Abstract:Background Recent study reported that microRNA-142 (miR-142) were up-regulated in the atherosclerotic plaques, which may be responsible for pathogenesis of atherosclerosis. However, whether it associates with presence of acute myocardial infarction (AMI), and its prognostic value is still unknown. We, therefore, investigated the association between miR-142 expression and presence of AMI, and its prognostic value in AMI patients. Methods We included 300 AMI patients and 100 subjects as the control group. MiR-142 content was detected by quantitative real-time polymerase chain reaction. MiR-142 level was identified in all subjects. The multivariate logistic regression analysis were performed to evaluate the risk factors of AMI. The Kaplan–Meier analysis was performed to determine the major adverse cardiovascular and cerebrovascular events (MACCE)-free survival. Results AMI group had significantly higher miR-142 level in comparison to the controls [4.10 (2.03–7.43) vs. 1.92 (0.91–2.91), p < 0.001], moreover, miR-142 content was significantly associated with cardiac troponin I (cTnI) level (r = 0.707, p < 0.001). The MACCE-free survival was significantly lower over 24-month for patients in miR-142 high expression group (72.4% ± 5.6% vs. 76.4% ± 5.1%) ( p = 0.022). After adjusting for the traditional risk factors, the odds ratios of miR-142 was 14.74 (95% CI, 2.15–101.24). The multivariate logistic regression analysis revealed that miR-142 level significantly associated with presence of AMI ( p < 0.001). Conclusion The serum level of miR-142 was increased in AMI patients when compared with health population. Furthermore, use of this marker may allow a certain predictor of the MACCE in AMI patients.
Circulating miR-208b: A Potentially Sensitive and Reliable Biomarker for the Diagnosis and Prognosis of Acute Myocardial Infarction.

Abstract:Background: MicroRNAs are present in human plasma and have been reported to be biomarkers for cardiovascular diseases. The aim of this study was to investigate the value of circulating microRNA-208b (miR-208b) for the diagnosis and prognosis of acute myocardial infarction (AMI). Methods: A total of 100 AMI patients, 80 unstable angina (UA) patients, and 80 healthy controls (HCs) were consecutively included in this study. Plasma was collected from each participant on admission, and the levels of circulating miR-208b were measured using reverse transcription-polymerase chain reaction (RT-PCR). The AMI patients who underwent percutaneous coronary intervention (PCI) were followed up at 6 months post-AMI. Results: The concentration of miR-208b was higher in the AMI patients than in the other two groups (p < 0.05), and it was positively correlated with the levels of creatine kinase (CK)-MB and cardiac troponin I (cTnI) (p < 0.01). Moreover, the receiver operating characteristic (ROC) curves showed that miR-208b was sensitive like CK-MB and cTnI for the diagnosis of AMI. In addition, the miR-208b concentration in AMI patients with three vessel coronary artery disease (CAD) was higher than that of single- or two-vessel CAD AMI patients (p < 0.05). Also, the miR-208b expression after PCI was significantly lower than on admission (p < 0.01). Furthermore, miR-208b expression in AMI patients with left ventricular remodeling/MACEs was higher than in those without after PCI (p < 0.05). Conclusions: Circulating miR-208b may serve as a sensitive biomarker for the diagnosis and prognosis of AMI patients.
Circulating Mirnas Related to Long-term Adverse Cardiovascular Events in STEMI Patients: A Nested Case-Control Study.

Abstract:BACKGROUND:Long-term morbidity and mortality of patients with ST-segment-elevation myocardial infarction (STEMI) after primary percutaneous coronary intervention (PCI) remain substantial. Circulating microRNAs (miRNAs) play an important role in cardiovascular disease development. We aimed to identify circulating miRNAs associated with adverse cardiovascular events after acute myocardial infarction (AMI).METHODS:We performed a prospective, nested, case-control study of 932 patients with STEMI who underwent primary PCI. A 3-phase approach was conducted to screen candidate circulating miRNAs in 70 patients who subsequently experienced cardiac death, hospitalization for heart failure, or recurrent AMI (major adverse cardiovascular events [MACE] group) and in 140 patients matched for age, sex, time from symptom onset to blood collection and dual-antiplatelet therapy who did not report adverse cardiovascular events during 2-year follow-up (non-MACE group).RESULTS:We found that miR-26a-5p, miR-21-5p, and miR-191-5p levels were lower in the MACE group than in the non-MACE group (all P < 0.001). Multivariate conditional logistic regression analysis revealed that miR-26a-5p, miR-21-5p, and miR-191-5p levels were significantly inversely associated with incident primary composite outcomes (all adjusted P < 0.01). Importantly, the combination of these 3 miRNAs plus B-type natriuretic peptide clearly improved the risk scores recommended in the current guidelines, as determined with the use of C-statistics, net reclassification, and integrated discrimination.CONCLUSIONS:Our study provides proof-of-concept in humans that circulating miRNAs are associated with increased rates of distinct cardiovascular events, suggesting that they can serve as effective prognostic biomarkers and therapeutic targets for patients with AMI.

Abstract:Objective: This study aims to investigate the value of interleukin 6 (IL-6) and high-mobility group box 1 (HMGB1) in predicting major adverse cardiovascular events (MACE) after percutaneous coronary intervention (PCI) in patients with acute coronary syndrome (ACS). Methods: Patients with ACS who were treated in our hospital from October 2022 to October 2023 were divided into MACE and no-MACE groups according to the occurrence of MACE after PCI. The baseline data and IL-6 and HMGB1 levels in the two groups were observed, and the influencing factors of MACE in patients with ACS after PCI were evaluated with a logistic regression test. The receiver operator characteristic curve (ROC) values of IL-6 and HMGB1 in the prediction of MACE after PCI in patients with ACS were calculated. Results: No significant differences in age, sex, body mass index (BMI), and other general data were found between the groups. Compared with the patients in the no-MACE group, the patients in the MACE group had a history of smoking (p = 0.011), hypertension (p < 0.001), diabetes (p < 0.001), more coronary lesions (p = 0.013), longer coronary lesions (p = 0.006), higher preoperative Gensini score (p < 0.001), and lower left ventricular ejection fractions (LVEF) (p < 0.001). The levels of IL-6 and HMGB1 in the MACE group were significantly higher than those in the no-MACE group. Coronary lesion length, Gensini score, LVEF, IL-6, and HMGB1 had good predictive value for MACE after PCI. The area under the curve (AUC) scores were 0.683, 0.941, 0.816, 0.878, and 0.737. The sensitivity was 53.13%, 81.25%, 84.37%, 78.12%, and 53.13%, and the specificity was 87.50%, 93.18%, 63.64%, 86.36%, and 86.36%, respectively. Analysis of IL-6 and HMGB1 levels showed that the AUC was 0.922, the sensitivity was 90.62%, the specificity was 82.95%, and the 95% confidence interval (CI) was (0.858–0.963; p < 0.05). Conclusion: IL-6 and HMGB1 have good predictive value for MACE after PCI for patients with ACS and can be used as clinical evaluation indexes.

Abstract:Acute myocardial infarction (AMI) is a heart lesion, that endangers the life safety of patients. This study focused on exploring the clinical effect of miR-542-3p on AMI and no-reflow after percutaneous coronary intervention (PCI). Serum samples were collected from 100 AMI emergency inpatients. The expression of miR-542-3p was quantified by qPCR. The predictive role of miR-542-3p was disclosed by plotting ROC curve. In addition, AMI subjects were cataloged into a group of no-reflow and normal reflow group. The risk factors of no-reflow were estimated by logistic regression analysis. In the serum samples of AMI patients, the level of miR-542-3p showed a pattern of decreasing. MiR-542-3p expression represented a high sensitivity and specificity of the prediction of AMI. A decrease of miR-542-3p content was revealed in AMI patients without reflow after PCI. Logistic regression results reflected that miR-542-3p was an independent biomarker for no-reflow. The declined miR-542-3p expression was a predictive marker for AMI and no-reflow in AMI patients.

Early Diagnostic Value of Circulating Micrornas in Patients with Suspected Acute Myocardial Infarction

Abstract:Background/AimsEvidence has shown that several microRNAs (miRNAs) may be involved in coronary plaque rupture and local thrombus. However, the diagnostic ability of these miRNAs in acute myocardial infarction (AMI) is less known. The aim of this study is to explore the diagnostic value of these circulating miRNAs in patients presenting with acute chest pain in the emergency department. Methods and resultsIn a nested case-control study, 140 of 1,206 patients finally diagnosed with AMI were matched with 70 unstable angina and 70 noncardiac chest pain patients. Five candidate miRNAs (miR-483-5p, miR-155-5p, miR-451, miR-19b, and miR-223) were selected for validation. Among them, miR-19b, miR-223, and miR-483-5p were significantly higher in AMI patients compared with those without AMI. A multivariate analysis showed that these miRNAs were independent predictors of AMI. The overall areas under the receiver operating curves (AUCs) for miR-19b, miR-223, and miR-483-5p were 0.74, 0.65, and 0.70, respectively. However, serial sampling in AMI patients showed that these miRNAs already peaked on admission, which was earlier than troponin I. Among 170 patients with a negative troponin result at presentation, a panel of three miRNAs improved the discrimination ability to a clinical model. In 119 patients presenting within 3hr after chest-pain onset, the diagnostic accuracy of each miRNAs was higher than Point of care (POC) troponin assay. And a panel of these miRNAs had an AUC of 0.92. ConclusionCirculating miR-19b, miR-223, and miR-483-5p may provide clinically useful information for diagnosis in the early phases of AMI.
The Clinical Value of Circulating Mir-99A in Plasma of Patients with Acute Myocardial Infarction.

Abstract:OBJECTIVE:Acute myocardial infarction (AMI) contributes to be a significant public health problem, and it is the leading cause of morbidity and mortality in the world. However, the molecular mechanisms underlying AMI is poorly known. The purpose of this study was to investigate the change of plasma microRNA-99a (miR-99a) level in AMI patients and its potential clinical value.PATIENTS AND METHODS:Plasma samples from 54 patients with AMI and 30 healthy volunteers were collected. Quantitative real-time PCR (qPCR) and ELISA assays were used to detect the expression of circulating plasma miR-99a and of cardiac troponin I (cTnI)/creatine kinase-MB (CK-MB), respectively. AMI patients were categorized into subgroups according to the number of stenosed coronary vessels, and the association between the plasma miR-99a level and the severity of AMI was analyzed. The expression level of miR-99a was also evaluated in patients receiving percutaneous coronary intervention (PCI).RESULTS:The expression of miR-99a was significantly downregulated in patients with AMI compared with the normal controls (p < 0.01). In the AMI patients, miR-99a level had a negative correlation with cTnI level (r = -0.8202, p < 0.01) and CK-MB (r = -0.6924, p < 0.01). Also, the expression of miR-99a was markedly lower in patients with more stenosed coronary vessels (p < 0.01). The relative expression level of miR-99a in AMI patients was significantly increased after receiving PCI (p < 0.01).CONCLUSIONS:The expression level of plasma miR-99a was remarkably reduced following AMI and closely associated with the severity of AMI. Therefore, it was a promising novel diagnostic biomarker for AMI.
Diagnostic and Prognostic Value of Circulating MicroRNA-133a in Patients with Acute Myocardial Infarction.

Abstract:BACKGROUND:The aim of this study was to examine the expression of circulating microRNA (miR)-133a in acute myocardial infarction (AMI) patients and assess its potential as a diagnostic and prognostic marker. METHODS:This study enrolled 222 consecutive patients who presented with chest pain symptoms suggestive of AMI in either the Department of Emergency or Department of Cardiology at Wuxi Second People's Hospital from October 2012 through December 2014. Of these, 102 were diagnosed with AMI and 120 with non-AMI chest pain. An additional 110 healthy individuals who received physical examinations in the same hospital during the same period were used as controls. RESULTS:MiR-133a expression was significantly elevated in the AMI patients compared to both non-AMI patients and healthy controls (p < 0.01 for both). MiR-133a levels were markedly increased in AMI patients within 2 hours after the onset of chest pain and remained elevated over the next 9 hours. MiR-133a concentration at 24 hours after percutaneous coronary intervention (PCI) was significantly lower compared to time of admission in the emergency PCI group (p < 0.01). The elevation in miR-133a was specific and sensitive for the diagnosis of AMI, with an optimal cutoff value of 0.87 (95% confidence interval (CI), 0.812 - 0.928). Kaplan-Meier analysis demonstrated that major adverse cardiovascular events occurred significantly more often in AMI patients who had miR-133a levels above the median (p < 0.01). CONCLUSIONS:Elevated levels of circulating miR-133a were strongly associated with AMI diagnosis. The concentration of miR-133a may provide prognostic information additive to traditional markers for clinical prognosis in AMI patients.
Longitudinal change of serum exosomal miR-186-5p estimates major adverse cardiac events in acute myocardial infarction patients receiving percutaneous coronary intervention

Abstract:Objective: Our recently published study discovers that exosomal microRNA (miR)-186-5p promotes vascular smooth muscle cell viability and invasion to facilitate atherosclerosis. This research aimed to explore the prognostic implication of serum exosomal miR-186-5p in acute myocardial infarction (AMI) patients receiving percutaneous coronary intervention (PCI). Methods: One hundred and fifty AMI patients receiving PCI and 50 healthy controls (HCs) were screened. Serum exosomal miR-186-5p was detected by reverse transcriptase-quantitative polymerase chain reaction assay in AMI patients at admission and after PCI, as well as in HCs after enrollment. Major adverse cardiac events (MACE) were recorded during follow-up in AMI patients receiving PCI. Results: Serum exosomal miR-186-5p was raised in AMI patients vs. HCs ( P < 0.001). Besides, serum exosomal miR-186-5p was positively linked to body mass index ( P = 0.048), serum creatinine ( P = 0.021), total cholesterol ( P = 0.029), and C-reactive protein ( P = 0.018); while it was reversely linked with estimated glomerular filtration rate ( P = 0.023) in AMI patients. Interestingly, serum exosomal miR-186-5p was correlated with the diagnosis of ST-segment elevation myocardial infarction ( P = 0.034). Notably, serum exosomal miR-186-5p was decreased after PCI vs. at admission ( P < 0.001). The 6-, 12-, 18-, and 24-month accumulating MACE rates were 4.5%, 8.9%, 14.8%, and 14.8% in AMI patients. Furthermore, serum exosomal miR-186-5p ≥3.39 (maximum value in HCs) after PCI ( P = 0.021) and its decrement percentage <median (35%)="" ( P = 0.044) estimated elevated MACE in AMI patients. Conclusion: Serum exosomal miR-186-5p is reduced after PCI, and its post-PCI high level or minor decrease estimates increased MACE risk in AMI patients.

Diagnostic and Prognostic Significance of Microrna-208A in Acute Myocardial Infarction

Abstract:Objective To determine the prognostic and diagnostic significance of microRNA-208a (miR-208a) in acute myocardial infarction (AMI). Methods Totally, 84 AMI patients hospitalized in our hospital between Jan. 2019 and Feb. 2021 were enrolled as the patient group (Pat group), and 50 healthy individuals over same time span as the control group (Con group). qRT-PCR assay was carried out to quantify serum miR-208a in the patients and receiver-operating characteristic (ROC) curves for analysing its diagnostic value in AMI patients and its predictive value in clinical efficacy and adverse events in the patients after therapy. The changes of miR-208a and clinical indexes ((lactate dehydrogenase (LDH), creatine kinase (CK) as well as Creatine kinase-MB (CK-MB)) in the patients before and after therapy were evaluated. Pearson's test was adopted to analyse the associations of miR-208a with clinical indexes. Additionally, the target genes of miR-208a were forecasted. Results The patient group showed a higher miR-208a level than the control group (p < 0.05), and the area under the curve (AUC) of miR-208a in diagnosing AMI was >0.9. After therapy, patients presented notable decreases in serum miR-208a, LDH, CK, and CK-MB (all p < 0.05). Serum miR-208a presented positive associations with LDH, CK, as well as CK-MB both before and after therapy (all p < 0.05). Before therapy, the ineffective group presented a higher miR-208a level than the effective group (p < 0.05), and miR-208a had an AUC of 0.784 in forecasting efficacy. Additionally, the group with adverse events presented a higher miR-208a level than the group without them before therapy (p < 0.05), and miR-208a had an AUC of 0.713 in forecasting adverse events. According to enrichment analysis, the target genes of miR-208a were bound up with signal pathways of cellular senescence, MTOR and Wnt. Conclusion With high expression in AMI cases, miR-208a is a promising potential biomarker for diagnosis and prognosis forecasting of AMI.

Abstract:Objective To evaluate the value of hypersensitive C-reactive protein(hs-CRP) for predicting the incidence of major adverse cardiac events(MACE) in patients with acute myocardial infarction(AMI) within 180 days after percutaneous coronary intervention(PCI). Methods The study comprised 216 consecutive patients with first attack of ST-elevation myocardial infarction(STEMI).They underwent primary PCI within 12 hours after the onset of chest pain.According to their serum levels of hs-CRP,these patients were divided into two groups: the High hs-CRP Group(>3 mg/L,n=52) and the Normal hs-CRP Group(≤ 3 mg/L,n=164).The incidence of MACE within 180 postoperative days was followed.Results There were no significant differences in age,hypertension,hyperlipidemia,left ventricle ejection fraction(LVEF),creatine kinase isoenzyme MB(CK-MB),and immediate success rate of PCI between the two groups(P<0.05).The WBC count was higher in the High hs-CRP Group than in the Normal hs-CRP Group(P<0.05).Follow-up for 180 days showed the High hs-CRP Group had significantly higher incidence of MACE than the Normal hs-CRP Group(22.0% vs 9.6%,P=0.048).Binary logistic regression analysis indicated that high concentration of hs-CRP remained an independent predicator of MACE within 180 postoperative days(OR=2.82,95% CI=1.28~5.83,P=0.021). Conclusions High concentration of hs-CRP when admission is a useful predictor for the incidence of MACE in patients with STEMI within 180 days after primary PCI.
Abnormal expression of circ_0013958 in patients with acute myocardial infarction (AMI) and its influence on prognosis

Abstract:Objective: The purpose of this study was to investigate the diagnostic value of circ_0013958 in acute myocardial infarction (AMI) patients and its influence on the prognosis of AMI patients. Methods: The GSE160717 dataset was downloaded from the NCBI database and differentially expressed genes were analyzed between the control group and the AMI group. The up-regulated genes included circ_0013958. The expression of circ_0013958 in both groups was further verified by RT-qPCR. The Receiver Operating Characteristic curve was used to evaluate the diagnostic value of circ_0013958 in AMI. Pearson correlation analysis was used to examine the correlation between circ_0013958 levles and biochemical indicators. Binary logistic regression was used to analyze the risk factors affecting the occurrence of AMI. Prognostic analysis was performed using COX regression analysis and the Kaplan-Meier Curve. Results: Compared to the control group, the level of circ_0013958 in AMI patients increased. Circ_0013958 can effectively distinguish AMI patients from non-AMI patients. Circ_0013958 levels were positively correlated with cTnI, LDH, CRP and TC levels. The elevated level of circ_0013958 was an independent risk factor for the occurrence of AMI. Higher circ_0013958 levels were also associated with the occurrence of major adverse cardiac events (MACEs) in AMI patients. Additionally, elevated circ_0013958 levels reduced the survival probability of AMI patients. Conclusion: Circ_0013958 levels were up-regulated in AMI patients. It can be used as a diagnosis biomarker for AMI. The level of circ_0013958 was correlated with the disease severity and was an independent risk factor for the occurrence of AMI. Elevated circ_0013958 levels were associated with poor prognosis in AMI patients.
Long non-coding RNA SNHG7 serves as a diagnostic biomarker for acute coronary syndrome and its predictive value for the clinical outcome after percutaneous coronary intervention

Abstract:Abstract Background Acute coronary syndrome (ACS) is one of the common causes of cardiovascular death. The related lncRNAs were novel approaches for early diagnosis and intervention. This paper focused on the clinical function of SNHG7 for patients after PCI. Methods The expression of SNHG7 was assessed in ACS patients. The predictive roles of SNHG7 were unveiled by the ROC curve. The relationship between SNHG7 and Gensini scores was judged by Pearson analysis. One-year follow-up was conducted and all patients were catalogued into different groups based on the prognosis. The qRT-PCR, K-M curve, and Cox regression analysis were performed to document the prognostic significance of SNHG7. Results SNHG7 was highly expressed in ACS and its three subtypes. SNHG7 showed a certain value in predicting ACS, UA, NSTEMI, and STEMI. Gensini is a closely correlated indicator of SNHG7. The declined expression of SNHG7 was observed in the non-MACE and survival groups. The risk of MACE and death was increased in the group with high expression of SNHG7. SNHG7 was an independent biomarker in patients with ACS after PCI. Conclusions SNHG7 might be a diagnostic and prognostic tool for ACS patients.

Abstract:Background: Periprocedural myocardial injury (PMI) is a frequent complication of percutaneous coronary intervention (PCI) associated with poor prognosis. However, no effective method has been found to identify patients at risk of PMI before the procedure. MicroRNA-133a (miR-133a) has been reported as a novel biomarker in various cardiovascular diseases. Herein, it was sought to determine whether circulating miR-133a could predict PMI before the procedure. Methods: Eighty patients with negative preoperative values of cardiac troponin T (cTnT) receiving elective PCI for stable coronary artery disease (CAD) were recruited. Venous serum samples were collected on admission and within 16-24 hours post-PCI for miRNA measurements. PMI was defined as a cTnT value above the 99% upper reference limit after the procedure. The association between miR-133a and PMI was further assessed. Results: Periprocedural myocardial injury occurred in 48 patients. The circulating level of miR-133a was significantly higher in patients with PMI before and after the procedure (both p < 0.001). Receiver operating characteristic curve analysis of the preoperative miR-133a level revealed an area under the curve of 0.891, with a sensitivity of 93.8% and a specificity of 71.9% to predict PMI. Additionally, a decrease was found in fibroblast growth factor receptor 1 (FGFR1) in parallel with an increase in miR-133a levels in patients with PMI. Conclusions: This study demonstrates for the first time that serum miR-133a can be used as a novel biomarker for early identification of stable CAD patients at risk of PMI undergoing elective PCI. Abstract:Background: In this study, we tested the hypothesis that miR-181a levels increase during acute myocardial infarction. We investigated circulating miR-181a as a potential novel biomarker for early diagnosis of acute myocardial infarction (AMI). Methods: From June 2014 to June 2016, 120 consecutive eligible patients with AMI (n = 60) or unstable angina (UA; n = 60) and 60 control subjects were enrolled. Plasma miR-181a levels were determined by quantitative reverse transcriptase-polymerase chain reaction. Results: Circulating miR-181a expression levels detected immediately after admission were higher in the AMI group than in the UA and control groups. Relative miR-181a levels in AMI patients were positively correlated with the concentrations of the creatine kinase-MB fraction and cardiac troponin I. Correlation analysis showed that plasma miR-181a was positively correlated with coronary Gensini score (r = 0.573, P < 0.05) and negatively correlated with left ventricular ejection fraction (r = -0.489, P < 0.05). Receiver operating characteristic curve analyses showed that plasma miR-181a was of significant diagnostic value for AMI (AUC, 0.834; 95% CI, 0.756-0.912, P < 0.05). Conclusion: Circulating miR-181a levels in patients with AMI were significantly changed in a time-dependent manner, indicating the value of plasma miR-181a as a novel biomarker for diagnosing AMI. Relationship between lncRNA-Ang362 and prognosis of patients with coronary heart disease after percutaneous coronary intervention

Abstract:The severity and complexity evaluation of coronary artery disease in patients with coronary heart disease (CHD) require objective and accurate prognosis indexes. We assessed the relationship between lncRNA-Ang362 and prognosis of CHD patients after percutaneous coronary intervention (PCI). Clinical follow-up data of CHD patients were prospectively collected. LncRNA-Ang362 levels were detected by real-time quantitative polymerase chain reaction. Survival rate was calculated by the Kaplan-Meier method, and risk ratios and 95% confidence intervals were computed using univariate and multivariate COX proportional hazard models. Finally, 434 patients were included in the follow-up cohort. The median follow-up time was 24.8 months (6.7-40). The incidence of adverse cardiovascular events was 13.6%. The high expression group significantly tended to be smoker and higher body mass index, low-density lipoprotein cholesterol, high-sensitivity C-reactive protein, creatinine, and uric acid levels compared with the low expression group. According to the SYNTAX grade, the high-risk and medium-risk groups had significantly higher lncRNA expression levels than the low-risk group. The univariate COX regression analysis indicated that high lncRAN-Ang362 expression significantly increased the risk of adverse cardiovascular events in CHD patients after PCI (hazard risk (HR) = 3.19, 95% confidence interval (CI): 1.29-7.92). Multivariate analysis found high lncRNA-Ang362 expression was independently related to worse prognosis in CHD patients after PCI (HR = 2.83, 95%CI: 1.34-6.02). Plasma lncRNA-Ang362 may be a prognosis factor in CHD patients after PCI. The patients with higher lncRNA-Ang362 expression usually have poor prognosis.

Abstract:Background: Hyperlipidemia (HLP) may intensify myocardial cell damage by disrupting lipophagy, a pivotal lipid metabolism pathway, thereby heightening the risk of acute myocardial infarction (AMI). This study aims to identify HLP- and lipophagy-associated biomarkers for AMI through a combined transcriptomic and Mendelian randomization (MR) approach. Methods: The mRNA expression data for AMI, along with HLP-related genes (HRGs) and lipophagy-related genes (LRGs), were sourced from public databases. Biomarkers were identified by conducting differential expression analysis, Weighted Gene Co-expression Network Analysis (WGCNA), MR analysis, and Receiver Operating Characteristic (ROC) analysis, complemented by two machine learning algorithms and expression validation. These biomarkers facilitated an investigation into the role of platelet activation-related genes (PARGs) in AMI, with enrichment analysis providing insights into their underlying mechanisms. Finally, reverse transcription-polymerase chain reaction (RT-PCR) was employed to validate biomarker expression in clinical samples. Results: Three biomarkers exhibited a consistently significant upregulation trend in AMI samples, corroborated by RT-qPCR findings. Notably, PLAUR [Odds Ratio (OR) = 1.115, 95% confidence interval (CI): 1.006-1.237, P = 0.038] and IVNS1ABP (OR = 1.047, 95% CI: 1.000-1.096, P = 0.048) emerged as AMI risk factors, while QKI (OR = 0.946, 95% CI: 0.903-0.991, P = 0.020) was identified as a protective factor. Additionally, PLAUR, QKI, and IVNS1ABP demonstrated robust diagnostic efficacy with Area Under the Curve (AUC) values of 0.773, 0.933, and 0.807, respectively; when integrated into a nomogram, the combined AUC reached 0.924. These genes were enriched in pathways linked to cardiovascular diseases, inflammatory responses, and cellular metabolic processes and appeared actively involved in platelet activation, as indicated by their strong associations with PARGs. Conclusion: In summary, the biomarkers PLAUR, QKI, and IVNS1ABP, connected to HLP and lipophagy, showed a causal relationship with AMI and marked diagnostic potential for predicting AMI risk, offering valuable support for clinical diagnostics and AMI research.
