Abstract:Objective To investigate the zirconium niobium alloy femoral head in young and middle-aged feasibility and the recent curative effect of total hip replacement.Methods Since May 2008 ～ Jan 2014 22 cases (22 hip) youth with femoral head avascular necrosis were treated with zirconium niobium alloy in our hospital.The Harris hip score and WOMAC osteoarthritis index scale to evaluate the hip joint function.According to the X ray film Gruen partition around the femoral prosthesis and the acetabulum prosthesis around each Deiee bone for recording and analysis of partition,judge whetherthereisbone dissolve and prosthesis loosening.Results All patients were followedup for38.3±5.3 months (range 25 ～ 63).The Harris score improved from 41.5±9.5 (range,11 ～ 67) preoperatively to 93.8±5.7 (83-99) at the time of final follow-up.There were significant differences in Harris score (t＝23.478,P＜0.01).The WOMAC index improved from 75.3±6.8 (range,58 ～ 91) preoperatively to 9.5±5.7 (range,1 ～ 12) at the time of final follow-up.There were significant differences in WOMAC Index (t＝19.163,P＜0.01).Infection,deep vein thrombosis,thigh pain,prosthesis dislocation,prosthesis crushing,sound,dissolution of bone and prosthesis loosening did not appear during the follow-up period.Conclusion The zirconium niobium alloy femoral head in young and middle-aged total hip replacement,short-term curative effect is satisfactory,can reduce the loose prosthesis and the surrounding bone resorption,but high risk for dislocation and difficult resetting patients should be careful to application.
Ageing, Shocks and Wear Mechanisms in ZTA and the Long-Term Performance of Hip Joint Materials

Abstract:The surface morphologies and microstructures of Zirconia Toughened Alumina (ZTA) femoral heads were analyzed following in vitro tests aiming to simulate in vivo degradation. Three phenomena potentially leading to degradation were investigated: shocks, friction and hydrothermal ageing. Shocks due to micro-separation created the main damage with the formation of wear stripes on the femoral head surfaces. Atomic Force Microscopy (AFM) images suggested the release of wear debris of various shapes and sizes through inter- and intra-granular cracks; some debris may have a size lower than 100 nm. A decrease in hardness and Young's modulus was measured within the wear stripes by nanoindentation technique and was attributed to the presence of surface and sub-surface micro-cracks. Such micro-cracks mechanically triggered the zirconia phase transformation in those worn areas, which in return presumably reduced further crack propagation. In comparison with shocks, friction caused little wear degradation as observed from AFM images by scarce pullout of grains. The long-term resistance of the ZTA composite material against hydrothermal ageing is confirmed by the present observations.

[Study on Low Temperature Degradation of Dental Zirconia Ceramic].

Abstract:OBJECTIVE:To evaluate the properties of low temperature degradation (LTD) of dental Y-TZP by experiments.METHODS:According to ISO 13356-2008 standards, 72 blanks of Y-TZP were sintered at different temperature (1 400 °C,1 450 °Cor 1 530 °C) and in different heat preserving time(2 h or 5 h), and then the specimens were grounded and polished. The blanks were divided into 12 groups according to the different sintering temperature, heat preserving time and aging time. The surface of specimens was observed by SEM to evaluate the grain size of specimens; Each group of specimens was exposed to 140 °C hydrothermal aging environment for 0 h, 5 h or 24 h. After aging, XRD analysis was undertaken to determine the amount of phase transformation from tetragonal to monoclinic for each groups. At last, the biaxial flexural strengths of all specimens were measured and calculated.RESULTS:The SEM showed that the grain size of each group of Y-TZP increased with the increase of sintering temperature and heat preserving time, however, all of them were below 1 μm. The XRD analysis showed the contents of monoclinic of each group varied with different sintering temperatures, heat preserving times and aging time. The flexural strength of 24 h aging groups declined significantly; but that of 5 h aging group showed increase or decrease according to the different sintering temperatures and heat preserving times.CONCLUSION:Different sintering procedures to some extent may affect the Y-TZP aging properties.

Abstract:Since the recent failure events of two particular series of zirconia femoral heads for total hip replacement prosthesis, a large decrease in the use of zirconia ceramics for orthopaedic implants has been observed. In spite of the biomedical success of this material during the last ten years, this was required for safety reasons, until the cause of the failures is known. It has been shown that these failures were related to the low temperature hydrothermal degradation (also known as ageing). Thus it is crucial to better understand the ageing behaviour, in order to be able to assess its importance and then control it if required. In this paper, various techniques relevant to assess the hydrothermal degradation sensitivity of biomedical grade yttria stabilized zirconia are discussed and compared. The expected outputs of conventional methods, i.e. X-Ray diffraction and scanning electron microscopy are examined. More recent methods like optical interferometry and atomic force microscopy are presented, with their respective benefits and drawbacks. An up to date comparison of these different techniques is provided, and their use for ensuring the long term reliability of a particular batch of zirconia in terms of ageing degradation is demonstrated.
Does artificial aging correctly predict the long-term in-vivo degradation behavior in zirconia hip prostheses?

Abstract:Background: Accelerated hydrothermal aging has long been one of the most widely accepted quality control tests for simulating low-temperature degradation (LTD) in zirconia-containing implants used in total hip arthroplasty (THA). However, it is still unclear how much consistency there is between the experimental prediction from the internationally-standardized tests and the actual measurements from surgically-removed implants after a long period of implantation. This question is fundamentally related to a lack of understanding of mechanical/tribological contribution to the in-vivo LTD kinetics. Objective: The main purpose of this study is to validate the clinical relevance of standardized accelerated aging by comparing artificially-aged and in-vivo used prostheses, and to clarify the long-term effects of in-vivo mechanics/tribology on the LTD progression upon service in the body environment. Methods: Surface magnitudes of phase transformation and residual stress in zirconia femoral head retrievals (13.1-18.4 yrs) were evaluated by using confocal Raman microspectroscopy. Results: The long-term aging behavior in unworn head surface was in agreement with the experimental prediction estimated as 1 h aging at 134 °C = 4 years in-vivo. However, the current aging protocols based on ASTM and ISO criteria were not accurately predictive for the worn surfaces, and the tribologically-induced phase transformation and tensile stress were up to 6.5-times and 3.3-times higher than the environmentally-induced ones. Conclusion: Our study suggests that wear/scratching, frictional heating, tribochemical reactions, and metal transfer may become far more intense triggers to phase transformation than the mere exposure to body fluid.

Abstract:Due to growing demand for metal-free dental restorations, dental ceramics, especially dental zirconia, represent an increasing share of the dental implants market. They may offer mechanical performances of the same range as titanium ones. However, their use is still restricted by a lack of confidence in their durability and, in particular, in their ability to resist hydrothermal ageing. In the present study, the ageing kinetics of commercial zirconia dental implants are characterized by X-ray diffraction after accelerated ageing in an autoclave at different temperatures, enabling their extrapolation to body temperature. Measurements of the fracture loads show no effect of hydrothermal ageing even after ageing treatments simulated a 90-year implantation.

Intraoral low-temperature degradation of monolithic zirconia dental prostheses: 5-year results of a prospective clinical study with ex vivo monitoring

Abstract:Objectives: To investigate the 5-year intraoral evolution and kinetics of low-temperature degradation (LTD) of second-generation monolithic prostheses made of 3% molar yttrium-doped tetragonal zirconia polycrystal (3Y-TZP) and the influence of masticatory mechanical stresses and glaze layer on this evolution. Methods: A total of 101 posterior tooth elements were included in this prospective clinical study, which comprised ex vivo LTD monitoring (at baseline, 6 months, 1 year, 2 years, 3 years, and 5 years) using Raman spectroscopy (n = 2640 monoclinic phase measurement points per evaluation time) and scanning electron microscopy (SEM). Four types of areas (1-2 mm2 surface, six on molars, and four on premolars) were analysed on each element surface: occlusal, axial, glazed, or unglazed. Raman mapping, high-resolution SEM, and focused ion beam-SEM were performed on selected samples. Results: The dental prostheses developed a tetragonal-to-monoclinic transformation at the extreme surface of the material after six months in a buccal environment, and this process increased significantly over time. Over the five years of monitoring, the transformation developed nonuniformly with the presence of localised clusters of monoclinic grains. Tribological stresses generate grain pull-out from these clusters, which may raise questions regarding the release of 3Y-TZP nanoparticles into the body. The prosthesis fracture rate was 4.5% after 5 years. Significance: LTD developed in vivo on the surfaces of 3Y-TZP dental prostheses and progressed slowly but significantly over time, up to 5 years investigation. However, the effects of aging on the failure rate recorded and of zirconia nanoparticles released into the body require further investigation.

Abstract:OBJECTIVE: To investigate the effect of aging on phase transformation and mechanical properties of yttria-tetragonal zirconia polycrystal (Y-TZP).MATERIALS AND METHODS: Fully-sintered Y-TZP slabs, IPS E-max ZirCAD (ZC - Ivoclar) and Z-5 ceramic (Z5 - C5 Medical Werks), were artificially aged in autoclave for: 0, 30, 60 or 90min. Flexural strength (FS), crystalline changes (X-ray diffraction analysis - XRD) and surface topography were analyzed. 0 and 90min-aged samples were evaluated by nanoindentation to measure hardness and modulus, and results were compared using Wilcoxan Mann Whitney rank sum test (p≤0.05). FS results were compared using two-way ANOVA and Tukey HSD (α=0.05).RESULTS: Material factor had significant effect (p=0.001) on flexural strength (Z5=966.95MPa; ZC=847.82MPa), but aging did not. Nanoindentation showed incidence of typical load/depth curves combined with some exhibiting features compatible with cracking. When typical curves were considered, aging had no effect on the modulus and hardness, but hardness was dependent on material type. A steady increase in the m phase related to aging time was observed for ZC samples. The maximum incidence of m phase was 6.56% for Z5/60min.SIGNIFICANCE: Flexural strength is not affected by surface transformation in dental Y-TZP. Hydrothermal aging has an effect on m content and surface topography of different zirconia brands, but mechanical tests that can precisely characterize surface changes in aged Y-TZP are still missing.

Abstract:Recently, several episodes of fracture of zirconia ceramic femoral heads of total hip prostheses have alarmed the medical and scientific community regarding aging problems in zirconia prostheses. Such fractures cause immediate local tissue reactions, which require urgent medical intervention to prevent further complications. As a result, it has been promoted that yttria-stabilized zirconia (Y-TZP) hip prostheses be substituted by alumina and alumina/Y-TZP ceramics. In the present investigation, we have found an upper limit of Y-TZP concentration in alumina/Y-TZP composites (16 vol.%) to avoid future aging problems. This limit coincides with the percolation threshold measured by infrared (IR) reflectance in a series of alumina/Y-TZP composites.

Abstract:The authors investigated the unique associations between family cohesion, family conflict, and depression for African American and European American adolescents (aged 12-17 years). In addition, they tested the influence of a cognitive variable on these relationships. Statistical analysis yielded some provocative findings. Specifically, low family cohesion was uniquely associated with depression for African American adolescents, whereas high family conflict uniquely predicted depression for European American adolescents. Also, a cognitive variable, high self-discrepancy, mediated the effect for the European American adolescents, but not for African American adolescents when analyses were conducted separately by ethnic group. In follow-up analyses, however, this descriptive finding did not meet conventional criteria for establishing moderated mediation. Implications for future research and for designing interventions and prevention strategies for children with depression are discussed.

Abstract:Changes in crystalline phases resulting from low-temperature ageing of different yttria doped and non-doped zirconia-toughened alumina composites and nanocomposites were investigated under controlled humidity and temperature conditions in autoclave. A classical powder mixing processing route and a new modified colloidal processing route were used to process the composites. Different compositions ranging from 2.5 wt.% zirconia in a matrix of alumina to pure zirconia (3Y-TZP) were studied. It was observed that Al2O3+yttria stabilised ZrO2 composites exhibited significant ageing. However, ageing was much slower than traditionally observed for Y-TZP ceramics, due to the presence of the alumina matrix. Ageing was clearly limited for zirconia content beyond 25 wt.%. On the other side of the spectrum, Al2O3+2.5 wt.% ZrO2 initially presented a monoclinic fraction but did not show any ageing degradation. These composites seem to represent the best choice between slow crack growth and ageing resistance.

Abstract:This paper considers the degradation of alumina and zirconia toughened alumina vs. alumina for hip implants. The materials are as assumed to be load bearing surfaces subjected to shocks in wet conditions. The load is a peak of force; 9 kN was applied over 15 ms at 2 Hz for 800,000 cycles. The volumetric wear and roughness are lower for ZTA than for alumina. The long ZTA ageing did not seem to have a direct influence on the roughness. The ageing increased the wear volumes of ZTA and it was found to have a higher wear resistance compared to alumina.
Hydrothermal aging affects the three-dimensional fit and fatigue lifetime of zirconia abutments

Abstract:Objective: Evaluate the effect of aging using two different methods on the three-dimensional fit of zirconia abutments at the implant-abutment connection and estimate the probability of survival of anterior crowns supported by straight and 17-degree angled abutments. Materials and methods: Two different zirconia abutment designs, straight and 17-degree angled abutments (n = 63/group), were evaluated in the current study. The abutments were randomly allocated into three experimental groups according to laboratory aging condition (134°C, 2.2 bar, 20 h): (i) control, (ii) autoclave aging, and (iii) hydrothermal reactor aging. Crystalline content was determined by X-Ray diffraction (XRD) and Raman spectroscopy, and microstructure was analyzed using field-emission gun scanning electron microscope (FEG-SEM). Implant-abutment volume misfit was determined in the straight abutments by micro-computed tomography using the silicone replica technique. For fatigue testing, abutments were torqued to the implants and connected to standardized maxillary incisor zirconia crowns. The assemblies were subjected to step-stress accelerated life testing (SSALT) in water until fracture or suspension. The use level probability Weibull curves and probability of survival for a mission of 50,000 cycles at 50, 100, 150 and 200 N were calculated and plotted. Fractured samples were analyzed using a stereomicroscope and scanning electron microscope. Results: The crystalline spectra depicted a zirconia system primarily composed of the tetragonal phase. Laboratory aging yielded a 20%- and 37%-increase in the monoclinic content for abutments aged in autoclave and hydrothermal reactor relative to control, respectively. A fully crystalline matrix with a regular grain size was observed in the FEG-SEM for control abutments, with a considerable presence of intergranular defects. While autoclave aging triggered no significant alteration to the microstructure, defect population was reduced after hydrothermal reactor aging. Control abutments presented a significantly higher volume misfit (2.128 ± 0.54 mm3) relative to aged abutments using autoclave (1.244 ± 0.48 mm3) or hydrothermal reactor (1.441 ± 0.41 mm3). The beta (β) values indicated that failures were predominantly controlled by material strength rather than fatigue damage accumulation for all groups, except for straight control abutments. Irrespective of aging, the probability of survival of straight and angled zirconia abutments was up to 95% (95-100%) at 50 and 100 N. A 50N-increase in the load resulted in wider range of survival estimate, with straight autoclave abutments percentage significantly lower probability of survival (77%) than angled hydrothermal reactor abutments (99%). At 200N, angled hydrothermal reactor (97%) or autoclave (82%) aged abutments demonstrated the highest probability of survival, angled control (71%) and straight hydrothermal reactor (69%) abutments intermediate values, and straight autoclave (23%) and control (7%) abutments the lowest estimate. The failure mode predominantly involved abutment and/or abutment screw fracture for both straight and angled abutments. Conclusions: Hydrothermal aging significantly influenced volume misfit, as well as the probability of survival of zirconia abutments at higher loads for both angled and straight abutments.
Critical effect of cubic phase on aging in 3 mol% yttria-stabilized zirconia ceramics for hip replacement prosthesis

Abstract:The isothermal tetragonal-to-monoclinic transformation of 3Y-TZP ceramics sintered at two different temperatures (1450°C and 1550°C) and duration (2 and 5 h) is investigated at 134°C in steam. Particular attention is paid to the presence of a cubic phase and its effect on isothermal aging. Sintering at 1550°C can result in a significant amount of large cubic grains in the specimens, that have a detrimental impact on aging resistance, especially for the first stage of the aging process. Cubic grains appear to be enriched in yttrium, which in turn leads to a depletion of yttrium in the neighboring tetragonal grains. These grains will act as nucleation sites for tetragonal-to-monoclinic transformation. Even for specimens sintered at lower temperature, i.e. 1450°C, the presence of a cubic phase is expected from the phase diagram, leading to a significant effect on aging sensitivity.

Abstract:Zirconia is known for its high strength but lacking translucency. Recently, a new type of high translucent zirconia, 5 mol% yttria partially stabilized zirconia (5Y-PSZ), with a larger fraction of cubic zirconia phase has become commercially available. However, the resistance to aging of these commercially available zirconia materials is not yet fully established. Purpose: The aim of the present study was to analyze the effects of artificial aging on surface roughness, transparency, phase transformation and biaxial flexural strength of two 5Y-PSZ products, DD cubeX2 and Prettau Anterior. Materials and methods: The artificial aging was performed in an autoclave under 2 bars of pressure at 134 °C for 10 hours, which is estimated to correspond to 30-40 years in vivo. Artificial aging for 10 hours had no significant effect on surface roughness, transparency, or phase transformation for either of the tested materials. Results: DD cubeX2 had higher mean flexural strength than Prettau Anterior both before and after artificial aging for 10 hours (p < .05). DD cubeX2 showed, however, a significant reduction in flexural strength after artificial aging (p < .05), whereas Prettau Anterior showed a slight increase in flexural strength after artificial aging but not at a significant level. Conclusion: Within the limitation of the present study, both DD cubeX2 and Prettau Anterior seems to be relatively resistant to aging. However, a wider range of measured flexural strength indicated that Prettau Anterior probably is a less stable material than DD cubeX2, which also means that the flexural strength of DD cubeX2 could be more predictable.
Influence of surface finish and residual stresses on the ageing sensitivity of biomedical grade zirconia

Abstract:We demonstrate in this paper the influence of surface finish on the ageing kinetics of biomedical grade zirconia. The critical influence of polishing has been systematically investigated by optical microscopy, atomic force microscopy and X-ray diffraction. The stress state around polishing scratches gives rise to preferential transformation in the zone of the induced scratches and consequently to accelerated ageing. The influence of residual stresses is analyzed semi-quantitatively by preparing samples with various surface finish, thus with various stress states. Rough polishing produces a compressive surface stress layer beneficial for the ageing resistance, while smooth polishing produces preferential transformation nucleation around scratches. When a thermal treatment of 2 h at 1200 {\deg}C is applied to relax the residual stresses, all the surfaces states exhibit the same sensitivity to ageing. These results demonstrate that roughness alone cannot be used for ensuring a long-term stability. The variation of ageing sensitivity is indeed related indirectly to the surface roughness via the induced surface stress state. The current ISO standards are not able to take these effects into account. Indeed, great variations in ageing kinetics were observed for samples with different surface states, although all well below the ISO requirements.

Abstract:Various protocols are available to preclinically assess the fracture resistance of zirconia oral implants. The objective of the present review was to determine the impact of different treatments (dynamic loading, hydrothermal aging) and implant features (e.g., material, design or manufacturing) on the fracture resistance of zirconia implants. An electronic screening of two databases (MEDLINE/Pubmed, Embase) was performed. Investigations including > 5 screw-shaped implants providing information to calculate the bending moment at the time point of static loading to fracture were considered. Data was extracted and meta-analyses were conducted using multilevel mixed-effects generalized linear models (GLMs). The Šidák method was used to correct for multiple testing. The initial search resulted in 1864 articles, and finally 19 investigations loading 731 zirconia implants to fracture were analyzed. In general, fracture resistance was affected by the implant design (1-piece > 2-piece, p = 0.004), material (alumina-toughened zirconia/ATZ > yttria-stabilized tetragonal zirconia polycrystal/Y-TZP, p = 0.002) and abutment preparation (untouched > modified/grinded, p < 0.001). In case of 2-piece implants, the amount of dynamic loading cycles prior to static loading (p < 0.001) or anatomical crown supply (p < 0.001) negatively affected the outcome. No impact was found for hydrothermal aging. Heterogeneous findings of the present review highlight the importance of thoroughly and individually evaluating the fracture resistance of every zirconia implant system prior to market release.

Abstract:PURPOSE: This study aimed to comparatively investigate the effects of accelerated aging on the physical and biological features of zirconia manufactured by digital light processing (DLP) and conventional subtractive manufacturing (SM) with similar composition.METHODS: Both the DLP- and SM-fabricated zirconia samples (7 mm × 7.5 mm × 1.5 mm) were grouped according to aging (134 °C, 0.2 MPa, 100% humidity) times, including 0 h, 5 h, and 10 h. Phase assemblage and surface topography of zirconia manufactured by different technologies were evaluated before and after aging. The biological effects of zirconia on human gingival fibroblast (HGF) cell events, including cell viability, proliferation, morphology and adhesion, were also evaluated by live/dead viability assay, cck-8 assay, scanning electron microscopy and confocal laser scanning microscopy respectively.RESULTS: The DLP-fabricated zirconia showed a higher initial cubic phase content and rate of phase transformation than the SM-fabricated zirconia. Among the different aging time-based groups, the 5 h-aged group exhibited significantly lower sub-micron scale surface roughness compared with the other groups. Aging did not significantly alter cellular behavior in any zirconia type, except for minor changes in adhesive cell numbers recorded in an aging time/culturing time-dependent manner. In addition to small differences in cell alignment patterns and overall cell morphology, the two zirconia types presented comparable biological performance before and after aging.CONCLUSION: Although the microstructure and surface characteristics of DLP-fabricated zirconia can be affected by autoclave aging, this newly manufactured zirconia is likely to maintain desirable long-term biocompatibility as an implant abutment material.

Abstract:INTRODUCTION: The use of zirconia in medicine and dentistry has rapidly expanded over the past decade, driven by its advantageous physical, biological, esthetic, and corrosion properties. Zirconia orthopedic hip replacements have shown superior wear-resistance over other systems; however, risk of catastrophic fracture remains a concern. In dentistry, zirconia has been widely adopted for endosseous implants, implant abutments, and all-ceramic crowns. Because of an increasing demand for esthetically pleasing dental restorations, zirconia-based ceramic restorations have become one of the dominant restorative choices. Areas covered: This review provides an updated overview of the applications of zirconia in medicine and dentistry with a focus on dental applications. The MEDLINE electronic database (via PubMed) was searched, and relevant original and review articles from 2010 to 2016 were included. Expert commentary: Recent data suggest that zirconia performs favorably in both orthopedic and dental applications, but quality long-term clinical data remain scarce. Concerns about the effects of wear, crystalline degradation, crack propagation, and catastrophic fracture are still debated. The future of zirconia in biomedical applications will depend on the generation of these data to resolve concerns.

The aging micromechanisms of alumina matrix composite (AMC) used in total hip arthroplasty

Abstract:Background: The aim of the present study is to simulate and better understand the long-term interplay between alumina matrix composite (AMC) femoral head and aqueous environment. In particular, we focused on clarifying the role of alumina grains on zirconia phase stability and mechanical equilibrium in AMC head during aging processes in a clinically-relevant time frame. Methods: The tested AMC head consists of an alumina matrix (82 vol.%) reinforced by yttria-stabilized zirconia (17 vol.%), chromium oxide (0.5 vol.%), and strontium aluminate (0.5 vol.%). The accelerated hydrothermal aging testing (at 134 °C, 2-bars pressure) was conducted up to 20 h. According to ASTM standard, 1-h aging under these conditions corresponds approximately to 2 yrs in vivo exposure. Confocal Raman and fluorescence spectroscopy were applied to quantify surface tetragonal-to-monoclinic phase transformation of zirconia and surface stresses in the AMC head. The Mehl-Avrami-Johnson (MAJ) theory was applied, which allowed modeling the micromechanisms of nucleation and growth of monoclinic sites during the transformation process. The obtained results were compared to those of monolithic zirconia (3Y-TZP) femoral heads. Results: The 3Y-TZP head showed a rapid increase of transformation rate beyond the aging time of 5 h (simulated as ∼10 yrs in vivo), suggesting the initiation of the transformation cascade toward the neighboring zirconia grains (growth mechanism). On the other hand, MAJ analysis revealed that the growth mechanism was completely absent and the nucleation of the monoclinic phase was partially prevented in the AMC head even after the 20-h aging (∼40 yrs in vivo). In addition, the stress accumulation in the AMC head was restricted at a quite low level throughout the aging simulation. Conclusion: Those results suggest that the presence of stable and hard alumina in the AMC can play a considerable role in slowing down the destabilization processes by spatially encompassing zirconia grains.
