Abstract:The mechanisms regulating coronary collateral circulation are largely unknown owing to both the complex and variable nature of clinical models and the difficulty to obtain quantitative and differentiated flow measurements within the various coronary tree portions. With the aim of assessing collateral flow reserve, we studied 19 patients with effort angina, without myocardial infarction and with isolated occlusion of either the left anterior descending coronary artery (n = 14) or the circumflex coronary artery (n = 5). Flow values were measured basally, during atrial pacing induced tachycardia and following ev dipyridamole infusion (0.56 mg/kg of body weight in 4 min), by means of positron emission tomography and nitrogen-13 ammonia as flow tracer, within both regions depending on collateral circulation and the remote ones. Results have been compared with those obtained in 13 normal subjects. Basal flow values in regions depending on collateral circulation and in the remote regions (0.61 +/- 0.11 vs 0.63 +/- 0.17 ml/min/g) were found to be similar, but lower than in normal subjects (1.00 +/- 0.20 ml/min/g, p < 0.01). During atrial pacing, flow increased to 0.83 +/- 0.25 and to 1.11 +/- 0.39 ml/min/g, in the regions depending on collateral circulation and in the remote regions, respectively (p < 0.05 as compared to baseline); again, values were lower than in normal subjects (1.86 +/- 0.61 ml/min/g, p < 0.01). Dipyridamole infusion further increased flow in the remote regions (1.36 +/- 0.57 ml/min/g, p < 0.01 as compared to atrial pacing) but it did not in the regions depending on collateral circulation (0.94 +/- 0.37 ml/min/g, NS as compared to atrial pacing); both values were lower than in normal subjects (3.46 +/- 0.78 ml/min/g, p < 0.01). Flow reserve in the regions depending on collateral circulation was found to have a direct linear correlation with the one in the remote regions (r = 0.83; p < 0.01). In conclusion, in spite of basal hypoperfusion, collateral circulation maintains a flow reserve which, even if reduced, is able to cope with moderate increments in oxygen consumption. An analogous flow reduction can be observed in the remote regions, suggesting that the entire coronary tree is involved, beyond the obstructive lesions of the main arterial branches.

Abstract:Aims: To identify the reasons why workers decide to enter, stay or leave the aged care workforce; and the factors influencing them to transition between community and residential sectors in Australia. Background: Factors affecting the recruitment and retention of suitable care workers in aged care are complex and influenced by personal, institutional and societal factors. Methods: A qualitative description study design. Results: In total, 32 staff participated in the study. Five main themes were identified: entering aged care with a passion for the job; entering aged care as it is the only employment option; factors attracting care workers to stay in aged care; factors influencing care workers to leave the job; and preferring to work in residential aged care rather than community aged care. Conclusion: Issues relating to the attraction and retention of aged care workers are influenced by personal, institutional and societal factors. Critical shortages in the aged care workforce make the industry more susceptible to crises such as COVID-19 outbreaks. Implication for nursing practice: Aged care organizations need to create a positive psychosocial work environment for staff to improve the attraction and retention of skilled care workers. They also need to develop staff recruitment guidelines to ensure care workers with the appropriate skills and training and a passion for working with older adults are selected. Staff development programmes need to focus on learning activities that enable staff to build peers support in the work environment. Implications for health policy: There is a need to mandate curriculum to enable nursing students to receive more gerontological education and exposure to aged care throughout their education. Aged care quality standards need to mandate transition support for new graduate nurses.

Abstract:We reconceptualize employee turnover to promote researchers' understanding and prediction of why employees quit or stay in employing institutions. A literature review identifies shortcomings with prevailing turnover dimensions. In response, we expand the conceptual domain of the turnover criterion to include multiple types of turnover (notably, involuntary quits) and multiple types of staying. Guided by the premise that "everyone eventually leaves; no one stays with an organization forever," we also suggest considering where leavers end up-or post-exit destinations, such as another job, full-time parenting, or educational pursuits. We propose "proximal withdrawal states" that motivate members to participate or withdraw from organizations as an expanded criterion. These motivational states precede turnover and are derived from 2 overarching dimensions: desired employment status (whether employees want to stay or leave) and perceived volitional control (whether quit or stay decisions are completely up to them or at least partially under external regulation). Crossing these dimensions yields 4 prime states: enthusiastic leavers and stayers and reluctant leavers and stayers. We further subdivide these mind-sets into subtypes by differentiating employer from other forms of external control (e.g., family). Focusing on more common subtypes, we explain how they arise from particular motivational forces and profile how they differ by attitudes, behaviors, and turnover speed and destinations. We further discuss ways to measure this expanded criterion and proximal states (and subtypes) and investigate the latter's profiled differences. Finally, we discuss scientific and practical implications and future research directions.

Abstract:In this study we establish the fact that the degree of satisfaction of certain personal needs supplied by a person's place of employment has a significant direct relationship to his continuing to work for that company. These personal needs are for recognition, for autonomy, for a feeling of doing work that is important, and for evaluation by fair standards. In addition, knowing important people in the organization is related to continued employment. There are some indications that anxiety develops in those employees who state that their needs for autonomy and fair evaluation are not satisfied. In addition to the degree of need satisfaction provided by the job, we examined the degree to which the employment situation limits satisfactions which the worker can receive from his family and from his community. We found that the extent to which the job interferes with family and community satisfactions is related to turnover as strongly as the failure to receive need satisfactions on the job. But interference with off‐the‐job sources of satisfaction is not related to experiencing dissatisfaction on the job. We interpret these results to mean that there are two essentially different kinds of reasons for leaving the employing organization. Some people resign for reasons of both kinds: the job itself does not satisfy needs, and it also keeps them from receiving satisfactions from other sources. Most of the people in this study who resigned, how‐ ever, felt only one or the other of these two forms of dissatisfaction. (The interference with off ‐the‐job satisfactions is probably higher among the people studied than in the popula‐ tion at large since shift work is prevalent in the industry in which this study was conducted.)

Turnover, COVID-19, and Reasons for Leaving and Staying Within Governmental Public Health

Abstract:Background and objectives: Public health workforce recruitment and retention continue to challenge public health agencies. This study aims to describe the trends in intention to leave and retire and analyze factors associated with intentions to leave and intentions to stay. Design: Using national-level data from the 2017 and 2021 Public Health Workforce Interests and Needs Surveys, bivariate analyses of intent to leave were conducted using a Rao-Scott adjusted chi-square and multivariate analysis using logistic regression models. Results: In 2021, 20% of employees planned to retire and 30% were considering leaving. In contrast, 23% of employees planned to retire and 28% considered leaving in 2017. The factors associated with intentions to leave included job dissatisfaction, with adjusted odds ratio (AOR) of 3.8 (95% CI, 3.52-4.22) for individuals who were very dissatisfied or dissatisfied. Odds of intending to leave were significantly high for employees with pay dissatisfaction (AOR = 1.83; 95% CI, 1.59-2.11), those younger than 36 years (AOR = 1.58; 95% CI, 1.44-1.73) or 65+ years of age (AOR = 2.80; 95% CI, 2.36-3.33), those with a graduate degree (AOR = 1.14; 95% CI, 1.03-1.26), those hired for COVID-19 response (AOR = 1.74; 95% CI, 1.49-2.03), and for the BIPOC (Black, Indigenous, and people of color) (vs White) staff (AOR = 1.07; 95% CI, 1.01-1.15). The leading reasons for employees' intention to stay included benefits such as retirement, job stability, flexibility (eg, flex hours/telework), and satisfaction with one's supervisor. Conclusions: Given the cost of employee recruitment, training, and retention of competent employees, government public health agencies need to address factors such as job satisfaction, job skill development, and other predictors of employee retention and turnover. Implications: Public health agencies may consider activities for improving retention by prioritizing improvements in the work environment, job and pay satisfaction, and understanding the needs of subgroups of employees such as those in younger and older age groups, those with cultural differences, and those with skills that are highly sought-after by other industries.

Abstract:Fundamental differences between assisted living facilities and nursing homes may prohibit the models of satisfaction and turnover developed in nursing home research from generalizing to assisted living facilities. The assisted living industry is growing rapidly, and many groups have voiced concerns about the staff turnover rate. As part of a training and education program, the Wisconsin Alzheimer's Institute collected data from the employees of assisted living facilities via self-administered questionnaires. Chi-square analyses were performed to determine if factors previously identified by nursing home literature had a significant relationship to job satisfaction. All of the factors examined showed a relationship to the job satisfaction item. In addition, employees who were not satisfied with their jobs were over 14 times more likely to report intentions to leave. These findings are used to suggest intervention strategies to reduce turnover.
To Stay or to Leave: Factors Shaping Early Childhood Teachers’ Turnover and Retention Decisions

Abstract:This qualitative study sought to understand whether there were difference in how early childhood education (ECE) teachers who stayed in or left their jobs interpreted their job demands and resources. It also sought to understand factors that shaped teachers' turnover and retention decisions. Twenty-six teachers who worked in subsidized ECE programs that blended multiple public funding sources were interviewed, 14 of whom stayed in their jobs and 12 of whom left their jobs. We found teachers' reasons for leaving or staying were complex and took into consideration workplace and family factors. Teachers who stayed tended to share the same job frustrations as those who left, namely a misalignment between job demands and resources, but those who stayed were more willing to engage creatively in solving workplace problems. Teachers who stayed placed greater value on professional development opportunities than teachers who left, and these key job rewards factored into retention decisions. Teachers who left often felt that lacking key job resources undermined their sense of competence at being an effective teacher and viewed the psychosocial workplace climate less favorably than teachers who stayed. For many teachers, the value they placed on different job rewards was influenced by family factors.

Abstract:This study examines factors associated with the intentions of nurses to leave their present employment in a long-term care (LTC) setting. The study examines differences in the personal characteristics, job-specific characteristics, job satisfaction, and perceived job mobility of two groups of nurses currently employed in LTC facilities; those who report that they intend to stay at the nursing home where they are employed (n = 147) and those who intend to leave (n = 45). Nearly half of the factors that were examined (9 of 20) were statistically significantly associated with nurses' self-reports of their intention to leave. Of the significant associations, seven were job-specific characteristics or dimensions of job satisfaction. The implications of these findings are that a significant majority of the factors that influence a nurse's intention to leave may be within the realm of control of nurse executives and nursing home administrators. Results are discussed within the context of three general areas where there may be opportunities for managers to take actions that will improve retention.

Does Coworkers’ Upward Mobility Affect Employees’ Turnover Intention? the Roles of Perceived Employability and Prior Job Similarity

Abstract:What would employees do if their coworkers move upwardly to other organizations? In the era of boundaryless career, voluntary turnover is more prevalent than ever before. Effects of coworkers' voluntary turnover on employees' turnover are attracting attention in human resource management. However, this stream of research has generated mixed findings showing both positive and negative effects on focal employees' turnover, and scholars still know little about the specific mechanism through which the effects occur. This study takes the first step to reconcile the mixed findings and unfold the black box of the effects. Specifically, we argue that the mobility direction of coworkers' turnover should not be overlooked by previous research, and we propose that coworkers' upward mobility plays an indispensable role in predicting employees' turnover intention. Based on social comparison theory, we hypothesize that coworkers' upward mobility positively relates to focal employees' turnover intention through focal employees' perception of employability. Furthermore, we propose prior job similarity as a contingency that activates the relationship mentioned above, such that the mediation effect is stronger with higher prior job similarity. Our hypotheses were tested and fully supported by two-wave data from a sample of 369 employees in China. Lastly, theoretical and practical implications are discussed along with possible limitations and directions for future turnover research.
Overqualified Employees' Actual Turnover: The Role of Growth Dissatisfaction and the Contextual Effects of Age and Pay

Abstract:Overqualified employees are those with more knowledge, skills, and abilities than their job requires. While these competent workers can bring benefits to firms, they also pose a high risk of turnover. However, the evidence is insufficient on whether, why, and when they actually leave and how organizations can manage their turnover effectively. Based on self-determination theory, we propose that unfulfilled internal aspirations for personal growth can be a major driving force of turnover among overqualified employees. Moreover, we explore how such a mechanism varies by employee's age and whether pay can exert a mitigating influence. Using longitudinal data over three years (from 2017 to 2019) with a large sample of workers ( N = 2473), we found an indirect effect where perceived overqualification reduces growth satisfaction, which in turn leads to actual turnover. Further, we found a three-way moderating effect of pay and age on the relationship between growth satisfaction and turnover, indicating that pay's effect of deterring turnover was stronger for older employees. This study has theoretical implications for a better understanding of overqualified employees' turnover driven by their internal impetus, as well as implications for pay policies that effectively manage overqualified workers in age-diverse modern organizations.

Abstract:Abstract Background Although the phenomenon of high turnover rate in the elderly care industry has existed for a long time, there are few studies that have constructed frameworks to comprehensively analyze the strength of the effects of various factors on the turnover intention of elderly care workers.. This study analyzed the impact of different types of perceived human resource management practices on elderly care workers’ turnover intentions and whether this relationship is moderated by person-job fit and ownership of elderly care institutions. Methods This is a cross-sectional and regional survey study. The study included questionnaire survey data from a total of 305 elderly care workers from 42 elderly care institutions in 21 provinces in China during June to September 2021. Descriptive statistics, Pearson correlation coefficient, multiple regression, and heterogeneity analyses were performed. Results Perceived work environment ( $$\beta$$ β =-0.5164, p < 0.01), perceived occupational protection ( $$\beta$$ β =-0.3390, p < 0.01), perceived welfare benefits ( $$\beta$$ β = -0.2620, p < 0.01) and perceived competency training ( $$\beta$$ β = -0.1421, p < 0.1) were all significantly and negatively related to turnover intentions, the quality of perceived work environment has the greatest impact on elderly care workers’ turnover intentions. Under the moderating effects of person-job fit and ownership of elderly care institutions, there existed heterogeneity between perceived human resource management and turnover intentions among elderly care workers. High level of person-job fit and elderly care institutions’ public feature can effectively weaken the negative impact of each type of perceived human resource management on turnover intention among elderly care workers. Conclusions The managers of elderly care institutions should optimize the management mechanism, typically pay attention to elderly care workers’ working environment, formulate and improve the professional standards and job requirements for elderly care workers, promote the public welfare value of nursing care services, and strengthen the sense of honor and responsibility of elderly care workers to reduce the turnover rate.

Abstract:Gibberellic acid (GA(3)) sensitivity (measured as alpha-amylase production) of the isolated aleurone tissue/deembryonated seed of two wheat (Triticum aestivum L. var Kite and var Aroona) varieties each containing either one of the dwarfing genes, Rht1 and Rht2, was increased significantly as a result of low temperature treatment. The magnitude of the low temperature-induced increase occurred without any change in the lag time of alpha-amylase production. This low temperature induction of GA(3) sensitivity was found to be operative in aleurone tissue of only those varieties having at least one of the three Rht alleles. It is likely, therefore, that the low temperature treatment effect which ;cures' or circumvents the genetic lesions manifest in the Rht1 and Rht2 genotypes is the same as that effective in the Rht3-containing genotype and probably involves an increase in hormone (GA(3)) receptor sites. Furthermore, this increase appears to be a quantitative temporal one.
"I'm too old for this!": A prospective, multilevel study of job characteristics, age, and turnover intention

Abstract:Introduction: Deciding to leave a job is often foreshadowed by burgeoning job dissatisfaction, which is in turn often attributed to characteristics of the job and work environment. However, while we know that job characteristics influence job satisfaction, health, and motivation, their associations with turnover intention is less clear. Moreover, despite aging workforces, an understanding of how working conditions influence workers across the lifespan is lacking. Therefore, drawing on job design theories and bridging turnover- and aging research, we studied 15 specific job characteristics to determine whether they predicted turnover intentions, and whether the predictive value was modified by age. Methods: Data were collected from various public and private enterprises in Norway. Moderated multilevel regressions were conducted cross-sectionally (N = 12,485) and prospectively over 2 years (N = 5,504). Results: Most work factors were associated with turnover intention at both the individual and work unit levels. A social climate of support, trust, and encouragement was most strongly inversely associated with turnover intentions, while role conflict was most strongly positively associated with turnover intentions. Organizational climate, leadership styles, and job control were more important with age while job demands, predictability and role stressors were more important to younger workers. Ten individual level- and four work-unit level factors predicted turnover intentions prospectively, suggesting turnover intentions due to poor working conditions persisted in employees that did not quit. Discussion: Our results highlight several specific, modifiable job characteristics that are likely to affect turnover intentions, and the impact of certain factors specifically for older workers.

Abstract:Background High nurse turnover in hospitals is a worldwide problem with dire consequences for patient care such as increased mortality and a decrease in patient safety. A specific effort to retain nurses is urgently needed due to the many vacant nursing positions, especially across the Medical hospital departments. Aim To identify the principal deteriorating factors experienced by nurses relating to their intentions to leave the medical department at a university hospital. Methods A rapid qualitative research methodology was used. Participants comprised 22 registered nurses employed in five university hospital medical department units. The Rigorous and Accelerated Data Reduction (RADaR) technique was therefore used for data collection through qualitative semi‐structured interviews and analysis. COREQ was used for reporting the study. Results The RADaR analysis detected a downward spiral of five principal deteriorating factors influencing nurses' intentions to leave their position in the medical department. The factors were resignations from several nursing colleagues combined with too few and inexperienced nurses present during shifts, additional tasks assigned, a management refraining from improving the problematic issues, leading to decisive consequences for patient care and a declining feeling of professional care. Conclusions The lack of nurses in the department caused missed nursing care, which affected the nurses' job satisfaction and intentions to leave their positions. Future research initiatives must focus on evaluating successful interventions to maintain the nurses in the positions. Further knowledge, is also needed, to investigate how we can change the downward spiral to a story of retention success.

Abstract:Scholars have explored the ways that conventional economic theory does not fully explain the distribution and characteristics of caring labor – the work, unpaid and paid, of caring for those who are young, elderly, or disabled. This paper explores a critical dimension of paid care – high turnover rates in the lowest-wage segment of the sector (including childcare, nursing homes, home health). Using longitudinal data from the 2008 Survey of Income and Program Participation (SIPP) in the US, it examines the "flip side" of that turnover by comparing occupational mobility among low-wage care workers to that of other low-wage service workers. The findings indicate that patterns of occupational transition among care workers are distinct in important ways. Understanding occupational attachment among paid care workers is critical to developing theoretical models about care and to creating care-specific policies to address employee turnover and its negative impact on care quality.<b>HIGHLIGHTS</b> High turnover in low-wage jobs in care-related fields has a negative impact on the quality of care.Low-wage care workers have longer job tenures and are more likely to stay in their field than other low-wage workers.Low-wage care workers experience more upward mobility than food service and cleaning workers, but less than office and sales workers.Low-wage care workers have high rates of transition to a period of not working.There is evidence of higher levels of occupational attachment among low-wage care workers than among other low-wage service workers.

Abstract:Turnover and retention of care workers in long-term care (LTC) settings is an important issue. However, much research about turnover in LTC settings has focused on licensed nurses or nurse assistants. Moreover, many studies have utilized quantitative methods. The purpose of this study was to understand the characteristics of elderly care work that influence the turnover intentions of care workers in LTC. In-depth interviews were conducted with 10 care workers and analyzed using the content analysis method. As a result, seven categories were extracted as the characteristics of the elderly care work associated with turnover, including low social appreciation about care work, precarious employment, unprotected labor rights and safety, an unfair wage system, unclear scope and role of work, absence of training and supervision to enhance professionalism, and emotional labor. For the turnover prevention and retention of care workers, it is necessary to resolve the insecurity of care work. In particular, guidelines for improving the wage level and working conditions of care workers should be instituted, and at the same time, government supervision is required. Education is necessary to strengthen the professionalism of care workers and ensure skilled care work.

Abstract:Tetradental cis-enaminoketone Ni (II) complexes with different molecular shape have been synthesized. Intramolecular hydrogen bonds, which stiffen the mesogenic core and restrict rotation of some molecular parts, have been introduced in these compounds. In the case of molecules with two hydrogen bonds and alkoxy terminal chains filling the inner molecular space, the uncommon phase sequence Iso-D(h)-Iso(re)-SmA (series III-3) was detected. For the first time, it was observed that the isotropic re-entrant (Iso(re)) phase (short-range order) is separating the columnar (D) (high-temperature) and the lamellar (SmA) (low-temperature) phases, both revealing long-range ordered structures.
Health Care Staff Turnover and Quality of Care at Nursing Homes

Abstract:Importance: Turnover in health care staff may disrupt patient care and create operational and organizational challenges, and nursing home staff turnover rates are particularly high. Empirical evidence on the association between turnover and quality of care is limited and has typically relied on low-quality measures of turnover, small and selected samples of facilities, and comparisons across facilities that are highly susceptible to residual confounding. Objective: To quantify the association between nursing home staff turnover and quality of care using within-facility variation over time in reliable turnover measures available for virtually all US nursing homes. Design, setting, and participants: In this cross-sectional study, data from the Centers for Medicare & Medicaid Services on health inspection citations and quality measures at US nursing homes were combined with turnover measures constructed from daily staffing payroll data for quarter 2 of 2017 (April 1 to June 30) to quarter 4 of 2019 (October 1 to December 31), covering 1.06 billion shifts for 7.48 million employment relationships at 15 869 facilities. A 2-way fixed-effects design was used to estimate the association between staff turnover (direct care nursing staff and administrators) and quality-of-care outcomes based on how the same facility performed differently in times of low and high turnover. Data analysis was performed from September 2022 to August 2023. Exposures: Facility turnover, defined as the share of hours worked in a period by staff hired within the last 90 days. Main outcomes and measures: Number, type, scope, and severity of health inspection citations, overall health inspection scores, and Nursing Home Compare quality measures. Results: The study sample included 1.45 million facility-weeks between April 1, 2017, and December 31, 2019, corresponding to 13 826 unique facilities. During an average facility-week, 15.0% of nursing staff and 11.6% of administrators were new hires due to recent turnover. After both administrator turnover and the overall staffing level were controlled for, an additional 10 percentage points in nursing staff turnover in the 2 weeks before a health inspection was associated with an additional 0.241 (95% CI, 0.084-0.399) citations in that inspection, compared with a mean of 5.98 citations. An additional 10 percentage points in nursing staff turnover was associated with a mean decrease of 0.035 (95% CI, 0.023-0.047) SDs in assessment-based quality measures and 0.020 (95% CI, 0.001-0.038) SDs in claims-based quality measures, with the strongest associations found for measures related to patient functioning. Conclusions and relevance: Within-facility variation in staff turnover was associated with decreased quality of care. These findings suggest that efforts to monitor and reduce staff turnover may be able to improve patient outcomes.

Methods: A case study of assistant and registered nurses' intentions to leave as well as staff turnover at a smaller Swedish public hospital was performed. Employee surveys and interviews with assistant and registered nurses who had quit their job at four units in the hospital during the period 2012– 2019 were performed. Critical factors regarding nurses' intention to leave and staff turnover are analyzed by combining narrative methods with a critical incidence technique. Results: Three main themes emerge from the analysis of factors contributing to the decision to quit, namely lack or loss of buffering factors, not owning your spare time and not feeling valued by and listened to by upper management . Conclusion: Decision-makers, including hospital management, need to consider how supportive factors in nurses' closest work environment that promote staff retention may be impacted by decisions at higher levels, such as health-care reorganizations and stricter governance. In this context, upper management adopting a servant leadership approach might contribute to employees to a greater extent feeling valued and being listened to. Finally, the results indicate that individual nurses' recovery, ability to work and ability to coordinate their personal life with their work life need to be supported by policies and decisions at higher levels in order to retain nurses in intensive and emergency health-care settings. Keywords: case study, health care, intention to leave, system theory Nursing shortages and staff turnover have been identified as one of the greatest threats regarding both employee health and patient safety in the health-care sector worldwide. 1,2 Recent studies imply that nurses turnover intention has increased significantly during the COVID-19 pandemic 3,4 with workplace stress as one of the main contributing predictors of turnover decisions. 3 This is alarming since statistics in Sweden indicate an existing shortage of nurses with both basic and specialized training. 5 However, nurses who quit a specific health-care workplace do not necessarily leave the nursing profession. A study shows that about 90% of registered nurses (RNs) in Sweden remain in the profession. The same study, however, also indicates that an increased proportion of RNs leave workplaces in Swedish regions, including hospitals, in order to work for other health-care providers. 6 A Swedish study shows that 50% of those quitting a hospital unit move on to another hospital unit, 40% move on to an external employer and 10% retire. 7 Some Swedish regions do not report the same difficulties in terms of recruiting assistant nurses, but the shortage of assistant nurses is still expected to accelerate over the next 10 years, which means that Sweden can expect a general shortage of assistant nurses in the coming years. 5 Numerous previous survey studies have identified factors correlated to staff turnover and the intention of nurses to leave. 2,8–10 However, there is also research indicating that intentions to leave and staff turnover do not always necessarily depend on the occurrence of single factors at a specific point in time, but could in many cases rather be seen as a result of individual and organizational factors interacting over a longer period of time. 2,11 There is, however, a shortage of studies on staff turnover based on this kind of system perspective. 12,13 This study adopts a narrative approach to achieve a deeper understanding of critical factors at different levels of health-care systems that over time contribute to intentions to leave and, consequently, actual staff turnover. These results are important for identifying relevant measures at different levels in a health-care system to prevent staff turnover.
