Abstract:A study was undertaken at Red Cross Children's Hospital to assess how aware parents were of motor vehicle accident (MVA) claim procedures on behalf of their children. Of 160 questionnaires sent out to the parents of road traffic injury (RTI) victims, 100 were answered. Sixty-six per cent of parents who responded had an income below R500 per month. The identity of the motorist responsible for injuring the child was known to only 26 parents. Third party damages were claimed by only 32 parents or guardians; the remaining 68 failed to do so principally through lack of awareness. The implications of these findings for both RTI victims and the hospitals where they are treated are discussed here.
A deep learning model based on the attention mechanism for automatic segmentation of abdominal muscle and fat for body composition assessment

Abstract:Background: Quantitative muscle and fat data obtained through body composition analysis are expected to be a new stable biomarker for the early and accurate prediction of treatment-related toxicity, treatment response, and prognosis in patients with lung cancer. The use of these biomarkers can enable the adjustment of individualized treatment regimens in a timely manner, which is critical to further improving patient prognosis and quality of life. We aimed to develop a deep learning model based on attention for fully automated segmentation of the abdomen from computed tomography (CT) to quantify body composition. Methods: A fully automatic segmentation deep learning model was designed based on the attention mechanism and using U-Net as the framework. Subcutaneous fat, skeletal muscle, and visceral fat were manually segmented by two experts to serve as ground truth labels. The performance of the model was evaluated using Dice similarity coefficients (DSCs) and Hausdorff distance at 95th percentile (HD95). Results: The mean DSC for subcutaneous fat and skeletal muscle were high for both the enhanced CT test set (0.93±0.06 and 0.96±0.02, respectively) and the plain CT test set (0.90±0.09 and 0.95±0.01, respectively). Nevertheless, the model did not perform well in the segmentation performance of visceral fat, especially for the enhanced CT test set. The mean DSC for the enhanced CT test set was 0.87±0.11, while the mean DSC for the plain CT test set was 0.92±0.03. We discuss the reasons for this result. Conclusions: This work demonstrates a method for the automatic outlining of subcutaneous fat, skeletal muscle, and visceral fat areas at L3.

Abstract:Body tissue composition is a long-known biomarker with high diagnostic and prognostic value in cardiovascular, oncological and orthopaedic diseases, but also in rehabilitation medicine or drug dosage. In this study, the aim was to develop a fully automated, reproducible and quantitative 3D volumetry of body tissue composition from standard CT examinations of the abdomen in order to be able to offer such valuable biomarkers as part of routine clinical imaging. Therefore an in-house dataset of 40 CTs for training and 10 CTs for testing were fully annotated on every fifth axial slice with five different semantic body regions: abdominal cavity, bones, muscle, subcutaneous tissue, and thoracic cavity. Multi-resolution U-Net 3D neural networks were employed for segmenting these body regions, followed by subclassifying adipose tissue and muscle using known hounsfield unit limits. The Sørensen Dice scores averaged over all semantic regions was 0.9553 and the intra-class correlation coefficients for subclassified tissues were above 0.99. Our results show that fully-automated body composition analysis on routine CT imaging can provide stable biomarkers across the whole abdomen and not just on L3 slices, which is historically the reference location for analysing body composition in the clinical routine.

A Deep Learning Model to Automate Skeletal Muscle Area Measurement on Computed Tomography Images

Abstract:Background: Muscle wasting (Sarcopenia) is associated with poor outcomes in cancer patients. Early identification of sarcopenia can facilitate nutritional and exercise intervention. Cross-sectional skeletal muscle (SM) area at the third lumbar vertebra (L3) slice of a computed tomography (CT) image is increasingly used to assess body composition and calculate SM index (SMI), a validated surrogate marker for sarcopenia in cancer. Manual segmentation of SM requires multiple steps, which limits use in routine clinical practice. This project aims to develop an automatic method to segment L3 muscle in CT scans. Methods: Attenuation correction CTs from full body PET-CT scans from patients enrolled in two prospective trials were used. The training set consisted of 66 non-small cell lung cancer (NSCLC) patients who underwent curative intent radiotherapy. An additional 42 NSCLC patients prescribed curative intent chemo-radiotherapy from a second trial were used for testing. Each patient had multiple CT scans taken at different time points prior to and post- treatment (147 CTs in the training and validation set and 116 CTs in the independent testing set). Skeletal muscle at L3 vertebra was manually segmented by two observers, according to the Alberta protocol to serve as ground truth labels. This included 40 images segmented by both observers to measure inter-observer variation. An ensemble of 2.5D fully convolutional neural networks (U-Nets) was used to perform the segmentation. The final layer of U-Net produced the binary classification of the pixels into muscle and non-muscle area. The model performance was calculated using Dice score and absolute percentage error (APE) in skeletal muscle area between manual and automated contours. Results: We trained five 2.5D U-Nets using 5-fold cross validation and used them to predict the contours in the testing set. The model achieved a mean Dice score of 0.92 and an APE of 3.1% on the independent testing set. This was similar to inter-observer variation of 0.96 and 2.9% for mean Dice and APE respectively. We further quantified the performance of sarcopenia classification using computer generated skeletal muscle area. To meet a clinical diagnosis of sarcopenia based on Alberta protocol the model achieved a sensitivity of 84% and a specificity of 95%. Conclusions: This work demonstrates an automated method for accurate and reproducible segmentation of skeletal muscle area at L3. This is an efficient tool for large scale or routine computation of skeletal muscle area in cancer patients which may have applications on low quality CTs acquired as part of PET/CT studies for staging and surveillance of patients with cancer.

Abstract:The latest advances in computer-assisted precision medicine are making it feasible to move from population-wide models that are useful to discover aggregate patterns that hold for group-based analysis to patient-specific models that can drive patient-specific decisions with regard to treatment choices, and predictions of outcomes of treatment. Body Composition is recognized as an important driver and risk factor for a wide variety of diseases, as well as a predictor of individual patient-specific clinical outcomes to treatment choices or surgical interventions. 3D CT images are routinely acquired in the oncological worklows and deliver accurate rendering of internal anatomy and therefore can be used opportunistically to assess the amount of skeletal muscle and adipose tissue compartments. Powerful tools of artificial intelligence such as deep learning are making it feasible now to segment the entire 3D image and generate accurate measurements of all internal anatomy. These will enable the overcoming of the severe bottleneck that existed previously, namely, the need for manual segmentation, which was prohibitive to scale to the hundreds of 2D axial slices that made up a 3D volumetric image. Automated tools such as presented here will now enable harvesting whole-body measurements from 3D CT or MRI images, leading to a new era of discovery of the drivers of various diseases based on individual tissue, organ volume, shape, and functional status. These measurements were hitherto unavailable thereby limiting the field to a very small and limited subset. These discoveries and the potential to perform individual image segmentation with high speed and accuracy are likely to lead to the incorporation of these 3D measures into individual specific treatment planning models related to nutrition, aging, chemotoxicity, surgery and survival after the onset of a major disease such as cancer.

Abstract:Quantification of fat and muscle on clinically acquired computed tomography (CT) scans is critical for determination of body composition, a key component of health. Manual tracing has been regarded as the gold standard method of body segmentation; however, manual tracing is time-consuming. Many semi-automated/automated algorithms have been proposed to avoid the manual efforts. Previous efforts largely focused on segmenting two-dimensional cross-sectional images (e.g., at L3/T4 vertebra locations) rather than on the whole-body volume. In this paper, we propose a fully automated three-dimensional (3D) body composition estimation framework for segmenting the muscle and fat from abdominal CT scans. The 3D whole body segmentations are reconstructed from a slice-wise multi-atlas label fusion (MALF) based framework. First, we use a low-dimensional atlas representation to estimate each class for each axial slice. Second, the abdominal wall and psoas muscle are segmented by combining MALF with active shape models and deformable models. Third, skeletal muscle, visceral adipose tissue (VAT) and subcutaneous adipose tissue (SAT) are measured to assess the areas of muscle and fat tissue. The proposed method was compared to manual segmentation and demonstrated high accuracy. Then, we evaluated the approach on 40 CT scans comparing the new method to a prior atlas-based segmentation method and achieved 0.854, 0.740, 0.887 and 0.933 on Dice similarity index for the skeletal muscle, psoas muscle, VAT and SAT, respectively. Compared with the baseline, our method showed significantly ($p\,{<}\,0.001$) higher accuracy on skeletal muscle, VAT and SAT estimation.

Abstract:Abstract Background Body composition is associated with survival outcome in oncological patients, but it is not routinely calculated. Manual segmentation of subcutaneous adipose tissue (SAT) and muscle is time-consuming and therefore limited to a single CT slice. Our goal was to develop an artificial-intelligence (AI)-based method for automated quantification of three-dimensional SAT and muscle volumes from CT images. Methods Ethical approvals from Gothenburg and Lund Universities were obtained. Convolutional neural networks were trained to segment SAT and muscle using manual segmentations on CT images from a training group of 50 patients. The method was applied to a separate test group of 74 cancer patients, who had two CT studies each with a median interval between the studies of 3 days. Manual segmentations in a single CT slice were used for comparison. The accuracy was measured as overlap between the automated and manual segmentations. Results The accuracy of the AI method was 0.96 for SAT and 0.94 for muscle. The average differences in volumes were significantly lower than the corresponding differences in areas in a single CT slice: 1.8% versus 5.0% ( p < 0.001) for SAT and 1.9% versus 3.9% ( p < 0.001) for muscle. The 95% confidence intervals for predicted volumes in an individual subject from the corresponding single CT slice areas were in the order of ± 20%. Conclusions The AI-based tool for quantification of SAT and muscle volumes showed high accuracy and reproducibility and provided a body composition analysis that is more relevant than manual analysis of a single CT slice.
Fully Automated and Standardized Segmentation of Adipose Tissue Compartments by Deep Learning in Three-dimensional Whole-body MRI of Epidemiological Cohort Studies

Abstract:Purpose: To enable fast and reliable assessment of subcutaneous and visceral adipose tissue compartments derived from whole-body MRI. Methods: Quantification and localization of different adipose tissue compartments from whole-body MR images is of high interest to examine metabolic conditions. For correct identification and phenotyping of individuals at increased risk for metabolic diseases, a reliable automatic segmentation of adipose tissue into subcutaneous and visceral adipose tissue is required. In this work we propose a 3D convolutional neural network (DCNet) to provide a robust and objective segmentation. In this retrospective study, we collected 1000 cases (66$\pm$ 13 years; 523 women) from the Tuebingen Family Study and from the German Center for Diabetes research (TUEF/DZD), as well as 300 cases (53$\pm$ 11 years; 152 women) from the German National Cohort (NAKO) database for model training, validation, and testing with a transfer learning between the cohorts. These datasets had variable imaging sequences, imaging contrasts, receiver coil arrangements, scanners and imaging field strengths. The proposed DCNet was compared against a comparable 3D UNet segmentation in terms of sensitivity, specificity, precision, accuracy, and Dice overlap. Results: Fast (5-7seconds) and reliable adipose tissue segmentation can be obtained with high Dice overlap (0.94), sensitivity (96.6%), specificity (95.1%), precision (92.1%) and accuracy (98.4%) from 3D whole-body MR datasets (field of view coverage 450x450x2000mm${}^3$). Segmentation masks and adipose tissue profiles are automatically reported back to the referring physician. Conclusion: Automatic adipose tissue segmentation is feasible in 3D whole-body MR data sets and is generalizable to different epidemiological cohort studies with the proposed DCNet.

Abstract:Ammonium is generated in culture media by the spontaneous deamination of amino acids at 37 °C and through the metabolism of amino acids by human embryos. The appearance of ammonium is a time-dependent phenomenon and can compromise embryo physiology, development and viability. In this study, the effects of a gradient of ammonium on the development, metabolism and transcriptome of human and mouse embryos were investigated. Pronucleate oocytes were cultured in the presence of an ammonium gradient that mimicked the spontaneous deamination of Eagle's amino acids together with 1 mM glutamine. All embryos were cultured in sequential media G1/G2 at 5% O2, 6% CO2 and 89% N2. Human embryo metabolism was assessed through a non-invasive fluorometric analysis of pyruvate consumption. Transcriptome analysis was performed on the resultant blastocysts from both species using a microarray technology. Embryo development prior to compaction was negatively affected by the presence of low levels of ammonium in both species. Human embryo metabolism was significantly inhibited after just 24 and 48 h of culture. Transcriptome analysis of blastocysts from both species revealed significantly altered gene expression profiles, both decreased and increased. Functional annotation of the altered genes revealed the following over represented biological processes: metabolism, cell growth and/or maintenance, transcription, cell communication, transport, development and transcription regulation. These data emphasize the enhanced sensitivity of the cleavage-stage embryo to its environment and highlight the requirement to renew culture media at frequent intervals in order to alleviate the in vitro induced effects of ammonium build-up in the environment surrounding the embryo.

Abstract:Pretreatment risk stratification is key for personalized medicine. While many physicians rely on an “eyeball test” to assess whether patients will tolerate major surgery or chemotherapy, “eyeballing” is inherently subjective and difficult to quantify. The concept of morphometric age derived from cross-sectional imaging has been found to correlate well with outcomes such as length of stay, morbidity, and mortality. However, the determination of the morphometric age is time intensive and requires highly trained experts. In this study, we propose a fully automated deep learning system for the segmentation of skeletal muscle cross-sectional area (CSA) on an axial computed tomography image taken at the third lumbar vertebra. We utilized a fully automated deep segmentation model derived from an extended implementation of a fully convolutional network with weight initialization of an ImageNet pre-trained model, followed by post processing to eliminate intramuscular fat for a more accurate analysis. This experiment was conducted by varying window level (WL), window width (WW), and bit resolutions in order to better understand the effects of the parameters on the model performance. Our best model, fine-tuned on 250 training images and ground truth labels, achieves 0.93 ± 0.02 Dice similarity coefficient (DSC) and 3.68 ± 2.29% difference between predicted and ground truth muscle CSA on 150 held-out test cases. Ultimately, the fully automated segmentation system can be embedded into the clinical environment to accelerate the quantification of muscle and expanded to volume analysis of 3D datasets.

Abstract:Background: Body composition is increasingly being recognized as an important prognostic factor for health outcomes across cancer, liver cirrhosis, and critically ill patients. Computed tomography (CT) scans, when taken as part of routine care, provide an excellent opportunity to precisely measure the quantity and quality of skeletal muscle and adipose tissue. However, manual analysis of CT scans is costly and time-intensive, limiting the widespread adoption of CT-based measurements of body composition. Summary: Advances in deep learning have demonstrated excellent success in biomedical image analysis. Several recent publications have demonstrated excellent accuracy in comparison to human raters for the measurement of skeletal muscle, visceral adipose, and subcutaneous adipose tissue from the lumbar vertebrae region, indicating that analysis of body composition may be successfully automated using deep neural networks. Key Messages: The high accuracy and drastically improved speed of CT body composition analysis (#x3c;1 s/scan for neural networks vs. 15 min/scan for human analysis) suggest that neural networks may aid researchers and clinicians in better understanding the role of body composition in clinical populations by enabling cost-effective, large-scale research studies. As the role of body composition in clinical settings and the field of automated analysis advance, it will be critical to examine how clinicians interact with these systems and to evaluate whether these technologies are beneficial in improving treatment and health outcomes for patients. Abstract:Rationale and Objectives We aim to develop a CT-based deep learning (DL) system for fully automatic segmentation of regional muscle volume and measurement of the spatial intermuscular fat distribution of the gluteus maximus muscle. Materials and Methods A total of 472 subjects were enrolled and randomly assigned to one of three groups: a training set, test set 1, and test set 2. For each subject in the training set and test set 1, we selected six slices of the CT images as the region of interest for manual segmentation by a radiologist. For each subject in test set 2, we selected all slices of the gluteus maximus muscle on the CT images for manual segmentation. The DL system was constructed using Attention U-Net and the Otsu binary thresholding method to segment the muscle and measure the fat fraction of the gluteus maximus muscle. The segmentation results of the DL system were evaluated using the Dice similarity coefficient (DSC), Hausdorff distance (HD), and the average surface distance (ASD) as metrics. Intraclass correlation coefficients (ICCs) and Bland-Altman plots were used to assess agreement in the measurements of fat fraction between the radiologist and the DL system. Results The DL system showed good segmentation performance on the two test sets, with DSCs of 0.930 and 0.873, respectively. The fat fraction of the gluteus maximus muscle measured by the DL system was in agreement with the radiologist (ICC = 0.748). Conclusion The proposed DL system showed accurate, fully automated segmentation performance and good agreement with the radiologist at fat fraction evaluation, and can be further used for muscle evaluation.

Automated segmentation of 3-D body composition on computed tomography

Abstract:Purpose: To develop and validate a computer tool for automatic and simultaneous segmentation of body composition depicted on computed tomography (CT) scans for the following tissues: visceral adipose (VAT), subcutaneous adipose (SAT), intermuscular adipose (IMAT), skeletal muscle (SM), and bone. Approach: A cohort of 100 CT scans acquired from The Cancer Imaging Archive (TCIA) was used - 50 whole-body positron emission tomography (PET)-CTs, 25 chest, and 25 abdominal. Five different body compositions were manually annotated (VAT, SAT, IMAT, SM, and bone). A training-while-annotating strategy was used for efficiency. The UNet model was trained using the already annotated cases. Then, this model was used to enable semi-automatic annotation for the remaining cases. The 10-fold cross-validation method was used to develop and validate the performance of several convolutional neural networks (CNNs), including UNet, Recurrent Residual UNet (R2Unet), and UNet++. A 3-D patch sampling operation was used when training the CNN models. The separately trained CNN models were tested to see if they could achieve a better performance than segmenting them jointly. Paired-samples t-test was used to test for statistical significance. Results: Among the three CNN models, UNet demonstrated the best overall performance in jointly segmenting the five body compositions with a Dice coefficient of 0.840+/-0.091, 0.908+/-0.067, 0.603+/-0.084, 0.889+/-0.027, and 0.884+/-0.031, and a Jaccard index of 0.734+/-0.119, 0.837+/-0.096, 0.437+/-0.082, 0.800+/-0.042, 0.793+/-0.049, respectively for VAT, SAT, IMAT, SM, and bone. Conclusion: There were no significant differences among the CNN models in segmenting body composition, but jointly segmenting body compositions achieved a better performance than segmenting them separately.

Abstract:By tuning the SET/RESET pulse amplitude conditions, the pulse endurance of our 40-nm HfO2/Hf 1T1R resistive-random-access-memory devices demonstrates varying failure behaviors after 106 cycles. For unbalanced SET/RESET pulse amplitude conditions, both low-resistance state (LRS) and high-resistance state (HRS) failures may occur, while varying the pulsewidths influences the LRS/HRS window and the stability of the LRS/HRS states. The failure of the HRS or LRS state during cycling is ascribed to the depletion or excess of oxygen vacancies at the switching interface. Through a dc SET/RESET recovery operation, LRS/HRS states can be recovered after failure, indicating that the distribution of oxygen vacancies can be restored. By optimally balancing the SET/RESET pulse conditions, more than 1010 pulse endurance cycles is achieved.
Fully-Automated Analysis of Body Composition from CT in Cancer Patients Using Convolutional Neural Networks

Abstract:The amounts of muscle and fat in a person's body, known as body composition, are correlated with cancer risks, cancer survival, and cardiovascular risk. The current gold standard for measuring body composition requires time-consuming manual segmentation of CT images by an expert reader. In this work, we describe a two-step process to fully automate the analysis of CT body composition using a DenseNet to select the CT slice and U-Net to perform segmentation. We train and test our methods on independent cohorts. Our results show Dice scores (0.95-0.98) and correlation coefficients (R=0.99) that are favorable compared to human readers. These results suggest that fully automated body composition analysis is feasible, which could enable both clinical use and large-scale population studies.

Abstract:Background Muscle quantification is an essential step in sarcopenia evaluation. Purpose To develop and evaluate an automated machine learning (ML) algorithm for segmenting the paraspinous muscles on either abdominal or lumbar (L) computed tomography (CT) scans. Material and Methods A novel deep neural network algorithm for automated segmentation of paraspinous muscle was developed, CT scans of 504 consecutive patients conducted between January 2019 and February 2020 were assembled. The muscle was manually segmented at L3 vertebra level by three radiologists as ground truth, divided into training and testing subgroups. Muscle cross-sectional area (CSA) was recorded. Dice similarity coefficients (DSCs) and CSA errors were calculated to evaluate system performance. The degree of muscle fat infiltration (MFI) recording by percentage value was the fat area within the region of interest divided by the muscle area. An analysis of the factors influencing the performance of the V-net-based segmentation system was also implemented. Results The mean DSCs for paraspinous muscles were high for both the training (0.963, 0.970, 0.941, and 0.968, respectively) and testing (0.950, 0.960, 0.929, and 0.961, respectively) datasets, while the CSA errors were low for both training (1.9%, 1.6%, 3.1%, and 1.3%, respectively) and testing (3.4%, 3.0%, 4.6%, and 1.9%, respectively) datasets. MFI and muscle area index (MI) were major factors affecting DSCs of the posterior paraspinous and paraspinous muscle groups. Conclusion The ML algorithm for the measurement of paraspinous muscles was compared favorably to manual ground truth measurements.

Abstract:Background: Body shape, an intuitive health indicator, is deterministically driven by body composition. We developed and validated a deep learning model that generates accurate dual-energy X-ray absorptiometry (DXA) scans from three-dimensional optical body scans (3DO), enabling compositional analysis of the whole body and specified subregions. Previous works on generative medical imaging models lack quantitative validation and only report quality metrics. Methods: Our model was self-supervised pretrained on two large clinical DXA datasets and fine-tuned using the Shape Up! Adults study dataset. Model-predicted scans from a holdout test set were evaluated using clinical commercial DXA software for compositional accuracy. Results: Predicted DXA scans achieve R2 of 0.73, 0.89, and 0.99 and RMSEs of 5.32, 6.56, and 4.15 kg for total fat mass (FM), fat-free mass (FFM), and total mass, respectively. Custom subregion analysis results in R2s of 0.70-0.89 for left and right thigh composition. We demonstrate the ability of models to produce quantitatively accurate visualizations of soft tissue and bone, confirming a strong relationship between body shape and composition. Conclusions: This work highlights the potential of generative models in medical imaging and reinforces the importance of quantitative validation for assessing their clinical utility.
