Abstract:Quantification of fat and muscle on clinically acquired computed tomography (CT) scans is critical for determination of body composition, a key component of health. Manual tracing has been regarded as the gold standard method of body segmentation; however, manual tracing is time-consuming. Many semi-automated/automated algorithms have been proposed to avoid the manual efforts. Previous efforts largely focused on segmenting two-dimensional cross-sectional images (e.g., at L3/T4 vertebra locations) rather than on the whole-body volume. In this paper, we propose a fully automated three-dimensional (3D) body composition estimation framework for segmenting the muscle and fat from abdominal CT scans. The 3D whole body segmentations are reconstructed from a slice-wise multi-atlas label fusion (MALF) based framework. First, we use a low-dimensional atlas representation to estimate each class for each axial slice. Second, the abdominal wall and psoas muscle are segmented by combining MALF with active shape models and deformable models. Third, skeletal muscle, visceral adipose tissue (VAT) and subcutaneous adipose tissue (SAT) are measured to assess the areas of muscle and fat tissue. The proposed method was compared to manual segmentation and demonstrated high accuracy. Then, we evaluated the approach on 40 CT scans comparing the new method to a prior atlas-based segmentation method and achieved 0.854, 0.740, 0.887 and 0.933 on Dice similarity index for the skeletal muscle, psoas muscle, VAT and SAT, respectively. Compared with the baseline, our method showed significantly (\(p\,{<}\,0.001\)) higher accuracy on skeletal muscle, VAT and SAT estimation.

Abstract:The latest advances in computer-assisted precision medicine are making it feasible to move from population-wide models that are useful to discover aggregate patterns that hold for group-based analysis to patient-specific models that can drive patient-specific decisions with regard to treatment choices, and predictions of outcomes of treatment. Body Composition is recognized as an important driver and risk factor for a wide variety of diseases, as well as a predictor of individual patient-specific clinical outcomes to treatment choices or surgical interventions. 3D CT images are routinely acquired in the oncological worklows and deliver accurate rendering of internal anatomy and therefore can be used opportunistically to assess the amount of skeletal muscle and adipose tissue compartments. Powerful tools of artificial intelligence such as deep learning are making it feasible now to segment the entire 3D image and generate accurate measurements of all internal anatomy. These will enable the overcoming of the severe bottleneck that existed previously, namely, the need for manual segmentation, which was prohibitive to scale to the hundreds of 2D axial slices that made up a 3D volumetric image. Automated tools such as presented here will now enable harvesting whole-body measurements from 3D CT or MRI images, leading to a new era of discovery of the drivers of various diseases based on individual tissue, organ volume, shape, and functional status. These measurements were hitherto unavailable thereby limiting the field to a very small and limited subset. These discoveries and the potential to perform individual image segmentation with high speed and accuracy are likely to lead to the incorporation of these 3D measures into individual specific treatment planning models related to nutrition, aging, chemotoxicity, surgery and survival after the onset of a major disease such as cancer.
Automated Body Composition Analysis Using DAFS Express on 2D MRI Slices at L3 Vertebral Level

Abstract:Body composition analysis is vital in assessing health conditions such as obesity, sarcopenia, and metabolic syndromes. MRI provides detailed images of skeletal muscle (SKM), visceral adipose tissue (VAT), and subcutaneous adipose tissue (SAT), but their manual segmentation is labor-intensive and limits clinical applicability. This study validates an automated tool for MRI-based 2D body composition analysis- (Data Analysis Facilitation Suite (DAFS) Express), comparing its automated measurements with expert manual segmentations using UK Biobank data. A cohort of 399 participants from the UK Biobank dataset was selected, yielding 423 single L3 slices for analysis. DAFS Express performed automated segmentations of SKM, VAT, and SAT, which were then manually corrected by expert raters for validation. Evaluation metrics included Jaccard coefficients, Dice scores, Intraclass Correlation Coefficients (ICCs), and Bland-Altman Plots to assess segmentation agreement and reliability. High agreements were observed between automated and manual segmentations with mean Jaccard scores: SKM 99.03%, VAT 95.25%, and SAT 99.57%; and mean Dice scores: SKM 99.51%, VAT 97.41%, and SAT 99.78%. Cross-sectional area comparisons showed consistent measurements with automated methods closely matching manual measurements for SKM and SAT, and slightly higher values for VAT (SKM: Auto 132.51 cm^2, Manual 132.36 cm^2; VAT: Auto 137.07 cm^2, Manual 134.46 cm^2; SAT: Auto 203.39 cm^2, Manual 202.85 cm^2). ICCs confirmed strong reliability (SKM: 0.998, VAT: 0.994, SAT: 0.994). Bland-Altman plots revealed minimal biases, and boxplots illustrated distribution similarities across SKM, VAT, and SAT areas. On average DAFS Express took 18 seconds per DICOM. This underscores its potential to streamline image analysis processes in research and clinical settings, enhancing diagnostic accuracy and efficiency.

Financial compensation following road traffic injury: who is footing the bill?

Abstract:A study was undertaken at Red Cross Children's Hospital to assess how aware parents were of motor vehicle accident (MVA) claim procedures on behalf of their children. Of 160 questionnaires sent out to the parents of road traffic injury (RTI) victims, 100 were answered. Sixty-six per cent of parents who responded had an income below R500 per month. The identity of the motorist responsible for injuring the child was known to only 26 parents. Third party damages were claimed by only 32 parents or guardians; the remaining 68 failed to do so principally through lack of awareness. The implications of these findings for both RTI victims and the hospitals where they are treated are discussed here.

Abstract:Abstract Background Body composition is associated with survival outcome in oncological patients, but it is not routinely calculated. Manual segmentation of subcutaneous adipose tissue (SAT) and muscle is time-consuming and therefore limited to a single CT slice. Our goal was to develop an artificial-intelligence (AI)-based method for automated quantification of three-dimensional SAT and muscle volumes from CT images. Methods Ethical approvals from Gothenburg and Lund Universities were obtained. Convolutional neural networks were trained to segment SAT and muscle using manual segmentations on CT images from a training group of 50 patients. The method was applied to a separate test group of 74 cancer patients, who had two CT studies each with a median interval between the studies of 3 days. Manual segmentations in a single CT slice were used for comparison. The accuracy was measured as overlap between the automated and manual segmentations. Results The accuracy of the AI method was 0.96 for SAT and 0.94 for muscle. The average differences in volumes were significantly lower than the corresponding differences in areas in a single CT slice: 1.8% versus 5.0% ( p < 0.001) for SAT and 1.9% versus 3.9% ( p < 0.001) for muscle. The 95% confidence intervals for predicted volumes in an individual subject from the corresponding single CT slice areas were in the order of ± 20%. Conclusions The AI-based tool for quantification of SAT and muscle volumes showed high accuracy and reproducibility and provided a body composition analysis that is more relevant than manual analysis of a single CT slice.

Abstract:Body tissue composition is a long-known biomarker with high diagnostic and prognostic value in cardiovascular, oncological and orthopaedic diseases, but also in rehabilitation medicine or drug dosage. In this study, the aim was to develop a fully automated, reproducible and quantitative 3D volumetry of body tissue composition from standard CT examinations of the abdomen in order to be able to offer such valuable biomarkers as part of routine clinical imaging. Therefore an in-house dataset of 40 CTs for training and 10 CTs for testing were fully annotated on every fifth axial slice with five different semantic body regions: abdominal cavity, bones, muscle, subcutaneous tissue, and thoracic cavity. Multi-resolution U-Net 3D neural networks were employed for segmenting these body regions, followed by subclassifying adipose tissue and muscle using known hounsfield unit limits. The Sørensen Dice scores averaged over all semantic regions was 0.9553 and the intra-class correlation coefficients for subclassified tissues were above 0.99. Our results show that fully-automated body composition analysis on routine CT imaging can provide stable biomarkers across the whole abdomen and not just on L3 slices, which is historically the reference location for analysing body composition in the clinical routine.

Abstract:Ammonium is generated in culture media by the spontaneous deamination of amino acids at 37 °C and through the metabolism of amino acids by human embryos. The appearance of ammonium is a time-dependent phenomenon and can compromise embryo physiology, development and viability. In this study, the effects of a gradient of ammonium on the development, metabolism and transcriptome of human and mouse embryos were investigated. Pronucleate oocytes were cultured in the presence of an ammonium gradient that mimicked the spontaneous deamination of Eagle's amino acids together with 1 mM glutamine. All embryos were cultured in sequential media G1/G2 at 5% O2, 6% CO2 and 89% N2. Human embryo metabolism was assessed through a non-invasive fluorometric analysis of pyruvate consumption. Transcriptome analysis was performed on the resultant blastocysts from both species using a microarray technology. Embryo development prior to compaction was negatively affected by the presence of low levels of ammonium in both species. Human embryo metabolism was significantly inhibited after just 24 and 48 h of culture. Transcriptome analysis of blastocysts from both species revealed significantly altered gene expression profiles, both decreased and increased. Functional annotation of the altered genes revealed the following over represented biological processes: metabolism, cell growth and/or maintenance, transcription, cell communication, transport, development and transcription regulation. These data emphasize the enhanced sensitivity of the cleavage-stage embryo to its environment and highlight the requirement to renew culture media at frequent intervals in order to alleviate the in vitro induced effects of ammonium build-up in the environment surrounding the embryo.

Abstract:BackgroundComputed tomography (CT) has been increasingly used to quantify abdominal muscle and fat in clinical research studies, and multiple studies have shown importance of body composition in predicting clinical outcome. The purpose of study is to compare newly developed semi-automated software (BodyCompSlicer) to commercially available validated software (Slice-O-Matic) for CT body composition analysis.MethodsCT scans of abdomen at L3 level in 30 patients were analyzed by two reviewers and using two softwares (BodyCompSlicer and Slice-O-Matic). Body composition analysis using BodyCompSlicer was semi-automated. The program automatically segmented subcutaneous fat (SF), skeletal muscle (SM), and visceral fat (VF) areas. Reviewers manually corrected the segmentation using computer–mouse interface as necessary. Body composition analysis using Slice-O-Matic was performed by manually segmenting each area using computer-mouse interface (brush tool). After segmentation, SM, SF, and VF areas were calculated using CT attenuation thresholds. Inter-observer and inter-software variability of measurements were analyzed using intraclass correlation coefficients (ICC) and coefficient of variation (COV).ResultsInter-observer ICC and COV using BodyCompSlicer were 0.997 and 1.5% for SM, 1.000 and 0.8% for SF, and 1.000 and 1.0% for VF, whereas those using Slice-O-Matic were 0.993 and 2.5% for SM, 0.995 and 3.1% for SF, and 0.999 and 2.3% for VF. Inter-software ICCs and COV were 0.995–0.995 and 2.0–2.1% for SM, 0.991–0.994 and 3.4–3.9% for SF, and 0.998–0.998 and 2.8–3.3% for VF. Time to analyze 30 cases was 70–100 min and 150–180 min using BodyCompSlicer and Slice-O-Matic, respectively.ConclusionBodyCompSlicer is comparable to Slice-O-Matic for CT body composition analysis.

Abstract:Purpose: To develop and validate a computer tool for automatic and simultaneous segmentation of body composition depicted on computed tomography (CT) scans for the following tissues: visceral adipose (VAT), subcutaneous adipose (SAT), intermuscular adipose (IMAT), skeletal muscle (SM), and bone. Approach: A cohort of 100 CT scans acquired from The Cancer Imaging Archive (TCIA) was used - 50 whole-body positron emission tomography (PET)-CTs, 25 chest, and 25 abdominal. Five different body compositions were manually annotated (VAT, SAT, IMAT, SM, and bone). A training-while-annotating strategy was used for efficiency. The UNet model was trained using the already annotated cases. Then, this model was used to enable semi-automatic annotation for the remaining cases. The 10-fold cross-validation method was used to develop and validate the performance of several convolutional neural networks (CNNs), including UNet, Recurrent Residual UNet (R2Unet), and UNet++. A 3-D patch sampling operation was used when training the CNN models. The separately trained CNN models were tested to see if they could achieve a better performance than segmenting them jointly. Paired-samples t-test was used to test for statistical significance. Results: Among the three CNN models, UNet demonstrated the best overall performance in jointly segmenting the five body compositions with a Dice coefficient of 0.840+/-0.091, 0.908+/-0.067, 0.603+/-0.084, 0.889+/-0.027, and 0.884+/-0.031, and a Jaccard index of 0.734+/-0.119, 0.837+/-0.096, 0.437+/-0.082, 0.800+/-0.042, 0.793+/-0.049, respectively for VAT, SAT, IMAT, SM, and bone. Conclusion: There were no significant differences among the CNN models in segmenting body composition, but jointly segmenting body compositions achieved a better performance than segmenting them separately.

Abstract:Computed Tomography (CT) allows detailed studies of body composition and its association with metabolic and cardiovascular disease. The purpose of this work was to develop and validate automated and manual image processing techniques for detailed and efficient analysis of body composition from CT data. The study comprised 107 subjects examined in the Swedish CArdioPulmonary BioImage Study (SCAPIS) using a 3-slice CT protocol covering liver, abdomen, and thighs. Algorithms were developed for automated assessment of liver attenuation, visceral (VAT) and subcutaneous (SAT) abdominal adipose tissue, thigh muscles, subcutaneous, subfascial (SFAT) and intermuscular adipose tissue. These were validated using manual reference measurements. SFAT was studied in selected subjects were the fascia lata could be visually identified (approx. 5%). In addition, precision of manual measurements of intra- (IPAT) and retroperitoneal adipose tissue (RPAT) and deep- and superficial SAT was evaluated using repeated measurements. Automated measurements correlated strongly to manual reference measurements. The SFAT depot showed the weakest correlation (r = 0.744). Automated VAT and SAT measurements were slightly, but significantly overestimated (≤4.6%, p ≤ 0.001). Manual segmentation of abdominal sub-depots showed high repeatability (CV ≤ 8.1%, r ≥ 0.930). We conclude that the low dose CT-scanning and automated analysis makes the setup suitable for large-scale studies.

A comparative study of two automated solutions for cross-sectional skeletal muscle measurement from abdominal computed tomography images

Abstract:Background: Measurement of cross-sectional muscle area (CSMA) at the mid third lumbar vertebra (L3) level from computed tomography (CT) images is becoming one of the reference methods for sarcopenia diagnosis. However, manual skeletal muscle segmentation is tedious and is thus restricted to research. Automated solutions are required for use in clinical practice. Purpose: The aim of this study was to compare the reliability of two automated solutions for the measurement of CSMA. Methods: We conducted a retrospective analysis of CT images in our hospital database. We included consecutive individuals hospitalized at the Grenoble University Hospital in France between January and May 2018 with abdominal CT images and sagittal reconstruction. We used two types of software to automatically segment skeletal muscle: ABACS, a module of the SliceOmatic software solution "ABACS-SliceOmatic," and a deep learning-based solution called "AutoMATiCA." Manual segmentation was performed by a medical expert to generate reference data using "SliceOmatic." The Dice similarity coefficient (DSC) was used to measure overlap between the results of the manual and the automated segmentations. The DSC value for each method was compared with the Mann-Whitney U test. Results: A total of 676 hospitalized individuals was retrospectively included (365 males [53.8%] and 312 females [46.2%]). The median DSC for SliceOmatic vs AutoMATiCA (0.969 [5th percentile: 0.909]) was greater than the median DSC for SliceOmatic vs. ABACS-SliceOmatic (0.949 [5th percentile: 0.836]) (p < 0.001). Conclusions: AutoMATiCA, which used artificial intelligence, was more reliable than ABACS-SliceOmatic for skeletal muscle segmentation at the L3 level in a cohort of hospitalized individuals. The next step is to develop and validate a neural network that can identify L3 slices, which is currently a fastidious process.

Abstract:The amounts of muscle and fat in a person's body, known as body composition, are correlated with cancer risks, cancer survival, and cardiovascular risk. The current gold standard for measuring body composition requires time-consuming manual segmentation of CT images by an expert reader. In this work, we describe a two-step process to fully automate the analysis of CT body composition using a DenseNet to select the CT slice and U-Net to perform segmentation. We train and test our methods on independent cohorts. Our results show Dice scores (0.95-0.98) and correlation coefficients (R=0.99) that are favorable compared to human readers. These results suggest that fully automated body composition analysis is feasible, which could enable both clinical use and large-scale population studies.
Estimating 3‐D whole‐body composition from a chest CT scan

Abstract:Background : Estimating whole‐body composition from limited region computed tomography (CT) scans has many potential applications in clinical medicine; however, it is challenging. Purpose : To investigate if whole‐body composition based on several tissue types (visceral adipose tissue (VAT), subcutaneous adipose tissue (SAT), intermuscular adipose tissue (IMAT), skeletal muscle (SM), and bone) can be reliably estimated from a chest CT scan only. Methods : A cohort of 97 lung cancer subjects who underwent both chest CT scans and whole‐body positron emission tomography (PET)‐CT scans at our institution were collected. We used our in‐house software to automatically segment and quantify VAT, SAT, IMAT, SM, and bone on the CT images. The field‐of‐views of the chest CT scans and the whole‐body CT scans were standardized, namely from vertebra T1 to L1 and from C1 to the bottom of the pelvis, respectively. Multivariate linear regression was used to develop the computer models for estimating the volumes of whole‐body tissues from chest CT scans. Subject demographics (e.g., gender and age) and lung volume were included in the modeling analysis. Ten‐fold cross‐validation was used to validate the performance of the prediction models. Mean absolute difference (MAD) and R‐squared were used as the performance metrics to assess the model performance. Results : The R‐squared values when estimating volumes of whole‐body SAT, VAT, IMAT, total fat, SM, and bone from the regular chest CT scans, were 0.901, 0.929, 0.900, 0.933, 0.928, and 0.918, respectively. The corresponding MADs (percentage difference) were 1.44±1.21 L (12.21%±11.70%), 0.63±0.49 L (29.68%±61.99%), 0.12±0.09 L (16.20%±18.42%), 1.65±1.40 L (10.43%±10.79%), 0.71±0.68 L (5.14%±4.75%), and 0.17±0.15 L (4.32%±3.38%), respectively. Conclusion : Our algorithm shows promise in its ability to estimate whole‐body compositions from chest CT scans. Abstract:Background: Muscle wasting (Sarcopenia) is associated with poor outcomes in cancer patients. Early identification of sarcopenia can facilitate nutritional and exercise intervention. Cross-sectional skeletal muscle (SM) area at the third lumbar vertebra (L3) slice of a computed tomography (CT) image is increasingly used to assess body composition and calculate SM index (SMI), a validated surrogate marker for sarcopenia in cancer. Manual segmentation of SM requires multiple steps, which limits use in routine clinical practice. This project aims to develop an automatic method to segment L3 muscle in CT scans. Methods: Attenuation correction CTs from full body PET-CT scans from patients enrolled in two prospective trials were used. The training set consisted of 66 non-small cell lung cancer (NSCLC) patients who underwent curative intent radiotherapy. An additional 42 NSCLC patients prescribed curative intent chemo-radiotherapy from a second trial were used for testing. Each patient had multiple CT scans taken at different time points prior to and post- treatment (147 CTs in the training and validation set and 116 CTs in the independent testing set). Skeletal muscle at L3 vertebra was manually segmented by two observers, according to the Alberta protocol to serve as ground truth labels. This included 40 images segmented by both observers to measure inter-observer variation. An ensemble of 2.5D fully convolutional neural networks (U-Nets) was used to perform the segmentation. The final layer of U-Net produced the binary classification of the pixels into muscle and non-muscle area. The model performance was calculated using Dice score and absolute percentage error (APE) in skeletal muscle area between manual and automated contours. Results: We trained five 2.5D U-Nets using 5-fold cross validation and used them to predict the contours in the testing set. The model achieved a mean Dice score of 0.92 and an APE of 3.1% on the independent testing set. This was similar to inter-observer variation of 0.96 and 2.9% for mean Dice and APE respectively. We further quantified the performance of sarcopenia classification using computer generated skeletal muscle area. To meet a clinical diagnosis of sarcopenia based on Alberta protocol the model achieved a sensitivity of 84% and a specificity of 95%. Conclusions: This work demonstrates an automated method for accurate and reproducible segmentation of skeletal muscle area at L3. This is an efficient tool for large scale or routine computation of skeletal muscle area in cancer patients which may have applications on low quality CTs acquired as part of PET/CT studies for staging and surveillance of patients with cancer.

Abstract:We present a model for cosmological inflation based on a no-scale supergravity sector with an SU(2,1)/SU(2)×U(1) Kähler potential, a single modulus T, and an inflaton superfield Φ described by a Wess-Zumino model with superpotential parameters (μ, λ). When T is fixed, this model yields a scalar spectral index n(s) and a tensor-to-scalar ratio r that are compatible with the Planck measurements for values of λ≃μ/3M(P). For the specific choice λ=μ/3M(P), the model is a no-scale supergravity realization of the R+R2 Starobinsky model.
A Cross-Sectional Validation of Horos and CoreSlicer Software Programs for Body Composition Analysis in Abdominal Computed Tomography Scans in Colorectal Cancer Patients

Abstract:Background: Body composition assessment using computed tomography (CT) scans may be hampered by software costs. To facilitate its implementation in resource-limited settings, two open-source segmentation programs (Horos and CoreSlicer) were transversally validated in colorectal cancer patients. Methods: Contrast-enhanced abdominal CT scans were analyzed following the Alberta protocol. The Cross-Sectional Area (CSA) and intensities of skeletal muscle tissue (MT), subcutaneous adipose tissue (SAT), visceral adipose tissue (VAT), and intramuscular adipose tissue (IMAT) were measured. The Skeletal Muscle Index (SMI) was calculated. Cutoff points were applied to the SMI, MT intensity, and VAT CSA to define muscle atrophy, myosteatosis, and abdominal obesity. The inter-software agreement was evaluated using different statistical tools. Results: A total of 68 participants were measured. The MT CSA and SMI displayed no differences. The MT CSA agreement was excellent, and both programs provided equal muscle atrophy prevalences. CoreSlicer underestimated the MT intensity, with a non-significant myosteatosis prevalence increase (+5.88% and +8.82%) using two different operative definitions. CoreSlicer overestimated the CSA and intensity in both VAT and SAT, with a non-significant increase (+2.94%) in the abdominal obesity prevalence. Conclusions: Both software programs were feasible tools in the study group. The MT CSA showed great inter-software agreement and no muscle atrophy misdiagnosis. Segmentation differences in the MT intensity and VAT CSA caused limited diagnostic misclassification in the study sample.

Abstract:Field-of-view (FOV) tissue truncation beyond the lungs is common in routine lung screening computed tomography (CT). This poses limitations for opportunistic CT- based body composition (BC) assessment as key anatomical structures are missing. Traditionally, extending the FOV of CT is considered as a CT reconstruction problem using limited data. However, this approach relies on the projection domain data which might not be available in application. In this work, we formulate the problem from the semantic image extension perspective which only requires image data as inputs. The proposed two-stage method identifies a new FOV border based on the estimated extent of the complete body and imputes missing tissues in the truncated region. The training samples are simulated using CT slices with complete body in FOV, making the model development self-supervised. We evaluate the validity of the proposed method in automatic BC assessment using lung screening CT with limited FOV. The proposed method effectively restores the missing tissues and reduces BC assessment error introduced by FOV tissue truncation. In the BC assessment for a large-scale lung screening CT dataset, this correction improves both the intra-subject consistency and the correlation with anthropometric approximations.
