Abstract:Background: The accurate transfer of the gingival contours of implant-supported restorations is essential for the optimal definitive prosthesis. In recent years, intraoral optical scanner (IOS) has been frequently used in clinics to obtain the soft tissue profile and the position of the implant for the permanent restoration. However, little scientific evidence is available regarding the accuracy of IOS systems for obtaining gingival contour in the esthetic zone. Aim/Hypothesis: This study is aimed to compare the accuracy of peri-implant soft-tissue in the esthetic zone when an intraoral optical scan is applied with or without an implant-supported provisional restoration (ISPR). Materials and Methods: Ten participants with single ISPR in the maxillary esthetic zone were recruited in this study. The thickness and texture of the peri-implant gingival tissue were evaluated INDIApendently by two clinicians. After that, two digital impression procedures were performed by making an intraoral scan (TRIOS, 3Shape, Copenhagen, Denmark) of the gingival contours before and immediately after ISPR removal. The control procedure was formed by fabricating definitive casts from the conventional polyether impression using the ISPR as a customized impression-transfer and making digital impressions of these definitive casts. Both images of paired groups were superimposed using image analysis software (Geomagic Control X) and the accuracy was evaluated through the deviation value of 3D comparison with the control group. Data were presented as mean ± 95% confidence interval, and a paired t-test was performed. Statistical difference was considered when P < 0.05. Results: The overall peri-implant soft tissue deviation equated to 197.2[146.7 247.7] μm and 397.4[303.4 491.4] μm when intraoral scans were performed with and without ISPR, respectively, which presented statistically significant differences (P = 0.0014). When the intraoral digital scan without an ISPR was performed, we observed a obvious gingival papilla collapse particularly in the palatine side of the buccal mucosa (Pbm) and the buccal side of the palatine mucosa (Bpm), regardless of patients’ gingival thickness and texture. The intraoral scanning without ISPR led to a tissue deviation of 456.6[286.3,626.3] μm and 484.5[334.4,634.5] μm measured from the middle point of the Pbm and the Bpm, respectively, which was significantly higher than the tissue deviation from the intraoral scanning with ISPR measured from the same position (P < 0.01) Conclusions and Clinical Implications: Our data showed that intraoral scanning without ISPR led to a significant peri-implant gingival tissue deviation, indicating the ISPR is crucial to maintain the gingival contour and emergence profile in the maxillary esthetic zone. Therefore, it is necessary to have the appropriate gingival support to improve the accuracy of intraoral scanning for peri-implant soft-tissue in the maxillary esthetic zone.
