Medial talonavicular-calcaneal fracture-dislocation in an adolescent: A challenging traumatic association (case report)

Abstract:Introduction: Talus fractures in children are rare, with a prevalence of 0.008 % of all pediatric fractures. Subtalar dislocation accounts for 1 % of dislocations in trauma cases, making this combination extremely rare. Case presentation: 14-year-old patient suffered an isolated, uncomplicated closed injury to the right ankle from an accident. X-Ray showed an medial subtalar dislocation and a Hawkins stage III fracture of the talar neck. One hour post-trauma, reduction was performed under general anesthesia using Bootjack Technique, and the ankle was immobilized with a plaster cast. The reduction was confirmed by CT-scan. Given the satisfactory anatomical reduction and maintained articular congruence, orthopedic treatment approach was indicated. After 06 weeks of immobilization, physiotherapy sessions were initiated. The patient did not experience any functional impairment and resumed sports activities 03 months post-trauma. Discussion: According to the literature, closed subtalar fracture-dislocation, has not been previously described in the pediatric population. Diagnosing dislocation is straightforward, with treatment involving reduction through a boot jack maneuver. If a fracture is present, irreducibility should raise suspicion of interposition, requiring open reduction. While surgical treatment is often used, it increases the risk of ischemia. Orthopedic treatment remains a reliable and effective option, particularly in children, as it allows quicker management and avoids iatrogenic complications. Conclusion: Subtalar fracture-dislocation in children is rare, and its management is not standardized. However, it is crucial to consider and identify this type of injury due to its rarity and significant risk of complications. Appropriate management is essential to accelerate treatment and avoid the risk of talar necrosis.

Abstract:Introduction: The subtalar dislocations are rare, this is described as a dislocation of the talo-calcaneal and talo-navicular joints but the tibio-talar joint is intact. The diagnosis is easy and done with x-ray: antéro-posterior and lateral view. A emergency reduction is necessary In a few cases, you need a surgical approach if the dislocation is irreducible or no stable after reduction. Case Report: A man aged of 19 years, felt in climbing, with a forced varus. In emergency room, the clinical exam showed a deformation and the x-ray: a talo-naviculo-calcaneal dislocation. A reduction is done in the operative room, the testing a showed a stable ankle so we stabilized with a posterior splint. During the post-operative consultation: the control x-ray showed a good reduction and the CT: a minimal osteo-cartilagny tearing. The sport is authorized after three month. Conclusion: The talo-naviculo-calcaneal dislocation is rare following a plantar flexion and inversion. A diagnosis is quickly and easily done and the reduction is done in the operative room under general anesthesia. A cast is applied during some weeks before starting the physiotherapy. A surgical treatment is done in a few rare: irreducible – instable – open dislocation.
A simple cavity model for radiation analysis of unidirectional dielectric radiator (UDR)

Abstract:In this paper, a simple cavity model is presented for the theoretical prediction of the radiation patterns of unidirectional dielectric radiator (UDR). The antenna structure is first modeled as a non-radiative dielectric waveguide (NRD) resonator, the electromagnetic fields at the radiating aperture of the antenna structure are then determined for the calculation of the radiation fields and patterns of the UDR antenna. A good agreement between the calculations and measurements shows the validity of the analysis model.

Abstract:Subtalar dislocation is an unusual clinical case with very little mention in literature constituting nearly 1% of all traumatic dislocations. Here a 40 year old male who sustained fall over inverted foot was diagnosed as closed isolated medial subtalar dislocation with no associated fractures. This is an even more rare entity. Immediate closed reduction was done and immobilised for 4 weeks with below knee cast. This case report describes the mechanism of injury, ligaments involved, manoeuvre of closed reduction. This case report highlights the importance of prompt and urgent reduction and short period of immobilisation to avoid complications and better prognosis. Abstract:IntroductionTotal subtalar dislocation without fracture of the ankle is a rare clinical entity; it is usually due to a traumatic high-energy mechanism. Standard treatment is successful closed reduction under general anesthesia followed by non-weight bearing and ankle immobilization with a below-knee cast for 6 weeks.Case presentationWe present the case of a 30-year-old Moroccan woman who was involved in a road traffic accident. She subsequently received a radiological assessment that objectified a total subtalar dislocation without fracture of her ankle. She was immediately admitted to the operating theater where an immediate reduction was performed under sedation, and immobilization in a plaster boot was adopted for 8 weeks. The management of this traumatic lesion is discussed in the light of the literature.ConclusionsMedial subtalar dislocation is a rare dislocation and is not commonly seen as a sports injury because it requires transfer of a large amount of kinetic energy. The weaker talocalcaneal and talonavicular ligaments often bear the brunt of the energy and are more commonly disrupted, compared to the relatively stronger calcaneonavicular ligament. Urgent reduction is important, and closed reduction under general anesthesia is usually successful, often facilitated by keeping the knee in flexion to relax the gastrocnemius muscle. Long-term sequelae include talar avascular necrosis and osteochondral fracture, as well as chronic instability and pain.

Abstract:Dislocation of the tibiotalar joint without associated fracture is rare. We present here a 21-year-old man who sustained open posteromedial dislocation of the left ankle without malleolar facture when he jumped and sprained his right ankle while playing basketball. The most likely mechanism is forced flexion applied to the ankle joint leading to a rupture of the anterior capsule and lateral structures of the ankle followed by an accelerating inversion stress leading to a posteromedial dislocation of the talus from the tibial condyle. Transient paresthesia was noted in the area of the superficial peroneal nerve. At surgery, the anterior part of the tibiotalar joint capsule and anterior talofibular ligament were detached from their original sites. The calcaneofibular ligament was also detached with its associated periosteum and a tiny avulsed bony fragment. The articular facets of the tibia and talus were intact. The treatment consisted of wound irrigation, debridement, reduction and capsular suture followed by immobilization with a short leg cast. About 10 degrees of loss in the range of dorsiflexion was observed. The patient achieved good long-term functional results.

Properties of neurons from dissociated fetal rat brain in serum-free culture

Abstract:Because of the unknown constituents and varying composition of serum, its presence in media used in cell culture unavoidably compromises attempts to study cellular mechanisms of growth and differentiation. To overcome this, we have devised a serum-free, chemically defined medium which maintains primary cultures of fetal rat brain cells for more than 6 weeks. This medium allows expression of characteristic properties of neurons and prevents overgrowth of non-neuronal elements without use of antimitotic agents. Cells prepared and plated without exposure to serum attach in less than 20 min to poly-D-lysine substratum and begin to extend processes within 1 hr. After 2 days in culture, process-bearing cells can be divided into those with characteristic neuronal morphology, including long processes which generally branch at a distance from the perikaryon, and those having the appearance of glial cells with many short, thin processes which branch frequently near the cell body. The remaining non-neuronal cells are large and flat with few or no processes. The presence of neurons and astroglia was demonstrated by immunofluorescence detection of bound tetanus toxin as a neuron- specific surface marker, and glial fibrillary acidic protein as an astroglial marker. By the 3rd day in culture, many cells of neuronal morphology were able to generate action potentials in response to electrical stimulation. The ionic composition of the inward current changes from Ca2+ to predominantly Na+ by about 10 days in culture. The presence of synaptic vesicles and myelin was demonstrated by electron microscopy. The ability of dissociated cells from mammalian brain to grow in defined medium without serum and acquire selected properties of mature cells in vivo demonstrates the potential of this culture system for neurobiological studies at the cellular level.

Abstract:Introduction: dislocations of carpal bones without associated fractures are considered a rare injury, the most common mechanism of injury being axial loading with wrist in extension plus ulnar deviation. The literature reports a wide variety of complex carpal injuries, even so, it is possible to identify previously undescribed injuries. Objective: to present an atypical case of a patient with an injury to the midcarpal stabilizing mechanism and the stabilizing mechanism of the proximal row of the wrist following trauma to the hand that required carpectomy as definitive surgical treatment. Presentation of case: a 48 year old male patient is presented who is admitted to our hospital unit after presenting injury to the left hand after being run over by a motor vehicle, with axial load mechanism, presenting deformity in the left thoracic extremity, fracture of the proximal metaphysis of the second phalanx of the third finger as well as dislocation of the proximal interphalangeal joint, with traumatic amputation of the second phalanx of the fourth finger plus extensor injury in zone V of the fifth finger with loss of skin coverage of the fourth and fifth finger, attending our hospital unit 24 hours after the injury. Conclusions: carpal bone dislocations are an orthopedic emergency, with 20% going unnoticed in trauma centers. Early closed reduction is the initial treatment to avoid severe complications, however, surgical treatment is the gold standard for fixation. Carpectomy is considered a mostly adequate sequelae management treatment, however it is well accepted for complex injuries to the wrist stabilization mechanisms, as it can be performed in a short surgical time and early rehabilitation can be initiated and functional ranges of motion can be achieved with low sequelae.

Abstract:Background: The talus bone is the second-largest bone in the tarsal region. Talar fractures are rare injuries that occur due to highenergy traumas. Open reduction and internal fixation (ORIF) is the treatment of choice in the talar body fracture-dislocation. Avascular necrosis (AVN), osteoarthritis (OA), malunion, non-union, skin infections, and ankylosis of the subtalar joint are complications that can happen following the talar body fracture. Case Report: We reported a rare open fracture-dislocation of the talar body concomitant with the anterior talofibular ligament (ATFL) rupture in a 24-year-old motorcyclist man. ORIF with two 4.5 mm cancellous screws and ATFL repair were performed. After 6 months of follow-up, the radiographic bone union was achieved, and the patient had no complaints of pain, disability, and discomfort. Conclusion: According to the blood supply of the talus bone, appropriate management and follow-up should be considered to prevent severe consequences. Due to the lack of literature on this lesion, in this report, we tried to provide more comprehensive information regarding the diagnosis, treatment, and follow-up of this group of patients. To the best of our knowledge, our case will probably be the second case of the talar body fracture concomitant with rupture of the ATFL.

Abstract:Background: Reports in the literature on talar head fractures are rare and limited to case reports and small case series. Questions/purposes: This study aimed to describe a national cohort of talar head fractures for fracture characteristics, associated injuries, treatment, and reoperations. Methods: We reviewed all radiographs of patients enrolled in the SFR between 2011 and 2021 showing talar head fractures (AO/OTA 81-A3). We excluded those with talar head avulsion fractures, misclassified, multiple registrations, or with a privacy notice in their medical records. The cohort was reviewed using medical records and radiographs at a minimum 2-year follow-up. Results: The study included 32 patients (33 fractures) ≥18 years of age. The median age was 32 (range 18-65) years, 84 % were men, and the main trauma mechanisms were motor vehicle accidents (28 %) and falls from heights (28 %) and graded as high-energy injuries in 50 % of the cases. 18 (55 %) were displaced and 15 (45 %) had comminuted fractures. 14 talar head fractures (42 %) had multiple injuries to the same foot. 21 patients (66 %) underwent surgical treatment, most commonly with screw fixation. Surgery was performed in 15 of 18 patients with displaced fractures. Four patients were reoperated, one with arthrodesis of the talonavicular joint and three for implant removal. No cases of avascular necrosis were identified. Conclusions: Talar head fractures are rare and occur mainly in men. They are associated with other foot injuries. Most talar head fractures are treated operatively. In this case series, we did not identify any case of avascular necrosis. Levels of evidence: IV, retrospective observational cohort study.

Abstract:The association of ipsilateral talar and calcaneal fractures is an uncommon combination and is typically the result of a high-energy trauma. It is often associated with comminution, marked fracture displacement, and soft-tissue compromise. Obtaining satisfactory clinical and radiographic results is very challenging. Residual deformities, multiple procedures, and limitations are usually the norm. Therefore, studies have suggested that primary arthrodesis may represent the best option of surgical treatment. In this study, we report a case of a 30-year-old male patient with a rare combination of a highly comminuted transcalcaneal fracture-dislocation associated with a talar neck fracture successfully treated with open reduction and internal fixation (ORIF) with an 18-month follow-up. This case demonstrates that even when there are associated fractures of the talus and calcaneus with severe bone loss, ORIF may provide satisfactory outcomes in the short-term postoperative period.Levels of Evidence: Level V: Case report.

Abstract:Objective Subtalar dislocation associated with fracture of the posterior process of talus is very rare. The diagnose and treatment about this rare case was reported and the related literatures were reviewed to summarize the anatomy of the posterior process, mechanism and treatment of this pattern injury. Methods The literatures about the Subtalar dislocation associated with fracture of posterior process of talus before October 2017 were searched in PubMed, CNKI, Weipu and Wanfang database by using "subtalar dislocation" and "posterior process fracture of talus" as key words. Six cases reported in English and 1 case reported in Chinese were detected. The clinical and imaging findings, mechanism of this pattern injury, treatment and clinical prognosis of 7 cases reported in literature and 1 case in this paper were observed and analysed. Results A 25-year-old male suffered an traffic accident was presented with pain, swelling, malformation and activity limitation at the right ankle.The X-ray revealed subtalar dislocation and an unconfirmed fracture. The closed reduction and plaster fixation were performed immediately to correct the subtalar dislocation but the result was not satisfying. X-ray and CT scan revealed subtalar medial dislocation associated with navicular fracture and posterior process fracture of talar. The patient underwent open reduction and internal fixation 27 days later after the accident and got a good result in 6 months after operation. Eight cases of subtalar dislocation associated with fracture of the posterior process of talus are involved, including 1 case in this paper and 7 cases reported, 7 males and 1 female, age ranges from 17 to 52 years old, all the patients were presented with pain, swelling, malformation and activity limitation at the ankle. Seven cases underwent open reduction and internal fixation and 1 case was treated by minimally invasive technique. The X-ray and CT scan revealed a good reduction after open reduction and internal fixation. There was no pain at the hindfoot and the patient regained a good activity 3 months after the surgery. Conclusions When at the time of the impact the foot is in inversion and plantarflexion medial subtalar dislocations occur. The avulsion force from talocalcaneal ligament lead to fracture of posterior process of talus. Subtalar dislocation associated with nondisplaced fracture can be treated conservatively, open reduction and internal fixation is recommended to the case that with fracture displaced more than 2 mm. Pure Ankle Dislocation: A Case Report and a Narrative Literature Review

Abstract:Pure dislocation of the ankle is an extremely rare injury accounting for only 0.065% of all ankle injuries and 0.46% of all ankle dislocations. The mechanism of the injury generally consists of high-energy trauma which is associated with a combination of plantar flexion and inversion or eversion of the foot. We present a case of a 22-year-old male patient who sustained a closed pure ankle dislocation after a fall from a small height. He was treated conservatively with closed reduction and circumferential cast immobilization for six weeks, followed by a functional rehabilitation program. The patient presented to the emergency department with an acutely painful and deformed right ankle after falling from a height of 1 m (stairs). Radiographs showed a posteromedial ankle dislocation without fracture. Urgent closed reduction of the dislocation was performed and a posterior below-knee back slab was applied to immobilize the ankle. Dorsalis pedis and posterior tibial arteries were intact. Check X-rays confirmed proper reduction of the ankle joint. Post reduction computed tomography (CT) scan did not show any associated fractures. Magnetic resonance imaging (MRI) revealed a multiligamentous ankle injury and a small osteochondral lesion of the anteromedial talar dome. The back slab was changed to a below-knee circular cast two weeks later, as soon as the soft tissue swelling subsided. The cast was removed at the six-week follow-up and physiotherapy was initiated in order to gain functional rehabilitation and improve the range of motion. At the final follow-up (12 months), the ankle range of motion (ROM) was the same as the pre-injury status and the patient was able to return to his work. Pure ankle dislocation is a rare injury. A satisfactory outcome can be expected, provided that the appropriate conservative treatment followed by a strict rehabilitation protocol is applied.
A Series of Pure Ankle Dislocations without Associated Fracture: Our Clinical Experience

Abstract:Abstract Background: Pure ankle dislocation without associated fracture is extremely rare, thus the literature almost limited to case reports and small case series. The standardized treatment protocol is in controversy and the studies of mechanism and outcome of the injury are still deficient. We report a series of eight cases of pure tibiotalar dislocations managed with emergency reduction and other heterogenic procedures, hoping to add some material to the published data on this topic and present our clinical experience.Methods: We retrospectively reviewed the eight cases of isolated ankle dislocations without associated fracture that were treated in our department from 2015 to 2019. Results: The eight cases were all posteromedial dislocations with six open and two closed. Emergency reduction was performed for all patients followed by average 6 weeks’ immobilization with external fixators in five and short leg cast in three. The mean follow-up period was 33 (range, 12 to 61) months. None of the eight patients showed obvious instability though only one patient underwent ligaments repair. The average range of motion (ROM) loss of the ankles was 10 degrees for plantarflexion and 3 degrees for dorsiflexion. Two patients complained of moderate stiffness and three complained of persistent mild pain in whom degenerative change was found. Neurovascular sequela was presented in one patient with numbness. Only one patient developed superficial infection. The average AOFAS score was 90 (range, 78 to 100) points at the final follow-up with five ankles rated as excellent and three rated as good. All of the eight patients returned to their prior daily life and the two closed patients with sports injuries resumed sports activity.Conclusion: Pure ankle dislocation is a rare ligamentous ankle injury with complicated mechanism. Most of the injuries treated with emergency reduction and thorough debridement followed by a short period of immobilization and functional rehabilitation have good clinical outcomes. Ligaments repair should only be considered in cases of chronic ankle instability after failed conservative treatment.
Posteromedial Dislocation of the Ankle without Fracture : A Case Report

Abstract:Ankle dislocation without fracture is an extremely rare injury. Open dislocations were more common in the previous reports. The authors report a case of closed posteromedial dislocation of the ankle in a 24year-old basketball player. Closed reduction was performed. The ankle was initially immobilized with the short leg cast before using the functional brace. Details of the rehabilitation program were described. Follow up examination at one-year demonstrated good clinical and functional results confirmed with the inversion stress radiographs. The patient can participate in sports activities at the same level as pre-injury.

A Rare Case of Transepiphyseal Distal Femur Fracture Dislocation With Delayed Recovery of a Common Peroneal Nerve Injury

Abstract:The aim of this study is to bring attention to a unique case and our approach to treatment in this context. We describe a case of an 11-year-old male who presented to us with an injury to his left knee following trauma with pain, swelling, shortening and deformity for one day. An X-ray revealed a transepiphyseal fracture dislocation of the left distal femur (Salter-Harris type 1 injury) and neurovascular examination was conclusive of foot drop which pointed towards injury to common peroneal nerve (CPN). The patient was taken up for closed reduction with percutaneous pinning under mobile C-arm guidance. The fracture was reduced and fixed with two cross K-wires and immobilized with the above knee anterior-posterior slab for six weeks. The wires were removed after six weeks but there was no improvement in the dorsiflexion of the left ankle. An electromyography (EMG) and nerve conduction velocity (NCV) study test was performed after 12 weeks which showed decreased amplitude and prolonged latency in the left CPN with early denervation of the muscles supplied by the left CPN. Fifteen weeks of follow-up showed complete recovery in the dorsiflexion of the left ankle with a slight lag in the extension of the left great toe making this an unusually delayed recovery of CPN palsy following a distal femur transepiphyseal fracture.

Abstract:Objective:To explore the epidemiological characteristics, clinical characteristics, treatment strategies and clinical results of non-dislocated hyperextension tibial plateau fracture.Methods:A total of 25 cases of non-dislocated hyperextension tibial plateau fracture patients were collected, including 12 males and 13 females, aged 27-79 years with an average age of 51±15 years. Causes of injury: 18 cases of traffic accident, 2 cases of heavy injury, 5 cases of falling injury. Schatzker classification of tibial plateau fracture: 4 cases of type II, 5 cases of type IV, 13 cases of type V, 3 cases of type VI. Three columns theoretical classification: 4 cases of simple lateral column, 5 cases of simple medial column, 7 cases of medial column+lateral columns, 9 cases of three columns. Preoperative tibial plateau posterior slope angle was -10-0 degrees, average-5.2 degrees. Preoperative MRI showed 5 cases of medial collateral ligament (MCL) injury, 3 cases of posterolateral complex (PLC) complex injury, 2 cases of PLC+ posterior cruciate ligament (PCL) injury and 10 cases of menisci injury. The change of tibial plateau posterior slope angle was more than 10 degrees in patients with ligament injury, 5 patients had a tibial plateau posterior slope angle change more than 15 degrees, 5 patients had a tibial plateau posterior slope angle change more than 10 degrees, and the patients with a tibial plateau posterior slope angle change less than 10 degrees had no ligament injury; 6 patients with simple lateral column or medial column fracture had a ligament injury, 2 patients with medial column+lateral column fracture had a ligament injury, and 2 patients with three column fracture had a ligament injury.Results:All patients were followed up for 12-24 months, with an average of 16.4 months. The operation time was 124±33 min (65-180 min), and the bleeding volume was 106±48 ml (20-200 ml). All patients were treated by open reduction and internal fixation. According to the characteristics of fracture, targeted reduction of articular surface and restoration of lower limb force line were carried out. The plate was placed on the anterior part of tibial plateau to fix the fracture fragment. Evaluation of postoperative fracture reduction: 20 cases were anatomic reduction, 5 cases were good reduction (between 2-5 mm articular surface collapse), and the excellent rate of fracture reduction was 100%. Internal fixation: 4 cases were treated with simple lateral plate, 2 cases with simple medial plate, 15 cases with medial+ lateral plate, 1 case with medial plate+ lateral anchor suture, 1 case with medial plate+ posterior screw, 1 case with lateral plate+ fibular screw, 1 case with medial plate + lateral plate+ lateral anchor suture. The meniscus was repaired in 8 patients, including 5 medial and 3 laterals. The fracture healing time was 3-6 months (mean 3.3 months). The postoperative knee Rasmussen score was 24.9±3.5 (18-29), and the postoperative knee joint mobility was 118°±9° (90°-130°). Superficial infection occurred in 2 patients.Conclusion:The main imaging characteristic of "non-dislocated hyperextension tibial plateau fracture" is the change of tibial plateau posterior slope angle. The injury of single anteromedial column/anterolateral column fracture is easy to combine with the "diagonal" injury, and when the tibial plateau posterior slope angle changes more than 10 degrees, it is easy to be associated with peripheral ligament injury. By using open reduction and internal fixation, restoring the joint articular surface and lower limb force line, repairing the soft tissue structure and ligament, and reconstructing the stability of knee joint, we can achieve satisfactory results.
