Abstract:The purpose of this research was to examine whether demographic variables, personality traits, and workplace variables (working in shifts, job tenure, and perceived organizational justice) contribute the most to the prediction of job satisfaction in nurses. The survey included 161 nurses. The instruments used in this research were as follows: the Demographic Data Questionnaire, the Perceived Organizational Justice Scale, the Job Satisfaction Scale, and the NEO five-factor inventory. The study findings indicated that age, health status, distributive justice, and procedural justice positively contribute to job satisfaction among nurses, while neuroticism contributes negatively. Older nurses, those in better health, those who are satisfied with the organization's decision-making process, and those who feel adequately rewarded for their contributions tend to be more satisfied with their jobs. Conversely, nurses with a higher level of the neuroticism personality trait tend to be less satisfied with their job. The strongest predictors of job satisfaction among nurses were found to be health status, the personality trait of neuroticism, and distributive and procedural justice, with the age of nurses being slightly less powerful but still significant.

Abstract:Background: The way the nursing profession is perceived plays a pivotal role in shaping the public's attitude toward its members, primarily influenced by interactions with nurses and media representation. However, it can be significantly impacted by historical and traditional stereotypes, coupled with enduring gender bias and other preconceived notions. Objectives: The study aimed to examined how the title "nurse" reflects the profession's identity, perception as a female occupation, and status as a healthcare activity. It also assessed awareness of nurses' roles, education and public presence, comparing general population and patients' opinions in the Republic of Croatia. Methods: Conducted as a cross-sectional study, this research employed a structured questionnaire and a convenience sampling approach. It encompassed both the general population and patients from Croatia, with data collection spanning from February 15, 2023, to March 16, 2023, and resulting in a total sample size of 624 participants. Statistical analysis included both descriptive and inferential techniques, and significance was set at p < .01 (two-tailed). Results: A substantial proportion of respondents, amounting to 38.1%, correlate the term "nurse" with the role of the physician's assistant, whereas only 49.5% associate it with a true professional functioning within their field of work. Furthermore, 73.7% study participants perceive the nursing profession as being desirable for men. Upon examining the perspectives of the general population and patients, statistically significant differences emerged in terms of their perceptions regarding the perceived work scope (p < .001) and the level of sufficient educational attainment (p < .001) of nursing professionals. Conclusion: The analysis uncovers varying perceptions of the nursing profession in the Republic of Croatia, as well as evolving perceptions of gender diversity within nursing roles. Differing viewpoints between the general population and patients in this study emphasize the need for tailored education and communication initiatives to improve the image of nursing.
Attitudes Towards Older People in Croatian and Slovenian Nursing Students

Abstract:Background: Croatia and Slovenia are neighboring countries with marked differences in high school and undergraduate nursing curricula. The aim was to assess and compare attitudes toward the elderly among undergraduate nursing students in Croatia and Slovenia and identify factors associated with positive/negative attitudes. Subjects and methods: This was a cross-sectional survey conducted between September, 2017 and July, 2018 among undergraduate nursing students at five higher education institutions: three in Slovenia and two in Croatia. The following data were gathered: age, sex, year of study, previous education, employment status, previous education in gerontolgy, desired professional role after graduating, previous experience with the elderly. Kogan`s Attitude Towards Old People Scale (KATOPS) score was the primary outcome measure. Pairwise comparisons were conducted between Croatian and Slovenian students. KATOPS score was dichotomized with values above the 3rd quartile considered excellent - the dichotomized score was used as the dependent variable in a binary logistic regression model. Results: Overall, 825 students completed the questionnaire, 85.5% were women, 417 from Slovenia, 408 from Croatia, and 80% were under 22 years of age (80%). The average score on the KATOPS was 127.4, 95% CI 126.6-128.2. Variables associated with excellent scores on the KATOPS, based on the binary regression were: studying in Slovenia (OR=2.05, 95% CI 1.39-3.03), age group 28-32 years (OR=3.9, 95% CI 1.53-9.98); previous education gerontic nursing (OR=2.45, 95% CI 1.34-4.47), and full-time study (OR=2.12, 95% CI 1.38-3.55). Variables not associated with excellent scores were: being married (OR=0.44, 95% CI 0.22-0.92) and having previous experience in working with the elderly (OR=0.5, 95% CI 0.3-0.86). Conclusion: Attitudes toward old age are mildly positive in Slovenian and Croatian nursing students. Slovenian students have significantly more positive attitudes toward old age and these differences are most probably due to marked differences in undergraduate nursing curricula between the two countries.

Abstract:Introduction: Physical restraint is a frequently debated procedure due to controversial ethical and legal issues related to patient autonomy and dignity. This study aimed to assess the level of knowledge, attitudes and practice of nurses regarding the use of physical restraint measures on patients and to determine whether there is a difference between the level of knowledge, attitudes and practice of nurses regarding the use of physical restraint measures to their sociodemographic and professional characteristics. Material and methods: This study was conducted as a cross-sectional study at three healthcare institutions of different medical care levels (n = 253). Instruments used in this research included a questionnaire to assess knowledge, attitude, intention, and practice and a sociodemographic questionnaire. Descriptive and inferential statistics were used for statistical analysis of the data, and statistical significance was determined at p < 0.05. Results: Nurses have a moderate level of knowledge, a positive attitude, and good practice in applying measures of physical restraint of patients. A significant difference in the level of knowledge regarding the application of measures of physical restraint of patients was determined by the age of the nurses, as well as that nurses with a positive attitude towards the application of measures of physical restraint of patients have a better practice. Conclusion: By implementing an appropriate educational pro-gram, which is certainly mandatory for all employees in the health sector, and creating a guide for the proper use of measures for physical restraint of patients, it is possible to influence the improvement of the quality of care and treatment of patients.

Abstract:Introduction. Patient safety during hospitalisation is one of the biggest concerns for hospitals worldwide and one obligation of all medical professionals is to create a safe environment for patients and prevent accidents. Approximately 50% of adverse events can be prevented with a systematic approach. Aim. This study aimed to examine the underlying dimensions and psychometric properties of the Hospital Survey on Patient Safety Culture questionnaire in Croatian hospital settings, and to compare the results with the original questionnaire from the United States of America. Methods. The sample consisted of 438 nurses from four Croatian university hospitals. All participants signed an informed consent document, and the questionnaires were provided to the nurses in their units by the head nurses of their departments. Data analysis was performed using IBM SPSS 25.0. Bartlett's test of sphericity and the Kaiser-Meyer-Olkin (KMO) indicator were performed with Cronbach’s alpha test and sample standard deviation. Results. All factors explain the total of 59% of variance of the measured questionnaire. Additionally, the reliability of the entire questionnaire was determined by using the internal consistency coefficient (Cronbach’s alpha) on the Hospital Survey on Patient Safety Culture with 12 of 0.88 particles, which is high internal consistency reliability. Our results show that the particles that make up the questionnaire are very homogeneous according to their object of measurement. Conclusion. The results of our study found that the survey can be applied to Croatian settings and used in hospitals. We also believe that more research on this topic is needed and is crucial for improving patient safety in hospitals. It is necessary to emphasize continuous education of nurses regarding patient safety in hospitals.

Abstract:Patient satisfaction is a crucial measure of the quality of healthcare, especially with regard to nursing services in hospital settings. Understanding and increasing patient satisfaction with nursing care is critical to improving overall healthcare and ensuring positive patient experiences during their hospital stay. The aim of this research was to evaluate the psychometric properties of the Croatian version of the Satisfaction with Nursing Care Quality Questionnaire (PSNCQQ), test the reliability and validity of the tool after translation, and investigate differences in patient satisfaction based on demographic variables, as well as their contribution to satisfaction with the quality of nursing care. After translation and adaptation, the Croatian version of the PSNCQQ was applied to 350 hospitalized patients (average age 51.19 years (range: 18–87)), of whom 194 (55.4%) were men and 156 (44.6%) were women. The results showed high internal consistency (Cronbach's α = 0.977) and confirmed the one-factor structure of the questionnaire, explaining 70.64% of the total variance. Confirmatory factor analysis supported the unidimensional model, showing strong fit indices (χ2 = 583.047, df = 149, p < 0.001, GFI = 0.861, AGFI = 0.818, NFI = 0.936, TLI = 0.946, CFI = 0.955, RMSEA = 0.080, PCLOSE < 0.001). In conclusion, the validation of the PSNCQQ in the Croatian language increases resources for evaluating and improving the quality of nursing care. This research lays the foundation for future studies and practical applications aimed at improving patient satisfaction and nursing care outcomes in Croatia, but there are also limitations to this study, including its one-institution scope, the possible influence of factors outside the current treatment on satisfaction, and the lack of comparison with objective clinical indicators.

Proficiency and Practices of Nursing Professionals in Meeting Patients' Spiritual Needs within Palliative Care Services: A Nationwide, Cross-Sectional Study

Abstract:Spirituality and spiritual needs are integral parts of the human experience, but they are often particularly important for palliative care patients. Spirituality has numerous positive effects, especially for those dealing with serious illness. Nevertheless, the spiritual dimension is sometimes overlooked in patient care. This study aims to determine the frequency of addressing the spiritual needs of palliative care patients in Croatia and to investigate the self-perceived confidence of caregivers in this task. A quantitative cross-sectional study was conducted involving 194 nurses in specialised palliative care services across Croatia. A specially developed and validated questionnaire was used for this study. The most common intervention undertaken by respondents was "promoting hope and optimism in patients" (88.4%), while the least common intervention was "reading books and other publications to patients" (13.9%). No statistically significant differences were found in the frequency of spiritual care in relation to the respondent's level of education, professional experience and nursing environment. Approximately two-thirds of the surveyed nurses stated that they "often" or "always" provided some kind of spiritual care to palliative care patients. However, study participants who indicated that they had received sufficient formal instruction in addressing spiritual needs and spiritual care interventions demonstrated a statistically significant tendency to engage in these practices, as well as greater confidence in their knowledge and skills in this area compared to those who lacked such training. The study suggests that there is a need to identify existing barriers to the provision of spiritual care and to develop strategies to overcome them. By placing emphasis on the spiritual needs and preferences of patients, nursing professionals and other healthcare providers have the opportunity to elevate the standard of holistic care and foster a sense of comfort and dignity among patients.

Abstract:This study tests the link between internal procedural justice (fair treatment of police officers by supervisors) and external procedural justice (fair treatment of citizens by police officers), proposed by Van Craen and previously tested only in a few Western democracies and Asian countries. This paper relies on a survey of police officers from Croatia, an East-European country in transition, and expands the model by incorporating the potential effect of the police officers’ support for community policing on the fair treatment of citizens. Our model, based on the data from a 2017 survey of 638 Croatian police officers, demonstrated that the latent variable internal procedural justice had a modest, positive association with trust in the public, while the latent variable of community policing values did not have a significant association with trust in the public. The antecedent variable community policing values serves as the model’s strongest predictor of external procedural justice.

Abstract:Introduction The safety and quality of patient care are basic guidelines in finding new and improved solutions in nursing. Important and influential factors shape the nurses’ work environment in hospitals. Purpose With the study, we intended to investigate whether the perception of nurses’ work environment is related to the safety culture and the quality of patient care and whether it differs according to nurses’ level of education. Methods of work The study with a quantitative research method was conducted at the six clinical departments of the University Medical Centre, Ljubljana in 2019. We used a survey questionnaire from the European survey Nurse forecasting in Europe (RN4CAST). Results 270 nurses were included in the study. The response rate was 54%. The study confirmed that there is a correlation between the assessment of the nurses’ work environment and the general assessment of patient safety (r = 0.36; p <0.001), the general assessment of the quality of nursing care (r = 0.32; p <0.001), the confidence in patient self-care at discharge (r = 0.29; p <0.001) and the quality of patient care in the previous year (r = 0.27; p = 0.001). The results showed frequent verbal abuse of nurses, in 44.9% by patients and their relatives and in 35.4% by staff. Graduate nurses rated the work environment more negatively than healthcare technicians (p = 0.003). Discussion and conclusion We confirmed the correlation between the assessment of nurses’ work environment and patient safety and the quality of health care, and that employees’ education influences the assessment and perception of the work environment.

Organizational hierarchies in Bulgarian hospitals and perceptions of justice

Abstract:Objectives: Health care reform in Bulgaria has been ongoing for two decades. Since 1990, it has been transforming from a socialized system of medical care with free access, to one which is decentralized, includes private health care services, the general practitioner model and a National Health Insurance Fund. In this context, we are conducting an international EC Framework 7 project: 'Improving quality and safety in the hospital: The link between organizational culture, burnout, and quality of care'. We focus on health professionals' perceptions of organizational hierarchies in Bulgarian hospitals and how doctors and nurses connect these to organizational justice. Methods: We conducted seven focus groups and four interviews, with a total of 42 participants (27 nurses, 15 physicians and medical residents) in three hospitals. Data were analysed through thematic analysis and discourse analysis with Atlas.ti. Results: From the perspective of health professionals, health reform has intensified traditional hierarchies and inequalities and has created new ones in Bulgarian hospitals. These hierarchies are continuously (re)constructed through language and practices and also destabilized through resistance. The health professionals protest fact that these hierarchies are permeated with unfairness and silence voices. All health professions (nurses, doctors, residents) in our study experience being unjustly positioned and disempowered in various hierarchies. They connect these experiences to stress and anxiety. Conclusions: Participatory action research needs to address multiple dimensions of organizational relationships in Bulgarian hospitals, including hierarchical relationships and ways of promoting organizational justice. Statement of contribution: What is already known on this subject? Health care organizations are hierarchically organized. Organizational injustice can contribute to burnout in health professionals. There is a high level of stress and burnout for health professionals in Bulgaria. What does this study add? This study adds understanding of changing hierarchies in hospitals during health care reform in the post-socialist period. Illuminates how health professionals' discourse sustains and resists hierarchical relationships in Bulgarian hospitals. Adds understanding of health professionals' perspectives on implications of injustice for their well-being.

Abstract:We study a one-dimensional Josephson-junction chain embedded in a magnetic field. We show that, when the magnetic flux per elementary loop equals half the superconducting flux quantum phi0 = h/2e, a local Z2 symmetry arises. This symmetry is responsible for a nematic Luttinger liquid state associated with bound states of Cooper pairs. We analyze the phase diagram and discuss some experimental possibilities to observe this exotic phase.

Abstract:Introduction: Burnout syndrome is a growing concern among nursing students, potentially impacting their academic success and future professional performance. This study aimed to explore the prevalence of burnout syndrome in Croatian nursing students and examine the associations between burnout and demographic features (age, gender, year of study, part-time or full-time study). Methods: A quantitative cross-sectional study was conducted with 423 nursing students from the University of Applied Health Sciences, Zagreb, and the Faculty of Health Sciences, Rijeka. The Maslach Burnout Inventory (MBI); MBI human services survey was used to assess burnout levels. Results: Nursing students at the Faculty of Health Sciences, Rijeka, experienced higher burnout levels than those at the University of Applied Health Sciences, Zagreb (χ2 = 10.214, ss = 2, p = 0.006). Significant associations were found between burnout levels and age, gender, year of study, and enrolment status. Specifically, younger students, female students, and full-time students reported higher levels of emotional exhaustion (EE). 2nd-year students reported lower EE than their first and 3rd-year peers. Conclusions: This study underscores the importance of recognizing and addressing the unique needs and stressors faced by different subgroups of nursing students. Tailored interventions and support systems are essential for alleviating burnout and promoting well-being in nursing students. Further research, including longitudinal studies, is required to better understand burnout progression and to inform the development of effective strategies for reducing burnout in nursing education.
The impact of ethical climate types on nurses' behaviors in Bosnia and Herzegovina

Abstract:Background: The performance of nurses has become vital in hospitals. Some studies have suggested that nurses' perceptions of the ethical climate in their hospitals are related to higher job satisfaction and organizational commitment and in turn lessen the issue of nursing shortage. Hypothesis: (1) The ethical climate types "caring," "independent," "law and code," and "rules" have a significant positive impact on overall job satisfaction. (2) The ethical climate types and overall job satisfaction have significant positive influences on normative and affective and significant negative influences on continuance commitment. Research design: The study uses path analysis to understand which types of ethical climate impact overall job satisfaction. It also tries to find the effect of different types of ethical climate and overall job satisfaction on the components of organizational commitment. The relationships between variables were evaluated using factor analysis, reliability, descriptive statistics, correlations, and regression in this study. Participants and research context: A total of 171 useful questionnaires were collected from nurses working in public and private hospitals in Bosnia and Herzegovina. Ethical considerations: Formal research approval was obtained from the administration of each study hospital. Questionnaires with a cover letter were mailed to the hospitals that agreed to participate in the study. In the cover letter, the researchers explained the study purpose, encouraged nurses' voluntary participation, and guaranteed the anonymity of participants. Findings: In the first path analyses, "rules" and "caring" climates significantly and positively affected overall job satisfaction. In the second one, while overall job satisfaction and "rules" climate significantly influenced normative commitment, "caring" climate and overall job satisfaction significantly affected affective commitment. Discussion: The findings of the study have been convenient with the literature. Conclusion: Public and private hospitals can enhance overall job satisfaction and organizational commitment by altering the ethical climate of organizations. Hospital administrations should nurture caring and rule types of ethical climate which influence overall satisfaction. By this way, they could reduce nursing shortage.

Abstract:Aim: To determine Croatian healthcare (HC) professionals' perspectives on HC management and leadership challenges. Methods: This cross-sectional study, conducted between May and June 2021, enrolled 1179 respondents from both the public and private sector, including medical doctors, nurses, pharmacists, economists, and other HC professionals. Their perspective on various facets of HC management, namely governance, ownership, accountability, financing, and potential for improvement, were investigated using an anonymous online survey. Results: Most respondents agreed that the system may be allocating resources ineffectively and that political mandates unduly influenced management decisions, impeding accountability. Competencies in organizational and financial experience, along with communication and leadership skills, were deemed fundamental for health care managers. Participants overwhelmingly supported data-driven decision-making, improved education, and the development of leadership skills as key avenues for system enhancement. Conclusions: The study underscores the need for better financial management and overall governance, in the Croatian HC, offering insights that can inform evidence-based policy decisions and reforms toward a more efficient and accountable HC system.

Abstract:Equity in the academic context is an important construct that affects the cognitive, emotional, and behavioral reactions of an individual. The study of fairness in the work context is extremely important, considering that a person spends a lot of time in the workplace during his life. Several studies so far have shown that affecting one's perception of equity can affect the individual itself- improving one's productivity, satisfaction, and such. (Galinec 2017). Distributive equity is based on Adams' (1965) equity theory, more specifically on its assumption that people do not care as much about the absolute outcome, compared to the equity of the outcome itself. Regarding stress, the academic staff in Slišković (2012) explored different sources of stress that result in reduced productivity. Some of the universal sources of stress, such as inadequate compensation and recognition within an organization, are linked to distributive equity. The goal of the research was to show if distributive inequity exists within an academic environment and if that inequity reflects stress levels inside that environment. The academic workers who took part in this research are employees of the following higher education institutions: the University of Zenica, the University of Sarajevo, the University “Džemal Bijedić” in Mostar, and the University of Bihać. The inventory contains 21 items. The research took place in July 2022. The descriptive method was used as a scientific research method. The results show that academic workers perceive distributive inequity in certain situations, and it correlates with an increase in their stress levels.

However, most studies on this topic have been conducted in Asian countries, and because there are differences in health care system performance across countries amid a global shortage of nurses, it is necessary to investigate professional quality of life in the European nursing context. Methods: This multicenter, comparative, and correlational study was conducted on a sample of 669 hospital nurses in Serbia and Poland. The survey data were collected between November 2019 and February 2020 using the Demographic and Occupational Questionnaire (DOQ), Professional Quality-of-Life Scale (ProQOL-5), and Satisfaction with Life Scale (SWLS). STROBE Statement guidelines for cross-sectional studies were followed. Results: Most nurses from both countries reported having moderate compassion satisfaction, moderate risk for compassion fatigue, and an average life satisfaction. The predictive factors of life satisfaction that most stand out include country of origin, financial situation, education, job satisfaction, and compassion fatigue. Discussion and conclusion: Quality of professional life is significantly affected by shift duration, age, work experience, and job satisfaction. Negative aspects of professional life are significant predictors of life satisfaction. Implications for nursing policy: The findings of this study indicate a need for implementing flexible shift patterns and enhancing interventions that will improve job satisfaction and compassion satisfaction while reducing the risk of compassion fatigue.

Abstract:This study aims identifying the effects of nurses' perceptions of organizational justice on their performance and to test the mediating role of job satisfaction. This is a cross-sectional study. This study conducted with 374 nurses working hospitals. The research model was tested using structural equation modeling and the bootstrap technique. Organizational justice has been found to have a significant impact on job performance and job satisfaction. Job satisfaction had a partial mediating role. The results of this study could guide the more effective and efficient use of human resources in nursing management and contribute to the literature of health and nursing management.

Abstract:Gender sensitivity, which is the capacity to recognize and address issues of gender discrimination and inequality, is initiated with an awareness of gender differences. This is particularly crucial in nursing, where care is tailored to the holistic needs of individuals. Given the sensitive nature of nursing to gender variances, it is essential that the influences of nurses' own experiences and perceptions on their gender sensitivity are explored. This study is aimed at assessing the effects of childhood experiences of domestic violence and perceptions of sexism among healthcare providers on their gender sensitivity. Additionally, it seeks to provide empirical data to support the enhancement of gender-sensitive practices within nursing environments, thereby fostering a culture of gender equality, and helping to promote the practical application of gender equality within nursing organizations.
