Abstract:Purpose: To determine the changes in surface gloss of different composite materials after laboratory toothbrushing simulation. Methods: 36 specimens were fabricated for each material and polished with 120-, 220-, 500-, 1200-, 2400- and 4000-grit SiC abrasive paper, respectively. Gloss measurements were made with a glossmeter (Novocurve) prior to testing procedures and then subjected to simulated toothbrushing for 5, 15, 30 and 60 minutes by means of an electric toothbrush with a pressure of 2N while being immersed in a 50 RDA toothpaste slurry. Four supplementary samples per group were analyzed under SEM immediately after polishing procedures and four samples after 60 minutes simulated toothbrushing in order to evaluate the causes of the gloss decrease. The tested resin composite materials were Filtek Supreme XTE, Durafill, HRi Enamel Plus, Miris 2, Empress Direct, Venus Diamond, Gradia Direct, Clearfil Photo Posterior and G-aenial. Natural enamel represented the control group. Statistical analysis was performed using Kruskal Wallis and Tukey post-hoc test, with a level of significance set at 0.05. Results: Resin composite initial gloss values ranged from 68.9 to 100.5 at baseline to 10.6 to 62.6 after 1 hour of brushing. Highest gloss values were obtained by Filtek Supreme XTE, followed by Empress Direct and Durafill. Lowest values were obtained by Clearfil Photoposterior, Miris 2, Enamel HRi and Venus Diamond. Natural enamel was the only substrate to maintain its gloss throughout the brushing procedure (110.4 after 60 minutes). SEM analysis revealed different patterns of surface degradation depending on the composite material.

Abstract:Background: Newly introduced bioactive resin composites are being used more often. Few studies have evaluated the influence of toothbrush abrasion on their surface characteristics. Methods: This study aims to assess the surface roughness (Ra) and gloss values (GU) of 3 bioactive composites and a conventional nanohybrid composite before and after simulated toothbrush abrasion. Five samples each of Filtek Z250 XT, Beautifil Flow Plus, Activa Presto and Predicta Bioactive Bulk were subjected to 10,000 cycles of simulated toothbrush abrasion. Ra and GU values were evaluated at baseline and after each 2,500 cycles. Results: Significant difference was found in both Ra and GU (P < 0.001). For each material, GU gradually decreased as the brushing cycles increased. GU values of Beautifil Flow Plus were significantly higher at the baseline, 2,500- and 5,000-cycles stages (P < 0.05). The GU values were 78.20 (7.20), 69.67 (6.17) and 63.30 (5.52) respectively. Activa Presto showed the lowest mean GU after 10,000 cycles. A significant increase in Ra at each of the measuring stages was observed in all materials compared to the baseline measurement (P 0.05). After 7,500 cycles, the mean Ra value of Activa Presto at 0.64 (0.14) was significantly higher than that of Z250 XT at 0.49 (0.03) (P < 0.001). At 10000 cycles, Activa Presto had significantly higher Ra at 0.70 (0.10) when compared to Z250 XT at 0.52 (0.08), Beautifil Flow Plus at 0.56 (0.07) and Predicta Bioactive Bulk at 0.59 (0.10) (P < 0.001). Conclusion: All materials showed an increase in Ra and decrease in GU after simulated toothbrushing.

Gloss Retention on Enamel and Resin Composite Surfaces After Brushing Teeth with Commercial and Modified Dentifrices

Abstract:Objectives: We examined the surface gloss and roughness of a dental composite and human enamel after brushing with a new bioactive glass (BCF201) additive designed to treat dentine hypersensitivity. Methods: We prepared 2 cohorts of samples: a resin-based composite (RBC) and human enamel. Each cohort received 20 000 brushing cycles with Colgate Optic White Enamel (Colgate Optic), Sensodyne Whitening Repair and Protect (Sensodyne), Colgate Enamel Health Sensitivity Relief (Colgate-EN) with and without BCF201 added or Germiphene Gel 7 HT (Gel 7) with and without BCF201 added. The average gloss and roughness of the enamel and RBC surfaces were measured before brushing and after 20 000 back-and-forth brushing cycles. A linear regression function was applied to the gloss results, and the data were analyzed using ANOVA and a Tukey post-hoc test (α = 0.05). Results: After 20 000 brushing cycles, the control (Gel 7) had no significant effect on the gloss or roughness of the RBC. However, the choice of dentifrice had a significant effect on both gloss and roughness (p < 0.001). With respect to RBC, after brushing, surface roughness was ranked from smoothest to roughest: Gel 7 = Gel 7 plus BCF201 > Colgate-EN plus BCF201 = Colgate Optic = Colgate-EN > Sensodyne. With respect to enamel, the smoothest to the roughest surfaces after brushing were: Gel 7 plus BCF201 = Sensodyne = Colgate-EN plus BCF201 > Gel 7 = Colgate Optic = Colgate-EN. Conclusion: The bioactive glass additive had no adverse effect on the surface roughness or gloss of human enamel or RBC. Significance: The addition of BCF201 appears to have a polishing effect on RBC and enamel and reduced the abrasive effects of Colgate-EN on RBC and enamel.

Methodology Composite resin specimens (Aura Bulk Fill, SDI) were subjected to 5,000 brushing cycles with regular toothpaste (Colgate Total 12, [RT]), activated charcoal toothpaste (Bianco Dental Carbon, [AC]), or hydrogen peroxide-containing toothpaste (Colgate Luminous White Advanced, [HP]), with or without coffee exposure. The pH, particle characterization by scanning electron microscopy (SEM), and weight% of solid particles in the toothpaste were assessed. Roughness (Ra) was evaluated using a surface profile-measuring device, color change (∆Eab/∆E00) by reflectance spectrophotometer, and gloss unit (GU) by glossmeter. Kruskal–Wallis, Dunn, Friedman, and Nemenyi tests were used, and the correlation coefficient test was performed between Ra and GU (α=0.05). Results RT presented a higher Ra after brushing and did not change after staining with coffee; moreover, the ∆Eab/∆E00 values were higher for RT than HP. Regarding gloss, AC and HP obtained higher values compared to RT. A significant negative correlation between gloss and Ra was found for RT exposed to coffee. All toothpastes had a neutral pH; however, RT had the largest percentage of solids by weight. SEM images showed particles of many sizes: irregular shape (RT), more regular particles (AC), and spherical clusters (HP). Although surface roughness, color change, and gloss may compromise the longevity of restorations, the whitening toothpastes tested did not promote more morphology changes than regular toothpastes.

Abstract:SUMMARY Objectives: To evaluate the effect of various toothpaste tablets on gloss and surface roughness of resin-based composite. Methods and Materials: Sixty-four resin-based composite specimens were divided into four groups of 16 specimens each. Gloss and roughness were measured before and after simulated brushing with three types of toothpaste tablets and one conventional toothpaste: CT: Chewtab Toothpaste Tablets; AT: Anticavity Toothpaste Tablets; HC: Charcoal Toothpaste Tablets; CP: Cavity Protection toothpaste. The Kruskal– Wallis procedure was performed to compare the differences by groups. Post-hoc comparisons were conducted with Bonferroni corrections (α=0.05). Results: There was a significant drop in gloss for all groups. CT and AT maintained the highest gloss with means of 81.6 GU and 74.1 GU, respectively. The lowest gloss of 24.5 GU was observed for HC. There was a significant increase in roughness for all groups except for CT. CT had the lowest roughness with a mean of 0.034 μm, while HC had the highest roughness with a mean of 0.074 μm. There was a significant correlation between post-brushing gloss and post-brushing roughness (p<0.001, r=−0.884). Conclusion: Chewtab Toothpaste Tablets had the least effect on gloss and roughness, while Charcoal Toothpaste Tablets had the most negative effect on the surface properties of resin-based composites.

The purpose of this study was to determine the level of whitening and abrasiveness of charcoal toothpastes in permanent teeth.Materials and Methods: In this in vitro study, 30 premolars were polished, sectioned, mounted, and stored for 5 days in a coffee solution at 37°C. The color and surface profile of the teeth were measured by spectrophotometry and a profilometric device, respectively. The specimens were divided into 3 groups of 10 and were brushed 2000 times (equivalent to 3 times a day for 1.5 months) in a brushing machine using 20 g of each toothpaste (Bencer, Beverly, and Colgate) mixed with 40 ml of distilled water. The color and surface profile were remeasured. Bonferroni test and repeated measures analysis of variance (ANOVA) were used to examine the abrasion. One-way ANOVA was used to assess the whitening.Results: The three toothpastes caused changes in the surface profile (P = 0.0001). ΔE was equal to 3.3 (within the acceptable range) in all groups (95% confidence interval). There was no significant difference in abrasion (P > 0.05) and color change (P = 0.884) among toothpastes.Conclusion: The results of this study showed that all the three used toothpastes have the abrasive and whitening effect on the samples significantly. The differences between the toothpastes were not significant.
Effect of Brushing with Whitening Toothpaste on Color Stability and Surface Roughness of Color-Adjustment Resin-Based Composites

Abstract:Objectives: Aim of this study is to compare the color stability and surface roughness of four different color-adjustment composites produced using smart chromatic technology and a highly esthetic nano-hybrid composite after brushing with whitening toothpaste. Materials and Methods: 4 different color adjustment composites and 1 nano hybrid composite are divided into 5 main groups according to their contents. A total of 120 disc-shaped specimens (8 x 2 mm) were prepared, 24 in each group. The prepared samples were sequentially kept in solutions (tea, coffee and cherry juice) for 12 days and randomly divided into 3 subgroups (n=8). Subgroup 1, brushing with distilled water; Subgroup 2, brushing with whitening toothpaste; Subgroup 3, home bleaching agent were applied (14 days). Color measurements were made with a spectrophotometer, ΔE values were calculated using the CIELAB formula. Surface roughness values were made using a profilometer device. Samples from each group were selected for SEM surface analysis. Results: Among the composite groups, the highest coloration was observed in the Omnichroma, while the lowest ΔE values were observed in the Vittra composite group. Although the ΔE values on the 14th day were higher than the 7th day in all groups, the differences between them were statistically insignificant(p>0.05) In all composite groups (except Omnichroma), the ΔE values of brushing with whitening toothpaste at the end of the 14th day were higher than brushing with distilled water(p<0.05). Conclusions: Although the application of home bleaching agent was found to be more effective in a short time in whitening color-adjustment composites; brushing with whitening toothpaste at the end of the 14th day was found to be as effective as the application of home bleaching agent. Coloring and bleaching procedures applied to color-adjustment restorative materials did not have a significant negative effect on surface roughness values.
Effects of two dentifrices on the surface properties and staining susceptibility of polymer-based materials

Abstract:Purpose: This study examined the effect of whitening and abrasive regular dentifrices on the surface characteristics and stain susceptibility of polymer-based CAD-CAM blocks subjected to artificial toothbrushing abrasion (TB). Materials and methods: Two resin composite blocks [CeraSmart (CS) and Grandio Blocs (GB)], one polymer-infiltrated ceramic [Vita Enamic (VE)], and one direct resin composite [GrandioSO (RC)] were used to produce 60 specimens. The baseline mass, gloss, roughness, Vickers hardness (Hv), and color were measured after 7 days of water storage. The specimens were then separated into three TB treatment groups (n = 5): water control (C), regular daily dentifrice (R), and whitening dentifrice (W). Measurements were repeated post-TB (20,000 cycles). All specimens were immersed in coffee, and the CIE ΔE00 was measured after 1, 7, and 14 days. Two-way, one-way ANOVA, and Tukey's post hoc tests were performed to determine any significant differences between the materials and TB groups. To determine the stain resistance, repeated measures of ANOVA, one-way ANOVA, and Tukey's post hoc tests were used (α = 0.05). Results: The R and W mass changes were minimal (-3.77 to 3.16 g%). W reduced the gloss of all the materials by 12.6%-65%. All materials in W were slightly rougher (Ra, 0.107-0.144 μm) than those in R (Ra, 0.049-0.072 μm). The ΔE00 ranged from 0.6 to 1.6 in W and 0.4 to 1.4 in R. VE was the hardest material at baseline (Hv = 362), whereas brushing and staining lowered Hv in all TB groups (282.8-300.6). After brushing in W, VE, and RC were more susceptible to coffee stains than were CS and GB. Conclusions: The whitening dentifrice roughened CAD-CAM specimens, reducing gloss, yet lightened some materials. Polymer-infiltrated ceramic and direct resin composite specimens brushed with abrasive regular or whitening dentifrice resulted in more intense staining than the CAD-CAM resin composites.

Abstract:Background/purpose: Few studies have focused on the influence of simulated toothbrush abrasion on the surface qualities of novel nanofilled and nanohybrid composites. The aim of the study was to evaluate the surface roughness and gloss values of resin-based composite (RBC) materials with various filler types before and after simulated toothbrush abra-sion.Materials and methods: One nanofilled (Filtek Z350 XT [FT3]), two nanohybrids (Harmonize [HM] and Clearfil Majesty [CM]) and one microhybrid (Filtek Z250 [FT2]) were evaluated. Twelve specimens of each material were made and polished with silicon carbide sandpapers. Initial surface roughness and gloss values were measured as negative controls. Then, all spec-imens were subjected to simulated toothbrush abrasion on a custom-made apparatus. After 2000, 4000 and 8000 cycles, the surface roughness and gloss values of all specimens were tested. One additional specimen from each group was selected for scanning electron micro-scope (SEM) analysis.Results: For FT3, Ra and GU values did not significantly change until after 8000 cycles during the process of toothbrushing (P > 0.05). For HM, CM and FT2, the Ra and GU values significantly decreased after 4000 and 8000 cycles of toothbrush abrasion (P < 0.05). After 8000 cycles of toothbrush abrasion, FT3 presented the lowest surface roughness and highest gloss values of all materials (P < 0.05). SEM images showed that surface textures and irregularities corresponded to the results of surface roughness and gloss.Conclusion: Surface roughness and gloss after simulated toothbrush abrasion were material dependent. Abstract:Abstract Background This study aimed to compare the effects of charcoal-containing, hydrogen peroxide-containing, and abrasive whitening toothpastes on color stability of a resin composite. Methods Forty-five specimens were fabricated of spectrum TPH3 composite resin and stored in artificial saliva for 24 h. Baseline color assessment was performed using a spectrophotometer device. Then, the specimens were randomly assigned into 5 experimental groups, namely distilled water (GC), Bencer (GB), colgate optic white (GO), perfect white black (GP) and colgate total whitening (GT) toothpastes. The specimens immersed in coffee solution for 10 min and brushed for 1 min with respective toothpaste and then stored in artificial saliva until the next day. This cycle was repeated for 30 days. After 30 days, the final color assessment was performed using the spectrophotometer. Data were analyzed using one-way ANOVA and Tukey tests. Results Experimental groups were not significantly different in terms of Δa and ΔE values. However, ΔL and Δb values showed significant difference among the groups. Regarding Δa, GT and GC groups showed red color shift while the other groups showed green color shift. Regarding Δb, all groups showed blue color shift except GT group which showed yellow color shift. Conclusion None of the whitening toothpastes could decrease discoloration caused by the coffee solution to the level below the perceptibility threshold except Colgate Optic White which reduced discoloration within the clinically acceptable perceptibility range.

Abstract:ObjectiveCharcoal based oral care products have gained popularity in the last few years. The aim of this in vitro study was to compare the effects of different charcoal based whitening toothpastes on color, surface roughness and microhardness of human enamel.Materials and methodsForty-eight specimens obtained from human permanent upper incisor teeth were randomly divided into 4 groups(n=12):Group-1:Colgate Total 12(CT); Group-2:Body Kingdom(BK); Group-3:Black is White(BW), Group-4:Colgate optic white(COW). Following 4 days cycle of darkening(2-min chlorhexidine and 60-min black tea per day), a 12- week brushing(twice daily for 1 min)was performed. Color of specimens was measured using a spectrophotometer. A contact type profilometer was used to measure surface roughness (Ra) and Vicker's hardness tester was used for the changes in microhardness(VHN). A representative sample from each group was visualized by SEM. Data were analyzed by One-way ANOVA, Welch, Fisher's, Kruskall-Wallis, Wilcoxon Sign Rank and Paired t-tests(p0.05).Only few scratches were observed on the enamel surfaces by SEM evaluations.ConclusionTwelve week brushing with charcoal based whitening toothpastes and a regular fluoridated toothpaste presented similar effects in color of enamel. Surface roughness was increased(except BW) while microhardness was not affected(except CT) with charcoal based whitening toothpastes.Clinical relevanceCharcoal based whitening toothpastes do not promise to whiten the human permanent teeth and their effects on enamel abrasion should not be disregarded.

Staining- and aging-dependent changes in color, translucency, and gloss of cloud-shade and one-shade resin composites

Abstract:Purpose: To compare staining and artificial accelerating aging-dependent changes in color, translucency, and gloss of cloud-shade and one-shade resin composite. Methods: The "cloud-shade" composites were Filtek Easy Match Universal Restorative (FU) and SimpliShade (SS): three shades each (light, medium and dark), while the "one shade" composites were Admira Fusion x-tra (AD) and Venus Diamond One (VE). Polymerized composite specimens (10 mm in diameter, 2 mm-thick) were polished using PoGo disks for 40 seconds. Color, translucency, and gloss measurements were performed before and after exposure to 3.8-day staining in coffee or wine (equivalent to a year of service with a 15-minute exposure per day), or exposure to 150 kJ/m² of accelerated aging (n= 5 per condition x three conditions, for a total of 15 specimens per shade). Color and translucency were measured using a benchtop spectrophotometer, and CIEDE2000 color differences (ΔE₀₀) and translucency parameter (TP₀₀) were calculated. Gloss (gloss units, GU) was measured using a small-area gloss meter. Gloss retention percentages after staining and aging were calculated. A two-way ANOVA was used to compare the effect of material and procedure, while a Tukey's post hoc multiple comparison test was used to assess differences among levels within each variable (α= 0.05). Results: The AD exhibited the best color stability upon staining, while VE was the most stable when exposed to artificial aging. Statistically significant differences were recorded among materials and procedures (P< 0.001), and their interactions (P< 0.01). AD also exhibited the highest TP stability upon staining, while the TP of FU was the most stable when exposed to artificial aging. Statistically significant differences were recorded among materials and procedures (P< 0.001), and their interactions (P< 0.01). Gloss retention upon staining and aging was greater than 90% for all tested materials. Statistically significant differences were recorded among materials (P< 0.001), procedures (P< 0.05), and their interactions (P< 0.01). Clinical significance: Staining and artificial accelerated aging changes in color, translucency parameter, and gloss retention of cloud-shade and one-shade resin composites were material- and procedure-dependent.

Abstract:In the context of optimizing dental care for patients who are elderly, the purpose of this in vitro study was to evaluate the surface gloss (with a micro-area gloss meter) of, surface roughness (with a compact surface roughness measuring instrument) of, and color change (with a dental colorimeter) in two commercially available injectable resin-based composites (Estelite Universal Flow (EUF) and Beautifil Flow Plus F00 (BFP)) as well as two glass-ionomer cements (GC Fuji II LC CAPSULE (FLC) and GC Fuji IX GP EXTRA CAPSULE (FGP)), before and after dental prophylaxis. After 24 h, the surfaces of each specimen were polished at 2500 rpm with a prophy brush (Mersage Brush, Shofu) and one-step prophylaxis paste (Prophy Paste Pro, Directa): under 100 or 300 gf load, and for 10 or 30 s, 4× cycles of cleaning. After mechanical cleaning, conditions were found for a significant reduction in the gloss level (EUF, BFP, or FLC; p < 0.05) and a significant increase in surface roughness (BFP; 300 gf load, 10 s × four cycles of cleaning). Overall, the longer time or higher prophylaxis load tended to decrease the surface gloss. However, the observed change in surface roughness varied between the restorative materials. There was no color change post-prophylaxis.

Abstract:Objective: To investigate surface properties of novel flowable composites after polishing and simulated brushing wear, compared to their pasty counterpart. Methods: Composites employed in this study were: three flowable composites (A1: Clearfil Majesty ES Flow; B1: Beautifil Flow Plus F00; C1: Filtek Bulk Fill) and three paste composites (A2: Clearfil Majesty; B2: Beautifil; C2: Filtek Z350. Eleven disk-shaped specimens were made for each material. The specimens were cured, then subjected to sandpaper finishing for 20 s, one-step polishing for 30 s, finally subjected to simulated brushing for 10 000 cycles. Surface roughness and glossiness were measured before finishing, after finishing, after polishing, after 5 000 brushing cycles and after 10 000 brushing cycles, respectively. Data obtained were analyzed using two-way ANOVA method. Scanning electron microscope was employed to examine the microscopic appearance of each material. Results: Surface roughness (0.11~0.22 μm) and glossiness (74.25~86.48 GU) of each material were similar after one-step polishing. After brushing simulation, roughness increased significantly and glossiness decreased significantly for each material (P<0.05). Group A1 presented the best gloss ([50.68±1.58] GU) after final wear (P<0.05). Flowable composites of group A1 and B1 tested in the present setup showed better surface properties compared to their pasty counterpart (group A2 and B2). Conclusions: Within the limit of this study, flowable composites tested in the present research can obtain similar surface polish or even better than the paste composite counterpart.
Evaluation of Color and Translucency of Stained and Glazed Monolithic Lithium Disilicates and Zirconia After Toothbrushing with Different Dentifrices and Thermocycling

Abstract:Purpose: To evaluate changes in color and translucency of stained and glazed monolithic lithium disilicates and zirconia after toothbrushing with conventional or whitening dentifrices followed by thermocycling. Materials and methods: Samples (n = 20/material) from CAD/CAM (IPS e.max CAD [EC]) and pressable (IPS e.max Press [EP]) lithium disilicates and 5Y-PSZ zirconia (Prettau 4 Anterior [PA]) were prepared and received staining and glazing procedures. Aging was performed by toothbrushing with conventional (n = 10) or whitening (n = 10) dentifrices, followed by thermocycling simulating 5, 10, and 15 years. Color coordinates L*, a*, b*, , and C* and h° values were assessed with a spectrophotometer at baseline and after each aging period to calculate color changes (ΔE00), lightness (ΔL*), and translucency parameter (TP). Data were analyzed using three-way repeated measures ANOVA's followed by Sidak post-hoc test (α = .05). Results: When comparing the dentifrices, EC and EP produced the greatest ΔE00 after 15 years with the whitening dentifrice (P < .05). Regardless of the dentifrice, PA showed greater ΔE00 than EC after 10 years (P < .05). When comparing the aging periods, EC and PA did not show differences in ΔE00 for both dentifrices (P ≥ .05). EC and EP produced an increased ΔL* after 15 years in the whitening group (P < .05). In the whitening groups, the ΔL* values of PA differ (P < .005) from those of lithium disilicates. Regardless of the experimental group, the TP values followed the inequality: PA < EC < EP (P < .05). Conclusions: Overall, the color of stained and glazed monolithic lithium disilicates and zirconia changed under aging with greater effects for whitening dentifrice. Aging did not influence translucency.
Charcoal dentifrices: A laboratory study of their safety and efficacy

Abstract:Purpose: To compare charcoal-containing dentifrices (CDs) to non-charcoal containing dentifrices (NCDs) through the following experiments: potentially available fluoride, 1-minute fluoride release, pH, cytotoxicity, heavy metals, enamel fluoride uptake (EFU) and relative dentin abrasivity (RDA). Methods: Nine fluoride dentifrices; six CDs and three NCDs were tested (n= 3) for available fluoride, the amount of fluoride released within 1 minute, pH cytotoxicity, heavy metals, EFU and RDA. Four CDs and 1 NCD contained sodium fluoride (NaF) as the active ingredient whereas two dentifrices contained stannous fluoride (SnF₂; 1 CD and 1 NCD), and two dentifrices contained disodium monofluorophosphate (Na₂FPO₃, or Na₂MFP; 1 CD and 1 NCD). Available samples were homogenized and diluted to 1-in-100 in deionized water (DIW). Release samples were prepared as 1-in-4 homogenized dilutions by mass in DIW. Available and release samples were measured in triplicate (n= 3) via fluoride ion-selective electrode (F-ISE) and ion chromatography (IC). ANSI/ADA 130 was followed for pH. L929 cells were cultured using the lactate dehydrogenase (LDH) assay and ISO 10993-5 Annex C MTT cytotoxicity test. Heavy metals testing was performed using a hydrofluoric acid digestion sample preparation method followed by inductively coupled plasma mass spectrometry (ICP-MS) detection. EFU was performed on enamel specimens that underwent treatment with a CD slurry (1-in-4 dilution) following Test Method #40 of FDA Monograph 21. RDA was performed following ISO 11609 Annex A and the Hefferren method. Data was analyzed using one-way ANOVA followed by post-hoc tests (α= 0.05). Results: Available fluoride for all nine dentifrices was between ~93-102% of the labeled amount. The amount of fluoride released after 1 minute of homogenous mixing ranged between 75-107% of the labeled amount. The pH values of the nine dentifrices ranged from 6.5 to 7.7. Charcoal did not significantly contribute to cytotoxicity in L929 cells. The concentrations of each heavy metal (Hg, Cd, As and Pb) present in each of the nine dentifrices were < 1 ppm, indicating trace amounts. The CDs were not significantly more abrasive than the NCDs. The SnF₂ CD had the highest EFU value (644.2 ±131.7 ppm) followed by the NaF CD and the Na₂MFP CD at 492.2± 69.5 ppm and 140.1± 28.1 ppm, respectively. Clinical significance: Charcoal-containing dentifrices were not found to be significantly more abrasive or cytotoxic than non-charcoal-containing dentifrices. Charcoal and non-charcoal-containing dentifrices were also found to be comparable through experiments determining their fluoride content, pH, enamel fluoride uptake and heavy metals.

Abstract:CLINICAL HISTORY A 24-year-old Zimbabwean refugee presented to his general practitioner with a 6-month history of persistent low back pain. He had fallen while running away from the Zimbabwean opposition party before entering the United Kingdom. He complained of pain in his right leg and lower back since this fall. Examination revealed reduced knee and absent ankle reflexes on the right side with normal sensation. Conventional radiographs of the pelvis and lumbar spine showed a large lytic lesion in the sacrum (Fig. 1). An outpatient magnetic resonance imaging (MRI) scan was performed (Fig. 2), which showed a large soft tissue mass of intermediate signal intensity on both T1 and T2 weighted imaging involving the sacrum. The lack of endplate destruction and disk involvement made pyogenic infection unlikely but raises the possibility of a granulomatous infection. Although neoplasia could give this appearance, the patient’s age and ethnic origin were thought to favor a diagnosis of granulomatous infection, and the patient was admitted for a computed tomography (CT)-guided biopsy (Fig. 3). A single acid-fast bacillus was seen on histologic examination; cultures later confirmed infection with Mycobacterium tuberculosis (TB). Human immunodeficiency virus (HIV) test results were negative, and a chest x-ray showed no evidence of pulmonary TB. He began therapy with isoniazid, rifampicin, pyrazinamide, ethambutol, and pyridoxine. Because of the mass involving the sacral nerves, he commenced taking steroids. He was pyrexial initially, but the fevers settled after 3 weeks of treatment. His symptoms of low back pain and neurologic deficit also resolved 1 month after treatment.
