Abstract:Background Recent study reported that microRNA-142 (miR-142) were up-regulated in the atherosclerotic plaques, which may be responsible for pathogenesis of atherosclerosis. However, whether it associates with presence of acute myocardial infarction (AMI), and its prognostic value is still unknown. We, therefore, investigated the association between miR-142 expression and presence of AMI, and its prognostic value in AMI patients. Methods We included 300 AMI patients and 100 subjects as the control group. MiR-142 content was detected by quantitative real-time polymerase chain reaction. MiR-142 level was identified in all subjects. The multivariate logistic regression analysis were performed to evaluate the risk factors of AMI. The Kaplan–Meier analysis was performed to determine the major adverse cardiovascular and cerebrovascular events (MACCE)-free survival. Results AMI group had significantly higher miR-142 level in comparison to the controls [4.10 (2.03–7.43) vs. 1.92 (0.91–2.91), p < 0.001], moreover, miR-142 content was significantly associated with cardiac troponin I (cTnI) level (r = 0.707, p < 0.001). The MACCE-free survival was significantly lower over 24-month for patients in miR-142 high expression group (72.4% ± 5.6% vs. 76.4% ± 5.1%) ( p = 0.022). After adjusting for the traditional risk factors, the odds ratios of miR-142 was 14.74 (95% CI, 2.15–101.24). The multivariate logistic regression analysis revealed that miR-142 level significantly associated with presence of AMI ( p < 0.001). Conclusion The serum level of miR-142 was increased in AMI patients when compared with health population. Furthermore, use of this marker may allow a certain predictor of the MACCE in AMI patients.
Association Between Elevated Plasma Microrna-223 Content and Severity of Coronary Heart Disease

Abstract:Bioinformatics indicate that miR-223 regulates many genes associated with cholesterol metabolism, and it could also control high-density lipoprotein-cholesterol (HDL-C) uptake. As reported in previous study, miR-223 was found to be upregulated from human subjects with familial hypercholesterolaemia, however, it remains to be determined using a larger group of coronary heart disease (CHD) patients. Moreover, whether it correlates with severity of atherogenesis, has never been elucidated before. We aim to further explore the association between circulating miR-223 content and severity of CHD. Sample was collected from 300 CHD patients and 100 subjects with angiographic exclusion of CHD. MiR-223 content was detected using quantitative real-time PCR. Gensini score was used to evaluate the severity of coronary stenotic lesions. Expression of miR-223 was identified on basis of the quartiles of the Gensini score, and association between the miRNA and CHD was analyzed. Diagnostic potential of miR-223 of CHD was performed by ROC analysis. CHD patients had higher miR-223 level (13.23, 9.29-17.59 vs. 4.05, 3.06-6.11, p < .001), and the miRNA content significantly elevated following increasing Gensini score (p < .001). Gensini score was significantly associated with miR-223 expression (r=.7289, p < .001). The optimal cut-off value of miR-223 was with a sensitivity of 86.0% and specificity of 91.3%. The AUC of miR-223 was 0.933 (95%CI, 0.905-0.961). These preliminary results suggest that the expression of miR-223 may be associated with atherogenesis. The level of circulating miR-223 in predicting the severity of coronary atherosclerosis may have a relatively certain value.
Diagnostic and Prognostic Significance of Microrna-208A in Acute Myocardial Infarction

Abstract:Objective To determine the prognostic and diagnostic significance of microRNA-208a (miR-208a) in acute myocardial infarction (AMI). Methods Totally, 84 AMI patients hospitalized in our hospital between Jan. 2019 and Feb. 2021 were enrolled as the patient group (Pat group), and 50 healthy individuals over same time span as the control group (Con group). qRT-PCR assay was carried out to quantify serum miR-208a in the patients and receiver-operating characteristic (ROC) curves for analysing its diagnostic value in AMI patients and its predictive value in clinical efficacy and adverse events in the patients after therapy. The changes of miR-208a and clinical indexes ((lactate dehydrogenase (LDH), creatine kinase (CK) as well as Creatine kinase-MB (CK-MB)) in the patients before and after therapy were evaluated. Pearson's test was adopted to analyse the associations of miR-208a with clinical indexes. Additionally, the target genes of miR-208a were forecasted. Results The patient group showed a higher miR-208a level than the control group (p < 0.05), and the area under the curve (AUC) of miR-208a in diagnosing AMI was >0.9. After therapy, patients presented notable decreases in serum miR-208a, LDH, CK, and CK-MB (all p < 0.05). Serum miR-208a presented positive associations with LDH, CK, as well as CK-MB both before and after therapy (all p < 0.05). Before therapy, the ineffective group presented a higher miR-208a level than the effective group (p < 0.05), and miR-208a had an AUC of 0.784 in forecasting efficacy. Additionally, the group with adverse events presented a higher miR-208a level than the group without them before therapy (p < 0.05), and miR-208a had an AUC of 0.713 in forecasting adverse events. According to enrichment analysis, the target genes of miR-208a were bound up with signal pathways of cellular senescence, MTOR and Wnt. Conclusion With high expression in AMI cases, miR-208a is a promising potential biomarker for diagnosis and prognosis forecasting of AMI.
Diagnostic and Prognostic Value of Circulating MicroRNA-133a in Patients with Acute Myocardial Infarction.

Abstract:BACKGROUND:The aim of this study was to examine the expression of circulating microRNA (miR)-133a in acute myocardial infarction (AMI) patients and assess its potential as a diagnostic and prognostic marker. METHODS:This study enrolled 222 consecutive patients who presented with chest pain symptoms suggestive of AMI in either the Department of Emergency or Department of Cardiology at Wuxi Second People's Hospital from October 2012 through December 2014. Of these, 102 were diagnosed with AMI and 120 with non-AMI chest pain. An additional 110 healthy individuals who received physical examinations in the same hospital during the same period were used as controls. RESULTS:MiR-133a expression was significantly elevated in the AMI patients compared to both non-AMI patients and healthy controls (p < 0.01 for both). MiR-133a levels were markedly increased in AMI patients within 2 hours after the onset of chest pain and remained elevated over the next 9 hours. MiR-133a concentration at 24 hours after percutaneous coronary intervention (PCI) was significantly lower compared to time of admission in the emergency PCI group (p < 0.01). The elevation in miR-133a was specific and sensitive for the diagnosis of AMI, with an optimal cutoff value of 0.87 (95% confidence interval (CI), 0.812 - 0.928). Kaplan-Meier analysis demonstrated that major adverse cardiovascular events occurred significantly more often in AMI patients who had miR-133a levels above the median (p < 0.01). CONCLUSIONS:Elevated levels of circulating miR-133a were strongly associated with AMI diagnosis. The concentration of miR-133a may provide prognostic information additive to traditional markers for clinical prognosis in AMI patients.
The Clinical Value of Circulating Mir-99A in Plasma of Patients with Acute Myocardial Infarction.

Abstract:OBJECTIVE:Acute myocardial infarction (AMI) contributes to be a significant public health problem, and it is the leading cause of morbidity and mortality in the world. However, the molecular mechanisms underlying AMI is poorly known. The purpose of this study was to investigate the change of plasma microRNA-99a (miR-99a) level in AMI patients and its potential clinical value.PATIENTS AND METHODS:Plasma samples from 54 patients with AMI and 30 healthy volunteers were collected. Quantitative real-time PCR (qPCR) and ELISA assays were used to detect the expression of circulating plasma miR-99a and of cardiac troponin I (cTnI)/creatine kinase-MB (CK-MB), respectively. AMI patients were categorized into subgroups according to the number of stenosed coronary vessels, and the association between the plasma miR-99a level and the severity of AMI was analyzed. The expression level of miR-99a was also evaluated in patients receiving percutaneous coronary intervention (PCI).RESULTS:The expression of miR-99a was significantly downregulated in patients with AMI compared with the normal controls (p < 0.01). In the AMI patients, miR-99a level had a negative correlation with cTnI level (r = -0.8202, p < 0.01) and CK-MB (r = -0.6924, p < 0.01). Also, the expression of miR-99a was markedly lower in patients with more stenosed coronary vessels (p < 0.01). The relative expression level of miR-99a in AMI patients was significantly increased after receiving PCI (p < 0.01).CONCLUSIONS:The expression level of plasma miR-99a was remarkably reduced following AMI and closely associated with the severity of AMI. Therefore, it was a promising novel diagnostic biomarker for AMI.
Changes and Diagnostic Significance of miR-542-3p Expression in Patients with Myocardial Infarction

Abstract:Acute myocardial infarction (AMI) is a heart lesion, that endangers the life safety of patients. This study focused on exploring the clinical effect of miR-542-3p on AMI and no-reflow after percutaneous coronary intervention (PCI). Serum samples were collected from 100 AMI emergency inpatients. The expression of miR-542-3p was quantified by qPCR. The predictive role of miR-542-3p was disclosed by plotting ROC curve. In addition, AMI subjects were cataloged into a group of no-reflow and normal reflow group. The risk factors of no-reflow were estimated by logistic regression analysis. In the serum samples of AMI patients, the level of miR-542-3p showed a pattern of decreasing. MiR-542-3p expression represented a high sensitivity and specificity of the prediction of AMI. A decrease of miR-542-3p content was revealed in AMI patients without reflow after PCI. Logistic regression results reflected that miR-542-3p was an independent biomarker for no-reflow. The declined miR-542-3p expression was a predictive marker for AMI and no-reflow in AMI patients.

Analysis of Plasma Mir-208A and Mir-370 Expression Levels for Early Diagnosis of Coronary Artery Disease

Abstract:Coronary artery disease (CAD) requires more accurate diagnostic methods, for which circulating microRNAs (miRNAs) are promising non-invasive biomarkers. miR-208a and miR-370 are two key molecules in cardiac hemostasis and lipid metabolism, respectively. The aim of the present study was to evaluate their potency as diagnostic biomarkers for CAD. Plasma miR-208a and miR-370 were quantitated by reverse transcription-quantitative polymerase chain reaction (RT-qPCR) using a TaqMan® MicroRNA Reverse Transcription and PCR kit in 95 CAD patients and 50 non-CAD control subjects. The association between the miRNA levels and CAD was analyzed statistically. The plasma levels of miR-208a (P=0.006) and miR-370 (P=0.003) were significantly higher in the CAD group than in the control group. Using receiver operating characteristic analysis it was shown that the area under the curve (AUC) of miR-208a and miR-370 was 0.819 and 0.745, respectively. The combination of miR-208a and miR-370 exhibited the largest AUC of 0.856. Thus, miR-208a and miR-370 are promising diagnostic biomarkers for discriminating CAD and may facilitate the management of patient care. The combination of the two miRNAs may be more efficacious than either miRNA alone for the diagnosis of CAD.
Circulating miR-143 and miR-145 as promising biomarkers for evaluating severity of coronary artery stenosis in patients with acute coronary syndrome

Abstract:BACKGROUND: Multiple studies have provided evidence that miR-143 and miR-145 play a crucial role in the pathogenesis and progression of atherosclerosis. In the present study, we aim to investigate the expression of plasma miR-143 and miR-145 in patients with acute coronary syndrome (ACS) and their association with the severity of coronary artery stenosis.METHODS: The study enrolled 279 patients with ACS, including 201 patients with unstable angina (UA) and 78 patients with acute myocardial infarction (AMI), and 65 matched subjects as the control group. The plasma levels of miR-143 and miR-145 were detected by quantitative real-time PCR (qRT-PCR). Gensini score was applied to evaluate the severity of coronary artery stenosis.RESULTS: Plasma levels of miR-143 and miR-145 in patients with ACS were both decreased compared with the control group (p < 0.001). Plasma levels of miR-143 and miR-145 were negatively correlated with Gensini score (miR-143: r = -0.246, p < 0.001; miR-145: r = -0.222, p < 0.001). Logistic regression analysis showed that miR-143 and miR-145 were protective factors for the onset of ACS, UA, or AMI separately. MiR-143 (AUC: 0.786, p < 0.001) and miR-145 (AUC: 0.793, p < 0.001) were able to predict the degree of coronary artery stenosis greater than 50 %.CONCLUSION: The plasma levels of miR-143 and miR-145 were significantly decreased in ACS patients and were negatively correlated with coronary stenosis. In conclusion, plasma miR-143 and miR-145 levels can be used as noninvasive biomarkers for evaluating coronary artery stenosis.

Abstract:Background: MicroRNAs are present in human plasma and have been reported to be biomarkers for cardiovascular diseases. The aim of this study was to investigate the value of circulating microRNA-208b (miR-208b) for the diagnosis and prognosis of acute myocardial infarction (AMI). Methods: A total of 100 AMI patients, 80 unstable angina (UA) patients, and 80 healthy controls (HCs) were consecutively included in this study. Plasma was collected from each participant on admission, and the levels of circulating miR-208b were measured using reverse transcription-polymerase chain reaction (RT-PCR). The AMI patients who underwent percutaneous coronary intervention (PCI) were followed up at 6 months post-AMI. Results: The concentration of miR-208b was higher in the AMI patients than in the other two groups (p < 0.05), and it was positively correlated with the levels of creatine kinase (CK)-MB and cardiac troponin I (cTnI) (p < 0.01). Moreover, the receiver operating characteristic (ROC) curves showed that miR-208b was sensitive like CK-MB and cTnI for the diagnosis of AMI. In addition, the miR-208b concentration in AMI patients with three vessel coronary artery disease (CAD) was higher than that of single- or two-vessel CAD AMI patients (p < 0.05). Also, the miR-208b expression after PCI was significantly lower than on admission (p < 0.01). Furthermore, miR-208b expression in AMI patients with left ventricular remodeling/MACEs was higher than in those without after PCI (p < 0.05). Conclusions: Circulating miR-208b may serve as a sensitive biomarker for the diagnosis and prognosis of AMI patients.
Longitudinal change of serum exosomal miR-186-5p estimates major adverse cardiac events in acute myocardial infarction patients receiving percutaneous coronary intervention

Abstract:Objective: Our recently published study discovers that exosomal microRNA (miR)-186-5p promotes vascular smooth muscle cell viability and invasion to facilitate atherosclerosis. This research aimed to explore the prognostic implication of serum exosomal miR-186-5p in acute myocardial infarction (AMI) patients receiving percutaneous coronary intervention (PCI). Methods: One hundred and fifty AMI patients receiving PCI and 50 healthy controls (HCs) were screened. Serum exosomal miR-186-5p was detected by reverse transcriptase-quantitative polymerase chain reaction assay in AMI patients at admission and after PCI, as well as in HCs after enrollment. Major adverse cardiac events (MACE) were recorded during follow-up in AMI patients receiving PCI. Results: Serum exosomal miR-186-5p was raised in AMI patients vs. HCs ( P < 0.001). Besides, serum exosomal miR-186-5p was positively linked to body mass index ( P = 0.048), serum creatinine ( P = 0.021), total cholesterol ( P = 0.029), and C-reactive protein ( P = 0.018); while it was reversely linked with estimated glomerular filtration rate ( P = 0.023) in AMI patients. Interestingly, serum exosomal miR-186-5p was correlated with the diagnosis of ST-segment elevation myocardial infarction ( P = 0.034). Notably, serum exosomal miR-186-5p was decreased after PCI vs. at admission ( P < 0.001). The 6-, 12-, 18-, and 24-month accumulating MACE rates were 4.5%, 8.9%, 14.8%, and 14.8% in AMI patients. Furthermore, serum exosomal miR-186-5p ≥3.39 (maximum value in HCs) after PCI ( P = 0.021) and its decrement percentage <median (35%)="" ( P = 0.044) estimated elevated MACE in AMI patients. Conclusion: Serum exosomal miR-186-5p is reduced after PCI, and its post-PCI high level or minor decrease estimates increased MACE risk in AMI patients.

Abstract:Background: To study the change of cardiac-specific microRNA-208 (miRNA-208) level in acute myocardial infarction (AMI) patients and to explore the role of miRNA-208 in AMI progression. Methods: The consecutive subjects including 42 AMI patients, 22 patients with unstable angina (UA), and 40 healthy subjects in our hospital from January 2013 to December 2013 were enrolled in this study. The peripheral miRNA-208 level was measured with real-time reverse-transcription polymerase chain reaction (RT-PCR), and the plasma cardiac troponin I (cTnI) and creatine kinase-MB (CK-MB) levels were determined using electrogenerated chemiluminescence (ECL) method. Patients in the AMI group were further grouped according to the number of stenosed coronary vessels and primary percutaneous coronary intervention (PCI) or not, and the difference in peripheral miRNA-208 level among these subgroups was analyzed. Results: The miRNA-208 level was significantly higher in AMI group than in UA group and healthy controls immediately after admission (P<0.01). In the AMI patients, the plasma miRNA-208 level had a positive correlation with serum cTnI level (r=0.700, P=0.000). In 24 AMI patients who had undergone coronary angiography, the expression of miRNA-208 was significantly higher in patients with two or three stenosed coronary vessels. In 17 AMI patients who had successfully received emergent PCI treatment, the 24-h plasma miRNA-208 level was significantly lower than that immediately after admission (P<0.01). Conclusions: The peripheral plasma miRNA-208 level remarkably increases after cardiac infarction and may dramatically change along with the increase of the myocardial damage. Thus, it may be a new biomarker for AMI.
Circulating Mirnas Related to Long-term Adverse Cardiovascular Events in STEMI Patients: A Nested Case-Control Study.

Abstract:BACKGROUND:Long-term morbidity and mortality of patients with ST-segment-elevation myocardial infarction (STEMI) after primary percutaneous coronary intervention (PCI) remain substantial. Circulating microRNAs (miRNAs) play an important role in cardiovascular disease development. We aimed to identify circulating miRNAs associated with adverse cardiovascular events after acute myocardial infarction (AMI).METHODS:We performed a prospective, nested, case-control study of 932 patients with STEMI who underwent primary PCI. A 3-phase approach was conducted to screen candidate circulating miRNAs in 70 patients who subsequently experienced cardiac death, hospitalization for heart failure, or recurrent AMI (major adverse cardiovascular events [MACE] group) and in 140 patients matched for age, sex, time from symptom onset to blood collection and dual-antiplatelet therapy who did not report adverse cardiovascular events during 2-year follow-up (non-MACE group).RESULTS:We found that miR-26a-5p, miR-21-5p, and miR-191-5p levels were lower in the MACE group than in the non-MACE group (all P < 0.001). Multivariate conditional logistic regression analysis revealed that miR-26a-5p, miR-21-5p, and miR-191-5p levels were significantly inversely associated with incident primary composite outcomes (all adjusted P < 0.01). Importantly, the combination of these 3 miRNAs plus B-type natriuretic peptide clearly improved the risk scores recommended in the current guidelines, as determined with the use of C-statistics, net reclassification, and integrated discrimination.CONCLUSIONS:Our study provides proof-of-concept in humans that circulating miRNAs are associated with increased rates of distinct cardiovascular events, suggesting that they can serve as effective prognostic biomarkers and therapeutic targets for patients with AMI.

Abstract:BACKGROUND: MicroRNA-34a (miR-34a) plays an essential role in regulating blood lipid, inflammation, cell adhesion molecules, and atherosclerosis, the latter factors are closely involved in the etiology of coronary heart disease (CHD). However, the clinical value of miR-34a in CHD patients' management is rarely reported. Hence, this study aimed to assess the correlation of miR-34a with disease risk, blood lipid, coronary artery stenosis, inflammatory cytokines, and cell adhesion molecules of CHD.METHODS: A total of 203 CHD patients and 100 controls were recruited in this study, then their plasma samples were collected to detect the miR-34a by reverse transcription quantitative polymerase chain reaction. Furthermore, serum samples from CHD patients were obtained for inflammatory cytokines and cell adhesion molecule measurement by enzyme-linked immunosorbent assay.RESULTS: MiR-34a was elevated in CHD patients compared to controls (p < 0.001) and it disclosed a good diagnostic value of CHD (area under curve: 0.899, 95% confidence interval: 0.865-0.934). Besides, miR-34a positively correlated with triglyceride (p < 0.001), total cholesterol (p = 0.022) and low-density lipoprotein cholesterol (p = 0.004), but not with high-density lipoprotein cholesterol (p = 0.110) in CHD patients. Moreover, miR-34a associated with Gensini score in CHD patients (p < 0.001). As to inflammation-related indexes and cell adhesion molecules, MiR-34a expression was positively linked with C-reactive protein (p < 0.001), tumor necrosis factor alpha (p = 0.005), interleukin (IL)-1β (p = 0.020), IL-17A (p < 0.001), vascular cell adhesion molecule-1 (p < 0.001), and intercellular adhesion molecule-1 (p = 0.010) in CHD patients, but not with IL-6 (p = 0.118) and IL-10 (p = 0.054).CONCLUSION: MiR-34a might serve as a biomarker in assistance of diagnosis and management of CHD.

Abstract:Background: MicroRNA (miRNA) is reported to be present in human plasma and has been increasingly suggested as a biomarker for diseases. Our study aimed to investigate the kinetics of cardiac-specific microR-499 (miR-499) in acute myocardial infarction (AMI). Methods: Circulating concentrations of cardiac enriched miR-499 were measured by quantitative PCR in 73 patients with acute coronary syndrome (ACS), including 53 with AMI and 20 with unstable angina (UA). Thirty healthy subjects were used as controls. Plasma samples in AMI group were obtained immediately after admission and at 12 h, 24 h, 3 d and 7 d after onset of symptoms. Plasma samples in UA and healthy control groups were collected immediately after admission. The severity and extent of coronary stenotic lesions were evaluated on the basis of coronary angiography using Gensini score. Results: miR-499 expression levels were significantly higher in the 53 AMI patients than in the 20 UA patients and 30 healthy controls immediately after admission (P<0.01). A measurable increase in miR-499 levels was observed in AMI patients within 24 h of the last onset of chest pain and the levels returned to the baseline after 7 d. Plasma miR-499 levels in the patients with AMI were positively-correlated with cTnI (r=0.384, P<0.01) and CK-MB (r=0.402, P<0.01). In addition, miR-499 levels in AMI patients with two- and three-vessel coronary artery disease (CAD) were significantly higher than those in patients with single-vessel CAD (P<0.05). Gensini scores were used to evaluate the severity of coronary stenosis. miR-499 were positively correlated with Gensini scores (r=0.52, P<0.01). miR-499 levels at admission were significantly higher than that those 24 h after percutaneous coronary intervention (PCI) in AMI patients (P<0.01) and were negatively correlated with LVEF (r=0.36, P=0.008). Conclusions: Cardiac-specific miRNA-499 levels were found to be linearly proportional to myocardial damage. MiRNA-499 might prove to be a new biomarker for AMI and a predictor of the risk of myocardial ischemia.

Abstract:<section class="article-section__content"><h3 class="article-section__sub-title section1"> Objective</h3><p>Acute Myocardial Infarction (AMI) is the most severe type of coronary atherosclerotic heart diseases. MiRNA is a class of endogenous noncoding small molecule RNA, which plays an important regulatory role in the development of some diseases.</p></section><section class="article-section__content"><h3 class="article-section__sub-title section1"> Methods</h3><p>We examined the miRNA expression profiles in 16 patients with AMI compared with 6 non‐AMI controls using RNA sequencing.</p></section><section class="article-section__content"><h3 class="article-section__sub-title section1"> Results</h3><p>Compared with the miRNA expression profiles of non‐AMI controls, a total of 181 differentially expressed miRNAs were discriminated in AMI patients, of which 96 upregulated miRNAs and 85 downregulated miRNAs. The top ten upregulated miRNAs were as follows: miR‐449a‐5p, miR‐126‐5p, miR‐93‐5p, miR‐199a‐3p, miR‐4454, miR‐6880‐3p, miR‐3135a, miR‐548ad‐5p, miR‐4508, and miR‐556‐5p; while the top ten downregulated were as follows: miR‐6805‐5p, miR‐1228‐5p, miR‐939‐5p, miR‐615‐3p, miR‐6780a‐5p, miR‐6857‐3p, miR‐5088‐55p, miR‐7155‐3p, miR‐184, and miR‐4525. And the qRT‐PCR results of differentially expressed miRNAs showed the same result as high‐throughput sequencing data. For these 181 differentially expressed miRNAs, 19 841 target genes were predicted by GO analysis. The enrichment analysis revealed 2061 involved in biological processes, 353 in molecular function and 303 in cellular components. To identify biological pathways in AMI as compared to non‐AMI, the target genes of differentially expressed miRNAs were mapped to the classical signal transduction pathway in KEGG, indicating that 214 classes were enriched. ROC analysis showed that the circulating miRNAs had the important value for AMI diagnosis and supported the previous conclusions that circulating miRNAs were effective to diagnose the AMI as a novel biomarker.</p></section><section class="article-section__content"><h3 class="article-section__sub-title section1"> Conclusions</h3><p>Our findings require further research to confirm. It may provide a meaningful reference for the diagnosis and treatment of AMI.</p></section>

Early Diagnostic Value of Circulating Micrornas in Patients with Suspected Acute Myocardial Infarction

Abstract:Background/AimsEvidence has shown that several microRNAs (miRNAs) may be involved in coronary plaque rupture and local thrombus. However, the diagnostic ability of these miRNAs in acute myocardial infarction (AMI) is less known. The aim of this study is to explore the diagnostic value of these circulating miRNAs in patients presenting with acute chest pain in the emergency department. Methods and resultsIn a nested case-control study, 140 of 1,206 patients finally diagnosed with AMI were matched with 70 unstable angina and 70 noncardiac chest pain patients. Five candidate miRNAs (miR-483-5p, miR-155-5p, miR-451, miR-19b, and miR-223) were selected for validation. Among them, miR-19b, miR-223, and miR-483-5p were significantly higher in AMI patients compared with those without AMI. A multivariate analysis showed that these miRNAs were independent predictors of AMI. The overall areas under the receiver operating curves (AUCs) for miR-19b, miR-223, and miR-483-5p were 0.74, 0.65, and 0.70, respectively. However, serial sampling in AMI patients showed that these miRNAs already peaked on admission, which was earlier than troponin I. Among 170 patients with a negative troponin result at presentation, a panel of three miRNAs improved the discrimination ability to a clinical model. In 119 patients presenting within 3hr after chest-pain onset, the diagnostic accuracy of each miRNAs was higher than Point of care (POC) troponin assay. And a panel of these miRNAs had an AUC of 0.92. ConclusionCirculating miR-19b, miR-223, and miR-483-5p may provide clinically useful information for diagnosis in the early phases of AMI.

Abstract:Objective: The purpose of this study was to investigate the diagnostic value of circ_0013958 in acute myocardial infarction (AMI) patients and its influence on the prognosis of AMI patients. Methods: The GSE160717 dataset was downloaded from the NCBI database and differentially expressed genes were analyzed between the control group and the AMI group. The up-regulated genes included circ_0013958. The expression of circ_0013958 in both groups was further verified by RT-qPCR. The Receiver Operating Characteristic curve was used to evaluate the diagnostic value of circ_0013958 in AMI. Pearson correlation analysis was used to examine the correlation between circ_0013958 levles and biochemical indicators. Binary logistic regression was used to analyze the risk factors affecting the occurrence of AMI. Prognostic analysis was performed using COX regression analysis and the Kaplan-Meier Curve. Results: Compared to the control group, the level of circ_0013958 in AMI patients increased. Circ_0013958 can effectively distinguish AMI patients from non-AMI patients. Circ_0013958 levels were positively correlated with cTnI, LDH, CRP and TC levels. The elevated level of circ_0013958 was an independent risk factor for the occurrence of AMI. Higher circ_0013958 levels were also associated with the occurrence of major adverse cardiac events (MACEs) in AMI patients. Additionally, elevated circ_0013958 levels reduced the survival probability of AMI patients. Conclusion: Circ_0013958 levels were up-regulated in AMI patients. It can be used as a diagnosis biomarker for AMI. The level of circ_0013958 was correlated with the disease severity and was an independent risk factor for the occurrence of AMI. Elevated circ_0013958 levels were associated with poor prognosis in AMI patients.
