Abstract:Purpose To evaluate the fracture resistance of zirconia overlays, considering various preparation designs and the presence of endodontic access. Materials and Methods Ninety translucent zirconia (5Y‐PSZ) overlay restorations were divided into six groups (n = 15/group) based on different preparation designs, with and without endodontic access: chamfer margin 4 mm above the gingival level without (group 1) and with endodontic access (group 2); margin 2 mm above the gingival level without (group 3) and with endodontic access (group 4); overlay with no chamfer margin without (group 5) and with endodontic access (group 6). Restorations were bonded to mandibular first molar resin dies, and the groups with endodontic access were sealed with flowable resin composite. All restorations underwent 100,000 cycles of thermal cycling between 5°C and 55°C, followed by loading until fracture. Maximum load and fracture resistance were recorded. ANOVA with Tukey post‐hoc tests were used for statistical comparison (α < 0.05). Results Fracture resistance significantly varied among overlay designs with and without endodontic access (p < 0.001), except for the no‐margin overlays (groups 5 and 6). Overlays with a 2 mm margin above the gingival margin with endodontic access (group 4) exhibited significantly higher fracture resistance compared to both the 4‐mm supragingival (group 2) and no‐margin (group 6) designs, even when compared to their respective intact groups (groups 1 and 5). There were no significant differences between the no‐margin and 4‐mm supragingival overlays. Conclusion The more extensive zirconia overlay for mandibular molars is the first choice since the 2 mm margin above the gingival level design withstood considerable loads even after undergoing endodontic access. A no‐margin overlay is preferred over the 4‐mm supragingival design as it preserves more tooth structure and there was no outcome difference, irrespective of endodontic access. Caution is warranted in interpreting these findings due to the in vitro nature of the study.

Abstract:STATEMENT OF PROBLEM: The existing knowledge is insufficient for comprehending the fatigue survival and fracture resistance of molars that have deep approximal direct and indirect restorations, whether with or without deep margin elevation (DME).PURPOSE: The aim of this laboratory and in silico study is to investigate the fatigue survival, fracture strength, failure pattern and tooth deformation of molars restored with DME in combination with a direct or indirect restoration.MATERIAL AND METHODS: This study utilized 45 extracted sound human molars, divided into three groups (n = 15). Standardized 100% inter-cuspal inlay preparations were performed, extending 2 mm below the CEJ and immediate dentin sealing (IDS) was applied. Group 1 (Co_1) was restored with direct composite; Group 2 (Hyb_2) with a 2 mm DME of direct composite and a glass-ceramic lithium disilicate restoration; Group 3 (Cer_3) a glass-ceramic lithium disilicate restoration. All specimens were exposed to a fatigue process involving thermal-cyclic loading (50N for 1.2 × 106 cycles at 1.7 Hz, between 5 and 55 °C), if teeth survived, they were fractured using a load-to-failure test and failure types were analyzed. Finite element analysis (FEA) was conducted to assess tooth deformation and tensile stress in the restorations. Statistical evaluation of fracture strength was conducted using the Kruskal-Wallis test. Fisher's exact test was utilized to analyze the fracture types and repairability. A statistical significance level of α < 0.05 was set for all analyses.RESULTS: All specimens successfully withstood the fatigue testing procedure, and no statistically significant differences in fracture strength were observed among the three groups (P > 0.05). The Fisher's exact test indicated a significant association between the restorative material and fracture type (F2 = 18.315, df = 2, P = 0.004), but also for repairability (F2 = 13.725, df = 2, P = 0.001). Crown-root fractures were significantly more common in the Cer_3 group compared to the Co_1 group (P = 0.001) and the Co_1 group had significantly more repairable fractures (F2 = 13.197, df = 2, P = 0.001). FEA revealed comparable outcomes of deformation among models and higher maximum tensile stress on models with higher frequency of catastrophic failures.CONCLUSIONS: All tested restoration materials exhibited comparable fatigue survival and fracture strength in this laboratory and in silico study. However, it is important to recognize the potential for more severe and irreparable fractures when opting for deeply luted glass-ceramic inlay restorations in clinical practice. In such cases, it would be prudent to consider the alternative option being a direct composite approach, because of its more forgiving fracture types and repairability.CLINICAL IMPLICATIONS: Molars with deep approximal direct and indirect restorations, whether with or without DME, are comparable in their fatigue survival and fracture resistance to withstand intra-oral forces. Deep direct restorations exhibit more repairable fractures compared to deeply luted glass-ceramics.

Post-Fatigue Fracture and Marginal Behavior of Endodontically Treated Teeth: Partial Crown vs. Full Crown vs. Endocrown vs. Fiber-Reinforced Resin Composite

Abstract:Objectives: To investigate in vitro post-fatigue fracture behavior of endodontically treated molars having been differently restored. Methods: A total of 120 extracted human molars were used. A total of 120 specimens in 14 test groups and one control group (n = 8) were root canal treated. After postendodontic sealing and build-up (AdheSE Universal, SDR), additional MOD preparations were cut. Postendodontic restorations were: Direct restorations (Tetric EvoCeram BulkFill bonded with AdheSE Universal and EverX Posterior/Essentia bonded with G-Premio Bond; as filling (F) or direct partial crown (DPC) after reducing the cusps 2 mm; indirect adhesive restorations (partial crown/PC vs. full crown/FC): e.max CAD, Celtra Duo, both luted with Variolink Esthetic; indirect zirconia restorations (partial crown/PC vs. full crown/FC), luted with RelyX Unicem 2; indirect non-bonded cast gold restorations (partial crown/PC vs. full crown/FC; Degunorm), luted with Ketac Cem. Before and after 300,000 thermocycles (5/55 °C) and 1.2 Mio. A total of 100 N load cycles, replicas were analyzed under a SEM for marginal quality in enamel and dentin (where applicable) and finally, specimens were loaded until fracture. Results: In direct groups, there was no difference between RC and FRC in fracture strength (p > 0.05); however, direct partial crowns showed higher post-fatigue fracture resistance. Regarding marginal quality, intracoronal FRC restorations exhibited more gap-free margins in enamel than RC. In the indirect groups, there was no significant difference between partial and full crowns in any of the adhesively luted ceramic groups regarding post-fatigue fracture resistance. Zirconia partial crowns exhibited significantly lower marginal quality in enamel. Indirect groups performed significantly better than direct groups in fracture resistance. Within the indirect restorations, both cast gold groups and zirconia full crowns exhibited the highest fracture resistance being superior to control teeth. Significances: Within the limits of this in vitro investigation, it can be concluded that any kind of indirect restoration with cusp replacement is suitable for ETT restoration when a certain cavity extension is exceeded. All indirect restorations, i.e., endocrowns, partial crowns, and full crowns showed a promising performance after in vitro fatigue-loading.

Effect of preparation design on fracture strength of compromised molars restored with direct composite resin restorations: An in vitro and finite element analysis study

Abstract:Statement of problem: More data are needed on the influence of preparation design on the fracture strength, failure type, repairability, and polymerization-induced cracks of molar teeth restored with direct composite resin restorations. Purpose: This in vitro and finite element analysis study investigated the effect of different preparation designs on fracture strength, failure type, repairability, tooth deformation, and the formation of polymerization-induced cracks of compromised molars restored with direct composite resin restorations. Material and methods: Human molars (n=64) were randomly assigned to 4 different preparation designs: undermined inlay (UI), extended inlay (EI), restricted overlay (RO), and extended overlay (EO). The teeth were restored using direct composite resin and subjected to artificial thermomechanical aging in a mastication simulator, followed by load-to-failure testing. Three-dimensional (3D) finite element analysis was conducted to assess tooth deformation. Polymerization-induced cracks were evaluated using optical microscopy and transillumination. The fracture strength data were analyzed using a Kruskal-Wallis test, while the failure mode, repairability, and polymerization cracks were analyzed using the Fisher exact test (α=.05). Results: All specimens withstood thermomechanical aging, and no statistically significant difference in fracture strength was observed among the 4 preparation designs (P>.05). The finite element analysis showed differences in tooth deformation, but no correlation was observed with in vitro fracture resistance. The RO and EO groups presented significantly more destructive failures compared with the UI and EI groups (P<.01). The RO group had significantly fewer repairable failures than the UI and EI groups (P=.024). A correlation was found between higher frequencies of repairability and higher tooth deformation. A significant correlation between the increase in microfractures and preparation design was observed (P<.01), with the UI group exhibiting a higher increase in microfracture size compared with the EO group (P<.05). Conclusions: No influence of preparation design on the fracture strength of compromised molars restored with direct composite resin restorations was evident in this study, but the failure mode of cusp coverage restorations was more destructive and often less repairable. The finite element analysis showed more tooth deformation in inlay preparations, with lower stresses within the root, leading to more reparable fractures. Since cusp coverage direct composite resin restorations fractured in a more destructive manner, this study suggests that even a tooth with undermined cusps should be restored without cusp coverage.

Abstract:Background: Endodontic treatment is the most common method for resolving pulpal and periapical pathology. However, various studies have reported that almost 11%-13% of all teeth that undergo extraction after endodontic treatment show the presence of cracks, craze lines, and vertical root fractures. Teeth with inadequate post endodontic restoration are more prone to fracture and coronal leakage, resulting in the diffusion of oral fluids, bacteria, bacterial products, and possibly root canal treatment failure. Furthermore, studies have advocated the use of endodontically treated teeth with restorative materials that have a similar or higher elastic modulus than the tooth for providing stiffness against forces that cause root fracture. Intraorifice barriers made of restorative materials that can bond to radicular dentin could thus be used to reinforce the radicular dentin while also preventing coronal microleakage. Although the sealing ability of intraorifice barriers has been widely compared in the literature, there have been few studies on the strengthening effect of the materials used in the study as intraorifice barriers when placed into the root canal. As a result, the current in vitro study aims to assess the effect of various materials as intraorifice barriers (Cention N, Resin modified glass ionomer cement, and short fiber reinforced flowable composite) on the force required fracture teeth after root canal treatment. Methods: This in vitro study will be done on extracted human mandibular premolars with single root canal where after doing root canal treatment 2-3 mm obturating material would be replaced by intra orifice barriers (Cention N, resin modified glass ionomer cement [RMGIC], and short fiber reinforced flowable composite). The force required to fracture teeth will be calculated using universal testing machine.
Fracture resistance and marginal gap formation of post-core restorations: influence of different fiber-reinforced composites

Abstract:Objectives: The aim was to explore the fracture behavior and marginal gap within the root canal of endodontically treated (ET) premolars restored with different fiber-reinforced post-core composites (FRCs). Further aim was to evaluate the composite curing at different depths in the canal. Materials and methods: Eighty-seven intact upper premolars were collected and randomly divided into six groups. After endodontic procedure, standard MOD cavities were prepared and restored with their respective fiber-reinforced post-core materials: group 1: prefabricated unidirectional FRC-post + conventional composite core; group 2: prefabricated unidirectional FRC-post + short fiber composite (SFRC) core; group 3: individually formed unidirectional FRC-post + conventional composite core; group 4: randomly oriented SFRC directly layered as post and core; group 5: individually formed unidirectional FRC + randomly oriented SFRC as post and core. After restorations were completed, teeth (n = 3/group) were sectioned and then stained. Specimens were viewed under a stereo microscope and the percentage of microgaps within the root canal was calculated. Fracture load was measured using universal testing machine. Results: SFRC application in the root canal (groups 4 and 5) showed significantly higher fracture load (876.7 N) compared to the other tested groups (512-613 N) (p < 0.05). Post/core restorations made from prefabricated FRC-post (group 1) exhibited the highest number of microgaps (35.1%) at the examined interphase in the root canal. Conclusions: The restoration of ET premolars with the use of SFRC as post-core material displayed promising performance in matter of microgap and load-bearing capacity. Clinical significance: Fracture resistance of ET premolar restored by bilayered composite restoration that includes both SFRC as post-core material and surface conventional resin seems to be beneficial.

Abstract:We describe the construction of expression vectors based on three of the most frequently used gene fusion affinity tags [glutathione S-transferase (GST), maltose binding protein (MBP), and the His6 peptide]. The polylinkers of pGEX4T1, pMal-c2, and a pET vector were replaced with the polylinker isolated from the baculovirus expression plasmid pFastBac. Once appropriate restriction sites have been introduced into a gene, it can be fused to all three affinity tags with little effort, allowing expression-screening experiments to be performed efficiently. We discuss the development and use of these vectors with respect to overcoming purification problems encountered for the RhoA GDP/GTP nucleotide dissociation inhibitor (RhoGDI) and their advantages over commercially available expression vectors.
Fracture Resistance of Endodontically Treated Maxillary Premolar Teeth Restored with Wallpapering Technique: A Comparative In Vitro Study

Abstract:Objectives. This in vitro study aimed to evaluate and compare the fracture resistance and mode of failure of endodontically treated maxillary premolar teeth restored with different direct composite restorative techniques. Materials and Methods. Forty freshly extracted maxillary premolar teeth with comparable sizes were used in this in vitro study. Each tooth received mesio-occluso-distal cavity preparation (3 mm width and 6 mm depth) followed by endodontic treatment. Canals were instrumented with RACE EVO rotary files (FKG, Dentaire, Switzerland) up to MAF 25/.06. Canals were obturated using a single cone technique, then the teeth were divided arbitrarily into five groups (n = 8): Group A: direct composite resin only using a centripetal technique, Group B: direct composite resin with glass fiber post, Group C: direct composite resin with short fiber-reinforced composite (everX Flow), Group D: direct composite resin with leno wave ultra high molecular weight polyethylene (LWUHMWPE) fibers placed on cavity floor, and Group E: direct composite resin with LWUHMWPE fibers placed circumferentially around the cavity walls (wallpapering technique). The teeth were then stored in distilled water at 37°C for 24 hr. The fracture resistance of each sample was measured using a universal testing machine in Newton (N). The data were analyzed using one-way analysis of variance (ANOVA) and the Bonferroni test with a significance level of 0.05. Results. Group E recorded the maximum mean of fracture load (2,139.375 N), while Group A recorded the minimum mean of fracture load (689.6250 N). The one-way ANOVA test revealed a significant difference between the groups. The Bonferroni test showed a significant difference between each two groups, with the exception of those between Groups B and C and between Groups D and E, where there were no statistically significant differences ( p > 0.05 ). Conclusion. Restoration of endodontically treated teeth using the wallpapering technique recorded the highest mean of fracture resistance with a repairable mode of fracture.
Influence of Direct Coronal Restoration Materials on the Fracture Resistance of Endodontically Treated Premolars: An In Vitro Study

Abstract:(1) Background: The long-term survival of an endodontically treated tooth depends on a successful root canal treatment as well as an adequate definitive coronal restoration. This study aimed to evaluate the strength of endodontically treated premolars with mesial-occlusal-distal (MOD) cavity preparation restored with different direct coronal restoration materials but from the same manufacturer against fracture. (2) Methods: sixty intact premolars were selected and placed into five groups (n = 12): G1-intact teeth, G2-endodontic treatment and unrestored MOD cavities, G3-endodontic treatment and MOD cavities restored with Tetric PowerFlow and Tetric EvoCeram, G4-endodontic treatment and MOD cavities restored with Multicore Flow and Tetric EvoCeram, and G5-endodontic treatment and MOD cavities restored with Multicore Flow. The specimens were subjected to an axial compression load at a speed of 1.6 mm/min and optically inspected before and after with a stereomicroscope. For each premolars group, the following data were recorded: the compression resistance, the compressive strength, and the maximum force supported. The microstructure of the samples after the compression test was analyzed using scanning electron microscopy (SEM). (3) Results: statistical analysis (ANOVA and Tukey test) showed that there was a statistically significant difference between G1 and the other groups. Even though there was no statistically significant difference between the restored groups, a better mechanical behavior was registered within the G3. (4) Conclusions: this in vitro study indicated that none of the materials used can lead to a higher or at least similar fracture resistance as the intact teeth. The coronal restoration only with nano-hybrid composites may lead to a higher therapeutic benefit for the fracture-susceptible premolars.
[Effects of the Resin Base and Retention Depth on Fracture Resistance of Molars Restored with Nano-Ceramic Endocrowns].

Abstract:OBJECTIVE:To compare the effects of resin base and different retention depth on the fracture resistance of mandibular molars restored with nano-ceramic endocrowns.METHODS:Forty mandibular molars selected and randomly divided into 5 groups: ① The control group which was consisted of intact teeth, ② the non-resin base group, ③ the 2 mm retention depth group, ④ the 3 mm retention depth group, ⑤ the 4 mm retention depth group, respectively. After tooth preparation, in vitro root canal therapy was conducted, which was followed by endocrown design, production and adhesive of groups ②-⑤. All the samples were under load (N) of the universal mechanical testing machine after embedding. The fracture pattern of each sample was observed under stereomicroscope. Then the microstructure of the fracture surface was observed by scanning electron microscopy.RESULTS:The fracture loads of each group were respectively: the control group fracture load was (3 069.34±939.50) N; experimental groups: fracture load of (2 438.04±774.40) N for the group without resin base; fracture load of (3 537.18±763.65) N for the group with 2 mm retention depth. The fracture load of the retention depth 3 mm group was (2 331.55±766.39) N; the fracture load of the retention depth 4 mm group was (2 786.98±709.24) N. There was statistical significance in the effect of resin base and different retention depth on the fracture loads of molars restored with nano-ceramic endocrown (P < 0.05). Repairable fractures in each group were as follows: control group 2/8, non-resin base group 1/8, retention depth of 2 mm group 1/8, retention depth of 3 mm group 2/8, and retention depth of 4 mm group 0/8. The effects of the retention depth and the presence of resin base on the fracture resistance of the resin nano-ceramic endocrowns were statistically significant (P < 0.05). Scanning electron microscopy showed more arrest lines and small twist hackles on the fracture surface of the restorations with resin base (retention depths of 2 mm, 3 mm, and 4 mm), with cracks extending towards the root. In addition to the characteristics above, more transverse cracks parallel to the occlusal surface, pointing outwards from the center of the pulp cavity retention, were also observed on the fracture surface of the non-resin base restorations.CONCLUSION:When molar teeth with nano-ceramic endocrowns are restored, resin base and the retention depth of 2 mm help the teeth to obtain optimal fracture strength.
Fracture Resistance in Fibre-Reinforced Resin Composite Restorations in Deciduous and Permanent Molars: An Ex Vivo Study

Abstract:Aim: The aim of this study was to compare the fracture resistance levels of restored deciduous teeth and permanent molars restored with different materials, including ultra-polyethylene fibre tape (Ribbond-Ultra), fibre-reinforced resin composite EverX posterior, fibre-reinforced flowable resin composite EverX Flow and bulk-fill flow restorative material (Tetric N-flow) in the posterior region. Methods: We tested sixty-four caries-free human mandibular molars (32 deciduous molars and 32 permanent molars). Deep and wide Class I cavities were prepared in each tooth. The teeth were embedded in poly(methyl methacrylate) resin up to the cementoenamel junction, leaving the crown completely exposed. Oral temperature fluctuations were then simulated using two thousand thermocycling cycles, each lasting 30 seconds in the hot phase (approximately 35 °C). The loading rate for our force-fracture tests was set at 0.5 mm/min. Direct restorations were made using a spherical tip and applying the force perpendicular to the sample surface. Visual inspection of the fractured specimens, in combination with adhesive scanning electron microscopy (SEM) and finite element analysis (FEA), provided detailed insights into the failure modes and stress distribution at the restoration-tooth interface. Results: Teeth restored with fibre-reinforced composite (EverX Posterior) had the highest fracture resistance, followed by fibre-reinforced flowable resin composite (EverX Flow). Teeth restored with the ultra- polyethylene fibre tape (Ribbond-Ultra), followed by the flowable bulk-fill composite (Tetric N-flow) had the lowest resistance. Statistical analysis revealed significant differences between the groups, except for EverX Posterior and EverX Flow. A predictive algorithm was also proposed for the likelihood of restoration failure. Conclusion: Modern fibre-reinforced resin composites, such as EverX Posterior, effectively reinforce teeth against fractures, with success in both restored deciduous and permanent molars. Meanwhile, the use of polyethylene fibre tapes is less effective, and they involve a time-consuming application.

The 60 teeth were divided randomly into 6 groups ( n = 10); Group1= AH Plus sealer; Group 2= TotalFill BC sealer; Group 3= BioRoot RCS sealer; Group 4= AH plus sealer + HGFP; Group 5= TotalFill BC sealer + HGFP; and, Group 6= Bio Root RCS sealer + HGFP. All specimens were thermocycled and subjected to cyclic loading. Next, the specimens were subjected to a static load using a universal testing machine. Data were analyzed using two-way ANOVA. Two micro-CTs of the same upper premolar, one without the HGFP and later one with HGFP inserted were made and used to create two finite element (FE) models. For the fracture resistance, two-way ANOVA revealed significant differences for the effect of HGFP ( p = 0.029), but no differences for the effect of root canal sealer type ( p = 0.561). The HGFP groups showed higher restorable fracture compared to groups without HGFP ( p = 0.013). Finite element analysis showed that the inclusion of HGFP reduces stress concentration at the occlusal interface and cervical region. Therefore, it can be concluded that HGFP significantly increased the fracture resistance of endodontically treated upper premolars with MOD cavities and reduced the risk for non-restorable fractures. The FE analysis supports the mechanical test results.

Fracture Resistance of Ceramic Crowns Supported with Indirect Chair-side Composite Cores

Abstract:Aims and objectives: To evaluate the influence of indirect chair-side polymerization of resin composite cores on the fracture resistance of overlaying IPS e.max Press crowns. Materials and methods: Root canals of 60 extracted premolars were prepared to receive #2 fiber posts after the crowns were sectioned 2 mm above the cervical line. In Groups 1-3 (n = 10 each), posts were luted to the prepared dowel spaces using self-adhesive resin cement. Resin composite cores were then bonded and incrementally built-up using Filtek Z250 XT, Filtek P60, and Filtek P90 resin composites. In Groups 4-6 (n = 10 each), the fabricated post-core systems were subjected to post-curing heat and pressure treatment before cementation to their respective teeth using self-adhesive resin cement. Another 10 sound premolars served as control. All teeth in the test and control groups were then subjected to standardized preparation to receive IPS e.max Press crowns before testing their fracture resistance and the mode of restorations' failure. The collected results were statistically analyzed using ANOVA, Kruskal-Wallis, and Tukey's tests on the past software used at α = 0.05 to stand on the significance of the detected differences. Results: Significant differences were detected between the fracture resistance of teeth in different groups (ANOVA, P = 2.857E-35). Crowns in Groups 4-6 provided higher fracture resistance than those in Groups 1-3 (Tukey's test, P < 0.05). Crowns in Groups 4 and 6 provided higher fracture resistance than the control, while those in Groups 2 and 3 provided lower fracture resistance than the control (Tukey's test, P < 0.05). Conclusion: Indirect composite cores improved the fracture resistance of IPS e.max Press crowns when compared to directly fabricated post and cores. The directly and indirectly polymerized nanohybrid, methacrylate-based composite (Filtek Z250 XT) cores yielded the highest fracture resistance for the utilized all-ceramic crowns.

Abstract Aim To evaluate the fracture resistance and fracture patterns of root filled maxillary first premolar teeth (with mesioocclusal cavity preparation) restored with several composite restoration designs. Methodology One hundred extracted sound human maxillary first premolars were randomly divided into five groups. Group 1 remained untreated (negative control). Conventional root canal treatment with additional mesialocclusal cavity preparation was carried out on teeth in groups 25. In group 2, the teeth were restored intra-coronally with direct composite resin (positive control). In group 3, the palatal cusps of the teeth were reduced, and the cavities were restored with composite resin covering the palatal cusp (partial coverage). In group 4, the buccal and palatal cusps along with the distal marginal ridges were reduced; the cavities and cusps were restored with composite resin (conventional full coverage). In group 5, the buccal and palatal cusps were reduced but the distal marginal ridges were conserved. The cavities and the cusps were restored with composite resin (modified full coverage). All teeth were subjected to a progressive compressive loading parallel to their longitudinal axis until fracture. Fracture resistance was analysed using the one-way ANOVA and Fishers LSD test. Fracture patterns were analyzed with chi-square test. The significance level was set at 0.05. Results The fracture resistance (mean +/- SD) of groups 15 was 1131 +/- 207N, 904 +/- 184N, 927 +/- 224N, 1095 +/- 289N and 1085 +/- 243N, respectively (groups 1, 4, 5 > groups 2, 3; P = 0.004). Cusp fractures were recorded as the fracture pattern in 20 (100%), 19 (95%), 16 (80%), 8 (40%) and 12 (60%) premolars in groups 15, respectively (groups 1, 2 > groups 4, 5; group 3 > group 4; P < 0.001). Conclusions When direct composite resin was used to restore root filled maxillary first premolar teeth involving a proximal surface, those restored with full-coverage designs had greater fracture resistance.

Abstract:This study compared the effectiveness of 0.5% ropivacaine and 0.5% bupivacaine for brachial plexus block. Forty-eight patients received a subclavian perivascular brachial plexus block for upper-extremity surgery. One group (n = 24) received ropivacaine 0.5% (175 mg) and a second group (n = 24) received bupivacaine 0.5% (175 mg), both without epinephrine. Onset times for analgesia and anesthesia in each of the C5 through T1 brachial plexus dermatomes did not differ significantly between groups. Duration of analgesia and anesthesia was long (mean duration of analgesia, 13-14 h; mean duration of anesthesia, 9-11 h) and also did not differ significantly between groups. Motor block was profound, with shoulder paralysis as well as hand paresis developing in all of the patients in both groups. Two patients in each group required supplemental blocks before surgery. Ropivacaine 0.5% and bupivacaine 0.5% appeared equally effective in providing brachial plexus anesthesia.
The effect of sealer type and obturation technique on the fracture resistance of endodontically treated roots

Abstract:Objective: This study evaluated the effect of sealer type with various obturation techniques on the fracture resistance of root filled teeth. Materials and methods: Eighty mandibular premolars were collected, and root canals of 70 of them were prepared by Mtwo system up to 40/04. Thirty teeth were obturated using TotalFill BC sealer, and 30 using AH Plus sealer, with one of the obturation techniques (N = 10), namely cold lateral compaction (CLC), warm vertical compaction (WVC), and single cone (SC). Ten teeth were left without obturation as a positive control, and the other 10 teeth were not prepared serving as a negative control. Teeth were stored for 3 weeks, and the crowns were separated and the roots were subjected to fracture test using a universal testing machine. Results: Data was analyzed by ANOVA followed by Tukey's test. Fracture resistance of the TotalFill groups was significantly higher than the AH Plus groups in each obturation technique, and SC had the highest fracture resistance regardless of sealer used (P < 0.05). Obturation of the root canal by TotaFill BC sealer increased the calcium/phosphorous ratio in the dentin of the root. Conclusions: Obturation with TotalFill BC sealer improved the fracture resistance of the roots more than AH Plus sealer, and obturation with SC resulted in higher fracture resistance than CLC and WVC. Clinical relevance: Obturation of the root canal with Bioceramic sealer with single cone obturation technique could reduce the incidence of vertical root fracture.

Abstract:This study aimed to evaluate the effect of direct restorations using unidirectional glass fiber orientations and a short-fiber-reinforced composite (SFRC) on the fracture resistance of endodontically treated premolars with mesio-occluso-distal cavities. Ninety double-rooted premolars were selected. Fifteen teeth were left intact/as a control group. The endodontic treatment and cavity preparations of seventy-five teeth were performed and divided into five experimental groups: Resin composite (RC), modified transfixed technique + RC, circumferential technique + RC, cavity floor technique + RC, and SFRC + RC. All teeth were fractured under oblique static loading at a 30° angle using a universal testing machine. The fracture patterns were observed and classified. Data were analyzed with one-way analysis of variance, Pearson chi-square, and Tukey HSD post hoc tests (p = 0.05). The highest fracture strength values were obtained in intact teeth (599.336 N), followed by modified transfixed + RC treated teeth (496.58 N), SFRC + RC treated teeth (469.62 N), RC (443.51 N), circumferential + RC treated teeth (442.835 N), and cavity floor + RC treated teeth (404.623 N) (p 0.05). Unrepairable fractures were observed at low rates (20%) in the modified transfixed + RC and SFRC + RC teeth, and at higher rates in RC (73.3%), cavity floor + RC (60%), and circumferential + RC (80%) teeth. The application of an SFRC or the modified transfixed technique yielded an improved fracture strength and the fracture pattern of ETPs being restored with a universal injectable composite.
