Abstract:Purpose: To investigate the biometric differences of anterior segment parameters between fellow eyes of acute primary angle closure (F-APAC) and chronic primary angle closure glaucoma (F-CPACG) to get information about differences between APAC and CPAC. Methods: Patients with F-APAC and F-CPACG without prior treatment were enrolled from glaucoma clinics. Parameters were measured on ultrasound biomicroscopy images, including pupil diameter, lens vault (LV), anterior chamber depth, anterior chamber width, iris area, iris thickness (IT 750 and 2000), angle-opening distance (AOD 500 and 750), trabecular-iris space area (TISA 500 and 750), trabecular iris angle (TIA 500 and 750), trabecular–ciliary angle, and ciliary process area. Multivariate logistic regression analysis was performed to determine the most important parameters associated with F-APAC compared with F-CPACG. Results: Fifty-five patients with APAC and 55 patients with CPACG were examined. The anterior chamber depth, IT 750, AOD 750, trabecular iris angle 750, and trabecular–ciliary angle were smaller, and LV and ciliary process area were greater in F-APAC as compared with F-CPACG (P ≤ 0.01). Multivariate logistic regression showed that thinner IT 750, smaller AOD 750, and larger LV were significantly associated with F-APAC (P < 0.01). IT 750 (area under the curve, 0.703) performed relatively better than AOD 750 (area under the curve, 0.696) in distinguishing F-APAC from F-CPACG, with the best cutoff of 0.404 mm and 0.126 mm, respectively. Conclusions: Compared with F-CPACG, F-APAC had thinner peripheral iris, narrower anterior chamber angle, shallower anterior chamber depth, greater LV, larger and anteriorly positioned ciliary body. IT 750, AOD 750, and LV played important roles in distinguishing eyes predisposed to APAC or CPAC.
Differences Between Fellow Eyes of Acute and Chronic Primary Angle Closure (glaucoma): an Ultrasound Biomicroscopy Quantitative Study

Abstract:Purpose To compare various biometric parameters between fellow eyes of acute primary angle closure (glaucoma) [APAC(G)] and fellow eyes of chronic primary angle closure (glaucoma) [CPAC(G)]. Methods Ultrasound biomicroscopy examinations were performed on 47 patients with unilateral APAC(G) and 41 patients with asymmetric CPAC(G) before laser peripheral iridotomy and pilocarpine treatment. Anterior chamber depth and width (ACD and ACW), lens vault (LV), iris curvature (IC), iris root distance (IRD), trabecular-ciliary process distance (TCPD), iris-ciliary process distance (ICPD), trabecular-ciliary angle (TCA), and other biometric parameters were compared between fellow eyes of APAC(G) and fellow eyes of CAPC(G). Results Compared with fellow eyes of CPAC(G), fellow eyes of APAC(G) had smaller ACD (P < 0.001), ACW (P = 0.007), TCPD (P = 0.016), ICPD (P = 0.008), and TCA (P = 0.006), as well as larger LV (P = 0.002), IC (P = 0.012), and IRD (P = 0.003). On multivariate logistic regression analyses, a 0.1 mm decrease in ACD (odds ratio [OR]: 0.705, 95% CI: 0.564 +/- 0.880, P = 0.002), ICPD (OR: 0.557, 95% CI: 0.335 +/- 0.925, P = 0.024), and a 0.1 mm increase in IRD (OR: 2.707, 95% CI: 1.025 +/- 7.149, P = 0.045), was significantly associated with occurrence of acute angle closures. Conclusions Fellow eyes of APAC(G) had smaller anterior segment dimensions, higher LV, more posterior iris insertion, greater IC, and more anteriorly rotated ciliary body compared with fellow eyes of CPAC(G). ACD, ICPD, and IRD were the three most important parameters that distinguish eyes predisposed to APAC(G) or CPAC(G).
Changes in Choroidal Thickness after Nd∶YAG Laser Peripheral Iridotomy in Patients with Primary Angle Closure

Abstract:Objective:To measure the choroidal thickness of patients with primary angle closure (PAC) before and after Nd:YAG laser peripheral iridotomy (LPI) using enhanced depth imaging spectral-domain optical coherence tomography (EDI-OCT);and to compare the difference between the choroidal thickness of patients with PAC and normal subjects.Methods:This was a prospective cohort study.Thirty patients (48 eyes) who were diagnosed with PAC were selected as the PAC group in the ophthalmology clinic of Shaoxing People's Hospital from October 2015 to February 2017.Thirty patients (50 eyes) undergoing regular physical examination were selected as normal controls in our hospital during the same period.Intraocular pressure and central anterior chamber depth were measured in the normal control group and PAC group before LPI,and 1 week,1 month,3 months and 6 months a~er LPI.Choroidal thickness was measured with EDI-OCT at the subfovea,as well as at 0.5,1.5 and 3.0 mm from the fovea superiorly,inferiorly,temporally,and nasally.Repeated measures ANOVA was used to compare data between the two groups at different time points,and to plot the choroidal thickness of the receiver operating characteristic curve to determine the best diagnostic limits.Pearson correlation analysis was used to assess the correlation among choroidal thickness,intraocular pressure and central anterior chamber depth.Independent t test was used to compare between the two groups.Results:The central anterior chamber depth of the PAC group was more shallow than that of the normal control group (t=-14.383,P＜0.001).The difference in central anterior chamber depth was statistically significant before LPI and 1 week,1 month,3 months and 6 months after LPI (F=10.313,P=0.001).And the central anterior chamber depth of the PAC group became deeper from preoperation to 3 months postoperation (P＜0.01).The choroidal thickness of the PAC group was greater than that of the normal control group at 13 points before LPI (all P ＜ 0.01).The difference in choroidal thickness was statistically significant before and after LPI (F=240.512,P＜ 0.001).The choroidal thickness of the PAC group at 1 week,1 month,3 months and 6 months after LPI was significantly thinner than that at pre-operation (P＜0.001).The choroidal thickness of all subjects in the two groups was negatively correlated with the depth of the central anterior chamber at SF,TI,T2,T3,S2,S3,I1,I2 and I3 (r=-0.249,-0.239,-0.416,-0.330,-0.184,-0.176,-0.189,-0.184,-0.160,P＜0.001).There was no correlation between choroidal thickness,intraocular pressure or axial length.Conclusions:The choroid in patients with PAC is thicker than in the normal group.The choroid in PAC patients became thinner after LPI surgery.Choroidal thickening may be one of the pathogenic factors of PACG.In addition,choroidal thickness plays a role in the early diagnosis of PAC and the disease observation and assessment of the therapeutic effect of LPI.

Abstract:Objective: The aim of this study is to quantify anterior chamber parameters to provide potential risk indicators for evaluating the progression of primary angle-closure suspect (PACS) eyes to acute primary angle closure (APAC) and the degree of intraocular pressure elevation in patients with APAC utilizing anterior segment optical coherence tomography (AS-OCT). Methods: Tomey CASIA2 AS-OCT was used to quantitatively measure various anterior chamber parameters, including anterior chamber depth (ACD), anterior chamber volume (ACV), lens thickness (LT), lens vault (LV), iris-trabecular contact index (ITC), iris thickness (IT), iris volume (IV), iris curvature (IC), iris area (IA), and iris thickness (IT), in APAC eyes (30 eyes) and contralateral PACS eyes (30 eyes) of 30 patients. The differences in these parameters between the two groups and their relationship with intraocular pressure were analyzed. Results: Compared to the PACS group, the APAC group exhibited significantly smaller IA and IC, and significantly larger pupil diameter (PD) and ITC (P < 0.05). There were no statistically significant differences in ACV, ACD, ACW, ACA, LV, IV, and IT750/2000 between the two groups. In APAC eyes, multivariable linear regression analysis showed a significant negative correlation between intraocular pressure and IV (β = -1.85; 95% confidence interval: -2.77 to -0.93; P=0.001), while no correlation was found in PACS eyes. In all 60 eyes, LT showed a negative correlation with ACV, ACD, ACA, and nasal IT750, and a positive correlation with LV and nasal IC. Conclusion: AS-OCT has multiple advantages in evaluating various anterior chamber parameters in patients with glaucoma. IA may serve as a predictive indicator of the progression of eyes from PAC or APAC. A significant negative correlation was found between intraocular pressure and IV during APAC attacks. LT can be considered a predictive factor for the occurrence of primary angle-closure disease.
Correlation Between Pupil Diameter and Angle Configuration in the Dark Room Provocative Test

Abstract:Purpose: To study the associations between changes in pupil diameter and in the anterior chamber angle configuration during the dark room provocative test.Methods: The study included 70 suspects of primary angle closure (study group) and 40 normal participants (control group). Using anterior-segment optical coherence tomography (AS-OCT), we measured the anterior chamber angle configuration and pupil diameter at 3 minutes and at 1.5 hours of dark adaptation.Results: In the study group, the mean number of closed angle segments was significantly (P < 0.001) higher at 3 minutes of dark adaptation (3.9 +/- 2.3) than at room light conditions (2.6 +/- 2.2) or at 1.5 hours of dark adaptation (3.1 +/- 2.1), with no statistically significant (P > 0.05) difference between the 2 latter values. In the study group, the pupil diameter was significantly associated with the number of closed angle segments at 3 minutes of dark adaptation (P < 0.01), but not at 1.5 hours of dark adaptation (P = 0.13). Defined as an increased number of closed angle segments, the dark room test was positive in a significantly higher number of participants at 3 minutes than at 1.5 hours of dark adaptation [44 (63%) eyes vs. 31 (44%) eyes; P < 0.05]. Final interocular pressure was statistically better correlated with the angle configuration segments assessed at 3 minutes (correlation coefficient r(2) = 0.33) than at 1.5 hours of dark adaptation (r(2) = 0.21).Conclusions: After short-term dark adaptation of 3 minutes, the assessment of the anterior chamber angle configuration by AS-OCT is more reliable than as if carried out after a long-term dark adaptation and may be used to identify suspects of primary angle closure.

Abstract:Primary angle closure disease (PACD) is a major cause of blindness worldwide. It has a high prevalence in East Asia, especially in China, which leads to a higher incidence of blindness than open-angle glaucoma. The aim of this study was to directly observe the circumlental space (CLS) in laser peripheral iridotomized eyes with PACD and to determine whether this structure plays a role in the pathogenesis of PACD. Fifty eyes of 50 patients with PACD, who had received laser peripheral iridotomy performed with neodymium:yttrium–aluminum-garnet were recruited from glaucoma clinics from March 2021 to May 2022, including 17 primary angle closure suspect (PACS), 16 primary angle closure (PAC) and 17 primary angle closure glaucoma (PACG). They were classified into two groups based on whether the ciliary process and the crystalline lens equator were in contact using slit-lamp photograph: the attached group and the unattached group. The demographic, clinical characteristics and anterior segment parameters measured from ultrasound biomicroscopy were compared between the attached group and the unattached group. Thirty-three eyes were assigned to the attached group and 17 eyes belonged to the unattached group. In the unattached group, the mean CLS was 0.10 ± 0.07 mm. No significant differences were identified between the different diagnosis groups in age, sex, best-corrected visual acuity, intraocular pressure, white-to-white, axial length, central corneal thickness, anterior chamber depth, flat keratometry, steep keratometry or iridotomy diameter ( p > 0.05). The unattached group had shorter trabecular-ciliary process distance ( p = 0.021) and larger ciliary process area ( p = 0.001) compared with the attached group. Small CLS and its potential effect (partial ciliary block) might be considered as one of the mechanisms of PACD.

Abstract:Background To compare intraocular pressure (IOP) and anterior segment parameters between eyes with unilateral primary angle closure glaucoma (PACG) and their fellow eyes with primary angle closure (PAC) or primary angle closure suspect (PACS). Abstract:To investigate the mechanism of primary angle closure glaucoma (PACG) especially the role of the changes of ciliary body using ultrasound biomicroscope (UBM) combined with general A scan.99 eyes with PACG, including 50 eyes with acute and 49 eyes with chronic PACG, were evaluated and 58 normal eyes were in the control group.Compared with the normal eyes the PACG eyes had their anatomic features. They had a smaller corneal diameter, shorter axial length, shallower anterior chamber, thicker lens, more relative anterior location of lens, swelling of ciliary process and anterior rotation of ciliary body. All these differences were significant statistically (P < 0.05). Compared with the chronic PACG eyes the acute PACG eyes had a shallower anterior chamber and a more anteriorly situated lens position. The differences were significant statistically (P < 0.01). Compared with the normal eyes the acute PACG eyes had a shorter ciliary body - lens distance and the difference was significant (P < 0.05); the chronic PACG eyes also had a shorter ciliary body-lens distance, but the difference was not significant (P > 0.05).The PACG eyes have their anatomic features. Among these factors the pupillary block caused by the anterior displacement of lens and the narrow angle induced consequently are the common mechanism. The swelling ciliary process, the positional changes of the ciliary body and age-related thicker lens are possibly the initial causes causing the forward movement of lens, and the pupillary block plays a more important role in the pathogenesis of acute PACG.

Abstract:Acute primary angle closure glaucoma (APACG) is an ophthalmic emergency that may lead to irreversible blindness. Although efforts were made to control intraocular pressure, disease progression still existed. Anterior segment optical coherence tomography (AS OCT) may provide a new insight into mechanism of APACG. In order to seek out associated factors by AS OCT, We compared anterior segment parameters between 74 APACG and 48 normal eyes. Analysis of variance, principle component analysis and logistic regression were used. APACG group had more women (75.7% vs 47.9%, p = 0.002), smaller anterior chamber (anterior chamber depth, ACD; anterior chamber area, ACA; all p = 0.001), narrower angle (AOD500, AOD750, angle opening distance at 500um and 750um; TISA500, TISA750, trabecular-iris space area at 500um and 750um; ARA500, ARA750, angle recess area at 500um and 750um; all p < 0.001), thinner iris (iris thickness at 750um, IT750; maximum of iris thickness, ITMAX; all p < 0.001), larger iris area (IA, p < 0.001) and lens vault (LV, p = 0.005). Principle component extracted were AOD500, AOD750, TISA500, TISA750, ARA500, ARA750, IA, PD (pupillary diameter), ACD, ACA and IT750. After adjusting for confounding factors, AOD750 (b = 12.40 ± 2.56, p < 0.001), IT750 (b = 10.50 ± 3.45, p = 0.002) and IA (b = −1.56 ± 0.77, p = 0.044) were significantly associated with occurrence of APACG.

Abstract:The potential neurotoxicity of iohexol and iopamidol in lumbar myelography was investigated in 20 patients using three criteria: clinical symptoms, EEG recordings with spectral analysis and CT scans of the brain. There was no significant difference in clinical side-effects between the two groups (Iopaminol, Iohexol). In 10 patients (iopaminol 4, iohexol 6) CT scans revealed an important penetration of the contrast media into the subarachnoid spaces at 3 hours and into the cerebral cortex at 24 hours. This cerebral contamination was unrelated to the headaches experienced by the patients or to the electrophysiological disturbances observed. Spectral analysis in brain-contaminated subjects showed a reduction of delta, beta 1 and beta 2 bands with iohexol. This reduction suggests that EEG activation is less pronounced with this agent than with iopaminol.
Comparision of the Anatomical Structure Between Acute Angle-Closure Glaucoma and Chronic Angle-Closure Glaucoma by A-mode Ultrasonic Scanning

Abstract:Objective To study the difference between acute angle-closure glaucoma and chronicangle-closure glaucoma in anatomical structure. Methods anatomical structures including the anterior cham-ber depth(ACD), lens thickness(LT) and the axial length(AL),were measured by A-mode ultrasonic scanning in33 eyes of 33 patients with AACG, 36 eyes of 36 patients with CACG and 36 eyes of 36 normal controls. Therelative location of lens(RLL) and LT/AL ratio were also calculated. All mean values were analyzed with t-test.Results There were significant differences between the AACG eyes and the CACG eyes in all items exceptRLL. Compared with normal eyes, both AACG eyes and the CACG eyes had significant differences in all items.Conclusions There were significant differences between the AACG eyes and the CACG eyes in anatomicalstructure. AACG eyes had a shallower ACD, thicker LT, shorter AL. It indicates that there is different mecha-nism between AACG and CACG.

Abstract:Objective To investigate the timing of laser peripheral iridotomy(LPI)for the treatment of primary angle closure(PACS).Methods A total of 126 patients(252 eyes) diagnosed with PACS were enrolled.There were 77 females(154 eyes) and 49 males(98 eyes).Mean age of the patients was 61±4.53 years(range,47~79 years).The main criteria for diagnosis of PACS were:1.no history of attack,signs and symptoms of acute PACG;2.normal intraocular pressure;3.the depth of peripheral anterior chamber≤1/4CK;4.narrow angle width(gradeⅡ or less,Shaffer'S grading system);5.no iris-trabecular contact demonstrated by compression gonioscopy;6.no glaucomatous optic nerve damage and visual field defect.The patients were re-examined every 3 months.The average follow-up was longer than 5 years.Results 35 of 252 eyes with PACS develop into PAC.prevalence of PAC was 13.9%(35/252).Conclusion Not all patients with PACS develop PAC.In patients with any of the following features,PACS is easy to develop into PAC,and prophylactic iridotomy may be indicated in these situations:(1)Critically narrow angle(grade Ⅰor less);(2)The adhesion of angle gradually develops;(3)Narrow angle with positive dark room test;(4)Narrow angle with a well-documented family history of PACG;(5)Narrow angle with history of previous angle-closure attacks;(6)Need for repeated pupillary dilation and extensive retinal treatment(e.g.,panretinal photocoagulation for abetic retinopathy);(7)Hyperopia with senile cataract;(8)Younger patients,especially those who live in urban area away from medical care.
Correlation between choroidal parameters and primary angle-closure suspect in different age groups

Abstract:Objective: To investigate the correlations between choroidal parameters and primary angle-closure suspect (PACS) in different age subgroups. Methods and analysis: Participants aged 50 years or older in a rural area of Daxing District, Beijing, were recruited. Swept-source optical coherence tomography was used to measure the choroidal parameters. Demographic, ocular biometry parameters and choroidal parameters were compared between the PACS and non-PACS (NPACS) eyes. Logistic analysis was performed to explore the association between the choroidal parameters and PACS. Results: 192 (26.89%) subjects with PACS and 509 (71.29%) with PACS were analysed. Subjects were divided into two groups: group 1 (50-60 years, n=286) and group 2 (>60 years, n=415). In group 1, the mean subfoveal choroidal thickness of PACS eyes was 341.82±88.23 µm and thicker than NPACS eyes (315.07±83.53 µm, p=0.035). The choroidal volume was greater in PACS eyes (10.61±2.78 mm3) compared with NPACS eyes (9.66±2.49 mm3, p=0.013). In group 2, no significant difference in any choroidal parameters between PACS and NPACS was found. Multivariate regression demonstrated that increased choroidal volume was associated with PACS (OR 1.298, 95% CI 1.117 to 1.510, p<0.001) in group 1. Conclusions: In the age group of 50-60 years, PACS eyes had greater choroidal thickness and volume than NPACS eyes, and the increased total choroidal volume was a predisposing factor for PACS. Abstract:Background: The purpose of this study was to analyze the ocular biometric parameters of primary angle-closure disease (PACD) in younger patients and compare them with those of elderly patients. Methods: This clinic-based, cross-sectional study included 154 eyes of 154 patients with PACD, consisting of 77 eyes of patients aged 40 years or younger and 77 eyes of patients older than 40. The PACD case definition was compatible with the ISGEO definition. Anterior segment parameters were measured by ultrasound biomicroscopy, axial length (AL) and lens thickness (LT) were measured by A-scan ultrasonography measurements, and the thickness of the retina and choroid were measured by optical coherence tomography. The differences in ocular biometric parameters between different age groups were compared by independent sample t- tests or Mann-Whitney U tests, and the correlation between the parameters and age was analyzed. Results: Compared to older PACD patients, the lens vault(LV),LV/LT and subfoveal choroidal thickness (SFCT) of younger patients were larger, while the peripheral and mean iris thickness (IT), trabecular-ciliary angle (TCA), ciliary body thickness (CBT), AL and LT were smaller (all P < 0.01). There was no significant difference in anterior chamber depth, anterior chamber width, pupil diameter, angle opening distance at 500 μm from the scleral spur, anterior chamber angle and iris convexity between the two groups (all P > 0.05). AL, LT, IT, TCA and CBT were positively associated with age (all P < 0.001), while LV and SFCT were negatively associated with age ( P = 0.027 and P < 0.001, respectively). Conclusions: Compared with elderly patients, younger PACD patients had more anteriorly positioned lenses, thinner and more anteriorly rotated ciliary bodies, thicker choroids, and shorter axial length. These characteristics might be important anatomical bases for the earlier onset of PACD and the higher risk of malignant glaucoma after filtering surgery.

Anterior Lens Curvature Matters in the Course of Primary Angle Closure: an Analysis Based on Ultrasound Biomicroscopic Imaging

Abstract:Purpose. To evaluate the effect of anterior lens curvature in primary angle closure (PAC) and find additional anatomical features of crystalline lens that may predispose primary angle closure to the acute course. Methods. 435 eyes (263 subjects) were enrolled in this study. Four groups of eyes were included based on angle configurations and clinical features: (i) acute primary angle closure (APAC, 140 eyes); (ii) chronic primary angle closure (CPAC, 116 eyes); (iii) primary angle closure suspect (PACS, 84 eyes); and (iv) normal controls (95 eyes). All patients underwent thorough ophthalmic exams including applanation tonometry, gonioscopy, low-coherence interferometry, and ultrasound biomicroscopic imaging. Based on the panoramic anterior segment images from ultrasound biomicroscopic imaging measurements, the radius of anterior lens curvature (ALR) was calculated using the least-squares curve fitting technique. ALR, in addition to axial length (AL), anterior chamber depth (ACD), and lens thickness (LT), was compared among different groups using univariate and multivariate analysis with mixed effects linear model. Results. APAC, CPAC, and PACS groups all had steeper ALR, shorter AL, shallower ACD, and thicker LT than normal control group. ACD and LT further differ between APAC and CPAC or PACS eyes. Moreover, a steeper ALR was also found in the APAC group as compared to CPAC, PACS, and normal control groups. Conclusions. A steeper ALR may predispose the acute attack of PAC. In addition to the relative lens position and size, lens curvature is another variable that contributes to the pathophysiological mechanisms of primary angle closure.

Abstract:Background Primary angle-closure glaucoma (PACG) is a major cause of visual morbidity in East Asia. Dark-room provocative test (DRPT) has been used to determine which narrow angles have the risk to develop angle closure. However, the accuracy of DRPT might be altered because that after emerging from the dark room, the configuration of the angle is affected by the light of the slit-lamp and the appositionally closed angle reopens. The aim of this study was to examine the pupillary diameter in different light conditions and use it as a parameter to assess the accuracy of dark-room provocative test.Methods Patients with suspected primary angle-closure glaucoma undergoing DRPT were recruited. The anterior chamber angle was examined by anterior segment optical coherence tomography under the following conditions: (1) in standard room illumination; (2) after short-term dark-adaptation and (3) after DRPT Mean values of pupil size and numbers of appositionally closed angle under different conditions were compared.Results A total of 47 eyes of 47 patients were analyzed. The pupil size after DRPT was smaller than that after short-term dark-adaptation (P<0.001) and smaller than that in standard room illumination (P=0.026). The numbers of appositionally closed angles after short-term dark-adaptation were significantly larger than those after DRPT (P<0.001). There was no significant difference between the numbers of appositionally closed angles in standard room illumination and after DRPT (P=0.157).Conclusions Constriction of pupil size immediately after prolonged dark room provocative test may lead to change in the angle configuration, which may lead to false negative results. We suggest a modified protocol of recording intraocular pressure immediately after DRPT and performing gonioscopy following short-term dark adaptation to improve the accuracy of angle closure assessment. Changes of Anterior Segment Parameters in Primary Angle Closure with Axial Myopia: a Retrospective Study of 369 Patients Grouped by Axial Length

Abstract:Myopia is uncommon in primary angle closure. A recent research showed that myopic primary angle-closure patients had no significant differences in anterior segment parameters compared with emmetropic and hyperopic counterparts (Yong et al. 2014). However, the refractive status of these patients is often misleading due to lenticular myopia, including cataract formation after the acute attack and age-related nuclear cataract. Therefore, refractive myopia might occur in short-axial eyes, which still reflects the biometric risk factor for these diseases. The real atypical primary angle closure is related to axial myopia rather than refractive myopia. We sought to establish whether anterior segment parameters would show significant differences when grouped by axial length (AL). This retrospective study was conducted of our primary angle-closure clinical database. Patients who received intra-ocular surgery or laser peripheral iridotomy or who had plateau iris or other ocular diseases were excluded. Primary angle-closure diseases were classified as primary angle-closure suspect (PACS), primary angle closure (PAC) and primary angle-closure glaucoma (PACG) based on the criteria proposed by Foster et al. (2002). We studied 369 patients (82 PACS, 150 PAC and 137 PACG). Anterior chamber depth (ACD), corneal curvature (CC) and AL were obtained by A-scan biometry (AL-3000 Biometer/Pachymeter; Tomey, Nagoya, Japan). Axial length (AL) was categorized as short (<22.5 mm), medium (≥22.5 to <23.5 mm) or long (≥23.5 mm). Data of left eyes were used for analyses. spss 19.0 was performed for all statistical analyses using Chi-square, anova and pairwise t-tests with Bonferroni adjustment. p values < 0.05 were considered statistically significant. Mean age of the patients was 60.8 ± 10.4 years, with most being female (n = 255, 69.1%). Short AL was present in 220 patients (59.6%), medium AL in 108 patients (29.3%) and long AL in 41 patients (11.1%). Across the angle-closure subgroups, there were no significant differences in mean age (p = 0.480) or gender ratio (p = 0.904). The proportions of angle-closure subgroups among different AL groups were similar (p = 0.821). Among the primary angle-closure patients, those with long AL had deeper ACD and flatter CC than those with medium (p = 0.029 for ACD; p = 0.001 for CC) or short AL (p < 0.001 for ACD; p < 0.001 for CC) (Fig. 1A). Significant correlations were found between AL and ACD (r = 0.273, p < 0.001) and between AL and CC (r = 0.576, p < 0.001) (Fig. 1B). Our findings (Fig. 1Cb) are contrary to those of Yong et al. (2014) who reported that there were no significant differences in ACD and CC in eyes with axial myopic angle closure by comparing different refractive status (Fig. 1Ca). Our results might explain the low incidence of myopia in primary angle-closure patients. Lowe (1970) found that 5.5% of primary angle-closure patients were myopic, whereas Chakravarti & Spaeth (2007) reported that 1.9% of 322 patients had high myopia. Additionally, our study might suggest different mechanisms of these atypical primary angle closure. Future research might assess other anterior segment parameters such as anterior chamber width, peripheral iris thickness and lens vault to further verify this. Our study had several limitations. Firstly, the retrospective nature may have led to bias. Secondly, lens thickness was not routinely measured. Thus, we could not directly evaluate lens element for angle closure. However, we did access ACD, which is regarded as the most cardinal factor for angle closure (Foster et al. 2000), and can indirectly represent lens condition. Moreover, AL and ACD were only weakly correlated, so larger cohort studies are needed to establish the clinical significance of such association.

Abstract:Use of combined oral contraceptives (OCs) has been associated with certain types of cardiovascular diseases (e.g. venous thromboembolism thrombotic stroke myocardial infarction). Nevertheless some researchers have criticized the data supporting this association. Shearman compiled their major criticisms in an article. The 1st criticism concerns the incompatibility of the national trends in cardiovascular mortality with estimates of risk attributable to the pill derived from case-control and cohort studies. However a proper analysis of mortality trends taking into account all relevant facts about OC use shows that the observed patterns are in fact compatible with a wide range of possible risks attributable to the pill. The 2nd point deals with the assertion that excess deaths in pill users have been almost entirely attributed to smoking. However an excess risk of fatal and nonfatal cardiovascular events associated with OC use has been observed both in nonsmokers and in comparisons of users and nonusers of OCs smoking similar number of cigarettes/day. The 3rd point concerns studies showing a relationship between OC use and venous thromboembolism because clinical diagnosis of deep venous thrombosis is unreliable and potentially subject to bias. However these studies have methodological defects and their findings are not conclusive. The final point focuses on the extent to which pill use is related to subarachnoid hemorrhage and whether or not there is a residual excess risk of cardiovascular disease in ex-OC users. With regard to the subarachnoid hemorrhage association the Royal College of General Practitioners found a strong association between OC use and subarachnoid hemorrhage in its cohort study (20 deaths in total relative risk users: nonusers 4.0:1). As to the residual excess risk in ex-OC users some studies suggest an excess risk and others do not. In the past few years recent changes in OC composition may have resulted in subsequent reduction in OC-related mortality.
Choroidal expansion as a mechanism for acute primary angle closure: an investigation into the change of biometric parameters in the first 2 weeks

Abstract:BACKGROUND/AIM: Choroidal expansion with anterior movement of the lens was recently proposed as a mechanism for acute primary angle closure (APAC). The aim of this study was to compare the biometric parameters, central anterior chamber depth, limbal chamber depth, lens thickness, and lens position, within 24 hours of presentation and 2 weeks later in eyes with APAC.METHODS: This was a prospective observational case series of 41 subjects with APAC. Subjects who presented with APAC were treated with medical therapy followed by laser iridotomy (LI) in both eyes once the acute attack was broken. Ocular biometry was performed in affected and fellow eyes before LI (baseline) and then 2 weeks later. Optical pachymetry was used to measure central anterior chamber depth (ACD) and the limbal chamber depth (LCD) was graded at the slit lamp. A-scan ultrasound was used to measure lens thickness (LT) and axial length (AL). Lens position (LP) was defined as ACD +(1/2)LT.RESULTS: The majority of subjects were Chinese (83%) and female (61%), and the mean age was 60.4 (SD 10.3) years. In affected eyes, the ACD was 1.81 (0.29) mm before and 1.80 (0.28) mm 2 weeks after LI (p = 0.63), while in fellow eyes, the ACD was 1.83 (0.29) mm and 1.81 (0.38) mm, respectively (p = 0.21). There was no significant change in lens position, relative lens position, or axial length in both affected and fellow eyes over the 2 weeks.CONCLUSIONS: There was no change observed in central anterior chamber depth, lens thickness, or lens position at the time of the acute attack compared to 2 weeks later in both APAC affected and fellow eyes. The findings do not support the hypothesis of lens movement due to choroidal expansion in APAC.

Abstract:Precis: Different mechanisms of angle closure represented distinct aspects of intraocular pressure (IOP) control after phacoemulsification. Classification of angle closure mechanisms is necessary for postoperative IOP management and glaucoma progression in primary angle closure eyes. Purpose: To investigate the relationship between the anterior chamber angle (ACA) characteristics, measured by swept-source anterior segment optical coherence tomography (SS AS-OCT), and intraocular pressure (IOP) control after phacoemulsification in eyes with primary angle closure disease (PACD) with different angle closure mechanisms. Methods: PACD eyes were classified into 3 groups according to angle closure mechanisms using preoperative SS AS-OCT images; pupillary block (PB), plateau iris configuration (PIC), exaggerated lens vault (ELV). This retrospective, clinical cohort study included eighty-five eyes of 85 PACD patients: 34 with PB, 23 with PIC, and 28 with ELV. ACA parameters were measured preoperatively and 1 month postoperatively using SS AS-OCT. IOP measurements were performed preoperatively and during six months postoperatively. Postoperative IOP reduction and fluctuation were calculated, and their correlations with SS AS-OCT parameters were analyzed. Results: PIC group showed the lowest postoperative IOP reduction compared to the other groups ( P =0.023). Preoperative ACA measurements were significantly associated with postoperative IOP reduction in ELV and PB groups, while postoperative measurements were in PIC group. Preoperative and postoperative change of iridotrabecular contact (ITC) index and area were correlated with postoperative IOP reduction in PB and ELV groups but not in PIC group. Postoperative ITC index ( P =0.031) and area ( P =0.003) showed significant correlations with postoperative IOP fluctuation only in PIC group. Conclusions: SS AS-OCT parameters including ITC index and area showed different associations with postoperative IOP control, which should be considered in determination of lens extraction and treatment of PACD eyes.
