Abstract:Purpose: To enable fast and reliable assessment of subcutaneous and visceral adipose tissue compartments derived from whole-body MRI. Methods: Quantification and localization of different adipose tissue compartments from whole-body MR images is of high interest to examine metabolic conditions. For correct identification and phenotyping of individuals at increased risk for metabolic diseases, a reliable automatic segmentation of adipose tissue into subcutaneous and visceral adipose tissue is required. In this work we propose a 3D convolutional neural network (DCNet) to provide a robust and objective segmentation. In this retrospective study, we collected 1000 cases (66$\pm$ 13 years; 523 women) from the Tuebingen Family Study and from the German Center for Diabetes research (TUEF/DZD), as well as 300 cases (53$\pm$ 11 years; 152 women) from the German National Cohort (NAKO) database for model training, validation, and testing with a transfer learning between the cohorts. These datasets had variable imaging sequences, imaging contrasts, receiver coil arrangements, scanners and imaging field strengths. The proposed DCNet was compared against a comparable 3D UNet segmentation in terms of sensitivity, specificity, precision, accuracy, and Dice overlap. Results: Fast (5-7seconds) and reliable adipose tissue segmentation can be obtained with high Dice overlap (0.94), sensitivity (96.6%), specificity (95.1%), precision (92.1%) and accuracy (98.4%) from 3D whole-body MR datasets (field of view coverage 450x450x2000mm${}^3$). Segmentation masks and adipose tissue profiles are automatically reported back to the referring physician. Conclusion: Automatic adipose tissue segmentation is feasible in 3D whole-body MR data sets and is generalizable to different epidemiological cohort studies with the proposed DCNet.
Fully-automated Body Composition Analysis in Routine CT Imaging Using 3D Semantic Segmentation Convolutional Neural Networks

Abstract:Body tissue composition is a long-known biomarker with high diagnostic and prognostic value in cardiovascular, oncological and orthopaedic diseases, but also in rehabilitation medicine or drug dosage. In this study, the aim was to develop a fully automated, reproducible and quantitative 3D volumetry of body tissue composition from standard CT examinations of the abdomen in order to be able to offer such valuable biomarkers as part of routine clinical imaging. Therefore an in-house dataset of 40 CTs for training and 10 CTs for testing were fully annotated on every fifth axial slice with five different semantic body regions: abdominal cavity, bones, muscle, subcutaneous tissue, and thoracic cavity. Multi-resolution U-Net 3D neural networks were employed for segmenting these body regions, followed by subclassifying adipose tissue and muscle using known hounsfield unit limits. The Sørensen Dice scores averaged over all semantic regions was 0.9553 and the intra-class correlation coefficients for subclassified tissues were above 0.99. Our results show that fully-automated body composition analysis on routine CT imaging can provide stable biomarkers across the whole abdomen and not just on L3 slices, which is historically the reference location for analysing body composition in the clinical routine.

Fully Convolutional Networks for Automated Segmentation of Abdominal Adipose Tissue Depots in Multicenter Water-Fat MRI

Abstract:Purpose: An approach for the automated segmentation of visceral adipose tissue (VAT) and subcutaneous adipose tissue (SAT) in multicenter water-fat MRI scans of the abdomen was investigated, using two different neural network architectures. Methods: The two fully convolutional network architectures U-Net and V-Net were trained, evaluated and compared on the water-fat MRI data. Data of the study Tellus with 90 scans from a single center was used for a 10-fold cross-validation in which the most successful configuration for both networks was determined. These configurations were then tested on 20 scans of the multicenter study beta-cell function in JUvenile Diabetes and Obesity (BetaJudo), which involved a different study population and scanning device. Results: The U-Net outperformed the used implementation of the V-Net in both cross-validation and testing. In cross-validation, the U-Net reached average dice scores of 0.988 (VAT) and 0.992 (SAT). The average of the absolute quantification errors amount to 0.67% (VAT) and 0.39% (SAT). On the multi-center test data, the U-Net performs only slightly worse, with average dice scores of 0.970 (VAT) and 0.987 (SAT) and quantification errors of 2.80% (VAT) and 1.65% (SAT). Conclusion: The segmentations generated by the U-Net allow for reliable quantification and could therefore be viable for high-quality automated measurements of VAT and SAT in large-scale studies with minimal need for human intervention. The high performance on the multicenter test data furthermore shows the robustness of this approach for data of different patient demographics and imaging centers, as long as a consistent imaging protocol is used.

Abstract:Background: Human brown adipose tissue (BAT), mostly located in the cervical/supraclavicular region, is a promising target in obesity treatment. Magnetic resonance imaging (MRI) allows for mapping the fat content quantitatively. However, due to the complex heterogeneous distribution of BAT, it has been difficult to establish a standardized segmentation routine based on magnetic resonance (MR) images. Here, we suggest using a multi-modal deep neural network to detect the supraclavicular fat pocket. Methods: A total of 50 healthy subjects [median age/body mass index (BMI) =36 years/24.3 kg/m2] underwent MRI scans of the neck region on a 3 T Ingenia scanner (Philips Healthcare, Best, Netherlands). Manual segmentations following fixed rules for anatomical borders were used as ground truth labels. A deep learning-based method (termed as BAT-Net) was proposed for the segmentation of BAT on MRI scans. It jointly leveraged two-dimensional (2D) and three-dimensional (3D) convolutional neural network (CNN) architectures to efficiently encode the multi-modal and 3D context information from multi-modal MRI scans of the supraclavicular region. We compared the performance of BAT-Net to that of 2D U-Net and 3D U-Net. For 2D U-Net, we analyzed the performance difference of implementing 2D U-Net in three different planes, denoted as 2D U-Net (axial), 2D U-Net (coronal), and 2D U-Net (sagittal). Results: The proposed model achieved an average dice similarity coefficient (DSC) of 0.878 with a standard deviation of 0.020. The volume segmented by the network was smaller compared to the ground truth labels by 9.20 mL on average with a mean absolute increase in proton density fat fraction (PDFF) inside the segmented regions of 1.19 percentage points. The BAT-Net outperformed all implemented 2D U-Nets and the 3D U-Nets with average DSC enhancement ranging from 0.016 to 0.023. Conclusions: The current work integrates a deep neural network-based segmentation into the automated segmentation of supraclavicular fat depot for quantitative evaluation of BAT. Experiments show that the presented multi-modal method benefits from leveraging both 2D and 3D CNN architecture and outperforms the independent use of 2D or 3D networks. Deep learning-based segmentation methods show potential towards a fully automated segmentation of the supraclavicular fat depot.

Abstract:Normal fetal adipose tissue (AT) development is essential for perinatal well-being. AT, or simply fat, stores energy in the form of lipids. Malnourishment may result in excessive or depleted adiposity. Although previous studies showed a correlation between the amount of AT and perinatal outcome, prenatal assessment of AT is limited by lacking quantitative methods. Using magnetic resonance imaging (MRI), 3D fat- and water-only images of the entire fetus can be obtained from two point Dixon images to enable AT lipid quantification. This paper is the first to present a methodology for developing a deep learning based method for fetal fat segmentation based on Dixon MRI. It optimizes radiologists' manual fetal fat delineation time to produce annotated training dataset. It consists of two steps: 1) model-based semi-automatic fetal fat segmentations, reviewed and corrected by a radiologist; 2) automatic fetal fat segmentation using DL networks trained on the resulting annotated dataset. Three DL networks were trained. We show a significant improvement in segmentation times (3:38 hours to < 1 hour) and observer variability (Dice of 0.738 to 0.906) compared to manual segmentation. Automatic segmentation of 24 test cases with the 3D Residual U-Net, nn-UNet and SWIN-UNetR transformer networks yields a mean Dice score of 0.863, 0.787 and 0.856, respectively. These results are better than the manual observer variability, and comparable to automatic adult and pediatric fat segmentation. A radiologist reviewed and corrected six new independent cases segmented using the best performing network, resulting in a Dice score of 0.961 and a significantly reduced correction time of 15:20 minutes. Using these novel segmentation methods and short MRI acquisition time, whole body subcutaneous lipids can be quantified for individual fetuses in the clinic and large-cohort research.

Abstract:Purpose: To develop and validate a computer tool for automatic and simultaneous segmentation of body composition depicted on computed tomography (CT) scans for the following tissues: visceral adipose (VAT), subcutaneous adipose (SAT), intermuscular adipose (IMAT), skeletal muscle (SM), and bone. Approach: A cohort of 100 CT scans acquired from The Cancer Imaging Archive (TCIA) was used - 50 whole-body positron emission tomography (PET)-CTs, 25 chest, and 25 abdominal. Five different body compositions were manually annotated (VAT, SAT, IMAT, SM, and bone). A training-while-annotating strategy was used for efficiency. The UNet model was trained using the already annotated cases. Then, this model was used to enable semi-automatic annotation for the remaining cases. The 10-fold cross-validation method was used to develop and validate the performance of several convolutional neural networks (CNNs), including UNet, Recurrent Residual UNet (R2Unet), and UNet++. A 3-D patch sampling operation was used when training the CNN models. The separately trained CNN models were tested to see if they could achieve a better performance than segmenting them jointly. Paired-samples t-test was used to test for statistical significance. Results: Among the three CNN models, UNet demonstrated the best overall performance in jointly segmenting the five body compositions with a Dice coefficient of 0.840+/-0.091, 0.908+/-0.067, 0.603+/-0.084, 0.889+/-0.027, and 0.884+/-0.031, and a Jaccard index of 0.734+/-0.119, 0.837+/-0.096, 0.437+/-0.082, 0.800+/-0.042, 0.793+/-0.049, respectively for VAT, SAT, IMAT, SM, and bone. Conclusion: There were no significant differences among the CNN models in segmenting body composition, but jointly segmenting body compositions achieved a better performance than segmenting them separately.

Abstract:Pancreatic volume and fat fraction are critical prognoses for metabolic diseases like type 2 diabetes (T2D). Magnetic Resonance Imaging (MRI) is a required non-invasive quantification method for the pancreatic fat fraction. The dramatic development of deep learning has enabled the automatic measurement of MR images. Therefore, based on MRI, we intend to develop a deep convolutional neural network (DCNN) that can accurately segment and measure pancreatic volume and fat fraction. This retrospective study involved abdominal MR images from 148 diabetic patients and 246 healthy normoglycemic participants. We randomly separated them into training and testing sets according to the proportion of 80:20. There were 2364 recognizable pancreas images labeled and pre-treated by an upgraded superpixel algorithm for a discernible pancreatic boundary. We then applied them to the novel DCNN model, mimicking the most accurate and latest manual pancreatic segmentation process. Fat phantom and erosion algorithms were employed to increase the accuracy. The results were evaluated by dice similarity coefficient (DSC). External validation datasets included 240 MR images from 10 additional patients. We assessed the pancreas and pancreatic fat volume using the DCNN and compared them with those of specialists. This DCNN employed the cutting-edge idea of manual pancreas segmentation and achieved the highest DSC (91.2%) compared with any reported models. It is the first framework to measure intra-pancreatic fat volume and fat deposition. Performance validation reflected by regression R2 value between manual operation and trained DCNN segmentation on the pancreas and pancreatic fat volume were 0.9764 and 0.9675, respectively. The performance of the novel DCNN enables accurate pancreas segmentation, pancreatic fat volume, fraction measurement, and calculation. It achieves the same segmentation level of experts. With further training, it may well surpass any expert and provide accurate measurements, which may have significant clinical relevance.

Abstract:Abdominal fat quantification is critical since multiple vital organs are located within this region. Although computed tomography (CT) is a highly sensitive modality to segment body fat, it involves ionizing radiations which makes magnetic resonance imaging (MRI) a preferable alternative for this purpose. Additionally, the superior soft tissue contrast in MRI could lead to more accurate results. Yet, it is highly labor intensive to segment fat in MRI scans. In this study, we propose an algorithm based on deep learning technique(s) to automatically quantify fat tissue from MR images through a cross modality adaptation. Our method does not require supervised labeling of MR scans, instead, we utilize a cycle generative adversarial network (C-GAN) to construct a pipeline that transforms the existing MR scans into their equivalent synthetic CT (s-CT) images where fat segmentation is relatively easier due to the descriptive nature of HU (hounsfield unit) in CT images. The fat segmentation results for MRI scans were evaluated by expert radiologist. Qualitative evaluation of our segmentation results shows average success score of 3.80/5 and 4.54/5 for visceral and subcutaneous fat segmentation in MR images.

Abstract:Background: Quantitative muscle and fat data obtained through body composition analysis are expected to be a new stable biomarker for the early and accurate prediction of treatment-related toxicity, treatment response, and prognosis in patients with lung cancer. The use of these biomarkers can enable the adjustment of individualized treatment regimens in a timely manner, which is critical to further improving patient prognosis and quality of life. We aimed to develop a deep learning model based on attention for fully automated segmentation of the abdomen from computed tomography (CT) to quantify body composition. Methods: A fully automatic segmentation deep learning model was designed based on the attention mechanism and using U-Net as the framework. Subcutaneous fat, skeletal muscle, and visceral fat were manually segmented by two experts to serve as ground truth labels. The performance of the model was evaluated using Dice similarity coefficients (DSCs) and Hausdorff distance at 95th percentile (HD95). Results: The mean DSC for subcutaneous fat and skeletal muscle were high for both the enhanced CT test set (0.93±0.06 and 0.96±0.02, respectively) and the plain CT test set (0.90±0.09 and 0.95±0.01, respectively). Nevertheless, the model did not perform well in the segmentation performance of visceral fat, especially for the enhanced CT test set. The mean DSC for the enhanced CT test set was 0.87±0.11, while the mean DSC for the plain CT test set was 0.92±0.03. We discuss the reasons for this result. Conclusions: This work demonstrates a method for the automatic outlining of subcutaneous fat, skeletal muscle, and visceral fat areas at L3.

Abstract:Quantification of adipose tissue (fat) from computed tomography (CT) scans is conducted mostly through manual or semi-automated image segmentation algorithms with limited efficacy. In this work, we propose a completely unsupervised and automatic method to identify adipose tissue, and then separate Subcutaneous Adipose Tissue (SAT) from Visceral Adipose Tissue (VAT) at the abdominal region. We offer a three-phase pipeline consisting of (1) Initial boundary estimation using gradient points, (2) boundary refinement using Geometric Median Absolute Deviation and Appearance based Local Outlier Scores (3) Context driven label fusion using Conditional Random Fields (CRF) to obtain the final boundary between SAT and VAT. We evaluate the proposed method on 151 abdominal CT scans and obtain state-of-the-art 94% and 91% dice similarity scores for SAT and VAT segmentation, as well as significant reduction in fat quantification error measure.
Automatic quantification of abdominal subcutaneous and visceral adipose tissue in children, through MRI study, using total intensity maps and Convolutional Neural Networks

Abstract:Childhood overweight and obesity is one of the main health problems in the world since it is related to the early appearance of different diseases, in addition to being a risk factor for later developing obesity in adulthood with its health and economic consequences. Visceral abdominal tissue (VAT) is strongly related to the development of metabolic and cardiovascular diseases compared to abdominal subcutaneous adipose tissue (ASAT). Therefore, precise and automatic VAT and ASAT quantification methods would allow better diagnosis, monitoring and prevention of diseases caused by obesity at any stage of life. Currently, magnetic resonance imaging is the standard for fat quantification, with Dixon sequences being the most useful. Different semiautomatic and automatic ASAT and VAT quantification methodologies have been proposed. In particular, the semi-automated quantification methodology used commercially through the cloud-based service AMRA R Researcher stands out due to its extensive validation in different studies. In the present work, a database made up of Dixon MRI sequences, obtained from children between 7 and 9 years of age, was studied. Applying a preprocessing to obtain what we call total intensity maps, a convolutional neural network (CNN) was proposed for the automatic quantification of ASAT and VAT. The quantifications obtained from the proposed methodology were compared with quantifications previously made through AMRA R Researcher. For the comparison, correlation analysis, Bland-Altman graphs and non-parametric statistical tests were used. The results indicated a high correlation and similar precisions between the quantifications of this work and those of AMRA R Researcher. The final objective is that the proposed methodology can serve as an accessible and free tool for the diagnosis, monitoring and prevention of diseases related to childhood obesity.

Abstract:Background : Many cardiovascular diseases are closely related to the composition of epicardial adipose tissue (EAT). Accurate segmentation of EAT can provide a reliable reference for doctors to diagnose the disease. The distribution and composition of EAT often have significant individual differences, and the traditional segmentation methods are not effective. In recent years, deep learning method has been gradually introduced into EAT segmentation task. Purpose : The existing EAT segmentation methods based on deep learning have a large amount of computation and the segmentation accuracy needs to be improved. Therefore, the purpose of this paper is to develop a lightweight EAT segmentation network, which can obtain higher segmentation accuracy with less computation and further alleviate the problem of false positive segmentation. Methods : Firstly, the obtained Computed Tomography (CT) was preprocessed. That is, the threshold range of EAT was determined to be (‐190, ‐30) HU according to prior knowledge, and the non‐adipose pixels were excluded by threshold segmentation to reduce the difficulty of training. Secondly, the image obtained after thresholding was input into the lightweight RDU‐Net network to perform the training, validating, and testing process. RDU‐Net uses a residual multi‐scale dilated convolution block in order to extract a wider range of information without changing the current resolution. At the same time, the form of residual connection is adopted to avoid the problem of gradient expansion or gradient explosion caused by too deep network, which also makes the learning easier. In order to optimize the training process, this paper proposes PNDiceLoss, which takes both positive and negative pixels as learning targets, fully considers the class imbalance problem and appropriately highlights the status of positive pixels. Results : In this paper, 50 CCTA images were randomly selected from the hospital, and the commonly used Dice similarity coefficient (DSC), Jaccard similarity (JS), Accuracy (ACC), Specificity (SP), Precision (PC), and Pearson correlation coefficient are used as evaluation metrics. Bland‐Altman analysis results show that the extracted EAT volume is consistent with the actual volume. Compared with the existing methods, the segmentation results show that the proposed method achieves better performance on these metrics, achieving the DSC of 0.9262. The number of false positive pixels has been reduced by more than half. Pearson correlation coefficient reached 0.992, and linear regression coefficient reached 0.977 when measuring the volume of EAT obtained. In order to verify the effectiveness of the proposed method, experiments are carried out in the cardiac fat database of VisualLab. On this database, the proposed method also achieved good results, and the DSC value reached 0.927 in the case of only 878 slices. Conclusions : A new method to segment and quantify EAT is proposed. Comprehensive experiments show that compared with some classical segmentation algorithms, the proposed method has the advantages of shorter time‐consuming, less memory required for operations, and higher segmentation accuracy. Automatic segmentation and quantification of epicardial adipose tissue from coronary computed tomography angiography

Abstract:Epicardial adipose tissue (EAT) is a visceral fat deposit, that's known for its association with factors, such as obesity, diabetes mellitus, age, and hypertension. Segmentation of the EAT in a fast and reproducible way is important for the interpretation of its role as an independent risk marker intricate. However, EAT has a variable distribution, and various diseases may affect the volume of the EAT, which can increase the complexity of the already time-consuming manual segmentation work. We propose a 3D deep attention U-Net method to automatically segment the EAT from coronary computed tomography angiography (CCTA). Five-fold cross-validation and hold-out experiments were used to evaluate the proposed method through a retrospective investigation of 200 patients. The automatically segmented EAT volume was compared with physician-approved clinical contours. Quantitative metrics used were the Dice similarity coefficient (DSC), sensitivity, specificity, Jaccard index (JAC), Hausdorff distance (HD), mean surface distance (MSD), residual mean square distance (RMSD), and the center of mass distance (CMD). For cross-validation, the median DSC, sensitivity, and specificity were 92.7%, 91.1%, and 95.1%, respectively, with JAC, HD, CMD, MSD, and RMSD are 82.9% ± 8.8%, 3.77 ± 1.86mm, 1.98 ± 1.50mm, 0.37 ± 0.24mm, and 0.65 ± 0.37mm, respectively. For the hold-out test, the accuracy of the proposed method remained high. We developed a novel deep learning-based approach for the automated segmentation of the EAT on CCTA images. We demonstrated the high accuracy of the proposed learning-based segmentation method through comparison with ground truth contour of 200 clinical patient cases using 8 quantitative metrics, Pearson correlation, and Bland-Altman analysis. Our automatic EAT segmentation results show the potential of the proposed method to be used in computer-aided diagnosis of coronary artery diseases (CADs) in clinical settings.

Abstract:Quantification of fat and muscle on clinically acquired computed tomography (CT) scans is critical for determination of body composition, a key component of health. Manual tracing has been regarded as the gold standard method of body segmentation; however, manual tracing is time-consuming. Many semi-automated/automated algorithms have been proposed to avoid the manual efforts. Previous efforts largely focused on segmenting two-dimensional cross-sectional images (e.g., at L3/T4 vertebra locations) rather than on the whole-body volume. In this paper, we propose a fully automated three-dimensional (3D) body composition estimation framework for segmenting the muscle and fat from abdominal CT scans. The 3D whole body segmentations are reconstructed from a slice-wise multi-atlas label fusion (MALF) based framework. First, we use a low-dimensional atlas representation to estimate each class for each axial slice. Second, the abdominal wall and psoas muscle are segmented by combining MALF with active shape models and deformable models. Third, skeletal muscle, visceral adipose tissue (VAT) and subcutaneous adipose tissue (SAT) are measured to assess the areas of muscle and fat tissue. The proposed method was compared to manual segmentation and demonstrated high accuracy. Then, we evaluated the approach on 40 CT scans comparing the new method to a prior atlas-based segmentation method and achieved 0.854, 0.740, 0.887 and 0.933 on Dice similarity index for the skeletal muscle, psoas muscle, VAT and SAT, respectively. Compared with the baseline, our method showed significantly ($p\,{<}\,0.001$) higher accuracy on skeletal muscle, VAT and SAT estimation.

Abstract:A study was undertaken at Red Cross Children's Hospital to assess how aware parents were of motor vehicle accident (MVA) claim procedures on behalf of their children. Of 160 questionnaires sent out to the parents of road traffic injury (RTI) victims, 100 were answered. Sixty-six per cent of parents who responded had an income below R500 per month. The identity of the motorist responsible for injuring the child was known to only 26 parents. Third party damages were claimed by only 32 parents or guardians; the remaining 68 failed to do so principally through lack of awareness. The implications of these findings for both RTI victims and the hospitals where they are treated are discussed here.
Artificial intelligence based automatic quantification of epicardial adipose tissue suitable for large scale population studies

Abstract:Abstract To develop a fully automatic model capable of reliably quantifying epicardial adipose tissue (EAT) volumes and attenuation in large scale population studies to investigate their relation to markers of cardiometabolic risk. Non-contrast cardiac CT images from the SCAPIS study were used to train and test a convolutional neural network based model to quantify EAT by: segmenting the pericardium, suppressing noise-induced artifacts in the heart chambers, and, if image sets were incomplete, imputing missing EAT volumes. The model achieved a mean Dice coefficient of 0.90 when tested against expert manual segmentations on 25 image sets. Tested on 1400 image sets, the model successfully segmented 99.4% of the cases. Automatic imputation of missing EAT volumes had an error of less than 3.1% with up to 20% of the slices in image sets missing. The most important predictors of EAT volumes were weight and waist, while EAT attenuation was predicted mainly by EAT volume. A model with excellent performance, capable of fully automatic handling of the most common challenges in large scale EAT quantification has been developed. In studies of the importance of EAT in disease development, the strong co-variation with anthropometric measures needs to be carefully considered.

Abstract:Background: Increased pericardial adipose tissue (PAT) is associated with many types of cardiovascular disease (CVD). Although cardiac magnetic resonance images (CMRI) are often acquired in patients with CVD, there are currently no tools to automatically identify and quantify PAT from CMRI. The aim of this study was to create a neural network to segment PAT from T2-weighted CMRI and explore the correlations between PAT volumes (PATV) and CVD outcomes and mortality. Methods: We trained and tested a deep learning model, PAT-CNN, to segment PAT on T2-weighted cardiac MR images. Using the segmentations from PAT-CNN, we automatically calculated PATV on images from 391 patients. We analysed correlations between PATV and CVD diagnosis and 1-year mortality post-imaging. Results: PAT-CNN was able to accurately segment PAT with Dice score/ Hausdorff distances of 0.74 +- 0.03/27.1 +- 10.9~mm, similar to the values obtained when comparing the segmentations of two independent human observers ($0.76 +- 0.06/21.2 +- 10.3~mm$). Regression models showed that, independently of sex and body-mass index, PATV is significantly positively correlated with a diagnosis of CVD and with 1-year all cause mortality (p-value < 0.01). Conclusions: PAT-CNN can segment PAT from T2-weighted CMR images automatically and accurately. Increased PATV as measured automatically from CMRI is significantly associated with the presence of CVD and can independently predict 1-year mortality.
