Abstract:Objective Due to the coronavirus pandemic and crisis, psychotherapists around the world were forced to switch to video‐ or tele‐based treatments overnight. To date, only a few studies on the effectiveness of video‐based psychodynamic psychotherapy via the Internet exist. Therefore, the goal of the present study was to examine symptom improvement, therapeutic relationship, nonverbal synchrony processes, and intersession processes within a systematic single case design and compare face‐to‐face to video‐based approaches in long‐term psychodynamic‐oriented psychotherapy. Methods We examined 85 sessions of a client with major depression whose psychodynamic psychotherapy changed from a face‐to‐face setting to a video‐based setting. Video recordings were analyzed using motion energy analysis, and nonverbal synchrony was computed using a surrogate synchrony approach. Time series analyses were performed to analyze changes in symptom severity, therapeutic relationship, and intersession processes. Results The results showed that symptom severity improved descriptively, but not significantly, across the entire course of psychotherapy. There were significant differences, however, in the therapeutic relationship, intersession experiences, and synchronous behavior between the face‐to‐face and video‐based settings. Conclusion The results indicate that the presented methodology is well situated to investigate the question whether psychodynamic psychotherapy in video‐based setting works in the sameway as in a face‐to‐face setting.

Abstract:The 18th volume of the American Journal of Geriatric Psychiatry (AJGP) begins on a positive note. In 2008 the Journal’s Impact Factor, released by the Institute for Scientific Information, crossed the 4.0 mark for the first time in the AJGP’s history. To put this number in context, fewer than 20% of all the 146 journals in psychiatry and psychology and the 36 journals in aging have Impact Factors above 4.0. Furthermore, the journals with higher Impact Factors are usually in fields that have long traditions and/or cover a broad field of research. It is unusual to find in that group a relatively young journal representing a small subspecialty such as geriatric psychiatry. Just a few short years ago the AJGP was a quarterly publication with about 100 submissions a year and an Impact Factor below 2.0. Now the Journal is a monthly, with approximately 300 submissions a year, with nearly 40% of them coming from outside the US, indicating that the AJGP has truly become an international journal.

Abstract:Clinical supervision is considered important in psychotherapy training, but little is known about the efficacy of specific supervision methods. We investigate two such methods (video-based vs. verbal report-based supervision) in a randomized controlled trial. Seventy-three supervisees were trained in common cognitive-behavioral therapy methods (i.e. behavioral activation and cognitive restructuring) by means of written information and a modelling video demonstrating the techniques. Supervisees had to apply the techniques in role plays with standardized patients (presenting depressive patients). Subsequently, supervisees were randomized to supervision, based on the video, or supervision based on the verbal report of the supervisees. Subsequently and after a three-month follow-up period, supervisees had to demonstrate the therapeutic techniques again. Therapist competence, therapeutic alliance, empathy, and anxiety were assessed through various different perspectives (i.e. independent raters, standardized patients, and supervisees' self-evaluation). Both supervision conditions lead to a significant improvement of therapeutic competences, therapeutic alliance, and empathy. No significant differences were found between the two supervision conditions. At three-month follow-up, training effects decreased on all perspectives except standardized patients. A training condition without supervision would be necessary to demonstrate that improvements are specific effects of the supervision conditions. Moreover, further supervision seems necessary to maintain training effects over time.

Abstract:Ziel der Studie Die Corona-Pandemie zwang viele Psychotherapeut*innen dazu, einen schnellen Übergang in das Videosetting zu ermöglichen. Auch nach der akuten Phase der Pandemie sind videobasierte Psychotherapiesitzungen (VP) zu einem gewissen Anteil in der Praxis bestimmbar, weshalb Indikations- (IN) und Kontraindikationsempfehlungen (KIN) zur Sicherstellung der Behandlungsqualität unerlässlich sind. Empirische Untersuchungen fehlen hierzu allerdings nahezu vollständig. Im Rahmen dieser Arbeit sollen Entscheidungs- und Abwägungsprozesse von Psychotherapeut*innen im Hinblick auf VP erfasst und jene Faktoren identifiziert werden, die einen Einfluss auf die Indikationsstellung haben können. Methodik Zweistufige, qualitativ angelegte Delphi-Befragung mit, als Expert*innen für die untersuchte Thematik eingestuften psychodynamisch orientierten Psychotherapeut*innen (PD; n=9) und Verhaltenstherapeut*innen (VT; n=7), die den Arbeitsfeldern Forschung und Praxis zugeordnet werden können. Ergebnisse Es zeigte sich, dass die Qualität der therapeutischen Allianz für die Teilnehmer*innen in der Wahl des Settings handlungsleitend ist. Eine Indikation für das Videosetting ergibt sich u. a. aus der Durchführbarkeit therapeutischer Interventionen und dem jeweiligen Behandlungsziel. Die Therapeut*innen wogen die Kosten und Nutzen des Videosettings für verschiedene Störungsbilder ab. Das beiderseitig veränderte Gefühl der Kontrolle und die Möglichkeit der Versorgungsverbesserung spielen dabei eine zentrale Rolle. Diskussion Auf Grundlage der Ergebnisse wurde ein Entscheidungsbaum für Psychotherapeut*innen erstellt, welcher die identifizierten Indikationskriterien aufgreift. Even after the acute phase of the pandemic, video-based treatments can be determined to a certain extent in practice. Thus, indication and contraindication recommendations are essential to ensure treatment quality. However, empirical studies are almost completely lacking in this regard. In the context of this work, the decision-making and consideration processes of psychotherapists regarding video-based treatments are to be recorded and those factors that can have an effect on the indication are to be identified. Methods Two-stage, qualitative Delphi survey with psychodynamically oriented psychotherapists (n=9) and behavioural therapists (n=7) who can be assigned to the fields of research and practice. Results The quality of the therapeutic alliance guides the participants' choice of setting. An indication for video setting results from the feasibility of therapeutic interventions and the respective treatment goal. The therapists weighed the costs and benefits of video setting for various disorders. The changed feeling of control on both sides and the possibility of improving health care are decisive factors. Discussion Based on the results, a decision tree for psychotherapists was created, which includes the identified indication criteria. Abstract:Speech therapy conversations between five dyads, consisting of senior speech pathology students and hearing-impaired adolescents enrolled in a Total Communication programme, were videorecorded for five weekly sessions. Samples over one-minute intervals at the start, middle, and end of the sessions were analysed for nonverbal behaviour at 3-second periods on 5-point scales covering body orientation, body position, eye gaze, facial expression, head position, and gesture/hand movement. Combination of the six resulting scores produced composite scores, which analysis of variance showed to differ significantly between dyads, sessions, intervals, and conversation partners (students versus adolescents). There were also significant interactions between dyads X sessions, dyads X partners, and sessions X partners. Examples using simple time series analysis complemented the analysis of variance by describing evidence for cyclical involvement between conversation partners.

Abstract:How does using video technology influence the emotional experience of communication in psychological counselling? In this paper, the experience of emotion—as an essential factor in the communication between counsellor and client—is systematically compared for face-to-face and video formats. It is suggested that the research methodology for studying computer-mediated forms of communication links lab and (virtual) reality in an ideal way. Based on a sample of 27 cases, significant differences and their observed effect sizes are presented. The aim of this study is to investigate the emotional experience in direct and mediated interaction and thus to contribute to the systematic search for evidence as to whether and how the emotional experience in psychological counselling interviews changes during video-mediated transmission. The results suggest, among others, that negative emotions are more intense in the video format and positive emotions are intensified in the face-to-face format.

Abstract:Background Depression and anxiety during the postpartum period are common, with psychotherapy often being the preferred method of treatment. However, psychological, physical, and social barriers prevent women from receiving appropriate and timely psychotherapy. The option of receiving psychotherapy through videoconferencing (VC) during the postpartum period presents an opportunity for more accessible and flexible care. Objective The aim of this study was to assess the feasibility, acceptability, and preliminary effectiveness of optional VC added to usual office-based psychotherapy, with a psychotherapist during the postpartum period. Methods We conducted a pilot randomized controlled trial with 1:1 randomization to office-based care (treatment as usual; TAU) or office-based care with the option of VC (treatment as usual plus videoconferencing; TAU-VC) for psychotherapy during the postpartum period. We assessed the ability to recruit and retain postpartum women into the study from an urban perinatal mental health program offering postpartum psychotherapy, and we evaluated the uptake, acceptability, and satisfaction with VC as an addition to in-person psychotherapy. We also compared therapy attendance using therapist logs and symptoms between treatment groups. Symptoms were assessed at baseline and 3 months postrandomization with the Edinburgh Postnatal Depression Scale, Generalized Anxiety Disorder 7-item, and Parental Stress Scale. Furthermore, 3-month scores were compared between groups with intention-to-treat linear mixed-effects models controlling for baseline score. Results We enrolled 38 participants into the study, with 19 participants in each treatment group. Attendance data were available for all participants, with follow-up symptom measures available for 25 out of 38 participants (66%). Among the 19 TAU-VC participants, 14 participants (74%) utilized VC at least once. Most participants were highly satisfied with the VC option, and they reported average savings of Can $26 and 2.5 hours in travel and childcare expenses and time per appointment. There were no significant differences between the 2 groups for psychotherapy attendance or symptoms. Conclusions The option of VC appears to be an acceptable method of receiving psychotherapy for postpartum women, with benefits described in costs and time savings. On the basis of this small pilot sample, there were no significant differences in outcomes between office-based care with or without the option of VC. This study has demonstrated the feasibility of such a program in an urban center, which suggests that a larger study would be beneficial to provide evidence that is more conclusive.

Results: All participants were satisfied with video therapy, stated that they used apps, VR, etc. less. Patients were more open, more tech-savvy and felt more competent in their use compared to therapists. Psychotherapists rated the digital therapeutic relationship better than patients. Discussion: The use of digital technologies in psychotherapy is not yet established in outpatient care. Measures to promote acceptance are necessary to break down barriers.

Abstract:Background Many patients with mental disorders remain untreated. Video-based mental health care demonstrates comparable effectiveness to face-to-face treatments and is a promising mode for delivering specialized care within primary care. Nevertheless, professionals struggle with implementing video consultations in their daily practice. Specifically, little is known about mental health specialists’ acceptance of mental health video consultations in routine practice. The PROVIDE (ImPROving cross-sectoral collaboration between primary and psychosocial care: An implementation study on VIDEo consultations) project aims to improve cross-sectoral collaboration between primary and psychosocial care through implementing video consultations in primary care. To increase the uptake of video consultations, it is crucial to account for necessary prerequisites and to tailor interventions to the needs of the target group prior to implementation. Objective The aim of this study was to explore the acceptance of video consultations embedded in primary care from the perspectives of mental health specialists in Germany. Methods We conducted a qualitative, exploratory, preimplementation study in urban and rural counties. We conducted three semistructured focus groups with 11 mental health specialists. We used qualitative content analysis combining an inductive-deductive approach, applying the Tailored Implementation in Chronic Diseases (TICD) framework to the text material, which comprises individual health professional factors; patient factors; professional interactions; incentives and resources; capacity for organizational change; social, political, and legal factors; and guideline factors. Results Against the background of long waiting times and a shortage of mental health specialists, especially in rural areas, participants valued video consultations as a potential means to improve access to mental health care. With respect to the TICD framework domains, the participants most often discussed individual health professional factors, followed by patient factors. All participants highlighted the importance of a trusting relationship between the patient and the therapist and doubted whether such a relationship could be established through video consultations (11/11, 100%). However, participants considered mental health specialist video consultations to be particularly suited for patients in rural areas, those with impaired mobility, and those who may otherwise remain untreated (6/11, 55%). Most participants expected video consultations to help the aforementioned patient groups avoid tedious searching for an available therapist and save on travel time and, therefore, improve access to specialized care for patients (7/11, 64%). Moreover, the participants expected video consultations to improve collaboration with the family physician (6/11, 55%). Finally, participants identified organizational aspects, such as reliable scheduling, the duration of the individual consultation (9/11, 82%), and reimbursement conditions (7/11, 67%), as key drivers for the acceptance and adoption of the model. Conclusions While mental health specialists expect video consultations to improve access to specialized care for some patients, they consistently wonder whether such consultations can establish a trusting patient-therapist relationship. When implementing video consultations, these concerns should be addressed by training providers in managing technology-based treatment settings, with extra consideration for fostering the patients’ and therapists’ engagement. Abstract:Ziel der Studie Globale Konflikte und humanitäre Krisen führten in den vergangenen Jahren zu einer erhöhten Fluchtmigration nach Deutschland. Um perspektivisch die Versorgungsstruktur für Menschen mit Fluchthintergrund im deutschen Gesundheitssystem zu verbessern, werden im Rahmen dieser Pilotstudie erste Erfahrungen in der videogestützten Durchführung der Gruppenpsychotherapie Empowerment für Geflüchtete vorgestellt. Methodik Empowerment ist eine kultursensible, dolmetschergestützte Gruppentherapie zur Behandlung von depressiven und stressbezogenen Symptomen bei Geflüchteten. Vier afghanische Männer nahmen an der Pilotstudie teil. Die Intervention umfasste 16 Sitzungen, die innerhalb von 12 Wochen per Video durchgeführt wurden. Ergebnisse Die Internetverbindung war teils instabil und führte zu organisatorischen Herausforderungen. Sprachlich und interaktionell war die Therapie durchführbar. Diskussion Eine stabile Internetverbindung stellt das zentrale Kriterium für eine erfolgreiche Durchführung der Therapie dar. Es werden mögliche Lösungsansätze und Einschränkungen für zukünftige Studien diskutiert. Schlussfolgerung Hinsichtlich der potenziellen Chancen auf eine zukünftige Erweiterung der Versorgungsoptionen für Menschen mit Fluchthintergrund, könnte das videogestützte Therapiekonzept in einer randomisierten kontrollierten Studie untersucht werden. Objective Global conflicts and humanitarian crises led to an increase in forced migration to Germany in recent years. To improve the mental health care system for refugees and asylum seekers in Germany perspectively, we aim to examine the feasibility of implementing the culturally sensitive group psychotherapy Empowerment for refugees with affective disorders in a video-assisted setting. Methods Empowerment is a culturally sensitive, interpreter-assisted intervention for the treatment of depressive and stress-related symptoms in refugees. Four male refugees from Afghanistan participated in a pilot study. The intervention included 16 modules delivered via video over a 12-week period. Results The internet connection was frequently unstable and led to organizational challenges. The therapy was feasible in terms of linguistic and interactional aspects. Discussion The stability of the internet connection represents the major criterion for a successful implementation of the therapy. Implications for future studies are discussed. Conclusion Regarding the potential opportunities to improve the mental health care provision to refugees and asylum seekers in the future, the video-assisted therapy concept could be investigated in a randomized controlled trial. Physiological synchrony in psychotherapy sessions

Abstract:Objective: In this proof-of-principle study, a convenience sample of 55 dyadic psychotherapy sessions conducted by one therapist was analyzed. This study aimed at exploring physiological synchrony in naturalistic psychotherapy sessions and the association of such synchrony with self-report ratings. Methods: The electrocardiograms and respiration behavior of both therapist and client were monitored simultaneously. Four clients were included, and session outcome was documented by session reports in two clients. From electrocardiograms, heart rate and heart rate variability were derived in consecutive 15-second intervals throughout sessions. Entire sessions (average duration, 51 min) were assessed for physiological synchrony of therapist's and client's respiration, electrocardiogram, heart rate, and heart rate variability. Two methods of synchrony computation were applied to the time series: windowed cross-correlation and correlation of local slopes (concordance). Both methods included surrogate controls using segment-wise shuffling. Results: Significant synchrony of three measures, but not of electrocardiograms, was present in this dataset. In regression models, we found associations between synchronies and alliance ratings, and further self-report variables. Conclusions: Results support the existence of physiological synchrony in this collection of psychotherapy sessions, which speaks for the sympathetic and parasympathetic coupling between this therapist and her clients and its link with ratings of the therapy process. The feasibility of deriving signatures of synchrony of physiological signals with the described methodology was corroborated. The findings now await generalization by further research.

Abstract:Despite the widespread use of audio or video recording in psychotherapy training and research, there has been surprisingly little exploration of patient reactions to the use of recordings in psychotherapy, and there is even less written about patient factors that influence their willingness to consent to recording practices or the impact of such a request on treatment. The present study examined the relationship between pretreatment patient symptomatology and patient attitudes toward the audio or video recording of psychotherapy sessions. Treatment refusal, duration, and outcome were also examined as they related to patient comfort with recording. A total of 390 participants completed an initial intake in a university-based community outpatient clinic. Pretreatment patient symptomatology was measured at the initial intake evaluation using the Brief Symptom Inventory (Derogatis, 1993), and patient attitudes toward audio or video recording were measured using an audio/videotape comfort form. The majority of patients expressed no or slight concerns (52%), and almost three quarters (71%) were willing to consider audio or video recording. It was found that higher levels of pretreatment interpersonal sensitivity and paranoia have a significant negative relationship to recording comfort (i.e., greater pathology related to lower comfort). However, treatment refusal, duration, and outcome were not significantly related to patient comfort with recording. Significant intake clinician effects were observed in regard to patient-rated comfort regarding audio or video recordings, indicating a relationship between patients’ intake clinician and their level of comfort. Therapist effects were examined with regard to treatment refusal, duration, and outcome, and all results remained nonsignificant. This research has implications for and supports the implementation of audio- or video-recording practices in clinical training, research, and practice.

Abstract:This paper reports the methodology of a research project based on and including a review of almost 1000 texts and more than 20 years of learning and supervised practice as clinician and supervisor in the psychotherapeutic professions on the nature of the therapeutic relationship. Following a pilot study by five years, the findings were reported in a 165,000 word document which encompasses poetry, personal experience, many textual extracts of theory or therapeutic dialogue, other research reports, theoretical considerations, clinical reflections, syllabus constructions—a postmodernist assemblage which nonetheless attempts coherence, validity and reliability. The findings have also been tested in the field for example by: (a) providing the framework for at least one four-year psychotherapy training course from which the first students have recently graduated by external assessment and (b) an independent psychotherapy accreditation process by a case study submission. It is argued that clinical practice or supervision should be inseparable from research—Particularly qualitative research in psychology.

Abstract:When carrying out medical imaging based on detection of isotopic radiation levels of internal organs such a lungs or heart, distortions, and blur arise as a result of the organ motion during breathing and blood supply. Consequently, image quality declines, despite the use of expensive high resolution devices and, such devices are not exploited fully. A method with which to overcome the problem is image restoration. Previously, we suggested and developed a method for calculating numerically the optical transfer function (OTF) for any type of image motion. The purpose of this research is restoration of original isotope images (of the lungs) by restoration methods that depend on the OTF of the real time relative motion between the object and the imaging system. This research uses different algorithms for the restoration of an image, according to the OTF of the lung motion, which is in several directions simultaneously. One way of handling the three-dimensional movement is to decompose the image into several portions, to restore each portion according to its motion characteristics, and then to combine all the image portions back into a single image. An additional complication is that the image was recorded at different angles. The application of this research is in medical systems requiring high resolution imaging. The main advantage of this approach is its low cost versus conventional approaches.

Abstract:Objective: Deficits of dyadic social interaction seem to diminish social functioning in schizophrenia. However, most previous studies are of a limited ecological validity due to decontextualized experimental conditions far off from real-world interaction. In this pilot study, we thus exposed participants to a more real-world-like situation to generate new hypotheses for research and therapeutic interventions. Methods: Dyads of either participants with schizophrenia (n = 21) or control participants without mental disorder (n = 21) were presented with a 5-min emotionally engaging movie. The subsequent uninstructed dyadic interaction was videotaped and analyzed by means of a semi-quantitative, software-supported behavioral analysis. Results: The patients with schizophrenia showed significant abnormalities regarding their social interaction, such as more negative verbalizations, a more open display of negative affect and gaze abnormalities. Their interaction behavior was mostly characterized by neutral affect, silence and avoidance of direct eye contact. Neutral affect was associated with poorer psychosocial performance. Verbal intelligence and empathy were associated with positive interaction variables, which were also not impaired by psychotic symptom severity. Conclusion: In this real-world-like dyadic interaction, participants with schizophrenia show distinct abnormalities that are relevant to psychosocial performance and consistent with a hypothesized lack of attunement to interaffective situations.

Abstract:Objectives Videoconference psychotherapy (VCP) is a crucial component of many health care systems, allowing for remote delivery of services. However, little is known about the mechanisms of change within VCP. Previous research has suggested that self‐disclosure may be greater in VCP than face‐to‐face modalities and was investigated in the current study. Design Young adults aged 18–25 years (N = 57) were randomly allocated to face‐to‐face or VCP interview conditions, with measures completed pre‐ and post‐interview. Methods Participants completed an autobiographical memory task, requiring them to describe specific memories in response to positive and negative valence cue words. Measures included self‐reported self‐disclosure, blind observer‐rated self‐disclosure, memory specificity, and mean number of words per response. Results No significant differences were found between conditions with regard to self‐reported self‐disclosure, capacity to recall specific memories, or words uttered per response. However, observer‐rated depth of self‐disclosure was significantly higher for participants in the face‐to‐face than VCP condition. Self‐disclosure and memory specificity were also significantly greater for negative than positive valence cue words, regardless of condition. Conclusions The findings indicate that whilst participants may be able to draw on memories with equal ease regardless of interview modality, in VCP, emotional processing of these memories may require increased support and guidance from the therapist.

Abstract:Objective: The present work aims to detect the role of the rate of speech as a mechanism able to give information on patient's intrapsychic activity and the intersubjective quality of the patient-therapist relationship. Method: Thirty clinical sessions among five patients were sampled and divided into idea units (N = 1276) according to the referential activity method. Each idea unit was rated according to referential activity method and in terms of speech rate (syllables per second) for both patient and therapist. A mixed-effects model was applied in order to detect the relationship between the speech rate of both the patient and the therapist and the features of the patient's verbal production in terms of referential activity scales. A Pearson correlation was applied to evaluate the synchrony between the speech rate of the patient and the therapist. Results: Results highlight that speech rate varies according patient's ability to get in touch with specific aspects detected through referential activity method: patient and the therapist speech rate get synchronized during the course of the sessions; and the therapist's speech rate partially attunes to the patient's ability to get in touch with inner aspects detected through RA method. Conclusion: The work identified speech rate as a feature that may help in the development of the clinical process in light of its ability to convey information about a patient's internal states and a therapist's attunement ability. These results support the intersubjective perspective on the clinical process.
