Abstract:Background: Multiple factors can fuel nurses' intention to leave their employing hospital or their profession. Job dissatisfaction and burnout are contributors to this decision. Sociodemographic and work context factors can also play a role in explaining nurses' intention to leave. Objective: To investigate the role of sociodemographic and work context factors, including job resources, job demands, job dissatisfaction, depersonalization, and emotional exhaustion, on nurses' intention to leave their hospital or their profession. Design: Multicentre cross-sectional study. Settings: Eight European hospitals, two per each country, including Belgium, the Netherlands, Italy, and Poland. Participants: From May 16 to September 30, 2022, we collected 1,350 complete responses from nurses working at the selected hospitals (13 % response rate). Methods: The intention to leave was assessed through two 5-Likert scale outcomes, agreeing with the intention to leave the profession and the intention to leave the hospital. Logistic regression models were used for statistical analysis. Results: At the multivariable analysis, a higher intention to leave the hospital was observed for: younger age, having served on the frontline against COVID-19, lack of quipment, living in the Netherlands, emotional exhaustion, dissatisfaction with work prospects, and dissatisfaction with the use of professional abilities. There was a higher intention to leave the profession for: younger age, living in the Netherlands, having work-related health problems, depersonalization, emotional exhaustion, low possibilities of professional development, dissatisfaction with work prospects, lack of use of professional abilities, overall ob issatisfaction, and dissatisfaction with salary. Nurses living in Italy expressed the lowest intention to leave. Conclusion: While confirming the role of job dissatisfaction and burnout, we found higher intention to leave for young nurses, nurses with work-related health problems, and caregivers during the COVID-19 pandemic. Dissatisfaction with work prospects, professional development, and salary also increased the intention to leave. We call for educators, managers, and policymakers to address these factors to retain at-risk nursing categories, implementing strategies to mitigate intentions to leave.
The impact of social work environment, teamwork characteristics, burnout, and personal factors upon intent to leave among European nurses

Abstract:Objectives: Europe's nursing shortage calls for more effective ways to recruit and retain nurses. This contribution aims to clarify whether and how social work environment, teamwork characteristics, burnout, and personal factors are associated with nurses' intent to leave (ITL). Methods: Our sample comprises 28,561 hospital-based nurses from 10 European countries. Different occupational levels have been taken into account: qualified registered nurses (n = 18,594), specialized nurses (n = 3957), head nurses (n = 3256), and nursing aides and ancillary staff (n = 2754). Results: Our outcomes indicate that ITL is quite prevalent across Europe, although we have found some differences across the countries depending on working conditions and economic situation. Quality of teamwork, interpersonal relationships, career development possibilities, uncertainty regarding treatment, and influence at work are associated with nurses' decision to leave the profession across Europe, notwithstanding some country-specific outcomes. A serious lack of quality of teamwork seems to be associated with a 5-fold risk of ITL in 7 countries. As far as personal factors are concerned, our data support the hypothesized importance of work-family conflicts, satisfaction with pay, and burnout. A high burnout score seems to be associated with 3 times the risk of ITL in 5 countries. Conclusions: To prevent premature leaving, it is important to expand nurses' expertise, to improve working processes through collaboration and multidisciplinary teamwork, and to develop team training approaches and ward design facilitating teamwork.
Intention to leave, depersonalisation and job satisfaction in physicians and nurses: a cross-sectional study in Europe

Abstract:The European healthcare sector faces a significant shortage of healthcare workers. Assessing the prevalence of this issue and understanding its direct and indirect determinants are essential for formulating effective recruitment programs and enhancing job retention strategies for physicians and nurses. A multicentric cross-sectional study was conducted, involving 381 physicians and 1351 nurses recruited from eight European hospitals in Belgium, the Netherlands, Italy, and Poland. The study focused on assessing turnover intentions among healthcare workers based on the Job Demands-Resources model, using an online questionnaire. Structural equation models were employed to test the data collection questionnaires' construct validity and internal consistency. The turnover intention was assessed by agreement with the intention to leave either the hospital or the profession. Among physicians, 17% expressed an intention to leave the hospital, while 9% intended to leave the profession. For nurses, the figures were 8.9% and 13.6%, respectively. The internal consistency of the questionnaires exceeded 0.90 for both categories of health workers. Depersonalization and job dissatisfaction were identified as direct determinants of turnover intention, with work engagement being particularly relevant for nurses. We found a higher intention to leave the hospital among physicians, while nurses were more prone to leave their profession. To mitigate turnover intentions, it is recommended to focus on improving job satisfaction, work engagement and fostering a positive working climate, thereby addressing depersonalisation and promoting job retention.

Methods: A case study of assistant and registered nurses' intentions to leave as well as staff turnover at a smaller Swedish public hospital was performed. Employee surveys and interviews with assistant and registered nurses who had quit their job at four units in the hospital during the period 2012– 2019 were performed. Critical factors regarding nurses' intention to leave and staff turnover are analyzed by combining narrative methods with a critical incidence technique. Results: Three main themes emerge from the analysis of factors contributing to the decision to quit, namely lack or loss of buffering factors, not owning your spare time and not feeling valued by and listened to by upper management . Conclusion: Decision-makers, including hospital management, need to consider how supportive factors in nurses' closest work environment that promote staff retention may be impacted by decisions at higher levels, such as health-care reorganizations and stricter governance. In this context, upper management adopting a servant leadership approach might contribute to employees to a greater extent feeling valued and being listened to. Finally, the results indicate that individual nurses' recovery, ability to work and ability to coordinate their personal life with their work life need to be supported by policies and decisions at higher levels in order to retain nurses in intensive and emergency health-care settings. Keywords: case study, health care, intention to leave, system theory Nursing shortages and staff turnover have been identified as one of the greatest threats regarding both employee health and patient safety in the health-care sector worldwide. 1,2 Recent studies imply that nurses turnover intention has increased significantly during the COVID-19 pandemic 3,4 with workplace stress as one of the main contributing predictors of turnover decisions. 3 This is alarming since statistics in Sweden indicate an existing shortage of nurses with both basic and specialized training. 5 However, nurses who quit a specific health-care workplace do not necessarily leave the nursing profession. A study shows that about 90% of registered nurses (RNs) in Sweden remain in the profession. The same study, however, also indicates that an increased proportion of RNs leave workplaces in Swedish regions, including hospitals, in order to work for other health-care providers. 6 A Swedish study shows that 50% of those quitting a hospital unit move on to another hospital unit, 40% move on to an external employer and 10% retire. 7 Some Swedish regions do not report the same difficulties in terms of recruiting assistant nurses, but the shortage of assistant nurses is still expected to accelerate over the next 10 years, which means that Sweden can expect a general shortage of assistant nurses in the coming years. 5 Numerous previous survey studies have identified factors correlated to staff turnover and the intention of nurses to leave. 2,8–10 However, there is also research indicating that intentions to leave and staff turnover do not always necessarily depend on the occurrence of single factors at a specific point in time, but could in many cases rather be seen as a result of individual and organizational factors interacting over a longer period of time. 2,11 There is, however, a shortage of studies on staff turnover based on this kind of system perspective. 12,13 This study adopts a narrative approach to achieve a deeper understanding of critical factors at different levels of health-care systems that over time contribute to intentions to leave and, consequently, actual staff turnover. Factors Influencing Intentions of Nursing Personnel to Leave Employment in Long-Term Care Settings

Abstract:This study examines factors associated with the intentions of nurses to leave their present employment in a long-term care (LTC) setting. The study examines differences in the personal characteristics, job-specific characteristics, job satisfaction, and perceived job mobility of two groups of nurses currently employed in LTC facilities; those who report that they intend to stay at the nursing home where they are employed (n = 147) and those who intend to leave (n = 45). Nearly half of the factors that were examined (9 of 20) were statistically significantly associated with nurses' self-reports of their intention to leave. Of the significant associations, seven were job-specific characteristics or dimensions of job satisfaction. The implications of these findings are that a significant majority of the factors that influence a nurse's intention to leave may be within the realm of control of nurse executives and nursing home administrators. Results are discussed within the context of three general areas where there may be opportunities for managers to take actions that will improve retention.

Abstract:Background: The organization of French medical institutions is changing, and the nursing personnel can encounter difficulties of adaption to these changes. The results of 2 questionnaires of the PRESST/NEXT study relative to working conditions (respectively, the questionnaires "Q0" and "Q12"), sent in a year interval to a sample of nurses were compared. Methods: Three samples were made up: a sample of 5376 nurses having answered the initial questionnaire (Q0); a sample of 2627 nurses having answered the questionnaire sent one year after (Q12), and working in the same establishment; a group of 208 nurses having answered the questionnaire "Qex", specific to nurses who left their establishment. Results: Among nurses who answered the "Qex", approximately 1/5 of nurses chose to leave the profession definitively or temporarily before the retirement age; of these, almost 13% began a new work not in the healthcare sector. Seven factors are significantly related to the intention of leaving the profession: Dissatisfaction regarding professional prospects (Q0: OR = 3.1 and Q12: OR = 2.82); Difficulties assuming the family responsabilities (Q0: OR = 1.36 and Q12: OR = 2.10); Mental health disorders for nurses not followed medically (Q0: OR = 1.76 and Q12: OR = 1.47), and for nurses receiving care for this (Q0 : OR = 1.90 and Q12: OR = 1.77); Dissatisfaction concerning the psychological support available at work (Q0: OR = 1.57 and Q12: OR = 1.87); Tiredness (Q0 : OR = 1.40 and Q12: OR = 1.6); Musculo-skeletal disorders diagnosed by doctor (Q0: OR = 1.32 and Q12: OR = 1.61); Dissatisfaction concerning the use of competences (Q0: OR = 1.27 and Q12: OR = 1.97). Conclusion: These results raise questions as to the benefits of new management which promotes flexibility in work and schedules, and which reduces the possibilities of support within the nursing teams. This type of management implies uncertainty for nurses as far as the technical aspects of their work are concerned, and a lack of time for the education and follow-up of patients.

Abstract:Aims: A shortage of nurses happens not only in developed countries, but also in developing countries, such as in China, but the nurse turnover here makes the situation worse. Why do Chinese nurses want to leave the nursing profession? Our hypothesis is that unfavourable psychosocial work environment could predict nurses' intention to leave (ITL). Methods: Collaborating with the EU NEXT study (Nurses' Early eXit sTudy), the longitudinal study was conducted in China, and the psychosocial work environment was measured with the Copenhagen Psychosocial Questionnaire (COPSOQ). A total of 3,088 registered female nurses working in hospitals were eligible for the baseline analyses by multivariate logistic regression, and 1,521 for the one-year follow-up analyses by multivariate Poisson regression. Results: A wide range of psychosocial factors at work - in particular, increased emotional demands, decreased meaning of work, decreased commitment to the workplace, and decreased job satisfaction - were associated with ITL in both baseline analyses and prospective analyses after adjusting for numerous confounders. Conclusions: The findings suggest that unfavourable psychosocial work environment predicts ITL in Chinese nurses. Improvements in the psychosocial work environment may be helpful in retention of the nursing workforce.

Abstract:Little information is available regarding donor-specific parameters that predict success or failure after heart transplantation. Furthermore, with increasing numbers of patients awaiting heart transplantation, there is tremendous pressure to expand the donor pool by stretching the margins of donor acceptability. To gain insight into donor-related and donor-recipient interrelated predictors of death after transplantation, 1719 consecutive primary transplantations performed at 27 institutions between Jan. 1, 1990, and June 30, 1992, were analyzed. Mean follow-up of survivors was 13.9 months, and actuarial survival was 85% at 1 year. By multivariable analysis, risk factors for death included younger recipient age (p = 0.006), older recipient age (p = 0.0005), ventilator support at time of transplantation (p = 0.0006), higher pulmonary vascular resistance (p = 0.02), older donor age (p < 0.0001), smaller donor body surface area (female donor heart placed into larger male patient) (p = 0.003), greater donor inotropic support (p = 0.01), donor diabetes mellitus (p = 0.01), longer ischemic time (p = 0.0003), diffuse donor heart wall motion abnormalities by echocardiography (p = 0.06), and, for pediatric donors, death from causes other than closed head trauma (p = 0.02). The overall 30-day mortality rate was 7% but increased to 11% when donor age exceeded 50 years and was 12% when inotropic support exceeded 20 micrograms/kg/min dopamine plus dobutamine and 22% with diffuse echocardiographic wall motion abnormalities. The interaction of donor risk factors was such that the heart of a smaller female donor given high-dose inotropes placed into a larger male recipient produced a predicted 30-day mortality rate of 26% and the heart of a 25-year-old male donor given high-dose inotropes with diffuse echocardiographic wall motion abnormalities transplanted into a 50-year-old male recipient led to a predicted 30-day mortality rate of 17%. This analysis supports cautious extension of criteria for donor acceptance but with an anticipated greater risk in the presence of diffuse echocardiographic wall motion abnormalities and long anticipated ischemic time, particularly in older donors given inotropic support.

Abstract:Background High nurse turnover in hospitals is a worldwide problem with dire consequences for patient care such as increased mortality and a decrease in patient safety. A specific effort to retain nurses is urgently needed due to the many vacant nursing positions, especially across the Medical hospital departments. Aim To identify the principal deteriorating factors experienced by nurses relating to their intentions to leave the medical department at a university hospital. Methods A rapid qualitative research methodology was used. Participants comprised 22 registered nurses employed in five university hospital medical department units. The Rigorous and Accelerated Data Reduction (RADaR) technique was therefore used for data collection through qualitative semi‐structured interviews and analysis. COREQ was used for reporting the study. Results The RADaR analysis detected a downward spiral of five principal deteriorating factors influencing nurses' intentions to leave their position in the medical department. The factors were resignations from several nursing colleagues combined with too few and inexperienced nurses present during shifts, additional tasks assigned, a management refraining from improving the problematic issues, leading to decisive consequences for patient care and a declining feeling of professional care. Conclusions The lack of nurses in the department caused missed nursing care, which affected the nurses' job satisfaction and intentions to leave their positions. Future research initiatives must focus on evaluating successful interventions to maintain the nurses in the positions. Further knowledge, is also needed, to investigate how we can change the downward spiral to a story of retention success.

Ultrastructural and histochemical observations on secretory phenomena in the resting human mammary gland.

Abstract:Ultrastructure and histochemistry of clinically normal appearing tissue and secretions of non-lactating human mammary glands have been investigated in order to document and analyse secretory phenomena in the resting gland. The material studied originated from women of different ages (18-74 years) who underwent plastic surgery or surgery for various disorders of the breast. The epithelia of small ducts and of alveolar enlargements as well as acini contained moderate amounts of mitochondria and of cisterns of the rough endoplasmic reticulum; the transcisterns of the Golgi apparatus which were surrounded by smooth and coated vesicles, exhibited modest dilatations, the number of lysosomes increased with age; regularly glycogen particles and bundles of intracellular filaments (phi 5 nm) were to be observed. Typical casein vesicles and stages of apocrine secretion of milk fat globules were not seen. The following features indicated secretory activity: differently sized vesicles and granules with flocculent, dense, or light contents were regularly to be seen in the apical cytoplasm often immediately below the apical plasma membrane of the epithelia of the small ducts and even more frequently in the alveolar enlargements. Secretory products of fine granular or filamentous structure, probably containing proteins, were frequently found within the lumen. Different types of lipid and liposome-like particles were detected both in intracellular localization as well as in ductal lumina. As demonstrated by lectin histochemistry the secretory products also contained a considerable amount of carbohydrate components. The composition of the secretory products of the resting gland is of clinical interest since the chronical deposition of secretions, which among others possibly contain enzymes producing oxygen radicals, may lead to pathological changes of mammary gland tissue.
Job satisfaction and leaving intentions of Slovak and Czech nurses

Abstract:Introduction: Shortages of healthcare workers in Czech and Slovak Republics are associated with factors such as raised migration of professional nurses, decreased number of nurse graduates and ageing workforce. The specific problem is migration of Slovak nurses to the Czech Republic, motivated by higher salaries. Aim: The study aims to investigate the relationship between turnover intentions and job satisfaction among Czech and Slovak nurses and to determine how the related variables differ between the two groups. Design: A cross-sectional descriptive survey was used to investigate the intended turnover and job satisfaction relationship among 1055 hospital nurses recruited from the Czech and Slovak Republics. Methods: Data were collected using a set of questionnaires that included questions regarding leaving intentions of nurses and The McCloskey/Mueller Satisfaction Scale; three further sets of questionnaires were used. For determining the associations between variables, the Pearson correlations and multiple regression analyses were used. For group comparisons, one-way and multifactorial analysis of variance and Pearson's Chi-square test procedure were performed. Findings: The intention to leave the workplace, the nursing profession and work abroad were predicted by the levels of satisfaction of nurses with their control/responsibility and scheduling. Czech nurses reported higher satisfaction in all subscales of the job satisfaction and less frequent intention to work abroad. An inverse relationship was confirmed between age and turnover intentions. Job satisfaction was positively associated with age and years of experience. Job satisfaction differed by all of three turnover intentions. Conclusion: Results highlight the importance of understanding nurses' leaving intentions and related factors and their impact on nurses' lives in both countries so that health care organizations can implement effective strategies to improve the retention of their nursing workforce.

Abstract:The mechanisms regulating coronary collateral circulation are largely unknown owing to both the complex and variable nature of clinical models and the difficulty to obtain quantitative and differentiated flow measurements within the various coronary tree portions. With the aim of assessing collateral flow reserve, we studied 19 patients with effort angina, without myocardial infarction and with isolated occlusion of either the left anterior descending coronary artery (n = 14) or the circumflex coronary artery (n = 5). Flow values were measured basally, during atrial pacing induced tachycardia and following ev dipyridamole infusion (0.56 mg/kg of body weight in 4 min), by means of positron emission tomography and nitrogen-13 ammonia as flow tracer, within both regions depending on collateral circulation and the remote ones. Results have been compared with those obtained in 13 normal subjects. Basal flow values in regions depending on collateral circulation and in the remote regions (0.61 +/- 0.11 vs 0.63 +/- 0.17 ml/min/g) were found to be similar, but lower than in normal subjects (1.00 +/- 0.20 ml/min/g, p < 0.01). During atrial pacing, flow increased to 0.83 +/- 0.25 and to 1.11 +/- 0.39 ml/min/g, in the regions depending on collateral circulation and in the remote regions, respectively (p < 0.05 as compared to baseline); again, values were lower than in normal subjects (1.86 +/- 0.61 ml/min/g, p < 0.01). Dipyridamole infusion further increased flow in the remote regions (1.36 +/- 0.57 ml/min/g, p < 0.01 as compared to atrial pacing) but it did not in the regions depending on collateral circulation (0.94 +/- 0.37 ml/min/g, NS as compared to atrial pacing); both values were lower than in normal subjects (3.46 +/- 0.78 ml/min/g, p < 0.01). Flow reserve in the regions depending on collateral circulation was found to have a direct linear correlation with the one in the remote regions (r = 0.83; p < 0.01). In conclusion, in spite of basal hypoperfusion, collateral circulation maintains a flow reserve which, even if reduced, is able to cope with moderate increments in oxygen consumption. An analogous flow reduction can be observed in the remote regions, suggesting that the entire coronary tree is involved, beyond the obstructive lesions of the main arterial branches.

Abstract:5.3% of all ruptures of the Achilles tendon occurring during alpin skiing are accompanied by an eversion fracture of the inner malleolus. The frequency of this type of combined trauma has not changed in the last fifteen years. A typical eversion trauma resulting in the inner malleolus being torn off, but not involving a rotation motion, occurs in the second phase of an acute angle foreward fall, in which in the first phase, the Achilles tendon has already been ruptured. Simply catching an edge, for the most part of the outer ski edge, does not lead to an eversion fracture as has formerly been supposed.
Addressing Nursing Resignation: Insights From Qualitative Studies on Nurses Leaving Healthcare Organisations and the Profession

Abstract:Aim The aim of this study is to explore the experiences of nurses who resigned from healthcare organisations or abandoned the profession and explore the reasons behind them. Design A systematic review of qualitative studies and meta‐summary. Data Sources Cumulative Index to Nursing and Allied Health Literature (CINAHL), Embase (Ovid), MEDLINE (Ovid), Social Science Citation Index (Web of Science), and Scopus. Methods The search was conducted up to May 2024. Primary qualitative studies focused on nurses who had resigned or left the profession were included. The meta‐summary was conducted using method: findings were extracted from the reports, edited, grouped, abstracted into key meta‐findings, and finally, their frequency effect sizes were calculated. Results A total of 282 findings were extracted from 12 studies, generating 49 statements of findings that were aggregated into nine key meta‐findings. Poor management practices presented a frequency effect size of 100%. Other key meta‐findings included excessive workload, teamwork hurdles, health issues related to work shifts and difficulty in maintaining work‐life balance, a lack of career growth opportunities and promotion chances, disillusionment with nursing, dissatisfaction due to salary, bullying and horizontal violence, and moral distress over ethical dilemmas. Conclusion The findings can help support the development of targeted strategies and the implementation of effective policies aimed at reducing nursing turnover. Impact and Implications for the Profession The major impact of these findings is the recognition of rising factors that negatively affect nurses' quality of life, including workload pressures and poor management strategies, which significantly lower job satisfaction. To address these challenges, the profession should prioritise tools that value nurses in their roles, implement strategies to manage workloads more effectively and advocate for policies promoting flexible scheduling. Additionally, investing in professional development and fostering a supportive work environment can help retain skilled nurses and nurture the growth of new talent. Effects of recognition of flexible work systems, organizational commitment, and quality of life on turnover intentions of healthcare nurses

Abstract:Background: It is necessary to adjust and mediate environmental, personal, and structural aspects to reduce the turnover rate among healthcare nurses, and awareness of flexible work systems, organizational commitment and quality of life are factors contained in this category. Objective: The purpose of this study was to identify the influence of awareness regarding flexible work systems, organizational commitment, and quality of life on turnover intentions among healthcare nurses. Methods: Two hundred and twenty-six healthcare nurses participated in the study. Data were collected from September 1 to October 1, 2018, and analyzed using SPSS/WIN 23.0 version. Results: The results in the first analysis revealed that being dissatisfied with work (β= 0.287) and having poor subjective health (β= 0.197) were significant influencing factors with a combined explanatory power of 14.9% (p< 0.001). In the second analysis, dissatisfied with work satisfaction (β= 0.143), organizational commitment (β=-0.389), and quality of life (β= 0.256) were significant influencing factors with a combined explanatory power of 29.5% (p< 0.001). Conclusion: It is necessary to reflect awareness of flexible work systems, organizational commitment and quality of life in interventions to reduce the turnover intention of healthcare nurses.

Abstract:Gibberellic acid (GA(3)) sensitivity (measured as alpha-amylase production) of the isolated aleurone tissue/deembryonated seed of two wheat (Triticum aestivum L. var Kite and var Aroona) varieties each containing either one of the dwarfing genes, Rht1 and Rht2, was increased significantly as a result of low temperature treatment. The magnitude of the low temperature-induced increase occurred without any change in the lag time of alpha-amylase production. This low temperature induction of GA(3) sensitivity was found to be operative in aleurone tissue of only those varieties having at least one of the three Rht alleles. It is likely, therefore, that the low temperature treatment effect which ;cures' or circumvents the genetic lesions manifest in the Rht1 and Rht2 genotypes is the same as that effective in the Rht3-containing genotype and probably involves an increase in hormone (GA(3)) receptor sites. Furthermore, this increase appears to be a quantitative temporal one.

Abstract:AIM: The aim of this study was to explore the association between the quality of the work environment, job characteristics, demographic characteristics and a pathway of job satisfaction, emotional exhaustion and turnover intention among nurses in emergency departments and perform subgroup analyses.BACKGROUND: Turnover intention among nurses is high. Multiple causes have been described, mostly in large studies of nurses working on general wards, often without considering complementarity of conceptual models and showing scant interest in the consistency of associations across subgroups of nurses.DESIGN: Cross-sectional multicentre survey.METHOD: Convenience sample of 294 nurses in 11 Belgian emergency departments during 2014-2015. Indirect effects in the form of mediation and serial mediation were estimated to assess the association between work environment (Magnet model), job characteristics (Job Demand Control Support model) and turnover intention via job satisfaction and emotional exhaustion. Consistency of these indirect effects across subgroups of nurses was examined using moderated mediation analysis (conditional indirect effects).RESULTS: Several Magnet and Job Demand Control Support dimensions were related to turnover intention, either via job dissatisfaction (mediation) or via job satisfaction and emotional exhaustion (serial mediation). In the case of social support from supervisor, these indirect effects were only significant for female nurses, among whom turnover intention was higher. Last, nurses with more years of experience were less likely to indicate turnover intention.CONCLUSION: To maximize prevention of turnover intention at emergency departments, interventions could target early career nurses, work environment and job characteristics. Female nurses in particular may also benefit from improved social support from their supervisor.

Abstract:Turnover intention is a predictor of the decision to leave an organization, which, if carried out, affects the quality of care provided. There is an association between turnover intention and organizational commitment. The more committed nurses are to the unit in which they work, the more committed they become to the unit's organizational goals; thus, they tend to continue working for the organization. Aiming to assess the turnover intention and the organizational commitment of nurses in primary healthcare, we conducted a quantitative, observational, descriptive, and cross-sectional study. The Intention of Turnover Scale and the Organizational Commitment Scale were applied in a sample of 297 nurses. Data were analyzed based on descriptive statistics. About 92.8% of the nurses intend to stay at their current workplace and only 7.3% plan to leave soon, suggesting low turnover intention; 84.5% of the nurses are willing to make an effort beyond what is normal to help their organization succeed, and 88.7% feel really interested in the destiny of the organization, which shows high organizational commitment. Pearson's Coefficient revealed the existence of a significant negative correlation between the factors "Intention to leave" and "Committed to the organization" (r = −0.51, p < 0.01). These findings suggest that, when nurses are more committed to their work and to the organization, they display less intention to leave, keeping the teams committed and motivated towards the organizational goals Our findings may guide nurse managers and policy-makers to develop strategies that retain nurses in organizations, keeping them motivated and engaged, and focusing on higher organizational commitment due to the influence it may have on the turnover intention.

Abstract:Aim To investigate whether education, tenure, being an advanced practice nurse, skill level, and time pressure impact perceptions of "having a place" and, further, turnover intentions. Background Nursing shortages persist worldwide. Nurses' turnover intentions are negatively related to their perceptions of "having a place" (i.e., the feeling that the nursing workplace is their territory). However, the sources of nurses' perceptions of the perception of "having a place" remain unknown. Methods Our research employed a cross‐sectional and correlational design. This research was conducted at a large‐scale hospital in northern Taiwan from December 2021 to January 2022. We used personnel data pertaining to 430 nurses as well as scales for time pressure, "having a place" and turnover intentions to assess nurses' intention to leave their place of employment. The inclusion criteria focused on full‐time nurses who worked for the hospital under investigation. Most of our participants were women. The STROBE statement was used as the EQUATOR checklist (supplemental file). Results "Having a place" was positively related to educational level, tenure, and skill level, while being an advanced practice nurse was negatively associated with perceptions of "having a place," which in turn were negatively related to turnover intention among nurses. Conclusion Our study is the first to examine the antecedents of nurses' perceptions of "having a place," which include education, tenure, and skill level. Implications for nursing policy Nursing policymakers could encourage nurses to pursue higher degrees and update their nursing skills while instilling perceptions of "having a place" in nurses with a brief tenure and advanced practice nurses.
