Abstract:Background To analyse the incidence and factors associated with the onset of consecutive esotropia after surgery for intermittent exotropia, and to investigate its clinical course. Methods We retrospectively reviewed the medical records of 526 patients who had undergone exotropia surgery. We evaluated the risk factors for consecutive esotropia, defined as esodeviation ≥10 prism dioptres (PD) at postoperative month 1 or later. We also evaluated the clinical course of consecutive esotropia with non-surgical and surgical management. Results Consecutive esotropia occurred in 26 (4.94%) of 526 patients. Divergence excess type of exotropia (p=0.036), amblyopia (p=0.046), bilateral lateral rectus recession (p=0.008), esodeviation of ≥20 PD at postoperative day 1 (p=0.000), younger age at diagnosis and surgery (p=0.010, p=0.003) and shorter duration from onset to surgery (p=0.039) showed significant association with consecutive esotropia. Among 26 patients, 15 became orthophoric with full-time patching ± Fresnel prism glasses. Eleven patients failed to show improvement of the esodeviation with non-surgical management; seven of these underwent surgery for sustained esotropia. Among these patients, five showed orthophoria at the final follow-up. Conclusions Divergence excess type, bilateral lateral rectus recession, amblyopia, younger age at diagnosis and surgery, shorter duration from onset to surgery and overcorrection of ≥20 PD at postoperative day 1 were predisposing factors for consecutive esotropia. In less than half the consecutive esotropia patients (11/26), esodeviation persisted with non-surgical management. Surgery for consecutive esotropia showed relatively favourable outcomes.

Abstract:Background Approximately 25 million people in the United States have limited English proficiency. Current developments in orthopaedic surgery, such as the expansion of preoperative education classes or patient-reported outcome collection in response to bundled payment models, may exacerbate language-related barriers. Currently, there are mixed findings of the associations between limited English proficiency and care processes and outcomes, warranting a cross-study synthesis to identify patterns of associations. Questions/purposes In this systematic review, we asked: Is limited English proficiency associated with (1) differences in clinical care processes, (2) differences in care processes related to patient engagement, and (3) poorer treatment outcomes in patients undergoing orthopaedic surgery in English-speaking countries? Methods On June 9, 2023, a systematic search of four databases from inception through the search date (PubMed, Ovid Embase, Web of Science, and Scopus) was performed by a medical librarian. Potentially eligible articles were observational studies that examined the association between limited English proficiency and the prespecified categories of outcomes among pediatric and adult patients undergoing orthopaedic surgery or receiving care in an orthopaedic surgery setting. We identified 10,563 records, of which we screened 6966 titles and abstracts after removing duplicates. We reviewed 56 full-text articles and included 29 peer-reviewed studies (outcome categories: eight for clinical care processes, 10 for care processes related to patient engagement, and 15 for treatment outcomes), with a total of 362,746 patients or encounters. We extracted data elements including study characteristics, definition of language exposure, specific outcomes, and study results. The quality of each study was evaluated using adapted Newcastle-Ottawa scales for cohort or cross-sectional studies. Most studies had a low (48%) or moderate (45%) risk of bias, but two cross-sectional studies had a high risk of bias. To answer our questions, we synthesized associations and no-difference findings, further stratified by adjusted versus unadjusted estimates, for each category of outcomes. No meta-analysis was performed. Results There were mixed findings regarding whether limited English proficiency is associated with differences in clinical care processes, with the strongest adjusted associations between non-English versus English as the preferred language and delayed ACL reconstruction surgery and receipt of neuraxial versus general anesthesia for other non-Spanish versus English primary language in patients undergoing THA or TKA. Limited English proficiency was also associated with increased hospitalization costs for THA or TKA but not opioid prescribing in pediatric patients undergoing surgery for fractures. For care processes related to patient engagement, limited English proficiency was consistently associated with decreased patient portal use and decreased completion of patient-reported outcome measures per adjusted estimates. The exposure was also associated with decreased virtual visit completion for other non-Spanish versus English language and decreased postoperative opioid refill requests after TKA but not differences in attendance-related outcomes. For treatment outcomes, limited English proficiency was consistently associated with increased hospital length of stay and nonhome discharge per adjusted estimates, but not hospital returns. There were mixed findings regarding associations with increased complications and worse postoperative patient-reported outcome measure scores. Conclusion Findings specifically suggest the need to remove language-based barriers for patients to engage in care, including for patient portal use and patient-reported outcome measure completion, and to identify mechanisms and solutions for increased postoperative healthcare use. However, interpretations are limited by the heterogeneity of study parameters, including the language exposure. Future research should include more-precise and transparent definitions of limited English proficiency and contextual details on available language-based resources to support quantitative syntheses. Language-Based Disparities in Route of Hysterectomy for Benign Disease

Abstract:Study objective: To assess the association between patient primary language and route of hysterectomy. Design: A retrospective cohort study was conducted using the Healthcare Cost and Utilization Project's State Inpatient Database (SID) and State Ambulatory Surgery and Services Database (SASD). Setting: All inpatient and outpatient hysterectomies from the most recent year of available data (2020 - 2021) from the six states that record patient primary language in the SID and SASD (Indiana, Iowa, Maryland, Michigan, Minnesota, and New Jersey) were queried. Patients or participants: Patients aged 18 and over undergoing an inpatient or ambulatory hysterectomy for benign indication. Interventions: Minimally invasive hysterectomy compared to abdominal hysterectomy. Measurement and main results: The association between patient primary language (English vs non-English) and route of hysterectomy (abdominal vs minimally invasive) was evaluated. The cohort included 52,226 patients who met inclusion criteria. The majority of patients were non-Hispanic White (71%), with a median age of 46 years (IQR 40.0-53.0). 91.4% of patients spoke English as their primary language, 3.6% spoke Spanish, and 5.0% spoke another non-English language. Patients with a non-English primary language were significantly less likely to undergo minimally invasive hysterectomy compared to patients who spoke English (OR 0.60, 95% CI 0.56-0.64, p<0.001). This association remained significant following adjustments for age, race, insurance, median income, state, and fibroid, abnormal uterine bleeding, prolapse or endometriosis diagnosis (aOR 0.77, 95% CI 0.71-0.84). In a sensitivity analysis of English vs Spanish vs other non-English language, the association remained significant for other non-English languages (aOR 0.67, 95% CI 0.60-0.75) but not for Spanish (aOR 0.95, 95% CI 0.83-1.09). Conclusion: Patients who are non-English speaking are significantly less likely to receive a minimally invasive hysterectomy. Addressing language disparities may improve access to a minimally invasive route of surgery, a possible surrogate for improved surgical outcomes, for our gynecologic patients.

Abstract:Objective Sociodemographic factors such as race/ethnicity and socioeconomic status affect primary cesarean delivery rates. Language is associated with disparate health care outcomes but has not been well studied in obstetrics. We examined the association between primary unscheduled cesarean delivery rate and preferred patient language. Study Design A retrospective cohort study was conducted at an urban medical center between January 2017 and January 2020. Nulliparous women with early or full-term gestation and having no obstetric or medical contraindication to vaginal delivery were included. We used multivariable linear and logistic regressions to evaluate language differences in cesarean delivery odds and indication for cesarean. Results Of the 1,314 eligible women, 76.8% of patients preferred English, 17.8% Spanish, and 5.4% other languages. Overall cesarean delivery rate was 27.6%. Controlling for age, race/ethnicity, body mass index, insurance, gravidity, pregnancy comorbidities, labor induction, and infant birth weight, Spanish- and other language-speaking women had significantly higher odds of undergoing cesarean compared with English-speaking women (adjusted odds ratio [OR]: 1.75; 95% confidence interval [CI]: 1.25, 2.46). Relative proportions of indications for cesarean did not differ by language group. Documented interpreter use was an effect modifier on the relationship between language preference and cesarean (OR with interpreter use: 2.89, 95% CI: 1.90, 4.39). Conclusion Primary cesarean delivery rates were significantly higher among nulliparous women who prefer languages other than English. This difference may reflect lack of communication, provider bias or discrimination, or other factors, and should be further studied. Interpreter services should be routinely utilized and documented effectively. Key Points Women who prefer languages other than English had higher odds of cesarean. Indication for cesarean did not differ by language. Interpreter use did not reduce risk for cesarean. Abstract:Objective In 2019, 22% of adults in the United States reported speaking a language other than English at home, representing 52% growth since 2000. This diversity in languages – and resulting possible communication barriers – represents a potential challenge to effective care. In this manuscript, we summarize clinical outcomes and healthcare utilization patterns of adult and pediatric neurosurgical patients who are non-English primary language speakers (NEPLS). Methods We systematically queried five databases from inception through October 2022. The Preferred Reporting Items for Systematic Reviews and Meta-Analyses guidelines were followed to identify studies for inclusion. The Newcastle-Ottawa Scale was used to assess the quality of studies. Additionally, a retrospective chart review was conducted to assess differences in postoperative communication patterns in a cohort of English and Spanish speaking patients with craniosynostosis at our institution. Results Our search yielded 442 abstracts; ten were included in the final cohort. Outcomes for 973 unique NEPLS with a neurosurgical condition were included; Spanish was the most represented language. Delivery and timing of surgical treatment was the most frequently reported metric; 75% of studies demonstrated a statistically significant delay in time to surgery or decreased likelihood for NEPLS to receive surgical treatment. Length of stay (LOS) was reported in three studies; all demonstrated that NEPLS had longer LOS. Conclusion There is a paucity of literature reporting outcomes among NEPLS. It is critical to examine NEPLS patients' outcomes and experiences, as language barriers are potentially modifiable demographic factors. We present a framework that demonstrates opportunities for further research to improve quality of care.

Abstract:BackgroundIn home health care, language barriers are understudied. Language barriers between patients and providers are known to affect a variety of patient outcomes. How a patient's language preference influences hospital readmission risk from home health care has yet to be determined.ObjectiveTo determine if home care patients' language preference is associated with their risk for hospital readmission from home health care within 30 days of hospital discharge.DesignRetrospective cross-sectional study of hospital readmissions from an urban home health care agency's administrative records and the national electronic home health care record for the United States, captured between 2010 and 2015.SettingNew York City, New York, USA.ParticipantsThe dataset comprised 90,221 post-hospitalization patients and 6.5 million home health care visits.MethodsFirst, a Chi-square test was used to determine if there were significant differences in crude readmission rates based on language group. Inverse probability of treatment weighting was used to adjust for significant differences in known hospital readmission risk factors between to examine all-cause hospital readmission during a home health care stay. The final matched sample included 87,561 patients with a language preference of English, Spanish, Russian, Chinese, or Korean. English-speaking patients were considered the comparison group to the non-English speaking patients. A Marginal Structural Model was applied to estimate the impact of non-English language preference against English language preference on rehospitalization. The results of the marginal structural model were expressed as an odds ratio of likelihood of readmission to the hospital from home health care.ResultsHome health patients with a non-English language preference had a higher hospital readmission risk than English-speaking patients. Crude readmission rate for the limited English proficiency patients was 20.4% (95% CI, 19.9% - 21.0%) overall compared to 18.5% (95% CI, 18.7% - 19.2%) for English speakers (p < 0.001). Being a non-English-speaking patient was associated with an odds ratio of 1.011 (95% CI, 1.004 – 1.018) in increased hospital readmission rates from home health care (p = 0.001). There were also statistically significant differences in readmission rate by language group (p<0.001), with Korean speakers having the lowest rate and Spanish speakers having the highest, when compared to English speakers.ConclusionsPeople with a non-English language preference have a higher readmission rate from home health care. Abstract:BACKGROUND/OBJECTIVESHispanics have lower advance directive (AD) completion than non‐Hispanic Whites. Few studies have assessed the role of language preference in end‐of‐life planning. We investigated whether language preference and needing an interpreter affected AD completion among older adults in an integrated health system. DESIGNRetrospective cohort investigation of electronic medical records. SETTINGNorthern California integrated health system. PARTICIPANTSA total of 620,948 Hispanic and non‐Hispanic White patients, aged 55 years and older, between January 1, 2013, and December 31, 2017. MEASUREMENTSDescriptive statistics and bivariate analysis were performed to compare AD completion among non‐Hispanic Whites, Hispanics, and Hispanic subgroups by language preference (English speaking, Spanish speaking, and needed interpreter). We conducted multivariable logistic regression to determine the relationship between language preference and having an AD while controlling for demographic, clinical, and utilization factors. RESULTSWe found 20.3% of non‐Hispanic Whites (n = 512,577) and 10.9% of Hispanics (n = 108,371) had completed an AD. Among Hispanics, after controlling for demographic, clinical, and utilization factors, compared with Spanish speakers requiring an interpreter, English speakers had nearly two‐fold increased odds of completing an AD (adjusted odds ratio (aOR) = 2.6; 95% confidence interval (CI) = 2.4–2.9), whereas Spanish speakers not requiring an interpreter had 20% increased odds (aOR = 1.2; 95% CI = 1.1–1.3). Additional predictors of successful AD completion were being female, being older, having more comorbidities, having more hospital and emergency department visits, and having higher socioeconomic status. There were no differences associated with primary care provider characteristics. CONCLUSIONThese findings indicate the need for a tailored outreach to Hispanics, particularly among those subgroups who require the need of an interpreter, to reduce AD completion disparities.

Abstract:Ectopic adrenocorticotropin syndrome (EAS) is classically associated with neuroendocrine tumours and small-cell lung cancer (SCLC) but may complicate many more commonmalignancies. Three cases of EAS secondary to cancers not typically associated with this condition are presented. Case 1: A 69-year-old woman with metastatic breast cancer presented with symptomatic hyperglycaemia, cardiac failure and hypokalaemia. Phenotypic features of Cushing syndrome (CS) were absent. Late night salivary cortisol, 24-h urine-free cortisol (24-hUFC) and serum adrenocorticotropin (ACTH) were elevated, consistent with EAS. Spironolactone and metyrapone therapy resulted in the resolution of hypokalaemia and cardiac failure, reduction in insulin requirements and normalisation of urine cortisol levels. Case 2: A 63-year-old previously normotensive woman presented with a painful hypogastric mass, weight loss, severe hypertension and hypokalaemic alkalosis. Further investigation revealed metastatic endometrial adenocarcinoma. Severe hypertension persisted despite four antihypertensive agents. Midnight serum cortisol, 24-hUFC and ACTH were elevated, consistent with EAS. Hypertension and hypokalaemia resolved with spironolactone monotherapy. The patient declined chemotherapy or bilateral adrenalectomy, and ketoconazole caused hepatotoxicity. The patient died 3 months after presentation. Case 3: Six months following surgery for parotid acinic cell carcinoma, a 52-year-old woman presented with myopathy, severe hypertension and hypokalaemic alkalosis. Imaging demonstrated widespread metastatic disease. EAS was confirmed by elevated midnight serum cortisol, 24-hUFC and serum ACTH. The original tumour stained positive for ACTH. Hypertension resolved with perindopril and spironolactone, and urinary cortisol levels fell with ketoconazole therapy. The patient declined bilateral adrenalectomy, proceeding to palliative chemotherapy. Neutropenic sepsis ensued, and the patient died 3 weeks after presentation. EAS may complicate an extensive variety of neoplasms (Table 1). EAS may be underdiagnosed in patients with cancer. Although only 10 cases of EAS have been reported with breast cancer, ACTH peptides were detected in 16.7 % of 150 cases of primary breast cancer. EAS may present with signs of predominant mineralocorticoid excess due to the inability of 11-betahydroxysteroid dehydrogenase-2 to inactivate cortisol at high concentrations; cortisol then acts on the mineralocorticoid receptor. Patients may therefore present with treatment-resistant hypertension and hypokalaemic alkalosis without developing a Cushingoid phenotype. Early diagnosis of EAS is important because of the significant morbidity and mortality associated with the syndrome due to sepsis, thromboembolism, cardiac failure and gastrointestinal haemorrhage. In patients with SCLC and neuroendocrine tumours, the presence of EAS is associated with shortened survival. The optimal treatment of EAS is surgical resection of the primary tumour or successful cytotoxic treatment. Medical therapies for hypercortisolaemia may have limited efficacy and significant toxicity. Laparoscopic bilateral adrenalectomy is the most effective and expedient treatment for hypercortisolaemia in EAS where cure of the primary tumour is not possible. Recently successful percutaneous computed tomography-guided microwave ablation of the adrenal gland has been described. In conclusion, EAS may complicate many neoplasms. The diagnosis should be considered in any patient with a known or suspected cancer presenting with hypokalaemia, resistant hypertension, hyperglycaemia or myopathy irrespective ofwhether phenotypic features of CS are present.
Abstract 16079: Outcomes Among Heart Failure Patients With Non English Language Preference

Abstract:Introduction: Non-English Language Preference(NELP) has been associated with inequitable care with prior studies highlighting increased risk of readmissions and lower patient satisfaction with care. However, outcomes of patients with heart failure with reduced ejection fraction (HFrEF) who identify as NELP in the United States have not been previously reported. Aims: To evaluate clinical outcomes of HFrEF patients with NELP compared to English-speaking patients. Methods: We performed a single center propensity score matched retrospective cohort study of HFrEF patients who received care between 2010-2022. We compared 5-year mortality, guideline directed medical therapy (GDMT) utilization, length of stay, and readmissions between NELP and English-speaking HFrEF patients. In secondary analyses, we divided NELP by major language group (Spanish, Russian, Other). Results: Our cohort consisted of 2136 English-speaking patients (mean age 82 years, 42% female) and 961 NELP patients (mean age 83 years, 41% female). NELP HFrEF patients had higher rates of angiotensin converting enzyme inhibitor/angiotensin receptor blocker and sodium glucose cotransporter 2 inhibitor use compared to English speakers (Table). Unadjusted 5-year mortality among NELP patients was lower compared to English speakers with a hazard ratio of 0.8 (95% CI 0.68-0.94), but this difference was attenuated after adjusting for race.NELP patients were more likely to have ED visits (69% vs. 65%) but less likely to have inpatient admissions (82% vs 85%) (both p<0.05). In our secondary analysis, Spanish speakers were more likely to be on more GDMT classes compared to English speakers while Russian speakers had increased risk of readmissions and longer length of stay compared to English speakers. Conclusions: NELP patients are heterogeneous with multiple group differences in GDMT use and admissions.Further research is needed to provide culturally-competent, equitable care for HFrEF patients.

Abstract:Background/Introduction Patients with limited English proficiency (LEP) constitute 8.6% of the United States population. They may have lower health literacy related to cardiovascular disease, reluctance to seek care, and communication difficulties in reporting symptoms, posing challenges to care provision. Throughout medical literature, these barriers reportedly lead to increased utilisation of diagnostic testing, hospital admission rate, and length of stay. Purpose Language barrier appears to negatively impact individuals' healthcare experience. The goal of this study is to examine whether disparities exist in the emergency department (ED) evaluation and management of chest pain in LEP patients by comparing their wait time and admission rate to that of English-proficient (EP) counterparts. Methods This retrospective cohort study included adults age 18 or older that presented to our hospital ED with a chief complaint of chest pain from 1/9/2022 to 30/9/2023. We identified all LEP patients during this period and matched a random sample of EP patients in a 1:2 ratio. We treated each encounter as an independent subject and eliminated those that left prior to disposition. Our primary outcomes were time from arrival to provider evaluation, and admission rate. Covariates were sex, age, race, acuity level, arrival mode, and insurance status to adjust for characteristic difference between cohorts. Descriptive comparisons between cohorts and bivariate outcomes were conducted using independent samples t-tests for continuous variables and chi-squared tests for categorical ones. Multivariable linear and logistic regression models were used to compute adjusted mean wait time for provider and adjusted odds of admission, respectively. All analyses were done on SAS 9.4. Results Our study sample included 690 encounters, with 239 LEP and 451 EP patients. The two cohorts were similar in sex, age, and acuity level, but there were significant disparities in insurance status, arrival mode, and race. A higher proportion of EP patients were admitted to the hospital or observational unit than LEP patients (45.0% vs. 28.0%, respectively, P<0.001). After multivariable adjustment, LEP patients were half as likely to be admitted compared to EP patients (adjusted odds ratio, 0.51 [95% CI, 0.35-0.74], P<0.001). LEP patients waited longer for provider evaluation from the point of ED arrival, though this was not statistically significant when adjusting for acuity and arrival mode (19.6 minutes, [95% CI, 5.9-45.1], P=0.132). Conclusions Whilst we did not observe a significant difference in wait time for provider between LEP and EP patients, LEP patients were less likely to be admitted when presenting with chest pain at the ED. One possibility is that language barrier may limit a patient's ability to convey cardiac versus non-cardiac chest pain, adversely influencing downstream management and outcomes. To address this, further large-scale investigation is needed.

Abstract:Background: Language preference is a contributing factor for prolonged time from symptom onset to appendectomy within pediatrics, but is poorly characterized in adults. We aimed to investigate associations between language barriers and delays in assessment and treatment for adults with acute appendicitis. Methods: In a multiethnic community, patients aged ≥18 years old who underwent appendectomy were identified between January 2017 and August 2022 at a single institution. Negative binomial regression was used to compare interval wait times to imaging, medication administration, and surgical evaluation between patients with limited English proficiency and those who are English proficient. Results: Of the 1469 patients included, 48% (n = 699) were with limited English proficiency. Average age was higher for patients with limited English proficiency (45 vs 36, P < .001). Most of them were Asian (54%) and without private insurance (65%, P < .001). Symptom duration, incidence of septic shock, and date/time of presentation to the emergency department were similar. Patients with limited English proficiency presented more frequently with gangrenous appendicitis (20% vs 15%, P = .013) but not perforated (23% vs 20%, P = .065). They experienced longer wait times for surgical evaluation (376 vs 348 min, incidence rate ratio [IRR], 1.08; P = .002) but similar times for imaging, and medications administered. After controlling for demographics, triage acuity, and hospital factors, significantly longer wait times for surgical evaluation persisted (IRR adjusted, 1.07; P = .038). There was no significant difference in hospital length-of-stay, postoperative infection, or 30-day readmission rate. Conclusion: Adult patients with limited English proficiency may experience longer wait times for surgical evaluation for acute appendicitis, but this may not result in clinically significant delays in the initiation of treatment.

Abstract:Protein kinase C (PKC) plays an integral part in T cell activation and IL-2 secretion. We investigated the role of a particular PKC isoform, PKCβ, in IL-2 production and secretion. The T cell lymphoma line HuT 78 secretes IL-2 in response to the phorbol ester PMA. A PKCβ-deficient clone of HuT 78, K-4, did not secrete IL-2 in response to PMA stimulation. As assessed by RT-PCR, K-4 expressed mRNA for IL-2 following PMA activation, and intracellular IL-2 protein was detected by immunofluorescence. An enhanced green fluorescent protein-linked PKCβ construct was microinjected into K-4 cells, which were then stimulated with PMA; those cells that expressed PKCβ could secrete IL-2, as determined by an in situ immunofluorescent assay. This study demonstrates that PKCβ is not necessary for transcription of the IL-2 gene or translation of mRNA to protein, but that expression of this PKC isoform is critical to the export of IL-2 molecules from T cells.

Abstract:Herein, we present theoretical results aimed at elucidating the origin of the kinetic preference for penicillins over cephalosporins characteristic of the TEM/SHV subgroup of class A beta-lactamases. First, we study the conformational properties of cephalothin showing that the C2-down conformer of the dihydrothiazine ring is preferred over the C2-up one by approximately 2 kcal/mol in solution (0.4-1.4 kcal/mol in the gas phase). Second, the TEM-1 beta-lactamase complexed with cephalothin is investigated by carrying out a molecular dynamics simulation. The DeltaG(binding) energy is then estimated using molecular mechanics Poisson-Boltzmann surface area (MM-PBSA) and quantum chemical PBSA (QM-PBSA) computational schemes. The preferential binding of benzylpenicillin over cephalothin is reproduced by the different energetic calculations, which predict relative DeltaDeltaG(binding) energies ranging from 1.8 to 5.7 kcal/mol. The benzylpenicillin/cephalothin DeltaDeltaG(binding) energy is most likely due to the lower efficacy of cephalosporins than that of penicillins in order to simultaneously bind the "carboxylate pocket" and the "oxyanion hole" in the TEM-1 active site.
Analyzing Linguistic Disparities in Telehealth Care Outcomes at a Multidisciplinary Craniofacial Center

Abstract:Objective: To examine linguistic disparities between English- and Spanish-speaking patients in access to care, satisfaction, and telehealth appointment attendance. Design: Retrospective study recording demographics for non-attendance analysis and conducting phone surveys assessing satisfaction with telehealth. Setting: Data was collected between March and December 2020 at the UCSF Craniofacial Center (CFC), a multidisciplinary pediatric clinic. Patients: English- and Spanish-speaking patients with a telehealth appointment. Interventions: The CFC offered language-concordant outreach, assistance with the telehealth platform, and interpreters at all telehealth appointments. Main outcome measures: Demographics and patient-reported satisfaction with telehealth, barriers, and instruction clarity. Results: Medicaid insurance was the only predictor of non-attendance. Surveys revealed that Spanish-speakers had 12.4 times the odds of lacking access to telehealth technology and 10.7 times the odds of needing help with logging on compared to English-speakers. There were no significant differences in satisfaction outcomes. Conclusions: We attribute this equity in satisfaction to our language-concordant outreach efforts.
Prenatal Care Clinician Preferences Among Patients With Spanish-Preferred Language

Abstract:Objective: To measure what patients with Spanish language preference and limited English proficiency value most when selecting a prenatal care clinician. Methods: A discrete choice experiment was administered at two large academic medical centers in Boston, Massachusetts. Participants were identified by electronic medical record, had preferred Spanish language and self-identified limited English proficiency, and either were pregnant with a completed fetal anatomy scan or had given birth within the past 12 months at the time of the study. The discrete choice experiment consisted of eight attributes to consider when selecting a prenatal care clinician: clinician Spanish proficiency, type of interpreter used, interpersonal dynamics (ie, making patients feel seen, heard, and cared for), cultural concordance, continuity of care, shared decision making, distance from home, and wait times for appointments. Descriptive statistics of demographic variables were calculated. Hierarchical Bayesian models were used to analyze discrete choice experiment data. Results: The attributes that were most important to 166 participants when choosing their prenatal care clinician were interpersonal dynamics within the patient-clinician dyad and clinician Spanish language proficiency (average importance 21.4/100 and 20.8/100, respectively). Of lowest importance were wait time to receive an appointment and continuity of care (average importance 5.1 and 6.1, respectively). Although participants highly preferred that their clinician be "fluent or near fluent in Spanish," the second most preferred option was having a clinician with only basic Spanish proficiency rather than a more advanced level of Spanish proficiency with some misunderstandings. Conclusion: Our study demonstrates the importance of positive clinician-patient interpersonal dynamics and language-concordant care for patients with Spanish language preference in prenatal care. Promoting equitable patient-centered care for patients with limited English proficiency requires responding to preferences regarding clinician language proficiency and demonstrating attentiveness, empathy, and concern for prenatal care experiences.
Increased Access to Professional Interpreters in the Hospital Improves Informed Consent for Patients with Limited English Proficiency

Abstract:Background: Language barriers disrupt communication and impede informed consent for patients with limited English proficiency (LEP) undergoing healthcare procedures. Effective interventions for this disparity remain unclear. Objective: Assess the impact of a bedside interpreter phone system intervention on informed consent for patients with LEP and compare outcomes to those of English speakers. Design: Prospective, pre-post intervention implementation study using propensity analysis. Subjects: Hospitalized patients undergoing invasive procedures on the cardiovascular, general surgery or orthopedic surgery floors. Intervention: Installation of dual-handset interpreter phones at every bedside enabling 24-h immediate access to professional interpreters. Main measures: Primary predictor: pre- vs. post-implementation group; secondary predictor: post-implementation patients with LEP vs. English speakers. Primary outcomes: three central informed consent elements, patient-reported understanding of the (1) reasons for and (2) risks of the procedure and (3) having had all questions answered. We considered consent adequately informed when all three elements were met. Key results: We enrolled 152 Chinese- and Spanish-speaking patients with LEP (84 pre- and 68 post-implementation) and 86 English speakers. Post-implementation (vs. pre-implementation) patients with LEP were more likely to meet criteria for adequately informed consent (54% vs. 29%, p = 0.001) and, after propensity score adjustment, had significantly higher odds of adequately informed consent (AOR 2.56; 95% CI, 1.15-5.72) as well as of each consent element individually. However, compared to post-implementation English speakers, post-implementation patients with LEP had significantly lower adjusted odds of adequately informed consent (AOR, 0.38; 95% CI, 0.16-0.91). Conclusions: A bedside interpreter phone system intervention to increase rapid access to professional interpreters was associated with improvements in patient-reported informed consent and should be considered by hospitals seeking to improve care for patients with LEP; however, these improvements did not eliminate the language-based disparity. Additional clinician educational interventions and more language-concordant care may be necessary for informed consent to equal that for English speakers.
Association of Language Barriers With Process Outcomes After Craniotomy for Brain Tumor

Abstract:BACKGROUND: Little is known about the independent association of language barriers on postoperative process outcomes after craniotomies.OBJECTIVE: To evaluate the association of limited English proficiency (LEP) with length of stay (LOS), discharge disposition, hospitalization costs, and rate of 30-day readmission after craniotomy for brain tumor.METHODS: This is a retrospective cohort study of adult patients who underwent craniotomies for brain tumor from 2015 to 2019 at a high-volume neurosurgical center. Multivariable logistic regression was used to evaluate the association of LEP with discharge disposition and 30-day readmission. Negative binomial regression was used to evaluate the association of LEP with LOS and hospitalization cost.RESULTS: Of the 2232 patients included, 7% had LEP. LEP patients had longer LOS (median [IQR] 5 [3-8] days vs 3 [2-5] days, P < .001), higher costs of hospitalization (median [IQR] $27 000 [$21 000-$36 000] vs $23 000 [$19 000-$30 000], P < .001), and were more likely to be discharged to skilled care facilities (37% vs 21%, P < .001) compared with English proficient patients. In multivariable models, the association between LEP and longer LOS (incidence rate ratio 1.11, 95% CI 1.00-1.24), higher hospitalization costs (incidence rate ratio 1.13, 95% CI 1.05-1.20), and discharge to skilled care (OR 1.76, 95% CI 1.13-2.72) remained after adjusting for confounders. There was no difference in 30-day readmission rates by language status.CONCLUSION: LEP is an independent risk factor for extended LOS, higher hospitalization cost, and discharge to skilled care in neurosurgical patients who undergo craniotomy for brain tumor. Future research should seek to understand mediators of these observed disparities.

Abstract:Objectives Previous studies have demonstrated a relationship between socioeconomic disparities and missed clinic visits (MCV). However, the relationship between patient‐preferred language and MCVs, particularly with respect to telemedicine, remains relatively underexplored. We sought to characterize the associations between MCV and patient‐level predictors, including preferred language, in a large single‐center pediatric gastroenterology, hepatology, and nutrition practice. Methods This retrospective longitudinal cohort study included all missed or completed outpatient visits in the Gastroenterology, Hepatology, and Nutrition Programs at Boston Children's Hospital from January 1, 2016 to May 20, 2022. Univariate and multivariate hierarchical generalized linear mixed models were employed to identify associations between visit‐ and patient‐level predictors and an MCV outcome. Results A total of 300,201 visits from 70,710 patients residing in Massachusetts were included. Univariate analyses revealed higher MCV odds for Hispanic patients and those from areas with the highest Social Vulnerability Index (SVI), and these odds increased with telemedicine (Hispanic in‐person odds ratio [OR] 5.21 [(95% confidence interval) 4.93–5.52] vs. telemedicine OR 8.79 [7.85–9.83]; highest SVI in‐person OR 5.28 [4.95–5.64] vs. telemedicine OR 7.82 [6.84–8.96]). Controlled multivariate analyses revealed that among six language groups, only Spanish language preference was associated with higher MCV odds, which increased with telemedicine (Spanish in‐person adjusted OR [aOR] 1.35 [1.24–1.48] vs. telemedicine aOR 2.1 [1.83–2.44]). Conclusions Patients preferring Spanish experience unique barriers to care beyond those faced by other language preference groups, and telemedicine may exacerbate these barriers.
