Abstract:Continuous monitoring of cortical blood flow provides real time information of CBF-changes during neurosurgical operations and on the neurointensive care unit. Laser Doppler flowmetry is a continuous, noninvasive technique suitable for measurement of the cortical microcirculation. In a number of experimental studies we have analyzed the characteristics of this method for cortical blood flow monitoring. The high spatial resolution of laser Doppler flowmetry and the heterogeneity of the cortical microvascular network results in a scatter of flow values over a wide range depending on the site of measurement. Data collection from different spots and calculation of frequency histogram may serve as a measure of cortical blood flow. For clinical application instead of single fiber probes a multispot measurement approach will provide a reliable cortical blood flow monitoring. The use of continuous techniques will lead to a better understanding of cerebral hemodynamics under pathological conditions.
The Optimal Measurement Period of Actigraphy for Circadian Rhythm in Relation to Adiposity: A Retrospective Case-Control Study

Abstract:Background This study focused on the relationship between adiposity and Rest-Activity Rhythms (RAR), utilizing both parametric cosine-based models and non-parametric algorithms. The emphasis was on the impact of varying measurement periods (7 to 28 days) on this relationship. Methods We retrieved actigraphy data from two datasets, encompassing a diverse cohort recruited from an obesity outpatient clinic and a workplace health promotion program. Participants were required to wear a research-grade wrist actigraphy device continuously for a minimum of four weeks. The final dataset included 115 individuals (mean age 40.7 ± 9.5 years, 51% female). We employed both parametric and non-parametric methods to quantify RAR using six standard variables. Additionally, the study evaluated the correlations between three key adiposity indices — Body Mass Index (BMI), Visceral Adipose Tissue (VAT) area, and Body Fat Percentage (BF%) — and circadian rhythm indicators, controlling for factors like physical activity, age, and gender. Results The obesity group displayed a significantly lower relative amplitude (RA) as per non-parametric algorithm findings, with a decreased amplitude noted in the parametric algorithm analysis, in comparison to the overweight and control groups. The relationship between circadian rhythm indicators and adiposity metrics over 7- to 28-day periods was examined. A notable negative correlation was observed between RA and both BMI and VAT, while correlation coefficients between adiposity indicators and non-parametric circadian parameters increased with extended durations of actigraphy data. Specifically, RA over a 28-day period was significantly correlated with BF%, a trend not seen in the 7-day measurement (p = 0.094) in multivariate linear regression. The strength of the correlation between BF% and 28-day RA was more pronounced than that in the 7-day period (p = 0.044). However, replacing RA with amplitude as per parametric cosinor fitting yielded no significant correlations for any of the measurement periods. Conclusion The study concludes that a 28-day measurement period more effectively captures the link between disrupted circadian rhythms and adiposity. Non-parametric algorithms, in particular, were more effective in characterizing disrupted circadian rhythms, especially when extending the measurement period beyond the standard 7 days.

Abstract:Objective: Circadian rhythm, which is controlled by circadian genes, regulates metabolic balance including the circulating levels of glucose, fatty acids, trigly-cerides, various hormones and so on. The study aimed to investigate the impact of potential polymorphisms in circadian genes on abdominal obesity among Chinese Han adults.Methods: A total of 260 cases with abdominal obesity and 260 controls were recruited by individual matching. Demographic characteristics and lifestyle information were collected by a validated questionnaire, and anthropometric parameters was measured by physical examination. Twenty-three single nucleotide polymorphisms (SNPs) in three circadian genes, CLOCK, CRY1 and CRY2, were genotyped by MassArray technique.Results: Five SNPs significantly deviated from Hardy-Weinberg equilibrium (HWE) among controls, so eighteen SNPs were taken into logistic regression analysis. Independently, CLOCK rs10002541 (CC genotype vs. TT genotype: OR: 0.45, 95% CI: 0.23-0.86), CLOCK rs6850524 (CC genotype vs. GG genotype: OR: 0.50, 95% CI: 0.25-0.99) and CRY1 rs10861688 (TT genotype vs. CC genotype: OR: 0.50, 95% CI: 0.25-0.97) were negatively associated with the risk of abdominal obesity. Haplotype analysis showed that the haplotypes of CG and TG for CLOCK rs10002541 and rs4864546 had significant associations with abdominal obesity. Compared with the carriers of TA, those of CG were observed to have a lower risk (OR: 0.74, 95% CI: 0.56-0.99) of abdominal obesity, and those of TG presented a higher risk (OR: 1.70,95% CI: 1.03-2.81).Conclusions: Our findings suggest that CLOCK and CRY1 polymorphisms might beinvolved in individual susceptibility to abdominal obesity in Chinese Han population. Abstract:Summary Background The health effects of rest–activity rhythm are of major interest to public health, but its associations with health outcomes remain elusive. We aimed to examine the associations between accelerometer-measured rest–activity rhythm amplitude and health risks among the general UK population. Methods We did a prospective cohort analysis of UK Biobank participants aged 43–79 years with valid wrist-worn accelerometer data. Low rest–activity rhythm amplitude was defined as the first quintile of relative amplitude; all other quintiles were classified as high rest–activity rhythm amplitude. Outcomes of interest were defined using International Classification of Diseases 10th Revision codes and consisted of incident cancer and cardiovascular, infectious, respiratory, and digestive diseases, and all-cause and disease-specific (cardiovascular, cancer, and respiratory) mortality. Participants with a current diagnosis of any outcome of interest were excluded. We assessed the associations between decreased rest–activity rhythm amplitude and outcomes using Cox proportional hazards models. Findings Between June 1, 2013, and Dec 23, 2015, 103 682 participants with available raw accelerometer data were enrolled. 92 614 participants (52 219 [56·4%] women and 40 395 [42·6%] men) with a median age of 64 years (IQR 56–69) were recruited. Median follow-up was 6·4 years (IQR 5·8–6·9). Decreased rest–activity rhythm amplitude was significantly associated with increased incidence of cardiovascular diseases (adjusted hazard ratio 1·11 [95% CI 1·05–1·16]), cancer (1·08 [1·01–1·16]), infectious diseases (1·31 [1·22–1·41]), respiratory diseases (1·26 [1·19–1·34]), and digestive diseases (1·08 [1·03–1·14]), as well as all-cause mortality (1·54 [1·40–1·70]) and disease-specific mortality (1·73 [1·34–2·22] for cardiovascular diseases, 1·32 [1·13–1·55] for cancer, and 1·62 [1·25–2·09] for respiratory diseases). Most of these associations were not modified by age older than 65 years or sex. Among 16 accelerometer-measured rest–activity parameters, low rest–activity rhythm amplitude had the strongest or second- strongest associations with nine health outcomes. Interpretation Our results suggest that low rest–activity rhythm amplitude might contribute to major health outcomes and provide further evidence to promote risk-modifying strategies associated with rest–activity rhythm to improve health and longevity. Abstract:Objective: Rest-activity rhythm is an essential behavior for human health. However, the association between restactivity rhythm and atherosclerotic cardiovascular disease (ASCVD) risk remains unclear. Therefore, this study aimed to elucidate the association. Methods: This study included 87,039 participants from the UK Biobank who had 7-day accelerometry data and were free of ASCVD at baseline. Relative amplitude was calculated as the difference between the most active continuous 10-h period (M10) and the least active continuous 5-h period (L5) in 24 h, and lower relative amplitude indicated the disruption of rest-activity rhythm. Cox proportional hazard model was used to examine the association of relative amplitude with ASCVD. Further, the linear association between relative amplitude and arterial stiffness measurements, including arterial stiffness index (ASI) and carotid intima-media thickness (cIMT), was examined. Results: During a mean follow-up period of 6.80 +/- 1.10 years, 2798 ASCVD cases were identified. A doseresponse relationship was observed between relative amplitude and ASCVD risk (P for trend<0.001). The adjusted hazard ratio, for the highest vs the lowest quintile of relative amplitude, was 1.54 (95 % confidence interval: 1.31, 1.79). Further, we found significant association of lower relative amplitude with ASI and cIMT. The onset timing of M10 at <= 06:00, 09:00, 10:00, or >= 11:00, as opposed to the reference time of 07:00, was associated with higher ASCVD risk. Conclusions: Low rest-activity rhythm amplitude was associated with a higher risk of ASCVD. Rest-activity rhythm amplitude may provide a method to identify individuals at risk of ASCVD in public health and clinical practice.

Abstract:BACKGROUND Optimal chemotherapeutic regimen in thymic carcinoma remains uncertain and the efficacy of second line chemotherapy has not been established either. PATIENTS AND METHODS We retrospectively evaluated the efficacy of an irinotecan plus cisplatin or carboplatin (IP) regimen as a salvage treatment for patients with unresectable thymic carcinoma that progressed after cisplatin, doxorubicin, vincristine and cyclophosphamide (ADOC) chemotherapy. Seven patients with histologically confirmed thymic carcinoma that was resistant to or who had relapsed after initial chemotherapy with ADOC were treated with IP. The treatment consisted of irinotecan (CPT-11, 60 mg/m2, days 1, 8 and 15) and cisplatin (80 mg/m2, day 1) or carboplatin (AUC 4) intravenously every 4 weeks, for at least 2 cycles. RESULT Two patients achieved partial responses. Although another two patients showed a significant reduction of the primary thoracic lesion, the appearance of a new lesion was found in one and a metastatic lesion was unchanged in the other. Neutropenia over grade 3 was observed in all patients but none of the patients developed serious infections. There were no severe non-hematological toxicities, including diarrhea. CONCLUSION We conclude that salvage chemotherapy may be useful in certain patients with thymic carcinoma and irinotecan may be a novel and alternative agent for relapsed thymic carcinoma.

Abstract:Objective This study examined the association between physical activity patterns and abdominal and general adiposity. Methods Data were extracted among 20‐ to 59‐year‐old participants in the National Health and Nutrition Examination Survey (NHANES) from 2011 to 2018. Abdominal and general adiposity was assessed by dual‐energy x‐ray absorptiometry (DXA) and anthropometric measures. DXA‐measured indicators were further normalized into z scores. Physical activity levels were collected by questionnaire and classified as inactive, "weekend warrior" (WW), and regularly active (RA). Survey linear regression models were used to assess associations between physical activity patterns and adiposity indicators. Results Among 9629 participants, 772 (8.2%) reported the WW pattern and 3277 (36.9%) reported the RA pattern. Compared with inactive, both WW and RA had lower DXA‐measured abdominal adiposity (WW: β: −0.24, 95% CI: −0.38 to −0.10; RA: −0.18, 95% CI: −0.29 to −0.07), waist circumference (WW: β: −1.94, 95% CI: −3.16 to −0.73; RA: −1.31, 95% CI: −2.32 to −0.29), whole‐body fat mass (WW: β: −0.16, 95% CI: −0.25 to −0.08; RA: −0.11, 95% CI: −0.18 to −0.04), and BMI (WW: β: −0.78, 95% CI: −1.27 to −0.28; RA: −0.47, 95% CI: −0.89 to −0.04). Conclusions The WW pattern was associated with similarly lower abdominal and general adiposity to the RA pattern versus the inactive pattern.

Abstract:Abstract Introduction Diet, sleep, and rest-activity rhythms (RAR) influence cardiometabolic health but the relationship among these behaviors is less clear. Evidence of associations between sleep and diet have focused on self-reported sleep duration and quality, and have rarely investigated sleep timing, regularity, and RAR metrics. This study assessed cross-sectional associations between sleep and RAR regularity and timing with overall diet quality. Methods Multi-ethnic Study of Atherosclerosis (MESA) participants who completed validated sleep and food frequency questionnaires and 7-day actigraphy monitoring (Actiwatch Spectrum) between 2010-2013 were eligible for study inclusion. Overall diet quality was measured with the Alternate Healthy Eating Index (AHEI) 2010 (range: 0-110, higher indicates healthier diet). Actigraphy records were processed using Actiware-Sleep (v5.59) software and manually annotated using a sleep diary. Sleep timing was measured with average sleep midpoint, and sleep regularity measures included within-individual standard deviation (SD) of daily sleep duration and onset. Parametric (extended cosine) and nonparametric 24-hour RAR measures were derived from 1-minute epoch-level activity counts. Associations between sleep and RAR metrics with AHEI were tested with multivariable linear regression adjusting for total energy intake, sociodemographic and lifestyle factors including smoking, total activity, and depressive symptoms. Results This study included 1828 participants (mean [SD]: age: 68.5 [9] years, AHEI: 59 [10.9]; 46% male). Later sleep midpoint and L5-time (start time of the 5 least active hours) were associated with lower AHEI (0.61% and 0.77% lower AHEI per SD later midpoint [p=0.005] and L5-time [p=0.001], respectively). Irregular sleep timing (sleep onset-SD >60 minutes [vs. less]) was associated with 1.1% lower AHEI (p=0.014). Rhythm robustness, measured by the pseudo-F (parametric RAR) and relative amplitude (RA, non-parametric RAR) was associated with higher AHEI (0.49% and 0.48% higher AHEI per SD increase in pseudo-F [p=0.031] and RA [p=0.036], respectively). The associations between later sleep midpoint and L5-time with lower AHEI were robust to false discovery rate (FDR)-corrected significance thresholds. Conclusion Actigraphy-based measures of later sleep timing, irregular sleep, and weakened rest-activity rhythms were associated with lower/worse diet quality in an older, multi-ethnic sample, suggesting their potential utility in multi-component lifestyle interventions. Abstract:Abstract Introduction Abnormal sleep duration is an established risk factor for obesity. However, the potential adverse effects of sleep's nightly variability, including fluctuations in duration, remain less explored. Irregular sleep duration may increase circadian disruption and subsequently contribute to metabolic alterations, behavioral and mood changes, and obesity development. Methods From 2013-2015, a subsample of the UK Biobank (UKB) participants wore accelerometers for a week. Our longitudinal study included 8,438 participants (mean age: 64 years) with ≥5 days of accelerometer data and repeated weight measurements who had BMI< 30 kg/m2 at baseline. Irregular sleep duration was assessed by the within-person standard deviation (SD) of 7-night accelerometer-measured sleep duration. Weight was objectively measured at UKB center visits or extracted from UK general practice records at two timepoints. The baseline measurement was taken in a period spanning from two years before to one year after the accelerometer study. The follow-up measurement was obtained on average 3.6 years after the baseline measurement (interquartile range: 1.8-5.0) and at least 6 months after the accelerometer study. Poisson regression with robust error variance was used to estimate relative risk (RR) for incident obesity (BMI≥30 kg/m2) according to sleep duration-SD categories. Results A total of 419 participants developed obesity at the follow-up measurement. Compared with participants with a sleep duration-SD < 30 minutes, the RR (95% CI) for incident obesity was 1.22 (0.86, 1.73) for 31-45 minutes, 1.60 (1.14, 2.26) for 46-60 minutes, and 2.01 (1.45, 2.80) for >60 minutes (p-trend=0.0024), after adjusting for age, sex, race, and follow-up period. Additional adjustment for lifestyle (smoking, physical activity, diet, and alcohol use), co-morbidities (dyslipidemia, hypertension, and depression), and other sleep-related factors including average sleep duration resulted in a weaker but statistically significant association (RR comparing >60 versus < 30 min: 1.73; 95% CI: 1.24, 2.41). The association was similar in men and women (P-interaction in the fully-adjusted model=0.84). Conclusion Irregular sleep duration was associated with higher risk of incident obesity in older adults, independent of average sleep duration and other obesogenic behaviors. Abstract:BACKGROUND:Physical activity and sleep are established modifiable lifestyle factors, but the optimal time of the day of these behaviours for health is unknown. This study examined the independent and joint associations of diurnal patterns of physical activity and sleep with all-cause mortality. METHODS:This prospective cohort study included 6,673 participants who have attended the accelerometer assessment in the 2011-2014 National Health and Nutrition Examination Surveys (NHANES). Diurnal patterns of accelerometer-measured physical activity and sleep were identified using K-means clustering analysis. All-cause mortality was ascertained from the accelerometer measurement to December 31, 2019 (median follow-up 6.8 years). Survey-weighted Cox proportional hazard models were performed to estimate the independent and joint associations of diurnal patterns of physical activity and sleep with all-cause mortality. RESULTS:Diurnal patterns identified were: early-morning (32.4%), midday (42.5%), and late-afternoon (25.1%) for physical activity; and irregular sleep (37.4%), morning lark (33.6%), and night owl (29.0%) for sleep. After adjusting for volume of physical activity, sleep duration and other potential covariates, the early-morning physical activity pattern (hazard ratio 1.36, 95% confidence interval 1.13-1.64) and irregular sleep pattern (1.42, 1.01-1.99) were independently associated with higher risk of all-cause mortality, compared with midday physical activity and morning lark sleep patterns, respectively. In addition, participants with the combined pattern of early-morning physical activity and irregular sleep had higher risk of all-cause mortality compared to those with midday physical activity combined with a morning lark sleep pattern (1.92, 1.33-2.78). Several sociodemographic differences were observed in the strength of these associations. CONCLUSIONS:Wearable activity-rest monitoring data showed that peak physical activity in the early morning and irregular sleep diurnal patterns are associated with increased mortality risk, and the combination of these patterns further exaggerated the risk. Public health program should acknowledge that the diurnal patterns of physical activity and sleep, in addition to their duration and frequency, may play a crucial role in lifestyle-based health promotion and management strategies.
Association between rest-activity rhythm and diabetic retinopathy among US middle-age and older diabetic adults

Abstract:Background The disruption of circadian rhythm has been reported to aggravate the progression of diabetic retinopathy (DR). Rest-activity rhythm (RAR) is a widely used method for measuring individual circadian time influencing behavior. In this study, we sought to explore the potential association between RAR and the risk of DR. Methods Diabetic participants aged over 40 from 2011-2014 National Health and Nutrition Examination Survey (NHANES) were enrolled. Data from the wearable device ActiGraph GT3X was used to generate RAR metrics, including interdaily stability (IS), intradaily variability (IV), most active 10-hour period (M10), least active 5-hour period (L5), and Relative amplitude (RA). Weighted multivariable logistic regression analysis and restricted cubic spline analysis were conducted to examine the association between RAR metrics and DR risk. Sensitivity analysis was also conducted to examine the robustness of the findings. An unsupervised K-means clustering analysis was conducted to identify patterns in IV and M10. Results A total of 1,096 diabetic participants were enrolled, with a DR prevalence of 20.53%. The mean age of participants was 62.3 years, with 49.57% being male. After adjusting covariates, IV was positively associated with DR (β: 3.527, 95%CI: 1.371-9.073). Compared with the lowest quintile of IV, the highest quintile of IV had 136% higher odds of DR. In contrast, M10 was negatively associated with DR (β: 0.902, 95%CI: 0.828-0.982), with participants in the highest M10 quintile showing 48.8% lower odds of DR. Restricted cubic spline analysis confirmed that these associations were linear. Meanwhile, sensitivity analysis confirmed the robustness. K-means clustering identified three distinct clusters, with participants in Cluster C (high-IV, low-M10) had a significantly higher risk of DR comparing with Cluster A (low-IV, high-M10). Conclusion A more fragmented rhythm and lower peak activity level might be associated with an increased risk of DR. These findings indicate that maintaining a more rhythmic sleep-activity behavior might mitigate the development of DR. Further research is necessary to establish causality and understand the underlying mechanisms, and focus on whether interventions designed to enhance daily rhythm stability and increase diurnal activity level can effectively mitigate the risk of progression of DR.

Abstract:Background: This study aimed to investigate the relationships between adiposity and circadian rhythm and compare the measurement of circadian rhythm using both actigraphy and a smartphone app that tracks human-smartphone interactions. Objective: We hypothesized that the app-based measurement may provide more comprehensive information, including light-sensitive melatonin secretion and social rhythm, and have stronger correlations with adiposity indicators. Methods: We enrolled a total of 78 participants (mean age 41.5, SD 9.9 years; 46/78, 59% women) from both an obesity outpatient clinic and a workplace health promotion program. All participants (n=29 with obesity, n=16 overweight, and n=33 controls) were required to wear a wrist actigraphy device and install the Rhythm app for a minimum of 4 weeks, contributing to a total of 2182 person-days of data collection. The Rhythm app estimates sleep and circadian rhythm indicators by tracking human-smartphone interactions, which correspond to actigraphy. We examined the correlations between adiposity indices and sleep and circadian rhythm indicators, including sleep time, chronotype, and regularity of circadian rhythm, while controlling for physical activity level, age, and gender. Results: Sleep onset and wake time measurements did not differ significantly between the app and actigraphy; however, wake after sleep onset was longer (13.5, SD 19.5 minutes) with the app, resulting in a longer actigraphy-measured total sleep time (TST) of 20.2 (SD 66.7) minutes. The obesity group had a significantly longer TST with both methods. App-measured circadian rhythm indicators were significantly lower than their actigraphy-measured counterparts. The obesity group had significantly lower interdaily stability (IS) than the control group with both methods. The multivariable-adjusted model revealed a negative correlation between BMI and app-measured IS ( P =.007). Body fat percentage (BF%) and visceral adipose tissue area (VAT) showed significant correlations with both app-measured IS and actigraphy-measured IS. The app-measured midpoint of sleep showed a positive correlation with both BF% and VAT. Actigraphy-measured TST exhibited a positive correlation with BMI, VAT, and BF%, while no significant correlation was found between app-measured TST and either BMI, VAT, or BF%. Conclusions: Our findings suggest that IS is strongly correlated with various adiposity indicators. Further exploration of the role of circadian rhythm, particularly measured through human-smartphone interactions, in obesity prevention could be warranted.

Abstract:Background: Adolescents with disrupted rest-activity rhythms (RAR) including shorter sleep duration, later sleep timing and low physical activity levels have higher risk for mental and behavioral problems. However, it remains unclear whether the same associations can be observed for within-subject changes in RAR. Methods: Our longitudinal investigation on RAR used Fitbit data from the Adolescent Brain Cognitive Development (ABCD) Study at the 2-year (FL2: aged 10-13 years) and 4-year follow-up (FL4: aged 13-16 years). 963 youths had good-quality Fitbit data at both time points. In this study we examined changes in RAR from FL2 to FL4, their environmental and demographic contributors as well as brain and behavioral correlates. Results: From FL2 to FL4, adolescents showed decreases in sleep duration and physical activity as well as delayed sleep timing (Cohen's d .44-.75). The contributions of environmental and demographic factors to RAR changes were greatest to sleep timing (explained 10% variance) and least to sleep duration (explained 1% variance). Delays in sleep timing had stronger correlations with behavioral problems including greater impulsivity and poor academic performance than reductions in sleep duration or physical activity. Additionally, the various brain measures differed in their sensitivity to RAR changes. Reductions in sleep duration were associated with decreased brain functional connectivity between subcortical regions and sensorimotor and cingulo-opercular networks and with enhanced functional connectivity between sensorimotor, visual and auditory networks. Delays in sleep timing were mainly associated with grey matter changes in subcortical regions. Conclusions: The current findings corroborate the role of sleep and physical activity in adolescent's brain neurodevelopment and behavior problems. RAR might serve as biomarkers for monitoring behavioral problems in adolescents and to serve as potential therapeutic targets for mental disorders.

Abstract:Irregular sleep duration may disrupt circadian rhythms and contribute to metabolic, behavioral, and mood changes, potentially increasing the risk for obesity. However, quantitative data on the relationship between sleep duration irregularity and weight change are lacking. In this prospective study, we analyzed data from 10,572 participants (mean age: 63 years) in the UK Biobank who wore accelerometers for a week between 2013-2015 and had two body mass index (BMI; kg/m ) measurements on average 2.5 years apart. Irregular sleep duration was assessed by the within-person standard deviation (SD) of 7-night accelerometer-measured sleep duration. Participants with sleep duration SD >60 minutes versus ≤30 minutes had 0.24 kg/m (95% CI: 0.08, 0.40) higher BMI change (kg/m ), standardized to three-year intervals, and 80% (95% CI: 1.28, 2.52) higher risk for incident obesity, after adjusting for sociodemographic factors, shift work, and baseline BMI or follow-up period (p-trend<0.02 for both). These associations remained consistent after adjusting for lifestyle, comorbidities, and other sleep factors, including sleep duration. Age, sex, baseline BMI, and genetic predisposition to higher BMI (measured with a polygenic risk score) did not appear to modify the association. Since irregular sleep duration is common, trials of interventions targeting sleep irregularity might lead to new public health strategies that tackle obesity.

Abstract:Abstract Introduction Circadian rest-activity rhythms (RARs), a behavioral manifestation of circadian rhythms, reflect physical activity and sleep patterns over the 24-hour day. With aging, disrupted RARs may be associated with higher cardiometabolic risk, fall risk and cognitive decline. Physical performance also declines with advancing age, leading to disability and increased risk of mortality. However, the associations between RARs and physical performance are understudied. Methods In the Osteoporotic Fractures in Men (MrOS) study, 2,924 men (age=76.3+/-5.5 years, 90% white) completed sleep ancillary study (baseline), had valid wrist-worn actigraphy data (mean=5 days) and physical performance measures. Physical performance measures included: 6-meter gait speed, grip strength, and chair stand pace assessed at baseline and up to two follow-up visits over 3.4+/-3.1 years. Using a shape-naive technique, functional principal components analysis (fPCA), we examined baseline patterns of RARs over several days and evaluated cross-sectional and longitudinal associations of these patterns with each physical performance measure separately using mixed effect models. Models were adjusted for age, race, education, body mass index, smoking, alcohol use, caffeine consumption, multimorbidity, medication use and depressive symptomology. Results At baseline, gait speed was 1.14+/-0.23 m/s, grip strength was 40.8+/-8.4 kg, and chair stand pace was 4.4+/-1.7 stands/10 sec. Four fPCA components (explaining 88% variance) were identified with higher eigenvalues meaning: 1) high overall amplitude (50%); 2) later rise and bed times (23%); 3) longer, biphasic activity patterns (9%); and 4) evening peaks in activity (6%). Cross-sectionally, higher fPCA1 was associated with faster gait speed and chair stand pace (p< 0.05); higher fPCA3 was associated with slower gait speed, worse grip strength, and slower chair stand pace (p< 0.05). Longitudinally, higher fPCA1 was associated slower decline in gait speed and in chair stand pace (p< 0.05); higher fPCA2 was associated with accelerated decline in chair stand pace (p< 0.05); higher fPCA3 and fPCA4 were both associated with accelerated decline in gait speed (p< 0.05). No longitudinal association was found with grip strength. Conclusion Specific disruptions in RARs are associated with poor physical performance and its subsequent decline in older men. Abstract:Abstract Background Rest–activity rhythms are directly related to health risks, but there are limited objective methods to assess them. This study aimed to investigate the relationship between rest–activity rhythms and cardiorespiratory fitness (CRF) in middle-aged workers. Methods Peak oxygen uptake was measured on a treadmill to assess CRF in 254 middle-aged workers who were divided into low, medium, and high-CRF groups based on tertiles. Participants were asked to wear an accelerometer (activPAL) on their thighs for 1 week, and the logarithmically transformed acceleration data were used for the analysis of a 24-hour rest–activity rhythm. Sex, age, body mass index, occupation, smoking status, and alcohol consumption were used as covariates in Model 1, with Model 2 also including walking count on non-workdays. Repeated measures analysis of variance was used to compare time course of rest–activity rhythms changes on workdays between groups, and post-hoc tests were conducted using Bonferroni’s correlation. Results Higher CRF correlated with increased physical activity. In model 1, higher CRF showed improved interdaily stability, but the significant difference disappeared in model 2 after adjusting for non-workday walking counts. A time-course group comparison showed that the high group had significantly higher activity levels than those of the low group from 6:00 to 8:59 and 17:00 to 17:59 and the medium group from 6:00 to 7:59 and 19:00 to 19:59. Conclusions Workers who have better rest–activity rhythms and engage in higher levels of physical activity on workdays tend to have higher CRF levels. Regular daily routines, influenced by physical activity during holidays, can positively impact cardiopulmonary endurance.

Abstract:Study objectives: People living with schizophrenia (PLWS) have increased physical comorbidities and premature mortality which may be linked to dysregulated rest-activity rhythms (RARs). This study aimed to compare RARs between PLWS and nonpsychiatric comparison participants (NCs) and to examine the relationships of RARs with age, sleep, metabolic, and physical health outcomes and, among PLWS, relationships of RARs with illness-related factors. Methods: The study sample included 26 PLWS and 36 NCs, assessed with wrist-worn actigraphy to compute RAR variables and general sleep variables. Participants completed assessments for clinical symptoms, physical health, sleep quality, medication use, and assays for fasting glycosylated hemoglobin (hemoglobin A1c) levels. We examined group differences in RAR and sleep variables, relationships of RAR variables with metabolic and physical health measures, and, among PLWS, relationships between RAR variables and illness-related measures. Results: PLWS had significantly shorter active periods, lower relative amplitude, and lower mean activity during their most active 10 hours compared to the NCs (Cohen's d = 0.79, 0.58, and 0.62, respectively). PLWS had poorer sleep quality, greater mean percent sleep, less wake after sleep onset, and higher total sleep time variability compared to NCs. PLWS had higher rates of antidepressant, anxiolytic, and antipsychotic medication use compared to NCs, which may have impacted sleep quality and objective sleep measures. Across both groups, more fragmented and variable RARs were associated with higher HbA1c levels (ηp2 = .10) and worse physical health (ηp2 = .21). Among PLWS, RARs were correlated with total sleep time (rs = .789, P < .01) and percent sleep (rs = .509, P < .05), but not with age, sleep quality, or other illness-related factors. Conclusions: RARs provide unique information about sleep and activity for PLWS and have the potential for targeted interventions to improve metabolic health and mortality. Abstract:Abstract Introduction Misalignment between behaviors (i.e., eating/sleeping) and the circadian timing system promotes increased risk of diabetes and obesity. Though circadian disruption has been shown to be associated with higher body fat percentage (BF%) and abdominal fat distribution among healthy weight males, but not females, these relationships may be altered in individuals with higher body mass index (BMI) given the complex interaction between obesity and circadian metabolic mechanisms. We therefore tested the association between circadian alignment and body composition in individuals with overweight/obesity. Methods BF% and android-gynoid (AG) percent fat ratio was assessed using dual energy x-ray absorptiometry in 28 participants (15 female). Dim-light melatonin onset (DLMO; 3pg/ml threshold) was used as a marker of circadian phase, as determined from saliva samples across an ~8h evening in-laboratory stay in dim-light (< 5 lux). Circadian alignment was determined via phase angle of entrainment, defined as the time difference between DLMO and diary-determined sleep onset over 7-days. Differences between females and males were assessed using independent t-tests and the relationships between phase angle of entrainment and body composition metrics were assessed using Pearson correlation analyses. Results There were no differences between groups in age (mean(SD), females vs. males, respectively; 34.2(8.2) y vs. 37.2(8.3) y; p=0.35), BMI (34.9(5.9) kg/m2 vs. 31.0(6.5) kg/m2; p=0.11), DLMO timing (19:35(1:15) vs. 19:37(0:57); p=0.95), or phase angle of entrainment (2.9(1.7) h vs. 3.0(1.1) h; p=0.77). Females had higher BF% than males (45(6) % vs. 34(8) %; p< 0.01) and lower AG ratios than males (1.06(0.13) vs. 1.33(0.15); p< 0.01). Phase angle of entrainment [i.e., later sleep] was negatively correlated with AG ratio only in males (r= -0.6; p=0.03) while phase angle of entrainment had a negative correlation with BF% only in females (r= -0.65; p=0.01). Conclusion Alignment of the circadian system (i.e., smaller phase angle of entrainment) may be a contributing factor to poorer body composition and, particularly amongst males, metabolically unhealthy body fat distribution in individuals with overweight/obesity. The mechanistic role of circadian timing on cardiometabolic disease risk may be differentially affected by sex and obesity, which could have implications for targeted interventions to improve health outcomes.
