Abstract:In this paper, an interleaved half-bridge (IHB) three-port converter (TPC) is proposed for a renewable power system. The IHB-TPC is used to interface three power ports: 1) one source port; 2) one battery port; and 3) one isolated load port. The proposed IHB-TPC is derived by integrating two half-bridge TPC modules. A parallel configuration is adopted for the primary side of the two half-bridge modules, while a parallel-series configuration is adopted for the secondary side of the two modules. The power can be transferred from the source and the battery to the load within the whole switching cycle with the proposed IHB-TPC. It means there are no additional conduction losses caused by the circulating current or the free-wheeling operation stage. Hence, the voltage gain can be extended, and the output filter can be reduced. Zero-voltage switching is realized for all the four main switches to reduce the switching losses. Two of the three ports can be tightly regulated by adopting pulsewidth modulation plus phase-shift control, while the third port is left unregulated to maintain power balance for the system. The operation principles and the performances of the proposed converter are analyzed in detail. The experimental results are given to verify the feasibility and the effectiveness of the proposed converter.

Abstract:Purpose: To establish the interrelationships between temporomandibular disorders (TMDs), pain, and otologic and psychologic comorbidities in Asian youths. Materials and methods: Youths from a local polytechnic, aged 17 to 24 years old, were enrolled and an electronic survey encompassing demographic variables-the DC/TMD TMD pain screener (TPS), Short- form Fonseca Anamnestic Index (SFAI), modified Maciel's Otologic/Pain Symptom Inventory, and Patient Health Questionnaire-4-was administered. Par.cipants were subsequently categorized into groups: no TMD pain (NP), with TMD pain (WP), no TMDs (NT), and with any TMDs (WT). Data were evaluated with chi-square test, nonparametric, and logistic regression analyses (α = .05). Results: Among the 198 participants (mean age 18.8 ± 1.7 years), 11.1% had painful TMDs and 18.2% experienced TMD pain or dysfunction. Significant differences in total-otologic symptoms (total-OS), vertigo, and dizziness plus otalgia, tinnitus, and hearing loss scores were observed between the WP-NP and WT-NT groups correspondingly. While total-comorbid pain (total-CP) and psychologic distress (total-PD) scores varied substanally between the WT-NT group, only a significant difference in total-PD scores was discerned between the WP-NP group. Total-OS scores were moderately correlated to TPS/SFAI, total-CP, and total-PD scores, and psychologic distress was a risk factor for painful TMDs. Conclusions: Otologic and pain comorbidities were prevalent among Asian youths with TMDs and appear to be interrelated. The complex interaction of TMDs with pain, otologic, and psychologic comorbidities must be considered when caring for youths with multiple somatic complaints.

Physical and psychological correlates were also explored along with the risk factors for TMDs/somatization.MATERIALS AND METHODS: Participants were enlisted from a local university and the presence of TMDs and somatic symptoms was determined with the Short-form Fonseca Anamnestic Index and Patient Health Questionnaire-15. The psychological variables of personality, coping, and distress was assessed with the Big-Five Personality Inventory-10, brief-COPE Inventory, and Depression, Anxiety, Stress Scales-21 accordingly. Statistical evaluations were performed with the Mann-Whitney U test, Spearman's correlation, and logistic regression analyses (α = 0.05).RESULTS: Among the 455 participants (mean age 22.7 ± 1.2 years), 18.2% and 5.7% had TMDs and medium-to-high somatization respectively. Participants with TMDs exhibited substantially higher somatization and psychological distress scores than those with no TMDs. Significant differences in TMD, conscientiousness, extraversion, and psychological distress scores were observed between participants with no-to-mild and medium-to-high somatization. The association between TMD and somatization scores, albeit significant, was weak. Neuroticism and dysfunctional coping style were moderately correlated to general distress, depression, anxiety, and stress (rs = 0.44-0.62).CONCLUSIONS: Findings suggest that anxiety is the main risk factor for the presence of TMDs and medium-to-high somatization in non-clinical young adults while conscientiousness is a protective factor for somatization.

Abstract:Background: Temporomandibular disorders and somatization have shown interrelation in many studies. The physical and psychological factors which contributed to the occurrence and relation of both conditions are yet to be determined. Objectives: The personality traits, coping styles and psychological distress of young adults with temporomandibular disorder (TMD) and somatic symptoms were characterized together with the determination of psychological risk factors for TMDs, somatization and combined conditions. Methods: Participants were recruited from university-attending young adults. TMD and somatic symptoms were appraised with the short-form Fonseca Anamnestic Index and Patient Health Questionnaire-15. Psychological variables were assessed with the Big Five Personality Inventory-10, Brief-COPE Inventory and Depression, Anxiety, and Stress Scales-21. Data were evaluated using chi-squared/non-parametric tests and logistic regression analyses (α = .05). Results: Among the 507 participants (mean age 22.2 ± 1.5 years), 46.4% reported no TMD/somatic symptoms (NS) while 7.5%, 34.5% and 11.6% had TMDs only (TS), somatization only (SS) and combined TMDs-somatization (CS), respectively. Significant differences in conscientiousness (NS > SS), agreeableness (NS, TS > CS; NS > SS), dysfunctional coping, general distress, depression, anxiety and stress (CS ≥ SS > NS) were discerned. Multivariate analyses indicated that the odds of TS were increased by anxiety (OR = 1.10; 95% CI = 1.01-1.21), while the odds of SS/CS were affected by anxiety (OR = 1.15; 95% CI = 1.06-1.25/OR = 1.34; 95% CI = 0.19-1.52) and problem-focused coping (OR = 0.71; 95% CI = 0.56-0.89/OR = 0.55; 95% CI = 0.39-0.78). Conclusion: Though individuals with TMDs and somatization have dissimilar psychological profiles, anxiety constantly increased their likelihood. Problem-focused coping strategies may help alleviate psychosocial and physical stressors associated with TMDs and somatization.

Abstract:Background Psychosocial function of Chinese temporomandibular disorders (TMD) pain patients and the correlation with somatosensory function has not been sufficiently studied. Objective The study aims at assessing the psychosocial function of Chinese TMD pain patients by visualisation method and evaluating the correlations with somatosensory function quantitatively. Methods The Symptom Checklist 90 (SCL‐90) questionnaire and standardised quantitative sensory testing (QST) were administered to 70 Chinese TMD pain patients and age‐ and gender‐matched healthy controls (HCs). Of these, 40 TMD arthralgia patients received QST before and after medication. Psychosocial and somatosensory parameters were transformed into standardised scores. Differences within groups were assessed through t tests. Correlations between psychosocial and somatosensory profiles were explored through correlation analyses with Bonferroni correction for multiple comparisons. Results 100% of the Chinese TMD pain patients exhibited psychosocial distress in contrast to HCs. Abstract:Objective: The association between the 'fear of missing out (FOMO)' and physical symptoms has not been widely explored. This study aimed to investigate the relationship between FOMO and other negative emotions with Temporomandibular disorder (TMD) and somatic symptoms in young adults. The correlations between the various physical and emotional variables were also established. Material and methods: TMD and somatic symptoms were appraised with the Short-form Fonseca Anamnestic Index, quintessential five TMD symptoms of the Diagnostic Criteria (DC)/TMD, and Patient Health Questionnaire-15. FOMO and other negative emotional states were assessed with the FOMO Scale and Depression, Anxiety, Stress Scales-21 (DASS-21). Data were evaluated using non-parametric tests/correlation and regression analysis (α = 0.05). Results: While only negative affectivity (total DASS), anxiety, and stress differed significantly between those without and with TMDs, significant variances in FOMO and all DASS-21 constructs were discerned between individuals without and with somatization. Conclusions: Individuals with orofacial pain and more severe somatic symptoms have higher levels of negative emotions including FOMO. While somatization increased the prospect of TMDs, being female, presence of TMDs, and negative affectivity were risk factors for somatization in young adults. Clinical relevance: Asian young adults appear to be disposed to somatization, and TMDs may be a form of functional somatic syndromes. Recognition of somatic symptoms and emotional distress, including FOMO, is essential for person-centric TMD care.

Abstract:OBJECTIVE: the aim of this study was to describe the frequency of psychosocial diagnoses in a large sample of patients attending a tertiary clinic for treatment of temporomandibular disorders (TMD).MATERIAL AND METHODS: six hundred and ninety-one patients who sought treatment for pain-related TMD were selected. Chronic pain-related disability (Graded Chronic Pain Scale, GCPS), depression [Symptoms Checklist-90 (SCL-90) scale for depression, DEP] and somatization levels (SCL-90 scale for non-specific physical symptoms, SOM) were evaluated through the Research Diagnostic Criteria for TMD (RDC/TMD) Axis II psychosocial assessment; TMD diagnoses were based on the Axis I criteria.RESULTS: the majority of patients presented a low disability or no disability at all, with only a small portion of individuals showing a severely limiting, high disability pain-related impairment (4.3%). On the other hand, abnormal scores of depression and somatization were high, with almost half of the individuals having moderate-to-severe levels of depression and three-fourths presenting moderate-to-severe levels of somatization. The prevalence of high pain-related disability (GCPS grades III or IV), severe/moderate depression and somatization was 14.3%, 44% and 74.1% respectively. Gender differences in scores of SCL-DEP (p=0.031) and SCL-SOM (p=0.001) scales were signficant, with females presenting the highest percentage of abnormal values.CONCLUSION: patients with TMD frequently present an emotional profile with low disability, high intensity pain-related impairment, and high to moderate levels of somatization and depression. Therefore, given the importance of psychosocial issues at the prognostic level, it is recommended that clinical trials on TMD treatment include an evaluation of patients' psychosocial profiles.

Abstract:Objective: The aim of this study was to correlate degree of depression, somatization, and chronic pain in asymptomatic women with clinical findings, using Research Diagnostic Criteria/Temporomandibular disorders (RDC/TMD). Methods: A total of 200 female participants, ages 18-65, filled out a standard RDC/TMD axis II form for the assessment of chronic pain, disability, depression, and non-specific physical symptoms and underwent clinical examination of the temporomandibular joint. Correlation of clinical findings (axis I) and axis II assessment was performed using Spearman's correlation test, with significance set at p < 0.05. Results: There was a significant correlation between depression scores (p < 0.04), chronic pain (p < 0.001), and non-specific physical symptoms without questions about pain (p = 0.008). Discussion: The highest scores on the Graded Chronic Pain Scale were observed in patients with arthralgia, while patients with myofascial pain scored higher on depression and somatization tests.

Abstract:Characteristics of painful temporomandibular disorders and their influence on jaw functional limitation and oral health‐related quality of life. Background There is limited knowledge about the impact of painful temporomandibular disorders (TMDs) and pain characteristics on jaw functional limitation and oral health‐related quality of life (OHRQoL) in TMD patients. Objectives The influence of painful TMDs and pain characteristics on jaw functional limitation and OHRQoL was investigated. Inter‐relationships between limitation in jaw function and various OHRQoL domains, along with facial pain attributes predicting impaired jaw function and diminished OHRQoL were also examined. Methods TMD patients were recruited from a university‐based hospital. A comprehensive questionnaire comprising demographic variables, the DC/TMD Symptom Questionnaire, Graded Chronic Pain Scale, Jaw Functional Limitation Scale‐8 (JFLS‐8) and Oral Health Impact Profile‐TMD (OHIP‐TMD) was administered. Participants underwent a protocolized physical examination, and TMD diagnoses were determined utilising the DC/TMD algorithms. Participants were subsequently stratified into intra‐articular/pain‐related/combined TMD groups, as well as no TMD pain, acute/chronic pain and low/high‐intensity pain groups. Data were assessed using non‐parametric and hierarchical linear regression analyses (α = .05). Results The final sample consisted of 280 participants (mean age 31.2 (SD 11.8) years; 79.3% women). Significant differences in pain characteristics, JFLS‐8, and global OHIP scores were observed across the various TMD subtypes, pain chronicity and pain intensity categories. Pain intensity and pain‐related interference exhibited moderate correlations with JFLS‐8 and global OHIP scores (rs = 0.53–0.60). Moderate associations were also noted between JFLS‐8 and global OHIP, as well as most OHIP domains (rs = 0.42–0.64). Both jaw functional limitation and OHRQoL were predicted by sex, pain intensity and pain‐related interference. Conclusions Sex, pain intensity and pain‐related interference are key determinants for both impaired jaw function and diminished OHRQoL, with pain‐related interference exerting a more pronounced effect.

Abstract:Introduction: Temporomandibular disorder (TMD) is defined as a set of pathological clinical conditions involving the masticatory muscles, the temporomandibular joint, or both, and the associated structures. It is often associated with chronic headaches, cervical pain, otologic symptoms and may be accompanied by some degree of physical or psychological disability in patients. Objective: To investigate the association of chronic pain-related disability, depression, and somatization with temporomandibular disorders. Methods: It was a transversal study with 275 patients evaluated using the RDC/TMD (Research Diagnostic Criteria for Temporomandibular Disorders). The variables of Axis I (myofascial pain, disc displacement and other joint conditions) and Axis II (chronic pain-related disability, depression and somatization) were crossed using Fischer’s Exact Test (p <0.05). Results: Statistically significant association was found among factors of chronic pain-related disability, depression and somatization with myofascial pain diagnosis (p =0.000, p =0.001, p =0.000, respectively), as well as joint conditions (arthralgia, osteoarthritis and osteoarthrosis) (p =0.000, p =0.008, p =0.003, respectively). Conclusion: There is a positive association between chronic pain, depression and somatization (Axis II variables) with and myofascial pain and other joint conditions (Axis I). Disc displacements were not associated to variables of Axis II.
Comparison of Psychological States and Oral Health-Related Quality of Life of Patients with Differing Severity of Temporomandibular Disorders

Abstract:Background Studies on temporomandibular disorder (TMD) severity in patient populations are scarce. Objectives This study sought to compare the psychological states and oral health-related quality of life (OHRQoL) among patients with differing TMD severity. Methods Adult patients (>= 18 years old) with and without (controls) TMDs were recruited from the TMD/oro-facial pain centre and prosthodontics department, respectively. The presence and severity of TMDs were established with the Fonseca Anamnestic Index (FAI), and TMD diagnoses were confirmed with the Diagnostic Criteria for TMDs (DC/TMD). Psychological states and OHRQoL were examined with the Depression, Anxiety, Stress Scales-21 (DASS-21) and Oral Health Impact Profile for TMDs (OHIP-TMD). Data were subjected to chi-square, Kruskal-Wallis/Mann-Whitney U tests and Spearman's correlation (alpha = .05). Results A total of 961 participants with a mean age of 32.99 +/- 13.14 years (71.19% women) were assessed. Frequencies of the various TMD categories were as follows: no TMD/controls (12.07%), mild TMD (24.56%), moderate TMD (40.37%) and severe TMD (23.00%). The three most common TMD-related symptoms were TMJ noises, mouth opening difficulty and muscle pain. Participants with moderate/severe TMD presented a higher proportion of intra-articular and/or combined disorders. They reported significantly higher levels of depression, anxiety, stress and poorer OHRQoL than their counterparts with no/mild TMD (p < .001). Moderate-to-strong correlations were observed between FAI and DASS-21/OHIP-TMD scores (rs = 0.42-0.72). Conclusions Patients with moderate/severe TMD had significantly higher levels of psychological disturbance and poorer OHRQoL. As OHRQoL and psychological states are correlated, psychological well-being must be considered when managing patients with moderate/severe TMDs.
Somatosensory Abnormalities in Chinese Patients with Painful Temporomandibular Disorders

Abstract:Background: The somatosensory phenotype of Chinese temporomandibular disorders (TMD) patients is not sufficiently studied with the use of contemporary techniques and guidelines.Methods: A standardized quantitative sensory testing (QST) battery consisting of 13 parameters with a stringent statistical protocol developed by the German Research Network on Neuropathic Pain was performed over the most painful and corresponding contralateral sites as well as the right hand of 40 Chinese patients with TMD and pain classified according to the Diagnostic Criteria for TMD (DC/TMD). The same QST protocol was performed bilaterally over the infraorbital, mental, and hand regions of 70 age-and gender-stratified healthy Chinese controls. Z-scores and loss/gain scores were computed for each TMD patient.Results: For patients, 82.5 % had somatosensory abnormalities in the painful facial region, while 60.0 % had abnormalities confined to the right hand. The most frequent abnormalities were somatosensory gain to pinprick (35.0 %) and pressure (35.0 %) stimuli, somatosensory loss to pinprick (25.0 %), cold (22.5 %), and heat (15.0 %) nociceptive stimuli. The most frequent loss/gain score was L0G2 (no somatosensory loss combined with a gain of mechanical somatosensory function) for both the facial (40.0 %) and hand (27.5 %) regions. Involving side-to-side differences in the evaluation increased the diagnostic sensitivity by 2.5-25.0 % across different parameters.Conclusions: Somatosensory abnormalities were commonly detected in Chinese TMD pain patients both within and outside the primary painful region, strongly indicating disturbances in the central processing of somatosensory stimuli. The individual variations in somatosensory abnormalities indicate a possible need for development of individualized TMD pain management.
The Association Between the Central Sensitisation Inventory and Temporomandibular Disorders Is Confounded by Migraine, Depression, Widespread Pain and Parafunction. A Cross-Sectional Telehealth Study

Abstract:Background: Temporomandibular disorders (TMDs) are conditions that involve the temporomandibular joints, masticatory muscles, and associated tissues, causing orofacial pain. Central sensitisation (CS) is a relevant part of the TMD pathophysiology. Migraine, psychological aspects, parafunctional oral habits and widespread pain are commonly associated with both TMD and CS and could confound the association between them. Objectives: To investigate the association between painful TMD and the Central Sensitisation Inventory (CSI) score, and to assess the presence of confounders in this association. Methods: Cross-sectional study that assessed women with and without orofacial pain complaints using telehealth. The TMD Pain Screener and an online physical examination determined the presence of painful TMD. The following questionnaires were applied: CSI, Headache Screening Questionnaire, Generalized Anxiety Disorder-7, Patient Health Questionnaire-9, Oral Behavior Checklist, Jaw Functional Limitation Scale and the Nordic musculoskeletal questionnaire. A single regression investigated the association between the CSI score and TMD, and a multiple regression investigated the effect of the other outcomes as possible confounders. Confounding was considered present when the association between TMD and the CSI score changed more than 10% after adding a possible confounder to the regression model. Results: Forty-two women with painful TMD and 53 without TMD were included. There was a significant association between the CSI score and the presence of painful TMD (R2 = 0.639; p < 0.001). This association changed when the following outcomes were added to the model: presence of migraine, symptoms of depression, widespread pain and parafunctional oral habits. Conclusion: The positive association between TMD and the CSI score was confounded by migraine, symptoms of depression, widespread pain and parafunctional oral habits.

The participants were requested to complete four tests including the NEO Five Factor Inventory (NEO-FFI) to assess personality factors and three Axis II DC/TMD tools; the Graded Chronic Pain Scale (GCPS) Version 2.0 to assess TMD pain intensity and pain-related disability, Patient Health Questionnaire-4 (PHQ-4) to assess psychological distress due to TMD pain, and Generalized Anxiety Disorder-7 (GAD-7) to assess patients' stress reactivity. Results showed that 49.2% of the participants experienced high intensity of characteristic pain, 14.2% reported pain related disability more than 30 days (grade 3 disability days) and had disability scores of 70 or above (grade 3 disability scores), 16.7% demonstrated severe distress, and 18.3% reported severe anxiety due to TMD pain. Females scored higher on all these variables than males ( P < .05). Multiple regression analyses indicated that higher extraversion scores, higher agreeableness scores and being a female were associated with higher intensity of characteristic pain ( p < .05). Also, higher neuroticism and agreeableness scores were associated with more disability days ( p < .05). In addition, higher agreeableness scores were associated with higher disability scores and disability point values ( p < .05). Furthermore, lower neuroticism scores and being a female were associated with higher PHQ total scores, while being a female and having a lower level of education were associated with higher GAD total scores ( p < .05). In conclusion, personality profiles and gender significantly impact the DC/TMD Axis II scores of psychosocial impairment among patients with pain related TMD.

Abstract:OBJECTIVETo evaluate the level of psychosocial difficulties in patients with temporomandibular disorders (TMD) by using the Symptom Checklist 90 revised (SCL-90-R), and determine whether there is an association between psychosocial status and chronic pain severity.METHODSOne hundred and sixty-two female patients who came to the Center for TMD and Orofacial Pain of Peking University Hospital of Stomatology were randomly included in this study. SCL-90-R was used to assess the psychological status of the TMD patients. This instrument was also used to assess psychological status and pain-related disability in chronic pain patients and a seven-item questionnaire was used to grade chronic pain severity (GCP). Univariate analysis of variance and correlation analysis were performed to determine the association between psychological status and chronic pain severity.RESULTSA total of 126 female TMD patients complained of pain in the temporomandibular joint. One hundred and sixteen patients had low disability, with 83 (51.3%) having low intensity pain (Grade I), 30 (18.5%) having high intensity pain (Grade II) and only 13 (8%) with high disability (Grade III pain). The patients were divided into four groups according to their GCP scores. The scores of all the factors of SCL-90-R were significantly different among the groups. The incidence of psychosocial symptoms was also significantly different among the groups. The psychosocial status of patients with high disability was the worst among the four groups.CONCLUSIONThe GCP severity is a suitable tool to assess the psychosocial and physical status of TMD chronic pain. Some TMD patients have psychological problems and symptoms of psychopathology, especially those with high disability. Physicians should keep this possibility in mind when treating patients with TMD chronic pain.
Conceptualising TMD symptom burden in youths: Symptom duration, frequency, intensity and interference

Abstract:Objective This study aimed to conceptualise Temporomandibular disorder (TMD) symptom burden and severity and explored their interrelationships with somatic symptoms and psychological distress. Methods Participants were recruited from a local polytechnic. The quintessential five TMD symptoms (5Ts) of the Diagnostic Criteria for TMDs (DC/TMD) were appraised and extended to evaluate the duration, frequency, intensity and interference of discrete TMD symptoms. Global TMD severity (GS) was computed by totaling the points for all TMD symptoms and dimensions. TMD (TS) and somatic symptom (SS) burden were assessed based on the Somatic Symptoms Scale‐8, while psychological distress was measured with the Depression, Anxiety and Stress Scales‐21. Statistical analyses were performed using Kruskal–Wallis/Dunn tests and Spearman's correlation (α = .05). Result Of the 366 eligible participants (mean age 19.1 ± 2.3 years), 51.4% were 5Ts‐negative and 48.6% were 5Ts‐positive. Among the 5Ts‐positive individuals, 25.3%/64.0% were 'bothered a little' whereas 4.5%/10.7% were 'bothered a lot' by TMD pain/headache. Correspondingly, 32.6%/12.4%/5.1% were 'bothered a little' while 2.8%/2.8%/1.1% were 'bothered a lot' by TMJ sounds/closed/open locking. TS burden was moderate‐to‐strongly correlated to aggregate symptom duration, frequency, intensity, interference, GS and SS burden (rs = .50–.88). While TS burden and GS were weakly associated with psychological distress (rs = .18–.36), SS burden was moderately related to depression, anxiety and stress (rs = .47–.53). Conclusions TS burden can serve as a proxy for global TMD severity and may be more meaningful than the mere presence of TMD symptoms in clinical and research settings.
