Three-Dimensional Internal Voids and Marginal Adaptation in Deep Margin Elevation Technique: Efficiency of Highly Filled Flowable Composites

Abstract:Purpose: To evaluate interfacial three-dimensional adaptation and internal voids of different flowable materials before and after cyclic fatigue in a simulated deep-margin elevation scenario. Methods: Eighty (n = 80) extracted premolars were selected and two Class II cavities were prepared. The mesial one with cervical margin 1 mm above the cementum-enamel junction (CEJ) and the distal one with cervical margin 1 mm below the CEJ. After performing adhesive procedures, specimens were divided into four groups according to the employed materials for 2 mm horizontal deep-margin relocation: nanohybrid composite (Clearfil ES2, Kuraray); conventional viscosity flowable composite (Tetric Flow, Ivoclar); medium viscosity flowable composite (Majesty ES2 Low Flow, Kuraray); high viscosity flowable composite (Majesty ES2 Super Low Flow, Kuraray). All restorations were finalized by oblique layering with nanohybrid composite (Clearfil ES2, Kuraray). To reveal interfacial and internal gap progression, specimens were scanned with a micro-CT (SkyScan 1172), before and after 500,000 cycles of mechanical chewing simulation (50 N, 1 Hz). Data were imported into Mimics software after smoothing and region growing. Only the 2 mm margin relocation volumes were considered. Obtained masks were analyzed for noise removal and volume calculation. At baseline, interfacial gap progression and internal voids, expressed in mm3, were collected and statistically analyzed with two-way ANOVA (α 0.05) for the variables substrate and restorative materials followed by Tukey post-hoc test. An additional two-way ANOVA test, followed by Tukey post-hoc test, was performed to evaluate variation in interfacial gap progression after mechanical aging. Results: At baseline, the ANOVA test showed a significant difference for the variable restorative materials (p = 0.01). More specifically, the Tukey post-hoc test revealed that the highly filled medium viscosity composite performed better than the conventional viscosity composite at baseline for the interfacial gap. The internal voids ANOVA test at baseline reported no significant differences for the variable tested. Analysis of variance for internal gap progression after thermocycling showed no differences for both substrate and restorative material employed. Conclusions: Highly filled medium viscosity composite performed significantly better than the conventional viscosity flowable composite for what concern baseline interfacial gaps. Artificial aging with a chewing simulator and thermocycling did not affect interfacial gap progression on enamel and dentin. The tested restorative materials performed equally after aging.
Computer Aided Design Modelling and Finite Element Analysis of Premolar Proximal Cavities Restored with Resin Composites

Abstract:This study evaluated the stress distribution in five different class II cavities of premolar models restored with conventional or bulk-fill flowable composite by means of finite element analysis (FEA) under shrinkage and occlusal loading. An upper validated premolar model was imported in the software, and five class II cavities with different occlusal extensions and dimensions were prepared: horizontal cavity on the mesial surface (horizontal slot), mesio-occlusal cavity, mesial cavity (vertical slot), tunnel type cavity and direct access cavity. The models were restored with conventional or bulk-fill flowable resin composite. The tested materials were considered as homogeneous, linear, and isotropic. The Maximum Principal Stress criteria was chosen to evaluate the tensile stress results. The lowest shrinkage stress value was observed in the direct access cavity restored with bulk-fill flowable resin composite (36.12 MPa). The same cavity, restored with conventional composite showed a score of 36.14 MPa. The horizontal slot cavity with bulk-fill flowable showed a score of 46.71 MPa. The mesio-occlusal cavity with bulk-fill flowable had a score of 53.10 MPa, while with conventional composite this was 55.35 MPa. Higher shrinkage stress was found in the vertical slot cavity with conventional resin 56.14 MPa, followed by the same cavity with bulk-fill flowable 56.08 MPa. Results indicated that the use of bulk-fill flowable composite resin more significantly decreased the polymerization shrinkage stress magnitude. The larger the cavity and the volume of material necessary to restore the tooth, the greater the residual stress on enamel and dentin tissue.

Abstract:3D tooth models were virtually restored: flowable composite resin + bulk-fill composite (A), glass ionomer cement + bulk-fill composite (B) or adhesive + bulk-fill composite (C). Polymerization shrinkage and masticatory loads were simulated. All models exhibited the highest stress concentration at the enamel–restoration interfaces. A and C showed similar pattern with lower magnitude in A in comparison to C. B showed lower stress in dentine and C the highest cusps displacement. The use of glass ionomer cement or flowable composite resin in combination with a bulk-fill composite improved the biomechanical behavior of deep class II MO cavities.

Effects of the lining material, thickness and coverage on residual stress of class II molar restorations by multilayer technique

Abstract:Objective: This study aims to analyse the influence of different lining material, thickness and coverage on residual stress of class II molar restoration by the multilayer technique through the three-dimensional (3D) finite element (FE) method. The objective is to reveal the correlations between the base layer configurations and the residual stress distributions of the tooth. Methods: A 3D reconstructed model of an extracted first molar was built through micro-CT images and a class II mesio-occlusal-distal cavity was prepared using computer-aided engineering (CAE) software. A bilayer technique was then applied, and 3D FE analyses were performed under polymerization contraction loading. Glass ionomer composites (Vitrebond and Ionosit) and flowable composite resin (Luxa Flow and Z350 Flowable Restorative) were used as lining materials. Moreover, several sensitivity analyses with dozens of hypothetical lining materials were conducted to provide definitive results. The thickness of the base layers was set to be distinct (0.5 mm or 1 mm) so as to reveal its effect on the stress alleviation. Various lining strategies with the liner covering different areas of the cavity walls (Closed Sandwich, Open Sandwich and Special Open Sandwich) were adopted to determine the effects of the base layers. Results: Adoption of an appropriate liner could effectively reduce the polymerization shrinkage stress. The mechanical properties of the base layer can affect the residual stresses, basically a lower Young's modulus and lower Poisson's ratio (of lining material) result in better stress mitigation, therefore reducing the stress transmitted to the dentin. Increase of the lining thickness within a certain range could lead to a decrease in the probability of stress concentration formation. Lining strategies had the strongest influence on the stress distributions. Different lining coverage could cause various stress responses, and covering all cavity walls with the lining material had the optimal performance among all the simulations in this study. Significance: The multilayer technique is an effective way to prolong the service life of resin composites restorations. A thorough evaluation of the sandwich technique through the FE method can provide a better understanding of the stress distributions of the restoration, and reveal its internal mechanisms.
[Micro-CT observations of the adaptation at gingival wall in Class II restorations with different dental restorative materials]

Abstract:Objective: To evaluate the adaptation of different materials for gingival layer in Class II restorations using Micro-CT. Methods: Eighteen extracted human premolars were selected, and Class II cavities were prepared. The teeth were randomly divided into six groups and restored using layering technique. Six materials were used for gingival layer, including four injectable materials: Beautifil Flow Plus F00 (F00), Beautifil Flow F10 (F10), Filtek Z350 Flowable (Z350F), FujiII LC CAPSULE (Fuji), and two packable materials: BeautifilII (BF), Filtek Z350 (Z350). The restored teeth were scanned with micro-CT and the images were 3D reconstructed to evaluate the volumes and the distribution of the voids on the restoration-tooth interface of the gingival layer. The volume of the voids were statistically analyzed using nonparametric Jonckheere-Terpstra tests. Results: The volumes (mm(3)) of the voids on the restoration-tooth interface were: Z350F (0.000 15), F10 (0.000 39), F00 (0.012), Fuji (0.070), Z350 (0.16) and BF (0.20). There were significant differences between Z350F/F10 and Fuji/Z350/BF (P < 0.05). Most of the voids were found on the point-line angles of the cavities. Conclusion: The voids on the restoration-tooth interface were mainly on the point-line angles of the cavities. Injectable materials with high flowablility could reduce the restoration-tooth interface voids significantly when used for the gingival layer in Class II restorations.

Abstract:Objectives of the study were to investigate biomechanical properties of severely compromised premolars restored with composite restorations using finite element analysis (FEA), and in vitro fracture resistance test. A 3-D model of an endodontically treated premolar was created in Solidworks. Different composite restorations were modelled (direct restoration-DR; endo-crown-EC; post, core, and crown-C) with two different supporting tissues: periodontal ligament/alveolar bone (B), and polymethyl methacrylate (PMMA). Models were two-point axially loaded occlusally (850 N). Von Mises stresses and strains were calculated. The same groups were further tested for static fracture resistance in vitro (n = 5, 6.0 mm-diameter ball indenter, vertical load). Fracture resistance data were statistically analyzed ( p < 0.050). The highest stresses and strains in all FEA models were observed on occlusal and vestibular cervical surfaces, corresponding to fracture propagation demonstrated in vitro. C showed the lowest stress in dentin, while EC showed lower stresses and strains in crown cement. B models demonstrated larger high stress areas in the root than PMMA models. No significant differences in fracture resistance (N) were observed between groups (DR: 747.7 ± 164.0, EC: 867.3 ± 108.1, C: 866.9 ± 126.3; p = 0.307). More conservative restorations seem a feasible alternative for endodontically treated premolars to conventional post-core-crown.

Measuring vibrational motion in the presence of speckle using off-axis holography.

Abstract:We present a holographic laser vibrometer designed to mitigate the effects of speckle noise when measuring the vibrational motion of a rough object. We show that multiplexing the interferometric measurement across 105 pixels provides a 50 dB reduction in the incoherent noise. Using a high-speed camera, this enables a displacement sensitivity of 50 fm/√Hz with a bandwidth of 12.5 kHz when measuring rough objects, representing a 20 dB improvement compared with a commercially available single-detector-based laser vibrometer. Finally, we show that the holographic vibrometer system is capable of stand-off acoustic sensing by measuring the acoustic-induced vibrations of a piece of paper with sensitivity as low as 10 dB (re 20 μPa). The ability to sensitively and noninvasively measure the vibrations of arbitrary rough surfaces could enable new applications in laser vibrometry.

Abstract:The aim of the present paper is to investigate the deformation–stress state of fillings of V-shaped tooth defects by finite element analysis (FEA). Two different materials are used—auto-cured resin-reinforced glass-ionomer cement (GIC) and flowable photo-cured composite (FPC). Two materials are placed into the cavity in one portion, as before the application of the composite the cavity walls are covered with a thin adhesive layer. Deformations and equivalent von Mises stresses are evaluated by FEA. Experimental study of micro-leakage is performed. It is established that there is an analogous non-homogeneous distribution of equivalent Von Mises stresses at fillings of V-shaped defects, made with GIC and FPC. Maximum stresses are generated along the boundaries of the filling on the vestibular surface of the tooth and at the bottom of the filling itself. Values of equivalent Von Mises stresses of GIC fillings are higher than that of FPC. Magnitude and character of deformation distribution at GIC and FPC fillings are similar—deformation is maximum along the vestibular surface of the filling and is 0.056 and 0.053 mm, respectively. In FPC fillings, the adhesive layer, located along the cavity/filling boundary, is characterized with greatest strain. The experimental study of micro-leakage has confirmed the adequacy of models used in FEA.

Abstract:This study aimed to investigate the mechanical behavior of resin composites and hybrid glass ionomer cement in class I adhesive dental restorations under loading and shrinkage conditions. Three CAD models of a mandibular first molar with class I cavities were created and restored with different techniques: a bi-layer of Equia Forte HT with Filtek One Bulk Fill Restorative composite (model A), a single layer of adhesive and Filtek One Bulk Fill Restorative (model B), and a single layer of Equia forte HT (model C). Each model was exported to computer-aided engineering software, and 3D finite element models were created. Models A and B exhibited a similar pattern of stress distribution along the enamel–restoration interface, with stress peaks of 12.5 MPa and 14 MPa observed in the enamel tissue. The sound tooth, B, and C models showed a similar trend along the interface between dentine and restoration. A stress peak of about 0.5 MPa was detected in the enamel of both the sound tooth and B models. Model C showed a reduced stress peak of about 1.2 MPa. A significant stress reduction in 4 mm deep class I cavities in lower molars was observed in models where non-shrinking dental filling materials, like the hybrid glass ionomer cement used in model C, were applied. Stress reduction was also achieved in model A, which employed a bi-layer technique with a shrinking polymeric filling material (bulk resin composite). Model C's performance closely resembled that of a sound tooth.

Abstract:STATEMENT OF PROBLEM: The existing knowledge is insufficient for comprehending the fatigue survival and fracture resistance of molars that have deep approximal direct and indirect restorations, whether with or without deep margin elevation (DME).PURPOSE: The aim of this laboratory and in silico study is to investigate the fatigue survival, fracture strength, failure pattern and tooth deformation of molars restored with DME in combination with a direct or indirect restoration.MATERIAL AND METHODS: This study utilized 45 extracted sound human molars, divided into three groups (n = 15). Standardized 100% inter-cuspal inlay preparations were performed, extending 2 mm below the CEJ and immediate dentin sealing (IDS) was applied. Group 1 (Co_1) was restored with direct composite; Group 2 (Hyb_2) with a 2 mm DME of direct composite and a glass-ceramic lithium disilicate restoration; Group 3 (Cer_3) a glass-ceramic lithium disilicate restoration. All specimens were exposed to a fatigue process involving thermal-cyclic loading (50N for 1.2 × 106 cycles at 1.7 Hz, between 5 and 55 °C), if teeth survived, they were fractured using a load-to-failure test and failure types were analyzed. Finite element analysis (FEA) was conducted to assess tooth deformation and tensile stress in the restorations. Statistical evaluation of fracture strength was conducted using the Kruskal-Wallis test. Fisher's exact test was utilized to analyze the fracture types and repairability. A statistical significance level of α < 0.05 was set for all analyses.RESULTS: All specimens successfully withstood the fatigue testing procedure, and no statistically significant differences in fracture strength were observed among the three groups (P > 0.05). The Fisher's exact test indicated a significant association between the restorative material and fracture type (F2 = 18.315, df = 2, P = 0.004), but also for repairability (F2 = 13.725, df = 2, P = 0.001). Crown-root fractures were significantly more common in the Cer_3 group compared to the Co_1 group (P = 0.001) and the Co_1 group had significantly more repairable fractures (F2 = 13.197, df = 2, P = 0.001). FEA revealed comparable outcomes of deformation among models and higher maximum tensile stress on models with higher frequency of catastrophic failures.CONCLUSIONS: All tested restoration materials exhibited comparable fatigue survival and fracture strength in this laboratory and in silico study. However, it is important to recognize the potential for more severe and irreparable fractures when opting for deeply luted glass-ceramic inlay restorations in clinical practice. In such cases, it would be prudent to consider the alternative option being a direct composite approach, because of its more forgiving fracture types and repairability.CLINICAL IMPLICATIONS: Molars with deep approximal direct and indirect restorations, whether with or without DME, are comparable in their fatigue survival and fracture resistance to withstand intra-oral forces. Deep direct restorations exhibit more repairable fractures compared to deeply luted glass-ceramics.

Abstract:Purpose: To investigate marginal integrity of restorations applied with preheated and non-preheated composite, flowable composite, sonically activated composite, and a new thermo-viscous bulk-fill composite using near-infrared technology for preheating, in class V cavities of human molars. Materials and methods: Standardized cavities were prepared on the buccal surfaces of 60 human mandibular molars and restored with one of the following resin composite materials after application of an etch-and-rinse adhesive (OptiBond FL, Kerr): non-preheated or preheated conventional composite (Filtek Supreme XTE, 3M Oral Care), preheated thermo-viscous composite (VisCalor bulk, Voco), soncially activated composite (SonicFill 3, Kerr), or flowable composite (Filtek Supreme XTE Flowable, 3M Oral Care) applied in bulk or as a lining material using the snow-plow technique. After light curing and polishing, the percentage of continuous margins (PCM) of the restorations in enamel and dentin was assessed using SEM both before and after thermomechanical loading (TML). TML was carried out with 3000 thermal cycles (5°C-50°C) and a simultaneous mechanical stress application with 1.2 million load-cycles (1.7 Hz, 49 N) in a computer-controlled masticator. Non-parametric statistical analysis was performed using Wilcoxon, Kruskal-Wallis, and Mann-Whitney U-tests (α = 0.05). Results: All groups revealed a significant decline in marginal integrity after TML in both enamel and dentin. Although the flowable group in enamel and the snow-plow group in dentin showed the highest PCM before TML, the differences between the groups were compensated after TML. Conclusion: All of the tested composites and application methods showed similar marginal integrities after thermomechanical loading and can be recommended for clinical implementation.

Abstract:Total knee arthroplasty (TKA) is an effective procedure for decreasing pain, improving functional capability, and increasing the overall quality of life for thousands of people with chronic knee osteoarthritis. Although patient outcomes and satisfaction remain high, a substantial percentage of patients report residual pain after TKA. Sources of postoperative pain include intra- and extra-articular etiologies as well as factors unrelated to the implants themselves. A patient-centered approach to the painful TKA may aid clinicians in diagnosing and treating patients with intra-articular causes of pain after TKA. A thorough understanding of the mechanisms involved may lead to improved preoperative planning and patient selection, ultimately decreasing the number of patients with less than optimal postoperative outcomes.

Abstract:SUMMARY Objectives To evaluate the effect of composite resins (one conventional and two low-shrink composites) and filling techniques on cuspal strains (CS), microtensile bond strength (μTBS), composite ultimate tensile strength (UTS), and mechanical properties of the composites at various depths in molars with large Class II restorations. Materials and Methods One hundred seventeen human molars received standardized Class II mesio-oclusal-distal cavity preparations and restorations with three composites (Filtek LS [3M-ESPE]; Aelite LS [BISCO]; and Filtek Supreme [3M-ESPE]) using three filling techniques (bulk, eight increments, and 16 increments). CS was measured using strain gauges, after which the same restored teeth were used to assess μTBS and UTS. The elastic modulus (E) and Vickers hardness (VH) at different depths were determined from microhardness indentations. The CS, μTBS, UTS, E, and VH data were statistically analyzed using split-plot analysis of variance and Tukey test (p=0.05). Results The CS was higher when using 16 increments. The ‘low-shrink' composites caused lower CS. The μTBS and UTS were similar for eight- and 16-increment techniques and higher when compared to the bulk filling in all composites. E and VH were constant through the depth when applied in eight or 16 increments. Conclusions Type of composite and filling technique affected the CS, μTBS, UTS, and mechanical properties of large Class II restorations. The eight-increments filling technique resulted in generally less CS with the same μTBS and UTS than was obtained with 16 increments, without affecting E and VH through the depth of the composites.

Severe loss of tooth structure was simulated with six restorative options: 1) glass-fiber-reinforced composite post and composite core (GFRC)+CAD/CAM leucite-reinforced glass-ceramic crown (LRC); 2) carbon-fiberreinforced composite post and composite core (CFRC)+LRC; 3) metal cast post-and-core (MPC)+LRC 4) GFRC+CAD/CAM composite resin crown (CC); 5) CFRC+CC; 6) MPC+CC. Three-point occlusal loading (150N) was simulated and von Misses and maximum principal stresses calculated. Results: Although maximum von Mises stresses in the crown and dentin were similar across groups (137.9-139.2MPa crown; 17.2-19.6 dentin), there were important differences in stress distribution in dentin. Only in MPC+CC group the maximum stresses were on the bottom of the post preparation cavity. Stress values within the posts were: CFRC(4.8MPa)>GFRC(6.7MPa)>MPC(10.3MPa). CC-restored models presented higher von Mises stresses within the post-and-core compared to the LRC groups. Maximum principal stresses were lower compared to von Mises stresses, following the same trend, and were distributed similarly in all the groups. Conclusions: Both GFRC and CFRC showed favourable stresses distribution in the dentin and restorative materials, while MPC increased stresses in core, post and post cement. The more rigid crown material seems to transmit less stresses to the underlying core and crown cement compared to CC.

Abstract:Early detection and prompt treatment of ocular disorders in children is important to avoid lifelong visual impairment. Examination of the eyes should be performed beginning in the newborn period and at all well-child visits. Newborns should be examined for ocular structural abnormalities, such as cataract, corneal opacity, and ptosis, which are known to result in visual problems. Vision assessment beginning at birth has been endorsed by the American Academy of Pediatrics, the American Association of Pediatric Ophthalmology and Strabismus, and the American Academy of Ophthalmology. All children who are found to have an ocular abnormality or who fail vision assessment should be referred to a pediatric ophthalmologist or an eye care specialist appropriately trained to treat pediatric patients.
Influence of margin design and restorative material on the stress distribution of endocrowns: a 3D finite element analysis

Abstract:Abstract Background This study aimed to evaluate the stress distributions in endocrown restorations as applied to endodontically treated teeth (ETT), according to the factors of “margin design” (four levels) and “restorative material” (six levels). Methods Four 3D-finite elements models were constructed for endocrown restored molars considering different margin designs. Model A was prepared with a flat butt joint margin and received an endocrown with a 2.0-mm occlusal thickness. Model B was prepared with a 20° bevel margin and received an endocrown with a 2.0-mm occlusal thickness. Model C was prepared with an axial reduction and 1-mm shoulder margin and received an endocrown with a 2.0-mm occlusal thickness. Model D was prepared with an anatomic margin and received an endocrown with a 2.0-mm occlusal thickness. The following endocrown materials were used: In-Ceram Zirconia (Zr), Vita Suprinity (VS), IPS Empress (IE), Grandio blocs (GR), VisCalor bulk (VS), and CopraPeek Light (CP). The Load application (600 N) was performed at the food bolus and tooth surface during the closing phase of the chewing cycle. The results for the endocrown and tooth remnants were determined according to the von Mises stress. The failure risk of the cement layer was also calculated based on the normal stress criterion. Results Model D (with an anatomic margin) showed the greatest stress concentrations, especially in the irregular and sharp angles of the restoration and tooth remnants. The stress concentrated on the dentin was significantly lower in Model B with a 20° bevel margin (20.86 MPa), i.e., 1.3 times lower than the other three margin designs (27.80 MPa). Restorative materials with higher elastic moduli present higher stress concentrations inside the endocrown and transmit less stress to the cement layer, resulting in lower bonding failure risks. In contrast, materials with an elastic modulus similar to that of dentin presented with a more homogeneous stress distribution on the whole structure. Conclusions An endocrown with a 20° bevel margin design could be a favorable preparation option for ETT. Composite resins (GR and VC) exhibit a more even stress distribution, and seem to be more promising materials for endocrown molars.

Abstract:Objectives: The aim was to investigate the two-body wear at the marginal area between direct filling composites and substrate of CAD/CAM composites or enamel. Materials and methods: Flat specimens were prepared from CAD/CAM composites (CERASMART 270 and SFRC CAD) and bovine enamel. A box-shaped cavity cut into CAD/CAM composites and enamel surfaces was made. The prepared cavity in CAD/CAM composites was treated with a primer, while in enamel, the cavity was treated with an adhesive. Three conventional composites (Universal Injectable, G-aenial A'Chord, and Filtek Bulk Fill) and one short fiber composite (everX Flow) were placed and cured in the prepared cavities. A two-body wear test was conducted with 15,000 chewing cycles using a dual-axis chewing simulator. The specimens (n = 5/per group) were positioned to produce wear (load = 20 N) across the marginal area between filling composites and substrates. The wear depth was analyzed using a 3D optical profilometer. SEM was used to evaluate the wear behavior and margins between the filling and substrate materials. Results: All composites used displayed different wear behavior (20-39 µm) (p < 0.05). The highest wear values were recorded for A'Chord and Filtek, while the lowest values were for Injectable and CERASMART 270. The data analysis showed that the wear behavior of substrate materials depends on the filling materials used at margins (p < 0.05). The marginal breakdown was seen only between bovine enamel and filling composites. Conclusions: The use of the two-body wear simulation method revealed important information about the behavior of the filling composites at the marginal area with CAD/CAM composites or bovine enamel substrates. Clinical relevance: The marginal breakdown related to the material combination at the bonding region.

Abstract:Statement of problem: By decreasing the proportion of filler, flowable composites are easy to adapt to small cavity preparations. However, the wear resistance of flowable composites is a clinical concern, as improved wear has been related to increased filler percentage. Purpose: The purpose of this study was to compare wear for flowable composites and comparable heavily filled materials. Material and methods: Wear by attrition and abrasion (microm) of microfilled flowable (Heliomolar Flow) and heavily filled microfill (Heliomolar HB) was measured and compared to a microhybrid flowable (Tetric Flow) and a heavily filled microhybrid (Tetric HB). There were 5 specimens, 2 mm thick and 15 mm in diameter, in each group. The Oregon Health Sciences University oral wear simulator was used to produce abrasive wear and attrition for all specimens. Abrasion and attrition of the substrates were measured using a profilometer. Data were subjected to a 3-way analysis of variance for the 3 main factors: composite type, filler load, and wear type (alpha=.05) Results: Composite wear rates were significantly influenced (P <.01) by both the type of wear and the filler amount. Abrasive wear occurred less than wear by attrition, and the heavily filled composites experienced less wear than the flowable composites. Conclusions: Results of this in vitro study suggest that highly filled microhybrid composites may have greater wear resistance for contact-supporting posterior restorations. However, in small noncontact restorations where longevity may be dictated by abrasion, flowable microfilled resins may be adequate.

Abstract:Background: The purpose of this in vitro study was to compare and evaluate the stress distribution of maxillary first premolar residual crowns restored with post-core crowns, endocrowns and inlay crowns after deep margin elevation, to explore the fitting restoration for residual crowns using finite element analysis. Methods: A healthy complete right maxillary first premolar from a male adult was scanned by cone beam computed tomography (CBCT). The finite element model of the tooth was established by reverse engineering software such as Mimics, Geomagic and Hypermesh. On this basis, the residual crown model after deep margin elevation was made, and the experimental group models were divided into three groups, those restored with post core crowns, endocrowns and inlay crowns. Vertical and oblique static loads were applied to the experimental models to simulate the force on the tooth during mastication (the loading position was located in the central fossa of the occipital surface, and the load was 100 N) using Abaqus software. Results: The peak value and distribution of von Mises stress in each part of the experimental model were observed. After deep margin elevation, the peak dentin von Mises stresses were lower than the tensile strength of normal dentin in the post-core crown, endocrown, and inlay crown groups; the lowest stress results were found in the post-core crown group for the dentin, restoration, enamel, and deep margin elevation (DME) layers under vertical and oblique loading. In terms of stress distribution clouds, the peak stresses in the dentin tissue were located in the apical 1/3 of the root after postcore crown restorations for both loads, while stress concentrations were evident in the cervical and root areas after endocrown and inlay crown restorations; regardless of the load and restoration method, the corresponding stress concentration areas appeared at the junction of the DME and dentin tissue at the loading site of the restorations; CONCLUSIONS: Post-core crowns, endocrowns and inlay crowns can be used to restore residual crowns after deep margin elevation, and post-core crowns can better protect the residual tooth tissue.
