Abstract:Background: This study aimed to compare the accuracy of linear measurements obtained using the classical (manual) method versus cone-beam computed tomography (CBCT) in craniofacial anthropometry, specifically targeting the infraorbital foramen (IOF). Methods: This study involved two sample groups: one of 40 dry skulls measured manually using digital calipers, and the other of 40 CBCT 3D images digitally measured. Measurements included IOF height, distances between the IOF and nasion (N), IOF and frontomalar orbital (FMO), and between the two IOFs. Statistical analysis was performed using an ANOVA, t-test, and Fisher's test with a significance level of 0.05. Results: The manual method recorded a mean IOF height of 2.1 mm, while CBCT showed a mean of 3.52 mm. Significant differences were recorded between the two methods when measuring IOF height, with a p 0.05). The distance between the two IOFs measured by both methods differed significantly (p = 0.03157), with CBCT measurements showing higher values. Conclusions: In conclusion, although no significant differences were found in the overall accuracy of the two methods, CBCT proved to be a more reliable tool for detailed craniofacial measurements due to its higher accuracy and reproducibility. CBCT demonstrated superior consistency in measurements, offering enhanced precision in assessing craniofacial structures. These findings support the growing preference for CBCT in maxillofacial surgery, where precision is critical for successful outcomes. Nonetheless, manual techniques remain valuable in settings where advanced imaging is not accessible.

Abstract:Abstract Objective Since the introduction of cone-beam computed tomography (CBCT) in dentistry, this technology has enabled distortion-free three-dimensional cephalometric analysis for orthodontic and orthognathic surgery diagnosis. However, CBCT is associated with significantly higher radiation exposure than traditional routine bidimensional examinations for orthodontic diagnosis, although low-dose protocols have markedly reduced radiation exposure over time. The objective of this preliminary feasibility study is to compare the accuracy and diagnostic capabilities of an already-validated three-dimensional cephalometric analysis on CBCT to those of an analysis on 3-T magnetic resonance imaging (3T-MRI) to assess whether the latter can deliver a comparable quality of information while avoiding radiation exposure. Materials and methods In order to test the feasibility of three-dimensional cephalometry on 3T-MRI, 18 subjects (4 male; 14 female) with mean age 37.8 ± SD 10.2, who had undergone both maxillofacial CBCT and maxillofacial 3T-MRI for various purposes within 1 month, were selected from the archive of the Department of Dentistry and Maxillofacial Surgery of Fondazione Ospedale Policlinico Maggiore, IRCCS, Milano, Italy. A three-dimensional cephalometric analysis composed of ten midsagittal and four bilateral landmarks and 24 measurements (11 angular, 13 linear) was performed on both scans using Mimics Research® v. 17.0 (NV, Technologielaan 15, 3001 Leuven, Belgium). Cephalometric analysis was performed twice by two independent orthodontists for each scan, and each orthodontist repeated the measurements 3 weeks later. Statistical analysis was performed with SPSS® 20.00 for Windows (IBM® Corporation, Sommers, NY, USA). A Bland-Altman test for each cephalometric value was performed to assess the agreement between the procedures. The intraclass correlation coefficient (ICC) was used to assess interobserver and intraobserver reliability. The coefficient of variation was used to evaluate precision. Results Both procedures showed good reliability, with mean intraobserver ICCs of 0.977/0.971 for CBCT and 0.881/0.912 for MRI. The average interobserver ICCs were 0.965 for CBCT and 0.833 for MRI. A Bland-Altman analysis for the cephalometric tracing revealed a similar range of agreement between the two modalities; the bias range (mean ± SD) was − 0.25–0.66 mm (0.174 ± 0.31) for distances and − 0.41–0.54° (0.12 ± 0.33) for angles. Conclusions Within the main limitation of this pilot study, that is, the small sample, it is possible to state that cephalometric measurements on 3T-MRI seem to possess adequate reliability and repeatability and that they show satisfying agreement with values measured on CBCTs. An MRI examination does not expose patients to ionizing radiation and could provide an alternative to CBCT for three-dimensional cephalometrics in the future.

Correlation Assessment Between Three-Dimensional Facial Soft Tissue Scan and CBCT Data in Orthodontic Diagnosis for Skeletal Class Ⅲ Adolescents

Abstract:Objective The aim of the current study was to investigate correlations between measurements on 3D facial soft tissue scan and CBCT skeletal structures.Methods The study sample consisted of 20 subjects living in Beijing area.A 3D face scan analysis including two distance values and 22 angle values based on 14 landmarks,and the corresponding CBCT analysis with three distance values and 9 angle values were performed for all subjects.All measurements were operated twice by the same researcher.Then,the reproducibility was evaluated by intraclass correlation coefficient (ICC),and the correlations were evaluated by Pearson correlation coefficient analysis.Besides,the thickness of facial soft tissue at relative points to explore possible reasons between the correlations.Results The reproducibility of measurements related to soft tissue gonion is medium.For sagittal measurement,medium correlations were found between soft tissue subspinale and hard tissue point A,as well as the relative position of maxilla and mandible.For vertical measurements,soft tissue gonions,mandibular plane angles showed medium correlations with hard tissue,and medium correlation were found between anterior upper and lower facial heights with corresponding hard tissue measurements and hard tissue mandibular plane angle.And the thickness of facial soft tissue at subspinale was larger than that at alare.Conclusions Soft tissue measurements of alare,the relative position between maxilla and mandible,upper anterior facial height and lower anterior facial height are reliable in providing a soft tissue diagnosis reference to the CBCT analysis.While the measurements related to soft tissue gonion should be used with caution.
Accuracy and reliability of craniofacial manual landmark identification on cone beam CT images of dry skulls

Abstract:Objective:To evaluate the accuracy and reliability of cone beam CT of craniofacial landmarks, so as to establish the 3D cephalometric system.Methods:This is a cross-sectional study. Three orthodontists marked 32 landmarks on unmarked cone beam CT of 15 dry skulls, and stainless steel beads were used to mark landmarks on dry skulls as the gold standard. The reliability of manual landmarking was evaluated by calculating the axial standard deviation and mean radial spherical error (MRSE) of all landmarks. The average error distance between individual landmark detections of each orthodontist and gold standard, between individuals and average detections, and between the average detections and the gold standard were calculated to evaluate the accuracy.Results:The average error between detections of individuals and the average landmark detections was (0.778±0.57) mm. It was (1.391±0.821) mm between the average detection and the gold standard. It was (1.599±1.081) mm between individuals and the gold standard. The landmark point of posterior edge of ramus had the largest MRSE and Jugal showed largest error compared to the gold standard.Conclusions:Averaging multiple manual detections can elevate accuracy. The landmarks of reference system and definite anatomical structure, like anterior and posterior nasal spines, are reliable and accurate. The reliability of landmarks on smooth surfaces, like point A and point B, is poor, and more strict three-dimensional definition is needed; while Jugal with three-dimensional definition showed less error.
Reliability and Validity of Miniscrews As References in Cone-Beam Computed Tomography and Intraoral Scanner Digital Models: Study on Goat Heads

Abstract:Abstract Background Miniscrews have been used to superimpose three-dimensional (3D) craniofacial images as well as explore stable structures in jaws. Our purpose was to evaluate the reliability and validity of linear and angular measurements made with miniscrews on a 3D cone-beam computed tomography (CBCT) at two voxel sizes and compared to models created by an intraoral scanner (IOS). Methods Altogether, 64 miniscrews were placed in 12 goat jaws. The jaws were scanned by CBCT machine at 0.12 mm and 0.3 mm voxels and by the IOS. Linear and angular measurements between miniscrews on CBCT at the two voxel settings and the IOS were compared with actual measurements and with each other. Results An intra-and inter-class correlation of 0.961–1.000 were obtained by each method. Linear measurements showed significant overestimations of 0.27 ± 0.24, 0.14 ± 0.22 and 0.15 ± 0.26 mm, and angular measurements showed non-significant differences of 0.11 ± 1.97°, 0.15 ± 2.79° and 0.41 ± 2.34° for the CBCT at 0.12-mm, 0.3-mm voxels and the IOS, respectively. Equal magnification of linear measurements was on homolateral and contralateral sides using CBCT, whereas significantly greater magnification on the homolateral side than on the opposite was observed using the IOS. There was no significant difference with angular measurements between digital CBCT models at two voxels and IOS. In addition, all angular measurements were comparable to actual measurement results. Conclusions Miniscrews in CBCT and IOS are reliable and clinical valid when used as a reference measuring tooth movement. However, when miniscrews are involved in high precision measurement in CBCT or IOS image, systematic error should be taken into consideration. When comparing CBCT images, using the same voxel size is recommended for miniscrew related measurements to reduce error.

Abstract:Several semi-automatic software are available for the three-dimensional reconstruction of the airway from DICOM files. The aim of this study was to evaluate the accuracy of the segmentation of the upper airway testing four free source and one commercially available semi-automatic software. A total of 20 cone-beam computed tomography (CBCT) were selected to perform semi-automatic segmentation of the upper airway. The software tested were Invesalius, ITK-Snap, Dolphin 3D, 3D Slicer and Seg3D. The same upper airway models were manually segmented (Mimics software) and set as the gold standard (GS) reference of the investigation. A specific 3D imaging technology was used to perform the superimposition between the upper airway model obtained with semi-automatic software and the GS model, and to perform the surface-to-surface matching analysis. The accuracy of semi-automatic segmentation was evaluated calculating the volumetric mean differences (mean bias and limits of agreement) and the percentage of matching of the upper airway models compared to the manual segmentation (GS). Qualitative assessments were performed using color-coded maps. All data were statistically analyzed for software comparisons.

Accuracy of cone-beam computed tomography at different resolutions assessed on the bony covering of the mandibular anterior teeth

Abstract:Introduction: The aim of this study was to determine the accuracy of cone-beam computed tomography (CBCT) with different voxel resolutions. Measurements were made of the bony covering of the mandibular anterior teeth because this region is crucial in orthodontic treatment planning. Methods: CBCT data at 2 resolutions (0.125-mm and 0.4-mm voxels) were collected from 8 intact cadaver heads. The vertical position of the mucogingival junction was clinically assessed. After removal of the gingiva, vertical and horizontal bony measurements were taken, and the buccal alveolar bone margin was determined. Anatomic bony measures were compared with the CBCT measures, and the correlation of the mucogingival junction measures to the buccal alveolar bone margin measures was evaluated. Results: Bony measures obtained with CBCT were accurate and differed only slightly from the physical findings. The mean differences, ranging from -0.13 to +0.13 mm, were statistically not significant, but the limits of agreement showed discrepancies in the measurements as large as 2.10 mm, depending on measurement and resolution. Buccal alveolar bone margin measurements correlated with the mucogingival junction measurements (P <0.001). On average, the mucogingival junction was 1.67 mm more apical than the buccal alveolar bone margin (CI 95%, 1.35-1.98 mm). Conclusions: CBCT renders anatomic measures reliably and is an appropriate tool for linear measurements. Presence of soft tissue as well as different voxel size affect the precision of the data. A customized resolution protocol must be chosen according to the accuracy needed. However, even the 0.125-mm voxel protocol does not depict the thin buccal alveolar bone covering reliably, and there is a risk of overestimating fenestrations and dehiscences. The mucogingival junction appears to follow the buccal alveolar bone margin in a parallel line.

Abstract:Objectives: To evaluate the accuracy of CBCT in assessing trabecular structures.Methods: Two human mandibles were scanned by micro-CT (Skyscan 1173 high-energy spiral scan micro-CT; Skyscan NV, Kontich, Belgium) and CBCT (3D Accuitomo 170; Morita, Japan). The CBCT images were reconstructed with 0.5 and 1mm thicknesses. The condylar images were selected for registration. A parallel algorithm for histogram computation was introduced to perform the registration. A mutual information (MI) value was used to evaluate the match between the images obtained from micro-CT and CBCT.Results: In comparison with the micro-CT image for the two samples, the CBCT image with 0.5mm thickness has a MI value of 0.873 and 0.903 while that with 1.0mm thickness has a MI value of 0.741 and 0.752. The CBCT images with 0.5mm thickness were better matched with micro-CT images.Conclusions: CBCT shows comparable accuracy with high-resolution micro-CT in assessing trabecular structures. CBCT can be a feasible tool to evaluate osseous changes of jaw bones.
Assessing the inter-observer and intra-observer reliability of radiographic measurements for size-specific dose estimates

Abstract:Background: Calculating size-specific dose estimates (SSDEs) requires measurement of the patient's anteroposterior (AP) and lateral thickness based on computed tomography (CT) images. However, these measurements can be subject to variation due to inter-observer and intra-observer differences. This study aimed to investigate the impact of these variations on the accuracy of the calculated SSDE. Methods: Four radiographers with 1-10 years of experience were invited to measure the AP and lateral thickness on 30 chest, abdomen, and pelvic CT images. The images were sourced from an internet-based database and anonymized for analysis. The observers were trained to perform the measurements using MicroDicom software and asked to repeat the measurements 1 week later. The study was approved by the institutional review board at Taibah University, and written informed consent was obtained from the observers. Statistical analyses were performed using Python libraries Pingouin (version 0.5.3), Seaborn (version 0.12.2), and Matplotlib (version 3.7.1). Results: The study revealed excellent inter-observer agreement for the calculated effective diameter and AP thickness measurements, with Intraclass correlation coefficients (ICC) values of 0.95 and 0.96, respectively. The agreement for lateral thickness measurements was lower, with an ICC value of 0.89. The second round of measurements yielded nearly the same levels of inter-observer agreement, with ICC values of 0.97 for the effective diameter, 1.0 for AP thickness, and 0.88 for lateral thickness. When the consistency of the observer was examined, excellent consistency was found for the calculated effective diameter, with ICC values ranging from 0.91 to 1.0 for all observers. This was observed despite the lower consistency in the lateral thickness measurements, which had ICC values ranging from 0.78 to 1.0. Conclusions: The study's findings suggest that the measurements required for calculating SSDEs are robust to inter-observer and intra-observer differences. This is important for the clinical use of SSDEs to set diagnostic reference levels for CT scans.
Reliability of Artificial Intelligence-based Cone Beam Computed Tomography Integration with Digital Dental Images

Abstract:This study aimed to introduce cone-beam computed tomography (CBCT) digitization and integration of digital dental images (DDI) based on artificial intelligence (AI)-based registration (ABR) and to evaluate the reliability and reproducibility using this method compared with those of surface-based registration (SBR). This retrospective study consisted of CBCT images and DDI of 17 patients who had undergone computer-aided bimaxillary orthognathic surgery. The digitization of CBCT images and their integration with DDI were repeated using an AI-based program. CBCT images and DDI were integrated using a point-to-point registration. In contrast, with the SBR method, the three landmarks were identified manually on the CBCT and DDI, which were integrated with the iterative closest points method. After two repeated integrations of each method, the three-dimensional coordinate values of the first maxillary molars and central incisors and their differences were obtained. Intraclass coefficient (ICC) testing was performed to evaluate intra-observer reliability with each method's coordinates and compare their reliability between the ABR and SBR. The intra-observer reliability showed significant and almost perfect ICC in each method. There was no significance in the mean difference between the first and second registrations in each ABR and SBR and between both methods; however, their ranges were narrower with ABR than with the SBR method. This study shows that AI-based digitization and integration are reliable and reproducible.

Abstract:Cone-beam CT (CBCT) was developed for the three-dimensional (3D) imaging of the maxillofacial area and has become popular in dentistry, orthodontics, and maxillofacial surgery. The advantages of CBCT include less radiation exposure (than conventional CT), less artifacts, and submillimeter spatial resolution. CBCT has been shown to produce accurate 3D images of the craniofacial region and a 1-to-1 image-to-reality ratio, which has greatly reduced errors of frontal cephalometry and improved our ability to diagnose asymmetry. Several new analyses to diagnose asymmetries on 3D images described in recent literature were reviewed, including 3D linear and angular measurements, mirror-image analysis (superimposition of the mirror image of the anatomically correct part of the anatomy over the deformity), 3D cephalometric analysis based on constructing an individual coordinate system for each facial unit or element (local coordinate systems) to differentiate them from the world coordinate system (whole head orientation), and method of application of asymmetry indices.
Accuracy and reliability of cone-beam computed tomography in measuring alveolar bone height

Abstract:Objective To evaluate the accuracy and reliability of cone-beam computed tomography (CBCT) in measuring alveolar bone height.Methods Eight human cadaver head samples were scanned with CBCT technology.Measurements were made on each tooth from the cusp or the middle point of incisal margin to the cementoenamel junction (T-CEJ),from the cusp or the middle point of incisal margin to the alveolar bone margin (T-BM) and from the cementoenamel junction to the alveolar bone margin (CEJ-BM) along the long axis of the tooth.Measurements were made both in the CBCT images and on the real samples.The paired-t test was made between the two different measurements to evaluate the accuracy of the CBCT measurements.And 210 sites from 6 samples were measured.The Interclass correlation coefficient (ICC) was calculated to evaluate the reliability of CBCT measurements.Results The values of CBCT measurement were smaller than those of the direct measurement.Significant difference was found in the values of T-BM and CEJ-BM.The mean deviations were - 0.268 mm and - 0.208 mm.The values of T-CEJ showed no significant difference and the mean deviation was -0.041 mm. The ICC of all the measurement was above 0.96.Conclusions Significant difference was found in the measurement of alveolar bone height between CBCT and direct measurement. But the mean deviation was very small. The CBCT measurement showed high reliability.
Tooth segmentation by low-dose CBCT for orthodontic treatment planning : Explorative ex vivo validation

Abstract:Purpose: Three-dimensional imaging has become an increasingly important component of orthodontics. Associated with this, however, is a higher radiation exposure for patients. New cone-beam computed tomography (CBCT) devices have been developed that can provide low-dose CBCT (LD-CBCT). We hypothesized that LD-CBCT is as precise and reproducible as standard high-dose CBCT (HD-CBCT) in segmenting roots and crowns as well as measuring tooth length. Methods: HD-CBCT and LD-CBCT scans were taken of four human cadaveric heads. Thirty single-rooted teeth were segmented twice by one investigator. The length of each tooth was also measured. Lin's concordance correlation coefficient (CCC) was calculated to assess the agreement of HD-CBCT and LD-CBCT measurements and the intraclass correlation coefficient (ICC) was calculated to assess intrarater reliability. Analyses were supported by Bland-Altman plots. Results: Volume measurements obtained using HD-CBCT were significantly higher than those obtained using LD-CBCT (p < 0.001). CCC was 0.975 (95% confidence interval [CI] = 0.956-0.986) indicating excellent agreement between the two modalities. Intrarater reliability between the two sets of LD-CBCT and HD-CBCT volume measurements was excellent (ICC = 0.998, 95%CI = 0.995-0.999 [HD-CBCT], ICC = 0.997, 95%CI = 0.992-0.998 [LD-CBCT]). CCC for tooth length measurements was 0.991 (95% CI = 0.983-0.995), indicating excellent agreement between HD-CBCT and LD-CBCT. Intrarater reliabilities between the two sets of tooth length measurements were also excellent for both methods (ICC = 0.998, 95%CI = 0.995-0.999 [HD-CBCT], ICC = 0.997, 95%CI = 0.992-0.998 [LD-CBCT]). Conclusions: Within the limitations of this experimental setting, LD-CBCT is as valid as HD-CBCT for measuring tooth length. Regarding the volume differences, in vivo studies are required to determine their clinical relevance.

Abstract:Identifying landscape elements that influence gene flow and migration in wild species is the current main topic of landscape genetics. Most landscape genetic studies infer gene flow and migration from genetic distances among populations or individuals and statistically relate these measurements to landscape composition and configuration. This approach assumes symmetrical gene flow between pairs of populations. Such an assumption, however, will often be violated, especially in source–sink systems. Source populations provide more emigrants than they receive immigrants, and sink populations get many immigrants, but release few emigrants. Source–sink dynamics cannot be explored using common landscape genetic approaches relying on genetic distances. In this issue of Molecular Ecology, Andreasen et al. ( ) apply an alternative approach allowing them to infer asymmetrical migration. They use a Bayesian assignment test among objectively defined populations of mountain lions (Puma concolor) in western USA to estimate recent and directional migration rates. The study shows that an area with a high amount of wildlife refuges and low hunting pressure harbours a source population for mountain lion dispersal, while areas with high hunting pressures form sink populations; a result helpful in making informed decisions in conservation management.

The authors conducted a study to compare 2-dimensional (2D) lateral cephalometric radiography (LCR), 2D cone-beam computer tomographic (CBCT)-generated cephalogram and 3-dimensional (3D) CBCT for assessing cephalometric measurements. Methods. The authors took 2D LCR, 2D CBCT-generated cephalogram, and 3D CBCT images involving 60 participants. They obtained 11 angular and 11 linear measurements for all images. They used 1-way analysis of variance and the Fisher least significant difference test for statistical comparisons. The authors used Pearson correlation and Pearson chi(2) test to assess the relationship of these imaging modalities for vertical cephalometric analyses. Results. Significant differences existed between the 2D cephalograms (LCR and CBCT-generated cephalogram) and the 3D CBCT in 2 angular measurements (maxillary first incisor-nasion (N) point A [A] and mandibular first incisor-N point B (B) (P = .027 and P <. 001, respectively) and 5 linear measurements (N menton[Me]/sella gonion [Go], condylion [Co]A, Co gnathion, Go-Me and anterior nasal spine-posterior nasal spine) (P < .004). These measurement values with significant differences were generally greater (approximately 5 degrees for angular measurements and 10 millimeters for linear measurements) on the 3D CBCT scans than on the 2D cephalograms. No significant difference was found between the 2 2D cephalograms (P > .164). No significant difference was found among the 3 imaging modalities for the vertical cephalometric analyses (P > .466). Conclusions. Significant differences existed between the 2D cephalograms (LCR and CBCT-generated cephalogram) and the 3D CBCT scans in 2 angular and 5 linear measurements. The 2 2D cephalograms were similar for cephalometric measurements. The 3 imaging modalities had no significant difference for the vertical cephalometric analyses. CBCT might not add value for every orthodontic situation.
Comparison of the Accuracy (trueness and Precision) of Virtual Dentofacial Patients Digitized by Three Different Methods Based on 3D Facial and Dental Images

Abstract:Statement of problem The accuracy of virtual dentofacial patients has been explored, but the accuracy of virtual patients established by using a straightforward and reliable method and the accuracy of different virtual patients are unclear. Purpose The purpose of this clinical study was to compare the accuracy of virtual dentofacial patients digitized by using registered-block impression, exposed anterior teeth, and cone beam computed tomography (CBCT) reconstruction methods based on 3-dimensional (3D) facial and dental images. Material and methods From the 15 selected participants who needed CBCT scanning, 3 kinds of virtual dentofacial patients were established by using 3 registration methods based on digital dental casts: 3D facial images, CBCT data, and registered-block impression. Compared with actual measurement, 25 linear distances of all virtual dentofacial patients were selected and measured by using a software program, and 3 separate measurements were calculated by the same person. The 1-way analysis of variance (ANOVA) was used to compare the deviations among 3 kinds of virtual dentofacial patients (trueness) and the deviations within groups (precision). The 1-sample t test was used to compare the difference between the deviation and the ideal error of 0.00 (α=.05). Results Compared with the actual measurement, the trueness of the average deviations for registered-block impression (1.02 ±1.24 mm) was better than that of exposed anterior teeth (2.35 ±1.71 mm) and CBCT reconstruction (2.86 ±1.61 mm). The precision of the average deviations for registered-block impression (1.29 ±1.43 mm) was better than that of exposed anterior teeth (2.00 ±1.72 mm) and CBCT reconstruction (2.12 ±1.94 mm). Significant differences in trueness and precision were found among the 3 groups of virtual dentofacial patients (P<.01). Significant differences among the deviations of all linear distances and the ideal error of 0.00 were observed for all groups of virtual dentofacial patients (P<.05). Conclusions The accuracy of registered-block impression was better than that of the exposed anterior teeth and CBCT reconstruction. The accuracy of exposed anterior teeth was lower than that of the other methods but could satisfy the requirements of clinical diagnostics and scientific methods. The accuracy of CBCT reconstruction was poor and could only be used for special situations that permitted low accuracy.
Accuracy Assessment of Three-dimensional Surface Reconstructions of in Vivo Teeth from Cone-beam Computed Tomography.

Abstract:Background:The accuracy of three-dimensional (3D) reconstructions from cone-beam computed tomography (CBCT) has been particularly important in dentistry, which will affect the effectiveness of diagnosis, treatment plan, and outcome in clinical practice. The aims of this study were to assess the linear, volumetric, and geometric accuracy of 3D reconstructions from CBCT and to investigate the influence of voxel size and CBCT system on the reconstructions results. Methods:Fifty teeth from 18 orthodontic patients were assigned to three groups as NewTom VG 0.15 mm group (NewTom VG; voxel size: 0.15 mm; n = 17), NewTom VG 0.30 mm group (NewTom VG; voxel size: 0.30 mm; n = 16), and VATECH DCTPRO 0.30 mm group (VATECH DCTPRO; voxel size: 0.30 mm; n = 17). The 3D reconstruction models of the teeth were segmented from CBCT data manually using Mimics 18.0 (Materialise Dental, Leuven, Belgium), and the extracted teeth were scanned by 3Shape optical scanner (3Shape A/S, Denmark). Linear and volumetric deviations were separately assessed by comparing the length and volume of the 3D reconstruction model with physical measurement by paired t-test. Geometric deviations were assessed by the root mean square value of the imposed 3D reconstruction and optical models by one-sample t-test. To assess the influence of voxel size and CBCT system on 3D reconstruction, analysis of variance (ANOVA) was used (&agr; = 0.05). Results:The linear, volumetric, and geometric deviations were −0.03 ± 0.48 mm, −5.4 ± 2.8%, and 0.117 ± 0.018 mm for NewTom VG 0.15 mm group; −0.45 ± 0.42 mm, −4.5 ± 3.4%, and 0.116 ± 0.014 mm for NewTom VG 0.30 mm group; and −0.93 ± 0.40 mm, −4.8 ± 5.1%, and 0.194 ± 0.117 mm for VATECH DCTPRO 0.30 mm group, respectively. There were statistically significant differences between groups in terms of linear measurement (P < 0.001), but no significant difference in terms of volumetric measurement (P = 0.774). No statistically significant difference were found on geometric measurement between NewTom VG 0.15 mm and NewTom VG 0.30 mm groups (P = 0.999) while a significant difference was found between VATECH DCTPRO 0.30 mm and NewTom VG 0.30 mm groups (P = 0.006). Conclusions:The 3D reconstruction from CBCT data can achieve a high linear, volumetric, and geometric accuracy. Increasing voxel resolution from 0.30 to 0.15 mm does not result in increased accuracy of 3D tooth reconstruction while different systems can affect the accuracy.
Reliability of Three-Dimensional Micro-Computed Tomography Root Canal Analysis Using Drishti Software: A Pilot Study

Abstract:In recent years, micro-computed tomography (micro-CT) has become a promising tool for non-destructive imaging and the study of root canal morphology in endodontics. However, the reliability of such analyses heavily relies on the accuracy and consistency of the software used for image processing. Objective: This study aims to investigate the reliability of two different micro-CT voxel sizes using open-source three-dimensional (3D) software (Drishti v3.0) used to identify and quantify the Apical Foramen (AF) and Apical Constriction (AC) in mandibular premolars. Methods: Eight mandibular premolars were divided into two groups, with four samples in each group. Samples were scanned using a ZEISS X-Radia 520 Micro-CT system with two different voxel sizes of 23 μm (Group A) and 17 μm (Group B), respectively. The micro-CT data were reconstructed and transferred to Drishti Software, and individual image rendering was performed to visualize the images for data analysis. The position and measurement of AF and AC to the anatomical apex were measured individually and independently by single operators. Results: The reliability of the measurements assessed using Intraclass Correlation Coefficients (ICCs) between two voxel sizes was excellent and on par with previous research. This pilot study suggested that results for voxel sizes of 23 μm and 17 μm were comparable. Both voxel size 3D images demonstrated excellent and high-quality visualization of the root canal configuration, lateral and accessory canals, apical deltas, major AF, and AC. Conclusion: It is feasible and reliable to visualize and analyze 3D micro-CT images using open-source software (Drishti v3.0). Both voxel sizes of 23 μm and 17 μm are recommended for use in root canal study.
Precision of Cephalometric Landmark Identification from Cone-Beam Computed Tomography

Abstract:Objective In this study, we compared the precision of landmark identification using cephalograms from cone-beam computed tomographic (CBCT) volumes and conventional lateral cephalograms (Ceph). Methods Twenty pre-orthodontic patients were radiographed with conventional Ceph and CBCT techniques. CBCT data was uploaded into InvivoDental software 5.0 to generate cephalograms(CCB). Three observers plotted 23 landmarks using computer displays of CCB and Ceph views during separate sessions. Absolute differences between CCB and Ceph of all observers were measured. The absolute difference between each observer was also measured. ANOVA and paired t tests were used to analyze variability differences. Results The difference of landmark identification between CCB and Ceph were significant at P＜0.05 calculations. 8 landmarks variability was statistically greater than Ceph views. The variability of CCB for each observer was consistently greater than in Ceph. The overall correlation of CCB and Ceph measurements was excellent at 0.99.Conclusions The CCB displays of CBCT volume images provide generally more precise identification than Ceph. More precise location of basion, porion, orbitale, ANS, A point, Gonion and tooth apex overcomes the problem of superimposition of these bilateral landmarks seen in Ceph. Greater variability of certain landmarks is probably related to inadequate definition of the landmarks in the third dimension.
