Abstract:Background Studies on temporomandibular disorder (TMD) severity in patient populations are scarce. Objectives This study sought to compare the psychological states and oral health-related quality of life (OHRQoL) among patients with differing TMD severity. Methods Adult patients (>= 18 years old) with and without (controls) TMDs were recruited from the TMD/oro-facial pain centre and prosthodontics department, respectively. The presence and severity of TMDs were established with the Fonseca Anamnestic Index (FAI), and TMD diagnoses were confirmed with the Diagnostic Criteria for TMDs (DC/TMD). Psychological states and OHRQoL were examined with the Depression, Anxiety, Stress Scales-21 (DASS-21) and Oral Health Impact Profile for TMDs (OHIP-TMD). Data were subjected to chi-square, Kruskal-Wallis/Mann-Whitney U tests and Spearman's correlation (alpha = .05). Results A total of 961 participants with a mean age of 32.99 +/- 13.14 years (71.19% women) were assessed. Frequencies of the various TMD categories were as follows: no TMD/controls (12.07%), mild TMD (24.56%), moderate TMD (40.37%) and severe TMD (23.00%). The three most common TMD-related symptoms were TMJ noises, mouth opening difficulty and muscle pain. Participants with moderate/severe TMD presented a higher proportion of intra-articular and/or combined disorders. They reported significantly higher levels of depression, anxiety, stress and poorer OHRQoL than their counterparts with no/mild TMD (p < .001). Moderate-to-strong correlations were observed between FAI and DASS-21/OHIP-TMD scores (rs = 0.42-0.72). Conclusions Patients with moderate/severe TMD had significantly higher levels of psychological disturbance and poorer OHRQoL. As OHRQoL and psychological states are correlated, psychological well-being must be considered when managing patients with moderate/severe TMDs.

Abstract:Characteristics of painful temporomandibular disorders and their influence on jaw functional limitation and oral health‐related quality of life. Background There is limited knowledge about the impact of painful temporomandibular disorders (TMDs) and pain characteristics on jaw functional limitation and oral health‐related quality of life (OHRQoL) in TMD patients. Objectives The influence of painful TMDs and pain characteristics on jaw functional limitation and OHRQoL was investigated. Inter‐relationships between limitation in jaw function and various OHRQoL domains, along with facial pain attributes predicting impaired jaw function and diminished OHRQoL were also examined. Methods TMD patients were recruited from a university‐based hospital. A comprehensive questionnaire comprising demographic variables, the DC/TMD Symptom Questionnaire, Graded Chronic Pain Scale, Jaw Functional Limitation Scale‐8 (JFLS‐8) and Oral Health Impact Profile‐TMD (OHIP‐TMD) was administered. Participants underwent a protocolized physical examination, and TMD diagnoses were determined utilising the DC/TMD algorithms. Participants were subsequently stratified into intra‐articular/pain‐related/combined TMD groups, as well as no TMD pain, acute/chronic pain and low/high‐intensity pain groups. Data were assessed using non‐parametric and hierarchical linear regression analyses (α = .05). Results The final sample consisted of 280 participants (mean age 31.2 (SD 11.8) years; 79.3% women). Significant differences in pain characteristics, JFLS‐8, and global OHIP scores were observed across the various TMD subtypes, pain chronicity and pain intensity categories. Pain intensity and pain‐related interference exhibited moderate correlations with JFLS‐8 and global OHIP scores (rs = 0.53–0.60). Moderate associations were also noted between JFLS‐8 and global OHIP, as well as most OHIP domains (rs = 0.42–0.64). Both jaw functional limitation and OHRQoL were predicted by sex, pain intensity and pain‐related interference. Conclusions Sex, pain intensity and pain‐related interference are key determinants for both impaired jaw function and diminished OHRQoL, with pain‐related interference exerting a more pronounced effect.

Subtypes of Acute and Chronic Temporomandibular Disorders: Their Relation to Psychological and Sleep Impairments.

Abstract:OBJECTIVES To determine the differences in psychological states and sleep quality in patients with various Temporomandibular disorder (TMD) subtypes, and to ascertain the relationships between TMD duration with psychological and sleep impairments. METHODS A total of 830 TMD patients were recruited categorized into pain-related (PT), intra-articular (IT), and combined (CT) TMD groups. Each group was further divided into acute and chronic subtypes. The Depression, Anxiety, Stress Scales-21 (DASS-21), and Pittsburgh Sleep Quality Index (PSQI) were used to assess emotional states and sleep problems. RESULTS Although chronic TMDs generally had higher levels of anxiety, depression, stress and sleep impairments than acute TMDs, significant differences were only observed for the PT group. Ranking of the mean depression, anxiety, and stress scores were as follows: Acute TMDs: CT > PT > IT; Chronic TMDs: PT > CT > IT. For both acute and chronic TMDs, the ranking of mean PSQI global and component scores was: PT ≥ CT ≥ IT. Logistic regression analyses indicated that stress (ORs=4.40) and depression (ORs=2.82) increased the risks of chronic pain-related TMDs (P<0.05). CONCLUSIONS Chronic pain-related TMDs are associated with high levels of psychological distress and poorer sleep, while chronic intra-articular TMDs are not. Stress and depression increased the probability of chronic pain-related TMDs.
Temporomandibular Disorder Severity and Diagnostic Groups: Their Associations with Sleep Quality and Impairments

Abstract:Objectives: This study evaluated the impact of TMD severity on sleep quality and established the associations between TMD diagnostic groups/pain subtypes and sleep impairments. Methods: A total of 1151 patients from a tertiary Dental Hospital were screened for eligibility. Consenting subjects who met the inclusion criteria were directed to complete a general/health questionnaire, the Fonseca Anamnestic Index (FAI), Diagnostic Criteria for Temporomandibular disorders (DC/TMD) Symptom Questionnaire, and Pittsburgh Sleep Quality Index (PSQI) at their intake visit. Patients who screened positive for TMDs with the FAI were subjected to a protocolized physical examination and TMD diagnostic groups/subtypes were subsequently derived based on the DC/TMD "diagnostic tree" and algorithms. Statistical analyses were conducted using non-parametric methods and logistic regression (alpha = 0.05). Results: The final sample consisted of 845 subjects with TMDs and 116 TMD-free controls. The mean age of the TMD and TMD-free subjects were 33.17 +/- 13.55 and 31.66 +/- 9.50 years. Subjects with severe and moderate TMDs had significantly greater global PSQI scores than those with mild and no TMDs (p < 0.001). Those with pain-related, intra-articular, and combined TMDs reported significantly poorer sleep quality than those with no TMDs (p < 0.001). Moreover, subjects with myalgia and myalgia plus arthralgia presented significantly greater sleep impairments than their counterparts with intra-articular disorders (p < 0.001). Multivariate logistic regression indicated that pain-related (OR = 3.23; CI = 1.69-6.14) and intra-articular TMDs (OR = 1.91; CI = 1.15-3.16) were most related to poor sleep. Conclusions: Sleep quality worsened with increasing TMD severity and the presence of painful and intra-articular TMDs increased the likelihood of poor sleep. (C) 2021 Elsevier B.V. Age-related Differences in Diagnostic Categories, Psychological States and Oral Health-Related Quality of Life of Adult Temporomandibular Disorder Patients

Abstract:Objectives The peak prevalence of temporomandibular disorders (TMDs) may occur in middle age. This study determined the proportion of matured adults seeking TMD treatment and compared their diagnostic, psychological and oral health-related quality-of-life (OHRQoL) profiles to younger patients. Methods Adult subjects were recruited from a tertiary TMD centre and assigned to three age groups, namely 18-44 years (young adults [YA]), 45-64 (middle-aged adults [MA]) and >= 65 (old adults [OA]). TMD diagnoses were established with the Diagnostic Criteria for TMDs and categorised as pain-related (PT), intra-articular (IT) and combined (CT) TMDs. Psychological states and OHRQoL were assessed with the Depression, Anxiety, and Stress Scale-21 (DASS-21) and Oral Health Impact Profile-TMDs (OHIP-TMDs). Demographic, DASS-21, and OHIP-TMDs data were analysed using chi-square test, one-way ANOVA and Pearson's correlation (P < .05). Results Middle-aged (19.7%; 136/692) and old (4.0%; 28/692) adults comprised about a quarter of the TMD patients. Although gender distribution was comparable, significant differences in TMD categories were observed (P < .001). Pain-related TMDs were more prevalent in the MA/OA groups while intra-articular TMDs were more frequent in the YA group. No significant difference in DASS-21 and total OHIP scores was noted among three groups. However, the MA and OA groups had significantly lower OHRQoL in the physical pain domain. Correlations between DASS-21 and OHIP-TMDs scores varied with age and ranged from r(s) = 0.47-0.92. Conclusions Matured patients constituted a quarter of TMD cohort and presented higher frequencies of painful TMDs. They have similar psychological profiles to younger patients but experienced lower OHRQoL in physical pain domain.
Sleep Disturbance and Psychologic Distress: Prevalence and Risk Indicators for Temporomandibular Disorders in a Chinese Population.

Abstract:AIMS:To investigate the prevalence of sleep disturbance and psychologic distress in a population of Chinese patients with temporomandibular disorders (TMD) and whether sleep disturbance and psychologic distress are risk indicators for TMD.METHODS:Validated Chinese versions of the Self-Rating Scale of Sleep (SRSS) and Depression, Anxiety and Stress Scales-21 (DASS-21) were used to measure sleep disturbance and psychologic distress of 510 TMD patients with a mean (± SD) age of 31.06 ± 14.40 years. TMD signs/symptoms and sociodemographic data were also collected. The patients were divided into seven diagnostic groups based on the Research Diagnostic Criteria for Temporomandibular Disorders (RDC/TMD). For statistical analysis, the patients were subsequently grouped into those with (n = 128) and without (n = 382) myofascial pain. The data were analyzed using chi-square tests, independent-samples t test, as well as stepwise logistic regression at a significance level of P < .05.RESULTS:The prevalence of moderate to severe sleep disturbance and psychologic distress was significantly higher in the myofascial pain group than in the non-myofascial pain group (P < .05). Stepwise logistic regression analysis demonstrated that sleep disturbance and anxiety were possible risk indicators for myofascial pain, with odds ratios of 2.41 and 4.10, respectively (P < .05).CONCLUSION:The Chinese population of TMD patients frequently reported a disturbed sleep condition and psychologic distress symptoms. Sleep disturbance and psychologic distress symptoms are possible risk indicators for myofascial pain in this population.

Abstract:Background: Literature concerning Temporomandibular disorders (TMDs) and the Covid-19 pandemic is limited and disparate findings related to TMD frequencies, psychological distress, and quality of life were presented. This study investigated the prevalence of painful Temporomandibular disorders (TMDs) and compared the psychological, sleep, and oral health-related quality of life profiles of patients seeking TMD care before and during the Covid-19 pandemic. Methods: Data were accrued from consecutive adult patients 12 months before (BC; control) and during (DC; case group) the Covid-19 pandemic. The Diagnostic Criteria for TMDs (DC/TMD), Depression, Anxiety, Stress Scales (DASS)-21, Pittsburgh Sleep Quality Index (PSQI), and Oral Health Impact Profile (OHIP)-TMDs were utilized and statistical analysis was performed using Chi-square/non-parametric tests (α = 0.05). Results: The prevalence of painful TMDs was 50.8% before and 46.3% during the pandemic. Significant differences in PSQI and OHIP component scores were discerned between the BC and DC groups contingent on TMD pain. Total-DASS was moderately correlated to total-PSQI/OHIP (rs = 0.41-0.63). Conclusion: The covid-19 pandemic did not appear to exacerbate psychological distress but affected sleep and increased unease over TMD dysfunction.
Degenerative Temporomandibular Joint Diseases and Their Relation with Sleep and Emotional Disturbance

Abstract:Objective The relation of degenerative temporomandibular joint (TMJ) diseases (DJDs) with sleep and emotional disturbance were investigated. Methods CBCT examination of patients (n = 358) with DC/TMD-defined intra-articular temporomandibular disorders was performed and stratified into NN: no DJD and no arthralgia; NA: no DJD with arthralgia; TO: osteoarthrosis; and TR: osteoarthritis. Sleep and emotional disturbance were assessed with the Pittsburgh Sleep Quality Index (PSQI) and Depression Anxiety Stress Scale-21 (DASS-21). Data were evaluated using non-parametric and multivariate logistic regression analyses (alpha = 0.05). Results Distributions of NN, NA, TO, and TR groups were 23.2%, 27.1%,19.0%, and 30.7%, respectively. No significant differences in total-PSQI/DASS scores were detected among the four groups. The presence of pain and stress predicted poor quality sleep with odds ratios of 10.75 and 1.07, accordingly. Conclusion Sleep quality was affected more by arthralgia and stress than the presence of TMJ DJDs.
Oral Health–related Quality of Life of Patients with Acute and Chronic Temporomandibular Disorder Diagnostic Subtypes

Abstract:Background. Studies have indicated the negative effects of temporomandibular disorders (TMDs) on oral health-related quality of life (OHRQoL). The authors investigated the OHRQoL of patients with acute and chronic TMD subtypes. Methods. The authors recruited a total of 830 patients. They derived TMD diagnoses using the Diagnostic Criteria for TMDs protocol involving symptom history, physical examination, and diagnostic imaging as indicated. The authors categorized patients into acute ( 3 months) or chronic ( 3 months) pain-related TMD (PT), nonpainful intra-articular TMD (IT), and combined TMD (CT) groups. They also gathered sociodemographic information and assessed OHRQoL with the Oral Health Impact Profile (OHIP)-TMDs. The authors evaluated data using 2-way analysis of variance and Bonferroni test and multiple regression analysis. Results. Patients in the chronic PT and CT subgroups had significantly higher mean global OHIP scores than their acute counterparts. The authors observed significant acute-chronic differences in OHIP-TMDs domain scores in 5 and 2 domains for the PT and CT groups, respectively. Patients in the acute IT group had significantly higher functional limitation scores than those in the chronic IT group. The ranking of mean global scores, in descending order was CT, PT, and IT for acute TMDs and PT, CT, and IT for chronic TMDs, with significant differences observed among the 3 TMD subtypes (P < .001). Conclusions. Both TMD chronicity and subtypes influenced OHRQoL. Painful TMDs (PT and CT) were associated with significantly poorer OHRQoL than nonpainful TMDs. TMD chronicity appeared to affect OHRQoL only for the painful TMD conditions. Future work on the impact of TMDs on OHRQoL should strive to stratify patients by TMD chronicity and subtypes. Practical Implications. TMD chronicity and subtypes influence the impact of TMDs on OHRQoL. Given that chronic painful TMDs impair quality of life, early biopsychosocial intervention of acute TMD pain is important for minimizing chronification and OHRQoL deterioration.

Abstract:Purpose: To establish the interrelationships between temporomandibular disorders (TMDs), pain, and otologic and psychologic comorbidities in Asian youths. Materials and methods: Youths from a local polytechnic, aged 17 to 24 years old, were enrolled and an electronic survey encompassing demographic variables-the DC/TMD TMD pain screener (TPS), Short- form Fonseca Anamnestic Index (SFAI), modified Maciel's Otologic/Pain Symptom Inventory, and Patient Health Questionnaire-4-was administered. Par.cipants were subsequently categorized into groups: no TMD pain (NP), with TMD pain (WP), no TMDs (NT), and with any TMDs (WT). Data were evaluated with chi-square test, nonparametric, and logistic regression analyses (α = .05). Results: Among the 198 participants (mean age 18.8 ± 1.7 years), 11.1% had painful TMDs and 18.2% experienced TMD pain or dysfunction. Significant differences in total-otologic symptoms (total-OS), vertigo, and dizziness plus otalgia, tinnitus, and hearing loss scores were observed between the WP-NP and WT-NT groups correspondingly. While total-comorbid pain (total-CP) and psychologic distress (total-PD) scores varied substanally between the WT-NT group, only a significant difference in total-PD scores was discerned between the WP-NP group. Total-OS scores were moderately correlated to TPS/SFAI, total-CP, and total-PD scores, and psychologic distress was a risk factor for painful TMDs. Conclusions: Otologic and pain comorbidities were prevalent among Asian youths with TMDs and appear to be interrelated. The complex interaction of TMDs with pain, otologic, and psychologic comorbidities must be considered when caring for youths with multiple somatic complaints.

Abstract:OBJECTIVE:To explore the relationship betweensleep/psychological distress and temporomandibular disorders (TMD) and to discuss the possibility of disturbed sleep and psychological distress as risk indicators in relation to TMD in a Chinese population. METHODS:The standardized and validated self-reported Chinese version questionnaires including Pittsburgh sleep quality index (PSQI) and depression, anxietyandstress scales-21 (DASS-21) were used to measure sleep quality and psychological distress. A total of 755 TMD patients (172 males and 583 females)with a mean age (29.99 ± 13.60) years were included in the study. The patients were divided into 7 diagnostic groups based on the Research Diagnostic Criteria for Temporomandibular Disorders (RDC/TMD), including Group I(exclusively myofascial pain), Group II(exclusively disc displacement), Group III (exclusively arthralgia or degenerative joint disease), Group IV (myofascial pain plus disc displacement), Group V (myofascial pain plus arthralgia or degenerative joint disease), Group VI (disc displacement plus arthralgia or degenerative joint disease) and Group VII (myofascial pain plus disc displacement plus arthralgia or degenerative joint disease). For statistical analysis, the patients were subsequently grouped into those with (181 patients) and without (574 patients) myofascial pain. Chi-square tests, independent-samples t test, partial correlation as well as stepwise Logistic regression analysis were used to analyze the data, using software SPSS 20.0 and P<0.05 was of significance. RESULTS:The prevalence of moderate to (extremely) severe disturbed sleep and psychological distress was significantly higher in the myofascial pain group (27.1%, 28.7%, 60.8% and 32.0%) than in the non-myofascial pain group (disc displacement and arthralgia or degenerative joint disease, 11.1%, 10.1%, 27.4% and 11.0%, P<0.05).The comorbidity of self-reported disturbed sleep and psychological distress was significantly higher in the myofascial pain patients than those without myofascial pain (P<0.05). Stepwise logistic regression analysis demonstrated that disturbed sleep (OR=1.74), more specifically, subjective sleep quality (OR=1.69) and sleep disturbance (OR=1.63) respectively, anxiety (OR=3.14) and stress (OR=2.15) were possible risk indicators for myofascial pain, and the results were still significant even after controlling for age, sex, educational level, disease duration, sleep quality, depression, anxiety and stress respectively (P<0.05). CONCLUSION:Disturbed sleep and psychological distress symptoms are common in TMD patients. Disturbed sleep, anxiety and stress are possible risk indicators for myofascial pain, compared with disc displacement and arthralgia or degenerative joint diseases.
Functional, Physical and Psychosocial Impact of Degenerative Temporomandibular Joint Disease

Abstract:OBJECTIVES:This study evaluated the functional, physical and psychosocial impacts of TMJ degenerative joint disease (DJD). The bearing of TMJ osteoarthrosis/osteoarthritis and early/late TMJ DJD on oral health-related quality of life (OHRQoL) were also compared.METHODS:Participants were enrolled from a TMD/oro-facial pain centre. Those diagnosed with intra-articular conditions based on the Diagnostic Criteria for Temporomandibular disorders (DC/TMD) were subjected to CBCT assessment and categorised into four discrete groups: NN-no TMJ DJD and no arthralgia; NA-no TMJ DJD with arthralgia; TO-TMJ osteoarthrosis; and TR-TMJ osteoarthritis. The TO/TR groups were subdivided into early/late TMJ osteoarthrosis (EO/LO) and osteoarthritis (ER/LR). OHRQoL was examined using the OHIP-TMD, and data were appraised with the Kruskal-Wallis/Mann-Whitney U tests (α = 0.05).RESULTS:The study participant (n = 358) had a mean age of 31.85 ± 12.39 years (85.6% women). Frequencies of the TMD groups were as follows: NN-23.2%; NA-27.1%; TO-19.0%; and TR-30.7%. Participants with TR/NA had significantly worse OHRQoL than those with TO/NN. Additionally, participants with ER/LR reported significantly poorer OHRQoL than their counterparts with EO/LO. For all TMD groups and TMJ DJD subgroups, the psychological discomfort domain was generally the most impaired. Differences in global OHIP scores were significant between participants with and without arthralgia (i.e., NA-NN, ER-EO and LR-LO).CONCLUSIONS:The presence of TMJ pain appeared to impair OHRQoL more than the severity of TMJ DJD. As psychological domains were most impacted, psychosocial care should be incorporated when managing patients with painful TMJ DJD.

Objectives: The aim of this observational, cross-sectional study was to evaluate the level of satisfaction with life among Polish patients with TMD and to assess the influence of pain severity on this parameter. A secondary goal was to investigate sleep quality within this patient group and explore its relationship with pain. Material and methods: A total of 219 patients from the Outpatient Clinic for Temporomandibular Disorders at the University Dental Polyclinic in Wroclaw, Poland, participated in this study. These individuals underwent a clinical examination using the Diagnostic Criteria for Temporomandibular Disorders (DC/TMD) protocol and completed 2 validated questionnaires, namely the Satisfaction With Life Scale (SWLS) and the Pittsburgh Sleep Quality Index (PSQI). Furthermore, the patients were assessed for the severity of masseter muscle pain (MMP) and temporal muscle pain (TMP), and the average pain in these muscles (AMP) was calculated. Subsequently, a statistical analysis was performed on the collected data. Results: The group of patients with average satisfaction with life exhibited significantly higher levels of MMP (p = 0.025) and AMP (p = 0.044) as compared to the high-satisfaction group. Regarding sleep quality, 50.23% of the patients experienced poor sleep quality. Poor sleep quality was found to be statistically associated with higher levels of TMP (p = 0.032) and AMP (p = 0.028). Moreover, women demonstrated significantly worse sleep quality as compared to men (p = 0.002). The findings indicate that PSQI has a greater impact on SWLS than vice versa. Conclusions: Due to a large number of TMD patients experiencing poor sleep quality and the associated reduced life satisfaction, these parameters should be considered as influential factors that modify the management of patients with TMD.
Catastrophizing and Hypervigilance Influence Subjective Sleep Quality in Painful TMD Patients

Abstract:Aims: To determine sleep quality and associated factors in a group of patients with painful TMDs. Methods: The medical records of 80 patients with arthralgia and/or myofascial pain were reviewed and compared to a healthy control group. Data about sex, age, subjective pain, physical activity, social activity, subjective sleep quality (Pittsburgh Sleep Quality Index [PSQI]), pain vigilance (Pain Vigilance and Awareness Questionnaire [PVAQ]), and pain catastrophizing (Pain Catastrophizing Scale [PCS]) were collected. Relationships between PSQI, age, pain intensity, PVAQ, and PCS in the TMD group were also analyzed. Data from the control group were used to transform the PSQI results into T-scores, which were then used to divide the TMD group into two subgroups: normal and impaired sleep. Results: TMD patients presented a significantly higher (P < .001) PSQI score than the control group. Also, in the TMD group, there was a low to moderate correlation between PSQI and pain intensity and a significant correlation between PVAQ and PCS. The impaired sleep group presented a significantly higher (P < .001) PSQI T-score than the normal sleep group. Univariate analysis showed that subjective pain, social activity, and the PCS total and subscale scores differed significantly between the different PSQI T-score groups. The comparison between TMD pain patients and control subjects showed a significantly higher prevalence of T-score discordance in almost all PSQI components in TMD patients with impaired sleep. Conclusion: Subjective sleep quality in painful TMD patients could be associated with and influenced by psychosocial factors (catastrophizing and hypervigilance), social activity, and pain intensity.

Abstract:Background Whether pain, jaw function and quality of life are correlated with disc positions is controversial, and similar studies evaluating disc positions by magnetic resonance imaging (MRI) are very limited. Objective This study evaluated the pain, mandibular function and quality of life of the temporomandibular disorders (TMDs) patients with different disc positions according to MRI, and the relationship among them. Methods Three hundred and thirty‐five participants were included. Patients completed questionnaires included the Numeric Rating Scale (NRS), Jaw Functional Limitation Scale 8‐item (JFLS‐8), the Generalised Anxiety Disorder 7‐item (GAD‐7), Patient Health Questionnaire 9‐item (PHQ‐9) and Oral Health Impact Profile for TMD (OHIP‐TMD). MRI was conducted to evaluate these diagnoses, resulting in the identification of three distinct categories: normal positioning (NP), disc displacement with reduction (DDwR) and disc displacement without reduction (DDwoR). Results Participants had the mean age of 28.55 ± 11.10 years (80.90% women). DDwR and DDwoR had a higher percentage of females compared with NP. Significant differences existed among patients in all questionnaires. The DDwoR group had significantly the highest pain, functional limitation and the worst quality of life. Moreover, they experienced the most difficulties in chewing tough foods, yawning, experiencing pain and psychological discomfort. Moreover, the multivariate regression showed that age, female gender, diagnosis as DDwoR, GAD‐7 and PHQ‐9 were significantly linked to higher functional limitation. Worse quality of life was associated with age, diagnosis as DDwoR, GAD‐7 and PHQ‐9. Conclusions Among patients with different disc positions, DDwoR showed the highest pain, functional limitation and the worst quality of life. Also, NP showed a proportion of chronic pain. Physical pain, psychological discomfort and chewing tough food were regarded as the most impaired. Women who experience anxiety and depression tended to have a higher propotion of dysfunction and a lower quality of life.

Previous studies have documented an association between sleep and TMD. Objectives This study aimed to further explore the association of night‐time sleep and daytime napping with painful TMD. Methods A total of 419 patients (aged 31.88 ± 11.54 years with women forming 85.4%) from a TMD/Orofacial Pain center were enrolled. Patients' sleep conditions were evaluated with the Pittsburgh Sleep Quality Index (PSQI) questionnaire, and information on night‐time sleep duration, napping duration and napping frequency was interviewed. TMD was diagnosed according to the Diagnostic Criteria for TMD protocol and stratified into myalgia (muscle pain), arthralgia (joint pain) and combined (muscle and joint pain) subgroups. The severity of TMD was measured with the Fonseca Anamnestic Index (FAI) questionnaire. Abstract:Objective. In this case-control study, we investigated the presence of differing numbers and types of temporomandibular disorder (TMD) symptoms, their association with psychological distress, and their impact on oral health-related quality of life (OHRQoL). Study Design. We recruited a total of 814 participants with TMD and 147 control subjects. The participants were instructed to complete the Diagnostic Criteria for Temporomandibular Disorders Symptom Questionnaire; the Depression, Anxiety and Stress Scale-21; and the Oral Health Impact Profile- Temporomandibular Disorders. The participants were subsequently categorized by their number and type of TMD symptoms. Data were analyzed with the Kruskal-Wallis/Mann-Whitney U test (alpha =.05). Results. The mean age of the participants (N = 961) was 32.99 +/- 13.14 years, and 79.2% were women. Participants with more and all types of TMD symptoms generally exhibited significantly higher levels of psychological distress and worse OHRQoL (P <.001). Those with TMD pain plus temporomandibular joint (TMJ) sounds/dysfunction had significantly greater psychological distress, whereas those with painful symptoms and TMJ dysfunction experienced significantly more impairment in OHRQoL than individuals with only TMJ sounds. Conclusions. Psychological states and OHRQoL are influenced by the number and type of TMD symptoms. Individuals with more and all types of pain-related TMD symptoms with/without intra-articular features had greater psychological distress and OHRQoL impairment.

Abstract:OBJECTIVE:To investigate the pain features of temporomandibular disorders (TMD) by qualitatively and quantitatively analyzing patient multi-dimension sensations.METHODS:Two hundred and fifty patients with painful TMD from January to December 2005 were included, short-form mcgill pain questionnaire (SF-MPQ) and visual analog scale (VAS) were administered to assess patients' sensation, affection and intensity of pain. The data were analyzed by SAS 8.0 software.RESULTS:All patients were assigned to three groups including 139 joint pain, 47 muscle pain and 64 both joint and muscle pain group. In joint pain group, the total number of descriptors was 250, 1.80 in average; in muscle pain group, the total number was 99, 2.11 in average; in both joint and muscle pain group, the total number was 107, 1.64 in average. The sensitive descriptors most frequently chosen in all three groups were "aching", "heavy", and "tender". "Tiring-exhausting" and "sickening" were high frequency descriptors in affective items. There were higher affective scores in the muscle pain group than in others. Muscle pain group had a higher VAS score at rest than the other two (P < 0.05), but had a lower VAS score during function than the other two (P < 0.001). All three groups usually had no pain at rest, and complained a slight to moderate pain during function.CONCLUSIONS:TMD pain was generally slight to moderate; "aching", "heavy", and "tender" were the most frequently sensitive descriptors, while unpleasant feelings were described as "tiring-exhausting" and "sickening"; pain generally occurred or exacerbated during mandibular function; compared to joint pain, muscle pain had its own features.
