Abstract:Objective To observe the effect of artificial bone implantation of hard cleft palate on the development of maxilla.Methods From January 1997 to December 1999,40 patients with hard cleft palate were randomly divided into two groups:control group and implantation group(n=20 each).The patients in the implantation group received an implantation of compound artiffcial bone of HA-Bone cement.All pailents had a follow-up since 16 years old.A tllree dimensional model was established with computed tomography and rapid prototype technique to analyze the maxilla in three dimension.At the same time,a dentognathic model was employed.Results There were no differences in the results between the three dimensional and dentognathic models.No difference was found in the development of maxilla in length and height between the control and implantation groups.There were marked differences in the development of maxilla in width between two groups(67.6 mm±4.3 mm vs 61.3 1131/1±4.1 mm.63.5 mln±3.9 mm vs 57.3 mm±3.1 mm,26.2 mm±1.8 mm vs 26.4 mm±1.9 mm,all P<0.05).The width of maxilla in the implantation group was markedly wider than that in the control group.Conclusions The application of three dimensional model for evaluating the development of maxilla is both straightforward and accurate.Bone implantation of hard cleft palate is an obvious boost to the development of maxilla in width.It should be included into a comprehensive orthodontic treatment for patients with hard cleft palate.
Midpalatal Cortex Osteotomy Assisted Rapid Maxillary Expansion for Correction of Maxillary Transverse Deficiency in Young Adults

Abstract:Objective: To evaluate the application of midpalatal cortex osteotomy assisted rapid maxillary expansion for correction of maxillary transverse deficiency in young adults. Methods: Fourteen young adult patients with maxillary transverse deficiency were treated with midpalatal cortex osteotomy assisted rapid maxillary expansion. Lateral cephalogram and cone beam CT (CBCT) were taken before and 3 months after treatment. The width of basal bone, arch of maxilla and the torque of anchorage teeth were compared before and after treatment. Results: The width of dental arch of maxilla was increased from 40.54±5.26 mm before treatment to 46.83±5.83 mm after treatment (P<0.05) and the width of basal bone was increased from 64.86±4.16 mm to 67.60±4.66 mm (P<0.05) at the plane of the maxillary first molars. Accordingly, the width of dental arch of maxilla was increased from 31.92±2.55 mm to 38.65±3.14 mm (P<0.05) and the width of basal bone was increased from 43.33±3.70 mm to 45.78±4.57 mm (P<0.05) at the plane of first premolar. And the torque of maxillary anchorage teeth were increased (P<0.05). Conclusion: Midpalatal cortex osteotomy assisted rapid maxillary expansion is an effective micro-invasive method in expansion of basal bone and arch of maxilla for young adult patients with maxillary transverse deficiency.

Abstract:To research the influence of palate bone implantation to the growth of maxilla in three dimensional direction.40 patients of congenital cleft palate type II with palate board damage were stochastically divide into the comparison group and bone planted group. In bone planted group, HA-Bone cement was used to repair palate tabular damage. All patients are 16 full year old, cephalometric radiograph and tooth cast model were measured and contrast analysis was applied.Planted bone group and the comparison group does not have the significance difference in both the maxilla length and maxilla hightness( P > 0.05); But there was significance difference in the width of post-part of the maxilla (P < 0.05).It could promote the growth of maxilla width in use of bone implantation, that advocated the bone repair in the sequence treatment process in cases of the hard palate bone damage.
[Multi-center study of recurrent nasal sinus polyposis: prognostic factors and possibility of prophylaxis].

Abstract:Today, surgery is the treatment of choice for nasal sinus polyposis. Nevertheless, although meticulous surgery does "per se" reduce the percentage of recurrences, there are cases where even the most painstaking removal of the entire pathology cannot prevent recurrence. Therefore recurrences do not appear linked to the type of surgery; rather onset appears linked to intrinsic, only partially recognizable factors responsible for the primary and secondary polypogenesis. In order to identify negative prognostic factors which might be implicated in recurrences, the present study extrapolated the data from forms on 181 patients who had undergone surgery for nasal sinus polyposis and subjected it to multivariance analysis. These patients were recruited during the course of a multicenter study with the participation of 12 ENT Centers in Piemont and Liguria. The recurrence rate was 13%. In analyzing unfavorable factors prognosticating recurrence, thirteen parameters were examined. Nine of these (age, sex, severe deviation of the septum causing restriction, severe turbinate hypertrophy, surgery or repeat surgery for recurrence, type of macro-micro endoscopic surgery, allergy to seasonal inhalants, allergy to perennial inhalants, mixed allergies) did not prove to have any significance in recurrences. The presence of bilateral involvement of the sinus system presented a negative trend as regards recurrences while involvement of more than one subsite (anterior ethmoid, posterior ethmoid, maxillary sinus, sphenoid), ASA and NSAID intolerance and abundant eosinophilic infiltration in the mucous chorion proved statistically significant (p < 0.05 for all three parameters) for recurrence. Post-operative topic prophylactic treatment with steroids (beclomethasone) or anti-H1 drugs (azelastin, HCl) did not appear to affect the onset of recurrence although it did have a positive effect on subjective symptoms.

Abstract:Objective: To investigate the efficacy of microimplant-assisted rapid palatal expansion (MARPE) to treat skeletal maxillary discrepancies during the post-pubertal growth spurt stage. Materials and Methods: Sixty patients with skeletal maxillary transverse deficiency during the post-pubertal growth spurt stage were randomly divided into MARPE and Hyrax groups. Thirty patients (mean age: 15.1 +/- 1.6 years) were treated using the four-point MARPE appliance; 30 patients (mean age, 14.8 +/- 1.5 years) were treated using the Hyrax expander. Cone beam computed tomography scans and dental casts were obtained before and after expansion. The data were analyzed using paired t-tests and independent t-tests. Results: The success rates of midpalatal suture separation were 100% and 86.7% for MARPE and Hyrax groups, respectively. Palatal expansion and skeletal to dental ratio at the first molar level were greater in the MARPE group (3.82 mm and 61.4%, respectively) than in the Hyrax group (2.20 mm and 32.3%, respectively) (P < .01). Reductions in buccal alveolar bone height and buccal tipping of the first molars were less in the MARPE group than in the Hyrax group (P < .01). Conclusions: MARPE enabled more predictable and greater skeletal expansion, as well as less buccal tipping and alveolar height loss on anchorage teeth. Thus, MARPE is a better alternative for patients with skeletal maxillary deficiency during the post-pubertal growth spurt stage.
Effects of microimplant-assisted rapid palatal expansion on maxillary transverse discrepancy in young adult patients: a CBCT analysis

Abstract:Objective To investigate the results of treating transverse maxillary discrepancy by microimplant assisted rapid palatal expansion(MARPE) in young adult patients.Methods 10 patients aged from 15.5 to 28.0 years of age with transverse maxillary discrepancy were included (average 20.1±5.4 years,3 male and 7 female).The constricted maxilla was expanded by MARPE.CBCT was taken before treatment,after expansion,and after 3 months retention.The data were measured and analyzed.Results The widths of anterior and posterior palatal suture increased by 3.49 mm and 2.94 mm,respectively.The mean posterior midpalatal suture opening was 47.9％ of the total screw expansion and 52.3 ％ of maxillary first molar expansion.Nasal width,basal bone width and alveolar width increased significantly after expansion(P＜0.05),and were stable after 3 months of retention.Alveolar height did not show any statistical differences(P＞0.05).Conclusions The palatal suture could be expanded by MARPE in young adult patients and the skeletal maxillary discrepancy could be corrected successfully.

Abstract:Objective : This study compared the skeletal and dentoalveolar changes induced by tissue-bone-borne expander (C-expander) and pure-bone-borne expander (ATOZ-expander) using CBCT analysis. We aimed to identify their distinct expansion patterns and potential clinical implications. Materials and Methods : A total of 60 subjects, comprising 14 males and 46 females with an average age of 18.5±7.36 years, were divided into two groups: the tissue-bone-borne expander group (CEXP group, N=30, average age 19.16 ± 7.09 years) and the pure-bone-borne expander group (ATOZ group, N=30, average age 17.83 ± 7.56 years). CBCT scans were obtained before (T1) and after expansion (T2). Zygomatic, palatal, and dentoalveolar dimensions were measured and analyzed using paired T-tests and Kruskal-Wallis tests. Results : The CEXP group demonstrated significantly greater dental expansion and an increase in dentoalveolar inclination. In contrast, the ATOZ group exhibited a decrease in dentoalveolar inclination in three coronal planes. In horizontal plane, the CEXP group showed more parallel expansion pattern than the ATOZ group. Both groups showed similar amount of palatal expansion. Conclusion : The tissue-bone-borne C-expander demonstrated more pronounced dentoalveolar effects with a vertically pyramidal and sagittally parallel pattern of expansion. In contrast, the pure bone-borne ATOZ-expander exhibited more substantial skeletal effects with a vertically parallel and sagittally V-pattern expansion.

Three-dimensional finite element analysis of maxillary protraction with labiolingual arches and implants

Abstract:Introduction: In this study, we aimed to evaluate the effects of maxillary protraction using traditional labiolingual arches and implant-type protraction devices before orthopedic treatment of patients with skeletal Class III malocclusion. Methods: A 3-dimensional finite element model of the maxillofacial bones with high biologic similarity and including the sutures was constructed. Through stress and displacement calculations, a biomechanical study was performed for the maxillofacial bones, mandible, and sutures. Results: We quantified detailed changes in the sutures with 2 protraction methods to analyze their effects on the growth of the maxillofacial bones. Conclusions: (1) The labiolingual arch is suitable for skeletal Class III patients with crossbite and deep overbite. The frontomaxillary and zygomaticomaxillary sutures played major roles in the forward displacement and counterclockwise rotation of the maxilla. The temporozygomatic and pterygopalatine sutures did not change significantly. (2) The implant type of protraction device is suitable for skeletal Class III patients with crossbite and open bite. Both the frontomaxillary and zygomaticomaxillary sutures played decisive roles in the forward displacement and clockwise rotation of maxilla. The temporozygomatic and pterygopalatine sutures showed small changes. (3) The labiolingual arch caused less stimulatory growth on the maxilla, whereas the implant caused greater stimulatory growth on the maxilla. Protraction with the labiolingual arch is more suitable for early skeletal Class III patients at a younger age; protraction with an implant is applicable to skeletal Class III patients in the late mixed dentition or early permanent dentition.
Finite Element Analysis Model for Assessing Expansion Patterns from Surgically Assisted Rapid Palatal Expansion

Abstract:Surgically assisted rapid palatal expansion (SARPE) was introduced to release bony resistance to facilitate skeletal expansion in skeletally mature patients. However, asymmetric expansion between the left and right sides has been reported in 7.52% of all SARPE patients, of which 12.90% had to undergo a second surgery for correction. The etiologies leading to asymmetric expansion remain unclear. Finite element analysis has been used to evaluate the stress associated with SARPE in the maxillofacial structures. However, as a collision of the bone at the LeFort I osteotomy sites occurs only after a certain amount of expansion, most of the existing models do not truly represent the force distribution, given that the expansion amount of these existing models rarely exceeds 1 mm. Therefore, there is a need to create a novel finite element model of SARPE that could perform a clinically required amount of expander activation for further analysis of the expansion patterns of the hemimaxillae in all three dimensions. A three-dimensional (3D) skull model from cone beam computed tomography (CBCT) was imported into Mimics and converted into mathematical entities to segment the maxillary complex, maxillary first premolars, and maxillary first molars. These structures were transferred into Geomagic for surface smoothing and cancellous bone and periodontal ligament creation. The right half of the maxillary complex was then retained and mirrored to create a perfectly symmetrical model in SolidWorks. A Haas expander was constructed and banded to the maxillary first premolars and first molars. Finite element analysis of various combinations of buccal osteotomies at different angles with 1 mm clearance was performed in Ansys. A convergence test was conducted until the desired amount of expansion on both sides (at least 6 mm in total) was achieved. This study lays the foundation for evaluating how buccal osteotomy angulation influences the expansion patterns of SARPE.

Abstract:As one of the basic areas of research for the improvement of the electromagnetic environment, this paper has developed a carbon plate having orientation characteristics. To exhibit the orientation characteristics in a plane wave, a slit was cut on the surface of a carbon plate. The value of the radio frequency (RF) magnetic shielding degree SD HP when holding the slit in the carbon plate perpendicularly, in the frequency region from 1 MHz to 1 GHz, increased with frequency and had a value of 48 dB at 1 GHz. It was also shown that the value of SD HH when holding the slit horizontally indicated about 15 dB in the frequency region from 1 MHz to 1 GHz. Namely, the difference in the values of shielding degree of the carbon plate versus the orientation of slit indicated the characteristics of the orientation. Experimental results revealed several characteristics of the slit carbon plate that include the influences of the slit length L, slit number N, and slit width w on the characteristics of the orientation. In this paper, the orientation characteristics were improved by the superposition of a slit ferrite plate over a slit carbon plate, termed the compound plate. In addition, important criteria are discussed for the design of a plate for effective RF magnetic shielding having orientation characteristics.
Comparison of alveolar bone morphology after expansion with hybrid and conventional Hyrax expanders

Abstract:Objectives: To compare the buccal and palatal bone changes of maxillary posterior teeth produced by hybrid hyrax (HH) and conventional hyrax (CH) expanders in growing patients. Material and methods: A sample of 32 patients with posterior crossbites in the late mixed dentition was recruited and randomly allocated into two groups. Group HH was composed of 18 individuals with a mean age of 10.7 years (six female, 12 male) treated with a hybrid expander with two anterior parasagittal miniscrews. Group CH was composed of 14 individuals with a mean age of 11.4 years (six female, eight male) treated with a conventional Hyrax expander. Cone-beam computed tomography (CBCT) exams were obtained before expansion (T1) and after 11 months when the expander was removed (T2). Buccal and palatal bone plate thickness and height of maxillary posterior teeth were measured. Intergroup comparisons were performed using t or Mann-Whitney tests (P < .05). Results: The CH group showed greater decreases of the buccal bone plate height (mean change: 1.27 mm) at the maxillary first premolars compared to the HH group (mean change: 0.11 mm, P = .001). No intergroup difference was found for changes in the buccal and palatal bone thickness. Conclusions: Hybrid expanders showed a tendency to cause less negative impact on the buccal bone plate height of first premolars compared to conventional Hyrax expanders. However, the difference was not clinically significant. Both hybrid and conventional Hyrax expanders are safe for the alveolar bone morphology in the late mixed dentition.

Abstract:Introduction: This study investigated the results of maxillary discrepancy treated by microimplant-assisted rapid palatal expansion (MARPE) at different ages. Methods: Sixty patients (aged 11.0-34.1 years; 23 male and 37 female) were treated by MARPE. Cone-beam computed tomography scans and dental casts were taken before and after expansion. The data were compared among 4 age groups: early adolescents (aged 11-14 years), late adolescents (aged 15-19 years), young adults (aged 20-24 years), and old adults (aged 25-34.1 years). Results: The success rates of midpalatal suture separation were 100%, 100%, 88.2%, and 85.7% for early adolescents, late adolescents, young adults, and old adults, respectively. Palatal sutures at the level of the first molar were expanded by 4.02 mm, 3.48 mm, 2.63 mm, and 2.10 mm, corresponding to 66.7%, 58.1%, 42.0%, and 37.9% of the total dental expansion. Significant differences were found in the amounts of palatal suture expansion and the ratio of skeletal dental expansion between patients aged <20 years and patients aged >20 years (P <0.05). Skeletal expansion constituted 69.4%, 51.3%, 39.0%, and 29.8% of the total screw expansion. Except for the comparison between young adults and old adults, there were significant differences between groups for the ratio of skeletal screw expansion (P <0.05). Conclusions: The midpalatal suture can be expanded by MARPE more easily in patients <20 years of age than in patients >20 years of age. The ratio of skeletal screw expansion decreases as age increases.
Design and application of personalized microimplant-assisted rapid palatal expander

Abstract:Objective:To investigate the efficiency of personalized microimplant-assisted rapid palatal expansion in treating skeletal narrow upper arch.Methods:60 patients with skeletal narrow maxilla were included. The patients have an average 20.4±5.9 years old (ranged from 11 to 34 years), consisting of 23 male and 37 female. The constricted maxilla was treated by personalized microimplant-assisted rapid palatal expansion. CBCT and casts were taken before and after expansion. The data were measured and analyzed.Results:The success rate of midpalatal suture expansion was 93.3% after treatment. The width of midpalatal suture and the width of dental arch at the maxillary first molar level increased significantly by (3.06±1.27) mm and (6.04±1.45) mm, respectively ( P<0.05). The ratio of skeletal expansion to teeth expansion was 0.509±0.196. The amount of expander opening was (6.76±1.31) mm, and the ratio of skeletal expansion to screw expansion was 0.473±0.205. Conclusions:The personalized microimplant-assisted rapid palatal expansion, which could successfully treat maxillary skeletal transverse discrepancy in adolescents and 88.9% young adult patients.

Abstract:This study evaluated the success rate and influencing factors of the pure-bone borne maxillary skeletal expander (the ATOZ expander) in 42 patients (12 males, 30 females, mean age 18.14 years, range 7.42 years) with maxillary transverse deficiency. Cone-beam CT scans were analyzed at baseline (T0) and after expansion (T1), comparing skeletal measurements between groups categorized by "total" and "clinical success (>1mm expansion)". Several factors, including age, sex, and midpalatal suture maturation stage, were assessed for their impact on expansion success and patterns. The ATOZ expander achieved a 92.8% success rate, and all skeletal measurements differed significantly post-expansion. Younger age and less mature sutures were associated with greater expansion success, while sex did not significantly influence "clinical success". Younger age predicted greater expansion, and midpalatal suture maturation influenced the outcomes of maxillary expansion. Considering individual factors like age and suture maturity can guide personalized treatment plans for optimal skeletal expansion outcomes.

Abstract:OBJECTIVE:To use the cone-beam computed tomography (CBCT) to evaluate the three-dimensional (3D) changes of maxillary landmarks in the maxillary protraction with alternating rapid palatal expansion and constriction and with rapid palatal expansion, and to provide some clinical suggestions for the early treatment of Class III malocclusion.METHODS:A total of 36 maxillary retrusive patients were included and randomized in a 1:1 ratio to either the intervention group (alternating rapid palatal expansion and constriction group, RPE/C) or the control group (rapid palatal expansion group, RPE). Randomization was accomplished with permuted block randomization based on participation sequence. The patients in the RPE/C were treated for 10 weeks (0.5 mm/d) with the repetition of two-week palatal expansion and two-week palatal constriction. The patients in the RPE were taught to complete rapid palatal expansion for 2 weeks (0.5 mm/d ). The patients were instructed to come to the office for the follow-up to ensure the correct procedures. Damaged expanders were repaired (or replaced) and rebanded quickly. Sequential CBCT images including pretreatment (T1), post-expansion (T2) and post-protraction (T3) were required for 3D reconstruction, establishment of landmarks, measurement and analysis by Mimics 10.01.RESULTS:There was significant forward movement of subspinale (A) in the RPE/C after the treatment with (3.06±1.29) mm, compared with RPE (2.16±1.27) mm, P<0.05. There were more symmetrical changes of the landmarks in the RPE/C and there was no statistic significance of the entire treatment time between the two groups. Moreover, the maxillary skeletal landmarks had the following 3D changes of a forward and downward movement during the expansion stages T2-T1, a forward and upward movement during the protraction stages T3-T2 and a forward and downward movement during the total treatments T3-T1 compared with the control group. And the width between the bilateral landmarks increased during the expansion stages T2-T1, narrowed down during the protraction stages T3-T2 and increased during the total treatments T3-T1.CONCLUSION:The maxillary protraction with alternating rapid palatal expansion and constriction provided clinical benefits on maxillary advancement and symmetrical changes in the orthopedic treatment of the patients with maxillary retrognathism and it required further study on the orthodontic analysis and measurements of CBCT.
[Effect of Repetitive Rapid Palatal Expansions and Constrictions by Double Hinged Expander in Maxillary Protraction Cases].

Abstract:OBJECTIVE:To investigate the benefits of repetitive rapid palatal expansions/constrictions therapy using double hinged expander over rapid palatal expansion alone with Hyrax expander in maxillary protractions cases and to discuss the indications of the combination therapy.METHODS:Thirty four children with retrognathic maxilla were selected and randomly divided into two treatment groups. Eighteen children were in group A and received maxillary protraction with Hyrax expander rapid palatal expansion alone. Sixteen children were in group B and received maxillary protraction with double hinged expander repetitive rapid palatal expansions/constrictions combin therapy. Cephalometric analysis was done to compare the pre and post-treatment lateral cephalograms.RESULTS:Significant difference in forward movement of maxilla (P<0.05)were observed between group B [A-Np increased 2.53 mm, A-vertical line (A-VRL) increased 2.78 mm] and group A (relatively 1.86 mm and 2.01 mm). Significant difference in soft tissue backward movement of mandible (P<0.05)were also observed between group A (LLC-VRL decreased 2.82 mm) and group B (relatively 1.63 mm).CONCLUSION:Repetitive rapid palatal expansions/constrictions using double hinged expander resulted in more maxilla forward movement compared with Hyrax rapid palatal expansion alone in maxillary protraction cases.

Abstract:ObjectivesThis study assessed the dental and skeletal effects of pure bone-borne, non-surgical maxillary expansion, using a modified force-controlled polycyclic protocol.MethodsRecords of 17 adult patients, mean age 24.1 years; range 18–39 years, who had undergone maxillary expansion using a bone-borne Quad-expander (with 4 mini-screws), were analysed. In all patients, 0.17 mm/day of expansion was completed for 1 week, followed by a cyclic protocol of expansion of forward and backward turns until the force needed to turn the expander was below 400 cN, assessed weekly. After this, expansion continued at a rate of 0.17 mm/day until the desired amount of expansion was achieved. Cone beam computer tomography scans were taken pre- and post-expansion.ResultsThe mid-palatal suture was successfully opened in 100% of patients included in this study. Axially, the amount of skeletal opening at the posterior nasal spine was 61% of the anterior nasal spine. Expansion was pyramidal in the coronal plane. Significant increases at the dental and skeletal levels were achieved, with changes at the skeletal level reaching 73%. The alveolar bone angle increased more than the angular changes at the molars and premolars.LimitationsThis is a retrospective study with short-term results.ConclusionThe Quad-expander, with a force-controlled polycyclic expansion protocol, effectively produced a significant increase in maxillary width in skeletally mature subjects in the short term.

Abstract:Objective The purpose of this study was to evaluate the effects of intraoral maxillary protraction with repetitive rapid palatal expansions and constrictions using double hinged expander in maxillary retrognathism cases.Methods 29 subjects aged 7 to 13 years were randomly assigned to the following two groups.Observation group:intraoral maxillary protraction with repetitive rapid palatal expansions and constrictions using double hinged expander.Control group:facemask maxillary protraction with rapid palatal expansion using Hyrax expander.Cephalometric analysis with traditional cephalometric measurements and a rectangular coordinate system was used to compare the pre and post-treatment cephalographs.Results The treatment course in observation group (7.04 months) was significantly shorter than that in control group (11.04 months) (P＜0.01).Significant difference in forward movement of maxilla was observed between observation group (A-Np increased 1.28mm) and control group (A-Np increased 2.03 mm) (P＜0.05).The quantity of mandibular downward and backward rotation was found significantly smaller in observation group (P＜0.05).Significant counterclockwise rotation of functional occlusal plane was found in observation group(P＜0.01),while the rotation in control group was clockwise.Conclusions Intraoral maxillary protraction with repetitive rapid palatal expansions and constrictions using double hinged expander is an effective and efficient method in the early treatment of maxillary retrognathism patients.It results in a slightly less maxillary forward movement than the conventional facemask maxillary protraction with rapid palatal expansion using Hyrax expander.
