Abstract:Human cytomegalovirus (CMV)-specific cytotoxic T lymphocyte (CTL) immune response must be reconstituted for long-term protection against CMV relapse and disease in hematopoietic stem cell transplantation (HSCT) recipients. We phenotypically quantitated absolute numbers of CMV-pp65 peptide-specific CTLs (CTL(CMV)) in 50 related donor unmanipulated allografts infused into HLA-matched or -mismatched recipients and examined the incidence of CMV reactivation. High CTL(CMV) with terminally differentiated effector CD45RO(-)CD62L(-) cell (T(EMRA)) phenotype in the allografts were associated with reduced risk of CMV reactivation, in the presence of sufficient CD45RO(+)CD62L(-) cell (T(EM)) infusion (>/=0.208 x 10(6)/kg). Early after transplantation, there was significant expansion of CTL(CMV) with the central memory CD45RO(+)CD62L(+) cell (T(CM)) phenotype when CMV was reactivated. The frequencies of CTL(CMV) T(Naive) (CD45RO(-)CD62L(+)), T(CM), and T(EM) at day 90 posttransplantation and of CTL(CMV) T(EMRA) at day 60 posttransplantation were greater in recipients with higher infusions of CTL(CMV) T(EMRA), suggesting protective immunity transferred by infusion of CTL(CMV) within allografts. Moreover, the majority of the CTL(CMV) identified in the recipients early after HSCT was of donor origin. Our findings support that measuring levels of CTL(CMV) and its subsets in the donor grafts and manipulating these cells early after transplantation may help control CMV reactivation, which is closely correlated with immune reconstitution and differentiation of CTL(CMV) subsets.

Abstract:Cytomegalovirus belonged to herpes virus group , which could be carried by health people.The patients with transplantation often suffered from the deficiency of innate and adaptive immune.Therefore, cytomegalovirus relative diseases were extremely common after transplantation , especially in (D+R-/D-R+) group.The clinical outcomes fluctuated between mild asymptomatic infection and multiple organ dysfunction syndromes .The benefit of prophylactic anti-virus drugs should be measured before transplantation for their own limitations ( adverse side effects and financial burdens).In the past 10 years, many single nucleotide polymorphisms in human genes had been analyzed to explore their relationships with cytomegalovirus infection , reactivation and tissue relative diseases, but the results were not consistent .Our study tried to illustrate and classify those associations by genes and transplantations .It would help to understand the progress of current researches and show the potential direction in the future .

Abstract:Abstract Abstract 3054 Background: Disease relapse, major infection and graft-versus-host disease (GVHD) are the leading causes of death following allogeneic hematopoietic stem cell transplantation (allo-HSCT). Donor T lymphocytes play a critical role in alloimmune recognition and their ability to detect non-self-antigens can lead to GVHD or contribute to prevention of relapse and infection. Three effector pathways have been described for T-cell cytotoxicity: granzyme B/perforin, Fas/Fas ligand (FasL) and secreted molecules such as tumor necrosis factor (TNF)-alpha. Granzyme B is a proapoptotic serine protease. Fas is widely expressed in immunocompetent cells and non-lymphoid tissues. FasL is mainly expressed on activated cytotoxic T lymphocytes (CTLs). When CTLs recognize target cells, they become activated and express FasL which induces apoptosis of target cells. The Fas gene has been found to be silenced in many tumor types. The importance of those effector pathways for the development of acute GVHD (aGVHD) has been well established. In our previous study, we have reported donor and/or recipient TNF-alpha gene polymorphism influenced on the risk of aGVHD. Recently several important polymorphisms have been identified in granzyme B, Fas, and FasL genes and reported to be relevant to allograft rejection in solid organ transplant patients. However, such information is less available in allo-HSCT. This present study was designed to investigate the influence of donor and recipient genetic variations of effector pathway molecules on the abilities of T-cell alloimmune responses including GVHD, cytomegalovirus (CMV) infection within 100 days and disease relapse following allo-HSCT. Methods: The entire study population consisted of 138 pairs of recipients and their unrelated donors, who underwent HSCT in our Center from January 2001 to March 2009. Genomic DNA was extracted from peripheral blood samples obtained from recipients and donors before transplantation. Five single nucleotide polymorphisms (SNPs) including granzyme B +55(A/G), -295(C/T), Fas -1377(G/A), -670(T/C) and FasL -844 (T/C) were analyzed. Results: (1) Patients receiving stem cells from a donor with granzyme B +55 AA genotype developed aGVHD more frequently than those with mutated allele (AG or GG), which has been reported to be associated with low-producing of granzyme B by CTLs and incapable of inducing apoptosis. Multivariate analysis confirmed that donor with the granzyme B +55 AA genotype was an independent risk factor for aGVHD (RR=1.545, 95%CI: 1.012–2.357, P=0.044). (2) Donor with FasL -844 TT genotype, a genotype reported to a reduction in the expression of FasL on T cells, conferred protection against the risk of aGVHD (RR=2.981, 95% CI: 1.320–6.736, P=0.009), however was an independent risk factor for early CMV infection (RR: 3.658, 95% CI: 1.534–8.722, P=0.003). Patients experiencing aGVHD have a higher risk of developing early CMV infection (RR=1.931, 95% CI: 1.242–3.003, P=0.003), early CMV infection also was a significant factor impacting aGVHD (RR=1.630, 95%CI: 1.294–2.053, P<0.001). (3) Recipient with Fas -670 C- allele positive, a mutated allele with higher Fas expression, was associated with a significantly lower risk of relapse (RR=3.453, 95% CI: 1.699–7.091, P=0.001) as well as superior overall survival (84.2% vs 21.2%, P<0.001) in patients with acute myeloid leukemia (AML), but not in patients with other myelogenous or lymphoid malignancies. The polymorphic sites granzyme B -295(C/T), Fas -1377(G/A), did not correlate with the T-cell alloimmune reactivities. Conclusions: Genetic variation, other than HLA polymorphism, regulating the immune response may influence outcomes of patients undergoing allo-HSCT. Transplant patients would thus benefit from accurate risk assessment and individualization of immunosuppressive therapy, taking into account donor and recipient genetic data, as well as other risk factors. This study is the first report of relationship between donor or recipient genetic variations of effector pathway molecules and the abilities of T-cell alloimmune responses. According to our results, special targeting granzyme B and Fas/FasL pathways may be a promising therapeutic strategy in patients with aGVHD, CMV infection and leukemia relapse after allo-HSCT at least in AML patients harboring Fas -670 wild-type genotype. Association of Cytomegalovirus Infection with Human Leukocyte Antigen Genotypes in Recipients after Allogeneic Liver Transplantation.

Abstract:BACKGROUND:Cytomegalovirus (CMV) infection is the important cause affecting the survival rate and function of the transplanted organ after transplantation. The occurrence of CMV infection after liver transplantation (LT) is associated with many factors. Lots of studies suggest that genetic mutation between hosts and CMV may play a role in the occurrence and development of CMV infection. CMV exists in an incubative state, affect or destroy the expression of human leukocyte antigen (HLA) molecules in the host cell surface, and interfere antigen's submission. This mechanism is the key of CMV to avoid immune defense mechanism of the host. To detect HLA and CMV antibody (CMV-Ab), CMV antigen (CMV-Ag) of transplantation recipients, we evaluated the association of CMV infection and the particular HLA genotypes in recipients after LT.METHODS:277 blood samples were collected from 39 LT recipients. CMV antibody and antigen were detected by ELISA or immunohistochemical methods. The HLA types of the recipients were determined by PCR. To analyze the association of HLA alleles and the occurrence of CMV antigenemia in the patients, relative risk degree (RR) was used as the parameter for the Chi-square test.RESULTS:The LT recipients were serum CMV IgG positive (100%), but none of them was CMV IgM positive (0%). Thirty-three LT recipients (84.6%) were CMV antigenic positive with 1-50 positive leukocytes per 50,000 leukocytes in extent and 7.2+/-4.2 positive leukocytes per 50,000 leukocytes on average. Thirteen patients developed CMV pneumonia, with CMV antigenic positive (100%) and 17.7+/-5.5 positive leukocytes per 50,000 leukocytes on average. Some HLA alleles were associated with the occurrence and extent of CMV antigenemia. HLA-A2 was the higher frequency allele for patients with antigenemia (P<0.05), and 7 patients carrying HLA-DR11 allele developed antigenemia (P<0.05). In the lower antigenemia group, HLA-A11 was higher in frequency than others (P<0.05). Besides, none of the patients carrying HLA-B16 allele developed clinical symptoms of CMV infection (P<0.05).CONCLUSIONS:The variability of HLA alleles might modulate immune response to CMV infection. HLA examination before transplantation should be made for prevention and treatment of CMV infection after operation.
Multicenter Analysis of Valganciclovir Prophylaxis in Pediatric Solid Organ Transplant Recipients

Abstract:Background: Valganciclovir is the only approved antiviral for cytomegalovirus (CMV) prevention in pediatric solid organ transplantation (SOT). Additional approaches may be needed to improve outcomes. Methods: A multicenter retrospective study from 2016 to 2019 was conducted of pediatric SOT recipients in whom at least 3 months of valganciclovir prophylaxis was planned. Episodes of CMV DNA in blood (DNAemia), CMV disease, drug-related toxicities, as well as other infections in the first year posttransplant and demographic and clinical data were collected. CMV DNAemia in the first year after prophylaxis or during prophylaxis (breakthrough) was analyzed by multivariate hazard models. Results: Among the 749 patients enrolled, 131 (17.5%) had CMV DNAemia at any time in the first year; 85 (11.4%) had breakthrough DNAemia, and 46 (6.1%) had DNAemia after prophylaxis. CMV disease occurred in 30 (4%). In a multivariate model, liver transplantation compared to kidney or heart, intermediate or high risk based on donor/recipient serologies, neutropenia, and valganciclovir dose modifications attributed to toxicity were associated with increased risk of total and/or breakthrough DNAemia. Bacteremia was also associated with increased hazard ratio for CMV DNAemia. In a separate multivariate analysis, rejection occurred more often in those with breakthrough CMV DNAemia (P = .002); liver transplants, specifically, had increased rejection if CMV DNAemia occurred in the first year (P = .004). These associations may be bidirectional as rejection may contribute to infection risk. Conclusions: CMV DNAemia in the first year posttransplantation occurs despite valganciclovir prophylaxis and is associated with medication toxicity, bacteremia, and rejection. Pediatric studies of newer antivirals, especially in higher-risk subpopulations, appear to be warranted.

Abstract:Cytomegalovirus (CMV) is one of the most common infections occurring after solid organ transplantation. This high burden of disease, which incurs sizeable morbidity, may be worsening with the proportion of high-risk D+/R- solid organ transplantation recipients increasing in some regions globally. Cohort studies continue to support either universal prophylaxis or preemptive therapy as effective prevention strategies. Letermovir prophylaxis was noninferior to valganciclovir in adult high-risk D+/R- kidney transplant recipients with fewer drug-related adverse events in a recent clinical trial and has now been approved for such use in some regions. Maribavir preemptive therapy failed to demonstrate noninferiority when compared with valganciclovir in hematopoietic stem cell transplant recipients but looked promising for safety. Donor matching could be useful in prevention CMV disease with a survival advantage demonstrated in seronegative recipients waiting up to 30 mo for a seronegative kidney. Immune-guided prophylaxis resulted in fewer CMV infection episodes in lung transplant recipients when compared with fixed-duration prophylaxis in a recent clinical trial. For treatment of refractory or resistant CMV infection, maribavir was more efficacious and better tolerated when compared with investigator-initiated therapy in its registration trial for this condition. Further research regarding best treatment and prophylaxis of resistant or refractory CMV infection is needed to reflect best clinical practice choices. Optimal use of immune globulin or CMV-specific T cells for prevention or treatment of CMV disease remains undefined. Standardized definitions for the design of CMV clinical trials have been developed. In this review, we highlight recent updates in the field from data published since 2018.
Impact and outcomes of primary cytomegalovirus disease in seronegative abdominal solid organ transplant recipients of cytomegalovirus unexposed donors (D‐/R‐)

Abstract:<section class="article-section__content"><h3 class="article-section__sub-title section1"> Background</h3><p>Primary cytomegalovirus (CMV) disease in high risk (D+/R‐) abdominal solid organ transplant recipients (aSOTRs) is well described, however little is known of primary CMV disease in low risk (D‐/R‐) patients.</p></section><section class="article-section__content"><h3 class="article-section__sub-title section1"> Methods</h3><p>Observational study of adult aSOTRs between 1/1/2009‐9/1/2019 screened based on serostatus at transplant; D‐/R‐ and D+/R‐ patients were included. Primary objective: Describe epidemiology of primary CMV in D‐/R‐ aSOTRs. Secondary objective: compare infectious and transplant related outcomes of primary CMV disease in the first 90 days (early CMV) between D‐/R‐ and D+/R‐.</p></section><section class="article-section__content"><h3 class="article-section__sub-title section1"> Results</h3><p>Of 782 D‐/R‐ aSOTRs in the study period, 13 developed CMV at any time after transplant to last follow‐up. Of 671 D+/R‐ patients, 186 developed CMV. Early CMV disease was significantly more common in the D‐/R‐ group (54% vs 15.6%, p=0.0005) despite populations being similar demographically, including allograft subtype. D‐/R‐ patients with early CMV disease had median viral load >100,000 IU/mL and 42.9% had end‐organ manifestations; 71.4% required hospital admission. Immunosuppressive therapy was adjusted in 100% of patients, there was an approximately 14.3% rate of antiviral resistance and 28.6% had concomitant opportunistic infection. These findings were similar to D+/R‐ patients. There was no difference in risk of rejection or all‐cause mortality associated with early CMV disease, however graft loss was significantly higher in D‐/R‐</p></section><section class="article-section__content"><h3 class="article-section__sub-title section1"> Conclusion</h3><p>D‐/R‐ aSOTRs infrequently develop CMV, however when it occurs, they present with disease manifestations similar to and graft outcomes inferior to D+/R‐ with CMV. Additionally, the majority of CMV disease in D‐/R‐ occurs in the first 90 days after transplant, suggesting possible donor subclinical infection or transfusion source. The complicated course in D‐/R‐ is likely due to low clinical suspicion. Awareness of disease severity and aggressive upfront management may promote positive outcomes.</p></section>

Abstract:Introduction Little is known of the impact of multiple viral infections on allograft injury and impact on donor-derived-cell-free DNA (dd-cfDNA) levels. Case report A 60-year-old male who received an orthotropic heart transplant two months prior presented to the emergency department with a worsening headache, fatigue and cough. Baseline immunosuppression consisted of tacrolimus (goal 10-12 ng/mL), mycophenolate (500 mg BID) and prednisone taper; the patient was compliant with his immunosuppression regimen. He was found to have tested positive for Parvovirus (IgM + and PCR +) and Epstein-Barr virus (543 IU/mL) requiring prolonged treatment with Intravenous Immunoglobulin (IVIG). During his one-month follow-up, he tested positive for Cytomegalovirus (PCR + <137 IU/mL) as well, that did not warrant treatment; however, mycophenolate was put on hold. In addition, he developed new Class 2 antibodies DR53 (MFI 1000) and DQ2 (MFI 6000) and the Allosure dd-cfDNA increased to 0.42%. (Figure) Graft function on echocardiogram was preserved and his biopsy was negative for T-cell and antibody mediated rejection. At time of discharge, the patient remained on routine medication and resumed at a lower dose mycophenolate (250 mg BID ). Additionally, upon completion of his treatment as an outpatient with IVIG for Parvovirus, his symptoms resolved but his test remained positive for Parvovirus. His dd-cfDNA score improved to 0.19%. Repeat heart biopsy revealed moderate T-cell mediated rejection (2R, pAMR0) which resolved with addition of outpatient increased pulse steroids. The presence of viral infections can cause allograft injury resulting in elevated dd-cfDNA levels, new donor specific antibodies and rejection. Further research is needed to validate these preliminary findings.

Factors Associated With Genotypic Resistance and Outcome Among Solid Organ Transplant Recipients With Refractory Cytomegalovirus Infection

Abstract:Genotypically resistant cytomegalovirus (CMV) infection is associated with increased morbi-mortality. We herein aimed at understanding the factors that predict CMV genotypic resistance in refractory infections and disease in the SOTR (Solid Organ Transplant Recipients) population, and the factors associated with outcomes. We included all SOTRs who were tested for CMV genotypic resistance for CMV refractory infection/disease over ten years in two centers. Eighty-one refractory patients were included, 26 with genotypically resistant infections (32%). Twenty-four of these genotypic profiles conferred resistance to ganciclovir (GCV) and 2 to GCV and cidofovir. Twenty-three patients presented a high level of GCV resistance. We found no resistance mutation to letermovir. Age (OR = 0.94 per year, IC95 [0.089-0.99]), a history of valganciclovir (VGCV) underdosing or of low plasma concentration (OR= 5.6, IC95 [1.69-20.7]), being on VGCV at infection onset (OR = 3.11, IC95 [1.18-5.32]) and the recipients' CMV negative serostatus (OR = 3.40, IC95 [0.97-12.8]) were independently associated with CMV genotypic resistance. One year mortality was higher in the resistant CMV group (19.2 % versus 3.6 %, p = 0.02). Antiviral drugs severe adverse effects were also independently associated with CMV genotypic resistance. CMV genotypic resistance to antivirals was independently associated with a younger age, exposure to low levels of GCV, the recipients' negative serostatus, and presenting the infection on VGCV prophylaxis. This data is of importance, given that we also found a poorer outcome in the patients of the resistant group.
Effect of CMV Mismatch on Heart Transplant Outcomes Using a Surveillance and Preemptive Strategy

Abstract:Purpose: The aim of the study was to determine outcomes after heart transplantation for cytomegalovirus (CMV) mismatched patients (D+/R-) who underwent a surveillance and preemptive therapy protocol, compared to nonmismatch patients. Methods: A review of patient records from January 2010 to December 2020 with follow-up to October 2023 was done. The protocol consisted weekly surveillance with CMV PCR starting 4 weeks after transplant continuing up until the patient seroconverts or up to 3 months posttransplant if the patient does not seroconvert. Valganciclovir was given for 2 weeks to those who seroconverted. Results: Two hundred and twenty-one patients were included, and 23% were mismatched patients. Overall survival was not different between CMV groups (p = NS). Causes of death and morbidities were also not significantly different (p = NS). Sixty-six percent of mismatch patients seroconverted, and there was also a significantly older donor age in the seroconverted patients compared to nonseroconverted patients (41 ± 11 vs. 29 ± 12 years, p < 0.005), indicating a higher risk donor profile. A multivariate Cox regression including donor age showed that there was no increase in mortality in the seroconverted mismatches compared to nonmismatch patients (p = NS). Conclusions: There is no significant increased mortality or morbidity using a CMV surveillance and preemptive therapy protocol. The effect of donor age on seroconversion of mismatches requires further validation.

Abstract:Importance: Cytomegalovirus (CMV) is associated with significant morbidity and mortality in solid organ transplant (SOT) recipients. The risk factors for CMV seropositivity in SOT recipients, including area-level social deprivation in the US, have not been fully characterized. Objective: To (1) evaluate CMV seroprevalence, (2) assess the recipient characteristics associated with CMV seropositivity, and (3) assess the association of area-level social deprivation index (SDI) scores with pretransplant CMV serostatus. Design, setting, and participants: This retrospective cross-sectional analysis of the Scientific Registry of Transplant Recipients database included all adult (aged ≥18 years) SOT recipients from January 1, 2008, to May 31, 2022. Data were analyzed from April 10 to October 25, 2023. Exposure: Recipient characteristics and area-level SDI. Main outcomes and measures: Multivariable generalized linear models were used to evaluate the association between (1) patient characteristics and CMV and (2) social deprivation (measured by SDI scores, which were assessed in quintiles, from lowest to highest) and CMV seropositivity. In addition, differences based on patient demographics and the transplanted organ(s) were evaluated. Results: Among the 389 288 SOT recipients included in the analysis, mean (SD) age was 53.3 (13.0) years; 63.0% were male, 21.4% were Black, 15.2% were Hispanic White, 56.2% were non-Hispanic White, and 62.7% were CMV seropositive. The mean (SD) age was higher among CMV seropositive (54.0 [12.7] years) compared with CMV seronegative (52.0 [13.5] years) patients. Seropositivity for CMV was higher among women (69.9%) than men (58.5%) and among Black (74.8%) and Hispanic White (80.2%) patients compared with non-Hispanic White patients (50.4%). Seropositivity for CMV was highest among kidney (64.5%), liver (63.6%), and kidney and liver (66.2%) recipients. Greater SDI scores were associated with greater CMV seropositivity, ranging from 51.7% for the least deprived to 75.5% for the most deprived quintiles (P < .001), independent of age, sex, or race. Conclusions and relevance: In this cross-sectional study, an association between SDI and CMV seropositivity was observed among SOT recipients, independent of age, sex, or race and ethnicity. To optimize posttransplant outcomes in CMV seropositive recipients, efforts targeting prevention of CMV reactivation need to be prioritized in these higher-risk populations.
Bidirectional transfer of human cytomegalovirus strains in donor and recipient seropositive lung transplant patients

Abstract:Donor and recipient human cytomegalovirus (HCMV) seropositive (D+R+) lung transplant recipients (LTRs) often harbor multiple strains of HCMV, likely due to transmitted donor (D) strains and reactivated recipient (R) strains. To date, the extent and timely occurrence of each likely source in shaping the post-transplantation (post-Tx) strain population is unknown. Here, we deciphered the D and R origin of the post-Tx HCMV strain composition in blood, bronchoalveolar lavage (BAL), and CD45+ BAL cell subsets. We investigated either D and/or R formalin-fixed paraffin-embedded blocks or fresh D lung tissue from four D+R+ LTRs obtained before transplantation. HCMV strains were characterized by short amplicon deep sequencing. In two LTRs, we show that the transplanted lung is reseeded by R strains within the first 6 months after transplantation, likely by infiltrating CD14+ CD163+/- alveolar macrophages. In three LTRs, we demonstrate both rapid D-strain dissemination and persistence in the transplanted lung for >1 year post-Tx. Broad inter-host diversity contrasts with intra-host genotype sequence stability upon transmission, during follow-up and across compartments. In D+R+ LTRs, HCMV strains of both, D and R origin can emerge first and dominate long-term in subsequent episodes of infection, indicating replication of both sources despite pre-existing immunity.

Abstract:Human cytomegalovirus (HCMV) infection may be asymptomatic in healthy individuals but can cause severe complications in immunocompromised patients, including transplant recipients. Breakthrough and drug-resistant HCMV infections in such patients are major concerns. Clinicians are first challenged to accurately diagnose HCMV infection and then to identify the most effective antiviral drug and determine when to initiate therapy, alter drug dosage, or switch medication. This review critically examines HCMV diagnostics approaches, particularly for immunocompromised patients, and the development of genotypic techniques to rapidly diagnose drug resistance mutations. The current standard method to identify prevalent and well-known resistance mutations involves polymerase chain reaction amplification of UL97, UL54, and UL56 gene regions, followed by Sanger sequencing. This method can confirm clinical suspicion of drug resistance as well as determine the level of drug resistance and range of cross-resistance with other drugs. Despite the effectiveness of this approach, there remains an urgent need for more rapid and point-of-care HCMV diagnosis, allowing for timely lifesaving intervention.

Abstract:BACKGROUND: Cytomegalovirus (CMV), the most common opportunistic infection of kidney transplantation (KT), is preventable by prophylactic and preemptive antiviral drugs in CMV-immunoglobulin (Ig)G-positive donors. Our preemptive therapy optimized immunosuppressive doses based on mixed lymphocyte response (MLR) results, regardless of preoperative CMV-IgG serostatus pairing. This study used the MLR to compare the anti-donor T-cell responses between CMV antigenemia-positive and -negative cases.METHODS: One hundred patients underwent KT using a cyclosporine (CsA)-based immunosuppressive regimen at Hiroshima University Hospital. CMV antigenemia-positive cells were defined as 4/50,000 CMVpp65-positive cells. T-cell responses to allo-antigens were measured using MLR assays to evaluate patients' anti-donor immune reactivity. After analyzing the proliferation of CD4<sup>+</sup> and CD8<sup>+</sup> T-cell subsets, the stimulation indices of CD4<sup>+</sup> or CD8<sup>+</sup> T cells were quantified. The study used no prisoners, and the participants were neither coerced nor paid. The manuscript was created in compliance with the Helsinki Congress and the Declaration of Istanbul.RESULTS: Forty-three patients tested positive for CMV antigenemia within 3 months after KT. No significant differences were found between the CMV antigenemia-positive and -negative groups in the stimulation indices for CD4<sup>+</sup> and CD8<sup>+</sup> T-cell responses to anti-donor stimulation. However, T-cell responses to third-party stimuli during the postoperative month 1 were significantly less in the CMV antigenemia-positive than -negative group.CONCLUSION: Anti-donor T-cell responses are not necessarily attenuated during CMV infection in KT recipients. In CMV-infected KT recipients, caution should be exercised against inadvertent dose reduction of immunosuppressants.

Abstract:In allogeneic hematopoietic stem cell transplantation recipients, cytomegalovirus (CMV) infection can cause overt CMV-associated disease, which is a main cause of transplantation-associated mortality. CMV infection correlates closely with donor's type. We therefore examined whether risk factors of CMV reactivation and clinical endpoints in patients with hematologic malignancies after allogeneic peripheral blood stem cell transplantation (PBSCT) differed between using matched-sibling donors (MSD-SCT) and haploidentical donors (HID-SCT). In this retrospective cohort study, we enrolled in 200 consecutive patients received an unmanipulated G-CSF-mobilized allogeneic PBSCT. Ninety (45%) patients received MSD-SCT and 110 (55%) received HID-SCT. Quantitative PCR was used for monitoring of CMV reactivation after transplantation. One-year cumulative incidence of CMV DNAemia was 55.0%, ranging from 23.5% in MSD-SCT group to 81.0% in HID-SCT group (p < 0.001). Although univariate analyses showed that non-myeloid malignancies, disease in complete remission status at transplantation, pretreatment with antithymocyte globulin, HLA-haploidentical donors, male donors, previous Epstein-Barr virus DNAemia, and absolute lymphocyte count on day 30 < 0.6 × 109/L were respectively associated with CMV reactivation after transplantation in total cohort of recipients (all p < 0.05), haploidentical donors were found to be the only independent predictor in multivariate analyses (Hazard ratio = 6.4, p < 0.001). Furthermore, univariate analyses revealed that non-myeloid malignancies and previous Epstein-Barr virus DNAemia were respectively associated with CMV reactivation in MSD-SCT recipients, and female was associated with CMV reactivation in HID-SCT recipients (all p < 0.05). In HID-SCT recipients, but not MSD-SCT recipients, previous CMV DNAemia was associated with a lower cumulative incidence of acute graft-versus-host disease (49.2% vs. 72.6%, p < 0.001). CMV DNAemia did not play a role in the relapse rate, but it was strongly associated with an increased risk of non-relapse mortality either in total cohort of recipients (30.5% vs. 13.7%; p = 0.003) or in the HID-SCT subgroup (36.0% vs. 16.7%; p = 0.030). Relapse-free survival and overall survival in total cohort of recipients with CMV DNAemia were both inferior to those without CMV DNAemia (45.3% vs. 57.6% and 54.8% vs. 65.8%, respectively; both p < 0.05). However, in subgroup analysis according to donor's type, neither relapse-free survival nor overall survival was impacted by CMV status (both p > 0.05). This study addressed differences in incidence, risk factors, and associations with clinical outcomes of CMV reactivation after haploidentical versus matched-sibling PBSCT.
Specific Immunity to Cytomegalovirus in Pediatric Cardiac Transplantation

Abstract:Background: Cytomegalovirus (CMV) infection is implicated in endothelial dysfunction and graft damage after pediatric heart transplantation. CMV-specific immune responses are thought to be necessary for CMV viral control but there is little data in pediatric heart transplantation. Methods: We studied 28 consecutive pediatric heart transplant recipients for 1 year posttransplant. CMV T-cell expressing IFN-γ, TNF-α, and IL-2 in response to ex vivo stimulation with CMV lysates or peptides were measured. Circulating cytokines were measured in plasma. Generalized Additive Models were applied to the data to model changes of cell population dynamics over time. Results: CMV-specific T cell-mediated responses were impaired in the first 8 weeks posttransplant. During this period, 25% of patients had CMV viremia, of which those with VLs of 10 000 or more CMV deoxyribonucleic acid copies/mL were given ganciclovir. In this group, the frequency of CD4+ and CD8+ T cells producing IFN-γ and the CD8+CD57+ granzyme B+ T-cell population increased at 12 to 24 weeks and remained elevated for the duration of the study. Conclusions: We have shown that CMV viremia is associated with CMV-specific immune responses and increased CD8+CD57+ granzyme B+ cells at 1 year posttransplant; however, early responses were not predictive of impending CMV viremia. It remains to be seen if the early CMV immune response detected is associated with endothelial and allograft damage, in light of previous studies demonstrating increased vasculopathy in pediatric patients with CMV viremia.

Abstract:Transplantation tolerance is achieved when recipients are unresponsive to donor alloantigen yet mobilize against third-party antigens, including virus. After transplantation, cytomegalovirus (CMV) reactivation in latently-infected transplants reduces allograft viability. To determine if pre-tolerized recipients are resistant to viral dissemination in this setting, we transfused chemically-fixed donor splenocytes (1-ethyl-3- (3′-dimethyl-aminopropyl)-carbo-diimide (ECDI)-treated splenocytes (ECDIsp)) to induce donor antigen tolerance without immunosuppression. In parallel, we implanted donor islet cells to validate operational tolerance. These pre-tolerized recipients were implanted with murine CMV (MCMV) latently-infected donor kidneys (a validated model of CMV latency) to monitor graft inflammation and viral dissemination. Our results indicate that tolerance to donor islets was sustained in recipients after implantation of donor kidneys. In addition, kidney allografts implanted after ECDIsp and islet implantation exhibited low levels of fibrosis and tubulitis. In contrast, kidney cellular and innate immune infiltrates trended higher in the CMV group and exhibited increased markers of CD8+ T cell activation. Tolerance induction was unable to prevent increases in MCMV-specific CD8+ T cells or dissemination of viral IE-1 DNA. Our data suggest that latently-infected allografts are inherently more susceptible to inflammation that is associated with viral dissemination in pre-tolerized recipients. Thus, CMV latently-infected allografts require enhanced strategies to protect allograft integrity and viral spread.
