Abstract:To research the influence of palate bone implantation to the growth of maxilla in three dimensional direction.40 patients of congenital cleft palate type II with palate board damage were stochastically divide into the comparison group and bone planted group. In bone planted group, HA-Bone cement was used to repair palate tabular damage. All patients are 16 full year old, cephalometric radiograph and tooth cast model were measured and contrast analysis was applied.Planted bone group and the comparison group does not have the significance difference in both the maxilla length and maxilla hightness( P > 0.05); But there was significance difference in the width of post-part of the maxilla (P < 0.05).It could promote the growth of maxilla width in use of bone implantation, that advocated the bone repair in the sequence treatment process in cases of the hard palate bone damage.

Abstract:Objective To observe the effect of artificial bone implantation of hard cleft palate on the development of maxilla.Methods From January 1997 to December 1999,40 patients with hard cleft palate were randomly divided into two groups:control group and implantation group(n=20 each).The patients in the implantation group received an implantation of compound artiffcial bone of HA-Bone cement.All pailents had a follow-up since 16 years old.A tllree dimensional model was established with computed tomography and rapid prototype technique to analyze the maxilla in three dimension.At the same time,a dentognathic model was employed.Results There were no differences in the results between the three dimensional and dentognathic models.No difference was found in the development of maxilla in length and height between the control and implantation groups.There were marked differences in the development of maxilla in width between two groups(67.6 mm±4.3 mm vs 61.3 1131/1±4.1 mm.63.5 mln±3.9 mm vs 57.3 mm±3.1 mm,26.2 mm±1.8 mm vs 26.4 mm±1.9 mm,all P<0.05).The width of maxilla in the implantation group was markedly wider than that in the control group.Conclusions The application of three dimensional model for evaluating the development of maxilla is both straightforward and accurate.Bone implantation of hard cleft palate is an obvious boost to the development of maxilla in width.It should be included into a comprehensive orthodontic treatment for patients with hard cleft palate.

Abstract:Background: Bone-anchored maxillary protraction (BAMP) aims to correct midfacial deficiencies, with proven positive skeletal changes without potential unwanted side effects. However, the influence of BAMP treatment on facial soft tissues, particularly in subjects with complete unilateral cleft lip, alveolus, and palate (CUCLAP), remains unclear. Methods: This single-center longitudinal cohort study examined the effects of 1.5 years of BAMP treatment on facial soft tissues in growing subjects with complete unilateral cleft lip, alveolus, and palate. The sample consisted of 25 patients, age range 9.7 to 12.6 years. Three-dimensional surface models derived from CBCT scans were superimposed on stable structures of the anterior cranial base and on the occipital area posterior of the foramen magnum to assess three-dimensional changes due to growth and BAMP therapy. Results: The results revealed a moderate positive correlation (Pearson's correlation coefficient from 0.203 to 0.560) between changes in hard tissue and soft tissue; some correlations were found to be weak (<0.300). Linear changes in soft tissue following BAMP were in the same direction as skeletal changes, showing downward, forward, and outward displacement. The only exception was in the vertical dimension. The lower facial third showed a slight but significant reduction, mainly in lip length (−1.2 mm), whereas the middle facial third showed a small increase (1.1 mm). Conclusions: It was concluded that during BAMP, soft tissue changes occur in the same direction as skeletal changes, although with a larger variability and less pronounced effects.

Abstract:Objective: To evaluate the application of midpalatal cortex osteotomy assisted rapid maxillary expansion for correction of maxillary transverse deficiency in young adults. Methods: Fourteen young adult patients with maxillary transverse deficiency were treated with midpalatal cortex osteotomy assisted rapid maxillary expansion. Lateral cephalogram and cone beam CT (CBCT) were taken before and 3 months after treatment. The width of basal bone, arch of maxilla and the torque of anchorage teeth were compared before and after treatment. Results: The width of dental arch of maxilla was increased from 40.54±5.26 mm before treatment to 46.83±5.83 mm after treatment (P<0.05) and the width of basal bone was increased from 64.86±4.16 mm to 67.60±4.66 mm (P<0.05) at the plane of the maxillary first molars. Accordingly, the width of dental arch of maxilla was increased from 31.92±2.55 mm to 38.65±3.14 mm (P<0.05) and the width of basal bone was increased from 43.33±3.70 mm to 45.78±4.57 mm (P<0.05) at the plane of first premolar. And the torque of maxillary anchorage teeth were increased (P<0.05). Conclusion: Midpalatal cortex osteotomy assisted rapid maxillary expansion is an effective micro-invasive method in expansion of basal bone and arch of maxilla for young adult patients with maxillary transverse deficiency.
Nonsurgical orthodontic treatment using bone-anchored maxillary protraction in a patient with unilateral cleft lip and palate

Abstract:Class III malocclusion due to a retrognathic maxilla is common in patients with cleft lip and palate. Skeletally anchored maxillary protraction using screw-anchored mini-plates combined with intraoral elastics has shown promising results in achieving orthopedic changes and maintaining the outcome until the completion of the growth. This case report presents the course of treatment in a patient with unilateral cleft lip and palate and multiple congenitally missing teeth treated with bone-anchored maxillary protraction until the end of growth. Four mini-plates (Bollard plates) were used during comprehensive fixed orthodontic treatment to protract the dentition and close the space where teeth were missing, extrude the canine, and force eruption of the second premolar using extension arms and cantilevers. A 2-year follow-up at age 17 showed stable occlusion and maintenance of soft tissue results. Bone-anchored maxillary protraction treatment in a patient with cleft lip and palate demonstrates proper orthopedic results and could be a viable alternative to orthognathic surgery.

Abstract:OBJECTIVE:To investigate the different changes after maxillary protraction between the cleft and non-cleft patients with anterior crossbite.METHODS:Prospective controlled clinical trial was carried out. Eighteen patients (aged 9.6 +/- 1.2 years) with unilateral cleft lip and palate (UCLP) were chosen as the experimental group. Eighteen non-cleft patients (aged 9.8 +/- 1.4 years) with similar malocclusions were selected as control group. All the patients were before puberty according to the evaluation of the hand wrist radiograms. Hyrax appliance and reverse headgear were used in both experimental and control groups. Lateral cephalometric radiographs were taken before and after treatment. Computerized cephalometric analysis was carried out. Paired t tests or t test were performed using software package SAS 8.1.RESULTS:After treatment, the main changes in experimental group were: SNA +1.45 degrees , A-TFH +2.19 mm, SNB -2.18 degrees , ANB +3.64 degrees , MP/SN +2.78 degrees , LL-EP +0.88 mm in UCLP group. And in control group were: SNA +1.42 degrees , A-TFH +2.12 mm, SNB -1.32 degrees , ANB +2.68 degrees , MP/SN +1.47 degrees , LL-EP -1.37 mm.CONCLUSIONS:Before puberty, maxillary protraction could accelerate maxillary forward growth in UCLP patients and the changes were similar to non-cleft patients. The occlusal relationship and soft tissue profile were improved significantly in both patients with or without cleft. The amount of mandibular posterior rotation in patients with UCLP was larger than in those with no cleft.

Abstract:Objective To investigate the effects of maxillary protraction on anterior crossbite in repaired pre-pubertal patients with complete unilateral cleft lip and palate(UCLP).Methods Prospective controlled clinical trial was carried out.Eighteen UCLP patients with anterior crossbite,aged 9.63±1.24 years,were the experimental subjects.Fourteen UCLP patients with similar malocclusion,aged 8.71±1.92 years,were selected as controls.All patients were before puberty according to the evaluation of the hand wrist radiograms.A Hyrax appliance and reverse headgear were applied.Lateral cephalometric radiographs were taken before and after the treatment.Computerized cephalometric analysis was carried out.Results After treatment in experimental group.the maxilla moved forward more significantly.The mandible rotated downward and backward.The intermaxillary relationship and soft tissue profile improved significantly.The rate of maxillary and mandibular Contribution was 1:1.7. ConelusionsMaxillary protraction could improve the imermaxillary relationship and soft tissue profile in UCLP patients with mild or moderate skeletal deformity before puberty.
Maxillary Protraction in Patients with Cleft Lip and Palate in Mixed Dentition: Cephalometric Evaluation after Completion of Growth

Abstract:Objective To clarify the short- and long-term effects of maxillary protraction (MP) in mixed dentition in patients with unilateral cleft lip and palate (UCLP). Design Retrospective study. Setting University of Tokyo Hospital. Patients and Intervention Eleven Japanese patients with UCLP in mixed dentition were treated with MP and followed up until the completion of growth. Multibracket treatment had been performed after MP treatment in all patients. Main Outcome Measure Lateral cephalograms taken before and after MP and after completion of growth were used. Posterior and anterior vertical reference lines (PV, AV) were used to measure the horizontal movements of point A, pogonion, and maxillary first molar (U6). SNA, SNB, ANB, maxillary and mandibular length, mandibular plane angle, Wits value, upper incisor inclination, overjet, and overbite were also measured. Results Large variation was found in the effects of MP, and five patients eventually required orthognathic surgery. In average change with MP, the maxilla showed favorable forward growth. Point A had moved forward from PV but not AV. The mandible rotated backward. However, ANB and the Wits value did not improve. U6 moved forward, and the overjet improved. After MP, the skeletal Class III relationship became severe. Conclusions MP was effective as an early treatment for UCLP patients. However, its effects showed large variation and were in conflict with facial growth. Conscientious explanation of the expected effects and associated problems should be given to the patients/parents before its application.
Long-term Stability of Maxillary Protraction Therapy in Class III Patients with Complete Unilateral Cleft Lip and Palate.

Abstract:Objectives: This study evaluated the long-term stability of maxillary protraction (MP) in patients with complete unilateral cleft lip and palate (UCLP) and identified factors influencing relapse and long-term outcomes. Materials and Methods: Twenty-three adolescents with UCLP who underwent MP therapy were recalled when craniofacial growth was close to completion. Subjects exhibiting reverse/positive overjets were assigned to unstable/stable groups. Lateral cephalometric measurements were made before treatment (T0), after active treatment (T1), and at the end of the growth spurt (T2). Results: About 63% of the subjects exhibited positive overjets during follow-up. The unstable group demonstrated higher B-x and Co-Gn distances than the stable group (both P < 05) at T0. More short-term (T0-T1) sagittal advancement of point A (A-y) was evident in the unstable group than in the stable group (P < 05), but no long-term difference was apparent between the two groups (P = .481). During the posttreatment period (T1-T2), the SNA angle and maxillary incisor protrusion (U1-SN angle) were considerably lower in the unstable group than in the stable group (both P < 05). Overall, the unstable group exhibited a lower increase in the vertical extent of point A (A-x) than the stable group from T0 to T2 (P < 05). Conclusions: In the long term, MP affords favorable maxillary advancement in patients with UCLP. A mandibular excess at T0 and vertical maxillary hypoplasia may contribute to the long-term relapse of a reverse overjet.

Abstract:As one of the basic areas of research for the improvement of the electromagnetic environment, this paper has developed a carbon plate having orientation characteristics. To exhibit the orientation characteristics in a plane wave, a slit was cut on the surface of a carbon plate. The value of the radio frequency (RF) magnetic shielding degree SD HP when holding the slit in the carbon plate perpendicularly, in the frequency region from 1 MHz to 1 GHz, increased with frequency and had a value of 48 dB at 1 GHz. It was also shown that the value of SD HH when holding the slit horizontally indicated about 15 dB in the frequency region from 1 MHz to 1 GHz. Namely, the difference in the values of shielding degree of the carbon plate versus the orientation of slit indicated the characteristics of the orientation. Experimental results revealed several characteristics of the slit carbon plate that include the influences of the slit length L, slit number N, and slit width w on the characteristics of the orientation. In this paper, the orientation characteristics were improved by the superposition of a slit ferrite plate over a slit carbon plate, termed the compound plate. In addition, important criteria are discussed for the design of a plate for effective RF magnetic shielding having orientation characteristics.
Maxillary Protraction Therapy in Class III Patients with and Without Cleft Lip and Palate: an Interim Report of a Prospective Comparative Study

Abstract:Objective: To investigate and compare the effects of maxillary protraction therapy on Class III patients with unilateral cleft lip and palate (UCLP) and Class III patients with noncleft. Design: Prospective controlled clinical trial. Patients: Twenty-six Class III patients with UCLP (mean age: 10.32 ± 1.29 years) and 26 Class III patients with noncleft (mean age: 9.82 ± 1.03 years) were included and treated with maxillary protraction therapy. Interventions: Maxillary protraction therapy was performed with an intraoral Hyrax appliance and extraoral facemask. Cone beam computed tomography scans were taken before and after treatment. Pretreatment skeletal and dental characteristics and treatment changes were analyzed and compared. Results: The average treatment duration was 18.44 ± 4.16 months in the UCLP group, which was substantially longer than the 12.46 ± 4.03-month average treatment duration in the noncleft group ( P < .001). No significant difference was found in the maxillary changes (length, advancement of point A, and SNA angle) and improvement of intermaxillary relationship (ANB angle) between the 2 groups. The UCLP group had 1.40° more mandibular clockwise rotation ( P = .034). Regarding dental changes, the UCLP group had more upper incisor proclination ( P = .006) and less lower incisor retroclination ( P = .023). Conclusions: Approximately extended maxillary protraction therapy in patients with UCLP could be as effective as in patients with noncleft. Further study is required to follow patients until completion of growth to elucidate the long-term stability of the treatment.

Abstract:The products of the recG and ruvAB genes of Escherichia coli are both thought to promote branch migration of Holliday recombination intermediates by their junction specific helicase activities in homologous recombination and recombination repair. To investigate the in vivo role of the recG gene, we examined the effects of a recG null mutation on cell division and chromosome partition. After UV irradiation at a low dose (5J/m2), delta recG mutant filamentous cells with unpartitioned chromosomes. A mutation in the sfiA gene, which encodes and SOS-inducible inhibitor of septum formation, partially suppressed filamentation of recG mutant cells, but did not prevent the formation of anucleate cells. The sensitivity of UV light and the cytological phenotypes after UV irradiation of a recA recG double mutant were similar to a recA single mutant, consistent with the role of recG, which is assigned to a later stage in recombinant repair than recA. The recG ruvAB and recG ruvC double mutants were more sensitive to UV, almost as sensitive as the recA mutant and showed more extreme phenotypes concerning filamentation and chromosome nondisjunction, both after UV irradiation and without UV irradiation than either recG or ruv single mutants. The recG polA12 (Ts) mutant, which is temperature sensitive in growth, formed filamentous cells with centrally located chromosome aggregates when grown at nonpermissive temperature similar to the UV irradiated recG mutant. These results support the notion that recG is involved in processing Holliday intermediates in recombination repair in vivo. We suggest that the defect in the processing in the recG mutant results in accumulation of nonpartitioned chromosomes, which are linked by Holliday junctions.
Maxillary protraction effects on anterior crossbites. Repaired unilateral cleft versus noncleft prepubertal boys.

Abstract:Objective: To test the hypothesis that there is no difference between the effects of maxillary protraction on anterior crossbites in repaired unilateral cleft lip and palate (UCLP) and noncleft prepubertal boys. Materials and Methods: Eighteen operated UCLP boys with anterior crossbite (aged 9.54 +/- 1.21 years) were the experimental subjects. Eighteen noncleft boys with similar malocclusion (aged 9.75 +/- 1.46 years) were selected as a control group. A Hyrax appliance and reverse headgear were applied. Lateral cephalometric radiographs were taken before and after the treatment. Computerized cephalometric analysis and statistics were carried out. Results: There were no significant differences in anterior movement of point A between the two groups (P >.05). ANB increased by 3.82 degrees and A-Pg (FH) increased by 5.89 mm in the UCLP group. ANB increased by 2.68 degrees and A-Pg (FH) increased by 3.66 mm in the noncleft group. The change of sagittal skeletal intermaxillary relationships was significantly larger in the UCLP group than in the noncleft group (P <.05). The change of lower lip in the UCLP group was less than in the noncleft group (P <.05). Conclusions: The hypothesis is rejected. The changes of sagittal skeletal intermaxillary relationship and mandibular clockwise rotation were larger in the operated UCLP patients than in the noncleft subjects. The amount of the lower lip retrusion was significantly less in the UCLP patients.
Long-term craniofacial morphology in young adults treated for a non-syndromal UCLP: A systematic review

Abstract:Minimizing mid-facial growth impairment is one of the treatment goals in cleft lip and palate surgery. As growth of the maxilla extends into young adulthood, long-term evaluation is essential to make a comprehensive assessment of a treatment protocol. There are numerous treatment approaches for cleft lip/palate surgery, and most have the characteristic distinction between either an early or a late cleft palate closure. PRISMA guidelines were applied to explore the quality of the current literature and to identify treatment factors influencing long-term cephalometric outcomes. The literature search was conducted in Pubmed, The Cochrane Library and Embase. We included studies evaluating cephalometric outcomes (SNA and ANB values on 2D cephalograms) in UCLP patients with a mean age of 16 years and older. Studies with an inadequate description of the timing of surgery were excluded. 17 studies comprising 906 patients were selected and included for critical appraisal. Treatment protocols differed considerably among the included studies and inconsistent methodology was common. Eight studies applied a one-stage procedure, 11 studies performed a two-stage reconstruction, and five studies made use of a vomer flap. Applying a multivariate model, we did not identify any treatment factors that significantly influenced growth (SNA/ANB values), except for the method of inclusion, suggesting the presence of significant selection bias within the studies. The current literature remains inadequate for evidence-based decision making and to advise parents if an early or late palate closure leads to a more favorable maxillary outgrowth. This manuscript will propose guidelines and recommended quality criteria for future studies.

Abstract:Background: The aim of this study was to present comprehensive skeletal, dental and facial aesthetic outcomes and long-term stability of maxillary anterior segmental distraction osteogenesis (MASDO) for the treatment of maxillary hypoplasia in CLP. Materials and methods: Six patients with maxillary hypoplasia treated with MASDO by a miniscrew assisted intraoral tooth-borne distractor were included. Cephalometric radiographs were obtained before distraction (T1), after the consolidation period (T2) and after orthodontic treatment or before orthognathic surgery (T3). Thirty-one cephalometric variables (12 skeletal, 9 dental, and 10 soft tissue variables) were used to evaluate changes in the dentofacial structures and the soft tissue profile. Friedman and Wilcoxon tests were applied to identify significant differences in hard and soft tissue changes during the T1-T2, T2-T3, and T1-T3 periods. Results: All patients successfully underwent MASDO without serious complications. From T1 to T2, forward movements of ANS and A (FH ⊥ N-A, VRL-ANS and VRL-A) were significant (p < 0.05). Significant increases in SNA and ANB were noted. Significant upward movement of points ANS (CFH-ANS) and A (CFH-A) was observed (p < 0.05). After distraction, a significant decrease in overjet and an increase in overbite were obtained (p < 0.05). Anterior tipping of the upper incisors (U1/ANS-PNS and U1/SN) was observed (p < 0.05). The soft tissue points of Pn, Sn, Ss, and ls showed significant anterior movement (p < 0.05). In addition, a significant increase in the nasolabial angle was measured (p < 0.05). All of the above data showed no statistically significant changes between T2 and T3 (p > 0.05). Conclusion: MASDO using a miniscrew assisted tooth-borne distractor presented significant maxillary advancement and favorable long-term stability in treating CLP patients with maxillary hypoplasia.
