to less than 13mm of the endometrial thickness on the window of implantation,and no pregnancy occurred when the endometrial thickness was less than 8mm or not less than 17mm.Conclusion:It is important of the endometrial appearance and the endometrial thickness evaluated by transvaginal sonographic in the assessment of endometrial receptivity,which could predict the clinical outcome of pregnancy and provide a reliable basis for adjustment of endometrial receptivity in assisted reproductive treatment.
The Role of Blood Flow in Corpus Luteum Measured by Transvaginal Two-Dimensional and Three-Dimensional Ultrasound in the Prediction of Early Intrauterine Pregnancy Outcomes.

Abstract:Objective: The purpose of this study was to explore the application of transvaginal two-dimensional and three-dimensional power Doppler ultrasound in pregnancy corpus luteum to predict the final outcome of early intrauterine pregnancy. Methods: This is a prospective observational cohort study. Six hundred early intrauterine pregnant women in Shanghai Changning Maternity and Infant Health Hospital were selected as the research objects from January 2015 to December 2015. According to the follow-up of 12 weeks, these pregnant women were divided into the normal pregnancy group (group A, n = 512) and the terminational pregnancy group (group B, n = 88). They all underwent both transvaginal two-dimensional ultrasound and three-dimensional power Doppler ultrasound to obtain relevant parameters of corpus luteum, namely, the average diameter of the corpus luteum (D), resistance index (RI), pulsatility index (PI), corpus luteum volume (V), vascularization index (VI), blood flow index (FI), and vascularized blood flow index (VFI). Among them, V, VI, FI, and VFI were calculated with the virtual organ computer-aided analysis method. Receiver operator characteristic (ROC) curves were drawn. The corresponding diagnostic cut-off, sensitivity, and specificity were calculated and compared. Results: Compared with group A, the D, V, VI, FI, and VFI of corpus luteum in group B were statistically significant lower while RI and PI were statistically significant higher (P < 0.05). The diagnostic cut-off values in the prediction of early intrauterine pregnancy outcomes were D: 14.48, RI: 0.56, PI: 0.81, V: 3.89, VI: 21.48, FI: 38.99, and VFI: 10.21 respectively, and the sensitivity and specificity were D (99.2%, 67.0%), RI (98.9%, 65.0%), PI (78.4%, 89.1%), V (95.1%, 78.4%), VI (74.%, 90.9%), FI (91.8%, 90.9%), and VFI (93.9%, 87.5%) respectively. The area under the ROC curve of the combined index (RI + FI) was 0.963, which was not significantly higher compared with any single index, and both the sensitivity and specificity were 94.3%. Conclusion: Both transvaginal two-dimensional and three-dimensional ultrasonography are of high diagnostic value in predicting the early intrauterine pregnancy outcomes.

Abstract:Establishing prediction models of pregnancy outcomes for recurrent pregnancy loss women at specific gestational weeks will provide patients and physicians with more precise information, ultimately leading to time and cost savings associated with unnecessary revisits. Therefore, our aim was to develop a prediction model for first trimester pregnancy loss in RPL patients. We used ultrasound indices during the first trimester of pregnancy in combination with demographic characteristics and commonly used serum markers. The independent risk factors for each week were as follows: age and P in the fifth week; age, mGSD and CRL in the sixth week; age, hCG and CRL in the seventh week; CRL in the eighth week; mGSD and CRL in ninth week. The corresponding AUC was 0.671, 0.796, 0.872, 0.871, 0.813, respectively. There is a linear relationship between age and first trimester pregnancy loss. hCG < 69,636.6 mIU/ml was associated with a higher risk of pregnancy loss in the seventh gestation week. An mGSD < 18.3 mm, adjusted for age, BMI, and previous pregnancy loss in the sixth week, was linked to an increased risk of first trimester pregnancy loss. A small CRL measurement (less than 2.4 mm, 9.9 mm, 16.9 mm, and 18.6 mm) in the sixth, seventh, eighth and ninth week was closely correlated with higher risk of first trimester pregnancy loss. Furthermore, an mGSD < 33.3 mm and > 48.3 mm in ninth gestational week was associated with a higher risk of pregnancy loss. These models and thresholds may help physicians and patients make more informed decisions together. Further studies are needed to confirm the results.

A total of 139 first-trimester pregnancies from 6 to 12 weeks of gestational age with SCH were included. The circumference, area, and volume of the SCH (Cs, As, and 3DVs) and gestational sac (Cg, Ag, and 3DVg) were automatically calculated by VOCAL. Interobserver agreement was assessed with the intraclass correlation coefficient and Bland-Altman plot. Odds ratios for the clinical and ultrasound characteristics of adverse pregnancy outcomes were estimated by univariate or multivariable logistic regression. There was high interobserver agreement in VOCAL measurements of the SCH and gestational sac (intraclass correlation coefficients, >0.900). By univariate regression analysis, women with adverse pregnancy outcomes were more likely to have vaginal bleeding, a larger As and 3DVs, and higher Cs/Cg, As/Ag, and 3DVs/3DVg ratios (all P < 0.05). In the multivariable regression analysis, a higher 3DVs/3DVg ratio of SCH (odds ratio, 1.034; 95% confidence interval [CI], 1.015-1.053; P = 0.000) and vaginal heavy bleeding had a significant positive relationship with the risk of adverse pregnancy outcomes (odds ratio, 7.846; 95% CI, 1.104-55.737; P = 0.039) remained associated with an increased risk of adverse pregnancy outcomes. With 11.87% as the cutoff, the receiver operating characteristic curve yielded a good predictive performance (area under the curve, 0.713) and high sensitivity (91.18%). The measurements of SCH by VOCAL provide high reliability for estimating the total hematoma burden in first-trimester pregnant patients. A greater volume proportion (3DVs/3DVg) of SCH worsens the pregnancy outcome, and a greater risk of adverse pregnancy outcome also associated with the severity of vaginal bleeding.

Abstract:Establishing prediction models of pregnancy outcomes for recurrent pregnancy loss women at specific gestational weeks will provide patients and physicians with more precise information, ultimately leading to time and cost savings associated with unnecessary revisits. Therefore, our aim was to develop a prediction model for first trimester pregnancy loss in RPL patients. We used ultrasound indices during the first trimester of pregnancy in combination with demographic characteristics and commonly used serum markers. The independent risk factors for each week were as follows: age and P in the fifth week; age, mGSD and CRL in the sixth week; age, hCG and CRL in the seventh week; CRL in the eighth week; mGSD and CRL in ninth week. The corresponding AUC was 0.671, 0.796, 0.872, 0.871, 0.813, respectively. There is a linear relationship between age and first trimester pregnancy loss. hCG < 69,636.6 mIU/ml was associated with a higher risk of pregnancy loss in the seventh gestation week. An mGSD < 18.3 mm, adjusted for age, BMI, and previous pregnancy loss in the sixth week, was linked to an increased risk of first trimester pregnancy loss. A small CRL measurement (less than 2.4 mm, 9.9 mm, 16.9 mm, and 18.6 mm) in the sixth, seventh, eighth and ninth week was closely correlated with higher risk of first trimester pregnancy loss. Furthermore, an mGSD 48.3 mm in ninth gestational week was associated with a higher risk of pregnancy loss. These models and thresholds may help physicians and patients make more informed decisions together. Further studies are needed to confirm the results.

Abstract:Background: Endometrial thickness (EMT) and blood flow are commonly used indicators of endometrial receptivity. However, the results of single ultrasound examination studies differ. Therefore, we used 3-dimensional (3D) ultrasound to study the influence of changes in EMT, endometrial volume, and endometrial blood flow on frozen embryo transfer cycles.Methods: This was a prospective cross-sectional study. Women who underwent in vitro fertilization (IVF) at Dalian Women and Children's Medical Group and met the inclusion criteria were enrolled between September 2020 and July 2021. Ultrasound examinations were performed in patients who underwent frozen embryo transfer cycles on the day of progesterone administration, the third day after progesterone administration, and the day of embryo transplantation. Two-dimensional ultrasound was used to record EMT, 3D ultrasound was used to record endometrial volume, and 3D power Doppler ultrasound imaging was used to record the following endometrial blood flow parameters: vascular index, flow index, and vascular flow index. Changes in the 3 inspections of the EMT, volume, vascular index, flow index, and vascular flow index and 2 inspections of estrogen levels were categorized as "declining" or "nondeclining". The relationship between changes in a certain indicator and the IVF outcome was analyzed by univariate analysis and multifactorial stepwise logistic regression.Results: In total, 133 patients were enrolled in this study, 48 were excluded, and 85 were included in the statistical analysis. Among these 85 patients, 61 were pregnant (71%), 47 had a clinical pregnancy (55%), and 39 had an ongoing pregnancy (45%). The results showed that if the first change in endometrial volume was nondeclining, the outcomes of clinical and ongoing pregnancies were more likely to be unfavorable (P=0.03, P=0.01). Additionally, if the second change in endometrial volume on the day of embryo transplantation was nondeclining, the outcome of an ongoing pregnancy was more likely to be favorable (P=0.03).Conclusions: The changes in endometrial volume was a factor helpful in predicting the IVF outcome, whereas the changes in EMT and endometrial blood flow analyses were not helpful in predicting IVF outcome.
