Abstract:Ankle dislocations, particularly those that are old and neglected, pose significant challenges in orthopedic management due to the development of arthritic changes and surgical difficulties of reduction. The optimal treatment often involves stabilization and realignment to ensure proper healing. The closed reduction is rarely achieved in old neglected dislocations. Open reduction with internal or external fixation is the treatment for old neglected dislocations. Ankle and subtalar arthrodesis ensure painless plantigrade foot. The case report describes a case of an 8-month-old unreduced ankle dislocation with equinus deformity managed with ankle and subtalar arthrodesis using tibiotalocalcaneal nail. The equinus deformity was corrected and painless stable ankle joint was achieved.
Properties of neurons from dissociated fetal rat brain in serum-free culture

Abstract:Because of the unknown constituents and varying composition of serum, its presence in media used in cell culture unavoidably compromises attempts to study cellular mechanisms of growth and differentiation. To overcome this, we have devised a serum-free, chemically defined medium which maintains primary cultures of fetal rat brain cells for more than 6 weeks. This medium allows expression of characteristic properties of neurons and prevents overgrowth of non-neuronal elements without use of antimitotic agents. Cells prepared and plated without exposure to serum attach in less than 20 min to poly-D-lysine substratum and begin to extend processes within 1 hr. After 2 days in culture, process-bearing cells can be divided into those with characteristic neuronal morphology, including long processes which generally branch at a distance from the perikaryon, and those having the appearance of glial cells with many short, thin processes which branch frequently near the cell body. The remaining non-neuronal cells are large and flat with few or no processes. The presence of neurons and astroglia was demonstrated by immunofluorescence detection of bound tetanus toxin as a neuron- specific surface marker, and glial fibrillary acidic protein as an astroglial marker. By the 3rd day in culture, many cells of neuronal morphology were able to generate action potentials in response to electrical stimulation. The ionic composition of the inward current changes from Ca2+ to predominantly Na+ by about 10 days in culture. The presence of synaptic vesicles and myelin was demonstrated by electron microscopy. The ability of dissociated cells from mammalian brain to grow in defined medium without serum and acquire selected properties of mature cells in vivo demonstrates the potential of this culture system for neurobiological studies at the cellular level.
Medial talonavicular-calcaneal fracture-dislocation in an adolescent: A challenging traumatic association (case report)

Abstract:Introduction: Talus fractures in children are rare, with a prevalence of 0.008 % of all pediatric fractures. Subtalar dislocation accounts for 1 % of dislocations in trauma cases, making this combination extremely rare. Case presentation: 14-year-old patient suffered an isolated, uncomplicated closed injury to the right ankle from an accident. X-Ray showed an medial subtalar dislocation and a Hawkins stage III fracture of the talar neck. One hour post-trauma, reduction was performed under general anesthesia using Bootjack Technique, and the ankle was immobilized with a plaster cast. The reduction was confirmed by CT-scan. Given the satisfactory anatomical reduction and maintained articular congruence, orthopedic treatment approach was indicated. After 06 weeks of immobilization, physiotherapy sessions were initiated. The patient did not experience any functional impairment and resumed sports activities 03 months post-trauma. Discussion: According to the literature, closed subtalar fracture-dislocation, has not been previously described in the pediatric population. Diagnosing dislocation is straightforward, with treatment involving reduction through a boot jack maneuver. If a fracture is present, irreducibility should raise suspicion of interposition, requiring open reduction. While surgical treatment is often used, it increases the risk of ischemia. Orthopedic treatment remains a reliable and effective option, particularly in children, as it allows quicker management and avoids iatrogenic complications. Conclusion: Subtalar fracture-dislocation in children is rare, and its management is not standardized. However, it is crucial to consider and identify this type of injury due to its rarity and significant risk of complications. Appropriate management is essential to accelerate treatment and avoid the risk of talar necrosis.
Primary Open Reduction and Wire-Pin Fixation for Total Talar Dislocation: a Report of Twelve Cases

Abstract:Objective To evaluate primary open reduction and wire-pin fixation for total dislocation of talus.Methods A retrospective study was conducted of the 12 patients who had been admitted into our hospital from October 2009 to December 2013 for total talar dislocation without fracture of the talar neck or body and had been available for complete follow-up.They were 10 males and 2 females.Their range of age was from 17 to 56 years (average,34.2 years).The total dislocation was open in 4 cases and close in 8.All of them underwent primary open reduction and wire-pin fixation.The functions of ankle joint and hindfoot were evaluated according to the American Orthopedic Foot and Ankle Society (AOFAS) ankle-hindfoot scale.Results The follow-up time ranged from 22 to 72 months (average,35.6 months).Postoperatively,one case suffered skin necrosis and another superficial infection,both of whom responded to skin flap transferring after debridement.Traumatic arthritis of the talotibial or subtalar joint was found more or less in 7 cases,one of whom complained of severe pain but refused the subtalar arthrodesis advised.Their AOFAS scores at the final follow-up ranged from 42 to 96 points (average,72.2 points).Five cases developed avascular necrosis of the talus,but no talar collapse happened.Conclusions Primary open reduction and wire-pin fixation is effective for treatment of close total talar dislocation and open total dislocation of Gustilo type Ⅲ A or below.Although avascular necrosis and traumatic arthritis are common postoperative complications,satisfactory functions can be achieved if there is no collapse,severe pain or infection.

Abstract:Introduction: The subtalar dislocations are rare, this is described as a dislocation of the talo-calcaneal and talo-navicular joints but the tibio-talar joint is intact. The diagnosis is easy and done with x-ray: antéro-posterior and lateral view. A emergency reduction is necessary In a few cases, you need a surgical approach if the dislocation is irreducible or no stable after reduction. Case Report: A man aged of 19 years, felt in climbing, with a forced varus. In emergency room, the clinical exam showed a deformation and the x-ray: a talo-naviculo-calcaneal dislocation. A reduction is done in the operative room, the testing a showed a stable ankle so we stabilized with a posterior splint. During the post-operative consultation: the control x-ray showed a good reduction and the CT: a minimal osteo-cartilagny tearing. The sport is authorized after three month. Conclusion: The talo-naviculo-calcaneal dislocation is rare following a plantar flexion and inversion. A diagnosis is quickly and easily done and the reduction is done in the operative room under general anesthesia. A cast is applied during some weeks before starting the physiotherapy. A surgical treatment is done in a few rare: irreducible – instable – open dislocation.

Abstract:In this paper, a simple cavity model is presented for the theoretical prediction of the radiation patterns of unidirectional dielectric radiator (UDR). The antenna structure is first modeled as a non-radiative dielectric waveguide (NRD) resonator, the electromagnetic fields at the radiating aperture of the antenna structure are then determined for the calculation of the radiation fields and patterns of the UDR antenna. A good agreement between the calculations and measurements shows the validity of the analysis model.

Abstract:Background: The talus bone is the second-largest bone in the tarsal region. Talar fractures are rare injuries that occur due to highenergy traumas. Open reduction and internal fixation (ORIF) is the treatment of choice in the talar body fracture-dislocation. Avascular necrosis (AVN), osteoarthritis (OA), malunion, non-union, skin infections, and ankylosis of the subtalar joint are complications that can happen following the talar body fracture. Case Report: We reported a rare open fracture-dislocation of the talar body concomitant with the anterior talofibular ligament (ATFL) rupture in a 24-year-old motorcyclist man. ORIF with two 4.5 mm cancellous screws and ATFL repair were performed. After 6 months of follow-up, the radiographic bone union was achieved, and the patient had no complaints of pain, disability, and discomfort. Conclusion: According to the blood supply of the talus bone, appropriate management and follow-up should be considered to prevent severe consequences. Due to the lack of literature on this lesion, in this report, we tried to provide more comprehensive information regarding the diagnosis, treatment, and follow-up of this group of patients. To the best of our knowledge, our case will probably be the second case of the talar body fracture concomitant with rupture of the ATFL.
Subtalar dislocation associated with fracture of the posterior process of the talus: a case report and review of the literature

Abstract:Objective Subtalar dislocation associated with fracture of the posterior process of talus is very rare. The diagnose and treatment about this rare case was reported and the related literatures were reviewed to summarize the anatomy of the posterior process, mechanism and treatment of this pattern injury. Methods The literatures about the Subtalar dislocation associated with fracture of posterior process of talus before October 2017 were searched in PubMed, CNKI, Weipu and Wanfang database by using "subtalar dislocation" and "posterior process fracture of talus" as key words. Six cases reported in English and 1 case reported in Chinese were detected. The clinical and imaging findings, mechanism of this pattern injury, treatment and clinical prognosis of 7 cases reported in literature and 1 case in this paper were observed and analysed. Results A 25-year-old male suffered an traffic accident was presented with pain, swelling, malformation and activity limitation at the right ankle.The X-ray revealed subtalar dislocation and an unconfirmed fracture. The closed reduction and plaster fixation were performed immediately to correct the subtalar dislocation but the result was not satisfying. X-ray and CT scan revealed subtalar medial dislocation associated with navicular fracture and posterior process fracture of talar. The patient underwent open reduction and internal fixation 27 days later after the accident and got a good result in 6 months after operation. Eight cases of subtalar dislocation associated with fracture of the posterior process of talus are involved, including 1 case in this paper and 7 cases reported, 7 males and 1 female, age ranges from 17 to 52 years old, all the patients were presented with pain, swelling, malformation and activity limitation at the ankle. Seven cases underwent open reduction and internal fixation and 1 case was treated by minimally invasive technique. The X-ray and CT scan revealed a good reduction after open reduction and internal fixation. There was no pain at the hindfoot and the patient regained a good activity 3 months after the surgery. Conclusions When at the time of the impact the foot is in inversion and plantarflexion medial subtalar dislocations occur. The avulsion force from talocalcaneal ligament lead to fracture of posterior process of talus. Subtalar dislocation associated with nondisplaced fracture can be treated conservatively, open reduction and internal fixation is recommended to the case that with fracture displaced more than 2 mm. Abstract:Objective To evaluate the surgical therapy on dislocated fracture of talus. Methods Retrospective analysis was mode in 21 patients with dislocated fracture of talus collected from Jan. 2004 to Jan.2010, which were treated with open reduction, cannulated screw fixation, and kept neutral position plaster fixation with no weight loading, to do functional exercise depending on the Ⅹ film demonstrations. Results All the patients were followed up from 6 months to 3.8 years post-operation, and according to the evaluation standard by American Foot-Ankle Surgery Society, good rate was 61.91%. Conclusion Treating dislocated fracture of talus with emergency operation, anatomical reduction, valid internal fixation and no weight loading plaster fixation post-operation, shows good effect with low rate of complication.

Abstract:Objective To evaluate open reduction and internal fixation (ORIF) combined with Ilizarov joint distraction in the treatment of comminuted talar body fractures.Methods Between July 2010 and May 2016,16 patients with comminuted talar body fracture underwent ORIF followed by ankle joint distraction by Ilizarov external fixator at our department.They were 9 males and 7 females,aged from 17 to 56 years (average,35.6 years).Radiological assessments were conducted by plain radiographs and computed tomography (CT) scans pre-and post-operatively.Clinical outcomes were evaluated according to visual analogue scale (VAS),American Orthopedic Foot and Ankle Society (AOFAS) hindfoot score,the short form (SF)-36 health survey scores and patients' satisfaction.Results Two patients were lost to follow-up.The remaining 14 patients were followed up for an average duration of 33.3 months (from 13 to 52 months).All the fractures united.Superficial wound infection occurred in one case.Eight patients developed post-traumatic osteoarthris;4 patients presented with avascular necrosis.At final follow-ups,the AOFAS scores ranged from 30 to 97 points (mean,69.8 points),the VAS scores from 0 to 6 points (mean,2.5 points),the physical component summary in SF-36 from 18.0 to 55 points (mean,41.7 points),and the mental component summary from 40 to 70 points (mean,54.1 points).Two patients reported strong satisfaction,5 satisfaction,4 moderate satisfaction,and 3 dissatisfaction.Conclusion For comminuted talar body fractures,ORIF combined with Ilizarov joint distraction is a new and effective treatment which can not only provide adequate fixation strength but also reduce the incidence of post-traumatic osteoarthris or avascular necrosis.

Abstract:Traumatic fractures of the ankle can occur with concomitant tibiotalar dislocations, necessitating complex treatment. These injuries have higher rates of loose bodies, open injuries, postoperative complications, and worse patient reported outcomes compared to ankle fractures without dislocation. Patients with neglected or delayed presentations are associated with even higher rates of postoperative complications and worse outcomes compared to acute injuries. The chronicity of the injury leads to soft tissue contractures and malunited fractures, obligating a care plan which involves gradual reduction with a multiplanar external fixator with or without internal fixation at a later date. We discuss a 60-year-old homeless man who presented four weeks after an open trimalleolar fracture-dislocation and was definitely treated with an acute one-stage procedure. Anatomic reduction and stable fixation was achieved through a lateral malleolus osteotomy, soft tissue releases, TAL, and a temporary intraoperative external fixator. This technique was advantageous in this instance of anticipated patient noncompliance. We advocate for the judicious use of the described technique in similar challenging situations.

Abstract:Background: The management of neglected traumatic knee dislocations is challenging and is commonly associated with suboptimal outcomes. In this article we present two cases of neglected knee dislocations in two young trauma victims who presented late due to restrictions during the COVID-19 pandemic. Methods: Two young patients presented at our center with neglected knee fracture-dislocations after eight weeks and six months of trauma. Comprehensive evaluation was performed using radiographs, computed tomography scans, and magnetic resonance imaging. Surgical intervention included open reduction and internal fixation of the mal-aligned intra-articular fracture fragments. Additionally, articular congruency was restored and repair or reconstruction of the posterior cruciate ligament, anterior cruciate ligament, and meniscus was conducted using suitable tendinous autografts and implants. Following surgery, the knees were immobilized for six weeks, followed by aggressive physical therapy. Results: After three months of surgical intervention, fracture union and joint stability were achieved. At the >36-month follow-up appointments, both patients were pain-free at rest with a stable knee joint, achieving ≥90 degrees of knee flexion and without extensor deficits. Concurrent with radiographic evidence of osteoarthritic changes in the knee joint, there was mild pain (VAS 2) after prolonged knee movement activities and walking for long distances (>1 km). Conclusion: Open reduction and internal fixation, along with simultaneous menisco-ligamentous reconstructions in neglected fracture-dislocations of the knee result in satisfactory clinical outcomes. This approach proves to be an effective joint preservation procedure in young patients, even in delayed and neglected conditions.

In this report, the primary computer vision subarea-segmentation-is investigated. Many of the latest publications on the subject of segmentation are detailed in a survey format. Special attention is given to a few specialized techniques for segmenting digital images. Powerful segmentation techniques are available; however. each technique is ad hoc. The creation of hybrid techniques seems to be a promising future research area with respect to current Navy digital mapping applications.
Total Talus Allograft Transplantation With Subtalar Arthrodesis for Missing Talus: A Report of a Rare Case

Abstract:Post-traumatic missing talus is a rare and severe injury that often results in poor functional outcomes, with no consensus on the optimal treatment approach as strategies vary based on injury severity. We present the case of a 44-year-old male who sustained a missing talus following a high-energy motorcycle accident. After initial wound management and application of an external fixator, the patient underwent size-matched, fresh-frozen talus allograft transplantation combined with subtalar fusion. Postoperative radiography and CT confirmed successful transplantation with solid subtalar fusion, although progressive osteonecrosis was noted in the medial shoulder region of the talus. At the two-year follow-up, the patient exhibited limited ankle and hindfoot motion but was able to bear weight and walk without assistance, reporting no pain in his feet and achieving a final American Orthopaedic Foot & Ankle Society hindfoot score of 72. This case underscores the potential of total talar allograft transplantation with subtalar arthrodesis in treating severe talar bone loss or missing talus, although long-term follow-up is necessary to assess the clinical implications of medial talar collapse and the possible need for revision surgery.

Abstract:IntroductionTotal subtalar dislocation without fracture of the ankle is a rare clinical entity; it is usually due to a traumatic high-energy mechanism. Standard treatment is successful closed reduction under general anesthesia followed by non-weight bearing and ankle immobilization with a below-knee cast for 6 weeks.Case presentationWe present the case of a 30-year-old Moroccan woman who was involved in a road traffic accident. She subsequently received a radiological assessment that objectified a total subtalar dislocation without fracture of her ankle. She was immediately admitted to the operating theater where an immediate reduction was performed under sedation, and immobilization in a plaster boot was adopted for 8 weeks. The management of this traumatic lesion is discussed in the light of the literature.ConclusionsMedial subtalar dislocation is a rare dislocation and is not commonly seen as a sports injury because it requires transfer of a large amount of kinetic energy. The weaker talocalcaneal and talonavicular ligaments often bear the brunt of the energy and are more commonly disrupted, compared to the relatively stronger calcaneonavicular ligament. Urgent reduction is important, and closed reduction under general anesthesia is usually successful, often facilitated by keeping the knee in flexion to relax the gastrocnemius muscle. Long-term sequelae include talar avascular necrosis and osteochondral fracture, as well as chronic instability and pain.
Isolated Closed Medial Subtalar Dislocation Without Ankle Fracture: A Rare Case Report

Abstract:Subtalar dislocation is an unusual clinical case with very little mention in literature constituting nearly 1% of all traumatic dislocations. Here a 40 year old male who sustained fall over inverted foot was diagnosed as closed isolated medial subtalar dislocation with no associated fractures. This is an even more rare entity. Immediate closed reduction was done and immobilised for 4 weeks with below knee cast. This case report describes the mechanism of injury, ligaments involved, manoeuvre of closed reduction. This case report highlights the importance of prompt and urgent reduction and short period of immobilisation to avoid complications and better prognosis. Reconstruction of Calcaneal Fracture Malunion With Osteotomy and Subtalar Joint Salvage

Abstract:Category: Hindfoot Introduction/Purpose: The goal of this study was to discuss the outcomes of treating calcaneal fracture malunion by restoring the subtalar joint with a reconstructive osteotomy. Methods: From May 2005 to November 2016, 85 patients (88 feet) with calcaneal malunions after a displaced intraarticular calcaneal fracture were treated by osteotomy and autogenous bone graft. The subtalar joint was preserved. The mean time from initial injury to reconstructive operation was 8.9 months (95% confidence interval, 4.5-11.6 months). The displaced posterior facet was restored through a reconstructive osteotomy, whereas the bone defect in the calcaneus after reduction was filled with the lateral exostosis that had been removed, iliac bone graft was used if necessary. All patients were evaluated clinically and radiographically at a minimum of 24 months. Sixty two patients (63 feet) were followed for a mean of 45.4 months (29.0-52.3 months). Results: According to American Orthopaedic Foot & Ankle Society (AOFAS) ankle and hindfoot score, the average score was 81.6 points (95% confidence interval, 78.5-90.4 points), which was significantly higher than the preoperative assessment. Radiographs showed that Böhler’s angle, Gissane’s angle, talus declination angle, and width and height of calcaneus were improved to a great extent. Eight patients had wound edge necrosis, and 4 had superficial infection. Two patient required a subtalar fusion for subtalar arthritis at 2 years after surgery. Conclusion: Restoring the subtalar joint with a reconstructive osteotomy and autogenous bone graft was an effective treatment method for selected calcaneal fracture malunions. It reconstructed calcaneal morphology and preserved the subtalar joint.
Intra-articular Transcalcaneal Fracture-Dislocation Associated With Ipsilateral Transtalar Fracture: A Report of a Rare Combination

Abstract:The association of ipsilateral talar and calcaneal fractures is an uncommon combination and is typically the result of a high-energy trauma. It is often associated with comminution, marked fracture displacement, and soft-tissue compromise. Obtaining satisfactory clinical and radiographic results is very challenging. Residual deformities, multiple procedures, and limitations are usually the norm. Therefore, studies have suggested that primary arthrodesis may represent the best option of surgical treatment. In this study, we report a case of a 30-year-old male patient with a rare combination of a highly comminuted transcalcaneal fracture-dislocation associated with a talar neck fracture successfully treated with open reduction and internal fixation (ORIF) with an 18-month follow-up. This case demonstrates that even when there are associated fractures of the talus and calcaneus with severe bone loss, ORIF may provide satisfactory outcomes in the short-term postoperative period.Levels of Evidence: Level V: Case report.
Neglected paediatric talus neck fracture: A successful clinical outcome without post-traumatic complications

Abstract:Paediatric talus fracture is an extremely rare type of fracture and occurs due to the structural properties of the elastic cartilage that is resistant to high forces. It has a much lower prevalence than in adults. The axial loading of the talus to the anterior tibia while the ankle is in dorsiflexion is the most common mechanism of talus fractures. Although talus neck fracture is the most common fracture, talus corpus fractures are the second most common. Here we present the case of an adolescent male patient who underwent a successful non-surgical treatment and followup of a neglected talus neck fracture.
