Abstract:BACKGROUND AND AIMS: The scenario in medical education is changing with objective structured clinical examination (OSCE) being introduced as an assessment tool. Its successful implementation in anaesthesiology postgraduate evaluation process is still limited. We decided to to evaluate the effectiveness of OSCE and compare it to conventional examinations as formative assessment tools in anaesthesiology.METHODS: We conducted a cross-sectional comparative study in defined population of anaesthesiology postgraduate students to evaluate the effectiveness of OSCE as compared to conventional examination as formative assessment tool in anaesthesiology. Thirty-five students appeared for the conventional examination on the 1(st) day and viva voce on the 2(nd) day and OSCE on the last day. At the conclusion of the assessment, all the students were asked to respond to the perception evaluation questionnaire. We analysed the perception of OSCE among the students.RESULTS: Results showed a positive perception of the objective structured physical examination (OSCE) as well as structured 9 (25.7%), fair 19 (54.2%) and unbiased 13 (37.1%) with more standardised scoring 9 (25.7%). The students perceived OSCE to be less stressful than other examination. Thirty-one (88.5%) students agreed that OSCE is easier to pass than conventional method and 29 (82.5%) commented that the degree of emotional stress is less in OSCE than traditional methods.CONCLUSION: OSCE is better evaluation tool when compared to conventional examination.

Abstract:Introduction: Objective Structured Clinical Examination (OSCE) is a well-known, widespread method of assessment of clinical skills. It is being widely used in Egyptian medical schools. This study aimed to explore the perception of the fifth-year medical students on the attributes, quality, validity, reliability and organization of the end-of-rotation Orthopedics Surgery and Trauma OSCE held at FOM-SCU in two academic years (2017-2018 and 2018-2019). It also aimed to assess the students rating of OSCE in relation to the other available assessment methods in clinical rotations. Material and Methods: This is a cross-sectional mixed-method study that was conducted at Suez Canal University Hospital. A convenient sample of the fifth-year medical students, who underwent the OSCE at the end of their Orthopedic Surgery and Trauma rotation during both academic years 2017-2018 and 2018-2019 were involved (n = 254). Quantitative data were collected through a validated questionnaire consisting of 32 items. Focus group discussions of students were conducted and qualitative data were recorded, coded, and thematically analyzed. Results: More than half of the students (55.5%) believed that the exam was fair and covered a wide range of knowledge (63.8%) and clinical skills (72.4%). Considerable percentages of students were doubtful regarding the standardization of OSCE scores (62.6%) and whether those scores provided a true measurement of their clinical skills (65%) and more than half of them were not sure whether gender, personality, or ethnicity affected their exam scores (55.5%) and whether OSCE provides them practical and useful experience (53.5%). OSCE and portfolio were reported as the easiest method among 55.5% and 63.8% of students, respectively, and 31.1% rated MCQs as the most difficult form of assessment. Qualitative analysis identified two themes; namely: “Challenges of implementing OSCE” and “Ways to overcome identified challenges”. Conclusion: Medical students positively perceived and provided good perception on the organization and implementation of the Orthopedics OSCE, although some of them were doubtful regarding its validity and reliability in assessing their clinical skills in Orthopedics and Trauma. The challenges regarding the OSCE can be overcome by more well-structured, practical training and orientation sessions for the examiners, students, and simulated patients.

Abstract:The assessment of undergraduate medical students consists of a written component and an advanced clinical competency test that evaluates the students' skills. The Objective Structured Clinical Examinations (OSCE) were only implemented in most medical schools in the last ten years, similar in many developing countries. It was first used with other clinical assessment methods. This study was designed to investigate how medical students in the fourth and sixth grades and examiners perceived the unique format, features, and quality of the Grand Objective Structured Clinical Examinations (GOSCE), a novel method for assessing students. The aim was to understand how this innovative method, distinct from traditional assessment methods, prepares students for real-world scenarios where they encounter different specialty cases. Additionally, the study aimed to understand the significant role and challenges faced by the Faculty of General Medicine at Koya University in implementing this type of OSCE and to identify potential improvements to enhance the quality of the assessment process.

Abstract:Purpose: The objective structured clinical examination (OSCE) is considered to be one of the most robust methods of clinical assessment. One of its strengths lies in its ability to minimise the effects of examiner bias due to the standardisation of items and tasks for each candidate. However, OSCE examiners' assessment scores are influenced by several factors that may jeopardise the assumed objectivity of OSCEs. To better understand this phenomenon, the current review aims to determine and describe important sources of examiner bias and the factors affecting examiners' assessments. Methods: We performed a narrative review of the medical literature using Medline. All articles meeting the selection criteria were reviewed, with salient points extracted and synthesised into a clear and comprehensive summary of the knowledge in this area. Results: OSCE examiners' assessment scores are influenced by factors belonging to 4 different domains: examination context, examinee characteristics, examinee-examiner interactions, and examiner characteristics. These domains are composed of several factors including halo, hawk/dove and OSCE contrast effects; the examiner's gender and ethnicity; training; lifetime experience in assessing; leadership and familiarity with students; station type; and site effects. Conclusion: Several factors may influence the presumed objectivity of examiners' assessments, and these factors need to be addressed to ensure the objectivity of OSCEs. We offer insights into directions for future research to better understand and address the phenomenon of examiner bias.

Abstract:Background: Clinical education is one of the important and basic principles in nursingeducation which is accepted by all programmers and managers of educational programs ofthis field as the main part of nursing education. The assessment of clinical competence istherefore an important issue in nursing education and the utilization of objective structuredclinical evaluation (OSCE) for that purpose is considered very important. The skillscomponent in nursing-related subjects contributes to at least 50% of the content of thesesubjects, with this high proportion also requiring that any assessment should reflect thissignificant emphasis on the skills (psychomotor) component. Objective: The aim of this studywas to assess objective structured clinical Examination’s regarding clinical competence andstressors among undergraduate nursing students. Setting: It was carried out at critical careand emergency nursing department, faculty of nursing, Damanhour University. Subjects: Thisstudy involved 85 undergraduate nursing students. Tools: Assessment of Objective StructuredClinical Examination regarding clinical competence and stressors structure questionnairewas used to collect data. The questionnaire consists of 20 statements. 10 statements related toclinical competence and the other related to stressors. For each statement students wereasked to rate their responses on a point scale rating from 1-5. (1) For strongly disagree, (2)for disagree, (3) for not sure, (4) for agree and (5) for strongly agree. Results: It was foundthat the majority of the respondents agreed or strongly agreed that they received enoughinformation, appropriate repetition of OSCE, the quality of information was quiet well, OSCEis a standardized exam for all students, OSCE is a fair exam, OSCE minimizes chances offailing, students prepare differently for OSCE than for other clinical examinations, OSCEprovides them with practical and useful experience, OSCE chance for developing confidence to conduct similar procedure on real patients, OSCE scores truly reflect competence inclinical skills, OSCE has a positive impact on clinical learning. Most of the studied sampledisagreed or strongly disagreed that level of stress was low, time to complete procedure orreading instructions was enough. Conclusion: Nursing students perceived OSCE as suitablemethod which improves their clinical competence. But there was time limitation while itsimplementation and the majority of students considered it as one of the important factors inproducing stress in OSCE. Recommendations: Determine time needed for each procedureand accommodate OSCE, and Increase frequencies of OSCE through open lab hour.
Implementation and evaluation of objective structured clinical examination for eight-year program medical students of clinical medicine in clerkship

Abstract:Objective:Objective structured clinical examination (OSCE) was implemented as an evaluation tool for clerkship trainees of eight-year program of clinical medicine. Its effect was evaluated in order to provide reference for its application in clerkship.Methods:Forty-nine medical students of 2017 grade who were trained in clerkship in Peking University First Hospital from June 2020 to January 2021 were involved in the examination. The examination was composed of 6 stations including 2 stations for clinical history taking, 2 stations for physical examination and 2 stations for essential skills. The effect of OSCE implementation was evaluated by the students' scores of OSCE, the degree of difficulty, the degree of discrimination, the reliability of OSCE and the students' questionnaire.Results:The student's total score was [9.23(8.98, 9.41)]. The overall difficulty was 0.087, discrimination was 0.126 and the Cronbach α of reliability was 0.739. There were 85.7%(42/49) of the students who considered that the overall design of the assessment was reasonable, and 91.8%(45/49) of them asked for feedback from examiners. The results also showed that 81.6%(40/49), 75.5%(37/49) and 85.7%(42/49) of the students believed that OSCE well evaluated the mastery of skills dealing with medical history taking, physical examination and essential clinical skills. Most students believed that it may promote competency building, especially communicator [93.9%(46/49)] and professional capacity [71.4%(35/49)]. However, only 16.3%(8/49) of the students were satisfied with their self-performance in OSCE.Conclusions:OSCE is characterized by reliability, fair discrimination and low difficulty. The evaluation of the OSCE assessment design is fine, and the feedback from examiners of the examination is needed. The assessment effect is good and can promote competency; since students have a low evaluation of self-performance, it is necessary to provide more facilities and resources to support practical training in the future.

Abstract:The objective structured clinical examination (OSCE) is effective for the evaluation of clinical competence. Studies examining the use of OSCEs in undergraduate mental health nursing education in the United States are limited. A pilot study and a follow-up study were conducted to establish the reliability and validity of a mental health OSCE to evaluate the clinical competence of prelicensure nursing students. International Nursing Association for Clinical Simulation and Learning Standards of Best Practice were used to guide the design and implementation. Results from both studies provide evidence for the use of OSCE in undergraduate mental health nursing education.

Abstract:Objective To learn the students ' evaluation of objective structured clinical examination (OSCE), and to supply data for improving OSCE and monitoring the quality of teaching. Methods To conduct a questionnaire survey among 196 eight-year program medical students, who were enrolled into Peking University in 2008. Results The overall evaluation of OSCE, quality of station question, examination efficiency, and self-evaluation of students are all fine. Conclusions OSCE could examine students' clinical competence objectively and truly, and students show positive attitude towards OSCE. However, OSCE examiners should get more training, the difficulty of some questions need to adjust or extend the examination duration time, the proportion of scores about medical decision need to increase, and the clinical training ought to be strengthened.

Abstract:Objective To investigate the effectiveness and rationality of objective structured clinical examination (OSCE) used as achievement assessment for Postgraduate Year 1 surgery residents.Then the form and content of training and examination can be improved according to test results.Methods 31 Postgraduate Year 1 surgery residents were involved in the examination.The examination was composed of 5 stations,which were case analysis,wound dressing change,plaster fixation,laparoscopic simulator basic skills and irregular wound debridement suture.Each station was evaluated by centesimal system score and limited in 15 minutes.A questionnaire on their opinion and perception of the examination was given to each resident at the end of examination.A total number of 31 questionnaires were issued and recycled.The difficulty,discrimination and reliability of each station were calculated,and the correlation between each station was analyzed.Results The average OSCE score was (74.66 ± 4.39).The difficulty of total stations was 0.747.The distinction was 0.578 and the reliability was 0.402.The second station was lowest difficulty,and the fifth station was most difficult.Every station's discrimination was larger than 0.400.The reliability of the second and the fourth station was low.There was moderate correlation between the first station and the second station,as well as the first station and the fourth station.But there was no significant correlation between other stations.Residents' questionnaire survey showed that the fourth and the fifth stations were considered as the most difficult,and the second station was the simplest.They thought the fourth was most helpful for clinic.They performed best at the second station and worst at the fifth station.Conclusions OSCE can effectively evaluate the efficacy of the resident training.The residents' practical competency skills can be assessed better by examination simulating complex clinical scenario.Besides,after examination,we need to objectively evaluate the assessment efficiency,and dynamically adjust the training content and examination form according to residents' subjective opinions.

This study presents an innovative method for digitizing OSCE evaluation system for medical students and explores its efficacy compared to the traditional paper-based system, through the analysis of a User Satisfaction Survey. Methods: A cross-sectional, questionnaire-based study involving a User Satisfaction Survey to evaluate assessors' attitudes toward and acceptance of the Computerized Web-based OSCE Evaluation System (COES) was used. Fifth-year medical students at a College of Medicine were assessed clinically through their 2019 end-of-year OSCE examination by 30 examiners in five different OSCE stations. The traditional paper-based stations were converted into an online electronic version using QuestionPro software. Answers were filled in using smart tablets (iPads). QR codes were used for students' identification at each station to fully digitize the process and save time. After the completion of the exam, a User Satisfaction Survey was sent electronically to all examiners to evaluate their experiences with the new system. Results: The response rate for the survey was 100% with an internal consistency of 0.83. Almost all the examiners (29, 97%) were satisfied with the application of the COES. Further, 72% of the examiners indicated that the electronic system facilitated the evaluation of the students' skills, and 84% found using a smart device (iPad) was easier than using a paper form. All examiners expressed their preference for using the electronic system in the future. Conclusion: Users were satisfied with the utilization of the customized COES. This concept of fully digitizing the OSCE assessment process shortened the time needed for both the analysis of results and providing students with feedback. Further observational studies are needed to assess examiners' behaviors when using this methodology. Abstract:Objective Structured Clinical Examinations (OSCE) is an objective assessment approach that enables the evaluation of students' practical skills by simulating real clinical scenarios. This scale is specifically designed to evaluate the attributes and characteristics of the OSCE from the perspective of health sciences students, thereby filling a crucial gap in the existing assessment methods. This research conducted a methodological design involving 220 students from the faculties of medicine and nursing between 2022 and 2023. Data were gathered using a "Descriptive Form" and "OSCEPS." Factor analyses were performed for research and validation to ascertain the scale's factorial structure. The factorial analysis was used to test the structural validity of the scale. A structure consisting of 19 items was developed. The Cronbach's alpha coefficient was 0.943. Fit indices resulting from the study were RMSEA = 0.084; GFI = 0.839; CFI = 0.902; IFI = 0.902; RFI = 0.824; NFI = 0.843; TLI = 0.889. This study proves that the OSCEPS is a highly reliable and valid scale with strong psychometric properties. It effectively assesses health sciences students' perceptions of OSCE, thereby enhancing the credibility and usefulness of this assessment tool.

Abstract:Introduction: The Objective Structured Clinical Examination (OSCE) is used globally for formative and summative purposes. The objective of this study was to examine the impact of tablet-scoring on sources of validity evidence for an Internal Medicine residency OSCE. Methods: We compared paper-scored OSCEs from 2014 to tablet-scored OSCEs in 2015 for missing data, amount of comments, and time to pass/fail decision. We then examined in 2016 the impact on learning of showing residents their score sheets and asking them to write an action plan immediately after an OSCE. Results: Tablet-scoring significantly reduced stations with missing data from 1.8 to 0.2%, and stations without comments from 42 to 28% with an increase in word count per comment. Time to official results reduced from 3 weeks to 12 h with tablet-scoring. Residents who wrote a learning plan after reviewing their OSCE score sheets were more likely (with medium to large effect sizes) to pursue further studying and/or change their behavior (e.g. history taking or physical examination) in the clinical environment. Conclusions: OSCE tablet-scoring improved many sources of validity evidence, especially educational impact with timeliness of feedback supporting a change in behavior, a hard to achieve goal of educational interventions.

Abstract:Background: Studies have shown objective structured clinical examinations (OSCEs) to be one of the most reliable tools in assessing clinical performance; however in Pediatrics they primarily use manikins, simulators or parent actors which limits the comprehensiveness of the assessment. In our Pediatric Clerkship, medical students are evaluated using a standardized rubric during a clinical evaluation exercise (CEX) with real patients. This study assessed medical students' perceived stress levels and the educational value of the CEX compared an OSCE. We hypothesized there would be equal stress and value for students with the CEX experience compared to the OSCE. Methods: Third year students anonymously completed questionnaires after required Pediatric CEX and Internal Medicine OSCE evaluations from July 2016-June 2017. The questionnaire included questions from the Intrinsic Motivation Inventory, a validated tool used to assess feelings of stress and perceived value of an exercise. Results: A total of 147 and 145 questionnaires were completed after the CEX and OSCE. There were no differences between groups regarding levels of "nervousness" (p = 0.543) and "relaxation" (p = 0.055); students felt more "at ease" (p = 0.002) and less "pressure" (p < 0.001) during the CEX. Students perceived the CEX to be more useful and important to improve skills compared to the OSCE for the history taking, physical exam and interpersonal skills. Conclusions: Our results indicate that the CEX was associated with lower stress levels and had higher perceived value when compared to the OSCE. This study supports the usefulness of incorporating real patients into the clinical evaluation of medical students.

Abstract:Serum human growth hormone (hGH) measurements were performed during transsphenoidal microsurgery for acromegaly and in the early period afterwards. There were 14 patients of which 2 were operated on twice. In 11 patients (2 patients at reoperation) the serum hGH concentration was normalized to less than 5 micrograms/l within 3 h after completion of surgery. On five occasions only partial restoration of the hGH values was obtained and additional treatment was given in 2 patients by successful re-operation. The early post-operative hGH concentrations were consistent with the clinical evaluation of the patients and the hGH concentrations during follow-up for an average of 27 months. We conclude that early post-operative serum hGH measurement is of value for evaluation of the efficiency of the tumour removal and the need for further treatment.
