Abstract:Objective: to evaluate the therapeutic effect of targeted endostatin-loaded microbubbles, combined with improved, focused, directional ultrasound radiation for inhibition of subcutaneous translocation in situ colon tumor angiogenesis in colon cancer. Methods: 65 BALB/c nude mice with subcutaneous translocation in situ colon tumors were randomly divided into five groups. Group A was the control group, without any treatments. In Group B, the mouse was treated with ultrasonic radiation. In Group C, the mouse was treated with ultrasonic radiation combined with empty SonoVue microbubbles. In Group D, the mouse was treated with ultrasonic radiation combined with empty Targestar-SA microbubbles. In Group E, the mouse was treated with ultrasonic radiation combined with endostatin microbubbles. The tumor size was measured before and 1, 14, and 28 days after irradiation. The peak intensity (PI), regional blood volume (RBV) and regional blood flow (RBF) were recorded using contrast-enhanced ultrasound. The tumor tissue was removed for pathological examination; the tumor necrosis area and microvascular density (MVD) were evaluated by immunohistochemistry. Results: Tumors in Groups C, D and E were significantly smaller than in Groups A and B at 28 days after irradiation, with Group E the smallest. PI, RBF and RBV of Groups C, D, and E were significantly decreased 28 days after radiation with Group E the lowest, and significantly lower than Groups A and B (all P < 0.05). The tumor tissue necrosis area of Group E was clearly greater while MVD was obviously lower than the other groups (all P < 0.01) at 28 days after treatment. Conclusion: The targeted endostatin microbubbles, combined with focused, directional ultrasound radiation can damage tumor microvasculature of subcutaneous colon translocation in situ colon cancer, as well as inhibit the tumor angiogenesis.
Orthotopic Esophageal Cancers: Intraesophageal Hyperthermia-enhanced Direct Chemotherapy in Rats

Abstract:Purpose: To determine the feasibility of using intraesophageal radiofrequency (RF) hyperthermia to enhance local chemotherapy in a rat model with orthotopic esophageal squamous cancers.Materials and Methods: The animal protocol was approved by the institutional animal care and use committee and the institutional review board. Human esophageal squamous cancer cells were transduced with luciferase lentiviral particles. Cancer cells, mice with subcutaneous cancer esophageal xenografts, and nude rats with orthotopic esophageal cancers in four study groups of six animals per group were treated with (a) combination therapy of magnetic resonance imaging heating guidewire-mediated RF hyperthermia (42 degrees C) plus local chemotherapy (cisplatin and 5-fluorouracil), (b) chemotherapy alone, (c) RF hyperthermia alone, and (d) phosphate-buffered saline. Bioluminescent optical imaging and transcutaneous ultrasonographic imaging were used to observe bioluminescence signal and changes in tumor size among the groups over 2 weeks, which were correlated with subsequent histologic results. The nonparametric Mann-Whitney U test was used for comparisons of variables.Results: Compared with chemotherapy alone, RF hyperthermia alone, and phosphate-buffered saline, combination therapy with RF hyperthermia and chemotherapy induced the lowest cell proliferation (relative absorbance of formazan: 23.4% +/- 7, 44.6% +/- 7.5, 95.8% +/- 2, 100%, respectively; P < .0001), rendered the smallest relative tumor volume (0.65 mm(3) +/- 0. 15, P < .0001) and relative bioluminescence optical imaging photon signal (0. 57 x 10(7) photons per second per square millimeter +/- 0.15, P < .001) of mice with esophageal cancer xenografts, as well as the smallest relative tumor volume (0.68 mm(3) +/- 0.13, P < .05) and relative photon signal (0.56 X 10(7) photons per second per square millimeter +/- 0.11. A Functional Stent Encapsulating Radionuclide in Temperature-Memory Spiral Tubes for Malignant Stenosis of Esophageal Cancer.

Abstract:Stent implantation is a commonly used palliative treatment for alleviating stenosis in advanced esophageal cancer. However, tissue proliferation induced by stent implantation and continuous tumor growth can easily lead to restenosis. Therefore, functional stents are required to relieve stenosis while inhibiting tissue proliferation and tumor growth, thereby extending the patency. Currently, no ideal functional stents are available. Here, iodine-125 (125 I) nuclides are encapsulated into a nickel-titanium alloy (NiTi) tube to develop a novel temperature-memory spiral radionuclide stent (TSRS). It has the characteristics of temperature-memory, no cold regions at the end of the stent, and a uniform spatial dose distribution. Cell-viability experiments reveal that the TSRS can reduce the proliferation of fibroblasts and tumor cells. TSRS implantation is feasible and safe, has no significant systemic radiotoxicity, and can inhibit in-stent and edge stenosis caused by stent-induced tissue proliferation in healthy rabbits. Moreover, TSRS can improve malignant stenosis and luminal patency resulting from continuous tumor growth in a VX2 esophageal cancer model. As a functional stent, the TSRS combines the excellent properties of NiTi with brachytherapy of the 125 I nuclide and will make significant contributions to the treatment of malignant esophageal stenosis.
Effects of Irradiation with Different Fractionations on Radioresistant Esophageal Cancer Cells and Lung Tissues

Abstract:Background: Local recurrence is the main cause of treatment failure for esophageal cancer and a large part of them require re-irradiation which needs to face the two major challenges of radioresistance and radiationinduced lung injury (RILI). This study aimed to evaluate the effects of irradiation with different fractionations on inhibition in radioresistant esophageal cancer cells in vitro and RILI in vivo. Methods: EC109R was selected from the human esophageal squamous carcinoma cell line EC109 by multiple exposures and verified to be radioresistant through colony forming assay and cell proliferation assay. EC109R was irradiated with different fractionations (conventional radiotherapy, CR: 2 Gy/d, 5 d; pulsed low dose-rate radiotherapy, PLDR: 0.2 Gy x 10 pulses with 3-min intervals/d, 5 d; hypo-fractionated radiotherapy, HR: 5 Gy/d, 2 d) to verify their inhibition by CCK-8. The protection of PLDR on RILI was assessed by H&E-staining paraffin sections and mice serum TGF-beta 1 levels. Results: EC109R exhibited much better radioresistance (p < 0.0001) than EC109 in colony forming and cell proliferation assays. For EC109R, PLDR showed slightly lower inhibition by detection of optical density than HR (p = 0.0025) but similar to CR (p = 0.1155). In addition, pathological sections of lung tissues from PLDR group showed less alveolar structural damage and inflammatory cells infiltration, and serum TGF-beta 1 levels were significantly lower than those in the other two fractionations (vs CR: p = 0.0092; vs HR: p = 0.0045). Conclusion: PLDR could effectively inhibit the proliferation of radioresistant esophageal cancer cells in vitro and mitigate RILI in vivo.
Dosimetric Evaluation and Clinical Application of Radioactive Iodine-125 Brachytherapy Stent in the Treatment of Malignant Esophageal Obstruction

Abstract:Objective To evaluate the dosimetric characteristics and the clinical application of radioactive iodine-125 brachytherapy stent (RIBS) in malignant esophageal obstruction. Methods The dose distribution of RIBS with different seed spacing, diameter and length was studied by treatment planning system (TPS) calculation, thermoluminescence dosimeter (TLD) measurement and Monte Carlo (MC) data fitting. And the data of esophageal cancer patients who were treat with this type of RIBS was analyzed retrospectively. Results Doses around the RIBS calculated by the TPS lay between those measured by the TLDs and those simulated by the MC, and the differences between the three methods were significant (p<0.05), the overall absolute dose differences among the three methods were small. Dose coverage at 1.5 cm from the center was comprehensive when the activity reached 0.6 mCi. Both the conformability and the uniformity of isodose lines produced by a seed spacing of 1.0 cm were superior to those produced by a seed spacing of 1.5 cm. The data of 50 patients treated with RIBS was analyzed. They were followed up until February 2020 when all of the patients died. The overall improvement rate of dysphagia after RIBS implant was 90%. Moderate and severe complications with an incidence of more than 10% were hematemesis (28%), pain (20%), and lung infection (10%). Stent restenosis occurred in 4 patients at a median interval of 108 days from the procedure. The overall incidence of fatal complications was 38% (including hematemesis, infection and asphyxia). The median survival time of patients with and without a history of radiotherapy were 3.4 months and 6 months, respectively, the difference of which was significant (p=0.021). No other factors affecting survival were identified. For patients with and without a history of radiotherapy, the incidences of fatal complications were 51.7% and 19%, respectively (p=0.019). No correlation between dose and stent restenosis was found. Conclusion TPS calculations are suitable for clinical applications. RIBS can effectively alleviate obstructive symptoms for patients with malignant esophageal obstruction, but the incidence of fatal complications was high, care should be taken when choosing this treatment.

Application of esophageal irradiation stents coated with 125I particles in advanced esophageal cancer

Abstract:Purpose: The current study was designed to investigate the primary efficacy of esophageal irradiation stents coated with 125I particles in the treatment of elderly patients with advanced esophageal cancer. Methods: Forty-three elderly patients with advanced esophageal cancer were treated with esophageal stents in the First Affiliated Hospital of Xinxiang Medical University between September 2009 and December 2010. Patients were randomly divided into group A (N=18), treated with irradiation stents, and group B (N=25), treated with ordinary stents. There were no significant intergroup differences in age, lesion length, degree of stenosis, or cancer stage. The stent implantation success rate, relief of dysphagia and complication rate, and survival were assessed. Results: The stent implantation success and short-term dysphagia relief rates were 100.0% in both groups. The mean survival time was 9.8 months and 4.8 months in groups A and B, respectively (p<0.01). However, no significant difference in pain (5/18) or esophageal restenosis (7/25) was found (both p>0.05). Conclusion: Dysphagia was relieved and survival was prolonged in advanced esophageal cancer cases treated with 125I particle-coated esophageal stents. This method may be superior to the traditional stents method.

125I Low Dose Rate Radiation Increases Radiotherapy Sensitivity on Esophageal Cancer Cells via Multiple Mechanisms

Abstract:Abstract Objective: To determine the biological effectiveness of iodine 125 (125I) radioactive seeds continuous low dose rate radiation on the human esophageal cancer cell line KYSE150 compare to single dose radiation and explore its potential cellular mechanisms.Method: Three groups of KYSE150 cells were explored: control group, single dose irradiation (SDR) and 125I seeds continuous low dose rate irradiation group (125I-CLDR). Dose-survival curves was obtained by colony formation assay. MTT proliferation assay was used to measure KYSE150 cell vibility. KYSE150 cell apoptosis was analysis by Annexin V-FITC/PI staining, meanwhile, cell cycle analysis was performed by flow cytometry. Bim, Bcl-2, caspase-3 and cleaved caspase-3 protein expression were measured by western blotting to vertify the apoptosis level, as well as CyclinB1 protein expression which values the fuction of G2/M check point.Cell morphology changes were observated under phase contrast microscope. Endoplasmic reticulum stress (ER stress) was measured by GPR78/Bip1 and PERK changes in gene expression, which was detected by Real-time PCR. Reactive oxygen species(ROS) changes and mitochondria were meaured by flow cytometry. ATP detection kit was used to measured ATP level afer two modes of irradiation. DNA-Pks, Ku70 and Ku80 expression were measured by western blotting to represent the damage of DNA damage and repair capabilities. Acridine orange (AO) staining was used to detect level of autophagy and quantitative was measured by flow cytometry. LC3-II, ATG5, Beclin1, p-Akt, p-mTOR, mTOR and p-S6 proteins expression were detectd by western blotting.Results: KYSE150 cells were more radiosensitive to 125I-CLDR than SDR. Two modes of irradiation could both inhibit the proliferation vability of KYSE150 cells, while the inhibition effects in 125I-CLDR was significantly stronger compared with SDR. Compared with SDR, 125I-CLDR showed more proportions of the early and late apoptosis rate as well as cells at G2/M phase. Apoptosis related proteins, such as Bim, caspase-3 and cleaved caspase-3, were elevated, while CyclinB1 expression was decreased in 125I-CLDR group. Cells became bigger and grainy in 125I-CLDR group, meanwhile, ROS levels and ER stress signal was elevated except ATP concentration. Cells stained by AO was anaysised by flow cytometry, results showed that red: green ratio in 125I-CLDR group increased as well as LC3 and ATG5 protein expression detected by western blotting. p-Akt, p-mTOR and p-S6 protein expression were decreased to some extent in 125I-CLDR group cells, while total mTOR protein showed no significant changes.Conclusion: Our results confirmed that 125I-CLDR could strong inhibite KYSE150 cancer cells. Furthermore, we revealed that cell cycle arrest and apoptosis were not the only fate after 125I-CLDR, autophagy might be another important choice which mTOR pathway might be involed in. These findings can support the application of esophageal stent loaded with 125I seeds in clinic.
Clinical effect and safety of 125I particle stent on treatment of postoperative esophageal stricture for patients with advanced esophageal cancer

Abstract:Objective To investigate clinical effect and safety of the 125I particle stent for treatment of postoperative esophageal stricture on patients with advanced esophageal cancer.Methods Sixty patients with postoperative esophageal stricture after esophageal cancer operation were randomly assigned to receive treatment with a conventional self-expanding stent (control group) or a stent loaded with 125I seeds (irradiation group).After stent implantation,the outcomes were compared in terms of the success rate of stent implantation,relief of dysphagia,survival time,and complications related to the procedure.Dysphagia was assigned a grade 0-4.The t test and the x2 test were used to make statistical analysis.Results The success rate of stents placement of two groups were 100％ (30/30).The results of preoperative dysphagia score showed that the patients of control group with 0,1,2,3,4 degree were 0,2,3,22 and 3 patients ; the patients of radiation group with 0,1,2,3,4 degree were 0,2,2,23 and 3 patients; in postoperative 1 week,the control group turned to 1,17,6,6 and 0 patients,the radiation group turned to 4,21,3,2 and 0 patients ; the difference between the 2 groups was not statistically significant (x2 =1.214,P =0.675) ; in postoperative 2 months,the patients with the above degree of dysphagia score of control group were 25,3,2,0,0; and the radiation group were 27,2,1,0,0,the difference between the 2 groups was not statistically significant (x2 =1.043,P =0.894).Followed up for 6 months,1 patient gastrointestinal bleeding,4 patients gastrointestinal pain and 8 patients esophageal restenosis were occurred in control group,corresponding in irradiated group were 0,5,and 3 patients respectively,the incidence of postoperative esophageal restenosis of irradiated group was lower than control group,and the difference was statistically significant (x2 =4.384,P =0.036).After follow-up for 18 months,10 patients were survival among the 30 patients of control group,the live time of the 20 patients dead patients were 2 to 14 months,control group of 30 patients,10 patients of survival of 20 deaths of patients from 2 to 14 months,the average value was (4.8± 1.7)months ; there were 15 survival patients among irradiation group,the survival time of the 15 deaths was 4 to 15 months,and the average value was (9.8 ± 2.9) months.By the end of follow-up,the average survival time for all patients of irradiated group was longer than the control group,the difference was statistically significant (t =8.917,P =0.002).Conclusion In patients with postoperative esophageal stricture and advanced esophageal cancer,stent loaded with 125I stent has potential benefit in that it allows a longer relief of dysphagia and a longer survival time. Abstract:201 Background: Despite treatment intensity escalation, the high local recurrence rate for esophageal cancer has frustrated clinicians for decades. Conventional High Dose Rate brachytherapy, particle therapies, and other sophisticated treatment methods are encouraging but often fiscally and/or biologically infeasible. Additionally, these technologies often burden patients with numerous hospital visits, decreasing patient quality of life. Very low dose-rate brachytherapy (vLDR), with sources emitting relatively low energy, can be more cost effective than conventional radiation treatments, particularly in underserved communities, and can replace many hospital visits with one procedure. The dose distribution advantages of vLDR are irrefutable, especially in regard to doses to other tissues. vLDR seeds are currently used regularly in the prostate and have been tested in several other anatomical sites with good safety results. In the present study, a radioactive(RA) stent platform was developed and tested to apply vLDR therapy to cancers of the gastrointestinal (GI) system. Methods: Monte Carlo simulations were used to guide design of a RA esophageal stent. A design was chosen that minimized surface dose variation while using commercially available RA seeds. RA stents loaded with 60 -125 Seed-in Carrier, GE Healthcare were endoscopically placed in the naïve esophagus of 3 Yucatan swine. Previously, mock treatments using dummy seeds were performed in three similar pigs. Stents were left in place for between 4 and 7 weeks and reaction of the mucosa was assessed macroscopically and histologically. Results: Porcine esophageal tissue received doses in the range of 132-160 Gy at the mucosal surface and 39-48 Gy at a distance of 5mm from the stent surface. Tissues were collected at 0, 1 and 10 weeks after stent removal. No macroscopic radiation damage was seen at autopsy and pathological analysis yielded no radiation-related damage in evaluable tissues. Conclusions: In this preliminary study, no local or systemic damage due to radiation was observed after treatment of normal esophageal tissue with a vLDR stent device. Ongoing research aims to determine dose tolerance in esophageal tissue and target application of vLDR in other locations in the GI tract.

Radioactive Stent for Malignant Esophageal Obstruction: A Meta-Analysis of Randomized Controlled Trials.

Abstract:Purpose: To compare the relative clinical efficacies of radioactive and normal stent insertion methods as a means of treating patients suffering from malignant esophageal obstruction (MEO). Materials and Methods: The Pubmed, Embase, and Cochrane Library databases were searched for relevant randomized controlled trials (RCTs) from the date of inception through to July 2020. RevMan v5.3 was used for all data analyses. Results: This meta-analysis included six RCTs that included a total of 194 patients who had undergone radioactive stent insertion and 209 who had normal stent insertion. There were no significant differences in pooled improvement of dysphagia scores (P = .40), rates of stent restenosis (24.7% versus 28.7%, P = .35), stent migration (3.3% versus 4.4%, P = .61), severe chest pain (22.8% versus 20.3%, P = .61), hemorrhage (11.0% versus 9.8%, P = .80), or fistula formation (6.1% versus 4.2%, P = .55) between two groups. The pooled time to restenosis (P < .00001) and survival (P < .00001) were significant longer in the radioactive stent group. Significant heterogeneity was detected in the endpoint of improvement of dysphagia score (I2 = 89%; P = .0002). Funnel plot analyses did not detect any evidence of publication bias pertaining to the selected study endpoints. Conclusions: Our meta-analysis demonstrated that radioactive stent insertion can prolong stent patency and survival for patients with MEO compared with normal stent insertion.

Abstract:BACKGROUND:A novel radioactive 125I seed-loaded biliary stent has been used for patients with malignant biliary obstruction. However, the dosimetric characteristics of the stents remain unclear. Therefore, we aimed to describe the dosimetry of the stents of different lengths - with different number as well as activities of 125I seeds. METHODS:The radiation dosimetry of three representative radioactive stent models was evaluated using a treatment planning system (TPS), thermoluminescent dosimeter (TLD) measurements, and Monte Carlo (MC) simulations. In the process of TPS calculation and TLD measurement, two different water-equivalent phantoms were designed to obtain cumulative radial dose distribution. Calibration procedures using TLD in the designed phantom were also conducted. MC simulations were performed using the Monte Carlo N-Particle eXtended version 2.5 general purpose code to calculate the radioactive stent's three-dimensional dose rate distribution in liquid water. Analysis of covariance was used to examine the factors influencing radial dose distribution of the radioactive stent. RESULTS:The maximum reduction in cumulative radial dose was 26% when the seed activity changed from 0.5 mCi to 0.4 mCi for the same length of radioactive stents. The TLD's dose response in the range of 0-10 mGy irradiation by 137Cs γ-ray was linear: y = 182225x - 6651.9 (R2=0.99152; y is the irradiation dose in mGy, x is the TLDs' reading in nC). When TLDs were irradiated by different energy radiation sources to a dose of 1 mGy, reading of TLDs was different. Doses at a distance of 0.1 cm from the three stents' surface simulated by MC were 79, 93, and 97 Gy. CONCLUSIONS:TPS calculation, TLD measurement, and MC simulation were performed and were found to be in good agreement. Although the whole experiment was conducted in water-equivalent phantom, data in our evaluation may provide a theoretical basis for dosimetry for the clinical application.

Abstract:With the widespread adoption of self-expandable esophageal stent loaded with 125I seeds, a number of parameters of the irradiation stent dosimetry were needed to identify. The aim of this study was to investigate potential dosimetry distribution for self-expandable esophageal stent loaded with 125I seeds and to propose standard dosimetry parameters. Six models of stents (Nanjing Micro-Tech Co Ltd, Nanjing, China) which were 8.0cm length×2.0cm diameter with 1.5cm spacing of the seed center (A), 8.0cm length×2.0cm diameter with 1.0cm spacing of the seed center (B), 8.0cm length×1.3cm diameter with 1.0cm spacing of the seed center (C), 8.0cm length×2.4cm diameter with 1.0cm spacing of the seed center (D), 12.0cm length×2.0cm diameter with 1.0cm spacing of the seed center (E), and 16.0cm length×2.0cm diameter with 1.0cm spacing of the seed center (F), were studied. The treatment planning system (TPS) used to calculate the dose delivery at specific points. The points were defined at polar angles of 0-90°in 10°increments and at radial distances from outer surface of the stent were 0.5, 0.75, 1, 1.25, 1.5, 2, 2.5, 3, 3.5, 4, 5, and 7cm. A cross section of the detector was arranged. The thermoluminescence detector (TLD) used for this study was LiF:Mg,Cu,P (GR 200A). The detectors were defined at polar angles of 0-90°in 10°increments and at radial distances from outer surface of the stent were 0.5, 0.75, 1, 1.25, 1.5, 2, 2.5, 3, 3.5, 4, 5, and 7cm. A cross section of the detector was arranged. The readout system was a TL Reader (Thermo, USA). The heating cycle consisted of preheat, readout, anneal and cool segments. The Monte Carlo (MC) were used for the dosimetry simulations around the irradiation stent. The range for photon and electron transport extends from 1 KeV to 100 MeV. The irradiation stents have been simulated in the centre of a spherical phantom of water with 20 cm radius, large enough to consider all the scattering effects of the surrounding medium. The detectors were defined at polar angles of 0-90°in 10°increments and at radial distances from outer surface of the stent were 0.5, 0.75, 1, 1.25, 1.5, 2, 2.5, 3, 3.5, 4, 5, and 7cm. A cross section of the detector was arranged. Compare all the results of these three methods and propose standard dosimetry algorithm. Source strength lineal density is equal for peripheral sources to interior sources. The results show the schematic diagram of the irradiation stents geometry and algorithm used with algorithm for dose calculations to a point approximation. The superposition principle assumed the total dose distribution to be a summation to the dose contributions of all individual sources loaded a stent. When the seed activity was ≤0.6mCi, the isodoses were not well-distrubuted in each plane. At the point of 145Gy from the stent outer surface, the isodose was discontinuous which leads to radiology cold on the surface of the stent. When the seed activity was ≥0.7mCi, the isodoses well-distrubuted and trended to nearly circular shaped in each plane, especially when the seed activity was ≥0.9mCi. At the point of 145Gy from the stent outer surface, the isodose was continuous. The “system” described allowed the users to administer a safe dose easily to the patient for the treatment of the tumor by providing the rules for arrangement and placement of the sources loaded with the stent. When the activity of 125I seeds were ≥0.7mCi, the isodoses of 145Gy were continuous whthout dosimetric cold spots where are the location of the tumor recurrence.

125)I particle esophageal stent in the treatment of 30 patients with advanced esophageal cancer

Abstract:Objective: To observe clinical results of expandable esophageal stent loaded with 125I seeds for advanced esophageal cancer.Materials and Methods: 30 patients who suffered from advanced esophageal cancer received expandable esophageal stent loaded with 125I seeds deployment.Clinical and imaging follow up were performed post procedure.Results: Stents,diameter,18-20mm;length,6-10mm,were successfully deployed in 30 patients.Barium meal study pre procedure showed all patients had esophageal stenoses greater than 80%.Stenotic lumina recovered to 90-100% of normal diameter after stent placement.Partial response(n=15),progressive disease(n=5) and stable disease(n=10) were found in 30 patients at the third follow up month by using CT angiography.Overall survival was 6-20.5 month,mean 9.9 month.All patients had improved dysphagia after stent deployment.25 patients had Karnofsky score ≥60 after procedure compared with 11 patients before operation.Conclusion: Expandable esophageal stent loaded with 125I seeds was an ideal therapeutic approach for patients with advanced esophageal cancer.

Abstract:To evaluate the dosimetric characterization of a new designed radioactive biliary stent loaded with 125I seeds and report results in a form suitable for clinical use. Three commonly used stent models were studied. Dose measurements were made using standard methods employing treatment planning system (TPS) calculation, thermoluminscent dosimeters (TLDs) measurement, and Monte Carlo (MC) simulation. A solid paraffin and cylindrical barrel model made of Polymethylmethacrylate (PMMA) were used as a equivalent water phantom for TPS calculation. After a radioactive biliary stent was positioned in the measurement phantom, a CT scan was performed to get the stent images. The images were then transferred to the TPS for treatment planning. The intermediate CT image of the stents was selected to calculate the surface radial dose distribution. Radial dose distribution around the stents was also measured in a special designed PMMA phantom using TLD-GR 200A circular chips. In preliminary experiment, corrections were made to study the TLDs linearity of dose response, energy response, and scaled for exposure duration. MC simulations were performed using the MCNPX v.2.4 general purpose code to calculate the biliary stent three-dimensional dose rate distribution in liquid water. The length of the stents (F = 3189.160, P < 0.001) and the activity of the 125I seeds (F = 811.509, P < 0.001) could both significantly affect the cumulative radial dose distribution of the radioactive stent. For the same length of radioactive stents, the cumulative radial dose of the stent was different (P < 0.05) when the seed activity difference was of 0.1 mCi. The simulations with MCNPX agree in acceptable agreement with the TPS calculations and TLD measurements. The MC simulated uncertainty of the three stent models were all within the scope of the report recommended by AAPM TG-43U1. Depth dose calculations of different length radioactive biliary stents in different activity of 125I seeds were performed at 0.1, 0.5, 1.0, 1.5 and 2.0 cm from surface of the stent. The radioactive biliary stent doses at the reference point could be adjusted by changing the seed numbers, and source activity. The TPS calculation results and the TLD experimentally defined radial dose rate were in good agreement with MCNPX simulations. Determinations of the values of the 3D dose rate in water have been simulated using Monte Carlo simulations.

Abstract:Hyperthermia by heating in-stent is a potentially new method to treat esophageal cancer. This study assessed the safety of heating the rabbit esophagus with a nitinol stent in an alternating magnetic field (AMF) and investigated whether this method offers a therapeutic option for esophageal cancer. Nitinol stents were placed in the cervical esophagi of healthy rabbits, which were heated in an AMF for different times at 43, 46 and 50°C. The esophagi were histologically examined after a week to observe whether there was transmural necroses. Then esophageal cancer of rabbit placed nitinol stents were heated in an AMF at 46°C for 10min. Stents were heated to a target temperature within 5min. The highest tolerated temperature and time for the healthy rabbit esophagus was at 46°C for 10min. Tumor growth was delayed by heating and it was statistically significant. Heating the rabbit esophageal wall at 46°C for 10min proves to be safe and effective in delaying tumor growth.

Abstract:e18642 Background: The intraoral stent is a mouth-opening device which may be used in nasopharyngeal carcinoma (NPC) patients during radiotherapy with the intention of preventing unnecessary irradiation in normal adjacent tissue. The purpose of this study was to investigate the effects of individualized intraoral stent on the intensity modulated radiation therapy(IMRT) by evaluation dose distribution and consequent radiobiological derived outcomes for NPC patients. Methods: Sixteen patients with NPC who used intraoral stent and 17 patients without intraoral stent were enrolled in this study. All patients underwent Helical Tomotherapy (HT) in our center. For the planning target volume (PTV), the conformity index (CI) and homogeneity index (HI) were used to compare two groups of HT plans (with or without intraoral stent). The mean dose of normal tissues were recorded for the two groups. Oral mucositis, gustatory sense, mouth opening and salivary function of each patients were weekly assessed at the beginning, during and after radiotherapy by clinical examination. Results: The mean dose metrics of oral cavity, mandible, left and right parotid gland in patients with intraoral stent was significantly decreased by 11.6%, 12.2%, 15.4%, and 8.7% on average, respectively (P < 0.05), while the conformity index (CI, P = 0.056) and homogeneity index (HI, P = 0.676) of the tumor target showed no statistically different. The rates of grade I，II, III of oral mucositis of patients with intraoral stent were 43.15%, 43.75%, 12.50% when radiotherapy was just over. While the rates of patients with intraoral stent were 23.53%, 17.65%, 58.82%. There were significantly different in two groups (p = 0.022). There was no significant difference in the decline of taste function among each groups. But the symptoms of xerostomia and mouth opening restriction in the group with intraoral stent had been relieved significantly compared the group without intraoral stent at 1 month and 6 months after radiation. Conclusions: The custom-made intraoral stents showed promising value at sparing normal tissue during radiotherapy for NPC without affecting target dose coverage or tumor control. Abstract:Purpose Despite a rise in clinical use of radiopharmaceutical therapies, the biological effects of radionuclides and their relationship with absorbed radiation dose are poorly understood. Here, we set out to define this relationship for Auger electron-emitters [ 99m Tc]TcO 4 and [ 123 I]I, and β − -particle-emitter [ 188 Re]ReO 4 . Studies were carried out using genetically-modified cells that permitted direct radionuclide comparisons. Methods and Materials Triple-negative MDA-MB-231 breast cancer cells, expressing the human sodium/iodide symporter (hNIS) and green fluorescent protein (GFP; MDA-MB-231.hNIS-GFP) were used. In vitro radiotoxicity of [ 99m Tc]TcO 4 , [ 123 I]I and [ 188 Re]ReO 4 was determined using clonogenic assays. Radionuclide uptake, efflux, and subcellular location were used to calculate nuclear-absorbed doses using the Medical Internal Radiation Dose formalism. In vivo studies were performed using female NSG mice bearing orthotopic MDA-MB-231.hNIS-GFP tumors and compared to X-ray-treated (12.6-15 Gy) and untreated cohorts. Absorbed dose per unit activity in tumors and NIS-expressing organs were extrapolated to reference human adult models using OLINDA/EXM®. Results [ 99m Tc]TcO 4 − and [ 123 I]I reduced the survival fraction only in hNIS-expressing cells, whereas [ 188 Re]ReO 4 reduced survival fraction in hNIS-expressing and parental cells. [ 123 I]I required 2.4-fold and 1.5-fold lower decays/cell to achieve 37% survival compared to [ 99m Tc]TcO 4 − and [ 188 Re]ReO 4 , respectively, following 72 hours incubation. Additionally, [ 99m Tc]TcO 4 − , [ 123 I]I and [ 188 Re]ReO 4 had superior cell killing effectiveness in vitro compared to X-rays. In vivo , X-ray led to a greater median survival compared to [ 188 Re]ReO 4 and [ 123 I]I (54 days versus 45 and 43 days, respectively). Unlike the X-ray cohort, no metastases were visualized in the radionuclide-treated cohorts. Extrapolated human absorbed doses of [ 188 Re]ReO 4 to a 1 g tumor were 13.8-fold and 11.2-fold greater than for [ 123 I]I in female and male models, respectively. Conclusions This work reports reference dose-effect data using cell and tumor models for [ 99m Tc]TcO 4 , [ 123 I]I, and [ 188 Re]ReO 4 , for the first time. We further demonstrate the tumor controlling effects of [ 123 I]I, and [ 188 Re]ReO 4 in comparison to EBRT.
