Abstract:Major depressive disorder (MDD) is a debilitating and prevalent disorder associated with lower quality of life and substantial economic burden. Recently, there has been strong interest in respiratory sinus arrhythmia (RSA) as a biological predictor of later depression. Theoretical work suggests that higher resting RSA indexes physiological flexibility and better emotion regulation whereas lower RSA may mark vulnerability for psychopathology. However, empirical findings have varied. This study examined whether lower resting RSA predicted later depressive symptoms in a sample of healthy young adults across one year (n=185). Results indicate that year one (Y1) resting RSA predicted Y2 depressive symptoms. This finding remained significant when accounting for the stability of RSA and depressive symptoms across both time points and when including trait anxiety, body mass index, and medication use in statistical models. Findings provide further support for RSA as a promising biological marker for understanding and predicting depressive symptoms.

Abstract:Abstract Introduction Previous research has demonstrated that rest-activity rhythm (RAR) profiles, formed by individual RAR characteristics, were linked to the severity of depressive symptoms. Further investigation into RAR profiles could potentially facilitate the identification of depression and validate its role in depression. This study aimed to identify the RAR profiles in samples with and without major depressive disorder (MDD). Methods Actigraphy data were collected from 70 Chinese adults (34 with current MDD and 36 age- and gender-matched healthy controls) aged between 18 and 64. RAR measures were computed based on the collected 7-day actigraphy data, and a data-driven cluster analysis was performed to identify distinct RAR profiles. The associations between the actigraphy-derived RAR profiles and various psychiatric symptom/health-related measures, including anxiety and depressive symptoms, insomnia severity, fatigue, and quality of life, were examined. Results Two distinct groups with different RAR features (“earlier/robust” group: n = 63, 90%; “later/irregular”; n = 7, 10%) were identified from the sample using cluster analysis. The “later/irregular” group exhibited later activity onset and acrophase, along with lower levels of robustness and regularity compared to the “earlier/robust” group. Regarding psychiatric symptom/health-related measures, the “later/irregular” group displayed significantly higher levels of depressive symptoms [t(8.77) = -3.36, p <.05], insomnia severity [t(15.3) = -6.85, p <.05], fatigue [t(8.80) = -2.68, p <.05], and a higher percentage of participants with current MDD [χ2(1) = 4.30, p <.05] relative to the “earlier/robust” group. However, insignificant differences in health-related quality of life [t(7.71) = 2.27, p =.054] were found between two groups. Conclusion The current findings contribute further evidence on the association between RAR profiles and MDD, suggesting that RAR profiles could potentially be a biological marker for identifying depression. The observed associations between RAR profiles and other related symptoms demonstrated the utility of RAR profiles in detecting other health-related concerns. However, it is important to note that confirmatory studies with larger sample sizes and longitudinal designs are necessary to validate these findings and reveal the underlying mechanisms and temporal relationship between circadian rhythms and depression. Abstract:Continuous monitoring of cortical blood flow provides real time information of CBF-changes during neurosurgical operations and on the neurointensive care unit. Laser Doppler flowmetry is a continuous, noninvasive technique suitable for measurement of the cortical microcirculation. In a number of experimental studies we have analyzed the characteristics of this method for cortical blood flow monitoring. The high spatial resolution of laser Doppler flowmetry and the heterogeneity of the cortical microvascular network results in a scatter of flow values over a wide range depending on the site of measurement. Data collection from different spots and calculation of frequency histogram may serve as a measure of cortical blood flow. For clinical application instead of single fiber probes a multispot measurement approach will provide a reliable cortical blood flow monitoring. The use of continuous techniques will lead to a better understanding of cerebral hemodynamics under pathological conditions.
Association between rest-activity rhythm and diabetic retinopathy among US middle-age and older diabetic adults

Abstract:Background The disruption of circadian rhythm has been reported to aggravate the progression of diabetic retinopathy (DR). Rest-activity rhythm (RAR) is a widely used method for measuring individual circadian time influencing behavior. In this study, we sought to explore the potential association between RAR and the risk of DR. Methods Diabetic participants aged over 40 from 2011-2014 National Health and Nutrition Examination Survey (NHANES) were enrolled. Data from the wearable device ActiGraph GT3X was used to generate RAR metrics, including interdaily stability (IS), intradaily variability (IV), most active 10-hour period (M10), least active 5-hour period (L5), and Relative amplitude (RA). Weighted multivariable logistic regression analysis and restricted cubic spline analysis were conducted to examine the association between RAR metrics and DR risk. Sensitivity analysis was also conducted to examine the robustness of the findings. An unsupervised K-means clustering analysis was conducted to identify patterns in IV and M10. Results A total of 1,096 diabetic participants were enrolled, with a DR prevalence of 20.53%. The mean age of participants was 62.3 years, with 49.57% being male. After adjusting covariates, IV was positively associated with DR (β: 3.527, 95%CI: 1.371-9.073). Compared with the lowest quintile of IV, the highest quintile of IV had 136% higher odds of DR. In contrast, M10 was negatively associated with DR (β: 0.902, 95%CI: 0.828-0.982), with participants in the highest M10 quintile showing 48.8% lower odds of DR. Restricted cubic spline analysis confirmed that these associations were linear. Meanwhile, sensitivity analysis confirmed the robustness. K-means clustering identified three distinct clusters, with participants in Cluster C (high-IV, low-M10) had a significantly higher risk of DR comparing with Cluster A (low-IV, high-M10). Conclusion A more fragmented rhythm and lower peak activity level might be associated with an increased risk of DR. These findings indicate that maintaining a more rhythmic sleep-activity behavior might mitigate the development of DR. Further research is necessary to establish causality and understand the underlying mechanisms, and focus on whether interventions designed to enhance daily rhythm stability and increase diurnal activity level can effectively mitigate the risk of progression of DR.

Abstract:BACKGROUND Optimal chemotherapeutic regimen in thymic carcinoma remains uncertain and the efficacy of second line chemotherapy has not been established either. PATIENTS AND METHODS We retrospectively evaluated the efficacy of an irinotecan plus cisplatin or carboplatin (IP) regimen as a salvage treatment for patients with unresectable thymic carcinoma that progressed after cisplatin, doxorubicin, vincristine and cyclophosphamide (ADOC) chemotherapy. Seven patients with histologically confirmed thymic carcinoma that was resistant to or who had relapsed after initial chemotherapy with ADOC were treated with IP. The treatment consisted of irinotecan (CPT-11, 60 mg/m2, days 1, 8 and 15) and cisplatin (80 mg/m2, day 1) or carboplatin (AUC 4) intravenously every 4 weeks, for at least 2 cycles. RESULT Two patients achieved partial responses. Although another two patients showed a significant reduction of the primary thoracic lesion, the appearance of a new lesion was found in one and a metastatic lesion was unchanged in the other. Neutropenia over grade 3 was observed in all patients but none of the patients developed serious infections. There were no severe non-hematological toxicities, including diarrhea. CONCLUSION We conclude that salvage chemotherapy may be useful in certain patients with thymic carcinoma and irinotecan may be a novel and alternative agent for relapsed thymic carcinoma.
The association between recreational physical activity and depression in the short sleep population: a cross-sectional study

Abstract:Background: Short sleep is more common in the modern society. Recreational physical activity (RPA) like exercise brings both mental and physiological benefits to depression; paradoxically, lack of sleep is harmful. Evidence on the association between RPA and depression in the short sleep population is limited. Methods: Participants with short sleep condition from the National health and Nutrition Examination Surveys (NHANES 2007–2018) were included in the present study. Short sleep condition was defined as ≤7 h per night. Sleep duration and RPA status were self-reported in NHANES by the Physical Activity Questionnaire using a 7-day recall method. Multivariable logistic regression was applied to evaluate the association between RPA and depression. Additionally, the non-linear relationship between RPA and depression was evaluated using the threshold effect analysis and restricted cubic spline. Results: This cross-sectional study comprised 6,846 adults' data, and the weighted participants were 52,501,159. The weighted prevalence of depression was higher in females, which took up 65.85% of all depression patients. In fully adjusted models, sufficient volume of RPA was associated with lower depression risks, with OR (95% CI) =0.678 (0.520, 0.883). Further analysis revealed a U-shaped association between RPA and incident depression, and the inflection point was 640 MET-minutes/week. When RPA <640 MET-minutes/week, increased RPA was associated with lower risk of incident depression, with OR (95% CI) = 0.891 (0.834, 0.953). When RPA ≥ 640 MET-minutes/week, the benefits of RPA seemed to be not significant, with OR (95% CI) = 0.999 (0.990, 1.009). Conclusion: Our findings observed associations between RPA condition and incident depression in the short sleep population. Moderate RPA was beneficial to maintain mental health and associated with lower incidence of depression for short sleepers, but excessive RPA might increase the risk of depression. For general short sleepers, keeping the RPA volume approximately 640 MET-minutes/week was beneficial to lower risks of depression. Gender difference should be considered as an important factor for further studies to examine these relationships and explore mechanisms.

Rest-Activity Rhythm Characteristics Associated With Depression Symptoms in Stroke Survivors

Abstract:Objective: To examine which 24-hour rest-activity rhythm (RAR) characteristics are associated with depression symptoms in stroke survivors. Design: Cross-sectional observational study examining associations of RAR characteristics with the presence of depression symptoms adjusting for age, sex, race, and medical comorbidity. Setting: Community setting. Participants: Stroke survivors: (1) recruited locally (N women=35, N men=28) and (2) a nationally representative probability sample (the National Health and Nutrition Examination Survey [NHANES]; N women=156, N men=124). Interventions: None. Measurements: Objective RAR characteristics derived from accelerometer recordings including activity onset/offset times and non-parametric measures of RAR strength (relative amplitude), stability (interdaily stability), and fragmentation (intradaily variability). The presence of depression symptoms was categorized using Patient Health Questionnaire scores. Results: In both samples, the only RAR characteristic associated with depression symptoms was intradaily variability (fragmentation): local sample, odds ratio=1.96 [95% confidence interval=1.05-3.63]; NHANES sample, odds ratio=1.34, [95% confidence interval=1.01-1.78]). In the NHANES sample, which included both mild and moderate/severe depression, the association between 24-hour sleep-wake fragmentation and depression symptoms was driven by moderate-to-severe cases. Conclusions: Stroke survivors with higher levels of RAR fragmentation were more likely to have depression symptoms in both samples. These findings have implications, given prior studies in general samples linking RAR fragmentation with future depression and dementia risk. Research is needed to establish the potential consequences, mechanisms, and modifiability of RAR fragmentation in stroke survivors.
Sleep-wake behavioral characteristics associated with depression symptoms: Findings from the Multi-Ethnic Study of Atherosclerosis

Abstract:Abstract Study Objectives To help prioritize target/groups for experimental intervention studies, we characterized cross-sectional associations between 24-hour sleep/wake measures and depression symptoms; and evaluated if similar sleep/wake-depression relationships existed in people with and without higher insomnia severity. Methods Participants had ≥3 days of actigraphy data (n=1884; mean age=68.6/SD=9.1; 54.1% female). We extracted 18 sleep, activity, timing, rhythmicity, and fragmentation measures from actigraphy. We used individual and multivariable regressions with the outcome of clinically significant depression symptoms (Center for Epidemiologic Studies Depression Scale ≥16). We conducted sensitivity analyses in people with higher insomnia severity (top quartile of the Women's Health Initiative Insomnia Rating Scale total score). Results From separate models in the overall sample, the odds of having depression symptoms were higher with: later timing (e.g., activity onset time odds ratio (OR) per 1 SD=1.32; 95% confidence interval (CI): 1.16-1.50), lower rhythmicity (e.g., pseudo-F OR per 1 SD=0.75; 95% CI: 0.66-0.85), less activity (e.g., amplitude OR per 1 SD=0.83; 95% CI: 0.72-0.95), and worse insomnia (OR per 1 SD=1.48, 95% CI: 1.31-1.68). In multivariable models conducted among people with lower insomnia severity, later timing, lower rhythmicity, and higher insomnia severity were independent correlates of depression. In people with higher insomnia symptom severity, measures of later timing were most strongly associated with depression symptoms. Conclusions These correlative observations suggest that experimental studies are warranted to test if: broadly promoting 24-hour sleep/wake functioning reduces depression even in people without severe insomnia; and if advancing timing leads to depression symptom reductions in people with insomnia.

Abstract:Abstract Introduction We investigated the association between circadian rest/activity rhythms (RARs) and daytime insomnia symptoms among older adults with insomnia. We hypothesized that: 1) RARs differ between older adults with insomnia and healthy sleepers; and 2) among older adults with insomnia, RARs are associated with daytime insomnia symptoms. Methods Older adults with insomnia (n=30) and healthy sleepers (n=33) were recruited from a tertiary academic medical center. Nonparametric RARs were derived from actigraphy data over 14 days: relative amplitude (RA), interdaily stability (IS), intradaily variability (IV), maximum ten-hour activity (M10) and minimum five-hour activity (L5). Secondary parametric RARs included amplitude, acrophase, and midline statistics of rhythm (MESOR). Daytime function was evaluated via the Daytime Insomnia Symptoms Scale (administered via smartphone 4x/day, i.e., 56 assessments per participant) and validated research questionnaires (administered once). Between-groups differences (insomnia vs healthy sleep) were evaluated using t-tests. Among individuals with insomnia, a series of 104 linear regression models examined the association between RARs and daytime symptoms. The Benjamini-Hochberg procedure for correcting false discovery rate (BH-FDR) was used to correct for multiple comparisons. Results Relative to healthy sleepers, older adults with insomnia demonstrated significantly lower RA (p=0.028,t=-2.28) and significantly higher L5 (p=0.018,t=2.48). Among older adults with insomnia, IS was positively associated with positive mood (p=0.013, t =2.67) and negatively associated with pre-sleep somatic arousal (p=0.01, t=-3.06). IV was negatively associated with DISS subscales including Alert Cognition (p=0.001, t=-3.64) and Positive Mood (p< 0.001, t=-4.15) and positively associated with Negative Mood (p< 0.001, t=4.74), and Sleepiness/fatigue (p=0.005, t=3.11), as well as validated questionnaires measuring depression (p=0.02, t=2.47), negative mood (p< 0.01, t=3.43), and pre-sleep somatic arousal (p=0.01, t=3.77). M10 was negatively associated with depression (p< 0.0,1 t=-3.29) and anxiety (p=0.01 t=-2.9). Of parametric RARs, amplitude was negatively associated with negative mood (p=0.005, t=-3.08) and depression (p=0.01, t=-2.75), and positively associated with positive mood (p=0.010, t=2.77). MESOR was negatively associated with depression (p=0.02, t=-2.48) and anxiety (p=0.01, t=-2.81). Conclusion Among older adults with insomnia, RARs were associated with daytime insomnia symptoms. Abstract:Objectives: Depression is largely preventable, and strategies that can effectively suppress its development are imperative. We aimed to examine whether physical activity and sedentary behavior were associated with depression and explore the possible mediatory role of complete blood count in this association. Methods: In this cross-sectional study, data were integrated from the National Health and Nutrition Examination Study (2007–2018). Depression was defined using the Patient Health Questionnaire-9. The risk for depression, expressed as odds ratio (OR) and 95% confidence interval (CI), was quantified by survey-weighted logistic regression analyses. Results: A total of 31,204 respondents were analyzed. Significance was identified for all, except walking or bicycling per week, types of physical activity, and sedentary behavior. Per 1 standard deviation (SD) increment in metabolic equivalent of task (MET) of weekly vigorous recreational physical activity was associated with 31.3% decreased depression risk (adjusted OR: 0.687, 95% CI: 0.5663–0.840). Per 1 SD increment in sitting time can increase depression risk by 22.4% (adjusted OR: 1.224, 95% CI: 1.131–1.325). In subsidiary analyses, the association with depression was reinforced in respondents aged ≤65 years and those overweight or obese. Mediation analyses revealed significant effects for red blood cell (RBC) on total MET (19.4%) and moderate work-related physical activity (MWPA) (22.0%), and for red cell distribution wide (RCDW) on vigorous work-related physical activity (17.7%), moderate work-related physical activity (13.1%), total MET (11.2%), and sitting time (16.4%) ( p < 0.01). Conclusion: Our findings indicate that more physical activity and less sitting time were associated with a lower likelihood of having depression among US adults, and this association was probably mediated by RBC and RCDW.

Abstract:AIMS:To assess the association of sleep factors (sleep duration, trouble sleeping, sleep disorder) and combined sleep behaviours with the risk of clinically relevant depression (CRD).METHODS:A total of 17 859 participants (8806 males and 9053 females) aged 20-79 years from the National Health and Nutrition Examination Survey (NHANES) 2007-2014 waves were included. Sleep duration, trouble sleeping and sleep disorder were asked in the home by trained interviewers using the Computer-Assisted Personal Interviewing (CAPI) system. The combined sleep behaviours were referred to as 'sleep patterns (healthy, intermediate and poor)', with a 'healthy sleep pattern' defined as sleeping 7-9 h per night with no self-reported trouble sleeping or sleep disorders. And intermediate and poor sleep patterns indicated 1 and 2-3 sleep problems, respectively. Weighted logistic regression was performed to evaluate the association of sleep factors and sleep patterns with the risk of depressive symptoms.RESULTS:The total prevalence of CRD was 9.5% among the 17 859 participants analysed, with females having almost twice as frequency than males. Compared to normal sleep duration (7-9 h), both short and long sleep duration were linked with a higher risk of CRD (short sleep: OR: 1.66, 95% CI: 1.39-1.98; long sleep: OR: 2.75, 95% CI: 1.93-3.92). The self-reported sleep complaints, whether trouble sleeping or sleep disorder, were significantly related with CRD (trouble sleeping: OR: 3.04, 95% CI: 2.59-3.56; sleep disorder: OR: 1.83, 95% CI: 1.44-2.34). Furthermore, the correlations appeared to be higher for individuals with poor sleep pattern (OR: 5.98, 95% CI: 4.91-7.29).CONCLUSIONS:In this national representative survey, it was shown that there was a dose-response relationship between sleep patterns and CRD.
Association between 24-hour movement behavior and depression in college students: A compositional data analysis

Abstract:Background: Evidence strongly suggests that moderate-to-vigorous physical activity (MVPA) can help reduce symptoms of depression. However, the relationship between 24-h movement behaviors and depressive symptoms, as well as the potential substitution effects between different behaviors, remains unclear. Therefore, this study seeks to investigate how the time spent in one behavior relative to others is associated with depressive symptoms. Methods: The study recruited 366 participants, who had their levels of MVPA, light PA (LPA), and sedentary behavior (SB) recorded using tri-axial accelerometers over 7 consecutive days. Participants also self-reported their sleep duration (SLP). Depression scores were measured using the Center for Epidemiological Studies Depression Scale (CESD). Using compositional linear regression, the study explored the connections between different compositions of time spent in movement behaviors and depression symptom scores. Results: Spending more time on LPA and MVPA relative to other behaviors were associated with lower depression scores, whereas a higher proportion of SB relative to other activities was linked to increased depression scores. Reallocating time spent on SB to LPA and MVPA was associated with decreased depression scores. Limitation: This study is a cross-sectional research design, and causality cannot be inferred. In addition, the sampling method of this study may have imposed some limitations on the representativeness of the sample. Further longitudinal studies could be conducted by selecting a large, representative sample. Conclusion: Within the spectrum of 24-h movement behaviors, increasing the duration of LPA and MVPA emerges as advantageous for reducing depression scores, suggesting a potential benefit in reallocating SB to PA, especially MVPA.

Abstract:Background The rapid pace of life nowadays has seen a gradual increase in public involvement in weekend warrior (WW), a physical activity (PA) pattern that allows people to exercise once or twice a week, the recommended moderate-to-vigorous PA per week, since regular PA takes much time. We aim at exploring the effect of WW activity and other PA patterns on depression symptoms in U.S adults. Methods The level of PA was measured by self-reporting activity patterns, (inactive, insufficiently active, WW and regularly active). Participants with Patient Health Questionnaire-9 (PHQ-9) scores above 10 are considered to have depression symptoms. Results A weighted sample of 23,258 participants representing 1049.8 million non-institutionalized U.S adults aged from 20 to 80. Compared with the inactive group, general adults who met the PA guidelines with PA once or twice per week [WW, adjusted odds ratio (AOR) = 0.790, 95%CI: 0.638, 0.987] or more frequent PA [Regularly active, (AOR = 0.761, 95%CI: 0.671, 0.864)], were inversely associated with depression symptoms, while the association has not been observed in adults with insufficiently active PA (AOR = 0.892, 95%CI: 0.783, 1.017). Increase in minutes, sessions and intensity of PA in regularly active and WW groups brought additional benefits for depression symptoms. Conclusion WW and other equivalent PA intensities patterns may be sufficient to reduce the risk of depression symptom. With the same recommended levels of PA, whether spread over the week or done in fewer days, adults may achieve the same benefits.

Abstract:Abstract Introduction Diet, sleep, and rest-activity rhythms (RAR) influence cardiometabolic health but the relationship among these behaviors is less clear. Evidence of associations between sleep and diet have focused on self-reported sleep duration and quality, and have rarely investigated sleep timing, regularity, and RAR metrics. This study assessed cross-sectional associations between sleep and RAR regularity and timing with overall diet quality. Methods Multi-ethnic Study of Atherosclerosis (MESA) participants who completed validated sleep and food frequency questionnaires and 7-day actigraphy monitoring (Actiwatch Spectrum) between 2010-2013 were eligible for study inclusion. Overall diet quality was measured with the Alternate Healthy Eating Index (AHEI) 2010 (range: 0-110, higher indicates healthier diet). Actigraphy records were processed using Actiware-Sleep (v5.59) software and manually annotated using a sleep diary. Sleep timing was measured with average sleep midpoint, and sleep regularity measures included within-individual standard deviation (SD) of daily sleep duration and onset. Parametric (extended cosine) and nonparametric 24-hour RAR measures were derived from 1-minute epoch-level activity counts. Associations between sleep and RAR metrics with AHEI were tested with multivariable linear regression adjusting for total energy intake, sociodemographic and lifestyle factors including smoking, total activity, and depressive symptoms. Results This study included 1828 participants (mean [SD]: age: 68.5 [9] years, AHEI: 59 [10.9]; 46% male). Later sleep midpoint and L5-time (start time of the 5 least active hours) were associated with lower AHEI (0.61% and 0.77% lower AHEI per SD later midpoint [p=0.005] and L5-time [p=0.001], respectively). Irregular sleep timing (sleep onset-SD >60 minutes [vs. less]) was associated with 1.1% lower AHEI (p=0.014). Rhythm robustness, measured by the pseudo-F (parametric RAR) and relative amplitude (RA, non-parametric RAR) was associated with higher AHEI (0.49% and 0.48% higher AHEI per SD increase in pseudo-F [p=0.031] and RA [p=0.036], respectively). The associations between later sleep midpoint and L5-time with lower AHEI were robust to false discovery rate (FDR)-corrected significance thresholds. Conclusion Actigraphy-based measures of later sleep timing, irregular sleep, and weakened rest-activity rhythms were associated with lower/worse diet quality in an older, multi-ethnic sample, suggesting their potential utility in multi-component lifestyle interventions. Abstract:Summary Background The health effects of rest–activity rhythm are of major interest to public health, but its associations with health outcomes remain elusive. We aimed to examine the associations between accelerometer-measured rest–activity rhythm amplitude and health risks among the general UK population. Methods We did a prospective cohort analysis of UK Biobank participants aged 43–79 years with valid wrist-worn accelerometer data. Low rest–activity rhythm amplitude was defined as the first quintile of relative amplitude; all other quintiles were classified as high rest–activity rhythm amplitude. Outcomes of interest were defined using International Classification of Diseases 10th Revision codes and consisted of incident cancer and cardiovascular, infectious, respiratory, and digestive diseases, and all-cause and disease-specific (cardiovascular, cancer, and respiratory) mortality. Participants with a current diagnosis of any outcome of interest were excluded. We assessed the associations between decreased rest–activity rhythm amplitude and outcomes using Cox proportional hazards models. Findings Between June 1, 2013, and Dec 23, 2015, 103 682 participants with available raw accelerometer data were enrolled. 92 614 participants (52 219 [56·4%] women and 40 395 [42·6%] men) with a median age of 64 years (IQR 56–69) were recruited. Median follow-up was 6·4 years (IQR 5·8–6·9). Decreased rest–activity rhythm amplitude was significantly associated with increased incidence of cardiovascular diseases (adjusted hazard ratio 1·11 [95% CI 1·05–1·16]), cancer (1·08 [1·01–1·16]), infectious diseases (1·31 [1·22–1·41]), respiratory diseases (1·26 [1·19–1·34]), and digestive diseases (1·08 [1·03–1·14]), as well as all-cause mortality (1·54 [1·40–1·70]) and disease-specific mortality (1·73 [1·34–2·22] for cardiovascular diseases, 1·32 [1·13–1·55] for cancer, and 1·62 [1·25–2·09] for respiratory diseases). Most of these associations were not modified by age older than 65 years or sex. Among 16 accelerometer-measured rest–activity parameters, low rest–activity rhythm amplitude had the strongest or second- strongest associations with nine health outcomes. Interpretation Our results suggest that low rest–activity rhythm amplitude might contribute to major health outcomes and provide further evidence to promote risk-modifying strategies associated with rest–activity rhythm to improve health and longevity. Association Between Joint Physical Activity and Dietary Quality and Lower Risk of Depression Symptoms in US Adults: Cross-sectional NHANES Study

Abstract:Background: Depression escalating public health concern and the modest efficacy of currently available treatments have prompted efforts to identify modifiable risk factors associated with depression symptoms. Physical inactivity, poor nutrition, or other lifestyle behaviors are among the potentially modifiable risk factors most consistently linked with depression. Past evidence regarding the single effect of physical activity (PA) or dietary quality (DQ) on reducing the risk of depression symptoms has been well-documented. However, the association of the joint effect of PA and DQ on depression symptoms has never been investigated in a representative sample of adults. Objective: This study investigates the association between PA and depression symptoms and between DQ and depression symptoms, and their combined effects on US adults. Methods: Data were obtained from the National Health and Nutrition Examination Survey (NHANES) 2007 to 2018 cycles. The primary exposures were DQ and PA, measured using the Healthy Eating Index (HEI)-2015 and the metabolic equivalent (MET) minutes per week reported in questionnaires, respectively. Depression symptoms were defined as a 9-item Patient Health Questionnaire (PHQ-9) score of ≥10. We created 4 lifestyle categories: healthy diet and active individuals, unhealthy diet but active individuals, healthy diet but inactive individuals, and unhealthy diet and inactive individuals. Participants were considered to have a healthy diet if they fell within the 60th percentile of the HEI-2015 or to be active if they met the current guidelines for PA. A survey-multivariable logistic regression approach was used to model adjust the variables relevant to the associations, and an age-adjusted prevalence for depression symptoms was calculated following the NHANES guidelines. Results: In total, 19,295 participants represented a weighted number of 932.5 million adults aged 20 to 80 years in the noninstitutionalized US population. The total age-adjusted prevalence of depression symptoms among all respondents was 7.08% (1507/19,295). Of the respondents, 81.97% (15,816/19,295) met the PA recommendation and 26.79% (5170/19,295) scored at or above the 60th percentile on the HEI-2015. Depression symptoms were inversely associated with a higher level of PA (adjusted odds ratio [AOR] 0.819, 95% CI 0.716-0.938) and healthy DQ (AOR 0.809, 95% CI 0.701-0.931), respectively. A healthy diet combined with recommended PA was associated with a significantly lower risk of depression symptoms (AOR 0.658, 95% CI 0.538-0.803) than those who consumed an unhealthy diet but were physically active (AOR 0.890, 95% CI 0.765-1.038) or consumed a healthy diet but were physically inactive (AOR 1.077, 95% CI 0.817-1.406). Conclusions: Our findings indicate that people with a healthy diet and recommended PA have a lower risk of depression symptoms than those with an unhealthy diet and a low level of PA. A healthy dietary habit and regular PA are potential precautions against depression.

Abstract:Abstract Introduction Sleep disparities are well-documented in the population. Rarely are multiple dimensions of sleep experience evaluated on a national level, however. Even more novel is the implication that these differences are partially explained by mental health disparities. Methods Data from the Sleep In America Poll included a population-level sample of US adults. N=1042 adults provided complete data. Sleep health metrics included weekday and weekend bedtime and waketime, time in bed, and sleep duration, perceived sleep need, sleep debt (sleep need – duration), and days/week of: feeling well-rested, difficulty falling asleep, difficulty staying asleep, sleep impacting daytime function, daytime sleepiness, and taking sleep medication. Covariates included age, sex, education, household income, and PHQ9 depression score (without the sleep item). Linear regression analyses examined sleep variable as dependent variable and race/ethnicity (Non-Hispanic White, Black/African-American, Hispanic/Latino or Asian/Other) as independent variable, adjusted for covariates. Secondary analyses examined race/ethnicity-by-depression interactions. Results In population-weighted, adjusted analyses, compared to non-Hispanic Whites, Blacks/African-Americans reported 0.78hr delayed weekend waketime (95%CI: 0.14,1.43) an additional 0.49 days/week of daytime sleepiness (95%CI: 0.05,0.93), 0.47 fewer days/week taking sleeping medications (95%CI: -0.80,-0.14), 0.41hr perceived sleep need (95%CI: -0.74,-0.10), and 0.51hr weekday less sleep duration (95%CI: -0.87,-0.15). Hispanics/Latinos reported 1.05 days/week difficulty staying asleep (95%CI: -1.42,-0.67) and 0.38 fewer days/week taking sleep medications (95%CI: -0.70,-0.07). Asians/others reported 0.73 fewer days/week with trouble staying asleep (95%CI: -1.26,-0.19) and 0.51 fewer days/week taking sleep medications. Significant group-by-depression interactions were seen for weekday and weekend bedtime, weekday time in bed, days/week well rested, perceived sleep need, weekday sleep duration, and sleep debt. Stratified analyses showed that Blacks/African Americans and Asians/Others especially reported worse sleep health in the presence of higher depression scores, compared to Non-Hispanic Whites. Conclusion Several dimensions of sleep health were disproportionately experienced across race/ethnicity groups in the US. In particular, worse sleep health in Blacks/African-Americans and Asians/Others was further impacted by depression symptoms. Future work should further explore the role of mental health disparities in sleep health and mental health interventions may reduce sleep disparities. Support (if any)

Circadian Rhythm Disorders and Risk of Subsequent Depression: A Systematic Review and Meta-Analysis of Longitudinal Studies

Abstract:Background: Patients with depression often suffer from circadian rhythm disruptions, such as insomnia and a lower amplitude of daily rhythms of locomotor activity. However, whether circadian disorders precede depression and act as a predictor for subsequent depression have not been unanimously approved. In light of increasing evidence of an association between disrupted circadian rhythms and affective symptoms, a meta-analysis was conducted to examine the effect of circadian disruptions on incident depression. Methods: A systematic search was performed in PubMed, Embase, Web of Science, PsycINFO, PsycArticles, CINAHL database. The search results were restricted to be published in English up to April 10, 2020.Reference lists from included studies and relevant reviews were also screened. We included longitudinal cohort studies that examined the predictive role of all various types of circadian disruption in incident depression. Two reviewers independently extracted information on study design, participants’ characteristics and diagnostic criteria for circadian disruption and depression. Study quality was assessed using the Newcastle-Ottawa Scale. Random-effect models were applied to all meta-analyses given potential heterogeneity. For insomnia, subgroup analyses were performed based on subject’s characteristics, insomnia symptom domains and numbers and the investigated methods. Pool-effect estimates of relative risks to develop incident depression in groups with circadian disruption compared to groups without. This study is registered with PROSPERO, CRD42020179135. Findings: A total of 49 studies were eligible for this meta-analysis. Of 49 eligible studies, included types of circadian disruption were sleep disturbances (n = 45), chronotype (n = 2), cortisol awakening response (n = 1) and rest - activity rhythm (n = 1). Insomnia, hypersomnia, excessive daytime sleepiness (EDS), short sleep duration, obstructive sleep apnea (OSA), restless legs syndrome (RLS), an evening preference, heightened levels of cortisol in awakening response and low robustness of rest - activity rhythm all acted as significant predictors of occurrence of depression. Among insomnia subtypes, difficulty initiating sleep (DIS) and difficulty maintaining sleep (DMS) predicted future depression. Of note, persistent insomnia (OR: 5.02) contributed to more than two - fold risk of incident depression compared to insomnia (OR: 2.34). Moreover, insomnia symptom numbers were associated with incident depression in a gradient trend. Interpretation: Different types of circadian disorders act as risk factors of onset of depression, which clinicians should be informed of. Future research should be aimed to determine the casual relationship between other types of circadian disorders and onset of depression, and moreover to further elucidate the underlying mechanisms.
