Abstract:This study evaluated the stress distribution in five different class II cavities of premolar models restored with conventional or bulk-fill flowable composite by means of finite element analysis (FEA) under shrinkage and occlusal loading. An upper validated premolar model was imported in the software, and five class II cavities with different occlusal extensions and dimensions were prepared: horizontal cavity on the mesial surface (horizontal slot), mesio-occlusal cavity, mesial cavity (vertical slot), tunnel type cavity and direct access cavity. The models were restored with conventional or bulk-fill flowable resin composite. The tested materials were considered as homogeneous, linear, and isotropic. The Maximum Principal Stress criteria was chosen to evaluate the tensile stress results. The lowest shrinkage stress value was observed in the direct access cavity restored with bulk-fill flowable resin composite (36.12 MPa). The same cavity, restored with conventional composite showed a score of 36.14 MPa. The horizontal slot cavity with bulk-fill flowable showed a score of 46.71 MPa. The mesio-occlusal cavity with bulk-fill flowable had a score of 53.10 MPa, while with conventional composite this was 55.35 MPa. Higher shrinkage stress was found in the vertical slot cavity with conventional resin 56.14 MPa, followed by the same cavity with bulk-fill flowable 56.08 MPa. Results indicated that the use of bulk-fill flowable composite resin more significantly decreased the polymerization shrinkage stress magnitude. The larger the cavity and the volume of material necessary to restore the tooth, the greater the residual stress on enamel and dentin tissue.

Fracture Resistance in Fibre-Reinforced Resin Composite Restorations in Deciduous and Permanent Molars: An Ex Vivo Study

Abstract:Aim: The aim of this study was to compare the fracture resistance levels of restored deciduous teeth and permanent molars restored with different materials, including ultra-polyethylene fibre tape (Ribbond-Ultra), fibre-reinforced resin composite EverX posterior, fibre-reinforced flowable resin composite EverX Flow and bulk-fill flow restorative material (Tetric N-flow) in the posterior region. Methods: We tested sixty-four caries-free human mandibular molars (32 deciduous molars and 32 permanent molars). Deep and wide Class I cavities were prepared in each tooth. The teeth were embedded in poly(methyl methacrylate) resin up to the cementoenamel junction, leaving the crown completely exposed. Oral temperature fluctuations were then simulated using two thousand thermocycling cycles, each lasting 30 seconds in the hot phase (approximately 35 °C). The loading rate for our force-fracture tests was set at 0.5 mm/min. Direct restorations were made using a spherical tip and applying the force perpendicular to the sample surface. Visual inspection of the fractured specimens, in combination with adhesive scanning electron microscopy (SEM) and finite element analysis (FEA), provided detailed insights into the failure modes and stress distribution at the restoration-tooth interface. Results: Teeth restored with fibre-reinforced composite (EverX Posterior) had the highest fracture resistance, followed by fibre-reinforced flowable resin composite (EverX Flow). Teeth restored with the ultra- polyethylene fibre tape (Ribbond-Ultra), followed by the flowable bulk-fill composite (Tetric N-flow) had the lowest resistance. Statistical analysis revealed significant differences between the groups, except for EverX Posterior and EverX Flow. A predictive algorithm was also proposed for the likelihood of restoration failure. Conclusion: Modern fibre-reinforced resin composites, such as EverX Posterior, effectively reinforce teeth against fractures, with success in both restored deciduous and permanent molars. Meanwhile, the use of polyethylene fibre tapes is less effective, and they involve a time-consuming application.
Effect of preparation design on fracture strength of compromised molars restored with direct composite resin restorations: An in vitro and finite element analysis study

Abstract:Statement of problem: More data are needed on the influence of preparation design on the fracture strength, failure type, repairability, and polymerization-induced cracks of molar teeth restored with direct composite resin restorations. Purpose: This in vitro and finite element analysis study investigated the effect of different preparation designs on fracture strength, failure type, repairability, tooth deformation, and the formation of polymerization-induced cracks of compromised molars restored with direct composite resin restorations. Material and methods: Human molars (n=64) were randomly assigned to 4 different preparation designs: undermined inlay (UI), extended inlay (EI), restricted overlay (RO), and extended overlay (EO). The teeth were restored using direct composite resin and subjected to artificial thermomechanical aging in a mastication simulator, followed by load-to-failure testing. Three-dimensional (3D) finite element analysis was conducted to assess tooth deformation. Polymerization-induced cracks were evaluated using optical microscopy and transillumination. The fracture strength data were analyzed using a Kruskal-Wallis test, while the failure mode, repairability, and polymerization cracks were analyzed using the Fisher exact test (α=.05). Results: All specimens withstood thermomechanical aging, and no statistically significant difference in fracture strength was observed among the 4 preparation designs (P>.05). The finite element analysis showed differences in tooth deformation, but no correlation was observed with in vitro fracture resistance. The RO and EO groups presented significantly more destructive failures compared with the UI and EI groups (P<.01). The RO group had significantly fewer repairable failures than the UI and EI groups (P=.024). A correlation was found between higher frequencies of repairability and higher tooth deformation. A significant correlation between the increase in microfractures and preparation design was observed (P<.01), with the UI group exhibiting a higher increase in microfracture size compared with the EO group (P<.05). Conclusions: No influence of preparation design on the fracture strength of compromised molars restored with direct composite resin restorations was evident in this study, but the failure mode of cusp coverage restorations was more destructive and often less repairable. The finite element analysis showed more tooth deformation in inlay preparations, with lower stresses within the root, leading to more reparable fractures. Since cusp coverage direct composite resin restorations fractured in a more destructive manner, this study suggests that even a tooth with undermined cusps should be restored without cusp coverage.
Fragment reattachment or direct restoration? An in vitro study

Abstract:Background/Aims The fracture resistance of teeth subjected to fragment reattachment may improve when the technique is performed using intermediate materials. The aim of this study was to evaluate the influence of different bonding materials, including pre‐heated composite resin, on the fracture resistance of teeth subjected to the reattachment technique compared with a direct composite resin restoration. Materials and methods After simulating a tooth fracture, each specimen was reattached by using one of the following intermediate materials (n=15): G0, negative control (sound teeth); G1, Z100; G2, pre‐heated Z100; G3, Filtek Z350 XT; G4, pre‐heated Filtek Z350 XT; G5, Filtek Z350 XT Flow; and G6, RelyX Veneer. In G7, direct class IV restorations were performed using Filtek Z350 XT composite resin. The fracture resistance was evaluated using a universal testing machine under a compressive load of 1 mm/min. One‐way analysis of variance (ANOVA) and the post hoc Tukey test (5%) were conducted. Results G0 showed the highest fracture resistance, and this was statistically different from the other experimental groups (p<0.05) except for G2 and G4, in which the fragments were bonded using pre‐heated resins. A statistically significant difference was found between groups G1 and G6 (one‐way ANOVA, p=0.04). For G1 and G2, a significant difference was found between bonding with or without pre‐heating (p 0.05). No statistically significant difference was found between the reattached and directly restored groups (t‐test, p=0.53). Conclusion The tooth reattachment technique using pre‐heated composite resin showed fracture resistance values similar to those of the sound tooth group. No difference was found between the fragment reattachment and direct composite resin techniques.

Abstract:OBJECTIVE: The aim was to evaluate the effect of short glass-fiber/filler particles proportion on fracture toughness (FT) and flexural strength (FS) of an experimental flowable fiber-reinforced composite (Exp-SFRC) with two methacrylate resin formulations. In addition, we wanted to investigate how the fracture-behavior of composite restorations affected by FT values of SFRC-substructure.METHODS: Exp-SFRC was prepared by mixing 50wt% of dimethacrylate based resin matrix (bisGMA or UDMA based) to 50wt% of various weight fractions of glass-fiber/particulate filler (0:50, 10:40, 20:30, 30:20, 40:10, 50:0wt%, respectively). FT and FS were determined for each experimental material following standards. Specimens (n=8) were dry stored (37°C for 2 days) before they were tested. Four groups of posterior composite crowns (n=6) composed of different Exp-SFRCs as substructure and surface layer of commercial particulate filler composite were fabricated. Crowns were statically loaded until fracture. Failure modes were visually examined. The results were statistically analysed using ANOVA followed by post hoc Tukey's test.RESULTS: ANOVA revealed that ratio of glass-fiber/particulate filler had significant effect (p<0.05) on tested mechanical properties of the Exp-SFRC with both monomer systems. Exp-SFRC (50wt%) had significantly higher FT (2.6MPam1/2) and FS (175.5MPa) (p<0.05) compared to non-reinforced material (1.3MPam1/2, 123MPa). Failure mode analysis of crown restorations revealed that FT value of the substructure directly influenced the failure mode.SIGNIFICANCE: This study shows that short glass-fibers can significantly reinforce flowable composite resin and the FT value of SFRC-substructure has prior importance, as it influences the crack arresting mechanism.

Influence of different viscosity and chemical composition of flowable composite resins: A 48-month split-mouth double-blind randomized clinical trial

Abstract:Objectives: To evaluate the clinical performance of two flowable composites based on methacrylate and one based on ormocer in treating non-carious cervical lesions (NCCLs) after 48-month evaluation in a split-mouth double-blind clinical study design. Methods: A total of 183 restorations were performed on NCCLs using a universal adhesive system (Futurabond U, Voco GmbH) with selective enamel etching on 27 participants: two participants received twelve restorations each, three received nine restorations each, and 22 participants received six restorations each. Three different flowable composites were employed (n = 61): a low-viscosity methacrylate-based composite (GrandioSO Flow, LVM), a high-viscosity methacrylate-based composite (GrandioSO Heavy Flow, HVM), and an ormocer-based flowable composite (Admira Fusion Flow, ORM). All restorations were evaluated using FDI and USPHS criteria after 48 months. Statistical analysis was conducted using Kaplan-Meier Survival analysis and Kruskal-Wallis analysis of variance rank (α = 0.05). Results: After 48 months, 17 restorations were lost: LVM 6, HVM 9, ORM 2. The retention rates (95 % confidence interval) were 89.4 % for LVM, 80.4 % for HVM, and 95.6 % for ORM, with a significant difference between HVM vs. LVM and HVM vs. ORM (p < 0.05). Minor defects were observed in 30 restorations for marginal staining criteria (LVM 12, HVM 10, ORM 8) and in 71 restorations for marginal adaptation criteria (LVM 24, HVM 20, ORM 27) without significant difference between groups (p > 0.05). No restorations showed postoperative sensitivity or recurrence of caries. Significance: The increased viscosity of flowable composites could reduce the clinical longevity in NCCLs after 48 months. Ormocer-based and low-viscosity methacrylate-based flowable composites showed a successful clinical performance in NCCLs after 48 months.
Finite element and in vitro study on biomechanical behavior of endodontically treated premolars restored with direct or indirect composite restorations

Abstract:Objectives of the study were to investigate biomechanical properties of severely compromised premolars restored with composite restorations using finite element analysis (FEA), and in vitro fracture resistance test. A 3-D model of an endodontically treated premolar was created in Solidworks. Different composite restorations were modelled (direct restoration-DR; endo-crown-EC; post, core, and crown-C) with two different supporting tissues: periodontal ligament/alveolar bone (B), and polymethyl methacrylate (PMMA). Models were two-point axially loaded occlusally (850 N). Von Mises stresses and strains were calculated. The same groups were further tested for static fracture resistance in vitro (n = 5, 6.0 mm-diameter ball indenter, vertical load). Fracture resistance data were statistically analyzed ( p < 0.050). The highest stresses and strains in all FEA models were observed on occlusal and vestibular cervical surfaces, corresponding to fracture propagation demonstrated in vitro. C showed the lowest stress in dentin, while EC showed lower stresses and strains in crown cement. B models demonstrated larger high stress areas in the root than PMMA models. No significant differences in fracture resistance (N) were observed between groups (DR: 747.7 ± 164.0, EC: 867.3 ± 108.1, C: 866.9 ± 126.3; p = 0.307). More conservative restorations seem a feasible alternative for endodontically treated premolars to conventional post-core-crown.

Clinical Evaluation of a Flowable Resin Composite and Flowable Compomer for Preventive Resin Restorations.

Abstract:This clinical study evaluated the retention and caries protection of a flowable resin composite (Flow Line) and a flowable compomer (Dyract Flow) used in preventive resin restorations as compared to the conventional preventive resin technique which uses a resin composite (Brilliant) and a sealant (Concise). This study observed 205 permanent molars with small carious cavities less than 1.5 mm in width, which were obtained from 165 children aged 7 to 15 years. Flowable resin composite was used to treat 75 teeth, and 71 teeth were treated with flowable compomer in both cavities and caries-free fissures. For the control group, 59 teeth were treated with resin composite in cavities and sealant in caries-free fissures. The teeth were evaluated at 3, 6, 12, 18 and 24-month intervals. After three months, all 205 treated teeth were completely intact. After six months, 66 of the 71 teeth treated with flowable resin composite and 65 of the 70 teeth treated with flowable compomer were complete, compared to 57 of the 58 teeth treated with the conventional preventive resin technique. After 12 months, 60 of the 67 teeth treated with flowable resin composite and 61 of the 67 teeth treated with flowable compomer were complete, compared to 51 of the 55 teeth treated with the conventional preventive resin technique. After 18 months, 53 of the 61 teeth treated with flowable resin composite and 54 of the 62 teeth treated with flowable compomer were complete, compared to 47 of the 53 teeth treated with the conventional preventive resin technique. After 24 months, 49 of the 58 teeth treated with flowable resin composite and 45 of the 57 teeth treated with flowable compomer were complete, compared to 42 of the 52 teeth treated with the conventional preventive resin technique. There were no statistically significant differences in retention rates among all groups after 3, 6, 12, 18 or 24-months (p>0.05). One tooth treated with flowable resin composite and one tooth treated with flowable compomer developed caries after 18 and 24 months, respectively, resulting from partial loss at "caries-free fissures." Five teeth developed caries in the conventional preventive resin group; one after 12 months, two after 18 months and one after 24 months, due to loss at cavities. The final caries occurred after 24 months, resulting from partial loss at "caries-free fissures." The differences in caries development among the three groups were not statistically significant (p>0.05). This study suggested that flowable resin composite and flowable compomer could be used for preventive resin restorations. Meanwhile, a vigilant recall should be followed-up due to the risk of failure for flowable materials in "caries-free" fissures. The repair should be performed immediately, in case the preventive resin restoration develops a fracture or loss.
Randomized prospective clinical trial of class II restorations using flowable bulk-fill resin composites: 4-year follow-up

Abstract:Objectives This randomized, prospective, and split-mouth study aimed to evaluate flowable bulk-fill resin composites in class II restorations, comparing it with a conventional layering technique after 4-year follow-up. Materials and methods Fifty-three subjects received three class II restorations according to the restorative systems: conventional microhybrid composite resin (PA – Peak Universal + Amelogen Plus, Ultradent); flowable bulk-fill and nanoparticulate composite resins (ABF – Adper Single Bond 2 + Filtek Bulk Fill Flow + Filtek Z350XT, 3M/Espe); and flowable bulk-fill and microhybrid composite resins, (XST – XP Bond + SDR + TPH3, Dentsply). The clinical performance and interproximal contacts were evaluated. Statistical analyses were performed using the Kaplan-Meier, equality test of two proportions, Friedman, Wilcoxon, Kruskal-Wallis, Mann-Whitney, and Logistic regression analysis tests (alpha = 0.05). Results In total, 106 restorations were evaluated at 4 years. Both systems with bulk-fill composites presented higher marginal discoloration than PA. About surface texture, ABF restorative system showed superior bravo scores being statistically similar to XST. Better performance for wear and surface staining was found for XST restorative system. All restorative systems resulted in the decreased interproximal contacts, occurring early for XST. Conclusions The restorative systems that used flowable bulk-fill resin composites showed satisfactory clinical performance compared with conventional resin composite after 4 years. All restorative systems had decreased proximal contact after 4 years. Clinical relevance Initial marginal discoloration was observed in more than 50% of class II restorations performed with restorative systems that used flowable bulk-fill resin composite. All restorative systems had decreased proximal contact strength over time.

Marginal Adaptation of Flowable vs Sonically Activated or Preheated Resin Composites in Cervical Lesions

Abstract:Purpose: To investigate marginal integrity of restorations applied with preheated and non-preheated composite, flowable composite, sonically activated composite, and a new thermo-viscous bulk-fill composite using near-infrared technology for preheating, in class V cavities of human molars. Materials and methods: Standardized cavities were prepared on the buccal surfaces of 60 human mandibular molars and restored with one of the following resin composite materials after application of an etch-and-rinse adhesive (OptiBond FL, Kerr): non-preheated or preheated conventional composite (Filtek Supreme XTE, 3M Oral Care), preheated thermo-viscous composite (VisCalor bulk, Voco), soncially activated composite (SonicFill 3, Kerr), or flowable composite (Filtek Supreme XTE Flowable, 3M Oral Care) applied in bulk or as a lining material using the snow-plow technique. After light curing and polishing, the percentage of continuous margins (PCM) of the restorations in enamel and dentin was assessed using SEM both before and after thermomechanical loading (TML). TML was carried out with 3000 thermal cycles (5°C-50°C) and a simultaneous mechanical stress application with 1.2 million load-cycles (1.7 Hz, 49 N) in a computer-controlled masticator. Non-parametric statistical analysis was performed using Wilcoxon, Kruskal-Wallis, and Mann-Whitney U-tests (α = 0.05). Results: All groups revealed a significant decline in marginal integrity after TML in both enamel and dentin. Although the flowable group in enamel and the snow-plow group in dentin showed the highest PCM before TML, the differences between the groups were compensated after TML. Conclusion: All of the tested composites and application methods showed similar marginal integrities after thermomechanical loading and can be recommended for clinical implementation.
Differential effects of chemical and physical restraint on carbohydrate tolerance testing in nonhuman primates.

Abstract:Ketamine hydrochloride, used during glucose or lactose tolerance tests to aid restraint of rhesus and stumptail macaques unconditioned to handling, prevented the establishment of definitive baseline plasma glucose tolerance curves from which meaningful interpretations could be derived. Repetition of the tests after the animals were restraint-chair conditioned provided clear baseline control data. It is suggested that ketamine might invalidate those carbohydrate tolerance tests which are based on the absorption of glucose from the gastrointestinal tract.
In vitro analysis of flowable and high viscosity bulk fill composite versus conventional composite in core build up of endodontically treated teeth

Abstract:This study aimed to test the load to fracture of flowable and high viscosity bulk-fill composites compared to a convetional (packable) composite used to build up pulpless teeth. 45 endodontically treated premolars were restored with fiber posts then divided into three groups (N = 15) according to the core build-up material. SDR group: (Smart Dentine Replacement), (BF): Filtek One Bulk Fill and (CC) Filtek Z350 packable composite. After core build up the teeth received full coverage crowns. Load to fracture (Newton) was recorded for each specimen. One-way ANOVA and Tukey multiple comparisons test were used. The highest load at fracture was shown by (CC) group (1067 N), followed by (BF) group (879 N), and the SDR group (838 N). One way ANOVA and Tukey's multiple comparisons test demonstrated no significant differences between CC, SDR, and BF groups (p > 0.05). The three materials showed comparable load to fracture. Further studies with simulation to occlusal forces and oral temperature are required.

Abstract:background: resin-bonded fixed dental prosthesis (RBFDP) represents a highly aesthetic and conservative treatment option to replace a single tooth in a younger patient. The purpose of this in vitro study was to compare the fracture strength and the different types of failure on anterior cantilever RBFDPs fabricated using zirconia (ZR), lithium disilicate (LD), and PMMA-based material with ceramic fillers (PM) by the same standard tessellation language (STL) file. Methods: sixty extracted bovine mandibular incisives were embedded resin block; scanned to design one master model of RBFDP with a cantilevered single-retainer. Twenty cantilevered single-retainer RBFDPs were fabricated using ZR; LD; and PM. Static loading was performed using a universal testing machine. Results: the mean fracture strength for the RBFDPs was: 292.5 Newton (Standard Deviation (SD) 36.6) for ZR; 210 N (SD 37.6) for LD; and 133 N (SD 16.3) for PM. All the failures of RBFDPs in ZR were a fracture of the abutment tooth; instead; the 80% of failures of RBFDPs in LD and PM were a fracture of the connector. Conclusion: within the limitations of this in vitro study, we can conclude that the zirconia RBFDPs presented load resistance higher than the maximum anterior bite force reported in literature (270 N) and failure type analysis showed some trends among the groups

Abstract:Background. The aim of this research was to carry out a comparative clinical study of a self-adhesive, light-curing composite material called Vertise Flow and a traditional flow material called Premise flowable used in combination with dedicated bonding systems. In order to standardize the clinical environment, the stability of oral hygiene (analyzed with the oral hygiene index and the approximal plaque index) was taken into consideration. Methods. The study involved 37 patients with 64 fillings. They were distributed into three groups: 22 fillings in Group I, 22 fillings in Group II and 20 fillings in Group III. In Group I (G I), Vertise Flow material was applied without the use of an etching agent or a bonding system; in Group II (G II), Premise flowable material was applied without the use of an etching agent, but with the use of the OptiBond All-In-One seventh-generation bonding system; in Group III (G III), Premise flowable material was applied after etching and treatment of the hard tooth tissues using a fifth-generation OptiBond Solo Plus bonding system. Then, at appropriate time intervals (0, i.e., right after filling and after 6, 12 and 24 months), the fillings were subjected to clinical evaluation, conducted according to the Ryge scale criteria with the use of registration by means of a fluorescent high-intensity visible light beam produced by a camera (Vista Proof). Results. The quality of fillings performed with the use of comparable materials was subjected to clinical evaluation using the Ryge scale of fillings after 6, 12, and 24 months; the examination showed significant differences between the tested materials. The Vertise Flow material used without an etching agent or a bonding system (G I) presented the weakest results with respect to marginal adaptation and smoothness among those evaluated in this study. The intensification of the degradation continued over time until the final clinical observation at 24 months. The results confirmed that the best quality was achieved with the Premise flowable material after etching and treatment of the hard tooth tissues using a fifth-generation OptiBond Solo Plus bonding system (G III) relative to others for all time points during the observation. Conclusions. The used preparation scheme and bonding system have an impact on the final quality of the composite filling. There is a need to carry out a qualitative clinical evaluation of dental restorative materials under uniform conditions using evaluation scales.

Abstract:A new ruthenium complex with an electron-donating aminoindenyl ligand induces a fast living radical polymerization of methyl methacrylate (MMA) in the presence of a chloride initiator to give polymers with controlled and variable molecular weights (Mn = 103-105) and very narrow molecular weight distributions (Mw/Mn < 1.1). The structure and high activity of the catalyst were analyzed by X-ray crystallography and cyclic voltammetry in comparison to those of a similar complex with an indenyl ligand.

Abstract:Background/purpose Recently, an effective core build-up system for teeth with flared root canals is needed. This research aimed to evaluate the effect of foundation restorations using a composite resin core with a fiber post reinforced with a zirconia tube for the surface strain at the cervical area and the fracture load of teeth with flared root canals. Materials and methods Bovine teeth were shaped to mimic human premolars with flared root canals and restored using three types of composite resin foundation restorations with each materials described below: a fiber post (FC), a zirconia tube (ZC), a fiber post and zirconia tube (ZFC). Each specimen was restored with a zirconia crown. The surface strains of the specimens at the cervical area and fracture loads were analyzed using a one-way analysis of variance (ANOVA), followed by Tukey's honest significant difference test. Results The surface strains of Groups ZFC and ZC were significantly lower than that of Group FC in the buccal root. The fracture strengths of Groups ZFC and ZC were significantly higher than that of Group FC. The strength of Group ZFC was significantly higher than that of Group ZC. Conclusion The use of a composite resin core with a zirconia tube for the simulated premolar with flared root canals reduced surface strain at the cervical area and provided higher fracture strength compared to using a composite resin core with a fiber post. And the zirconia tubes provided even higher fracture strength when used with a fiber post.

Abstract:STATEMENT OF PROBLEM: The existing knowledge is insufficient for comprehending the fatigue survival and fracture resistance of molars that have deep approximal direct and indirect restorations, whether with or without deep margin elevation (DME).PURPOSE: The aim of this laboratory and in silico study is to investigate the fatigue survival, fracture strength, failure pattern and tooth deformation of molars restored with DME in combination with a direct or indirect restoration.MATERIAL AND METHODS: This study utilized 45 extracted sound human molars, divided into three groups (n = 15). Standardized 100% inter-cuspal inlay preparations were performed, extending 2 mm below the CEJ and immediate dentin sealing (IDS) was applied. Group 1 (Co_1) was restored with direct composite; Group 2 (Hyb_2) with a 2 mm DME of direct composite and a glass-ceramic lithium disilicate restoration; Group 3 (Cer_3) a glass-ceramic lithium disilicate restoration. All specimens were exposed to a fatigue process involving thermal-cyclic loading (50N for 1.2 × 106 cycles at 1.7 Hz, between 5 and 55 °C), if teeth survived, they were fractured using a load-to-failure test and failure types were analyzed. Finite element analysis (FEA) was conducted to assess tooth deformation and tensile stress in the restorations. Statistical evaluation of fracture strength was conducted using the Kruskal-Wallis test. Fisher's exact test was utilized to analyze the fracture types and repairability. A statistical significance level of α < 0.05 was set for all analyses.RESULTS: All specimens successfully withstood the fatigue testing procedure, and no statistically significant differences in fracture strength were observed among the three groups (P > 0.05). The Fisher's exact test indicated a significant association between the restorative material and fracture type (F2 = 18.315, df = 2, P = 0.004), but also for repairability (F2 = 13.725, df = 2, P = 0.001). Crown-root fractures were significantly more common in the Cer_3 group compared to the Co_1 group (P = 0.001) and the Co_1 group had significantly more repairable fractures (F2 = 13.197, df = 2, P = 0.001). FEA revealed comparable outcomes of deformation among models and higher maximum tensile stress on models with higher frequency of catastrophic failures.CONCLUSIONS: All tested restoration materials exhibited comparable fatigue survival and fracture strength in this laboratory and in silico study. However, it is important to recognize the potential for more severe and irreparable fractures when opting for deeply luted glass-ceramic inlay restorations in clinical practice. In such cases, it would be prudent to consider the alternative option being a direct composite approach, because of its more forgiving fracture types and repairability.CLINICAL IMPLICATIONS: Molars with deep approximal direct and indirect restorations, whether with or without DME, are comparable in their fatigue survival and fracture resistance to withstand intra-oral forces. Deep direct restorations exhibit more repairable fractures compared to deeply luted glass-ceramics.

Abstract:The objective of the study was to evaluate the effect of different restorative protocols on fracture resistance of root canal-treated molars. 48 mandibular first molars were used and divided into six groups (n = 8); G1 (negative control): teeth kept intact. G2 (positive control): teeth had root canal treatment and standard MOD cavity preparations but kept unrestored. G3: prepared as G2 and directly restored with VitaEnamic ceramic overlays (CO). G4: as G3, but the pulp chamber was restored first with smart dental restorative (SureFil SDR flow = SDR) bulk-fill flowable composite base. G5: as G3, but the pulp chamber was restored first with SonicFill (SF) bulk-fill composite base. G6: as G3, but the pulp chamber was restored first with a fiber-reinforced composite (FRC) base. All samples were subjected to thermocycling between 5 °C and 55 °C in a water bath for a total of 2000 cycles with 10 s dwell time. Then specimens were individually mounted on a computer-controlled testing machine with a load cell of 5 kN, and the maximum load to produce fracture (N) was recorded. Data were analyzed using one-way ANOVA followed by Tukey's post hoc test (P = 0.05). There was a significant difference between the groups (P < 0.001). Teeth restored with FRC and ceramic overlays had the highest load-bearing capacity. Pulp chamber restoration with either FRC or SDR before ceramic overlay fabrication provided significantly better tooth reinforcement than ceramic overlay alone (P < 0.001). Fracture modes were analyzed to determine the type of fracture as repairable or catastrophic, where FRC + CO and SDR + CO groups had favorable fracture modes that were mostly repairable. When restoring root canal-treated molars with overlays, the pulp chamber should be sealed with either FRC or SDR to ensure the best possible fracture resistance. The clinical relevance of the study is that a new simple restorative protocol is presented to enhance the survival of root canal-treated molars using ceramic overlays.

Abstract Aim To evaluate the fracture resistance and fracture patterns of root filled maxillary first premolar teeth (with mesioocclusal cavity preparation) restored with several composite restoration designs. Methodology One hundred extracted sound human maxillary first premolars were randomly divided into five groups. Group 1 remained untreated (negative control). Conventional root canal treatment with additional mesialocclusal cavity preparation was carried out on teeth in groups 25. In group 2, the teeth were restored intra-coronally with direct composite resin (positive control). In group 3, the palatal cusps of the teeth were reduced, and the cavities were restored with composite resin covering the palatal cusp (partial coverage). In group 4, the buccal and palatal cusps along with the distal marginal ridges were reduced; the cavities and cusps were restored with composite resin (conventional full coverage). In group 5, the buccal and palatal cusps were reduced but the distal marginal ridges were conserved. The cavities and the cusps were restored with composite resin (modified full coverage). All teeth were subjected to a progressive compressive loading parallel to their longitudinal axis until fracture. Fracture resistance was analysed using the one-way ANOVA and Fishers LSD test. Fracture patterns were analyzed with chi-square test. The significance level was set at 0.05. Results The fracture resistance (mean +/- SD) of groups 15 was 1131 +/- 207N, 904 +/- 184N, 927 +/- 224N, 1095 +/- 289N and 1085 +/- 243N, respectively (groups 1, 4, 5 > groups 2, 3; P = 0.004). Cusp fractures were recorded as the fracture pattern in 20 (100%), 19 (95%), 16 (80%), 8 (40%) and 12 (60%) premolars in groups 15, respectively (groups 1, 2 > groups 4, 5; group 3 > group 4; P < 0.001). Conclusions When direct composite resin was used to restore root filled maxillary first premolar teeth involving a proximal surface, those restored with full-coverage designs had greater fracture resistance.
