Abstract:This study was designed to evaluate and compare the usefulness of clear aligners and conventional appliances on Oral Health-Related Quality of Life (OHRQoL) in pediatric population. Emphasis was placed on the relative benefits and implications of employing clear aligners owing to their escalating prevalence and acceptability. The study participants were divided into four groups: Clear Aligner Group (CAG), Conventional Appliance Group (ConAG), Malocclusion Control Group (MCG), and Normal Control Group (NCG). Parameters including sociodemographic indicators and daily routines were assessed. OHRQoL was evaluated via the Child Perceptions Questionnaire (CPQ). Psychological conditions were assessed through the Depression, Anxiety and Stress Scale (DASS). Statistical differences were found between the four groups regarding CPQ subscales and total scores (p < 0.05). CAG was better than ConAG (p < 0.05) regarding the scores of functional limitations, emotional and social well-being, and total score, however no significant difference was discovered in the oral symptoms scores (p = 0.62). Moreover, all the treatment groups had worse OHRQoL compared to NCG (p < 0.05). Malocclusions and their treatments did not increase the psychological distress as per the DASS results. A novel correlation between the excessive tooth brushing and reduced OHRQoL was also observed (p < 0.05). The study herein emphasized the benefits of clear aligners in children and adolescents with OHRQoL. It was highlighted that the clear aligners had potential and were preferred for the adolescent orthodontic treatment.
Efficacy of Clear Aligners on Mandibular Molar Distalization: A Retrospective Study

Abstract:Background: Clear aligners (CAs) can distalize mandibular molars. The present study aimed to study the efficiency and influencing the factors of CAs for mandibular molar distalization and to provide guidance for clinical design. Materials and Methods: The present study evaluated 32 adult orthodontic patients who needed mandibular molar distalization and were treated with CAs between September 2018 and September 2020 at the school of medicine. The cone-beam computed tomography images of T0 (before orthodontic treatment) and T1 (after mandibular molars were fully distalized) were collected, and the actual movements of mandibular molars were measured and compared with the expected movements. The efficiency of mandibular molar distalization during treatment was comprehensively evaluated, and the relevant factors were analyzed. Results: For mandibular first molar distalization, the average efficiency of the crown was 67.19% 9swn. 13%, and that of the root was 37.87% 7stn. 72%. There was a significant difference between the achieved movement amount and the expected amount (P < 0.05). For mandibular second molars, the average efficiency of the crown was 58.47% 7swn. 07%, and that of the root was 57.03% 3stn. 48%. There was a statistically significant difference between the achieved movement amount and the expected amount (P < 0.05). There was a significant negative correlation between the expected movement and the efficiency of the crown. Conclusions: CAs can achieve mandibular molar distalization, and the movement pattern of molars is mainly a tipping movement. To accomplish bodily movement, overcorrection should be fully considered. In addition, it is suggested that close attention should be given to observing root movement during molar distalization.
Clinical effectiveness of regenerative periodontal surgery and orthodontic tooth movement with clear aligners in stage IV periodontitis: a case series

Abstract:Objectives: To evaluate the clinical effectiveness of regenerative treatment of intrabony defects in combination with consecutive orthodontic therapy with clear aligners in stage IV (type 2) periodontitis. Method and materials: Ten patients with a total of 103 intrabony defects were analyzed after regenerative surgery using collagen-deproteinized bovine bone mineral with or without collagen membrane or enamel matrix derivative followed by orthodontic therapy with clear aligners. Changes in radiographic bone level and probing pocket depths were evaluated after 1 year (T1) and at final splinting (T2) after orthodontic tooth movement. Results: Mean radiographic bone level gain was significant, with 2.13 ± 1.64 mm at T1 and 3.02 ± 2.00 mm at T2. Mean probing pocket depth was significantly reduced from 5.40 ± 1.80 mm at baseline to 3.78 ± 1.73 mm at T1, and remained stable with 3.73 ± 1.70 mm at T2. Pocket closure (≤ 4 mm probing pocket depth) was accomplished in 76% of all defects. Tooth loss amounted to 2.9%. Conclusion: Within the limitations of the retrospective study design, the findings suggest that the interdisciplinary treatment of periodontitis stage IV by regenerative periodontal surgery and consecutive orthodontic therapy with clear aligners can lead to favorable results.

Abstract:Improper diagnosis and design, misusing orthodontic technique, relapse after orthodontic treatment, and poor patient compliance may lead to treatment failure, requiring orthodontic retreatment. While in such cases, patients usually have already had their teeth extracted, or are with periodontal, joint, and other dental problems, which often require multidisciplinary treatment. The retreatment of orthodontic patients is difficult, and the treatment options are limited. The characteristics of clear aligner treatment, such as comfortable and beautiful appearance, high degree of digital precision, and personalized tooth arrangement to simulate the final outcome, make it popular in the orthodontic retreatment. This article discusses and summarizes the details of orthodontic design, case monitoring, and the application of clear aligner in orthodontic retreatment of failed cases. Before starting treatment, the difficulty assessment tool (clear aligner treatment complexity assessment tool) can be used to evaluate the difficulty of clear aligner treatment. Orthodontists are recommended to treat orthodontic patients with clear aligners within their ability to avoid treatment failure. The key to the success of the extraction case treatment with clear aligner is to make the correct diagnosis and select the right cases. In the treatment planning, attention should be paid to the restoration of anterior tooth torque, making good use of molar distalization to obtain the space, vertical control, and improving posterior tooth relationship. Proper selection of cases and reasonable design in clear aligner treatment are expected to obtain tooth alignment, good intercuspation, normal overbite and overjet, periodontal health, parallel roots, and to achieve the goal of aesthetic, functional, stable, and healthy orthodontic treatment.
Sequential soft- and hard-tissue augmentation after clear aligner-mediated adjustment of traumatic occlusion: A case report

Abstract:Background: Oral health and esthetic concerns have increasingly motivated adults with periodontitis to seek orthodontic care. Patients with periodontitis often have other dental complications that can make treatment more challenging or less likely to be successful. The predictability of multidisciplinary regenerative and orthodontic treatment approaches in a patient with periodontitis, thin periodontal phenotype, and anterior crossbite is described. Case description: A 35-year-old man was designated as having a high risk of experiencing soft-tissue recession owing to his thin periodontal phenotype. After the initial periodontal therapy, clear aligners were used to eliminate the anterior crossbite that was causing traumatic occlusion. In addition, a subepithelial connective tissue graft was used to alter the thin periodontal phenotype. Three months later, guided bone regeneration with corticotomy was performed to increase bone mass, and orthodontic traction was used at 2 weeks after surgery. Orthodontic treatment was continued until all the spaces had been closed. Clinical and radiographic evaluations conducted at the 6-month follow-up revealed substantial improvements in both the soft- and hard-tissue phenotypes. Practical implications: Outcomes in this case indicate that sequential soft- and hard-tissue augmentation, after eliminating traumatic occlusion via using a clear aligner, may be a valuable approach for improving soft- and hard-tissue support in patients with periodontitis.
Class III correction and enhanced periodontal health with aligner treatment in a 53-year-old patient

Abstract:In this case report, we describe the successful camouflage treatment of a 53-year-old female with dental and skeletal Class III malocclusion combined with anterior crossbite, gingival recession, and mobility of the lower incisors, using clear aligners. The treatment involved periodontal debridement followed by orthodontic treatment. The mandibular posterior teeth were distalized to correct the anterior crossbite and to establish Class I molar relationships. During treatment, the mandibular incisors were intruded, and the mandibular occlusal plane underwent a clockwise rotation due to slight extrusion of the maxillary buccal segments and distalization of the mandibular posterior teeth. Following treatment, all objectives were achieved, including resolution of the anterior crossbite, significant reduction of gingival recession and tooth mobility, and improved functional occlusion. The dental and skeletal Class III malocclusion was corrected, and the marginal alveolar bone dehiscence was significantly reduced. The results remained stable over a 3-year retention period, with enhanced molar intercuspation and gingival growth progression. This case adds to the evidence supporting the adaptability and effectiveness of clear aligners in treating orthodontic patients with compromised periodontium. The treatment outcomes support that orthodontic treatment using clear aligners, combined with periodontal monitoring, can assist in managing alveolar bone defects, gingival recession, and tooth mobility.
Effects of different intrusion patterns during anterior teeth retraction using clear aligners in extraction cases: an iterative finite element analysis

Abstract:Background: Overtreatment design of clear aligner treatment (CAT) in extraction cases is currently primarily based on the clinical experience of orthodontists and is not supported by robust evidence on the underlying biomechanics. This study aimed to investigate the biomechanical effects of overtreatment strategies involving different maxillary anterior teeth intrusion patterns during anterior teeth retraction by CAT in extraction cases. Materials and methods: A finite element model of the maxillary dentition with the first premolar extracted was constructed. A loading method of clear aligners (CAs) based on the initial state field was proposed. The iterative method was used to simulate the long-term orthodontic tooth movement under the mechanical load exerted by the CAs. Three groups of CAs were utilized for anterior teeth retraction (G0: control group; G1: incisors intrusion group; G2: anterior teeth intrusion group). Tooth displacement and occlusal plane rotation tendency were analyzed. Results: In G0, CAT caused lingual tipping and extrusion of the incisors, distal tipping and extrusion of the canines, mesial tipping, and intrusion of the posterior teeth. In G1, the incisors showed minimal extrusion, whereas the canines showed increased extrusion and distal tipping tendency. G2 showed the smallest degree of posterior occlusal plane angle rotation, while the inclination tendency of the canines and second premolars decreased. Conclusion: 1. In CAT, tooth displacement tendency may change with increased wear time. 2. During anterior teeth retraction, the incisor intrusion pattern can provide effective vertical control for the lateral incisors but has little effect on the central incisors. Anterior teeth intrusion patterns can alleviate the inclination of canines and second premolars, resulting in partial relief of the roller-coaster effect.

Abstract:Background To evaluate the incidence and severity of open gingival embrasures (OGEs) in adult patients treated with clear aligners and fixed appliances. Methods Two hundred non-extraction adult subjects with less than 5 mm of crowding (mean age, 24.6 ± 3.8 years) were enrolled in this retrospective study. The subjects were divided into the clear aligner ( n = 100) and fixed appliance group ( n = 100). The intraoral photographs were utilized to determine the incidence of OGEs in the upper arch between maxillary central incisors, as well as the lower arch between mandibular central incisors. Crown overlap, crown shape, posttreatment root angulation, the distance from the interproximal contact point (ICP) to the alveolar bone crest (ABC) after treatment and interproximal enamel reduction (IPR) were determined in the two groups. Results The incidence of OGEs between maxillary and mandibular central incisors after orthodontic treatment was 35.0% and 38.0% in the clear aligner group, respectively, significantly higher than that (18.0% and 24.0%) in the fixed appliance group ( P < 0.05). The average area of an OGE after clear aligner treatment was larger both in the maxilla (0.16 ± 0.12mm 2 ) and mandible (0.21 ± 0.24mm 2 ) compared with that (0.05 ± 0.03mm 2 and 0.05 ± 0.06mm 2 ) after fixed appliance treatment ( P < 0.05). No difference was found regarding pretreatment crown overlap, crown shape, treatment duration, posttreatment root angulation, amount and distribution of IPR and the distance from ICP to ABC. Conclusions The incidence and severity of OGEs were higher in adults treated with clear aligners. Clinicians should be aware of the risk of OGEs during treatment with clear aligners.
Periodontal Effect of Periodontally Accelerated Osteogenic Orthodontics in Skeletal Angle Class III: A Nonrandomized, Controlled Trial.

Abstract:The objective of this study was to evaluate the effect of periodontally accelerated osteogenic orthodontics (PAOO) on gingivae and alveolar bone by analysis of clinical and cone beam computed tomography (CBCT) parameters in the treatment of 20 skeletal Class III patients. The patients included in this study were divided into test and control groups. Periodontal parameters such as probing depth (PD), gingival recession (GR), keratinized gingival width, and alveolar bone thickness of CBCT scans were measured and recorded preoperation (T0) and at 6 months postoperative (T1). The difference in PD from T0 to T1 between the two groups was not statistically significant (0.01 ± 0.46 mm vs 0.22 ± 0.65 mm, respectively; P > .05). No significant difference in GR was observed from T0 to T1 between the two groups (0.03 ± 0.26 mm vs -0.03 ± 0.27 mm, respectively; P > .05). Alveolar bone thickness (4 mm apical to the cementoenamel junction [CEJ]) change from T0 to T1 was -0.31 ± 0.35 mm for the control group and 0.06 ± 0.69 mm for the test group (P < .05). Meanwhile, alveolar bone thickness (6 mm apical to CEJ) changes from T0 to T1 were -0.38 ± 0.54 mm and 0.10 ± 0.80 mm for the control and test groups, respectively (P < .05). It was determined that PAOO in the treatment of skeletal Class III patients is effective and safe to periodontium on the basis of clinical and CBCT parameters.

Abstract:Annealing an unsaturated sample of soft amorphous magnetic ribbon or thin film displays a characteristically pinned hysteresis loop. 1,2. In this paper narrow ribbons of a cobalt-rich nonmagnetostrictive metallic glass (dimensiobs 75mmxlrnmx35um) are studied in more detail in order to understand the characteristics of this phenomenon. Figure 1 shows a characteristic hysteresis curve obtained after annealing when the sample is exposed to a moderate magnetic field of a few Oersteds maximum. This loop can be unstable at high maximum fields where it can change to that of a normal sample with high demagnetization field. The model that was used assumed that the anisotropy induced by the anneal followed the direction of the magnetization at the time of the anneal even when the magnetization made a domain wall. This induced helical anisotropy could then trap the wall as it passed the site creating a loop as seen in Fig. 1. The model has now been confirmed by direct observation of the pinning site. Figure 2 shows an example of a direct observation of the domain wall and the pinning site, obtained with the digitally enhanced magnetooptic Kerr microscope adjusted to observe the 1ongitudina.l Kerr effect along the axis prependicular to the ribbon axis 3. A finger shaped domain can be seen on the left with a single domain on the right. Even without a domain wall on the right, a contrast appears at the pinning site indicating a "quasi wall"-a magnetization excursion with a zero angle of total rotation, induced by the local anisotropy.

Abstract:The incidence of open gingival embrasures (OGE) in patients after fixed appliance treatment is relatively high, while there are no detailed reports on patients after clear aligner therapy. Also, no clinical studies with large sample size have investigated whether interproximal enamel reduction (IPR) can actually avoid OGE. The purpose of this study was to determine the prevalence of OGE in adults after clear aligner therapy and to investigate the risk of OGE associated with IPR treatment and attachment design, focusing on the amount and distribution in mandibular anterior teeth.

Abstract:ABSTRACT Introduction: Orthodontists have been using clear aligners to treat malocclusions, and one potential effect of treatment with orthodontic aligners is the intrusion and/or resists extrusion of the posterior teeth. This “bite-block effect” is primarily anecdotal due to the frequent occurrence of posterior open bites in patients after clear aligner therapy. Objective: The purpose of this study was to compare changes promoted by clear aligners and traditional fixed appliances in cephalometric measurements of the vertical dimension and molar position in adult patients with Class I malocclusion treated with non-extraction. Methods: Pre- and post-treatment lateral cephalometric radiographs of adult patients treated with either clear aligners (n=44) or traditional fixed appliances (n=22) were selected for retrospective analysis. Eight interval measurements and one nominal measurement were evaluated: anterior overbite (OB), mandibular plane angle related to cranial base (SN_MP) and related to Frankfort (FMA), lower molar height (L6H) and upper molar height (U6H), palatal plane to mandibular plane angle (PP_MP), lower facial height (LFH), total facial height (TFH), and posterior open bite (Posterior_OB). A single evaluator traced all cephalographs, and changes in select measures of the vertical dimension were compared within and between groups. Results: OB decreased (1.15 mm) and L6H increased (0.63 mm) in the traditional fixed appliance group. Mandibular plane angles (related to cranial base and to Frankfort) increased (0.43° and 0.53°, respectively) in the clear aligner group, but just FMA showed significant difference between groups (difference of 0.53°). LFH and TFH increased (ranging from 0.52 mm to 0.80 mm) in both groups, with no differences between treatment modality. Presence of visible posterior open bite significantly increased over the course of treatment. OB, FMA and L6H exhibited an interaction between treatment stage (pre- and post-treatment) and modality (clear aligner therapy and traditional fixed appliances), but no interaction among these three variables was found. Conclusions: The evidence does not support the theory that clear aligner therapy produces better vertical dimension control than traditional fixed appliances. Traditional fixed appliance therapy slightly extruded the lower molar, and clear aligner therapy produced a slightly mandibular backward rotation.

Abstract:Abstract : We consider a class of dynamic resource allocation problems a specific example of which is the Weapon-Target Assignment problem. This problem is concerned with the optimal assignment of resources in a military engagement. These problems are, in general, NP-Complete, so our aim is to provide insight into the problem and its solution. We will provide analytical results for simple cases of the problem. We will also provide sub-optimal algorithms, together with bounds on the objective function, under certain assumptions. The battle scenario of the military engagement being modeled is as follows. The offense launches a number of weapons ( the targets) which are aimed at valuable assets of the defense. The defense has a number of defensive weapons each of which can engage at most one target. The outcome of such an engagement is stochastic. In the static scenario all weapons are fired simultaneously. In the dynamic scenario some weapons are assigned and fired and the outcomes of these engagements are observed before further assignments are made.
Effectiveness of Modified Periodontally Accelerated Osteogenic Orthodontics in Skeletal Class II Malocclusion Treated by a Camouflage Approach.

Abstract:Skeletal Class II malocclusion is a complex orofacial condition. Here, we translated the single clinical problem to a multidisciplinary approach that combined the periodontal surgery, biomaterial implantation, and orthodontics to treat this condition. This study aimed to explore the clinical effectiveness of modified periodontally accelerated osteogenic orthodontics (PAOO) in adult patients with skeletal Class II discrepancy via a camouflage orthodontic approach. This clinical trial has been registered in the Chinese Clinical Trial Registry (trial number: ChiCTR2100052638, trial URL: http://www.chictr.org.cn/showproj.aspx?proj=135827). A total of 26 adult female patients with similar skeletal Class II malocclusions and similar dental and skeletal discrepancies were enrolled. A total of 13 patients in the experimental group received modified PAOO characterized by vertical incisions in interradicular areas and random punching made by a piezoelectric device, whereas 13 patients in the control group received orthodontic treatment only. All patients underwent extraction of four premolars and orthodontic treatment with the sliding MBT straight-wire technique. Treatment courses were recorded and changes in dental and skeletal parameters were evaluated by cephalometric analysis and translated into upper digital cast for further measurements. The results showed that the durations of the alignment and leveling stage (4.51±1.53 mos versus 9.41±1.46 mos. P<0.01), space closing stage (13.56±2.57 mos versus 17.09±3.50 mos. P<0.05) and total treatment course (24.43±2.53 mos versus 31.16±4.17 mos. P<0.01) in the PAOO group were significantly shorter than in the control. The modified PAOO accelerated orthodontic tooth movement in adult extraction cases with skeletal Class II discrepancy and convex profile; the modified PAOO achieved dual goals of periodontal health and long-term stability of orthodontics. Ultimately, this multidisciplinary approach and the use of translational 3D measurements constitute powerful tools that can be used to solve complex clinical problems.

Abstract:PURPOSE Orthognathic surgery for dentofacial deformities is typically preceded and followed by orthodontic treatment. Traditionally, orthodontic hardware is secured to the dentition to allow dental movement and stabilization. Clear-aligner therapy (eg, Invisalign) provides an aesthetic alternative, consisting of a series of transparent trays. Its use has not been described in complex triple-jaw orthognathic surgery. The purpose of this study is to evaluate perioperative outcomes and 3-dimensionally quantify postoperative edema in Invisalign patients undergoing triple-jaw orthognathic procedures, comparing this to patients treated with conventional fixed appliances. The surgical approach to patients with clear-aligners is also outlined. METHODS The authors conducted a retrospective chart review and 3-dimensional morphometric study of Invisalign patients undergoing triple-jaw surgery (LeFort I osteotomy, bilateral sagittal split osteotomy, and genioplasty). An identical assessment of demographically matched patients treated with conventional fixed appliances was performed and compared with the Invisalign group. RESULTS Thirty-three patients, with a mean age of 19.99 years, were included: 13 with Invisalign and 20 with conventional fixed appliances. No significant difference was observed in operating time, concurrent extraction of teeth, fat grafting, duration of hospital stay, diet advancement, and use of narcotic analgesics between the 2 groups. Nine patients had sufficient 3-dimensional images for volumetric analysis (4 with Invisalign and 5 with conventional fixed appliances). Postoperative edema was not significantly different (P = 0.712) when comparing conventional fixed appliances (44.29 ± 23.16 cm) to Invisalign (37.36 ± 31.19 cm). CONCLUSION The present study demonstrates that complex multiple-jaw orthognathic procedures can be successfully performed in Invisalign patients. Perioperative and short-term clinical outcomes are not compromised.

Abstract:Clear aligners have become increasingly popular because of their esthetics and comfort. The authors' aim in this systematic review was to compare periodontal health in patients undergoing orthodontic treatment with clear aligners with that of those undergoing orthodontic treatment with fixed appliances.The authors systematically searched the PubMed, Web of Science, Cochrane Library, and Embase databases to collect related studies. After extracting data and assessing quality, the authors performed a meta-analysis and trial sequential analysis. The authors used the Grading of Recommendations Assessment, Development and Evaluation system to assess the quality of the evidence.The authors included 9 studies in the quantitative synthesis analysis. Clear aligners were better for periodontal health, including plaque index (mean difference [MD], -0.53; 95% confidence interval [CI], -0.85 to -0.20; P = .001), gingival index (MD, -0.27; 95% CI, -0.37 to -0.17; P < .001), and probing depth (MD, -0.35; 95% CI, -0.67 to -0.03; P = .03), than were fixed appliances. However, the trial sequential analysis outcome indicated a false-positive meta-analysis result for probing depth. The authors downgraded the level of the evidence because of the risk of bias and inconsistency.Clear aligners were better for periodontal health than fixed appliances and might be recommended for patients at high risk of developing gingivitis. However, high-quality studies still are required.

Abstract:Background: Align technology has developed greatly over past few years. Patients tended to prefer clear aligners over conventional brackets because of the superior comfort and esthetics, while the effectiveness of clear aligners was still controversial. The aim of this systematic review was to verify whether the treatment effectiveness of clear aligners was similar to the conventional fixed appliances. Methods: A comprehensive search of the Pubmed, Web of Science, Embase, Scopus, and Cochrane Central Register of Controlled Clinical Trials Register databases for studies published through to August 20, 2018 was conducted. Comparative clinical studies assessing the effectiveness of clear aligners compared with braces were included. Results: Eight papers were included in this study. Two of the included papers were randomized controlled trials and six were cohort studies. Clear aligners might not be as effective as braces in producing adequate occlusal contacts, controlling teeth torque, increasing transverse width and retention. While no statistically significant difference was found between two groups in Objective Grading System score (WMD = 8.38, 95% CI [- 0.17, 16.93]; P = 0.05). On the other hand, patients treated with clear aligners had a statistically significant shorter treatment duration than with braces (WMD = - 6.31, 95% CI [- 8.37, - 4.24]; P < 0.001). Conclusion: Both clear aligners and braces were effective in treating malocclusion. Clear aligners had advantage in segmented movement of teeth and shortened treatment duration, but were not as effective as braces in producing adequate occlusal contacts, controlling teeth torque, and retention.
Treatment Effectiveness of Clear Aligners in Correcting Complicated and Severe Malocclusion Cases Compared to Fixed Orthodontic Appliances: A Systematic Review

Abstract:This systematic review aimed to critically assess the available evidence regarding the effectiveness and efficiency of clear aligners in the comprehensive treatment of complex cases accompanied by premolars extraction. An electronic literature search by two reviewers was independently done on 27 February 2023 in the following databases without time and language limitations: Pubmed®, Scoups®, Google Scholar, Cochrane Library database, Web of Science™, and Proquest Database Open. Randomized controlled trials (RCTs) of any type, non-randomized clinical trials (CCT), cohort studies, and prospective, retrospective, and cross-sectional studies were reviewed. The risk of bias in included studies was assessed using the Risk of Bias (RoB 2.0) tool for randomized trials and the Risk of Bias in Non-randomized Studies (ROBINS-I) tool for non-randomized studies. After carefully searching the literature, six trials were included in this systematic review, three RCTs, two retrospective cohort studies, and one CCT. Two hundred eighty-three patients were included (186 females, 97 males). Three studies found that there were no differences between the clear aligners and fixed appliances when evaluations were done using the American Board of Orthodontists Objective Grading System (ABO-OGS) or the Peer Assessment Rating (PAR) index. Two studies found that there were some differences between predicted and achieved tooth movements when clear aligners were used in premolars extraction cases. Based on the included studies, the duration of treatment was shorter with fixed appliances than the clear aligners when applied to orthodontic extraction cases. Both clear aligners and fixed appliances were found effective in the orthodontic treatment of premolar extraction-based cases. Fixed appliances have the advantage of achieving better buccolingual inclination and occlusal contacts in a shorter treatment duration. Treatment with clear aligners might be associated with differences between predicted and achieved tooth movements. Therefore, the characteristics of these techniques should be considered when making a treatment decision.
Comparison Between the Designed and Achieved Mesiodistal Angulation of Maxillary Canines and Posterior Teeth and Influencing Factors: First Premolar Extraction Treatment with Clear Aligners

Abstract:Introduction: This study aimed to compare the designed and achieved mesiodistal angulation of maxillary canines and posterior teeth (MCPT) for first premolar extraction with clear aligner treatment and identify the main influencing factors for preventing MCPT tipping toward the extraction space. Methods: A total of 21 adults with first premolar extraction were recruited. The designed and achieved tooth movement of MCPT was measured by superimposing their respective pretreatment and posttreatment cone-beam computed tomography images and compared with the designed tooth movement in ClinCheck using the paired t test and scatter plot analysis. Influencing factors, including dental arch length change, canine distalization, and initial mesiodistal angulation, were analyzed using the linear mixed-effect model. Results: Designed distal crown tipping (second premolar, 10.73 +/- 3.22 degrees; first molar, 9.83 +/- 3.60 degrees; second molar, 7.18 +/- 2.36 degrees) significantly increased the distal inclination of the second premolar (2.50 degrees +/- 5.15 degrees; P<0.001), first molar (1.07 degrees +/- 4.14 degrees; P<0.001), and second (0.70 degrees +/- 3.78 degrees; P <0.001). Furthermore, mesial tipping (8.59 degrees +/- 6.03 degrees; P <0.001) achieved appropriate distal crown tipping of canines (-6.43 degrees +/- 5.04 degrees; P<0.001). The implemented preliminary formulas showed that shortening of the dental arch length, the distance of canine distalization, and initial mesiodistal angulation were closely related to the antitipping design. Conclusions: Designed distal crown tipping of posterior teeth and mesial crown tipping of canines might prevent unwanted crown tipping toward the extraction space during space closure. The proposed preliminary formula could guide antitipping designs in clear aligner treatment.

Abstract:Hematological parameters were studied in normotensive and spontaneous hypertensive rats along with those treated with chronic subcutaneous injections of CCl4. The normotensive animals treated with CCl4 demonstrated an erythrocytosis with an increase in the hematocrit levels. A lymphocytosis was seen with neutropenia in both CCl4 treated and untreated normotensive groups. The SHR animals compared to their normotensive counterparts had a lesser degree of lymphocytosis with an increase in the number of neutrophils. There was no blood pressure change in the normotensive CCl4 treated group, however a significant blood pressure difference was observed in the SHR group after CCl4 treatment.
