Abstract:Annals of Pharmacotherapy, Ahead of Print. Background:Low-dose valganciclovir (VGC) for cytomegalovirus (CMV) prophylaxis post-transplant has been employed due to cost and safety. The incidence of CMV disease in CMV intermediate-risk liver recipients at 1-year after standard-dose prophylaxis is approximately 5%. However, there are limited data on outcomes after using a "true" low-dose VGC prophylaxis regimen in liver and dual-abdominal transplant recipients as VGC was not dose-adjusted in all patients with impaired renal function in prior studies.Objective:The objective was to assess the incidence of CMV associated with low-dose VGC prophylaxis in CMV intermediate-risk liver, simultaneous pancreas-kidney (SPK), and simultaneous liver-kidney (SLK) recipients with creatinine clearance (CrCl) >60 mL/min.Methods:This was a retrospective review of CMV intermediate-risk liver, SPK, and SLK recipients with CrCl >60 mL/min transplanted January 2018 to June 2022 who received VGC 450 mg daily for prophylaxis. The primary outcome was incidence of CMV infection 6-months post-transplant.Results:Ninety-nine transplant recipients were included (79 liver, 11 SPK, 9 SLK). The primary outcome occurred in 13% of patients (liver 10%, SPK 36%, SLK 10%), including 1 case of CMV disease and 3 breakthrough infections. In addition, 6 patients experienced CMV infection between 6-months and 1-year. Recurrence occurred in 3 patients. There was no evidence of CMV resistance. Thirty patients experienced neutropenia within 1-year, 32 were prescribed granulocyte-colony stimulating factors, and 5 experienced thrombocytopenia. Two patients died due to graft-vs-host disease.Conclusion and Relevance:Low-dose VGC prophylaxis led to comparable CMV infection rates at 6-months in CMV intermediate-risk liver and SLK recipients. However, as SPK recipients displayed higher rates of CMV infection, low-dose VGC should be avoided in this population.

Abstract:The utilization of CMV CD4+ cellular immunity to guide duration of CMV prophylaxis in high‐risk patients did not lead to increased risk of clinically significant CMV viremia in kidney and/or pancreas transplant recipients. Background Valganciclovir prophylaxis against cytomegalovirus (CMV) is recommended for solid organ transplant recipients, but is associated with drawbacks, including expense and leukopenia. Our center adopted a strategy of serial assessment with a CMV‐specific T cell immunity panel (CMV‐TCIP) and cessation of valganciclovir prophylaxis upon demonstration of adequate CD4+ responses in kidney transplant patients at high risk of CMV disease. Methods We retrospectively reviewed adult recipients of a kidney or pancreas transplant between August 2019 and July 2021 undergoing serial CMV‐TCIP monitoring. Included patients were considered high risk for CMV, defined by donor positive (D+)/recipient negative (R−) CMV IgG serostatus, or recipient positive (R+) patients who received induction with a lymphocyte‐depleting agent. Prophylaxis was discontinued after a patient's first CMV‐specific CD4+ T cell value of ≥0.20%. Risk of clinically significant CMV infection (csCMVi) in those who underwent early discontinuation of CMV prophylaxis and predictors of CMV T cell immunity were analyzed. Results Of 54 included patients, 22 stopped prophylaxis early due to CMV‐specific CD4+ T cell immunity at a median of 4.7 (IQR: 3.8–5.4) months after transplant. No instances of csCMVi were observed in the 22 patients who had prophylaxis discontinued early, of whom 19/22 were CMV R+ and 3/22 were CMV D+/R−. Effect of CMV Mismatch on Heart Transplant Outcomes Using a Surveillance and Preemptive Strategy

Abstract:Purpose: The aim of the study was to determine outcomes after heart transplantation for cytomegalovirus (CMV) mismatched patients (D+/R-) who underwent a surveillance and preemptive therapy protocol, compared to nonmismatch patients. Methods: A review of patient records from January 2010 to December 2020 with follow-up to October 2023 was done. The protocol consisted weekly surveillance with CMV PCR starting 4 weeks after transplant continuing up until the patient seroconverts or up to 3 months posttransplant if the patient does not seroconvert. Valganciclovir was given for 2 weeks to those who seroconverted. Results: Two hundred and twenty-one patients were included, and 23% were mismatched patients. Overall survival was not different between CMV groups (p = NS). Causes of death and morbidities were also not significantly different (p = NS). Sixty-six percent of mismatch patients seroconverted, and there was also a significantly older donor age in the seroconverted patients compared to nonseroconverted patients (41 ± 11 vs. 29 ± 12 years, p < 0.005), indicating a higher risk donor profile. A multivariate Cox regression including donor age showed that there was no increase in mortality in the seroconverted mismatches compared to nonmismatch patients (p = NS). Conclusions: There is no significant increased mortality or morbidity using a CMV surveillance and preemptive therapy protocol. The effect of donor age on seroconversion of mismatches requires further validation.
Prevention of Resistant CMV Infection Using Persistent Valganciclovir Therapy through Leukopenia and Extending the Duration

Abstract:Purpose Cytomegalovirus (CMV) infection increases morbidity and mortality in lung transplant recipients. Our center identified CMV-resistant (rCMV) disease resulting in the death of 3 out of 4 cases. We attributed rCMV to erratic dosing, inconsistent medication levels, and a shortened duration of valganciclovir (Val) prophylaxis. We describe the effect of changes in a prophylaxis protocol in order to prevent rCMV in at-risk patients. Methods and Materials The Pre-intervention cohort (Pre 3/08-4/11) received prophylaxis with Val for 3 (D+ and D-/R+) to 6 (D+/R-) months; Val was held for WBC 6 months and followed ≥12 months were included. Frequency of CMV events, time to first event (unadjusted and adjusted for changes in Val or IS), frequency of changes in Val dosing or IS (due to adverse effects) and frequency of rCMV were compared. Results 54 Pre and 23 Post patients were included. CMV event frequency (Pre 23/54 (43%) vs. Post 9/23 (39%) (p=0.77)), median time to events (Pre 54 days, Post 69 days) and time to event analysis was not different between groups (HR 1.087, 95% CI 0.50-2.35). Changes in IS or Val dosing were not more frequent Pre (11/54 (20%)) versus Post (6/23 (26%)) (p=0.57). Pre was associated with development of rCMV in 4/54 (7%) patients, but rCMV in Post has not yet been detected. Conclusions Our combination therapy was successful in preventing rCMV in at-risk patients even though the frequency of CMV events and the changes in IS or Val dosing were unchanged. Our findings suggest that a longer duration of prophylaxis with consistent dosing and perhaps the addition of CMV-IVIg leads to a reduction in rCMV.

Abstract:Background. The role of cytomegalovirus (CMV) infection in renal allograft rejection remains controversial; moreover, there are few studies on pretransplant infections. This study sought to investigate whether pretransplant CMV infections had negative effects on acute rejection episodes (ARE) and to evaluate the effect of preemptive treatment.Methods. This retrospective single-center study of 416 transplant recipients from October 1, 2000 to September 1, 2003 had CMV infections diagnosed by CMV antigenemia tests. The incidences of ARE were compared between CMV-infected and noninfected groups. Risk factors for ARE were analyzed. Based on preemptive treatment, pretransplant CMV-infected recipients were divided into ganciclovir-treated and non-treated groups and the incidence of ARE was compared between the two groups.Results. One hundred eighty four recipients had CMV infections pretransplant; the infection rate was 44.2%. Fifty five recipients had ARE among the pretransplant CMV-positive group, which was significantly higher than that in the noninfected group (29.9% vs 19.5%, P = .014). But the rejection subgroups and renal function recovery had no significant differences. While the presence of pretransplant infection was an independent predictor of ARE (RR = 1.807), severity showed no significant impact on ARE. Among 184 pretransplant CMV infection recipients, the incidences of ARE were 14.3% and 18.0% in ganciclovir-treated versus nontreated patients, respectively (P = .650).Conclusions. Pretransplant CMV-positive recipients were at greater risk of ARE. Pretransplant CMV infection was an independent risk factor for ARE. Preemptive antiviral treatment did not show protective effects against ARE related to CMV infection-mediated immunological injuries.

Abstract:Background: Valganciclovir is the only approved antiviral for cytomegalovirus (CMV) prevention in pediatric solid organ transplantation (SOT). Additional approaches may be needed to improve outcomes. Methods: A multicenter retrospective study from 2016 to 2019 was conducted of pediatric SOT recipients in whom at least 3 months of valganciclovir prophylaxis was planned. Episodes of CMV DNA in blood (DNAemia), CMV disease, drug-related toxicities, as well as other infections in the first year posttransplant and demographic and clinical data were collected. CMV DNAemia in the first year after prophylaxis or during prophylaxis (breakthrough) was analyzed by multivariate hazard models. Results: Among the 749 patients enrolled, 131 (17.5%) had CMV DNAemia at any time in the first year; 85 (11.4%) had breakthrough DNAemia, and 46 (6.1%) had DNAemia after prophylaxis. CMV disease occurred in 30 (4%). In a multivariate model, liver transplantation compared to kidney or heart, intermediate or high risk based on donor/recipient serologies, neutropenia, and valganciclovir dose modifications attributed to toxicity were associated with increased risk of total and/or breakthrough DNAemia. Bacteremia was also associated with increased hazard ratio for CMV DNAemia. In a separate multivariate analysis, rejection occurred more often in those with breakthrough CMV DNAemia (P = .002); liver transplants, specifically, had increased rejection if CMV DNAemia occurred in the first year (P = .004). These associations may be bidirectional as rejection may contribute to infection risk. Conclusions: CMV DNAemia in the first year posttransplantation occurs despite valganciclovir prophylaxis and is associated with medication toxicity, bacteremia, and rejection. Pediatric studies of newer antivirals, especially in higher-risk subpopulations, appear to be warranted.

Abstract:<section class="article-section__content"><h3 class="article-section__sub-title section1"> Background</h3><p>Primary cytomegalovirus (CMV) disease in high risk (D+/R‐) abdominal solid organ transplant recipients (aSOTRs) is well described, however little is known of primary CMV disease in low risk (D‐/R‐) patients.</p></section><section class="article-section__content"><h3 class="article-section__sub-title section1"> Methods</h3><p>Observational study of adult aSOTRs between 1/1/2009‐9/1/2019 screened based on serostatus at transplant; D‐/R‐ and D+/R‐ patients were included. Primary objective: Describe epidemiology of primary CMV in D‐/R‐ aSOTRs. Secondary objective: compare infectious and transplant related outcomes of primary CMV disease in the first 90 days (early CMV) between D‐/R‐ and D+/R‐.</p></section><section class="article-section__content"><h3 class="article-section__sub-title section1"> Results</h3><p>Of 782 D‐/R‐ aSOTRs in the study period, 13 developed CMV at any time after transplant to last follow‐up. Of 671 D+/R‐ patients, 186 developed CMV. Early CMV disease was significantly more common in the D‐/R‐ group (54% vs 15.6%, p=0.0005) despite populations being similar demographically, including allograft subtype. D‐/R‐ patients with early CMV disease had median viral load >100,000 IU/mL and 42.9% had end‐organ manifestations; 71.4% required hospital admission. Immunosuppressive therapy was adjusted in 100% of patients, there was an approximately 14.3% rate of antiviral resistance and 28.6% had concomitant opportunistic infection. These findings were similar to D+/R‐ patients. There was no difference in risk of rejection or all‐cause mortality associated with early CMV disease, however graft loss was significantly higher in D‐/R‐</p></section><section class="article-section__content"><h3 class="article-section__sub-title section1"> Conclusion</h3><p>D‐/R‐ aSOTRs infrequently develop CMV, however when it occurs, they present with disease manifestations similar to and graft outcomes inferior to D+/R‐ with CMV. Additionally, the majority of CMV disease in D‐/R‐ occurs in the first 90 days after transplant, suggesting possible donor subclinical infection or transfusion source. The complicated course in D‐/R‐ is likely due to low clinical suspicion. Awareness of disease severity and aggressive upfront management may promote positive outcomes.</p></section>

Abstract:In this single‐center study, high‐risk patients are at risk for prolonged and breakthrough cytomegalovirus infections. Most breakthrough infections appear under substantial valganciclovir prophylactic dose reduction and low estimated glomerular filtration rate. Background Cytomegalovirus (CMV) infections are a common complication after kidney transplantation (KTx) and negatively affecting patient outcome. Valganciclovir (VGC) prophylaxis is often limited by drug‐induced side effects and dose reduction due to decline in kidney function. Method In the present study, episodes of CMV viremia in the first year after KTx in a cohort of 316 recipients were analyzed retrospectively to identify risk factors linked to persistent infections. Results In the studied cohort, 18.7% of patients showed a high‐risk (HR) constellation (D+/R–) for CMV infections. CMV viremia affected 22% of our cohort, with HR patients being the most affected cohort (44.1%). Within this group, most viremic events (65.3%) occurred while patients were still on prophylactic therapy, showing significantly higher viral loads and a longer duration compared to seropositive recipients. Conclusion The analysis at hand revealed that detection of viremia under ongoing antiviral prophylaxis bears an increased risk for sustained viral replication and antiviral drug resistance in HR patients. We identified low estimated glomerular filtration rate (eGFR) and lower dose VGC prophylaxis post‐KTx as a risk factor for breakthrough infections in HR patients in our single center cohort. These patients might benefit from a closer CMV monitoring or novel prophylactic agents as letermovir.

Abstract:Background Cytomegalovirus (CMV) infection has broad implications for morbidity and mortality in renal transplant recipients (RTR). Routine surveillance for CMV replication with PCR‐based quantitative nucleic acid testing (qNAT) assays is standard practice in most transplant centers, but the impact of assay sensitivity on antiviral decision‐making and virologic outcomes has not been studied. We investigated the effects of an ultrasensitive CMV qNAT assay on multiple clinical outcomes, including time to detection and duration of CMV DNAemia. Methods We conducted a single‐center cohort study contrasting RTRs monitored with a qNAT with a higher lower limit of quantification (LLOQ >300 IU/mL) with those monitored with a more sensitive qNAT (LLOQ >35 IU/mL). Patients were stratified by donor (D)/recipient (R) CMV serostatus (D+/R−: high risk; any R+: moderate risk). CMV viral load monitoring was performed monthly post transplantation, with the primary outcomes being time to CMV DNAemia and its duration. Results Total 1382 patients were analyzed from 2014 to 2016 and 2019 to 2021. Moderate‐risk RTRs monitored with the more sensitive assay experienced a greater hazard for the development of a first episode of CMV DNAemia (aHR: 1.95, 95% confidence interval [CI]: 1.55–2.46) and an average of 24 (95% CI: 16.40–31.98) additional days of DNAemia. There was no difference in CMV end‐organ disease or 1‐year all‐cause mortality between moderate‐risk RTRs. Conclusions The more sensitive assay was associated with earlier detection and extended durations of CMV DNAemia in moderate‐risk RTRs, without altering clinical outcomes. These findings inform optimal use of these assays and antiviral stewardship in RTRs. Key Summary The use of ultrasensitive CMV qNAT assays in moderate‐risk CMV renal transplant recipients is associated with earlier detection and longer durations of CMV DNAemia without impacting CMV end‐organ disease or 1‐year mortality.

Abstract:BackgroundThere is no method of predicting human cytomegalovirus (HCMV) outcomes in allogeneic hematopoietic stem cell transplant recipients clinically, leading in some cases to excessive or insufficient antiviral therapy. We evaluated the early immune response of recipients with disparate HCMV outcomes.MethodsThe HCMV outcomes of recipients were determined by long-term monitoring of HCMV DNA levels posttransplant. HCMV IgG and IgM concentrations at 1 week before and 1 week after transplantation, absolute lymphocyte counts, and HCMV-specific IFN-γ secreting cells at 1 month posttransplant were evaluated based on HCMV outcome.ResultsAll recipients were negative for HCMV IgM. Significant differences between recipients with and without HCMV reactivation were observed in pre- and post-transplant HCMV IgG antibody levels, absolute lymphocyte counts, and HCMV-specific IFN-γ secreting cells (P < 0.05). HCMV IgG antibody levels significantly increased after transplantation in recipients with HCMV reactivation (P = 0.032), but not in those without reactivation. Multivariate analysis revealed that except for the absolute lymphocyte count these biomarkers were related to HCMV reactivation, independent of other clinical factors. In time-to-event analyses, lower levels of these biomarkers were associated with an increased 150-day cumulative incidence of HCMV reactivation (log-rank P < 0.05). In recipients with HCMV reactivation, the duration of HCMV DNAemia had negative correlation with HCMV-specific IFN-γ-secreting cells (P = 0.015, r = -0.372). The relationships between the peak HCMV DNA load and absolute lymphocyte count and HCMV-specific IFN-γ-secreting cells followed the same trends (P = 0.026, r = -0.181 and P = 0.010, r = -0.317).ConclusionsHCMV IgG, absolute lymphocyte count, and HCMV-specific IFN-γ secreting cells represent the humoral and cellular immune response. Early monitoring of these immune markers could enable prediction of HCMV outcomes posttransplant and assessment of the severity of HCMV DNAemia.
Impact of Multiple Episodes of Cytomegalovirus Infection on Patient Outcomes in Allogeneic Hematopoietic Stem Cell Transplant Using a Pre-emptive Monitoring Strategy

Abstract:The impact of multiple episodes of clinically significant cytomegalovirus infection (csCMVi) on clinical outcomes during pre-emptive CMV monitoring in allogeneic hematopoietic stem cell transplant recipients (HCT) is not well understood. We performed a retrospective cohort study of all consecutive patients undergoing their first allogeneic HCT at Duke between January 1, 2009 and December 31, 2013, during an era of pre-emptive CMV monitoring, prior to widespread use of letermovir prophylaxis. Consensus definitions were utilized for refractory and resistant CMV infection, bacteremia, invasive fungal disease, and renal dysfunction. The Kaplan-Meier method was used to estimate survival. Three hundred eighty-eight adult patients underwent allogeneic HCT during the study time period with 91 patients (23%) having one episode of csCMVi and 79 patients (20%) having ³ one episode of csCMVi. Risk factors for multiple episodes of csCMVi included having a transplant from a matched unrelated donor, cord blood as HCT source, and graft-versus-host disease prophylaxis with highly T-cell suppressive agents such as alemtuzumab. Patients with multiple episodes tended to develop csCMVi earlier post-transplant and were more likely to have CMV disease and to develop resistant CMV infection. There was no difference in rates of refractory CMV infection, bacteremia, invasive fungal disease, or survival. However, patients with multiple episodes of csCMV had reduced one-year relapse-related mortality compared to patients with one csCMVi episode.

Abstract:• Preemptive or prophylaxis (P) strategy in high‐risk liver transplant recipients: the same survival. • Preemptive strategy is associated with more cytomegalovirus infections. • Ganciclovir‐resistant infections are only seen with preemptive strategy. •The cost of preemptive strategy is by far higher than P 'cost. Objectives Prophylaxis (P) or pre‐emptive strategy (PS) in high‐risk liver transplant recipients (LTRs) are either recommended. We compared the results of each strategy. Methods Two groups of LTR transplanted during two consecutive periods were compared. Only cytomegalovirus (CMV)‐mismatched LTR (Donor +/ Recipient ‐) were included. The primary endpoints were: the onset of polymerase chain reaction‐based DNAemia and the proportion of patients with CMV disease. A number of episodes of CMV infection, antiviral therapy, ganciclovir resistance, infectious or immunological complications, cost of both strategies, and survival (1, 5, and 10 years) were also compared. Abstract:Cytomegalovirus (CMV) is one of the most common infections occurring after solid organ transplantation. This high burden of disease, which incurs sizeable morbidity, may be worsening with the proportion of high-risk D+/R- solid organ transplantation recipients increasing in some regions globally. Cohort studies continue to support either universal prophylaxis or preemptive therapy as effective prevention strategies. Letermovir prophylaxis was noninferior to valganciclovir in adult high-risk D+/R- kidney transplant recipients with fewer drug-related adverse events in a recent clinical trial and has now been approved for such use in some regions. Maribavir preemptive therapy failed to demonstrate noninferiority when compared with valganciclovir in hematopoietic stem cell transplant recipients but looked promising for safety. Donor matching could be useful in prevention CMV disease with a survival advantage demonstrated in seronegative recipients waiting up to 30 mo for a seronegative kidney. Immune-guided prophylaxis resulted in fewer CMV infection episodes in lung transplant recipients when compared with fixed-duration prophylaxis in a recent clinical trial. For treatment of refractory or resistant CMV infection, maribavir was more efficacious and better tolerated when compared with investigator-initiated therapy in its registration trial for this condition. Further research regarding best treatment and prophylaxis of resistant or refractory CMV infection is needed to reflect best clinical practice choices. Optimal use of immune globulin or CMV-specific T cells for prevention or treatment of CMV disease remains undefined. Standardized definitions for the design of CMV clinical trials have been developed. In this review, we highlight recent updates in the field from data published since 2018.
Survey of Post‐Prophylaxis Delayed‐Onset Cytomegalovirus Management Strategies Among Transplant Providers

Abstract:Background Preventive strategies for cytomegalovirus (CMV) in the posttransplant period have changed the pattern of CMV infections, now more commonly manifesting as late‐onset occurrences known as post‐prophylaxis delayed‐onset CMV disease (PPDOC). We conducted a survey to investigate provider practices in managing PPDOC. Methods A web‐based provider survey on the management of PPDOC was developed using Research Electronic Data Capture (REDCap). It was distributed to the online forums of the American Society of Transplantation communities of practice (COP) for Infectious Diseases (IDCOP), Kidney and Pancreas (KPCOP), Liver and Intestinal (LICOP), and Thoracic and Critical Care (TCCOP). The survey was posted twice within a span of a month. Results Fifty‐six respondents, comprising 50 (89%) transplant physicians and 6 (11%) transplant pharmacists, from 46 distinct transplant centers, completed the survey. Universal antiviral prophylaxis (UAP) was the predominant preventive approach for both high‐risk (85%) and moderate‐risk (85%) transplant recipients. Out of 56, 51 respondents completed the questions regarding management of PPDOC. Regular surveillance with nucleic acid amplification tests (NAAT) (88%) was the most commonly used approach in high‐risk recipients, while symptom monitoring (73%) was the most common strategy in moderate‐risk recipients. Immunologic monitoring was used only by a few respondents who found it moderately useful in high‐risk recipients. Conclusion Management of PPDOC was highly variable among providers and strategies differed based on patient risk profile. These findings could help shape future studies and guidelines to harmonize CMV management.

Abstract:Background: The goal was to determine trends in immunosuppression use and its impact on cytomegalovirus (CMV) outcomes over the past 10 years. Methods: This was a single-center longitudinal cohort study of adult kidney recipients transplanted between Jan 2012 and June 2021. Baseline and follow-up data were gathered via chart abstraction and analyzed using univariate and multivariate analyses. Results: Of 2392 kidney transplants conducted, 131 patients did not meet inclusion criteria. The mean age was 52 years, 41% were female, 57% were black, and 19% were CMV high-risk. The use of rabbit anti-thymocyte globulin (RATG) induction (odds ratio [OR] 1.6, 1.3-2.1), tacrolimus (FK) level >8 ng/mL (OR 1.1, 1.09-1.11), CMV D+/R- rates (OR 1.06, 1.02-1.10), white blood cell count <3000 (OR 1.22, 1.18-1.26) and valganciclovir prophylaxis (OR 1.7, 1.6-1.9) have significantly increased over the past 10 years. Rejection rates (OR 0.86, 0.82-0.91) and BK viremia >2000 (OR 0.91, 0.91-0.98) have decreased. RATG induction (adjusted hazard ratio [aHR] 1.35, 1.2-1.5), FK >8 ng/mL (aHR 3.5, 3.2-3.9), Belatacept conversion (aHR 2.5, 2.1-3.1), and rejection (aHR 1.8, 1.6-2.0) were significant risk factors for developing CMV infection, while mycophenolate mofetil <1500 mg (aHR 0.52, 0.47-0.59), mammalian target of rapamycin inhibitor (mTORi) conversion (0.77, 0.56-0.89), cyclosporine-A conversion (aHR 0.68, 0.56-0.84) were associated with lower risk of CMV infection. Conclusion: Increasing use of potent immunosuppression coupled with higher CMV D+/R- F rates may be driving higher rates of CMV infection. Cyclosporine and mTORi conversion appears to be protective against CMV. A more individualized immunosuppression regimen based on infection risk merits consideration.

Abstract:Abstract Cytomegalovirus (CMV) infection is still a problem for organ transplant recipients despite studies that long‐term prophylaxis with high dose of acyclovir or ganciclovir given to all organ recipients may limit the consequences of infection and disease. In the present report of 160 consecutive renal transplant patients, we used a diagnostic assay for CMV antigenemia (detection of CMV antigen in peripheral blood leukocytes) and treated with ganciclovir only those patients who had a positive test. No patient in this series had routine prophylaxis. Out of the 160 patients, 71 had clinical and/or laboratory signs of infection, and were tested for early antigen in peripheral leukocytes. The test was positive in 35, all of whom received a course of 3 wk ganciclovir treatment which effectively cured CMV in 34 out of 35. One patient was ganciclovir‐resistant, but responded to foscarnet. None of the 36 patients who had no early antigenemia and did not receive specific treatment developed the disease. The treatment was extremely well tolerated in all our patients with no adverse events. Thus, even though this was not a controlled study, our present results may be taken to indicate that long‐term acyclovir or ganciclovir for all organ transplant recipients might be no longer totally justified. We conclude that detecting viral antigen in circulating leukocytes identifies patients who are indeed at increased risk of developing severe CMV disease. When these patients are treated early enough, CMV is eliminated with a relatively short course of ganciclovir, which has virtually no side effects.

Abstract:Background There is variability in recommended viral monitoring protocols after kidney transplant. In response to increased demand for laboratory testing during the COVID‐19 pandemic, the Transplant Manitoba Adult Kidney Program updated its monitoring protocols for cytomegalovirus (CMV), Epstein‐Barr virus (EBV), and BK polyomavirus (BKV) to a reduced frequency. Methods This single‐center nested case‐control study evaluated 252 adult kidney transplant recipients transplanted from 2015 to 2021, with the updated protocols effective on March 19th 2020. Cases included recipients transplanted after the protocol update who developed CMV, EBV, and BKV DNAemia and were matched to controls with DNAemia transplanted prior to the protocol update. The primary outcome was the difference in maximum DNA load titers between cases and matched controls. Secondary outcomes included time to initial DNAemia detection and DNAemia clearance. Safety outcomes of tissue‐invasive viral disease were described. Results There were 216 recipients transplanted preupdate and 36 recipients postupdate. There was no difference between cases and controls in maximum or first DNA load titers for EBV, CMV, or BKV. Cases experienced earlier EBV DNAemia detection (26 (IQR 8, 32) vs. 434 (IQR 96, 1184) days, p = .005).
