Abstract:Purpose: To provide an overview of an innovative surgical approach to guided bone regeneration, the Pack Into Bone (PIB) technique. Methods: Twenty subjects, eight men and 12 women, aged 34 to 68 (mean 51.5) were selected. They were treated with the PiB technique, and upon re-entry surgery, performed 8 months postoperatively, implants were placed, and bone samples were collected by using trephine burs for histological and histomorphometric analysis purposes. Results: Mean ridge width varied between 4.71 (W0) mm and 7.37 mm (W1), measured 8 months postoperatively, with a mean W augmentation of 2.69 mm ± 0.19. Mean height ranged between 8.0 mm (H0) and 11.15 mm (H1), measured 8 months after surgery with a mean augmentation of 3.53 mm ± 0.14 (P< 0.05). Clinical significance: Stability and versatility are the pillars of the PIB technique compared to common GBRs. The PIB technique is effective and reduces the difficulty in stabilizing the graft, is more feasible for operators, and decreases morbidity for the subjects.
Horizontal ridge augmentation with a resorbable membrane and particulated autogenous bone with or without anorganic bovine bone-derived mineral: a prospective case series in 22 patients

Abstract:Purpose: This prospective case series evaluated the use of a new synthetic resorbable membrane with autogenous bone, either alone or in combination with anorganic bovine bone-derived mineral, for horizontal ridge augmentation and subsequent implant placement. Materials and methods: Particulated autogenous bone, either alone or in combination with anorganic bovine bone-derived mineral, was used for lateral ridge augmentation and covered with a new synthetic resorbable membrane (glycolide and trimethylene carbonate) to treat knife-edged ridges and prepare them for implant placement. Ridge measurements were obtained before and after augmentation, complications were recorded, and biopsy specimens were examined histologically. Results: Fifty-eight implants were placed in 22 patients with 25 surgical sites in knife-edged ridges. No complications were associated with this treatment. Clinical measurements revealed an average of 5.56 mm (± 1.45 mm) of lateral ridge augmentation after an average of 8.12 months (± 2.32 months) of graft healing. Clinically, all treated ridges were sufficient in width for subsequent implant placement. All implants have survived, with an average follow-up period of 45.88 months (± 12.43 months). Histologic analysis of the selected augmentation sites showed new bone formation and good incorporation of the bovine bone mineral particles. Conclusion: The high implant survival rate and the low complication rate show the potential of this technique for the treatment of horizontal augmentation of lateral ridges and the efficacy of the new resorbable synthetic membrane.
Horizontal Guided Bone Regeneration of the Posterior Mandible to Allow Implant Placement: 1‐Year Prospective Study Results

Abstract:Objective Assess whether horizontal ridge augmentation with guided bone regeneration (GBR) using deproteinized bovine bone mineral (DBBM), autologous bone, and a resorbable collagen membrane supports successful implant placement. Materials and Methods This open, prospective, single‐cohort, multicenter clinical study included patients with ridge defects that required GBR prior to implant insertion. The primary endpoint was radiologically assessed bone gain after 8 months post‐GBR, measured at the center of planned implant sites. Secondary endpoints included implant survival and success, marginal bone levels (MBLs), MBL changes, and soft tissue health. Results Of 45 patients evaluated 8 months post‐GBR, nine experienced dehiscence in the first 3 weeks of the healing period. GBR led to radiologically determined mean bone width gain of 4.0 ± 1.5 mm and 4.8 ± 1.7 mm, measured 1 and 3 mm from the top of the crest, respectively, allowing successful implant placement in 44 patients (97.8%). The cumulative implant survival and success rates were 98.9% and 95.5%, respectively. MBLs were stable: −1.18 ± 0.64 mm at definitive prosthesis placement (DPP) and − 1.07 ± 0.74 mm at 1 year. Soft tissue health and esthetics (plaque and bleeding indices, papilla, keratinized mucosa, and pink esthetic score) improved from DPP to 1 year. Patients were highly satisfied with implant function and esthetics, and their oral health‐related quality of life improved. Conclusions GBR using DBBM and a collagen membrane offered a safe and effective treatment option for horizontal ridge augmentation sufficient to support implant‐based tooth rehabilitation. Abstract:Background: The purpose of this study was to compare clinical results of two different horizontal ridge augmentation techniques: guided bone regeneration with sticky bone (SB) and the bone-shell technique (BS). Methods: Records of patients who underwent horizontal ridge augmentation with SB (test) and BS (control) were screened for inclusion. Pre-operative and 6-month post-operative ridge widths were measured on cone beam computer tomography (CBCT) and compared. Post-operative complications and implant survival rate were recorded. Results: Eighty consecutive patients were included in the present study. Post-operative complications (flap dehiscence, and graft infection) occurred in ten patients, who dropped out from the study (12.5% complication rate). Stepwise multivariate logistic regression analysis showed a significant inverse correlation between the occurrence of post-operative complications and ridge width (p = 0.025). Seventy patients (35 test; 35 control) with a total of 127 implants were included in the final analysis. Mean ridge width gain was 3.7 ± 1.2 mm in the test and 3.7 ± 1.1 mm in the control group, with no significant difference between the two groups. No implant failure was recorded, with a mean follow-up of 42.7 ± 16.0 months after functional loading. Conclusions: SB and BS showed comparable clinical outcomes in horizontal ridge augmentation, resulting in sufficient crestal width increase to allow implant placement in an adequate bone envelope.

Abstract:OBJECTIVE To evaluate the diagnostic efficacy of single-pass contrast-enhanced multidetector CT (CE-MDCT) performed with a low-radiation high-contrast (LR-HC) dose protocol in selected patients with non-traumatic acute bowel disease. METHODS 65 (32 males, 33 females; aged 20-67 years) consecutive patients with non-traumatic acute bowel disease underwent single-pass CE-MDCT performed 70-100 s after i.v. bolus injection of a non-ionic iodinated contrast medium (CM) (370 mgI ml-1). In 46 (70%) patients with a clinical and/or ultrasonographic suspicion of inflammatory bowel disease, up to 1.2-1.4 l of a 7% polyethylene-glycol solution was orally administered 45-60 mins prior to the CT examination. Patients were then divided into two groups according to age: Group A (20-44 years; n = 34) and Group B (45-70 years; n = 31). Noise index (NI) and CM dose were selected as follows: Group A (NI = 15; 2.5 ml kg-1) and Group B (NI = 12.5; 2 ml kg-1). All patients of Group A underwent thyroid functional tests at 4-6 weeks. Final diagnoses were obtained by open (n = 12) or laparoscopic surgery (n = 4), endoscopy w/without biopsy (n = 24) and clinical (n = 19) and/or instrumental (ultrasonography) (n = 6) follow-up at 11 ± 4 months (range 6-18 mo.). Statistical analysis was performed by χ2 and Student's t-test for categorical and continuous variables, respectively. RESULTS Sensitivity and specificity were 91.3 vs 95.4% (p = 0.905) and 90.9 vs 88.8% (p = 0.998) with an overall diagnostic accuracy of 91.1 vs 93.5% (p = 0.756), whereas the radiation (in millisievert) and CM dose (in millilitre) were 7.5 ± 2.8 mSv and 155 ± 30 ml for Group A and 14.1 ± 5.3 mSv and 130 ± 24 ml for Group B (p < 0.001), respectively. No patients of Group A showed laboratory signs of thyrotoxicosis at follow-up. CONCLUSION The LR-HC has proved to be a safe and a dose-effective protocol in the evaluation of selected young patients with non-traumatic acute bowel disease. Advances in knowledge: (1) As reaching the highest diagnostic benefit to risk ratio (AHARA) appears to be the current principle of MDCT imaging, an increased amount of iodinated CM (0.7-0.9 gI ml-1) can be safely administered to young patients (<40 years) with normal thyroid and renal function to compensate for the lower image quality resulting from low-dose CT protocols performed with the standard filter back-projection algorithm. Such an approach will result in a significant reduction of the radiation dose, which could be otherwise achieved only using iterative reconstruction algorithms combined with either low tube voltage and/or low tube current protocols. (2) An optimal scan delay (Tdelay) for a venous phase caudocranial acquisition can be calculated by the following formula: Tdelay = CI + 25 - TSD, where CI is the duration of the contrast injection, 25 is the average of the sum of abdominal aortic and peak hepatic arrival times and TSD is the scan duration. With such an approach, the radiation exposure resulting from bolus tracking, albeit performed with low-dose scans, can be spared in patients with normal transit times.
Alveolar regeneration of the post-extraction site with cortical deficiency using the Lamina Socket Sealing technique: A retrospective study with clinical, radiographic and histomorphometric analysis. Part 1/2

Abstract:Purpose: This retrospective observational study aims to evaluate the outcome of a new alveolar ridge preservation (ARP) technique, "Lamina Socket Sealing" (LSS) technique, using a porcine barrier. Methods: 36 subjects with maxillary premolars to be extracted and extensive alveolar wall defects were enrolled and treated. Porcine-derived barriers and mesenchymal membrane were used to seal the extraction socket with alveolar particulate graft. The outcome variables were: radiographical bone changes and clinical outcomes. Results: 36 subjects, 21 females and 15 males were treated. The change in mm (mean + SE) of horizontal ridge width from baseline to 4 months after surgery at the coronal level of the alveolus was +1.25 ± 0.20 mm (P< 0.05), mean level was +0.34 ± 0.1 mm (P< 0.05) and at the apical level was +0.08 ± 0.17 mm (P> 0.05) which was not statistically significant. The buccal and palatal height changes in mm (mean + SE) from baseline to 4 months after surgery were respectively 1.21 ± 0.17 mm for the buccal wall (P< 0.05) and 1.06 ± 0.24 mm for the palatal wall (P< 0.05). The bone volume of all sites allowed for the placement of the planned implants. Clinical significance: The alveolar ridge preservation technique (Lamina Socket Sealing) using resorbable heterologous cortical lamina with flapless approach allowed not only the preservation of the damaged post-extraction socket, but also minimal ridge augmentation, according to the principles of guided bone regeneration.

Abstract:The rapidly growing wealth of structural and functional information about kinase genes and kinase inhibitors that is fueled by a significant therapeutic role of this protein family provides a significant impetus for development of targeted computational screening approaches. In this work, we explore an ensemble-based, protein-centric approach that allows for simultaneous virtual ligand screening against multiple kinase genes and multiple kinase receptor conformations. We systematically analyze and compare the results of ligand-based and protein-centric screening approaches using both single-receptor and ensemble-based docking protocols. A panel of protein kinase targets that includes ABL, EGFR, P38, CDK2, TK, and VEGFR2 kinases is used in this comparative analysis. By applying various performance metrics we have shown that ligand-centric shape matching can provide an effective enrichment of active compounds outperforming single-receptor docking screening. However, ligand-based approaches can be highly sensitive to the choice of inhibitor queries. Employment of multiple inhibitor queries combined with parallel selection ranking criteria can improve the performance and efficiency of ligand-based virtual screening. We also demonstrated that replica-exchange Monte Carlo docking with kinome-based ensembles of multiple crystal structures can provide a superior early enrichment on the kinase targets. The central finding of this study is that incorporation of the template-based structural information about kinase inhibitors and protein kinase structures in diverse functional states can significantly enhance the overall performance and robustness of both ligand and protein-centric screening strategies. The results of this study may be useful in virtual screening of kinase inhibitors potentially offering a beneficial spectrum of therapeutic activities across multiple disease states.
One-stage horizontal guided bone regeneration with autologous bone, anorganic bovine bone and collagen membranes: Follow-up of a prospective study 30 months after loading

Abstract:Purpose: To present the medium-term results of one-stage guided bone regeneration (GBR) using autologous bone and anorganic bovine bone, placed in layers, in association with resorbable collagen membranes, for the reconstruction of horizontal bony defects. Materials and methods: This study was designed as an uncontrolled prospective study. Partially edentulous patients, having less than 6.0 mm and more than 4.0 mm of residual horizontal bone width were selected and consecutively treated with simultaneously implant installation and bone regeneration by using 2.0 mm of autologous bone and 2.0 mm of anorganic bovine bone that was placed in layers and then covered with a resorbable collagen membrane. Outcome measures were: implant and prosthesis failures, any complications, peri-implant marginal bone level changes, probing pocket depth (PPD) and bleeding on probing (BOP). Results: In total, 45 consecutive patients (20 male, 25 female) with a mean age of 52.1 years each received at least one GBR procedure, with contemporary placement of 63 implants. At the 3-year follow-up examination, no patient had dropped out and no deviation from the original protocol had occurred. No implant or prosthesis failed. In six patients (13.3%) the collagen membrane was slightly exposed 1 to 2 weeks after bone reconstruction. Four of these patients were moderate smokers. Post-hoc analysis using Fisher's exact test found significant association (P = 0.0139) between a smoking habit and early membrane exposure. Mean marginal bone loss experienced between initial loading and 30 months afterwards was 0.60 ± 0.20 mm (95% CI 0.54 - 0.66). The mean BOP values measured at the definitive restoration delivery were 1.23 ± 0.93, while 2 years later they were 1.17 ± 0.78. The difference was not statistically significant (-0.06 ± 0.76; P = 0.569). The mean PPD values measured at the definitive restoration delivery were 2.62 ± 0.59 mm, while 2 years later they were 2.60 ± 0.54 mm. The difference was not statistically significant (-0.03 ± 0.62; P = 0.765). Conclusions: Within the limitations of the present study, the use of a 2.0 mm layer of particulated autologous bone on the implant threads, and a 2.0 mm layer of anorganic bovine to cover the resorbed ridge, in combination with the resorbable collagen membrane, seems to be a viable treatment option for the reconstruction of horizontal bony defects.
Kinetic disposition of flunixin meglumine in the camel (Camelus dromedarius).

Abstract:The kinetic disposition of flunixin meglumine was investigated in 5 camels after a single intravenous administration (1.1 mg/kg bwt). The plasma clearance was 1.012 ml.min-1.kg-1, the steady-state volume of distribution 0.168 l/kg and the elimination half-life 2.51 h. The results are discussed and compared with those reported in cattle and horses.

Abstract:BACKGROUND: Implant rehabilitation of posterior mandibular defects is frequently associated to a horizontal bone loss. There exist several regenerative techniques to supply this bone deficiency, one of which is the Periosteal Pocket Flap Technique (PPF) proposed by Steigmann et al. to treat small horizontal bone defects. The present study proposes a modification of this technique based on the concurrent use of PPF with the use of xenogeneic and autologous bone and Plasma Rich in Growth Factors (PRGF). The aim of this study is to evaluate clinical and radiographic outcomes of the PPF with the use of xenogeneic and autologous bone and PRGF in comparison with conventional Guided Bone Regeneration (GBR) procedures.METHODS: Nine patients were enroled in the study (7 women and 2 men, mean age: 53 ± 2.74 years) and allocated to PPF or GBR. In both groups implant placement was performed simultaneously to bone regeneration. Preoperative CBCT scans were performed for each patient. Surgical time and postoperative pain were recorded, as well as tissue healing. Moreover, horizontal bone gain (mm), graft surface area (mm2) and graft volume (mm3) were evaluated.RESULTS: Nine surgeries were performed: 6 PPF and 3 GBR. Regarding clinical outcomes, operative time was significative greater in GBR group than in PPF group (51.67 ± 3.51 min vs. 37 ± 5.69 min; p = 0.008). Postoperative pain was higher in GBR compared to PPF (p = 0.011). Regarding radiographical results, there were not significant differences in horizontal bone gain (PPF: 9.43 ± 1.8 mm; GBR: 9.28 ± 0.42 mm), surface area (PPF: 693.33 ± 118.73 mm2; GBR: 655.61 ± 102.43 mm2), and volume (PPF: 394.97 ± 178.72 mm3; GBR: 261.66 ± 118 mm3) between groups.CONCLUSIONS: This prospective study demonstrates that the combination of autograft/xenograft and PRGF in PPF technique is a simpler, cheaper, and faster technique than GBR technique for achieving moderate lateral bone augmentation in implant treatment. Future randomised clinical studies are needed to confirm the results.

Abstract:Background/Objectives: Various bone substitutes have been recommended to augment the horizontal gap following immediate implantation. The purpose of this study was to compare the effectiveness of an autogenous mineralized dentin graft or a deproteinized bovine bone in horizontal gap augmentation following immediate implant placement in the maxillary anterior region. Methods: A total of 110 patients underwent tooth extraction followed by immediate implant placement. The patients were divided into two groups. The first group received an autogenous mineralized dentin graft (the test group) while the second group received a deproteinized bovine bone (the control group) to augment the horizontal gap. Preoperative (T0), immediate postoperative (T1), and 1-year postoperative (T2) cone beam computed tomography scans were taken from all the patients. Linear measurements were recorded 1 mm (R1) and 5 mm (R2) points apical to the implant platform at both T1 and T2 time intervals. Pink Esthetic Scores and prosthetic complications were evaluated as well. Results: There were 57 patients with a mean age of 45.42 ± 9.86 (range 24–63 years) selected as the test group and 53 patients with a mean age of 40.28 ± 11.69 (range 20–63 years) as the control group. The mean reduction in the buccal bone plate at R1 was 6.39 ± 3.78% in the test group and 6.99 ± 5.01% in the control group (p > 0.05). The mean reduction in the buccal bone plate at R2 was 5.46 ± 4.98% in the test group and 6.77 ± 7.60% in the control group (p 0.05). Conclusions: The efficiency of using an autogenous mineralized dentin graft for horizontal gap augmentation showed similar results in comparison to using a deproteinized bovine bone in relation to buccolingual socket reduction following immediate implantation.

Abstract:Objective: To evaluate the efficacy and long-term outcome of the bilaminar cortical wall building grafting technique for reconstruction of vertical and horizontal alveolar ridge defects in the esthetic zone. Methods: The retrospective study was conducted between January, 2007 and December, 2015. The sample was composed of 24 patients who received bilaminar cortical wall building augmentation in the esthetic zone in Department of Fourth Clinical Division, Peking University School of Stomatology. The 24 patients (11 female and 13 male) had age of (37.8±13.4) years (20-54 years). A bone block harvested from the lateral aspect of the mandibular ramus was bisected into two cortical laminae, which were then used to reconstruct the buccal and palatal walls of an alveolar ridge defect. The inter-laminar space was filled with particulate autogenous bone and the whole graft was covered with anorganic bone graft and collagen membrane. Bone width and vertical measurements were measured at the time of surgery and reentry surgery. Integration of the graft, implant survival rates and complications were recorded. Results: Overall, 24 patients with 35 sites who were included were followed for (7.1±1.9) years. Graft integration (24/24) and implant survival rates (35/35) were determined as 100%. The average horizontal and vertical bone gain was (6.47±2.46) and (5.01±1.12) mm with resorption rates of 9.0% and 10.9%, respectively. One patient showed soft-tissue dehiscence with peri-implant mucositis were observed 9 years after surgery. More than 1.5 mm of buccal bone plate were observed using cone beam computed tomography. Conclusions: This technique was effective and reliable for three-dimensional reconstruction of severely atrophic alveolar ridges in anterior maxillae. Autogenous graft combined with coverage by allogeneic bone graft and collagen membrane provided successful augmentation with low bone resorption.

Abstract:PURPOSE: Inadequate bone width in atrophic edentulous jaws is a challenge for successful oral rehabilitation with endosseous dental implants. The aim of this clinical study was to evaluate the effectiveness of a new method for ridge expansion with sagittal splitting using a new surgical device (Crest-Control Bone Splitting System, Meisinger) and to determine whether it is necessary to fill the expansion area with bone substitute to maintain the expanded bone volume.MATERIALS AND METHODS: During a 3-year period, a prospective study was performed in 32 patients (59% women, 41% men). All participants needed implants in the horizontally atrophied edentulous mandible and were treated in a private practice and a hospital. The only inclusion criteria were a mandibular ridge width between 3 and 4 mm and ridge height of at least 11 mm. Expansion with horizontal splitting of the ridge was performed simultaneously with implant placement. In 17 of the 32 arches, selected alternately, the expanded ridges were filled with a biphasic calcium phosphate (hydroxyapatite 60% and beta-tricalcium phosphate 40%) synthetic bone substitute (SBS 60/40). The other 15 expanded arches were left unfilled. All areas were covered with a resorbable collagen membrane (Bio-Gide, Geistlich). Results were analyzed with the Mann-Whitney and Kruskal-Wallis tests (α=.05).RESULTS: There was a significant difference (α=.02) between the patients who received SBS 60/40 (17 cases) and those who did not (15 cases). The ridges that received SBS 60/40 after expansion showed no bone resorption.CONCLUSION: Horizontal expansion of the ridge is easily reproducible. In this study, in very narrow ridges, a lack of bone substitute resulted in significant resorption of 3- to 4-mm-wide crests (5%), even after expansion. A bone substitute should be placed to maintain the alveolar bone walls after expansion.

Abstract:This prospective study examines whether a patient medication instruction sheet (PMI) given to clinic patients by their health care provider affects knowledge and/or attitudes with thiazide diuretic use as part of an antihypertensive regimen. Adult male patients ( N = 285) in a general medicine clinic were assigned to groups receiving the American Medical Association PMI describing their diuretic. Patients getting the PMI obtained it either directly from their provider or at the pharmacy dispensing window. All patients were surveyed by phone 1 week following the clinic visit with regard to the PMI, knowledge of medication use, and attitudes toward drug use. Results indicate that a provider-dispensed PMI results in higher levels of drug knowledge and greater patient satisfaction with their knowledge than a pharmacy-dispensed PMI. In addition, the PMIs educational value may be lessened by an incomplete verbal consult. This study demonstrates that the AMA PMI is an effective educational tool when distributed by a provider and can promote better understanding and use of prescribed medications.

Abstract:Among different therapeutic strategies proposed in the case of bone volume deficit, guided bone regeneration (GBR) is a consolidated surgical procedure. The objective of this study is to retrospectively evaluate the behavior of two bone grafts with different consistencies in the GBR procedure by measuring the volumetric tissue changes 1 year after surgery. For this retrospective analysis, 25 cases of GBR with simultaneous implant insertion were selected. A total of 13 were grafted with a porcine cortico-cancellous bone mix (CCBM group), and 12 were grafted with a pre-hydrated granulated cortico-cancellous bone mix of porcine origin blended with 20% TSV gel (Collagenated-CCBM). A collagen membrane was fixed to cover the bone defect. A total of 42 implants were placed with computer-guided surgery. Preoperative and 12-month postoperative digital impressions were used to evaluate dimensional changes. Student's t-test used for independent samples showed no statistically significant differences between the integrated distance (p = 0.995) and mean distance (p = 0.734). The mean integrated distance in the CCBM group was 41.80 (SD. 101.18) compared to a mean of 42.04 (SD. 66.71) in the Collagenated-CCBM group. Given the limitations of this study, in patients with peri-implant bone dehiscence, simple heterologous and collagenated heterologous cortico-cancellous bone grafts are suitable for filling the bone defect to promote bone regeneration, although further studies are needed.

Abstract:Aims (1) To assess the effectiveness of the Sausage TechniqueTM when applied for lateral bone augmentation by multiple experienced clinicians; (2) To identify risk indicators for a poor outcome and to assess the need for adjunctive surgery. Materials and Methods All patients who had been treated with the Sausage TechniqueTM for lateral bone augmentation by three experienced surgeons between January 2019 and December 2021 were included in a retrospective case series. The Sausage TechniqueTM technique includes the use of autogenous bone chips and deproteinized bovine bone mineral (1:1 ratio), covered with a stretched and pinned collagen membrane. The increase in alveolar width between the pre‐operative situation and 9 months was assessed at different levels on superimposed cone‐beam CT scans. Results Twenty‐five augmentations performed in 25 patients (17 males, 8 females, mean age 51 years) were available for evaluation. Abstract:In this study, the clinical outcomes of horizontal ridge augmentation using half-columnar bone grafts from the ramus (group I: 27 patients, 32 implants) versus rectangular bone grafts from the symphysis (group II: 19 patients, 27 implants) were compared; grafts were combined with organic bovine bone and collagen membrane. Cone beam computed tomography images were obtained preoperatively, immediately after restoration (baseline), and 1 year after loading. Four months after grafting, horizontal bone resorption at the alveolar crest did not differ significantly between the two groups (P=0.291). At 4mm apical to the alveolar crest, horizontal bone resorption in group I was significantly less than that in group II (P=0.041). One year after loading, horizontal bone resorption in group I was lower than that in group II, with no significant difference. The residual thickness of the labial bone at the implant site in group I was significantly higher than that in group II. Horizontal ridge augmentation with either a half-columnar autogenous graft from the ramus or a rectangular autogenous graft from the symphysis can provide acceptable results in aesthetic regions. The half-columnar group demonstrated better graft stability both at 4 months after augmentation and 1 year after loading.
