Abstract:Objective To evaluate the application value of fetal heart echocardiography intelligent navigation (5D Heart) in the display of key diagnostic elements in five rapid screening fetal echocardiographic views.Methods Three hundred and eighty-four 3D volume datasets of 220 normal singleton fetuses in the second and third trimesters were acquired.After processing the datasets using 5D Heart,five rapid screening views were acquired,and 21 key diagnostic elements of the five cardiac diagnostic sections were graded.The 2 test was used to compare the difference in the key diagnostics elements of the five rapid screening views between 5D heart and the traditional technique.Pearson correlation was used to compare the detection rate of key diagnostic elements between 5D Heart technique and traditional two-dimensional echocardiography in normal fetuses and fetuses with congenital heart disease.The rank sum test was used to detect the consistency and reliability of the results.Results A total of 384 fetal volume datasets were obtained from 220 fetuses,of whom 317 heart volume datasets were available for analysis,with a success rate of 82.6％.There was no difference in the key diagnostic elements of the five rapid screening views between 5D heart and the traditional technique (x2=5.17,P=0.27).Compared with the abnormal fetal heart group (89.6％),the detection rate of diagnostic elements in the normal fetal heart group was 85.2％ (r=0.94,P=0.02).No significant differences were found when comparing the grading results of one observer in different periods of time,the grading results between two experienced doctors,or the grading results among the experienced doctor and one relatively inexperienced doctor (P ＞ 0.05).Conclusions Fetal cardiac ultrasound intelligent navigation technique has high reliability and repeatability in obtaining fetal rapid screening views and displaying fetal elemental diagnostic information.It has tremendous potential in standardized data collection and analysis of cardiac diagnostic sections in fetuses with congenial heart disease,as well as in training and teaching,remote medical consultation,and screening in primary hospitals.
Diagnostic Value of Ultrasonic Screening for Fetal Heart Malformations

Abstract:Aim: To access the diagnostic value of ultrasonic screening for fetal heart malformations in non-selected pregnant women. Methods: 29430 pregnant women were included in this study, the fetal heart was scanned with four-chamber view, left and right ventricular outflow tract view, if congenital heart pathology was found, we performed detailed echocardiography. The results of autopsy and postpartum echocardiography were traced. Results: Of 29430 cases, 194 cases (6.59‰) were diagnosed with congenital heart disease. 116 cases (6.63‰) of them were detected in the second trimester, and 78 cases (6.54‰) in the third trimester. The earliest diagnostic time was in 13 pregnant week. The heart malformation in 70 cases of them were confirmed by autopsy and postpartum echocardiography. The result of echocardiography in 6 cases were false negatire, and in 1 case was false positive. In non-selected pregnant women, the sensitivity of echocardiography for cardiac anomalies was 92.11%; the specificity was 99.42% res. Conclusion: Four-chamber view, left and right ventricular outflow tract view are a very simple and reliable method in the evaluation of the fetal cardiovascular system, with higher specificity and sensitivity.
Comparison on high-frequency and routine ultrasound in diagnosis of early pregnancy

Abstract:Objective: To observe the diagnostic value of high-frequency ultrasound in the early pregnancy. Methods: Six hundred women of early pregnancy were assigned into three groups according to pregnancy weeks: The first group: <4 weeks, gestational sac (GS) <10 mm; the second group: ≥4 weeks and <7 weeks, GS 10-26 mm; the third group: ≥7 weeks, GS ≥27 mm. Followed by high-frequency ultrasound, routine ultrasound was performed firstly. The ultrasound images of GS, yolk sac (YS) and embryo were compared between high-frequency and routine ultrasound in the three groups. Results: In the first group, GS, YS and embryo were displayed in 178, 137 and 28 women with high-frequency ultrasound, respectively, while GS and YS were displayed in 105 and 12 women, but no embryo was detected with routine ultrasound. The differences of displaying rate between high-frequency and routine ultrasound were statistical (all P<0.005). In the second group, GS, YS and embryo were displayed in 200, 193 and 178 women with high-frequency ultrasound, while in 200, 105 and 92 women with routine ultrasound. The displaying rates of yolk sac and embryo between high-frequency and routine ultrasound were statistically different (both P<0.005). In the third group, GS, YS and embryo were displayed in 200, 197 cases and 193 women with high-frequency ultrasound, and in 200, 189 and 182 women with routine ultrasound, respectively. No statistical difference was found between high-frequency and routine ultrasound (all P>0.05).Conclusion: When gestational age is less than 7 weeks, high-frequency ultrasound can improve the displaying rate of GS, YS and embryo. But for those which gestational age ≥7 weeks, the three images mentioned above present no difference of both high-frequency and routine ultrasound, but the images of high-frequency ultrasound are clearer.
[Evaluation of the Prenatal Screening Mode for Fetal Congenital Heart Diseases by Ultrasound].

Abstract:OBJECTIVE:To assess prenatal heart disease screening program by ultrasound.METHODS:A total of 11,544 second-trimester screening scans were performed before 24 weeks' gestation on 11,410 women between February 2004 and May 2007 in Obstetrics and Gynecology Hospital of Fudan University. Fetal heart screening was based on four-chamber and outflow tract views (left ventricular outflow + three vessel view). The sensitivity and specificity of different views were evaluated. Follow-up data of newborns was obtained.RESULTS:(1) Among 11,544 cases,48 cases of congenital heart disease (CHD) were diagnosed in utero. Six cases were false negative, and 2 cases were false positive. The incidence of CHD was 0.47% (54/11 544). (2) Thirty-three CHDs were detected based on the four-chamber view, including 18 ventricular septal defect (9 with conotruncal anomalies), 6 anomalous atrioventricle valve, 9 disproportion of left/right ventricle. The sensitivity of the four-chamber view alone was 61.11% (33/54), and the specificity was 99.98% (11 488/11 490). Fifteen CHDs were detected based on the left ventricular outflow and three vessel views, including 1 pulmonary atresia, 3 pulmonary valve stenosis, 2 transposition of the great arteries (TGA), 1 pulmonary stenosis with TGA, 6 tetralogy of Fallot, and 2 pulmonary stenosis. The sensitivity of the combination of the four-chamber and outflow tract views was 88.89% (48/54), and the specificity was 99.98% (11 488/11 490). (3) Of 48 CHDs, 11 cases were accompanied by other malformations. Eleven cases were performed amniocentesis, among whom 5 cases were trisomy 21.CONCLUSION:The screening program based on four-chamber and outflow tract views shows good sensitivity and excellent specificity. Our prenatal heart screening program is clinically feasible.
Comparison of high-frequency and transvaginal ultrasound in the diagnosis of early pregnancy

Abstract:Objective To investigate the diagnostic value of high-frequency ultrasound in the first trimester pregnancy.Methods Pregnant women with gestational sac within 70 mm from the skin were selected by abdominal ultrasonography and further examined with high-frequency ultrasound and transvaginal ultrasound respectively.The images of gestational sac,yolk sac and embryo were compared between two examinations.Results Of 108 pregnant women,high-frequency ultrasound and transvaginal ultrasound showed gestational sac,yolk sac and embryo in 94(87.04%) vs.97(89.81%),82(75.93%) vs.85(78.70%),and 58(53.70%) vs.58(53.70%),respectively.There was no significant difference in the detection rates of these indicators between the two methods(P>0.1).Conclusion Both high-frequency ultrasound and transvaginal ultrasound examinations may effectively show gestational sac,yolk sac and embryo in early pregnant women with thin belly.

Abstract:Objective The aim of this study was to know about the prenatal ultrasound screening and diagnosis of fetal congenital heart disease.Method Fetus who were diagnosed congenital heart disease through prenatal fetal echocardiography or after birth from April 1st 2010 to December 31st 2011 were collected.We studied the mean gestational weeks of prenatal diagnosis,major abnormalities of heart disease and the outcome of fetus.Results 33 cases of congenital heart disease were prenatal diagnosed.9 of them were born alive and 24 of the others had termination of pregnancy.11 ventricular septal defects,2 pulmonary stenosis and 1 pulmonary atresia were discovered after birth.Conclusions We could discover severe congenital heart disease through ultrasound screening during 20-24 gestational weeks and made a prenatal diagnosis by fetal echocardiography.Thus we could reduce neonatal mortality.
Importance of "guidelines for Performing Fetal Cardiac Scan" in Prenatal Screening for Fetal Congenital Heart Disease

Abstract:OBJECTIVE:To investigate the application of "Guidelines for performing fetal cardiac scan", issued by the International Society of Ultrasound in Obstetrics and Gynecology in 2006, in prenatal screening of fetal congenital heart disease (CHD).METHOD:Totally, 5000 singleton pregnancies presented at the Maternal-Fetal Medical Center of the Affiliated Drum Tower Hospital of Nanjing University Medical School from September 2006 to July 2007, for prenatal screening were included in this study, with the median maternal age of 28 (range, 18 approximately 48) and the median gestation of 27 (range, 18 approximately 40) weeks. Ultrasound screenings were performed on each fetal heart according to "Guidelines for performing fetal cardiac scan" via the four-chamber and outflow tracts & three-vessel views and fetal echocardiographies were further conducted for suspected cases. Once congenital heart disease was confirmed, amniocentesis or cordocentesis was suggested for fetal karyotyping for ongoing pregnancies and autopsy was performed when the pregnancy was terminated after formal consent. Born babies were followed up at 2 approximately 6 months of age using echocardiography.RESULT:The four-chamber views were successfully obtained in 97.64% (4882/5000) of all the pregnancies, among which the left ventricular and right ventricular outflow tracts and three-vessel views were obtained in 87.69% (4281/4882), 82.51% (4028/4882) and 96.29% (4701/4882), respectively. Higher successful rate was found in the second trimester than the third trimester in obtaining the standard views (P < 0.05). Finally, 73 (1.50%) among the 4882 cases were diagnosed as CHD. Fifty of them were diagnosed prenatally (24 cases in the second trimester and 26 cases in the third trimester) and 23 were missed and 1 misdiagnosed by prenatal ultrasound. Eighteen cases were found with extracardiac malformations. Autopsy was performed in 19 CHD which diagnosed prenatally, and all autopsy reports were consistent with ultrasound foundings. Twelve babies received postnatal echocardiography among which 11 were unanimous, and 1 baby diagnosed as tricuspid insufficiency prenatally was confirmed normal after birth. Abnormal karyotype was found in 7 out of the 23 who had karyotyping performed. Altogether, 28 cases were diagnosed by four chamber view only and 50 cases by combining other views, giving the sensitivity, specificity, false negative rate and false positive rate of 69% (50/73), 99.98% (4808/4809), 0.48% (23/4831) and 2% (1/51), respectively.CONCLUSION:The "Guidelines for performing fetal cardiac scan" is practical and easy to abide by. The optimal time for fetal cardiac examination is at 18 approximately 27 weeks of gestation. Four-chamber view together with the outflow tracts and three-vessel views examination can detect 69% of CHD in utero.

Pregnancy Ultrasound Examine for Application in Fetal Heart Structure Abnormity

Abstract:Objective To study the early fetal heart stracture malformations in order to avoid birth defect.Methods 2D doppler ultrasound examine of fetal hearts was carried out in 16 weeks of pregnant women among whom,111 were in the high risk groups,362 in the normal group.Results There were 52 cases of malformation in which 8 fetal had cardiac malgormation,47 fetal had malformation outside of the heart,and 7 fetals with congenital heart malformation were in the high risk group.The detection rate is 6.31%.Only one fetal with congenital heart malformation was in the normal group.The detection rate was 0.28%.Significantly difference was noted between two groups(P<0.001).Conclusion The congenital malformation detected in heart and outside part of heart is positively correlated to high risk fectors.Using ultrasound screening,we should emphasize examine the plane of four cavity heart in which the higher detection rate of abnormal heart structure can be increased.We suggest that fetal echocardiography as well as a prenatal fetal caidiac screening system should be undertaken for high rish pregancy.
Dynamic sequential cross-sectional scanning increases detection rate of congenital heart disease in sonographers: a prenatal ultrasound training program

Abstract:Background: Prenatal ultrasound is the preferred modality for diagnosing fetal congenital heart disease. Given issues of physician proficiency and hospital distribution, we propose a dynamic sequential cross-sectional scanning (SCS) to explore the feasibility of cardiac screening by sonographers with less than 5 years of experience in ultrasound. Materials and methods: Twenty residents were randomly divided into two groups, receiving training in the American Institute of Ultrasound in Medicine (AIUM) fetal echocardiography and the SCS method. According to the needs of training, the professional staff developed the theoretical knowledge question bank, the CHD ultrasonic video disease bank, and the assessment scale. Trainees completed the pre-training examination, theory and skill operation training, and post-training assessment. For the two groups, the theoretical knowledge, skill operation and disease diagnosis were analyzed statistically before and after training. Results: After training, the trainees in both groups had significantly improved knowledge and diagnostic abilities, their diagnostic thinking about CHD was clear, and they could identify major or even all structural abnormalities and make a definite diagnosis. In terms of skill operation, both groups could complete all required scanning within the specified time. The scanning time of the SCS group was significantly lower than that of the AIUM group, and the effect of the receptor site in the AIUM group was significantly higher than that in the SCS group. Conclusion: SCS can be used as a new rapid fetal cardiac scanning method and try to popularize among echocardiographer.

Abstract:Objective To evaluate the role of ultrasound screening in fetus at 11-13+6 weeks and 14-17+6 weeks in unselected pregnant women. Methods From January 2002 to April 2008,first-time prenatal ultrasound routine examinations were performed for 3645 pregnant women at 11-17+6 gestationalweeks in the First Affiliated Hospital of Sun Yat-sen University,including 1556 cases of 11-13+6 and 2089 of 14-17+6 gestational weeks. Detection and types of structural anomalies were compared between these two groups. Results Primary detection identified fetal structural abnormalities in 2.3% (36/1556) and 3.3%(68/2089) of women at 11-13+6 and14-17+6 gestational weeks,respectively,with no statistically significant difference. However,constituents of structural anomalies varied (P=0.03),with the first three leading types being nuchal abnormalities[31.6% (18/57)](increased nuchal translucency thickness and cystic hydroma),fetal hydrops[15.8%(9/57)]and abdominal wall defects[12.3%(7/57)]in the 11-13+6 weeks group,and cardiovascular defects[16.7% (18/108)],central nervous system malformations[14.8% (16/108)]and abdominal wall defects[12.0%(13/108)]in the 14-17+6 weeks group. Conclusions The ultrasound examinations performed at 11-13+6 weeks can be useful in screening the soft markers for chromosomal abnormalities and certain fatal malformations,while those performed at 14-17+6 weeks may identify more details of fetal structure. The 11-13+6 weeks of gestation may be an optimal window for early ultrasound screening of the fetus. Key words: Ultrasonography,prenatal; Screening; Fetus; Malformation; Early pregnancy

Application of SonoVCAD in Fetal Heart

Abstract:Objective To explore the application value of SonoVCAD for the routine screening of fetal heart.Methods Forty-eight normal fetuses in the second trimester were divided into two groups according to gestational age:group(19- 22 w)and group B(22-25 w).All the fetal hearts were examined by SonoVCAD and two-dimentional ultrasound.One- time recurrence of LVOT,RVOT and stomach slice view could be realized by SonoVCAD.The image quality was evaluated and graded,and the scores achived by two means were compared.Results There was no significant difference in qualified rate of image quality between SonoVCAD and two-dimentional ultrasound in group A(P0.05).In group B,there was no significant difference in qualified rate of image quality except LVOT(P0.05).The time of sampling of three-dimentional volume data is significantly shorter than that of two-dimentional ultrasound(P0.05).Conclusion By standardizing the acquisition of data and imaging recurrence,SonoVCAD can meet the requirement of the routine fetal heart screening.
The Correlation for Prenatal Ultrasound Screening of Fetal Malformations and Fetal Chromosomal Abnormality

Abstract:Objective:To assess the effectiveness of systematic ultrasound in second or third trimester to detect fetal malformations for instructing the necesity of further invasive prenatal diagnosis.Methods:This retrospective study included pregnancy women from January 2008 to June 2009 in the Affiliated Drum Tower hospital of Nanjing University Medical College.Those women had systematic ultrasound and echocardiogram examination within the second or third trimester of pregnancy.When fetal malformations were found,the invasive prenatal diagnosis(puncture of amniotic fluid or umbilical vein) was suggested to check the chromosomal abnormalities and analyze their relationship.Results:105 pregnant women who were found fetal abnormalities were recruited and had the invasive prenatal diagnosis,26 cases of chromosomal abnormalities were found in 77 obvious ultrasound abnormalities.There was significant difference in chromosomal abnormalities between obvious and minor ultrasound abnormalities(χ2=12.566,P0.001),especially for fetal congenital heart disease(CHD) combined with extracardiac malformations,their chromosomal abnormalities could reach 55%(11/20).Conclusions: Prenatal systematic ultrasound and echocardiogram could find most of the fetal malformations,especially for CHD,and offer valuable evidence to accept the invasive prenatal diagnosis.
The Construction and Application of an Ultrasound and Anatomical Cross‐sectional Database of Structural Malformations of the Fetal Heart

Abstract:Objectives Establish a fetal heart anatomical cross-sectional database that correlates with screening transverse ultrasound images suggested by international professional organizations to detect congenital heart defects. Methods Fetuses with suspected congenital heart defects identified using the following cardiac image sequences obtained from transverse slices beginning from the upper abdomen and ending in the upper thorax were the subjects of this study: (1) four-chamber view, (2) left ventricular outflow tract view, (3) three-vessel right ventricular outflow tract view, and (4) the three-vessel tracheal view. A database of digital two-dimensional images of the transverse sweep was created for fetuses with confirmed congenital heart defects. In addition, using four-dimensional ultrasound spatial-temporal image correlation, selected transverse ultrasound images were acquired as part of the database. Ultrasound-detected congenital heart defects were confirmed postnatally from pathological specimens of the heart and lungs using a cross-sectional technique that mirrored the ultrasound images described above. When anatomical specimens were not available, prenatal ultrasound-detected congenital heart defects were confirmed using postnatal echocardiography and/or following surgery. Results The four screening views described in the Methods section identified 160 fetuses that comprised the database. Forty-five datasets consisted of both ultrasound and anatomical cross-sectional images. Thirteen percent (6/45) only had abnormalities of the four-chamber view (eg, endocardial cushion defects). Twenty-four percent (11/45) had abnormalities of the four-chamber view as well as right and left outflow tracts (eg, complex malformations). Of these, 10 of 11 had an abnormal tracheal view. Sixteen percent (7/45) had an abnormal four-chamber view and abnormal right outflow tract (eg, pulmonary stenosis). Thirty-three percent (15/45) had a normal four-chamber view but had abnormal right and left outflow tracts as well as an abnormal tracheal view (eg, tetralogy of Fallot, D-transposition of the great arteries). Conclusions Combining both ultrasound and anatomical imaging may be of assistance in training imagers to recognize cardiovascular pathology when performing the screening examination of the fetal heart.
