Abstract:Successful reactive balance control requires coordinated modulation of hip, knee, and ankle torques. Stabilizing joint torques arise from feedforward neural signals that modulate the musculoskeletal system's intrinsic mechanical properties, namely muscle short-range stiffness, and neural feedback pathways that activate muscles in response to sensory input. Although feedforward and feedback pathways are known to modulate the torque at each joint, the role of each pathway to the balance-correcting response across joints is poorly understood. Since the feedforward and feedback torque responses act at different delays following perturbations to balance, we modified the sensorimotor response model (SRM), previously used to analyze the muscle activation response to perturbations, to consist of parallel feedback loops with different delays. Each loop within the model is driven by the same information, center of mass (CoM) kinematics, but each loop has an independent delay. We evaluated if a parallel loop SRM could decompose the reactive torques into the feedforward and feedback contributions during balance-correcting responses to backward support surface translations at four magnitudes. The SRM accurately reconstructed reactive joint torques at the hip, knee, and ankle, across all perturbation magnitudes (R2>0.84 & VAF>0.83). Moreover, the hip and knee exhibited feedforward and feedback components, while the ankle only exhibited feedback components. The lack of a feedforward component at the ankle may occur because the compliance of the Achilles tendon attenuates muscle short-range stiffness. Our model may provide a framework for evaluating changes in the feedforward and feedback contributions to balance that occur due to aging, injury, or disease.

Abstract:The role of the cortex in shaping automatic whole-body motor behaviors such as walking and balance is poorly understood. Gait and balance are typically mediated through subcortical circuits, with the cortex becoming engaged as needed on an individual basis by task difficulty and complexity. However, we lack a mechanistic understanding of how increased cortical contribution to whole-body movements shapes motor output. Here we use reactive balance recovery as a paradigm to identify relationships between hierarchical control mechanisms and their engagement across balance tasks of increasing difficulty in young adults. We hypothesize that parallel sensorimotor feedback loops engaging subcortical and cortical circuits contribute to balance-correcting muscle activity, and that the involvement of cortical circuits increases with balance challenge. We decomposed balance-correcting muscle activity based on hypothesized subcortically- and cortically-mediated feedback components driven by similar sensory information, but with different loop delays. The initial balance-correcting muscle activity was engaged at all levels of balance difficulty. Its onset latency was consistent with subcortical sensorimotor loops observed in the lower limb. An even later, presumed, cortically-mediated burst of muscle activity became additionally engaged as balance task difficulty increased, at latencies consistent with longer transcortical sensorimotor loops. We further demonstrate that evoked cortical activity in central midline areas measured using electroencephalography (EEG) can be explained by a similar sensory transformation as muscle activity but at a delay consistent with its role in a transcortical loop driving later cortical contributions to balance-correcting muscle activity. These results demonstrate that a neuromechanical model of muscle activity can be used to infer cortical contributions to muscle activity without recording brain activity. Our model may provide a useful framework for evaluating changes in cortical contributions to balance that are associated with falls in older adults and in neurological disorders such as Parkinson's disease. Our goal is to gain a mechanistic understanding of how cortical and subcortical systems shape motor output in whole-body movements. Using novel neuromechanical models, we identify how cortical and subcortical control mechanisms differentially contribute to balance-correcting muscle activity across reactive balance task difficulty. Bursts of balance-correcting muscle activity in young adults were decomposed into parallel sensorimotor feedback components driven by balance error but acting at different loop latencies. Corroborating the transcortical nature of the longer latency loop, we showed that evoked cortical responses to perturbations were also driven by balance error. While the contributions of the faster subcortical loop to muscle activity was present in all perturbation levels, the slower cortically-mediated burst of muscle was increasingly engaged with difficulty level. Our computational framework thus dissociates cortical and subcortical muscle activity components of balance control which may be useful to inform balance training and rehabilitation.

Abstract:Exploring the torque modulation mechanisms of human joints is critical for analyzing the human balance control system and developing natural human-machine interactions for balance support. However, the knee joint is often overlooked in biomechanical models because of its limited range of motion during balance recovery. This poses a challenge in establishing mathematical models for the knee joint's torque modulation mechanisms using computer simulations based on the inverted pendulum model. This study aims to provide a simplified linear feedback model inspired by sensorimotor transformation theory to reveal the torque modulation mechanism of the knee joint. The model was validated using data from experiments involving support-surface translation perturbations. The goodness-of-fit metrics of the model, including values and root mean square errors (RMSE), demonstrated strong explanatory power ( ranged from 0.77 to 0.90) and low error (RMSE ranging from 0.035 to 0.072) across different perturbation magnitudes and directions. Through pooling samples across various perturbation conditions and conducting multiple fits, this model revealed that knee torque is modulated using a direction-specific strategy with adaptable feedback gains. These results suggest that the proposed simplified linear model can be used to develop assistive systems and retrieve insights on balance recovery mechanisms.

Reactive Balance Control in Response to Perturbation in Unilateral Stance: Interaction Effects of Direction, Displacement and Velocity on Compensatory Neuromuscular and Kinematic Responses

Abstract:Unexpected sudden perturbations challenge postural equilibrium and require reactive compensation. This study aimed to assess interaction effects of the direction, displacement and velocity of perturbations on electromyographic (EMG) activity, centre of pressure (COP) displacement and joint kinematics to detect neuromuscular characteristics (phasic and segmental) and kinematic strategies of compensatory reactions in an unilateral balance paradigm. In 20 subjects, COP displacement and velocity, ankle, knee and hip joint excursions and EMG during short (SLR), medium (MLR) and long latency response (LLR) of four shank and five thigh muscles were analysed during random surface translations varying in direction (anterior-posterior (sagittal plane), medial-lateral (frontal plane)), displacement (2 vs. 3 cm) and velocity (0.11 vs. 0.18 m/s) of perturbation when balancing on one leg on a movable platform. Phases: SLR and MLR were scaled to increased velocity (P<0.05); LLR was scaled to increased displacement (P<0.05). Segments: phasic interrelationships were accompanied by segmental distinctions: distal muscles were used for fast compensation in SLR (P<0.05) and proximal muscles to stabilise in LLR (P<0.05). Kinematics: ankle joints compensated for both increasing displacement and velocity in all directions (P<0.05), whereas knee joint deflections were particularly sensitive to increasing displacement in the sagittal (P<0.05) and hip joint deflections to increasing velocity in the frontal plane (P<0.05). COP measures increased with increasing perturbation velocity and displacement (P<0.05). Interaction effects indicate that compensatory responses are based on complex processes, including different postural strategies characterised by phasic and segmental specifications, precisely adjusted to the type of balance disturbance. To regain balance after surface translation, muscles of the distal segment govern the quick regain of equilibrium; the muscles of the proximal limb serve as delayed stabilisers after a balance disturbance. Further, a kinematic distinction regarding the compensation for balance disturbance indicated different plane- and segment-specific sensitivities with respect to the determinants displacement and velocity.

Neuromechanical Model-Based Corrective Torque Estimation During Weight Shifting in Lower Limb Amputees

Abstract:As a consequence of limb loss, unilateral lower limb amputees (LLAs) exert additional flexion torque in the intact limb during postural balance, leading to other secondary complications. A prosthetic device with torque delivery is required to avoid such complications. However, estimating desired torque is an acknowledged challenge in this domain. Motivated by the above, this paper focuses on estimating the corrective torque required from an assistive device to balance the individual while performing weight-shifting exercises. Further, a healthy individual’s inverted pendulum (IP) model is explored to model LLA’s dynamics during weight-shifting. The model and the control strategy are validated using forward and backward body lean angles recorded from healthy and LLA individuals. Finally, a robust Proportional Integral Derivative controller is designed to estimate the corrective torque for maintaining the postural balance during weight-shifting. Along with the IP dynamics, a suitable control law based on Coefficient Diagram Method achieves the experimentally recorded body lean angle. The proposed control scheme is validated through simulation studies to evaluate the tracking performance in two different biomechanically relevant IP postural models. The findings assist in determining the external torque required to maintain the postural balance during gait initiation and compensatory measures for pathologically reduced torque. Note to Practitioners—Gait initiation is a transition between vertical posture and gait. Since vertical stance is inherently unstable, a postural adjustment occurs before gait initiates, propelling the center of mass forward and towards the first stance leg. The absence of flexor torque in the amputated limb causes a loss of postural stability during weight shifting activities. In the case of powered exoskeletons and prosthetic devices, a prior estimation of corrective torque through a computational approach helps to select a suitable actuator that provides an appropriate joint torque while maintaining the postural balance. As a result, an efficient computational model of gait initiation that accounts for neuromuscular system components before designing assistive devices is necessary. The controllers further provide corrective action to the assistive device to obtain robust and optimal coordination between the prosthetic element and gait. Therefore, this study demonstrates the significance of neuromuscular controller and assists in determining how much corrective torque from an external device is required to maintain postural stability and adaptability during weight shifting exercises. The effectiveness of the neural controller is also tested for real-time data to compensate for the effect of external disturbance and noise, thereby achieving a proper and controlled degree of movement in amputee users. These findings could provide one of the fundamental aspects of gait initiation model paradigms of compensatory measures for pathologically diminished torque and contribute to developing low-cost prosthetic solutions for the benefit of the rehabilitation society in developing countries.

Abstract:Despite the abundance of studies on the control of standing balance, insights about the roles of biomechanics and neural control have been limited. Previous work introduced an analysis combining the direction and orientation of ground reaction forces. The “intersection point” of the lines of actions of these forces exhibited a consistent pattern across healthy, young subjects when computed for different frequency components of the center of pressure signal. To investigate the control strategy of quiet stance, we applied this intersection point analysis to experimental data of 15 healthy, young subjects balancing in tandem stance on a narrow beam and on the ground. Data from the sagittal and frontal planes were analyzed separately. The task was modeled as a double-inverted pendulum controlled by an optimal controller with torque-actuated ankle and hip joints and additive white noise. To test our prediction that the controller that minimized overall joint effort would yield the best fit across the tested conditions and planes of analyses, experimental results were compared to simulation outcomes. The controller that minimized overall effort produced the best fit in both balance conditions and planes of analyses. For some conditions, the relative penalty on the hip and ankle joints varied in a way relevant to the balance condition or to the plane of analysis. These results suggest that unimpaired quiet balance in a challenging environment can be best described by a controller that maintains minimal effort through the adjustment of relative ankle and hip joint torques.

Sensorimotor integration in human postural control

Abstract:It is generally accepted that human bipedal upright stance is achieved by feedback mechanisms that generate an appropriate corrective torque based on body-sway motion detected primarily by visual, vestibular, and proprioceptive sensory systems. Because orientation information from the various senses is not always available (eyes closed) or accurate (compliant support surface), the postural control system must somehow adjust to maintain stance in a wide variety of environmental conditions. This is the sensorimotor integration problem that we investigated by evoking anterior-posterior (AP) body sway using pseudorandom rotation of the visual surround and/or support surface (amplitudes 0.5-8 degrees ) in both normal subjects and subjects with severe bilateral vestibular loss (VL). AP rotation of body center-of-mass (COM) was measured in response to six conditions offering different combinations of available sensory information. Stimulus-response data were analyzed using spectral analysis to compute transfer functions and coherence functions over a frequency range from 0.017 to 2.23 Hz. Stimulus-response data were quite linear for any given condition and amplitude. However, overall behavior in normal subjects was nonlinear because gain decreased and phase functions sometimes changed with increasing stimulus amplitude. "Sensory channel reweighting" could account for this nonlinear behavior with subjects showing increasing reliance on vestibular cues as stimulus amplitudes increased. VL subjects could not perform this reweighting, and their stimulus-response behavior remained quite linear. Transfer function curve fits based on a simple feedback control model provided estimates of postural stiffness, damping, and feedback time delay. There were only small changes in these parameters with increasing visual stimulus amplitude. However, stiffness increased as much as 60% with increasing support surface amplitude. To maintain postural stability and avoid resonant behavior, an increase in stiffness should be accompanied by a corresponding increase in damping. Increased damping was achieved primarily by decreasing the apparent time delay of feedback control rather than by changing the damping coefficient (i.e., corrective torque related to body-sway velocity). In normal subjects, stiffness and damping were highly correlated with body mass and moment of inertia, with stiffness always about 1/3 larger than necessary to resist the destabilizing torque due to gravity. The stiffness parameter in some VL subjects was larger compared with normal subjects, suggesting that they may use increased stiffness to help compensate for their loss. Overall results show that the simple act of standing quietly depends on a remarkably complex sensorimotor control system.
Characterization of Human Balance through a Reinforcement Learning-based Muscle Controller

Abstract:Balance assessment during physical rehabilitation often relies on rubric-oriented battery tests to score a patient's physical capabilities, leading to subjectivity. While some objective balance assessments exist, they are often limited to tracking the center of pressure (COP), which does not fully capture the whole-body postural stability. This study explores the use of the center of mass (COM) state space and presents a promising avenue for monitoring the balance capabilities in humans. We employ a musculoskeletal model integrated with a balance controller, trained through reinforcement learning (RL), to investigate balancing capabilities. The RL framework consists of two interconnected neural networks governing balance recovery and muscle coordination respectively, trained using Proximal Policy Optimization (PPO) with reference state initialization, early termination, and multiple training strategies. By exploring recovery from random initial COM states (position and velocity) space for a trained controller, we obtain the final BR enclosing successful balance recovery trajectories. Comparing the BRs with analytical postural stability limits from a linear inverted pendulum model, we observe a similar trend in successful COM states but more limited ranges in the recoverable areas. We further investigate the effect of muscle weakness and neural excitation delay on the BRs, revealing reduced balancing capability in different regions. Overall, our approach of learning muscular balance controllers presents a promising new method for establishing balance recovery limits and objectively assessing balance capability in bipedal systems, particularly in humans.

Abstract:The contribution to balance of spinal and transcortical processes including the long-latency reflex is well known. The control of balance has been modelled previously as a continuous, state feedback controller representing, long-latency reflexes. However, the contribution of slower, variable delay processes has not been quantified. Compared with fixed delay processes (spinal, transcortical), we hypothesize that variable delay processes provide the largest contribution to balance and are sensitive to historical context as well as current states. Twenty-two healthy participants used a myoelectric control signal from their leg muscles to maintain balance of their own body while strapped to an actuated, inverted pendulum. We study the myoelectric control signal (u) in relation to the independent disturbance (d) comprising paired, discrete perturbations of varying inter-stimulus-interval (ISI). We fit the closed loop response, u from d, using one linear and two non-linear non-parametric (many parameter) models. Model M1 (ARX) is a generalized, high-order linear-time-invariant (LTI) process with fixed delay. Model M1 is equivalent to any parametric, closed-loop, continuous, linear-time-invariant (LTI), state feedback model. Model M2, a single non-linear process (fixed delay, time-varying amplitude), adds an optimized response amplitude to each stimulus. Model M3, two non-linear processes (one fixed delay, one variable delay, each of time-varying amplitude), add a second process of optimized delay and optimized response amplitude to each stimulus. At short ISI, the myoelectric control signals deviated systematically both from the fixed delay LTI process (M1), and also from the fixed delay, time-varying amplitude process (M2) and not from the two-process model (M3). Analysis of M3 (all fixed delay and variable delay response amplitudes) showed the variable (compared with fixed) delay process 1) made the largest contribution to the response, 2) exhibited refractoriness (increased delay related to short ISI) and 3) was sensitive to stimulus history (stimulus direction 2 relative to stimulus 1). For this whole-body balance task and for these impulsive stimuli, non-linear processes at variable delay are central to control of balance. Compared with fixed delay processes (spinal, transcortical), variable delay processes provided the largest contribution to balance and were sensitive to historical context as well as current states.

Combined translational and rotational perturbations of standing balance reveal contributions of reduced reciprocal inhibition to balance impairments in children with cerebral palsy

Abstract:Balance impairments are common in cerebral palsy. When balance is perturbed by backward support surface translations, children with cerebral palsy have increased co-activation of the plantar flexors and tibialis anterior muscle as compared to typically developing children. However, it is unclear whether increased muscle co-activation is a compensation strategy to improve balance control or is a consequence of reduced reciprocal inhibition. During translational perturbations, increased joint stiffness due to co-activation might aid balance control by resisting movement of the body with respect to the feet. In contrast, during rotational perturbations, increased joint stiffness will hinder balance control as it couples body to platform rotation. Therefore, we expect increased muscle co-activation in response to rotational perturbations if co-activation is caused by reduced reciprocal inhibition but not if it is merely a compensation strategy. We perturbed standing balance by combined backward translational and toe-up rotational perturbations in 20 children with CP and 20 TD children. Perturbations induced forward followed by backward movement of the center of mass. We evaluated reactive muscle activity and the relation between center of mass movement and reactive muscle activity using a linear feedback model based on center of mass kinematics. In typically developing children, perturbations induced plantar flexor balance correcting muscle activity followed by tibialis anterior balance correcting muscle activity, which was driven by center of mass movement. In children with cerebral palsy, the switch from plantar flexor to tibialis anterior activity was less pronounced than in typically developing children due to increased muscle co-activation of the plantar flexors and tibialis anterior throughout the response. Our results thus suggest that a reduction in reciprocal inhibition causes muscle co-activation in reactive standing balance in children with CP. Children with cerebral palsy often have balance impairments. When standing balance is perturbed using support-surface translations, children with cerebral palsy have high agonist-antagonist co-activation. Increasing joint stiffness by co-activating agonists and antagonists might aid standing balance control during translational, but not rotational perturbations. If children with cerebral palsy show muscle co-activation during rotational perturbations, we can assume that this co-activation is a consequence of neurological impairments (e.g., not be able to suppress antagonist activation upon stretch of the agonist). We found that both in typically developing children and children with cerebral palsy reactive muscle activity could be explained by delayed feedback of center of mass kinematics. Our combined translational and rotational perturbations induced a switch in center of mass movement (forward to backward). We found that upon this switch in center of mass movement, typically developing children switched from plantarflexor to tibialis anterior muscle activity. This switch in muscle activity was less clear in children with cerebral palsy, although center of mass movement was the same as in typically developing children, due to muscle co-activation. Therefore, our results suggest that neural impairments cause muscle co-activation in reactive standing balance in children with cerebral palsy.

Abstract:The human’s upright standing is a complex control process that is not yet fully understood. Postural control models can provide insights into the body’s internal control processes of balance behaviour. Using physiologically plausible models can also help explaining pathophysiological motion behaviour. In this paper, we introduce a neuromusculoskeletal postural control model using sensor feedback consisting of somatosensory, vestibular and visual information. The sagittal plane model was restricted to effectively six degrees of freedom and consisted of nine muscles per leg. Physiological plausible neural delays were considered for balance control. We applied forward dynamic simulations and a single shooting approach to generate healthy reactive balance behaviour during quiet and perturbed upright standing. Control parameters were optimized to minimize muscle effort. We showed that our model is capable of fulfilling the applied tasks successfully. We observed joint angles and ranges of motion in physiological plausible ranges and comparable to experimental data. This model represents the starting point for subsequent simulation of pathophysiological postural control behaviour.

Abstract:Virtual Learning Environments (VLE) have become popular in higher education in recent years due to their ability to provide additional and flexible solutions for students and researchers. However, the limitations of VLEs have led to the development of a new generation of VLE – the Personal Learning Environment (PLE). PLEs avoid these limitations and have new features that allow students to control and develop new applications, such as Web 2.0 applications and social networks. Whilst PLEs have resolved some of the drawbacks of VLEs, it is argued that PLEs also have greater potential to cover a wider range of aspects. This paper presents a proactive context-aware architecture for PLE supporting two major objectives: lifelong access and learner-centric study, covering both traditional formal (institution-based) and informal (private, non-institution-based) academic learning. Bayesian Networks are graphical modeling tools that have been used for modeling uncertain knowledge. Moreover, BN has been used in this research to implement the proposed architecture.

Abstract:The ratio of the total turnover rate of glucose calculated with [2-3H]glucose as the tracer and the irreversible disposal rate of glucose, calculated with [U-14C]glucose as the tracer was found to be 1.35 +/- 0.28 SD (+/- 0.08 SEM) in dogs 1-33 days of age. This value was unrelated to age and to the level of glucose in the plasma. Accordingly, under basal conditions the overall re-incorporation of carbon atoms into newly released glucose in pups is nearly identical with that found in grown dogs.
Reciprocal Inhibition and Coactivation of Ankle Muscles in Low- and High-Velocity Forward and Backward Perturbations

Abstract:Reciprocal inhibition and coactivation are strategies of the central nervous system used to perform various daily tasks. In automatic postural responses (APR), coactivation is widely investigated in the ankle joint muscles, however reciprocal inhibition, although clear in manipulative motor actions, has not been investigated in the context of APRs. The aim was to identify whether reciprocal inhibition can be observed as a strategy in the recruitment of gastrocnemius Medialis (GM), Soleus (So) and Tibialis Anterior (TA) muscles in low- and high-velocity forward and backward perturbations. We applied two balance perturbations with a low and a high velocity of displacement of the movable platform in forward and backward conditions and we evaluated the magnitude and latency time of TA, GM and So activation latency, measured by electromyography (EMG). In forward perturbations, coactivation of the three muscles was observed, with greater activation amplitude of the GM and lesser amplitude of the So and TA muscles. For backward, the pattern of response observed was activation of the TA muscle, a decrease in the EMG signal, which characterizes reciprocal inhibition of the GM muscle and maintenance of the basal state of the So muscle. This result indicates that backward perturbations are more challenging.

Abstract:The central nervous system (CNS) exploits anticipatory (APAs) and compensatory (CPAs) postural adjustments to maintain the balance. The postural adjustments comprising stability of the center of mass (CoM) and the pressure distribution of the body influence each other if there is a lack of performance in either of them. Any predictable or sudden perturbation may pave the way for the divergence of CoM from equilibrium and inhomogeneous pressure distribution of the body. Such a situation is often observed in the daily lives of Multiple Sclerosis (MS) patients due to their poor APAs and CPAs and induces their falls. The way of minimizing the risk of falls in neurological patients is by utilizing perturbation-based rehabilitation, as it is efficient in the recovery of the balance disorder. In light of the findings, we present the design, implementation, and experimental evaluation of a novel 3 DoF parallel manipulator to treat the balance disorder of MS. The robotic platform allows angular motion of the ankle based on its anthropomorphic freedom. Moreover, the end-effector endowed with upper and lower platforms is designed to evaluate both the pressure distribution of each foot and the CoM of the body, respectively. Data gathered from the platforms are utilized to both evaluate the performance of the patients and used in high-level control of the robotic platform to regulate the difficulty level of tasks. In this study, kinematic and dynamic analyses of the robot are derived and validated in the simulation environment. Low-level control of the first prototype is also successfully implemented through the PID controller. The capacity of each platform is evaluated with a set of experiments considering the assessment of pressure distribution and CoM of the foot-like objects on the end-effector. The experimental results indicate that such a system well-address the need for balance skill training and assessment through the APAs and CPAs.

Controlling balance during quiet standing: Proportional and derivative controller generates preceding motor command to body sway position observed in experiments

Abstract:To compensate for significant time delays in the control of human bipedal stance, it was suggested that a feed-forward control mechanism is needed to generate a preceding motor command to the body sway position observed in quiet standing. In this article, we present evidence that a feedback proportional-derivative (PD) controller can effectively generate a desired preceding motor command. We also discuss the following characteristics of the proposed PD controller: (1) the level of robustness of the controller with respect to neurological time delays and (2) how well the controller replicates the system's dynamics observed in experiments with able bodied subjects, i.e. how well the controller generates the observed preceding motor command. Human quiet stance was simulated using an inverted pendulum model regulated by a PD controller. The simulations were used to calculate the center of mass (COM) position and velocity data, and the motor command (ankle joint torque) data as a function of time. These data and the data obtained in the experiments were compared using cross-correlation functions (CCFs). The results presented herein imply that a PD feedback controller is capable of ensuring balance during human bipedal quiet stance, even if the neurological time delays are considerable. The proposed feedback controller can generate the preceding motor command that was observed in the experiments. Therefore, we conclude that a feed-forward mechanism is not necessary to compensate for the long closed-loop time delay in human bipedal stance as suggested in recent literature, and that the PD controller is a good approximation of the control strategy applied by able bodied subjects during quiet stance.

A neuromusculoskeletal modelling approach to bilateral hip mechanics due to unexpected lateral perturbations during overground walking

Abstract:Background: Current studies on how external perturbations impact gait dynamics have primarily focused on the changes in the body's center of mass (CoM) during treadmill walking. The biomechanical responses, in particular to the multi-planar hip joint coordination, following perturbations in overground walking conditions are not completely known. Methods: In this study, a customized gait-perturbing device was designed to impose controlled lateral forces onto the subject's pelvis during overground walking. The biomechanical responses of bilateral hips were simulated by subject-specific neuromusculoskeletal models (NMS) driven by in-vivo motion data, which were further evaluated by statistical parameter mapping (SPM) and muscle coactivation index (CI) analysis. The validity of the subject-specific NMS was confirmed through comparison with measured surface electromyographic signals. Results: Following perturbations, the sagittal-plane hip motions were reduced for the leading leg by 18.39° and for the trailing leg by 8.23°, while motions in the frontal and transverse plane were increased, with increased hip abduction for the leading leg by 10.71° and external rotation by 9.06°, respectively. For the hip kinetics, both the bilateral hip joints showed increased abductor moments during midstance (20%-30% gait cycle) and decreased values during terminal stance (38%-48%). Muscle CI in both sagittal and frontal planes was significantly decreased for perturbed walking (p < 0.05), except for the leading leg in the sagittal plane. Conclusion: The distinctive phase-dependent biomechanical response of the hip demonstrated its coordinated control strategy for balance recovery due to gait perturbations. And the changes in muscle CI suggested a potential mechanism for rapid and precise control of foot placement through modulation of joint stiffness properties. These findings obtained during actual overground perturbation conditions could have implications for the improved design of wearable robotic devices for balance assistance.
Does proprioceptive reweighting contribute to reactive balance strategies during slip-like perturbations? a proof-of-concept in healthy adults

Abstract:Falls commonly occur during walking, particularly when struggling to respond to unexpected perturbations. Proprioception plays a significant role in detecting body destabilization even before reactions to perturbations are required. This study investigates the contribution of proprioceptive reweighting strategies to reactive balance during walking. This cross-sectional, proof-of-concept study included fifteen healthy adults (18–40 years). Ankle and back muscle vibrators disrupted proprioceptive input in stance, allowing calculation of the proprioceptive reweighting index. Walk-slip perturbations were then administered on an ActiveStep treadmill. A linear regression model assessed the significance of proprioceptive reweighting in predicting post-slip stability (margin of stability). Participants shifted from an ankle-steered to a central-steered proprioceptive strategy on a foam surface with closed eyes (Difference = 15.70 % (SD=37.87), 95 %CI [0.41, 30.99], p = 0.045). The regression model explained 22.7 % of the variance in pre-touchdown margin of stability, with proprioceptive reweighting on foam significantly contributing to post-perturbation postural control (p < 0.001). Proprioceptive reweighting provides a moderate explanation for the mechanisms of reactive balance, highlighting that the key to effective balance recovery strategies may lie in the person's ability to both detect and respond to imbalances. Further research should explore if these proprioceptive strategies are a matter of directional control and if responses differ in older adults.
