Abstract:Background: The fact that each inflammatory indicator has a forecasting capacity on the occurrence of periprocedural myocardial infarction (PMI) has a controversial existence. The purpose of this study was to explore the role of inflammation biological indicators on PMI in a group of patients undergoing selective percutaneous coronary intervention (PCI).Methods: The study was carried out both in a retrospective and prospective manner in 7,413 and 1,189 subjects, respectively. In the retrospective cohort study, the association between inflammation biomarkers and PMI was assessed by univariate and multivariate logistic regression. WBC, CRP, and NLR were distributed using k-means clustering into a virtual variable "Inflammatory Trend", and multivariate logistic regression and subgroup analysis were performed. In the prospective cohort study, the endpoints were PMI, cardiovascular death or cardiac arrest. The chi-square test was performed to calculate the relative risk (RR).Results: In the retrospective cohort study, except WBC, CRP, NLR and virtual variable "Inflammatory trend" were independent risk factors for PMI. The subgroup analysis revealed that CRP can serve as the most stable predictor. In the prospective cohort study, WBC (RR =1.134, P=0.416) has no effect on the incidence of PMI. However, an elevation in the incidence of PMI was observed with an increase of NLR (RR =1.354, P=0.041) and CRP (RR =1.412, P=0.025).Conclusions: In patients with elective PCI for single-vessel lesions, high CRP increases the risk for PMI. The increase of NLR was an independent risk factor for PMI, especially for patients with hypertension and under the age of 70. WBC has no influence on the occurrence of PMI.

Abstract:Objective: To investigate the role of C-reactive protein (CRP ) and fibrinogen(FG ) in the the pathogenesis of acute coronary syndrome (ACS). Method: Rate nephelometry with the Beckman systems was used to measure the plasma levels of CRP, FG, cTnI, CPK, CPK-MB, LDH,α-HAD and GOT in patients with ACS and none ACS as control. Result: The peak levels of CRP, FG, cTnI, CPK, CPK-MB, LDH,α-HAD and GOT in ACS group were significantly higher than those of control group(P 0.01). The peak levels of CRP, FG, cTnI, CPK, CPK-MB, LDH,α-HAD and GOT in STAMI and NSTAMI groups were also obviously higher than those of control group(P 0.01). Although the levels of CRP and FG in UAP group were significantly higher than those of control group(P 0.01), the levels of cTnI, CPK, CPK-MB, LDH,α-HAD and GOT in UAP group didn't significantly increase compared with non-ACS group(P 0.05). The level of CRP in ACS group was positively correlated with the peak level of cTnI, CPK,CPK-MB, LDH,α-HAD and GOT(P 0.01), the level of FG in ACS group was correlated with the peak level of α-HAD. Conclusion: The plasma levels of CRP and FG in ACS group are significantly elevated; the plasma level of CRP in ACS group is associated with the severity of the disease (myocardial damage), indicating that CRP play an important role in the pathogenesis of ACS.
Relationship Between Serum Concentrations of IL-18, IL-10, IL-6 and Acute Coronary Syndrome

Abstract:AIM: To examine the relation between serum concentrations of interleukin-18, interleukin-10, interleukin-6 and acute coronary syndrome (ACS). METHODS: Serum concentrations of IL-18, IL-10, IL-6 were measured in 17 patients with acute myocardial infarction (AMI), 30 patients with unstable angina pectoris (UAP), 15 patients with stable angina pectoris (SAP) and 20 controls by enzyme-linked immunosorbent assay (ELISA) and radioimmunoassay (RIA).The relation between IL-18, IL-6 and IL-10 was compared. RESULTS: Serum concentrations of IL-18, IL-6 were significantly increased in the AMI and UAP groups in comparison with the SAP and control groups. Conversely, serum concentrations of IL-10 were significantly decreased in the AMI and UAP groups in comparison with the SAP and control groups. The correlation of concentrations of IL-18 and IL-6 had no significance; but the levels of IL-18 and IL-6 were negatively correlated with IL-10. CONCLUSION: Serum IL-18, IL-6 concentrations increase while serum IL-10 concentration decreases in patients with acute coronary syndromes. The inflammatory imbalance between IL-18, IL-6 and IL-10 may play an important role in the instability of atherosclerotic plaque.
Predictive Value of Baseline C-reactive Protein for Periprocedural Myocardial Infraction of Higher Risk Stratifications: A Retrospective Cohort Clinical Study

Abstract:OBJECTIVE:It is controversial whether preprocedural elevated high sensitivity C-reactive protein (CRP) could increase the incidence of periprocedural myocardial infraction (PMI) of higher risk stratifications. The primary aim of this study was to evaluate whether preoperative elevated CRP level was related to the incidence of PMI in patients who underwent percutaneous coronary intervention (PCI).METHODS:A total of 4,426 patients [66 y (59, 75); 72.3% males] with normal preprocedural cardiac enzymes were prospectively divided into two groups; the elevated CRP group was defined as CRP >3 mg/L, which was approximately 30.4% of the patients. The relationship between CRP and the incidence of PMI was established by multivariate logistic regression analysis, and multivariate linear regression analysis was used to assess the correlation between CRP and the severity of myocardial injury.RESULTS:The incidence rates were similar between the two groups with periprocedural myocardial minor necrosis (34.23% versus 32.74%, p=0.607), but significantly differed based on the 2007 (defined as cardiac enzymes >3-fold elevations), 31.25% in high CRP group versus 26.25% in low group [odds ratio (OR) 1.19; p=0.046] and the 2012 universal PMI (defined as cardiac enzymes >5-fold elevations with at least one clinical evidence, such as chest pain, ECG changes or imaging diagnosis of heart ischemia), 19.79% versus 15.35% (OR 1.26, p=0.023); besides, the PMI ratios increased in line with the elevation of CRP (p=0.006 for the 2007 and p=0.011 for the 2012 universal PMI). However, no significant linear relationship was found between CRP and high sensitivity cardiac troponin I peak post-PCI.CONCLUSION:Elevated baseline CRP was an independent risk factor for the incidence of the 2007 and the 2012 universal PMI rather than minor necrosis. However, CRP may not correlate with the severity of minor myocardial necrosis in patients with PMI.
Association of postprocedural neutrophil count and myocardial perfusion in ST elevation myocardial infarction patients after percutaneous coronary intervention

Abstract:Objective:To explore the correlation of postprocedural neutrophil count,cardiac function and myocardial perfusion in ST elevation myocardial infarction patients after percutaneous coronary intervention(PCI).Method: A total of 195 STEMI patients underwent successful PCI were enrolled.Electrocardiograms were recorded before PCI and 12 hours after PCI.Main adverse cardiac events(MACE) were also recorded during the follow-up survey after 6 months(P<0.05) According to the level of Neutrophil after PCI,they were divided into 3 groups: <4.78×109/L(A group);4.78-8.79×109/L(B group);>8.79×109/L(C group).Result: ①After PCI,The proportion of patients with TIMI myocardial perfusion garde(TMPG) 0-1 in Band C group was greater than that of A group(P<0.05).The proportion of patients with TMPG 3 in B and C group was less than that of A group(P<0.05).②There were more patients with grade of Killp class ≥2 in C group than that in A and B group.③Compared with A and B group,patients in C group showed higher incidence of 6 months MACE.Conclusion:There is a strong association between postprocedural neutrophil count,cardiac function and myocardial perfusion,sugesting the early measurement of postprocedural neutrophil count is helpful in evaluating the reperfusion of myocardium and the prognosis of patients with AMI.

Abstract:Objective To observe the relationship between interleukin-6(IL-6),leukocytes count,percent monocytes,high-sensitivity C-reactive protein and left ventricular function and cardiac events in patients with acute myocardial infarction(AMI) after percutaneous coronary intervention(PCI).Methods Serum level of interleukin-6 was measured on the 3rd day and leukocytes count,percent monocytes and high-sensitivity C-reactive protein(hs-CRP) 24 hours after AMI in 32 patients with AMI after PCI,who were then followed 6 months.25 healthy persons without coronary heart disease were recurited as control.Results The level of IL-6 in patients with AMI was higher than that in control group(P<0.05).The level of IL-6 in AMI patients was correlated positively with degree of Killip(r=0.64,P<0.01),and negatively with left ventricular ejection fraction at 3 months and 6 months after AMI(all P<0.01).The level of IL-6 was higher in patients with Killip class≥2,severe arrhythmia and pericardial effusion than that in patients without cardiac events(P<0.05).The level of hs-CRP was higher in patients with Killip class≥2 than that in patients with Killip class 1.Conclusion The level of IL-6 in patients with AMI is the prognostic factor for left ventricular systolic function impairment and cardiac events.
Detection of Inflammatory Factors in Acute Myocardial Infarction Patients Undergoing Percutaneous Coronary Intervention and their Relationships with Prognosis

Abstract:Objective:To explore the change of inflammatory factors in acute myocardial infarction patients who underwent percutaneous coronary intervention and their relationships with prognosis.Methods:Selected 126 cases with acute myocardial infarction patients who would get percutaneous coronary intervention as research subjects.Serum high-sensitivity c-reactive protein(hs-CRP) and tumor necrosis factor α(TNF-α) were measured before and after PCI.Hs-CRP and TNF-α level between the patients who had cardiovascular events and those who had no cardiovascular events in the follow-up period were compared.Results:Serum hs-CRP and TNF-αlevel greatly increased after PCI as compared with preoperative(P0.05);Hs-CRP and TNF-αlevel on 12 hours after PCI in the patients who had cardiovascular events were much higher than that in those who had no card iovascular events(P0.05);Logistic regression results showed that hs-CRP and TNF-α level had significant correlation with the occurrence of cardiovascul arevents(P0.05).Conclusions:Acute inflammation occurred in acute myocardial infarction patients after PCI.The high levels of hs-CRP and TNF-α had close relationship with the occurrence of cardiovascular events,so they could be taken as the serological indexes for evaluating prognosis.

Abstract:Objective To explore the correlation of postprocedural neutrophil count,ST-segment resolution(STR) and NT-proBNP in ST elevation myocardial infarction patients(STEMI) after percutaneous coronary intervention(PCI).Methods A total of 195 STEMI patients who underwent successful PCI were enrolled.Electrocardiograms were recorded before PCI and two hours after PCI.Neutrophil counts and NT-proBNP were measured within 12 hours after PCI.Cardiac main adverse cardiac events(MACE) were also recorded during the follow-up survey after six months(P<0.05).According to the level of neutrophil after PCI,the patients were divided into three groups:group A(neutrophil<4.78×109/L),group B(neutrophil 4.78-8.79)×109/L,group C(neutrophil>8.79×109/L).Results Compared with group A,patients in group C less frequently had complete STR(17.8% vs 43.8%,P<0.05).The percentages of high neutrophil and plasma NT-proBNP were significantly higher in patients with STR<30%,compared those in patients with STR>70%(33.3% vs 13.8%,34.9% vs 12.3%,P<0.05 or <0.01).Linear relativity analysis showed that postprocedural neutrophil count was positively correlated with the level of NT-proBNP in group C.No statistical differences were found between the groups at incidence of six months' MACE(P>0.05).Conclusion There is a strong association between postprocedural neutrophil count,STR and NT-proBNP,suggesting the early measurement of postprocedural neutrophil count is helpful in evaluating the reperfusion of myocardium and the prognosis of patients with acute myocardial infarction.
[Relation Between Inflammation Related Cytokines and Myocardial Microcirculatory Reperfusion State].

Abstract:OBJECTIVE:To observe the dynamic fluctuation of inflammatory cytokines (IL-1beta, TNFalpha) and anti-inflammatory cytokine (IL-10) in acute myocardial infarct (AMI) patients before and after recanalization of infarct related artery (IRA) and analyze the relationship between fluctuation of cytokines and reperfusion state of myocardial tissue. METHODS:(1) In 22 AMI patients and 8 healthy subjects, plasma IL-1beta, TNFalpha and IL-10 were measured with ELISA before emergency percutaneous coronary interposition (PCI), 12 h and 24 h post-intervention. (2) The 22 AMI patients were further divided into 2 groups according to ST-segment change in ECG at 2h after reperfusion: group A, good reperfusion of myocardium, ST decrease >or= 70% (n = 12) and group B, poor reperfusion of myocardium, ST decrease < 70% (n = 10). The change of plasma levels of IL-1beta, TNFalpha and IL-10 of the two groups was compared. RESULTS:(1) Plasma TNFalpha and IL-10 in group A and B were not significantly higher those that the healthy controls (P > 0.05) before emergency PCI, but IL-1beta was significantly higher (P < 0.05). Plasma IL-1beta and TNFalpha in group A and B and IL-10 in group B at 12 h and 24 h post-intervention were significantly higher than those before PCI (P < 0.01, P < 0.05, P < 0.05), while IL-10 in group A was not (P > 0.05). (2) Plasma IL-1beta, TNFalpha and IL-10 in group B were not higher than those in group A before PCI, but they were significantly higher at 12 h post-PCI (P < 0.01, P < 0.05, P < 0.05) and IL-1beta and IL-10 were still higher at 24 h post-PCI (P < 0.05). The increment of IL-10 was significantly less than that of IL-1beta and TNFalpha in group A and B (P < 0.01, P < 0.05). CONCLUSIONS:Cytokines may be involved in myocardial ischemia-reperfusion injury and increase of inflammatory cytokines may be a marker of poor myocardial microcirculatory reperfusion.
Effect of the Plasma Interleukin- 1β Level on Prognosis of Patients with ST-segment Elevation Acute Myocardial Infarction

Abstract:Objective Atherosclerosis is widely accepted as a chronic inflammatory disease. Serum biomarkers for vulnerable plaques not only serve as diagnostic tools for the identification of patients with acute coro-nary syndrome, but also assist the identification of high-risk patients. However, the existing data are limited and conflicting. In the present study, we determined whether the plasma levels of interleukin-1β (IL-1β) are correlated with adverse cardiac outcomes in patients with ST-evaluate acute myocardial infarction (STEAMI) undergoing pri-mary percutaneous coronary intervention (PCI). Effect of the plasma intedeukin-1β level on prognosis of patients with ST-segment elevation acute myocardial infarction. Method This prospective single-center study included 96 patients with SIEAMI with onset < 12 h who underwent primary PCI, 271 patients with stable angina pectoris (SAP) and 148 control subjects without coronary artery disease who were consecutively admitted to hospital be-tween Mar, 2006 and Mar, 2008. Plasma IL-1β levels were measured by enzyme-linked immunosorbent assay in all subjects. The patients with STEAMI were then followed prospectively for the occurrence of major adverse car-diac events (MACE) (including cardiovascular death, non-fatal myocardial infarction, heart failure, and cardio-genie shock) during hospitalization. We determined the association between IL-1β levels with the risk of MACE using multivariate logistic regression. Results Compared with the SAP patients and control subjects, patients with STEAMI had higher levels of IL-1β (P < 0.05). During hospitalization, 32 patients (33.3%) experienced MACE [23 males, 9 females; age: (75.44±13.45) years]. In the STEAMI patients, IL-1β was elevated in patients with MACE compared with patients without MACE (median [range]: 26.52 [12.010 to 155.244] pg/mL vs 2.157 [0.433 to 83.021] pg/mL; P < 0.01) by non-parameter analysis. Significant and positive correlations be-tween IL-1β and cardiac troponin-I (cTnI) (r = 0.353, P =0.004) were observed by Spearman's correlations analysis. Multivariate logistic regression analysis revealed that IL-1β levels ≥20 pg/mL were significantly and in-dependently associated with MACE during hospitalization (odds ratio: 32.05; 95% confidence interval: 4.28 to 240.151; P =0.001). Conclusions The present study revealed that patients with STEAMI had elevated IL-1β levels on admission. The plasma IL- 1β level is an independent inflammatory predictor for in-hospital MACE in pa-tients with STEAMI undergoing percutaneous coronary intervention.
Renal Dysfunction and hsCRP Predict Long-term Outcomes of Percutaneous Coronary Intervention in Acute Myocardial Infarction.

Abstract:Background: This study assessed the combined utility of estimated glomerular filtration rate (eGFR) and serum high-sensitivity C-reactive protein (hsCRP) levels to predict long-term mortality and cardiovascular outcomes of patients with acute ST-segment elevation myocardial infarction (STEMI) undergoing primary percutaneous coronary intervention (PCI). Elevated CRP levels and renal dysfunction have both been shown to independently and jointly predict mortality and cardiovascular outcomes after PCI in the short term. However, long-term results in patients with acute STEMI undergoing PCI have not been reported. Methods: A total of 262 patients with acute STEMI undergoing primary PCI were classified at admission into quartiles according to eGFR (<60, 60-70, 70-80 and >= 80 mL.min(-1).1.73 m(-2)) and hsCRP (< 3 and >= 3 mg/L). Mortality, nonfatal myocardial infarction (MI) and major adverse cardiac events (MACEs) were compared among the groups. Results: During a median follow-up of 48.3 months, the composite of all-cause mortality and nonfatal MI (mortality + MI) was significantly higher (35.09%) in the group with the lowest eGFR compared with that of the other 3 eGFR groups (14.29%, 3.77% and 9.43%, respectively, P, < 0.0001) and the group with elevated hsCRP (34.29%) versus that with hsCRP <3 mg/L (4.41%, P, 0.0001). A combined analysis showed an exaggerated hazard in patients with the lowest eGFR and highest hsCRP (hazard ratio: 44.658; 95% confidence interval: 5.955-111.890). Conclusions: Renal dysfunction and elevated hsCRP predict a high long-term incidence of MACE in patients with acute STEMI undergoing primary PCI, with the combination being of prognostic significance for long-term mortality and MI in these patients.

Abstract:OBJECTIVE:To investigate the temporal changes of serum cytokines IL-6, IL-8, MCP-1 and IL-10 immediately before and after primary PCI (percutaneous coronary intervention) in patients with ST-segment elevation myocardial infarction (STEMI) and explore the interaction of these cytokines. METHODS:A total of 59 STEMI patients were recruited. And their serum concentrations of IL-6, IL-8, MCP-1 and IL-10 were measured by ELISA before primary PCI and 4-6 hours, 12 hours, 24 hours and 7 days post-intervention. For each cytokine, the level at each time-point post-PCI was compared to that at pre-PCI. Correlation coefficient test was used to analyze the interactions of these four cytokines. RESULTS:At 12 hours post-PCI, the median serum levels of IL-6 and IL-8 were higher than those before PCI (IL-6: 8.51 ng/L vs 6.76 ng/L, IL-8: 4.67 ng/L vs 2.95 ng/L, both P < 0.05). At 4-6 hours and 12 hours post-PCI, the median values of MCP-1 were increasing significantly compared to those at pre-PCI (35.04 ng/L, 34.24 ng/L vs 30.45 ng/L, both P < 0.05) while those of IL-10 decreased (18.15 ng/L, 18.82 ng/L vs 20.95 ng/L, both P < 0.05). The levels of IL-6 and IL-10 at pre-PCI were associated with the Killip classification on admission (IL-6: r = 0.293, P < 0.05; IL-10: r = -0.287, both P < 0.05). Except for IL-8, other cytokines had no significant relation with the time length from onset to admission (P > 0.05). Additionally, these four cytokines were not found to be related with the location and extension of myocardial infarction, ejection fraction and NT-proBNP. At each time-point, there were a positive relationship among the natural logarithms of the concentrations of IL-6, IL-8 and MCP-1 (P < 0.01), all of which were inverse to the natural logarithm of the concentration of IL-10 (P < 0.01). CONCLUSIONS:The pro-inflammatory cytokines increase while the anti-inflammatory cytokines decrease after myocardial ischemia/reperfusion. An imbalance of inflammatory cytokines may be present.
Interleukin-18 Levels on Admission Are Associated with Mid-Term Adverse Clinical Events in Patients with ST-Segment Elevation Acute Myocardial Infarction Undergoing Percutaneous Coronary Intervention

Abstract:The long-term prognostic value of interleukin (IL)-18 in patients with ST-segment elevation acute myocardial infarction (STEM!) has been conflicting. Thus, the purpose of this study was to test whether the level of interleukin-18 measured on admission can predict long-term adverse clinical events in patients with STEMI who were undergoing percutaneous coronary intervention (PCI).We recruited 288 consecutive STEMI patients (210 men, average age [71.42 +/- 10.32] years) with onset <6 hours who were undergoing primary PCI, and 148 age- and gender-matched control subjects. Plasma levels of IL-18 were measured by enzyme-linked immunosorbent assay (ELISA) in all subjects. The patients with STEMI were then followed prospectively over 434 days (range, 0 to 642 days) for the occurrence of composite major adverse clinical events (MACE) (cardiac mortality, recurrent myocardial infarction, or readmission due to advanced heart failure). Patients with STEMI exhibited higher levels of plasma IL-18 (P < 0.001) compared with the control subjects. Positive correlations between IL-18 and cardiac troponin-I (cTnI) (r = 0.353, P = 0.0004) and IL-18 and high-sensitivity C-reactive protein (hs-CRP) (r = 0.420, P < 0.001) were observed by Spearman's correlations analysis. Logistic regression analysis demonstrated that IL-18 >= 450 pg/mL (OR 10.854, 95% CI 2.328 to 50.594, P < 0.0001) was a significant independent predictor of composite MACE at 60 days. Cox regression analysis demonstrated that high plasma IL-18 levels were not correlated with the occurrence of long-term composite MACE.The level of plasma IL-18 on admission may predict 60-day adverse clinical outcome, but not the long-term adverse clinical events in patients with STEMI undergoing PCI, and may be useful for mid-term risk stratification. The Influence of Emergency Percutaneous Coronary Intervention on Plasma Cytokines in Acute Myocardial Infarction

Abstract:Objective To observe the dynamic changes of interleukin-1β,interleukin-6 and interleukin-10 in AMI patients before and after recanalization of infarct-related artery(IRA).Methods In 22 AMI patients and 8 healthy men,plasma IL-1β,IL-6 and IL-10 were measured by ELISA before emergency PCI,12 h and 24 h post-intervention.Amplitudes of changes in plasma IL-1β,IL-6 and IL-10 were compared.Results Plasma(IL-10) in AMI was not significantly higher than that in healthy controls(P0.05) before emergency PCI,but IL-1β and IL-6 were significantly higher[(27.98±8.76) ng/L vs(20.44±11.32) ng/L,P0.05;((31.89±)6.89) ng/L vs(15.55±3.81) ng/L,P0.05].Plasma IL-1β,IL-6 and IL-10 at 12 h and 24 h post-intervention were significantly higher than those before PCI(P0.01,P0.01,P0.05).In patients with successful PCI and having TIMI grade-3 flow in IRA,the amplitude of IL-10 increase was significantly less than that of IL-1β and IL-6(P0.01).Conclusions IL-1β,IL-6 and IL-10 may be involved in myocardial ischemia-reperfusion injury.The imbalance between inflammation-promoting and anti-inflammatory cytokines may be one of the mechanisms resulting in myocardial ischemia-reperfusion injury.

Abstract:Objective: This study aims to investigate the value of interleukin 6 (IL-6) and high-mobility group box 1 (HMGB1) in predicting major adverse cardiovascular events (MACE) after percutaneous coronary intervention (PCI) in patients with acute coronary syndrome (ACS). Methods: Patients with ACS who were treated in our hospital from October 2022 to October 2023 were divided into MACE and no-MACE groups according to the occurrence of MACE after PCI. The baseline data and IL-6 and HMGB1 levels in the two groups were observed, and the influencing factors of MACE in patients with ACS after PCI were evaluated with a logistic regression test. The receiver operator characteristic curve (ROC) values of IL-6 and HMGB1 in the prediction of MACE after PCI in patients with ACS were calculated. Results: No significant differences in age, sex, body mass index (BMI), and other general data were found between the groups. Compared with the patients in the no-MACE group, the patients in the MACE group had a history of smoking (p = 0.011), hypertension (p < 0.001), diabetes (p < 0.001), more coronary lesions (p = 0.013), longer coronary lesions (p = 0.006), higher preoperative Gensini score (p < 0.001), and lower left ventricular ejection fractions (LVEF) (p < 0.001). The levels of IL-6 and HMGB1 in the MACE group were significantly higher than those in the no-MACE group. Coronary lesion length, Gensini score, LVEF, IL-6, and HMGB1 had good predictive value for MACE after PCI. The area under the curve (AUC) scores were 0.683, 0.941, 0.816, 0.878, and 0.737. The sensitivity was 53.13%, 81.25%, 84.37%, 78.12%, and 53.13%, and the specificity was 87.50%, 93.18%, 63.64%, 86.36%, and 86.36%, respectively. Analysis of IL-6 and HMGB1 levels showed that the AUC was 0.922, the sensitivity was 90.62%, the specificity was 82.95%, and the 95% confidence interval (CI) was (0.858–0.963; p < 0.05). Conclusion: IL-6 and HMGB1 have good predictive value for MACE after PCI for patients with ACS and can be used as clinical evaluation indexes.

Abstract:Objective Recent views hold atherosclerosis to be an active inflammatory process rather than a simple lipid infiltration.This study was to investigate the changes in the levels of serum IL-6,IL-10,IL-6/IL-10,TNF-α and HsCRP and their implications in patients with acute coronary syndrome(ACS).Methods A total of 189 patients undergoing coronary angiography(CAG) were assigned to a control group(n=60) and an ACS group(n=129) according to the results of CAG,the latter including 66 cases of acute myocardial infarction(AMI) and 63 cases of unstable angina(UA).The levels of serum IL-6,IL-10 and TNF-α were measured by ELISA,and that of HsCRP determined by immunityturbidimetry.Result The level of serum IL-6 was significantly more activated in the ACS group(6.38±2.52 ng/L,P0.01) as well as in the subgroups of AMI(4.84±8.74 ng/L) and UA(2.57±2.87 ng/L) than in the control group(1.62±1.71 ng/L,P0.05).So was the level of IL-10 in the AMI(17.56±2.08 ng/L) and UA patients(16.84±2.74 ng/L) than in the controls(15.69±1.78 ng/L,P0.05),but with no significant difference between AMI and UA(P0.05).The ratio of IL-6/IL-10 was significantly higher in the AMI(0.27±0.48 ng/L) and UA patients(0.22±0.17 ng/L) than in the controls(0.10±0.09 ng/L,P0.05).So was the level of TNF-α(44.56±30.62 pg/ml and 41.12±24.34 pg/ml vs 28.61±17.52 pg/ml,P0.05),but with no significant difference between AMI and UA(P0.05).The same was true for the level of HsCRP(25.5±29.5 mg/L and 9.93±11.05 mg/L vs 4.02±4.29 mg/L,P0.05).In the ACS patients,the level of serum IL-6 was positively correlated with that of HsCRP(r=0.395,P0.05) and that of TNF(r=0.249,P0.05).Conclusion Changes in the levels of L-6/IL-10,TNF-α and HsCRP are correlated with ACS,and may have a predictive value for its plaque instability.
Periprocedural Myocardial Infarction Enhances the Predictive Value of Inflammatory Biomarkers for Patients with Obstructive Coronary Artery Disease after Implantation of Drug-Eluting Stent

Abstract:Objective: The present study aims to clarify the long-term clinical importance of interleukin (IL)-6 in predicting major adverse cardiac events (MACE) for an entire cohort of patients with coronary artery disease after implantation of a drug-eluting stent ( DES), and its interplay with periprocedural myocardial infarction (PMI).Background: The correlation of proinflammatory biomarkers with occurrence of clinical events, including PMI and mortality, is controversial.Methods: IL-6 was measured in 1,991 patients at admission. The participants were then assigned to two groups: IL-6 level >= 9 pg/mL and IL-6 level <9 pg/mL. The primary endpoint was the occurrence of cardiac death or myocardial infarction (MI) at year 3 after indexed percutaneous coronary intervention (PCI) procedure.Results: The in-hospital rate of the primary endpoint in the IL-6 level >9 pg/mL group was 9.1%, statistically significantly different to 6.3% in the IL-6 <9 pg/mL group (P=0.026), mainly driven by the increased rate of MI (9.1% vs 6.1%, P=0.025). Those differences in MI/death and MI was sustained through to the 3-year follow-up (10.9% vs 7.6%, P=0.017 and 10.1% vs 7.5%, P=0.049). At the 3-year follow-up after the assigned procedure, more frequent MI was also the main reason for increased composite MACE between the IL-6 >= 9 pg/mL and IL-6 <9 pg/mL groups (20.7% vs 15.8%, P=0.007). In the IL-6 >9 pg/mL group, PMI was strongly correlated with mortality at 1-year through to the 3-year (hazard ratio: 2.96, 95% confidence interval: 1.35-6.49, P=0.005) follow-up after PCI procedure.Conclusion: Elevated preprocedural serum IL-6 level was correlated with death, MI, and MACE after implantation of the DES. PMI enhances the predictive value of IL-6 for post-DES events.

Abstract:OBJECTIVE:To investigate the changes of high-sensitive C-reactive protein (hs-CRP) and monocyte chemotactic factor-1 (MCP-1) following percutaneous coronary interventional procedures (PCI) in patients with coronary artery disease (CAD), and evaluate the impact of PCI on the inflammatory indices and postoperative vascular restenosis. METHODS:This study involved 80 patients undergoing PCI procedures for CAD compromising a single coronary artery. Forty healthy individuals with normal findings by coronary angiography were selected as the control group. Before and after PCI or coronary angiography, plasma hs-CRP and MCP-1 were measured in all the subjects by immunonephelometry and enzyme-linked immunosorbant assay (ELISA), respectively. RESULTS:In the CAD patients, the plasma hs-CRP level was significantly elevated after PCI as compared with the preoperative level (2.37-/+0.56 microg/L vs 1.59-/+0.41 microg/L, P<0.01), whereas in the control group, the hs-CRP level underwent no significant changes after coronary angiography (1.18-/+0.37 microg/L vs the preoperative level of 1.13-/+0.32 microg/L, P>0.05). PCI procedures also resulted in significant elevation of plasma MCP-1 level in the CAD patients (26.04-/+5.43 pg/L vs the preoperative level of 18.07-/+4.30 pg/L, P<0.01), but in the control group, MCP-1 showed no significant variation after coronary angiography (9.80-/+2.64 pg/L vs the preoperative level of 9.63-/+2.52 pg/L, P>0.05). CONCLUSION:Plasma hs-CRP and MCP-1 are elevated in CAD patients following PCI procedures, but their roles in the vascular restenosis following the procedures need further investigation.

Abstract:The inflammatory hypothesis of atherosclerosis is appealing in acute coronary syndromes, but the dynamics and precise role are not established.The study investigates the levels of C reactive protein (CRP), interleukin 1β (IL-1β) and stromal-derived factor 1α (SDF-1α) at the time of acute myocardial infarction (AMI) and at 1 and 6 months afterwards, compared with a control group.In the acute phase of AMI, CRP and SDF-1α were significantly higher, while IL-1β showed lower levels compared with controls. CRP positively correlated with coronary stenosis severity (rho = 0.3, p=.05) and negatively related with left ventricle ejection fraction (LVEF) at 1 month (rho= −0.43, p=.05). IL-1β weakly correlated with the severity of coronary lesions (rho =0.29, p=.02) and strongly with LVEF (rho= −0.8, p=.05). SDF-1α, slightly correlated with LVEF at 1 month (rho = 0.22, p=.01) and with the severity of coronary atherosclerosis (rho= −0.41, p=.003).CRP, IL-1β and SDF-1α have important dynamic in the first 6 months after AMI and CRP and SDF-1α levels correlated with the severity of coronary lesions and LVEF at 1 month after the acute ischaemic event.

Abstract:Background High-sensitivity C-reactive protein (hs-CRP) has been proposed as an indicator of inflammation and cardiovascular risk. However, little is known of the comparative temporal profile of hs-CRP and its relation to outcomes according to the disease acuity. Methods We enrolled 4,263 East Asian patients who underwent percutaneous coronary intervention (PCI) for acute myocardial infarction (AMI) and stable disease. hs-CRP was measured at baseline and 1 month post-PCI. Major adverse cardiovascular events (MACE: the composite occurrence of death, myocardial infarction, or stroke) and major bleeding were followed up to 4 years. Result The AMI group ( n = 2,376; 55.7%) had higher hs-CRP baseline than the non-AMI group ( n = 1,887; 44.3%) (median: 1.5 vs. 1.0 mg/L; p < 0.001), which remained higher at 1 month post-PCI (median: 1.0 vs. 0.9 mg/L; p = 0.001). During 1 month, a high inflammatory-risk phenotype (upper tertile: hs-CRP baseline ≥ 2.4 mg/L) was associated with a greater MACE in the AMI group (adjusted hazard ratio [HR adj ]: 7.66; 95% confidence interval [CI]: 2.29–25.59; p < 0.001), but not in the non-AMI group (HR adj : 0.74; 95% CI: 0.12–4.40; p = 0.736). Between 1 month and 4 years, a high inflammatory-risk phenotype (upper tertile: hs-CRP 1 month ≥ 1.6 mg/L) was associated with greater MACE compared to the other phenotype in both the AMI (HR adj : 2.40; 95% CI: 1.73–3.45; p < 0.001) and non-AMI groups (HR adj : 2.67; 95% CI: 1.80–3.94; p < 0.001). Conclusion AMI patients have greater inflammation during the early and late phases than non-AMI patients. Risk phenotype of hs-CRP baseline correlates with 1-month outcomes only in AMI patients. However, the prognostic implications of this risk phenotype appears similar during the late phase, irrespective of the disease acuity. REGISTRATION: URL: https://www.clinicaltrials.gov .
