Abstract:Objective: In 2021, several patients across the United States received bone allograft contaminated with Mycobacterium tuberculosis (TB). TB is typically a pulmonary infection with many possible extrapulmonary manifestations, including skeletal tuberculosis. However, TB is a rare causative organism of postoperative surgical site infection. Iatrogenic skeletal TB infections are not widely reported in the medical literature; therefore, treatment and associated outcomes are relatively unknown. In this series, the authors report 6 cases of patients who received a mesenchymal stem cell-enhanced bone graft infected with TB at their institution, including the clinical courses, imaging findings, management plans, and outcomes at 1 year postoperatively. Methods: A retrospective review was performed of 6 consecutive patients who underwent spinal fusion surgery at the authors' institution and received bone graft from a lot contaminated with TB. Collected data included patient demographic characteristics, indications for surgery, surgical procedures performed, timing of contamination discovery, medical treatment, and follow-up information including reoperation, healing progress, and imaging findings. Results: Five of 6 patients (83.3%) eventually tested positive for TB via interferon-gamma release assay or wound culture. They experienced significant complications, including surgical site infections with neck swelling, pain, dysphagia, and wound dehiscence. Extensive soft-tissue infection was common; however, significant bony involvement was not observed. Surgical wound debridement was required in 4 patients, and all patients received medical management with standard RIPE (rifampin, isoniazid pyrazinamide, pyridoxine, and ethambutol) therapy for 8 weeks with extension of rifampin and isoniazid for scheduled 12 months. All patients (excluding 1 patient who died of COVID-19) showed signs of improvement with adequately healing wounds at the most recent follow-up at a median (range) of 12 (6-13) months postoperatively. To date, no patients have developed pulmonary TB. Conclusions: Direct inoculation with TB via contaminated bone grafts resulted in a high rate of severe soft-tissue infection, although extensive skeletal and pulmonary involvement has not been observed at 1 year postoperatively; this review includes the longest reported follow-up period for this TB outbreak. Medical management remains the mainstay of therapy for these patients, with most patients showing recovery with oral antibiotic therapy. The severity of these infections arising from mesenchymal stem cell-containing bone allografts that undergo an alternative sterilization process than standard allografts raises concerns regarding the added risks of infection, which should be weighed against the expected benefits of these grafts.

Abstract:During a 9‐year period, from January 1981 to December 1989, 6 mycobacterial infections were diagnosed among 2380 orthotopic liver transplants performed at the University of Pittsburgh. These infectious complications, although rare, were a major source of morbidity and mortality (40%). There were several associated risk factors in these patients, including extensive travel through or birth in endemic areas, severe pretransplant illness, post‐transplant rejection requiring adjunct (i.e., OKT 3 ) immunosuppression, and HIV infection. Five patients had M. tuberculosis (MTB) infections; the 6th patient developed a solitary pulmonary nodule which grew mycobacterium avium intracellulare (MAI). Invasive diagnostic studies were usually necessary to document MTB infection and tissue samples often revealed early granuloma formation. Multiple drug regimens (INH, Rifampin, Ethambutol, and Pyrazinamide) were initiated early in the clinical course and effective therapy was judged by resolution of the clinical illness and/or radiographic resolution. The patient with MAI had no pulmonary or infectious symptoms. He was observed without anti‐tuberculous therapy and the pulmonary nodule decreased in size 3 months after the diagnosis was made. We recommend that all patients have pre‐operative PPD skin tests. Only those patients who fall into high‐risk categories, demonstrating x‐ray evidence of pulmonary disease or those who have a documented recent skin conversion, should receive INH prophylaxis in the peri‐operative period because of the associated risk of INH hepatotoxicity in these patients. MAI infection in asymptomatic patients may be managed with expectant observation. If the patient develops pulmonary symptoms, or the lesion persists, pulmonary nodule resection and antituberculous chemotherapy guided by culture sensitivities may be appropriate.

Abstract:After two multistate outbreaks of allograft tissue‐transmitted tuberculosis (TB) due to viable bone, evidence‐based donor screening criteria were developed to decrease the risk of transmission to recipients. Exclusionary criteria, commentary, and references supporting the criteria are provided, based on literature search and expert opinion. Both exposure and reactivation risk factors were considered, either for absolute exclusion or for exclusion in combination with multiple risk factors. A criteria subset was devised for tissues containing viable cells. Risk factors for consideration included exposure (e.g., geographic birth and residence, travel, homelessness, incarceration, healthcare, and workplace) and reactivation (e.g., kidney disease, liver disease, history of transplantation, immunosuppressive medications, and age). Additional donor considerations include the possibility of sepsis and chronic illness. Donor screening criteria represent minimal criteria for exclusion and do not completely exclude all possible donor TB risks. Additional measures to reduce transmission risk, such as donor and product testing, are discussed but not included in the recommendations. Careful donor evaluation is critical to tissue safety.

Analysis of Nanohydroxyapatite/Polyamide-66 Cage, Titanium Mesh, and Iliac Crest in Spinal Reconstruction of the Patients with Thoracic and Lumbar Tuberculosis

Abstract:Background: The standard recommended and common reconstruction method for spinal tuberculosis is titanium mesh bone graft and autogenous iliac crest. However, these methods have their own disadvantages. Objective: To evaluate the clinical efficacy of one-stage posterior debridement with iliac bone graft, titanium mesh bone graft, or nanohydroxyapatite/polyamide-66 cage in thoracic and lumbar tuberculosis. Materials and methods: Between January 2013 and December 2018, 57 patients with thoracic or lumbar tuberculosis were treated by interbody bone graft combined with posterior internal fixation after debridement. Thirteen patients were treated with iliac bone graft to construct the stability of the vertebral body, 26 patients were treated with titanium mesh bone graft, and 18 patients were treated with nanohydroxyapatite/polyamide-66 cage bone graft. The main clinical results were evaluated by intervertebral height, cage subsidence, operation time, operative blood loss, postoperative hospitalization, postoperative complications, visual analog scale (VAS) score, erythrocyte sedimentation rate (ESR), C-reactive protein (CRP), American Spinal Injury Association (ASIA) grade, and bone graft fusion time. All the outcomes were recorded and analyzed by statistical methods. Results: The mean follow-up time was 24.5 months. Neurologic function was improved in most patients at the last follow-up. There were significant differences in ESR, CRP, and VAS score between preoperative and postoperative values; however, there were no significant differences in ESR, CRP, and VAS score among the three groups. There were no significant differences in operation time, blood loss, postoperative hospitalization, and postoperative complications among the three groups at discharge. There was no significant difference in ASIA grade among the three groups at the last follow-up. Nanohydroxyapatite/polyamide-66 cage group had a lower cage subsidence (P = 0.013). The bone graft fusion time of the nanohydroxyapatite/polyamide-66 cage group was significantly shorter than the iliac bone graft group and the titanium mesh bone graft (P < 0.05). Conclusions: The follow-up outcomes showed that the method involving one-stage posterior debridement and internal fixation, interbody graft, and fusion is an effective and safe surgical method for patients with thoracic and lumbar tuberculosis. The incidence rate of cage subsidence was less and the bone graft fusion time was shorter with nanohydroxyap atite/polyamide 66 cage when compared with iliac bone graft and titanium mesh bone graft in the surgical treatment of thoracic and lumbar tuberculosis. Nanohydroxyapatite/polyamide-66 cage has a promising application prospect to be a new bone graft material.

Abstract:Osteomyelitis caused by nontuberculous mycobacteria (NTM) can have severe consequences and a poor prognosis. Physicians therefore need to be alert to this condition, especially in immunocompromised patients. Although the pathogenesis of NTM osteomyelitis is still unclear, studies in immunodeficient individuals have revealed close relationships between NTM osteomyelitis and defects associated with the interleukin-12-interferon-γ-tumor necrosis factor-α axis, as well as human immunodeficiency virus infection, various immunosuppressive conditions, and diabetes mellitus. Culture and species identification from tissue biopsies or surgical debridement tissue play crucial roles in diagnosing NTM osteomyelitis. Suitable imaging examinations are also important. Adequate surgical debridement and the choice of appropriate, combined antibiotics for long-term anti-mycobacterial chemotherapy, based on in vitro drug susceptibility tests, are the main therapies for these bone infections. Bacillus Calmette-Guerin vaccination might have limited prophylactic value. The use of multiple drugs and long duration of treatment mean that the therapeutic process needs to be monitored closely to detect potential side effects. Adequate duration of anti-mycobacterial chemotherapy together with regular monitoring with blood and imaging tests are key factors determining the recovery outcome in patients with NTM osteomyelitis.
Lumbar Spine Infected by Mycobacterium tuberculosis and Cryptococcus neoformans: a Rare Case Report

Abstract:Background: In July 2023, our hospital confirmed one case of lumbar spine infected complicated by Mycobacterium tuberculosis and Cryptococcus neoformans. The patient was admitted due to lower back pain for 1 year and a hard lump for 3 months. Symptoms and signs: Dressing can be seen fixed on the lower back, with severe bleeding. When the dressing is removed, a hard and protruding lump with a size of 6 cm x 8 cm, a sinus tract can be seen near the mass, with a slightly red wound and a sinus depth of about 3 cm. Light red fluid can be seen flowing out. There are no symptoms such as redness, swelling, or heat in the rest of the lower back, and the patient has no other underlying diseases or surgical history. Methods: Lumbar magnetic resonance imaging and lumbar CT examination; Percutaneous puncture lumbar vertebral biopsy was performed, and the biopsy tissue was subjected to pathological examination, mNGS (metagenomic next-generation sequencing), and acid-fast staining; Extract pus from the lump for fungal culture and ink staining, and identify the fungi through MALDI-TOF MS. Results: Bone destruction and bone marrow edema in the L5 vertebral body, compression of the spinal canal at the L5 vertebral body level; The pathological results of the biopsy tissue indicate granulomatous lesions. The acid-fast staining of the tissue is positive, and the mNGS of the tissue indicates infection with Mycobacterium tuberculosis. A single fungus was cultured from pus and identified by MALDI-TOF MS as Cryptococcus neoformans. Clinically, isoniazid 0.3 g ivgtt + rifampicin 0.45 g qd po + ethambutol 0.25 g qd po + pyrazinamide 0.75 g qd po + fluconazole 0.3 g qd po was administered for treatment. After 11 days, there was slight pain at the incision site, and the original symptoms were significantly relieved. The wound dressing was fixed in place, dry and without obvious exudation. Improved and discharged, followed up for 3 months with no recurrence of the lesion. Conclusions: mNGS is an effective identification technique that can be used to accurately diagnose suspected infection cases. MALDI-TOF MS has significant advantages over traditional detection methods in shortening detection time. This case achieved satisfactory treatment results for patients through a reasonable treatment plan, which is of great significance for exploring the diagnosis and treatment of similar disease infections.
[Tuberculous prosthetic knee joint infection: a case report and literature review]

Abstract:Objective: Prosthetic late infection occurs in the second month after surgery in the context of haematogenous spread from another source. Prosthetic mycobacterial infection is a rare complication whose clinical management is not standardized. Case: Patient of 77 years with no personal history except for diabetes and a prosthetic replacement of right knee with osteoarthritis three years ago. Patient goes to hospital emergency box for 6 months pain in the right knee with mechanical inflammatory signs but no fever associated. After their return within 5 days and clinical worsening is reporting growth of Mycobacterium tuberculosis in knee aspirate and antitubercular treatment is established for 9 months. Nuclear magnetic resonance imaging studies also confirmed the diagnosis of tuberculosis spondylitis in the clinical context of the patients. After surgery, M. tuberculosis was again isolated from intraoperative samples and therefore the patient received another batch of treatment for 9 months. After a year of monitoring, the development was acceptable but few months later, the patient died for cardiovascular causes. In the literature review, 15 publications with a total of 17 clinical cases of prosthetic infection by M. tuberculosis were found from 1980 to 2014. Conclusions: Prosthetic tuberculous arthritis, although it is a rare presentation, it should be noted, especially in patients with predisposing conditions with a history of tuberculosis infection.
Nontuberculous mycobacterial infection after lung transplantation: a report of four cases

Abstract:BackgroundNontuberculous mycobacterium (NTM) infection in a patient in an immunosuppressed state caused by increased use of immunosuppressive or biological agents is a serious clinical problem. Mycobacterium avium complex is the most common involved pathogen, followed by Mycobacterium abscessus (MABSC), while Mycobacterium kansasii is not a major concern. The rate of infection rate in lung transplant recipients is reported to range from 1.5–22.4%.Case presentationWe report here four cases of NTM pulmonary infection and disease among 63 patients who underwent lung or heart-lung transplantation at our hospital. Those four occurred following living-donor transplantation in two patients, one with pulmonary arterial hypertension and one with bronchiectasia, and deceased donor lung transplantation in two patients, lymphangioleiomyomatosis and interstitial pneumonia, respectively. NTM was not detected in any of the patients prior to transplantation. The involved pathogens were Mycobacterium gordonae in one, MAC in one, and MABSC in two of these patients, which were isolated from broncho-alveolar lavage (BAL) in two and sputum in two. The one case of MAC and two of MABSC were symptomatic with consolidation shown in chest CT images indicating possible pneumonia, while the one with M. gordonae had no symptoms and was detected by surveillance BAL. Onset time from detection of NTM was greater than 3 years in the three with MABSC and M. gordonae and less than 3 years in the one with MAC. Each patient required a decrease in immunosuppressive agents according to their condition, while antibiotics therapy was performed in the three who were symptomatic. Sputum culture findings became negative after several months and were maintained thereafter in all.ConclusionAn NTM infection leading to pulmonary disease can occur at any time following lung transplantation. Treatment should be considered depending on the involved pathogens, individual status, and disease severity.
Middle- and long-term follow-up of freeze-dried cancellous allograft in the treatment of spinal tuberculous spondylitis

Abstract:AIM: To investigate the ability of freeze-dried cancellous allograft, which was packed in a cylindrical mesh titanium cage, in reconstructing the anterior column and facilitating osseous fusion for thoracic and lumbar tuberculousis surgery. METHODS: There were 24 cases of thoracic and lumbar tuberculousis, who underwent radical débridement and freeze-dried cancellous allograft fusion between January 1999 and March 2005, were followed by mail and telephone. And 19 patients accepted the follow-up, which included 11 thoracic and 8 lumbar tuberculousis patients. The patients with thoracic tuberculousis underwent anterior transpleural and extrapleural for T12 radical débridement, then the titanium mesh-allograft composite and anterior instrumentations were used for anterior spinal column reconstruction. For the lumbar tuberculousis patients, after retroperitoneal débridement, the titanium cage-allograft composite and anterior and /or posterior instrumentations were also used. Cage subsidence was measured in relation to the rhomboid-shaped fenestrations on the cages, each measuring 9 mm in height, measuring the penetration in the cephalad and caudal endplates separately. The evaluation of osseous fusion was made as follows: Grade Ⅰ, fused with remodeling and trabeculae Grade Ⅱ, graft intact,not fully remodeled and incorporated and no lucency Grade Ⅲ, graft intact with potential lucency at the top or bottom of graft and Grade Ⅳ, fusion absent with collapse or resorption of graft. RESULTS: Nineteen patients completed the follow-up for at least 30 months 30-73 month with an average of 51.2 months, and the follow-up rate was 79.2%. ① The average in-hospital time was 6-26 days mean 10.1 days. ② At the final follow-up, the cage subsidence was measured to be an average of 0.60±0.06 rhomboid fenestrations at the cephalad endplate and 0.40±0.06 rhomboid-shaped cage fenestrations at the caudal endplate, with a range of 0-1.0 fenestrations of subsidence at each endplate. The total mean subsidence was 1.0 of the height of the cage fenestrations, or approximately 9 mm. ③ Bone fusion was observed in all the 19 patients at the final follow-up. X-ray showed that the formation of bone bridge along the titanium cage, and no lucency at the top or bottom of graft. Among the thoracic tuberculousis patients, 9 cases of Grade I and 2 cases of Grade Ⅱ bone fusion were achieved While 6 cases of Grade I and 2 cases of Grade Ⅱ bone fusion were achieved among the lumbar tuberculousis patients.CONCLUSION: Freeze-dried cancellous bone is a liable graft material in spinal turberculosis arthrodesis. For the sake of allograft incorporation and remodeling, the rigid instrumentation should be achieved.

Abstract:OBJECTIVESTo summarize the clinical results in the treatment of spinal tuberculosis with debridement, bone grafting and anterior fixation and to evaluate the safety and the value of this procedure.METHODSFrom June 1997 to May 2001, 18 patients with spinal tuberculosis were treated using anterior debridement, autograft of bone and primary internal instrumentation. They were 8 men and 10 women, aged from 25 to 59 years (mean 41 years). The degree of kyphosis before surgery was 27.0 degrees to 75.5 degrees (mean 47.5 degrees +/- 11.4 degrees ). The involved spines included cervical spine (1 patient), thoracic spine (10), thoracic-lumbar spine (2), and lumbar spine (5). Average 2.8 intervertebral bodies in each patient were afflicted with tuberculosis disease. Spinal fusions were done with iliac bone grafts.RESULTSAll patients were followed up for an average of 25 months. No deep wound infection and sinus were observed after surgery. The grafted bones were fused in all patients with an average time of 3.6 months. The degree of spine kyphosis correction was 32.7 degrees +/- 8.3 degrees, and 3.2 degrees +/- 2.8 degrees was lost on average in the late stage.CONCLUSIONAnterior instrumentation for spinal tuberculosis could stabilize the spine, correct kyphosis and fuse the grafted bone.
DOPS assessment: A study to evaluate the experience and opinions of trainees and assessors

Abstract:Background: Workplace based assessments (WBAs) have been part of UK training for the last 3 years. Carrying out procedures efficiently and safely is of paramount importance in anaesthesia. Aims: To explore opinions and experiences of Direct Observation of Procedural Skills (DOPS) assessments in a regional anaesthetic training programme. Methods: 19 and 20-item questionnaires were distributed to trainees and consultants respectively. Results: Questionnaire response rate was 76% (90/119) for trainees and 65% (129/199) for consultants. 43% of consultants and 33% of trainees were not trained in DOPS use. Assessments were usually not planned. 50% were ad hoc and the remainder mainly retrospective. Time spent on assessment was short with DOPS and feedback achieved in ≤15 minutes in the majority of cases with lack of suggestions for further improvement. Both trainees and consultants felt that DOPS was not a helpful learning tool (p = 0.001) or a reflection of trainee competency. Conclusions: DOPS assessments are currently not valued as an educational tool. Training is essential in use of this WBA tool which needs to be planned and sufficient time allocated so as to address current negative attitudes.
APPLICATION OF FREEZE-DRIED CANCELLOUS ALLOGRAFT IN TREATMENT OF SPINAL TUBERCULOSIS

Abstract:OBJECTIVE:To investigate the efficacy of freeze-dried cancellous allograft in the treatment of spinal tuberculosis.METHODS:From January 1999 to August 2004, there were 31 cases of spinal tuberculosis who underwent surgery. The freeze-dried cancellous allograft was used as grafting material in all the cases. The cancellous allograft was packed in a titanium mesh cage or an artificial vertebrae, and then used as a strut graft anteriorly to implant into the bone defect after the radical debridement, and the instrumentation was done.RESULTS:Twenty-three cases were followed up 1.5 years to 5 years (3.7 years on average), and bony fusion was achieved in 21 cases 6 months later. In 2 cases ceasing antituberculous therapy after 2 months of operation, the local recurrence was obvious. The loosened screw was noticed in one of these two cases, who had tuberculosis in lumbar spine. When antituberculous therapy continued, the bony fusion was observed in these two cases 12 months later. No further position change of the instrument was noticed in the patient carrying loosened screw, but the kyphosis of the thoracolumbar spine aggravated.CONCLUSION:Freeze-dried cancellous allograft could be used in the treatment of spinal tuberculosis. To achieve good results of allograft incorporation and remodeling, the rigid instrumentation should be performed, postoperative antituberculous therapy is also important.

Abstract:Autogenous granular bone graft (AG), autogenous massive bone graft (AM), and titanium mesh bone graft (TM) are the three commonly utilized bone implant methods for spinal tuberculosis. However, the gold standard is still controversial. Therefore, this study aimed to compare the clinical efficacy and surgical safety of three primary bone graft modalities. For systematic literature review, several databases, including PubMed, Embase, and Web of Science, were searched up to December 2022. Stata (version 14.0) was employed for data analysis. Our network meta-analysis included 517 patients from 7 articles whose qualities are acceptable based on our quality assessment criteria. In direct comparison, AG was associated with a shorter operation time (MD = 73.51; CI 30.65–116.37) and a lesser blood loss (MD = 214.30; CI 7.17–421.44) than AM. TM had fewer loss of Cobb angle than AG (MD = 1.45; CI 0.13–2.76) and AM (MD = 1.21; CI 0.42–1.99). Compared with AG, TM (MD = 0.96; CI 0.06–1.87) was related to a shorter bone graft fusion time. Abstract:We report 3 cases of tuberculosis occurring after liver transplantation. Two patients presented with fever, 2 and 8 months after transplantation, while the 3rd patient was asymptomatic. In the 1st case, disseminated disease developed rapidly; the other 2 patients presented with pulmonary disease only. In all patients the mycobacterial infection was treated with INH, rifampin and pyrazinamide, as well as ethambutol in the 1st case. Anti‐tuberculous treatment was complicated in the 1st case by development of severe INH hepatitis after 9 months, necessitating drug withdrawal. The prophylaxis, diagnosis and treatment of mycobacterial infections in liver transplant recipients are discussed.

Abstract:BACKGROUND CONTEXT Exploring artificial materials for anterior column reconstruction following the removal of thoracolumbar tuberculosis focus is of great value. The low possibility of adhesion and biofilm formation of Mycobacterium tuberculosis (M.tuberculosis) on nano-hydroxyapatite/polyamide 66(n-HA/PA66) provides a foundation for successful internal fixation in spinal tuberculosis. However, evidence on both clinical and microbiological perspectives is still lacking. PURPOSE To evaluate the adherence ability of M.tuberculosis in n-HA/PA66 and the long-term results of n-HA/PA66 in clinic application to spinal tuberculosis. STUDY DESIGN/SETTING Not Applicable PATIENT SAMPLE Not Applicable OUTCOME MEASURES Not Applicable METHODS M.tuberculosis were cultured with 4 types of metal segments, smooth or rough surfaced n-HA/PA66, and titanium, respectively. The characteristics of adhesion and biofilm formation were observed under scanning electron microscope. There were twenty patients who underwent the debridement and strut graft with n-HA/PA66 cage combined with anterior instrumentations between June 2009 and January 2011 included in this study. There were 9 male and 11 female patients with the mean age of 39.81±7.34 years. The level of surgery was T3-T10 in 8 cases, T11-L2 in 10 cases, and L3-L4 in 2 cases. The AIS grade was C in 4, D in 11, and E in 5 patient. Patients with cold abscess was found in 15 cases, intraspinal involvement in 16 cases, sinus in 2 cases. The neurological functions of patients with spinal fracture were evaluated by ASIA grading. Visual analog scale (VAS) and Japanese Orthopaedic Association (JOA) score were used to evaluate the improvement of the clinical symptoms. The X-ray and three-dimensional CT were followed up in all these patients to observe bony fusion, cage subsidence and translocation. RESULTS The average value of colony forming units (CFUs) of M. tuberculosis in smooth or rough surfaced n-HA/PA66 were significantly lower than those in titanium (P<0.01). M. tuberculosis biofilms had not formed at the n-HA/PA66. All patients successfully completed one-stage lesion debridement, the mean operating time was 187.4±44.3 min, intraoperative blood loss was 382.61±89.26 ml. Twenty cases of patients with postoperative neurological damage did not worsen, including 1 case of neurological function improved from preoperative grade C to postoperative grade D, but two days later the neurological function decreased to grade C due to paravertebral effusion. Finally, the last follow-up neurological function recovered to grade D after incision and drainage. The VAS score was relieved from 7.11±1.75 points before operation to 2.42±0.61 points after operation and 1.38±0.42 points at the last follow-up. The JOA score was increased from 6.7±3.1 points before operation to 15.7±4.5 points after operation and 23.4±3.3 points at the last follow-up. All cases showed no screw loosening, implant displacement or breakage. The preoperative kyphosis Cobb angle range was 14° to 82°(32.41°±11.45°), which decreased to 2° to 47° (mean of 19.55°±10.21°) postoperatively. However, at the final follow-up, the mean kyphosis Cobb angle slightly increased to 3° to 57° (mean of 23.44°±9.15°). The mean correction of kyphosis Cobb angle was 10° to 36° (mean of 14.07°±4.01°), while the mean loss of kyphosis Cobb angle was 1° to 11° (mean of 4.02°±0.77°). Bony fusion was achieved in 12 patients (60%) at 6 months, in 19 patients (95%) at 1 year, and in all patients at 2 years. However, during the follow-up period, cage subsidence was common in this group of cases, occurring in 11 cases (55%). The subsidence mainly occurred within 6 months after operation, ranging from 0 to 6 mm (mean of 2.45±0.81 mm). CONCLUSIONS The use of n-HA/PA66 has been proven effective in reducing the attachment of M. tuberculosis to the implant surface. Additionally, long-term clinical follow-up results have confirmed that this material is an ideal option for anterior column reconstruction following the removal of thoracolumbar tuberculosis focus. Effectiveness and Safety Analysis of Titanium Mesh Grafting Versus Bone Grafting in the Treatment of Spinal Tuberculosis: a Systematic Review and Meta-Analysis

Abstract:BACKGROUND:To systematically assess the safety and effectiveness of titanium mesh grafting compared with bone grafting in the treatment of spinal tuberculosis.METHODS:Electronic databases, including PubMed, Embase, Web of Science, and Cochrane Library, were searched from their inception until April 2023. The outcome indicators for patients treated with titanium mesh grafting or bone grafting for spinal tuberculosis include surgical duration, intraoperative blood loss, graft fusion time, American Spinal Injury Association (ASIA) Spinal Cord Injury Grade E assessment, VAS score, lumbar pain score, post-graft kyphotic angle, and postoperative complications. The Newcastle-Ottawa Scale (NOS) and the Grading of Recommendations Assessment, Development, and Evaluation (GRADE) approach were used for quality assessment and evidence grading of clinical studies. Funnel plots and Begg's test were employed for bias assessment.RESULTS:A total of 8 studies were finally included, comprising 523 patients, with 267 cases of titanium mesh fixation and 256 cases of bone grafting. The meta-analysis showed no significant statistical differences in surgical duration (Weighted Mean Difference (WMD) = -7.20, 95% Confidence Interval (CI): -28.06 to 13.67, P = 0.499), intraoperative blood loss (WMD = 16.22, 95% CI: -40.62 to 73.06, P = 0.576), graft fusion time (WMD = 0.97, 95% CI: -0.88 to 2.81, P = 0.304), ASIA Spinal Cord Injury Grade E assessment (Relative Risk (RR) = 1.03, 95% CI: 0.97 to 1.09, P = 0.346), and overall complications (RR = 0.87, 95% CI: 0.49 to 1.55, P = 0.643). Differences in VAS score, ODI lumbar pain score, and post-graft kyphotic angle between the titanium mesh grafting group and the bone grafting group were not significant within the 95% CI range. The rate of postoperative implant subsidence was slightly lower in bone grafting than in titanium mesh grafting (RR = 9.30, 95% CI: 1.05 to 82.22, P = 0.045).CONCLUSIONS:Both bone grafting and titanium mesh grafting are effective and safe for the surgery, with no significant statistical differences in the results. Considering the limitations of the present study, large-scale randomized controlled trials are warranted to further verify the reliability of this finding.

It also seeks to summarize strategies for preventing complications and provide a reference for clinical treatment. Methods: We retrospectively analyzed 105 patients with spinal tuberculosis treated in our hospital from January 2018 to December 2022. Patients were divided into two groups: Group A (n=38), who underwent endoscopic spinal tuberculosis focus removal, and Group B (n=67), who underwent posterior pedicle lesion removal, bone grafting, and internal fixation. All patients received preoperative quadruple anti-tuberculosis drug chemotherapy (Isoniazid, Rifampicin, Pyrazinamide, Ethambutol) for 2– 4 weeks. We recorded and compared parameters such as operation time, intraoperative blood loss, postoperative drainage, recovery time, hospital stay, blood transfusion, and complications. We also compared pre- and postoperative lumbar VAS, ODI, erythrocyte sedimentation rate, C-reactive protein, Procalcitonin, D-Dimer, Neutrophil to Lymphocyte ratio, hemoglobin, total protein, and Cobb angle to evaluate kyphotic deformity and correction. Results: Preoperative data including age, sex, body mass index, segment involvement, past medical history, smoking history, preoperative erythrocyte sedimentation rate, C-reactive protein, Procalcitonin, D-Dimer, Neutrophil to Lymphocyte ratio, hemoglobin, total protein, lumbar VAS score, lumbar ODI score, and spinal Cobb's angle showed no significant difference between the groups (P> 0.05). Main postoperative indicators revealed that Group A had significantly lower lumbar VAS scores, higher hemoglobin and total protein levels, and a lower recurrence rate than Group B at the last follow-up (P< 0.05). Secondary postoperative indicators showed that Group A required significantly less blood transfusion, had shorter operative time, less intraoperative blood loss, shorter postoperative hospitalization time, and lower Cobb Angle than Group B (P< 0.05). Conclusion: Endoscopic spinal tuberculosis focus removal can achieve comparable safety to posterior pedicle lesion removal, bone grafting, and internal fixation, with shorter operation time, less intraoperative bleeding, lower postoperative recurrence rate, and less postoperative drainage. This method can effectively improve blood sedimentation, lumbar function, restore Cobb angle, and reduce pain, making it worthy of promotion and application. Despite a declining incidence due to increased emphasis and standardization of treatment, China remains one of the countries with the highest tuberculosis burden worldwide. 2 According to World Health Organization (WHO) data, the annual number of tuberculosis cases reported in China accounts for approximately 10% of the global total. 3,4 Spinal tuberculosis is the most common type of extrapulmonary TB, with thoracolumbar tuberculosis being the most prevalent form, accounting for 50% of osteotuberculosis cases. It is more common in the elderly due to factors such as decreased immune function, weakened resistance, and the presence of chronic diseases. Abstract:【Objectives】To evaluate the long-term treatment effect of debridement, decompression and bone graft for treatment of thoracolumbar tuberculosis with paraplegia. 【Method】 There were 39 cases in this series, 2 cases were thoracic tuberculosis and 21 cases were thoracolumbar tuberculosis, 6 cases were lumbar tuberculosis. Anterior decompression and bone graft were used in all cases, All patients were performed with chemotherapy from 6 months to 12 years postoperatively,and immobilization in a brace for 6 months. The patients were followed up periodically, and neurologic status, roentgenogram, ESR and liver function test were the outcomes to be concerned. 【Results】 The mean follow-up period was 35 months (range 12-72 months). There was no recurrence of the disease or a little loss of Cobb angulations. Most of the patients had obvious recovery (ASIA scale was E). 【Conclusion】 The key which treat thoracolumbar tuberculosis with paraplegia is radical decompression, the long-term treatment effect of simple bone graft and immobilization in a brace for treatment of thoracolumbar tuberculosis with paraplegia was safe and effective.

Abstract:Mycobacterium tuberculosis (MTB) infection of a prosthetic joint is rarely reported in developed countries.1 Typically, MTB infection involves the hips or knees, and the infection can occur secondary to crushing and degradation of the granuloma during surgery or, less commonly, from distant foci spreading through the blood. In the present case, MTB infection likely resulted from haematogenous spread since multiple hot spots suggestive of MTB infection were noted in other sites. Early diagnosis allows for antitubercular therapy with retention of the prosthesis, while late diagnosis frequently results in removal and reimplantation of the joint. To avoid major surgery, a high index of suspicion is required to diagnose prosthetic joint tuberculosis.
