Abstract:AbstractAimTo evaluate the long‐term (≥2 years) stability of root coverage procedures for single gingival recessions.Materials and MethodsA complete literature search was performed up to July 2018. Randomized controlled trials (RCTs) following ≥2 years were selected. Primary outcomes were complete root coverage (CRC) and mean root coverage (MRC). Secondary outcomes were width of keratinized tissue (KTW) and patient‐centred parameters. Meta‐analysis was conducted when possible.ResultsA total of fifteen RCTs were included. The results demonstrated significantly higher MRC in short‐term than long‐term after coronally advanced flap (CAF; 7.29%, p = 0.006). When CAF combined with connective tissue graft (CTG), no significant difference was observed in CRC or MRC for short‐term versus long‐term (1.00, p = 0.97; 2.35%, p = 0.09), and it resulted in better long‐term efficacy than CAF alone in terms of CRC (0.69, p = 0.0006) and KTW (−0.63 mm, p = 0.04). For CAF plus enamel matrix derivative, the meta‐analysis showed no significant difference between the short‐term and long‐term results of CRC (1.26, p = 0.21).ConclusionsCAF alone could result in decreased postoperative percentage of root coverage with time. CAF + CTG could maintain long‐term stability and result in better root coverage outcomes than CAF.

Abstract:OBJECTIVE: The use of periodontal probes harbors the risk of measurement errors. The aim of this study was to investigate the accuracy of the digital determination of gingival recession and papilla height on virtual jaw models, given the hypothesis that they show lower intrarater and interrater variability than conventional linear measurements taken clinically or on cast models.MATERIALS AND METHODS: Gingival recessions and papilla heights were measured at 30 sites by five examiners using the following methods: (A) direct measurements using a periodontal probe intraorally or (B) on cast models using a caliper, (C) digital measurements on virtual models obtained by optical scans taken intraorally, or (D) made of cast models using the same software. Measurements were repeated after 1 week and intraclass, intrarater, and interrater correlations of the measurements using the four different methods were analyzed.RESULTS: The greatest disagreement between the 1st and 2nd measurement was identified for method A. Recessions were less reliably measured than papillae. The best agreement between methods was found in the digital ones (C and D). Regarding papilla height, increased values were obtained when method D was applied as compared with both clinical evaluations. For gingival recession, method A measured the highest values.CONCLUSION: In the present study, the use of digital technologies by intraoral scanning or scanning of cast models improved the reproducibility and lowered the variance of measurements within one individual and between different investigators.CLINICAL SIGNIFICANCE: Providing a more reliable and examiner-independent technique for measurements of the soft tissue architecture could improve data quality in periodontal research and in the analysis of different clinical treatment modalities.

Abstract:BACKGROUND To better understand local migration of inflammatory cells in psoriasis, we compared immunohistochemically the expression of cell adhesion molecules on endothelial cells of papillary microvessels, a subpapillary microvessel (SPMV), with the phenotypic profile of infiltrating T cells in initial, active, and involuting psoriatic lesions. RESULTS Intercellular adhesion molecule-1, E-selectin, and vascular cell adhesion molecule-1 (VCAM-1) on papillary microvessel E-selectin and VCAM-1 on SPMV that had not been detected in normal/noninvolved skin were induced in psoriatic lesions. Intercellular adhesion molecule-1 on SPMV was constitutively expressed in normal/noninvolved skin and augmented in the degree of expression in psoriatic lesions. Intraepidermal and dermal angiocentric T cells in the initial and active lesions belonged predominantly to the CD3+, CD4+, CD45RO+, lymphocyte function-associated antigen-1+, and very late antigen-4+ helper-inducer/memory subset. The number of these memory T cells around SPMV was significantly correlated with the degree of expression of E-selectin and VCAM-1 on SPMV in the initial lesion. Intraepidermal memory T cells in the active lesion showed significant correlation with the expression of E-selectin and VCAM-1 on PMV. The CD8+ cells were dominant in the epidermis of the involuting phase. None of the adhesion molecules studied seemed to play a role in infiltration of this cell type. CONCLUSIONS The results suggest (1) participation of memory T cells in the formation of the initial and active stages of psoriatic plaques, and (2) E-selectin and VCAM-1 on endothelium as the critical adhesion molecule for initial trafficking of memory T cells into psoriatic lesions.

Abstract:Abstract Background: The aim of this study to investigate three-dimensional quantitative analysis buccal augmented tissue alterations after modified coronally advanced tunnel (MCAT) technique combined with subepithelial connective tissue graft (SCTG) within 1-year based on intraoral scanning.Methods: 25 Cairo class I gingival recession defects were treated by MCAT technique with SCTG. Two examiners performed digital three-dimensional measurements, including the preoperative gingival recession height (GH), width (GW) and root exposed area (REA), postoperative gingival height gain (HG), area gain (AG), volume gain (VG) and mean thickness gain (MTG) of region of interest (ROI). Then, dynamic alterations of ROI and buccal tissue at 2 weeks, 6 weeks, 3 months and 1-year post-op were analysis. Results: Intra-class correlation coefficients (ICC) showed almost perfect inter-examiner reliability for digital measurement (ICC > 0.89). Postoperative HG, AG, VG and MTG were encountered distinct at 2 weeks post-op, then gradually decreased. At 1-year, AG, VG and MTG were 7.614 ± 2.511mm2, 7.690 ± 4.335mm3 and 0.965±0.372mm, respectively. Significant decreases were recorded between 6 weeks and 1-year in terms of HG, AG and VG. The MTG was sustained after 6 weeks with an increase of nearly 1mm at 1-year.Conclusions: Digital measurement by intraoral scanning offered an advantage of clinically feasible, non-invasive and reliable evaluation method for achieving volumetric outcomes after periodontal plastic surgery. Augmented tissue acquired by MCAT with SCTG tends to be stable after 3 months post-op.Trial registration：This study was retrospectively registered in Chinese Clinical Trial Registry: ChiCTR1900026768. Abstract:Background The aim of this study is to investigate three-dimensional quantitative analysis of buccal augmented tissue alterations after surgery using a modified coronally advanced tunnel (MCAT) technique combined with a de-epithelialized gingival graft (DGG) within 1 year post-op, based on intraoral scanning. Methods 25 Cairo class I gingival recession defects were treated using an MCAT technique with DGG. Digital impressions were taken using an intraoral scanner at baseline, 2 weeks, 6 weeks, 3 months, and 1 year after the surgery. Three-dimensional quantitative measurements within 1 year were analyzed for buccal augmented tissue after surgery, including postoperative gingival height gain (GHG), area gain (GAG), volume gain (GVG) and mean thickness (GMT) of region of interest, as well as the tissue thickness change at 1, 2, and 3 mm (TTC1, TTC2, and TTC3) apical to the cemento-enamel junction. Results Postoperative GHG, GAG, GVG, and GMT were distinctly encountered at 2 weeks post-op, then gradually decreased. At 1 year, GHG, GAG, GVG, and GMT were 2.211 ± 0.717 mm, 7.614 ± 2.511 mm 2 , 7.690 ± 4.335 mm 3 and 0.965 ± 0.372 mm, respectively. Significant decreases were recorded between 6 weeks and 1 year in terms of GHG, GAG, and GVG. The GMT was sustained after 6 weeks with an increase of nearly 1 mm at 1 year. TTC1 and TTC2 yielded thicker tissue change than TTC3. Conclusions Three-dimensional quantitative measurements taken via intraoral scanning showed that buccal augmented tissue acquired via MCAT with DGG tends to be stable after 3 months post-op. Digital measurement can be applied in periodontal plastic surgery as a clinically feasible and non-invasive evaluation method for achieving volumetric outcomes. Digital three-dimensional assessment of free gingival graft remodeling over 12 months

Abstract:Objective: To digitally evaluate the three-dimensional (3D) remodelling of FGG used to treat RT2 gingival recessions and lack of keratinized tissue on mandibular incisor teeth. Methods: Data from 45 patients included in a previous multicentric RCT were analyzed. Silicone impressions were taken before (baseline) and 3, 6 and 12 months after standardized FGG placement. Casts were scanned and images were superimposed, using digital software, to obtain measurements of estimated soft tissue thickness (eTT; 1, 3, and 5 mm apical to baseline gingival margin). In addition, soft tissue volume (STV) and creeping attachment (CA) were assessed. Results: All patients exhibited postoperative eTT and STV increases, at all time points. The greatest mean thickness gain was observed at eTT3 (1.0 ± 0.4 mm) at 12 months. At 12 months, STV was 52.3 ± 21.1 mm3, without relevant changes compared to the 3- and 6-month follow-up. CA, which was observed as early as six months postoperatively, was evident in ∼85 % of teeth at 12 months. Conclusions: Application of FGG was an effective phenotype modification therapy, as shown by the significantly increased tissue thickness postoperatively. Despite the use of FGG technique not aiming for root coverage, digital 3D assessment documented the early and frequent postoperative occurrence of CA, which helped improve recession treatment outcomes. Clinical significance: The use of 3D assessment methodology allows precise identification of the tissue gain obtained with FGG, which, regardless of technique, results in predictable phenotype modification and frequent occurrence of creeping attachment.

Abstract:To assess the predictive value of baseline digitally measured exposure root surface area (ERSA) on the effectiveness of modified coronally advanced tunnel and de-epithelialized gingival grafting (MCAT + DGG) technique for the treatment of multiple adjacent gingival recessions (MAGRs). A total of 96 gingival recessions (48 RT1 and 48 RT2) from 30 subjects were included. ERSA was measured on the digital model obtained by intraoral scanner. Generalized linear model was used to analyze the possible correlation of ERSA, Cairo recession type (RT), gingival biotype, keratinized gingival width (KTW), tooth type, and cervical step-like morphology on the mean root coverage (MRC) and complete root coverage (CRC) at 1-year after MCAT + DGG. The predictive accuracy of CRC is tested using receiver-operator characteristic curves. At 1-year postoperatively, the MRC for RT1 was 95.14 ± 10.25

Abstract:Hyper-redundant manipulators have very large degrees of redundancy, thus possessing unconventional features such as the ability to enter a narrow space while avoiding obstacles. In this study a time-optimal control scheme was proposed for the coupled tendon-driven manipulators, in which a pair of tendons for driving a joint is pulled from base actuators via pulleys mounted on the base-side joints and the degrees of actuation redundancy exist. The time-optimal trajectory planning problem was solved by using the phase-plane analysis and the linear programming technique. Computer simulations were also performed to demonstrate the validity of the proposed scheme.
A novel digital way to measure the vertical and horizontal dimensions of the supra-crestal peri-implant soft tissues

Abstract:Aim: To report on a novel digital superimposition workflow that enables measuring the supra-crestal peri-implant soft tissue dimensions all along implant treatment and afterwards. Materials and methods: A preoperative CBCT and intra-oral scans (IOS) are successively taken before surgery, at the end of the healing period, at prosthesis delivery, and over time; they are digitally superposed on a dedicated software. Then, the stereolithography files (STL) of the healing abutment, of the prosthetic abutment and the crown are successively merged into the superposition set of IOSs. Result: The workflow protocol of merging successively the STL of each item into the superposition set of IOSs enables capturing the dimensions of the height and width of the supra-crestal soft tissues, at every level of the healing abutment, the prosthetic abutment and the crown. In addition, it allows measuring the vertical distance that the crown exerts pressure on the gingiva and the thickness of the papillae at every level of the abutment. Conclusion: This novel digital superimposition workflow provides a straightforward method of measuring the vertical and horizontal dimensions of the supra-crestal peri-implant soft tissues, including the papillae, at each stage of the implant treatment process. It allows investigating a certain number of soft tissue variables that were previously inaccessible to clinical research. It should help enhancing our comprehension of the peri-implant soft tissue dynamics.
Digital Planning to Enhance Diagnosis and Precision in Correcting Excessive Gingival Display in the Presence of Asymmetrical Maxillary Position: A Case Report

Abstract:This case report addresses the treatment of excessive gingival display (EGD) in the context of maxillary asymmetry, zenith irregularities, and occlusal plane inclination. Digital planning was pivotal in formulating a precise treatment strategy by incorporating facial photographs, digital models, and cone-beam computed tomography data. Parameters, including occlusal plane inclination, teeth position, and lip-to-gingival margin relationships, were considered to ensure treatment alignment with the patient's facial characteristics. Notably, during the planning phase, it was evident that the conventional approach using the cementoenamel junction as the apical limit for incisions would result in asymmetry. Consequently, the gingival margin position was defined in accordance with facial and lip features. The chosen treatment, flapless crown lengthening, was tailored to the patient's thin phenotype and guided by measurements derived from digital planning. Postsurgery, the patient experienced a swift and painless recovery. A harmonious smile with a stable gingival margin position was achieved at the 1-year follow-up, seamlessly complementing the patient's facial attributes. This case underscores the importance of personalized EGD treatment and the value of digital planning in enhancing diagnostic accuracy and precise treatment planning, ultimately facilitating optimal treatment strategies.
Comparative evaluation of recession coverage with sub-epithelial connective tissue graft using macrosurgical and microsurgical approaches: A randomized split mouth study

Abstract:Aims: The aim was to compare the recession coverage outcomes when done macrosurgically and microsurgically. Background: Increasing interest in esthetics and the related problems such as hypersensitivity and root caries have favored the development of many root coverage procedures. Recession coverage up to a certain extent has solved these problems, but these procedures need good maintenance after the surgery for long-term benefits. With increasing advances in the field of recession coverage, microscope has added another dimension in undertaking the surgical procedure. Materials and methods: Thirty Miller's Class I and II recession were treated using the sub-epithelial connective tissue graft from the palate. In 15 sites, the graft was placed at the recipient site with unaided eye (Group A) and in other 15 sites the graft was placed using surgical microscope (Group B). Clinical evaluation was done at baseline, 12 weeks and 24 weeks postoperatively using plaque index, gingival index, vertical recession (VR), probing depth, clinical attachment level (CAL), width of attached gingiva, papilla height (PH) and width, malalignment index (MI) and esthetic appearance. Statistical analysis used: Paired and unpaired Student's t-test along with Wilcoxon Z-test were used to analyze the results and probability of P < 0.05 were accepted to reject the null hypothesis. Pearson correlation was used to correlate two parameters such as VR and CAL and MI and VR. Results: Both the techniques demonstrated predictable mean root coverage (Group A 61.78% and Group B 67.58%) at 6 months postsurgery. CAL gain was slightly better in Group B patients when compared to Group A patients. A moderate positive correlation for Group A while a mild correlation in Group B was seen between the MI and VR. Conclusion: The use of the microscope enhances the results, but obtaining an expertise in using needs a lot of practice. The periodontal healing by both techniques should be evaluated histologically.
Acellular dermal matrix seeded with autologous gingival fibroblasts for the treatment of gingival recession: a proof-of-concept study

Abstract:Background: One of the most common esthetic concerns associated with periodontal tissues is gingival recession. There are multiple periodontal plastic surgery approaches documented in the literature for the treatment of such defects. With the tremendous advances being made in periodontal science and technology, tissue engineering could be considered among the latest exciting techniques for recession management. Methods: In this split-mouth, controlled, double-masked clinical case series, 20 sites from 10 patients with Miller Class I or II recessions affecting canines or premolars in the maxillary arch were selected. One tooth in each patient was randomized to receive either a subepithelial connective tissue graft (SCTG) (control group) or an acellular dermal matrix allograft (ADMA) seeded with autologous gingival fibroblasts (test group) under a coronally positioned flap. Clinical parameters, including recession depth, probing depth, clinical attachment level, width of keratinized tissue, attached gingiva, and plaque scores, were recorded by a calibrated examiner at baseline and 3 and 6 months. The inflammation of grafted sites was scored, and the healing time was calculated. The final esthetic outcome of treated sites was assessed by the root coverage esthetic score at the end of 6 months. Results: There were no significant differences between test and control sites for all measured clinical parameters. However, the test sites demonstrated less inflammation in the early postoperative period. Conclusion: Within the limits of this case series, the results indicate that an ADMA seeded with autologous gingival fibroblasts by tissue-engineering technology may be explored as a substitute to an SCTG for the treatment of Miller Class I and II recession defects.

Abstract:Introduction Gingival recession is one of the most common mucogingival deformities requiring surgical correction. The American Academy of Periodontology Regeneration Workshop recommended connective tissue graft (CTG) combined with coronally advanced flap (CAF) for the treatment of Miller Class I and II single-tooth gingival recession. The disadvantages of harvesting autogenous tissue include postoperative bleeding, pain and discomfort at the donor site, restricted tissue supply, increased morbidity and prolonged operative times. Acellular dermal matrix (ADM) contains undamaged collagen and elastin matrices that can be used as a substitute for CTG during root coverage procedures. However, the use of ADM is still controversial. The objective of this split-mouth; randomised, controlled, clinical study is to evaluate the long-term effects of ADM graft (ADMG) combined with CAF on root coverage, aesthetics and patient satisfaction for the treatment of single gingival recession with thin gingival phenotype. Methods and analysis Forty participants with bilateral Miller Class I/II gingival recession will be randomised to receive an ADMG on one side and CTG on the contralateral side, combined with CAF. Gingival recession depth, gingival recession width and keratinised tissue width will be measured at baseline, 2 weeks and 1, 3, 6, 12 and 24 months. Mean root coverage, complete root coverage, root coverage aesthetic score, colour change (∆E) and patient satisfaction will be assessed during follow-up visits. Ethics and dissemination The present study has received approval from the Ethics Committee of Peking University School and Hospital of Stomatology (PKUSSIRB-202054029). Data of this study will be registered with the International Clinical Trials Registry Platform. Additionally, we will disseminate the results through scientific journal. Trial registration number ChiCTR2000033230.

Abstract:In an era of increasing life expectancy and growing patient demands towards lifelong natural tooth retention, accurate assessment of gingival recessions is crucial for diagnosing periodontal diseases, planning preventive or restorative interventions, and evaluating their outcomes. The traditional two‐dimensional (2D) methods, while useful, often fall short in capturing the complex topography of gingival tissue margins and their changes over time. By examining relevant published studies, this review highlights the transition from 2D to 3D techniques, analyzing the limitations of widely used 2D approaches, while emphasizing the potential of novel 3D tools and techniques. It discusses their comparative effectiveness, accuracy, and application challenges in clinical and research settings. Advancements in three‐dimensional (3D) imaging regarding methodologies for the precise evaluation and quantification of free gingival margin changes and gingival recessions are explored and critically evaluated. The review underscores the potential for these technologies to enhance patient outcomes through more precise diagnosis and data generation. It also identifies gaps in current research and suggests directions for future investigation. Abstract:In the article the problem of construction of mobile networks for solving computationally complex problems is researched. It is proposed to use the modified secret sharing scheme (SSS) Asmuth-Bloom for distributed computing in the mobile network. The SSS constructed on the basis of residue number system (RNS) allows to parallelize arithmetic operations of addition and multiplication of numbers, but make it difficult to perform a division operation. The proposed method allows the efficient implementation of arithmetic operations of addition, multiplication and division in the RNS. To evaluate the performance of a mobile computing network the problem of solving systems of linear equations was used. It is shown that the use of the proposed approach makes it possible to parallelize computations and to ensure their safety.

In recent years, enamel matrix derivatives (EMDs) have been used for their regenerative potential in periodontics. The aim of this split-mouth and randomized controlled study was to assess the clinical and aesthetical impacts of EMD application in combination with SCTG+CAF in patients with Miller's Class I and II gingival recessions in contralateral canines of the maxilla. Participants who underwent SCTG+CAF+EMD application were identified as the test group (n = 19) and those who underwent SCTG+CAF as control group (n = 19). The outcome parameters were recession depth/width, root coverage percentage, and root coverage aesthetic score (RES). RES was evaluated by two calibrated blind periodontists one year after the treatment. Statistically significant root coverage percentage was observed at one year post-treatment for both groups (p < 0.05). However, significant differences between the groups were not observed in terms of total RES and complete root coverage rate (p > 0.05). The test group had significantly better results than the control according to the soft tissue texture and mucogingival junction alignment results (p < 0.05). These results indicate that EMDs contribute to the healing of soft tissue without scarring. As a result of better wound healing, the EMD-added group exhibited better results in terms of the harmony of the mucogingival junction between adjacent teeth. This paper is the first split-mouth study in which SCTG+CAF and SCTG+CAF+EMD were compared using RES in bilateral canines.

Abstract:PURPOSE To establish a digital assessment method for changes in gingiva morphology following initial periodontal therapy. METHODS Ten periodontal-healthy participants were selected, and digital models obtained by intraoral scanning and digitizing conventional impressions. Using dentition as a reference, best fit alignment between digital models was carried out. Root mean square (RMS) was calculated to evaluate differences in models, and gingival volume discrepancy (GVD) was calculated after combining separated models. Trueness of intraoral scanning used on the gingiva was evaluated using RMS and GVD between intraoral and conventional models with conventional models as references; precision was evaluated among different intraoral models of one participant. Twenty-three periodontitis-affected participants underwent intraoral scanning immediately after supragingival scaling and two weeks after initial periodontal therapy. The GVD of gingiva between two digital models was calculated to assess gingival changes and related factors after therapy. RESULTS Trueness of intraoral scanning used on the entire gingiva was 83.65 ± 14.43 μm; precision was 70.71 ± 25.58 μm; GVD error measured by digital models was 15.28 ± 10.00 mm³. Gingival volume in periodontitis-affected participants decreased 104.04-1155.09 mm³ after therapy. Probing depth, bleeding index, and keratinized gingival width positively correlated with changes in gingival volume. CONCLUSION Intraoral scanning can be recommended as a method of evaluating morphological changes in gingiva.

Abstract:The type 9 adenylyl cyclase (AC9) is a widely distributed adenylyl cyclase that was originally cloned from a mouse cDNA library. Here we report the cloning, chromosomal mapping, and regulatory properties of human AC9 (HGMW-approved symbol ADCY9). Although the human AC9 sequence shows 86% homology with mouse AC9, divergence at the C2a/C2b junction results in an alternative C2b amino acid sequence. In situ hybridization localized the human AC9 gene to both human and mouse chromosomes 16. AC9 mRNA is present in all tissues examined, with the highest levels found in skeletal muscle, heart, and brain. To characterize the regulatory properties of human AC9 in vivo, the enzyme was expressed in HEK-293 cells. Human AC9 is stimulated by beta-adrenergic receptor activation but is insensitive to forskolin, Ca2+ and somatostatin. In contrast to mouse AC9, the activity of human AC9 is unaffected by inhibitors of calcineurin. These data emphasize the importance of determining the regulatory properties of human adenylyl cyclases.
Crown Accuracy and Time Efficiency of Cement-Retained Implant-Supported Restorations in a Complete Digital Workflow: A Randomized Control Trial

Abstract:Purpose This is a clinical study to compare complete digital workflows generated using intraoral scanning and the split-file technique with a conventional workflow for cement-retained implant-supported restorations. Materials and methods Forty patients requiring posterior single-unit implant restorations were included. Twenty patients were randomly assigned to the complete digital workflow group, involving intraoral scanning and manufacture of cement-retained crowns using the split-file technique (test group). The remaining 20 patients were assigned to the hybrid workflow group (control group), involving conventional impressions and CAD-CAM fabricated crowns based on stone casts. Scanning of the crowns was performed before and after clinical adjustment using an intraoral scanner (TRIOS Color; 3Shape). Two 3D digital models were trimmed and superimposed to evaluate changes in dimensions using Geomagic Control 2014 software. Chair-side and laboratory times for the entire workflow were recorded. Independent-sample t tests were used for the statistical analysis. Results All crowns were inserted without refabrication. The average maximum occlusal adjustment of the crowns, measured as maximum deviation of occlusal area in superimposed pre and post scans, was -212.7 +/- 150.5 and -330.7 +/- 192.5 mu m in the test and control groups, respectively (p = 0.037). The average area of occlusal adjustment, measured as area of deviation larger than 100 mu m, was 8.4 +/- 8.1 and 17.1 +/- 12.3 mm(2) in the test and control groups, respectively (p = 0.012). The mesial and distal contact adjustment amounts, maximum deviations of proximal area, were -33.0 +/- 96.2 and -48.6 +/- 70.5 mu m in the test group, and -3.7 +/- 66.7 mu m and -11.4 +/- 106.7 mu m in the control group, respectively. The mean chair-side time was 20.20 +/- 3.00 and 26.65 +/- 4.53 minutes in the test and control groups, respectively (p < 0.001). The mean laboratory time was 43.70 +/- 5.56 and 84.55 +/- 5.81 minutes in the test and control groups, respectively (p < 0.001). Conclusions Single-unit cement-retained crowns with complete digital workflows required fewer crown adjustments and had shorter clinical and laboratory times compared to conventional impressions and hybrid workflows. Digital impressions and the split-file technique provided customized abutments and cement-retained crowns, thus broadening the indications for digital workflows for implants.
